Abstract:OBJECTIVES:Hyperuricaemia is closely related to metabolic diseases and is receiving increasing attention from all over the world. This study aimed to investigate the prevalence and factors associated with hyperuricaemia in non-obese Chinese population.DESIGN:Retrospective cross-sectional study.SETTING:A large general hospital that can provide health check-ups in Hangzhou, China.PARTICIPANTS:A total of 5731 apparently healthy Chinese adults (2349 men and 3382 women) who took their health check-ups during the year of 2019.EXCLUSION CRITERIA:(1) those with body mass index ≥24 kg/m2; (2) those with incomplete anthropometric and biochemical data; (3) those with a history of malignancy and (4) those under urate-lowering treatment.PRIMARY AND SECONDARY OUTCOME MEASURES:The prevalence and factors associated with hyperuricaemia in non-obese Chinese adults.RESULTS:Of the 5731 non-obese subjects enrolled, 538 (9.4%) were identified as having hyperuricaemia, specifically 16.3% in men and 4.6% in women. The prevalence of hyperuricaemia markedly increased in women aged above 50 years. The prevalence of hyperuricaemia was significantly higher in metabolically unhealthy participants with normal weight than in metabolically healthy participants with normal weight. Participants with hyperuricaemia showed a higher prevalence of metabolic syndrome and fatty liver disease than participants with normouraemia. Age, waist circumference, estimated glomerular filtration rate, blood urea nitrogen, excessive drinking and fatty liver were associated with hyperuricaemia in both genders.CONCLUSION:The prevalence of hyperuricaemia was 9.4% in non-obese Chinese adults. Non-obese participants with hyperuricaemia also showed multiple metabolic disorders. We suggest that clinicians pay attention to serum uric acid level in non-obese patients.

Abstract:Uric acid is the end product of dietary or endogenous purines degradation, and hyperuricemia is one of the most common metabolic disorders. It has been widely accepted that hyperuricemia increases risks of gout, cardiovascular diseases, and type 2 diabetes. A growing body of evidence, comprising a great deal of cross-sectional studies and several prospective ones, also indicates that hyperuricemia is associated with increased prevalence, incidence and disease severity of non-alcoholic fatty liver disease (NAFLD). On the contrary, NAFLD can predict hyperuricemia as well. However, no causal relationship can be drawn from this point. With a well-established relationship between uric acid and NAFLD prevalence as well as disease severity in addition to the role of potential therapeutic target, the prognostic role of uric acid is also worth investigating. Further studies should focus on the prospective role of uric acid on NAFLD progression and its underlying mechanisms, as well as its clinical implications.

Abstract:Background: As a part of metabolic syndrome, hyperuricemia has a higher incidence in patients with diabetes than in the general population owing to various underlying factors.Objectives: The objective of the present study was to investigate the prevalence of hyperuricemia among patients with diabetes and identify associated factors.Design: A cross-sectional study.Methods: Herein, we included patients with diabetes managed at nine healthcare centers in Chenghua District, Chengdu, from February 2021 to November 2021. Clinical data, lifestyle habits, and laboratory data were collected to determine the prevalence and factors associated with hyperuricemia.Results: In total, we included 1577 patients with diabetes (males, 50.35%; females, 49.65%). The median serum uric acid level was 337.9 mu mol/L, and the prevalence of hyperuricemia in patients with diabetes was 21.24%. The prevalence of hyperuricemia in male patients was significantly higher than in females (29.35% in males versus 13.03% in females, p < 0.001). Male patients with obesity (p = 0.006) or triglyceride (TG) >= 1.7 mmol/L (p < 0.001) had a high risk of developing hyperuricemia, and hyperuricemia was negatively associated with estimated glomerular filtration rate (eGFR) >= 60 mL/min/1.73 m(2) (p < 0.001), glycosylated hemoglobin (HbA1c) >= 7% (p < 0.001), fenofibrate (p = 0.010), and sodium-glucose cotransporter 2 (SGLT-2) inhibitors (p = 0.035). Considering females, overweight (p = 0.004), alanine transaminase (ALT) > 40 U/L (p < 0.001), and TG >= 1.7 mmol/L (p = 0.015) showed a significant positive correlation with hyperuricemia, while eGFR >= 60 mL/min/1.73 m(2 )(p < 0.001) was negatively associated with the risk of hyperuricemia.Conclusion: Hyperuricemia is highly prevalent in patients with diabetes, especially in males. In addition to traditionally associated factors, fenofibrate and SGLT-2 inhibitors were also associated with the risk of hyperuricemia.Registration: The study protocol was registered in the Chinese Clinical Trial Registry (http://www.chictr.org.cn/), and the registration number was ChiCTR 2100042742.
The Effect of Uric Acid Level, Hyperuricemia and Gout on the Risk of Type 2 Diabetes: A Meta-Analysis of Cohort Studies

Abstract:Background: Epidemiology evidence on the effect of serum uric acid levels (SUAs), hyperuricemia, and gout on the risk of type 2 diabetes (T2DM) is inconsistent. We conducted a meta-analysis of cohort studies to comprehensively summarize the issue.Methods: We searched the PubMed and Embase databases to November 2019 for studies conformed to the inclusion criteria. Pooled relative risks (RRs) and 95% confidence intervals (CIs) were calculated with random-effect meta-analysis. We conducted dose-response meta-analysis to investigate the relation between SUA and T2DM risk. Meta-regression analyses, subgroup analyses and sensitivity analyses were performed to identify potential sources of heterogeneity.Findings: A total of 23 prospective studies involving 737468 participants were identified. A significantly positive association between higher uric acid level, hyperuricemia, gout and T2DM risk was observed, respectively(RR=1.20, 95% CI 1.14, 1.27; RR=1.52, 95% CI 1.21, 1.91; RR=1.35, 95% CI 1.16, 1.57). Piecewise dose-response analysis found a linear trend for SUA higher than 4.5mg/dl, with one mg/dl increase of SUA level associated with 8% increase of T2DM risk (RR=1.08, 95%CI= 1.03, 1.13, P=0.001). Unadjusted for hypertension found a positive association between gout and T2DM risk (RR=1.40, 95% CI 1.22, 1.66), while adjusted for hypertension found the relation to be non-significant (RR=1.31, 95% CI 0.78, 2.21).Interpretation: The present findings suggest that higher SUA levels, hyperuricemia and gout are risk factors for T2DM. It is necessary for hyperuricemia and gout patients to test blood glucose regularly and prevent T2DM. Abstract:Hyperuricemia has a close correlation with diabetes, and also participates in the incidence and development of diabetic cardiovascular diseases and nephropathy. Hyperuricemia is involved in the pathogenesis of diabetes mainly through insulin resistance, and also contributes to the occurrence of diabetic cardiovascular diseases and nephropathy by endothelial dysfunction and inflammatory cascade reaction. This review mainly elucidates the prevalence and possible mechanism of diabetes and its vascular complications caused by hyperuricemia.
Hyperuricemia and the Prognosis of Hypertensive Patients: A Systematic Review and Meta-Analysis.

Abstract:The aim of this study was to assess the prognostic value of hyperuricemia in patients with established hypertension by systematic review and meta-analysis of cohort studies. MEDLINE, Embase, and the Chinese Biomedical Literature Database were searched through January 2015. Seventeen cohort studies were included and their methodological quality was moderate to high, with Newcastle-Ottawa Scale scores ranging from 6 to 9. Random-effects model meta-analyses showed that in terms of adjusted categorical data, hyperuricemia significantly correlated with cardiovascular diseases in hypertensive patients (hazard ratio [HR], 1.51; 95% confidence interval [CI], 1.13-2.03), all-cause mortality (HR, 1.12; 95%CI, 1.02-1.23), and diabetes (HR, 1.84; 95% CI, 1.02-3.30) but not with stroke (HR, 0.85; 95%CI, 0.57-1.27); while, in terms of adjusted continuous data, the corresponding pooled HRs were 1.17 (95% CI, 1.07-1.27), 1.05 (95% CI, 0.98-1.13), 1.28 (95% CI, 1.18-1.38), and 1.06 (95% CI, 0.98-1.16), respectively. The findings of our meta-analysis suggest that hyperuricemia could slightly increase the risk of cardiovascular diseases and diabetes in patients with hypertension.
Prevalence of Hyperuricemia among Patients with Type 2 Diabetes Aged over 30 in Shanghai

Abstract:Objective:To investigate the prevalence of hyperuricemia among the Chinese patients diagnosed with type 2 diabetes aged over 30 in Shanghai central area and evaluate the relationship between hyperuricemia and metabolic syndrome(MS). Methods: 1039 Chinese patients diagnosed with type 2 diabetes aged over 30 were investigated by randomized cluster sampling in Shanghai central area and 1008 patients were analyzed in this study. Body mass measurements, resting blood pressure, fasting blood measures were investigated. The prevalence of hyperuricemia and the risk factors associated with hyperuricemia were investigated. The relationship between hyperuricemia and metabolic syndrome (MS) was evaluated. Results:(1) Age, body mass index, waist circumference, serum triglyceride, systolic blood pressure and diastolic blood pressure in hyperuricemia subgroup were significantly higher than those in non-hyperuricemia subgroup. Fasting glucose concentration, HbA1c and HDL in hyperuricemia subgroup were lower than those in non-hyperuricemia subgroup. (2) The prevalence of hyperuricemia was 10. 0% among the Chinese patients diagnosed with type 2 diabetes aged over 30 in Shanghai central area, 11.1% with male and 9.4% with female. The prevalence of hyperuricemia in female increased following the aging. (3) The percentage of MS in hyperuricemia subgroup was significantly higher than that in non-hyperuricemia subgroup (63. 4% vs. 49. 8% ,x2 6. 660,P=0. 012). (4) Aging,obesity and lower HDL were risk factors of hyperuricemia by stepwise Logistic regression. Conclusion:The prevalence of hyperuricemia was 10. 0% among the Chinese patients diagnosed with type 2 diabetes aged over 30 in Shanghai central area, and hyperuricemia was correlated with MS and its components.
Association between serum uric acid levels and diabetic peripheral neuropathy in type 2 diabetes: a systematic review and meta-analysis

Abstract:Background: The evidence supporting a connection between elevated serum uric acid (SUA) levels and diabetic peripheral neuropathy (DPN) is controversial. The present study performed a comprehensive evaluation of this correlation by conducting a systematic review and meta-analysis of relevant research. Method: PubMed, Web of Science (WOS), Embase, and the Cochrane Library were searched for published literature from the establishment of each database to January 8, 2024. In total, 5 cohort studies and 15 cross-sectional studies were included, and 2 researchers independently screened and extracted relevant data. R 4.3.0 was used to evaluate the included literature. The present meta-analysis evaluated the relationship between SUA levels and the risk of DPN in type 2 diabetes (T2DM) by calculating the ratio of means (RoM) and 95% confidence intervals (CIs) using the method reported by JO Friedrich, and it also analyzed continuous outcome measures using standardized mean differences (SMDs) and 95% CIs to compare SUA levels between DPN and non-DPN groups. Funnel plot and Egger's test were used to assess publication bias. Sensitivity analysis was conducted by sequentially removing each study one-by-one. Results: The meta-analysis included 20 studies, with 12,952 T2DM patients with DPN and 16,246 T2DM patients without DPN. There was a significant correlation between SUA levels and the risk of developing DPN [odds ratio (OR) = 1.23; 95% CI: 1.07-1.41; p = 0.001]. Additionally, individuals with DPN had higher levels of SUA compared to those without DPN (SMD = 0.4; 95% CI: -0.11-0.91; p < 0.01). Conclusion: T2DM patients with DPN have significantly elevated SUA levels, which correlate with a heightened risk of peripheral neuropathy. Abstract:Purpose of review: Although gout's cardinal feature is inflammatory arthritis, it is closely associated with insulin resistance and considered a manifestation of the metabolic syndrome. As such, both gout and hyperuricemia are often associated with major cardiometabolic and renal comorbidities that drive the persistently elevated premature mortality rates among gout patients. To that end, conventional low-purine (i.e., low-protein) dietary advice given to many patients with gout warrant reconsideration. Recent findings: Recent research suggests that several healthy diets, such as the Mediterranean or Dietary Approaches to Stop Hypertension (DASH) diets, in combination with weight loss for those who are overweight or obese, can drastically improve cardiometabolic risk factors and outcomes. By treating gout as a part of the metabolic syndrome and shifting our dietary recommendations to these healthy dietary patterns, the beneficial effects on gout endpoints should naturally follow for the majority of typical gout cases, mediated through changes in insulin resistance. Summary: Dietary recommendations for the management of hyperuricemia and gout should be approached holistically, taking into consideration its associated cardiometabolic comorbidities. Several healthy dietary patterns, many with similar themes, can be tailored to suit comorbidity profiles and personal preferences.
Hyperuricemia prevalence and its association with metabolic disorders: a multicenter retrospective real-world study in China

Abstract:BACKGROUND: The prevalence of hyperuricemia (HUA) and gout continues to increase in China. Research suggests that HUA may be related to many diseases other than gout. However, further population research is required to investigate the association between HUA and metabolic syndromes. This study sought to investigate the prevalence of HUA in an average population in China, and the association between serum uric acid (UA) levels and related metabolic disorders.METHODS: This multicenter retrospective real-world study examined the hospital information system data of 4 tertiary hospitals in 3 provinces in China. The data of patients aged between 18 and 80 years, who had attended at least 1 medical appointment at which their UA level was recorded, were analyzed to evaluate associations between UA levels and metabolic disorders.RESULTS: Among the 374,506 enrolled subjects (49.7% male; mean age 51.5 years old), the overall prevalence of HUA and gout were 14.8% and 0.5%, respectively. The prevalence was higher among males than females (17.6% <i>vs.</i> 12.0%, 0.8% <i>vs.</i> 0.1%; both P<0.001). Groups exhibiting higher UA levels had increased adjusted odds ratios for dyslipidemia and chronic kidney disease (CKD) in both sexes. Changes in UA levels from the baseline were negatively correlated with changes in the estimated glomerular filtration rate and hemoglobin A1c among both sexes (all P<0.001), and were positively correlated with changes in total cholesterol (TC), triglyceride (TG), and low-density lipoprotein cholesterol (LDL-C) (all P<0.05) among males, and changes in TC, TG, LDL-C and glucose (all P<0.001) among females.CONCLUSIONS: HUA is associated with dyslipidemia and CKD both cross-sectionally and longitudinally. Similar phenomena were observed in both sexes.

Association Between Hyperuricemia and Urinary Calculus in Patients with Diabetes and Non-Diabetic Healthy Subjects

Abstract:Objective To investigate the association between hyperuricemia and urinary calculus in diabetic patients and non-diabetic healthy subjects. Methods Retrospectively analyzed the clinical data of diabetic patients and non-diabetic healthy subjects between January 2011 and September 2014, including body mass index, fasting plasma glucose, serum uric acid, serum creatinine, blood fat, estimated glomerular filtration rate(eGFR) and urinary ultrasound. A total of 2 186 non-diabetic healthy subjects and 4 045 diabetic patients were included in this study. The mean age of non-diabetic healthy subjects was (44 ± 14) years;762 males and 1 424 females. The mean age of diabetic patients was(59±13) years;2 260 males and 1 785 females. The prevalences of urinary calculus in subjects with hyperuricemia and without in the two groups were compared withc2 test;and the prevalences of urinary calculus in subjects with different levels of uric acid were calculated too. Besides, the influence of hyperuricemia, diabetes on the prevalence of urinary calculus were also analyzed. Results The prevalence of urinary calculus in hyperuricemia group(7.9%) was significantly higher than that in normal serum uric acid group(3.5%) in non-diabetic group (c2=5.453, P<0.05);and it was significantly higher in hyperuricemia group than that in the subjects with normal serum uric acid in diabetic patients (13.9%vs 10.5%,c2=4.786, P<0.05). The prevalence of urinary calculus increased with serum uric acid (non-diabetic subjects: c2=9.008, P<0.05; diabetic subjects: c2=9.278, P<0.05). The prevalence of urinary calculus was significantly higher in the group with serum uric acid>420μmol/L than the subjects with serum uric acid≤300μmol/L (non-diabetic subjects:OR=3.037, 95%confidence interval (CI):1.386-6.655, P<0.01;diabetic subjects:OR=1.520, 95%CI:1.120-2.063, P<0.01). The risk of urinary calculus increased significantly in patients with hyperuricemia, diabetes or both diseases compared with that in non-diabetic non-hyperuricemia subjects (hyperuricemia: 7.9% vs 3.5%, OR=2.405, 95%CI: 1.125-5.140, P<0.05;diabetes:10.5%vs 3.5%, OR=3.292, 95%CI:2.543-4.262, P<0.01;both hyperuricemia and diabetes: 13.9% vs 3.5%, OR=4.501, 95%CI: 3.170-6.390, P<0.01). Conclusions The prevalence of urinary calculus in hyperuricemia patients is higher than that in normal serum uric acid subjects among non-diabetic healthy subjects and diabetic patients, and higher prevalence of urinary calculus occurs in hyperuricemia patients complicated with diabetes.
Comparison of Progression Between Diabetic Patients with Different Levels of Uric Acid and Analysis on Influencing Factors

Abstract:Objective To understand the progression of the diabetic patients with different levels of uric acid and to analyze its influencing factors,and to provide evidence for the prevention and treatment of diabetes complicated with hyperuricemia.Methods A clinical record database for type 2diabetes mellitus(T2DM)patients was established with EpiData 3.0software and 1 411 cases of complete data were selected.According to serum uric acid(SUA)levels,1 411 cases of T2 DM patients were divided into high uric acid(HUA)group and normal uric acid(NUA)group.The differences of the main clinical indicators and the incidence of complications of the patients between two groups were compared.Univariate and multivariate Logistic regression were carried out to analyze the influencing factors of diabetes complicated with hyperuricemia.Results The incidences of diabetic macroangiopathy and diabetic microvasculopathy of the patients in HUA group were significantly higher than those in NUA group(P0.05),but there was no significant difference of the incidence of dabetic neuropathy between two groups(P0.05).Univariate analysis showed that there were significant differences of the gender,body mass index(BMI),systolie pressure(SBP),diastolic pressure(DBP),asparagine aminotransferase(AST),alaninamide transferase(ALT),gamma-glutamyltransferase(GGT),cholinesterase(CHE),triglyeerides(TG),low-density lipoprotein(LDL-C),high-density lipoprotein(HDL-C),gycosylated hemoglobin(HbA1c)between two groups(P0.05).The multivariable non-conditional Logistic analysis results showed that female,higher HbA1 c,higher HDL-C were the protective factors of diabetes complicated with hyperuricemia(female:OR=0.705,95%CI=0.501-0.993,P0.05;HbA1c:OR=0.912,95%CI=0.840-0.991,P0.05;HDL-C:OR=0.539,95%CI=0.335-0.868,P0.05);but the BMI≥25kg·m-2,higher SBP,and higher TG were the risk factors of diabetes complicated with hyperuricemia(BMI:OR=2.107,95%CI=1.473-3.014,P0.05;SBP:OR=1.767,95%CI=1.210-2.581,P 0.05;TG:OR=1.111,95% CI=1.057-1.168,P 0.05).Conclusion The macrovascular and microvascular complications for the patients with diabetes complicated with hyperuricemia should be screened;the BMI of the patients should be controlled as well as SBP and TG,so the incidence of diabetes complicated with hyperuricemia should be prevented.

Abstract:Fructose plays an important role in the complex metabolism of uric acid in the human body. However, high blood uric acid concentration, known as hyperuricemia, is the main risk factor for development of gout. Therefore, we conducted an updated meta-analysis on the prevalence and geographical distribution of hyperuricemia among the general population in mainland China using systematic literature search.Five electronic databases were used to search for relevant articles published until 2019. All calculations were conducted using the Comprehensive Meta-Analysis (CMA) software. We included 108 eligible articles (172 studies by sex, 95 studies by regions, and 107 studies by study type) and an overall sample size of > 808,505 participants.The pooled prevalence of hyperuricemia among the general population in mainland China was 17.4% (95% CI: 15.8-19.1%). Our subgroup analysis indicated that the pooled prevalence by regions ranged from 15.5 to 24.6%. Those living Northeast region and being males had the highest prevalence (P < 0.001). In addition, some provinces in South Central, East and Northeast regions reported a high prevalence (> 20%), particularly in males. An increasing prevalence was reported since 2005-2009 until 2015-2019. No publication of bias was observed as indicated by a symmetrical funnel plot and Begg and Mazumdar rank correlation (P = 0.392).Prevalence of hyperuricemia is increasing in China, and future studies should investigate the association between the prevalence of hyperuricemia and its risk factors in order to tackle the issue, particularly among the vulnerable groups. Also, our study was the first comprehensive study to investigate the overall prevalence of hyperuricemia in mainland China covering the six different regions.
Knowledge, Attitudes, and Practices about Hyperuricemia and Gout in Community Health Workers and Patients with Diabetes

Abstract:Hyperuricemia exhibits a high incidence among individuals with diabetes; however, the significance of hyperuricemia and gout is often underestimated. This study aimed to assess the knowledge, attitude, and practice of hyperuricemia and gout among community health workers and patients with diabetes. Two questionnaires were designed to investigate knowledge, attitudes, and practices of hyperuricemia and gout among community health workers and patients with diabetes in Chenghua District, Chengdu, from August 2021 to January 2022. A total of 709 community health workers were included, whose average score was 17.74/30. Approximately half of general practitioners (GPs) demonstrated knowledge regarding the target serum uric acid levels for hyperuricemia and gout. Only 11.2% of GPs were fully aware of the preferred medicine for acute gout. The majority of GPs (86.7%) demonstrated limited awareness regarding the contraindications associated with colchicine, while a significant proportion (65.1%) lacked knowledge about the specific classes of drugs that inhibit uric acid synthesis. Among the 508 patients with diabetes included in this survey, 32.3% demonstrated awareness of hyperuricemia, while 60.8% exhibited knowledge regarding gout. The average score attained by these individuals was recorded at 7.21 out of a total of 26 points. The majority of patients with diabetes (87.8%) held the mistaken belief that hyperuricemia definitely led to the development of gout. Almost 66% agreed that a massage or a hot compress could be used when acute gouty arthritis attacks. The knowledge rate of hyperuricemia and gout among community health workers was moderate, while it was low in patients with diabetes.

Abstract:The morbidity of hyperuricemia/gout increases gradually,which is characterized as hyperendemic,younger in age,higher male morbidity and higher morbidity in coastal areas.Obesity,hypertention,hyperlipidemia and hyperglycemia are closely related to the development and progression of hyperuricemia/gout.Low dose of aspirin,loop diuretics and thiazide diuretics all contribute to the increase of serum uric acid.Hyperuricemia is an independent risk factor for type 2 diabetes,hypertention,atherosclerosis,cardiovascular events,cerebral stroke and chronic kidney disease.Hyperuricemia is also the major biochemical basis and direct cause of gout attack.So the importance of early detection and early treatment should be emphasized for hyperuricemia/gout patients.
Analysis of clinical characteristics and risk factors of different types of hyperuricemia in patients with type 2 diabetes

Abstract:Objective To investigate the proportion of different types of hyperuricemia(HUA) and their respective risk factors in patients with type 2 diabetes mellitus.Methods A total of 435 inpatients with normal renal function were enrolled in the Metabolic Disease Hospital of Tianjin Medical University from January to December 2016 by stratified random sampling method.According to serum uric acid,fraction of uric acid excretion (FEUA) and uric acid excretion rate (UEUA),the patients were divided into four groups:normal serum uric acid group,synthesis-increased HUA group,mixed type of HUA group and excretion-decreased HUA group.One-factor analysis of variance(ANOVA) test was used to analyze the general information and biochemical indicators of each group.The independent risk factors of each group were analyzed by multivariate binary logistic regression.Results The overall detection rate of HUA in type 2 diabetes was 20.68％ (90/435).The proportions of synthesis-increased HUA was 28％ (25/435),mixed type of HUA was 23％ (21/435),excretion-decreased HUA was 49％ (44/435).The 24 hour urinary uric acid and UEUA in synthesis-increased HUA group were higher than those in normal uric acid group(F =16.496,40.042,all P ＜ 0.05),and the independent risk factors of synthesis-increased HUA group were estimated glomerular filtration rate (OR =0.939,95％ CI:0.891-0.990,P ＜0.05),total bilirubin (OR =0.755,95 ％ CI:0.594-0.960,P ＜ 0.05),and triglyceride (OR =1.215,95 ％ CI:1.001-1.473,P ＜ 0.05).The 24 h urinary uric acid and UEUA in mixed type of HUA group were higher than those in normal uric acid group (F=16.496,40.042,all P＜0.05),FUEA was lower than that in normal uric acid group (F =22.692,P ＜ 0.05),and its independent risk factor was triglyceride (OR =1.230,95％ CI:1.083-1.396,P ＜ 0.01).The 24 hour urinary uric acid,FEUA and UEUA in excretion-decreased HUA group were lower than those in normal uric acid group (F =16.496,22.692,40.042,all P ＜ 0.05),and its independent risk factor was body mass index(OR =1.163,95％ CI:1.086-1.247,P ＜0.001).Conclusions In patients with type 2 diabetes and HUA,about 50％ patients are synthesis-increased HUA or mixed type of HUA.Different types of HUA have different characteristics and risk factors,therefore should be treated with different methods.

Abstract:Background: Several epidemiological studies have reported a possible correlation between risk of gout and metabolic disorders including type 2 diabetes, insulin resistance, obesity, dyslipidemia, and hypertension. However, it is unclear if this association is causal. Methods: We used Mendelian randomization (MR) to evaluate the causal relation between metabolic conditions and gout or serum urate concentration by inverse-variance-weighted (conventional) and weighted median methods. Furthermore, MR-Egger regression and MR-pleiotropy residual sum and outlier (PRESSO) method were used to explore pleiotropy. Genetic instruments for metabolic disorders and outcome (gout and serum urate) were obtained from several genome-wide association studies on individuals of mainly European ancestry. Results: Conventional MR analysis showed a robust causal association of increasing obesity measured by body mass index (BMI), high-density lipoprotein cholesterol (HDL), and systolic blood pressure (SBP) with risk of gout. A causal relationship between fasting insulin, BMI, HDL, triglycerides (TG), SBP, alanine aminotransferase (ALT), and serum urate was also observed. These results were consistent in weighted median method and MR-PRESSO after removing outliers identified. Our analysis also indicated that HDL and serum urate as well as gout have a bidirectional causal effect on each other. Conclusions: Our study suggested causal effects between glycemic traits, obesity, dyslipidemia, blood pressure, liver function, and serum urate as well as gout, which implies that metabolic factors contribute to the development of gout via serum urate, as well as potential benefit of sound management of increased serum urate in patients with obesity, dyslipidemia, hypertension, and liver dysfunction.

Abstract:This study aimed to gather the best evidence on the relationship between dietary factors and hyperuricaemia and gout. We searched databases including PubMed, Embase, Cochrane, and Web of Science from database creation to July 2023. Meta-analysis showed that consumption of alcohol (OR: 1.41, 95% CI: 1.29-1.55; 1.60, 95% CI: 1.33-1.93, respectively), red meat (OR:1.27, 95% CI: 1.18-1.37; 1.32, 95% CI: 1.18-1.47, respectively), fructose (OR: 1.29, 95% CI: 1.21-1.38; 1.65, 95% CI: 1.36-2.01, respectively) and seafoods (OR: 1.40, 95% CI: 1.20-1.64; 1.29, 95% CI: 1.00-1.67, respectively) were positively associated with the risk of hyperuricaemia and gout, while vegetables (OR: 0.78, 95% CI: 0.71-0.85; 0.96,95% CI 0.74-1.24, respectively) were inversely associated. Dairy products (OR: 0.69, 95% CI: 0.61-0.78) and nuts (OR: 0.75, 95% CI: 0.60-0.93) were also inversely associated with the risk of hyperuricaemia. Soy products (OR: 0.86, 95% CI: 0.75-0.98) and coffee (OR: 0.56, 95% CI: 0.39-0.81) were negatively associated with the risk of gout.

Abstract:Abstract Introduction Diabetes mellitus (DM) and gout cohabitation severely reduces patient life quality while raising financial burden on individual and society. The aim of this study was to elucidate the association between physical activity (PA) and the prevalence of gout among type 2 DM (T2DM) and hyperuricemia (HUA) patients. Methods In all, we recruited 2291 T2DM patients with HUA. Among them, 448 had gout and 1843 did not. We collected patient data, such as anthropometry, laboratory reports, and medical history, for our analyses. The PA assessment was based on the Chinese version of International PA Questionnaire-short (IPAQ). Moreover, the relationship between PA and gout risk was examined using multivariate logistic regression models. Results Total PA was markedly low among gout patients, relative to controls ( p < 0.05). Based on the IPAQ categorical score, 38.2% exhibited “low,” 26.8% “moderate,” and 35.0% “high” PA among gout patients. In comparison, 12.4% performed “low,” 53.8% “moderate,” and 33.8% “high” PA among controls. Multivariate analysis revealed that, after adjustment of confounding factors, both low (OR 6.382) and high PA (OR 2.048) had a higher prevalence of gout, as compared to moderate PA. Moreover, we revealed that the male sex, age, HUA duration, serum uric acid, glycated hemoglobin, dyslipidemia history, and drinking status were also independent indicators of the prevalence of gout. Furthermore, stratification analyses revealed results consistent with our prior results. Conclusions PA intensity was associated with the prevalence of gout among T2DM and HUA patients, and the lowest prevalence was achieved from moderate intensity PA. Abstract:With the deepening of the researches on uric acid, especially in the study of metabolic diseases, uric acid has been found to be closely related to obesity, metabolic syndrome, nonalcoholic fatty liver disease, diabetes, and other metabolic diseases. Uric acid causes a series of pathophysiological changes through inflammation, oxidative stress, vascular endothelial injury, and so on and thus subsequently promotes the occurrence and development of diseases. This review confirmed the positive correlation between uric acid and diabetes mellitus and its chronic complications through the pathogenesis and clinical studies aspects.
