Abstract:Psychiatry shares most ethical issues with other branches of medicine, but also faces special challenges. The Code of Ethics of the World Psychiatric Association offers guidance, but many mental health care professionals are unaware of it and the principles it supports. Furthermore, following codes of ethics is not always sufficient to address ethical dilemmas arising from possible clashes among their principles, and from continuing changes in knowledge, culture, attitudes, and socio‐economic context. In this paper, we identify topics that pose difficult ethical challenges in contemporary psychiatry; that may have a significant impact on clinical practice, education and research activities; and that may require revision of the profession's codes of ethics. These include: the relationships between human rights and mental health care, research and training; human rights and mental health legislation; digital psychiatry; early intervention in psychiatry; end‐of‐life decisions by people with mental health conditions; conflicts of interests in clinical practice, training and research; and the role of people with lived experience and family/informal supporters in shaping the agenda of mental health care, policy, research and training. For each topic, we highlight the ethical concerns, suggest strategies to address them, call attention to the risks that these strategies entail, and highlight the gaps to be narrowed by further research. We conclude that, in order to effectively address current ethical challenges in psychiatry, we need to rethink policies, services, training, attitudes, research methods and codes of ethics, with the concurrent input of a range of stakeholders, open minded discussions, new models of care, and an adequate organizational capacity to roll‐out the implementation across routine clinical care contexts, training and research.

Abstract:It was not so long ago, at least in the United States, that practitioners of the various mental health disciplines who were called upon to work side by side under the same roof were each consigned a signature task and turf. When we four editors of this journal were clocking our hours toward licensure in clinics and hospitals, psychiatrists were, almost always, the team leaders in clinical settings and the primary investigators in research. Social workers attended to systems: families, couples, groups, and to the agencies that provided social services. Psychologists were the data scientists proficient in psychotherapy as well as psychometrics and psychological assessment. Therapy was something we were invited to augment with empirical research, perhaps with some background in philosophy and social science.

Abstract:In this article, the author synthesizes 25 years of his research on the personal therapy of mental health professionals. The author summarizes the high prevalence of personal treatment, its use by seasoned practitioners, its personal and professional goals, its typically positive outcomes, and its lasting lessons concerning the practice of psychotherapy. Particular attention is devoted to the therapist's selection of a personal therapist and the conduct of treatment with patients who are themselves psychotherapists in comparison with patients who are laypersons. The cumulative results indicate that personal therapy is an emotionally vital, interpersonally dense, and professionally formative experience that should be central to the development of health care psychologists.

[Ethical foundations of institutional psychotherapy]

Abstract:Historical aspects: The idea behind this work is to have an ethical examination of the institutional psychotherapy movement which has long influenced French public psychiatry and which has progressively, since the 80s, been subject to growing doubts. In the first part, institutional psychotherapy is presented. It is a model for theoretical development and practice in psychiatric care. It came into being just following the end of the Second World War at the same time as modern medical ethics. Its principles come on the one hand, from recognition of asylums' pathogenic effects--which led to the crushing of the patient's being--and on the other, through recognition of the uniqueness of each person and the subjectivity of mental suffering. These elements gave rise to creativity within the world of medicine and, in the sector, generated the science of psychiatry which advocated for continuity in care (both inpatient and outpatient) and preventive work directed at the population. This movement called for the use of the institution in its dynamic aspect which promotes exchanges and allows patients to situate or resituate themselves in historic and symbolic dimensions. It privileges a high level of transversality, maximum communication, favouring speaking out loud and responsibility. It requires a permanent analysis of the institutional counter transference (emotional reactions of the caregivers involved, their interrelations and the social and material organization of the institution) which determines the therapeutic action itself. THEORICAL BASIS: In a second part, its theoretical foundations and its practice shall be investigated in light of the guiding concepts of medical ethics (justice, autonomy, beneficence, non-malfeasance). Institutional psychotherapy responds to the need for justice by considering the patient as a whole and by conceiving each patient as being like oneself despite the differences (associated with the mode of hospitalization, the social or diagnostic category). The principle of autonomy lies always at care's horizon, made concrete in practice by notions of habitability, orientation to place and time, references, by activities and by meetings. The principle of beneficence was the basis for round-the-clock intakes, the use of an established theoretical model and clinical practice centred on the patient's words. Institutional analysis attempts not to repeat the alienation felt by the patient, alienation being associated with the treatment environment, and draws from the principle of non-malfeasance. It therefore appears that the approach of institutional psychotherapy conforms with ethical requirements, given its major interest in the subject who aspires to find meaning in life. Current situation: The third part discusses current orientations in care related to the explosion in neuroscience and technology, the promotion of the citizen as an individual, and legal doctrine, budgetary constraints, and new demands made on psychiatry from the social and political domains. The widespread trend towards simplification, swollen with hypotheses from neuroscientific research, is progressively reducing mental illness to target symptoms. The recovery of the notion of citizenship through technological capture and ideological strains in contemporary culture have also affected a suspension of the subject as a thinking and desiring being, and exempted caregivers from considering transferral phenomena, indicated with the appearance of new signifiers: user, stress, plague, network. The new medical-technical jargon of scales, tables, and management participates in the same process of patient objectification and care compartmentalization. In this context, under the cover of science and generally good actions, psychiatry has become biologisized, whilst being diluted from its social aspect, even as it becomes more repressive for patients. Conclusion: This observation leads to the conclusion, in the fourth part, that there is a need for psychiatry, which within its own discipline has a hard time finding sufficient resources, to refocus itself ethically. This enlightenment could come within a multi-disciplinary ethical space, uniting practitioners and psychiatric caregivers, psychoanalysts, philosophers, sociologists, neurobiologists etc. The research should concern the legitimacy of the principles that underlie psychiatric action in its care and preventive aspects.

Abstract:Embracing a history of psychoanalytic thought, this paper reimagines therapeutic practice to honor the dignity and subjectivity of persons with psychosis. Drawing on psychoanalytic perspectives, the author critiques the mechanisms that segregate individuals with severe psychiatric distress, perpetuating isolation and marginalization. Highlighting the institutional circuit that confines many to a cycle of transient care, the paper argues for a psychoanalytic clinic of solidarity that fosters genuine relationality. It critiques prevailing treatment paradigms that prioritize symptom suppression over existential engagement, advocating instead for therapeutic approaches rooted in empathetic presence and mutual recognition. Ultimately, it proposes solidarity as a foundational principle for transformative care that seeks to restore agency and belonging to persons with psychosis.

During their twice-weekly rounds, Dr Stern and other members of the Consultation Service discuss diagnosis and management of hospitalized patients with complex medical or surgical problems who also demonstrate psychiatric symptoms or conditions. Abstract:The author traces the prestigious history of the Burghölzli, cradle of Swiss psychiatry. He also describes the outstanding role played by Zurich as the city in which integration took place between the developing psychiatry, psycho-analysis, psychodynamics and biological research. The author shows how psychiatry, which had been faced with the mechanistric and philosophical dilemma of an epoch, could disengage from the dangers of a reductive approach thanks to Freud's influence, as well as the knowledge produced by biological and psychopathological discoveries. A new equilibration was, thus, found and a multidimensional model came into being. Today psychiatrists may choose to become either clinicians, or psychoanalysts, or devote themselves to research. Contract has become essential as well as the recognition of these different domains, this in order to improve the understanding of mental disorder.

No special contribution can be singled out as good or worse than average. What makes this volume unusual and exciting is "A Short History of Child Analysis" by Anna Freud, presented at the first scientific meeting of the American Association for Child Psychoanalysis. Abstract:This book is a true story about a precociously talented young man who develops severe psychosis but perseveres despite debilitating symptoms and graduates from Yale Law School. Essentially, it is a love letter from his best friend from childhood, the author Johnathan Rosen. On another level, it is a clinical dissection of the chaos of mental health policymaking in the USA from the 1960s onwards. There was a heady brew of forces ranging from Marxism, postmodernism, libertarianism and anti-statism to the family experiences of powerful people such as John F. Kennedy (JFK), which led to the wholesale rewriting of mental health policy and provision. Our protagonist, Michael Laudor, was very much caught in this crossfire with disastrous personal consequences for him and others. For me, the book also has a personal resonance as all the story takes place during my lifetime, including my training and practice as a psychiatrist. There are clues early in the book that Michael is not neurotypical. He is nicknamed 'toes' as he always seems to be on tiptoes. He also discloses in his early teens that his paternal grandma had schizophrenia. In his late teens, he develops erratic circadian rhythms spending 'whole days in his room with the lights out'. During his undergraduate years at Yale, a clear and severe psychosis gradually develops, leading to hospital admission and treatment with antipsychotics. He responds somewhat to medication but remains psychotic for the duration of the book. The 'community care' he clearly requires is missing. He is supported by friends and family (a number of whom are psychiatrists), but his suffering and risk are both minimised by the prevailing winds of Szasz's The Myth of Mental Illness , Reference Szasz 1 which 'elevated untreated victims of a terrible disease to the status of free spirits'. Michael stops taking his medication and has a severe relapse with fatal consequences. JFK signed the Community Care Act in 1963 as one of his final bills prior to his assassination. He aspired to replace the 'cold mercy of mental hospitals' with the 'open warmth of community, concern and capability'. Reference Kennedy 2 This led to the widespread closure of asylums but a failure to re-provide the care and support in the community. This failure of community care along with the triumph of 'autonomy' over all other principles (such as duty of care) is something that I am sure rings bells with many modern-day psychiatrists and which played its part in the disastrous outcome of the story. This book should be essential reading for all psychiatrists who should continue to remind the populace that mental illness is real and severe.

Abstract:It is clear that we cannot distinguish the sane from the insane in psychiatric hospitals. The hospital itself imposes a special environment in which the meanings of behavior can easily be misunderstood. The consequences to patients hospitalized in such an environment—the powerlessness, depersonalization, segregation, mortification, and self-labeling—seem undoubtedly countertherapeutic. I do not, even now, understand this problem well enough to perceive solutions. But two matters seem to have some promise. The first concerns the proliferation of community mental health facilities, of crisis intervention centers, of the human potential movement, and of behavior therapies that, for all of their own problems, tend to avoid psychiatric labels, to focus on specific problems and behaviors, and to retain the individual in a relatively non-pejorative environment. Clearly, to the extent that we refrain from sending the distressed to insane places, our impressions of them are less likely to be distorted. (The risk of distorted perceptions, it seems to me, is always present, since we are much more sensitive to an individual's behaviors and verbalizations than we are to the subtle contextual stimuli that often promote them. At issue here is a matter of magnitude. And, as I have shown, the magnitude of distortion is exceedingly high in the extreme context that is a psychiatric hospital.) The second matter that might prove promising speaks to the need to increase the sensitivity of mental health workers and researchers to the Catch 22 position of psychiatric patients. Simply reading materials in this area will be of help to some such workers and researchers. For others, directly experiencing the impact of psychiatric hospitalization will be of enormous use. Clearly, further research into the social psychology of such total institutions will both facilitate treatment and deepen understanding. I and the other pseudopatients in the psychiatric setting had distinctly negative reactions. We do not pretend to describe the subjective experiences of true patients. Theirs may be different from ours, particularly with the passage of time and the necessary process of adaptation to one's environment. But we can and do speak to the relatively more objective indices of treatment within the hospital. It could be a mistake, and a very unfortunate one, to consider that what happened to us derived from malice or stupidity on the part of the staff. Quite the contrary, our overwhelming impression of them was of people who really cared, who were committed and who were uncommonly intelligent. Where they failed, as they sometimes did painfully, it would be more accurate to attribute those failures to the environment in which they, too, found themselves than to personal callousness. Their perceptions and behavior were controlled by the situation, rather than being motivated by a malicious disposition. In a more benign environment, one that was less attached to global diagnosis, their behaviors and judgments might have been more benign and effective.

Abstract:Many patients have lasting disorders due, for example, to excessive and chronic childhood stress. For these patients, certain psychotherapeutic approaches may be maximally effective, and this may be universally the case. This piece is intended to give providers optimal tools for reaching and helping these patients who, otherwise, may remain among those worst off. These interventions should enhance patients' trust, the quintessential precondition for enabling these patients to change. Specific interventions discussed include anticipating ambiguity and clarifying this before ambiguity occurs, therapists indicating that they will support patients' and families' wants over their own views, feeling and disclosing their emotions, validating patients' anger, laughing, going beyond usual limits, explaining why, asking before doing, discussing religion and ethics, and informing whenever this could be beneficial.
