Abstract:Objective To study the clinicopathologic characteristics of pulmonary sclerosing hemangioma(SH).Method Eleven cases of SH were studied by the clinical data and immunohistochemistry and review of the literature.Results SH has a characteristic architecture composed of round cell and surface cubic cell.It has a variety of tissular structure,mainly expression with four regions that contain solid regions,papillary regions,angiomatoid regions and sclerosing regions.Immunohistochemically,these two kinds of cells were both positive for TTF-1 and EMA.Conclusions SH is a tumor that originates from the epithelial cell of primary respiratory tract.Both the solid round cells and surface cubic cells are neoplastic.SH should differentiate with inflammatory,typeⅡpneumocyte papillary tumor,alveolus adenoma and typical carcinoid tumors.The parts of SH have invade grow tendency.Surgical cure of SH is suited for operating a lobe of the lung but not just peel of the tumor.

Abstract:Objective To demonstrate the computed tomography (CT) features of sclerosing pneumocytoma (SP).Methods Sixteen cases of pathologically proven sclerosing pneumocytoma were included in this retrospective review.Resuits Female patients of SP were markedly frequent (n =14,87.5％).14 patients ≥50 years old.These nodules (n =15) were round (n =8),oval (n =5),mildly lobulated (n =2) in shape with a smooth boundary.Among the 16 patients,15 patients had a single lesion,1 patient had multiple lesions.The presence of overlying vessel sign (n =7),halo sign or GGO (n =6),egg and bird's nest sign (n =2),air-trapping sign (n =5) were the features in our cases.Two of 16 patients had two tumours in the lung,with the histopathology of SP and adenocarcinoma in situ,respectively.One case had been suspected for adenocarcinoma in small biopsies.Another case of SP with neuroendocrine differentiation (neuroendocrine tumor).Conclusion The features of tumor morphology and tumor periphery have important implications for the diagnosis and differential diagnosis with other tumors.
Pulmonary Sclerosing Pneumocytoma: An Essential Differential Diagnosis for a Lung Nodule

Abstract:Pulmonary sclerosing pneumocytoma, previously known as pulmonary sclerosing hemangioma, is a rare benign lung tumor with a low prevalence. We present this condition in a 26-year-old, young, non-smoking female with a slow-growing pulmonary nodule incidentally noted on imaging. Serial computed tomography(CT) scans revealed slow growth, and invasive testing was recommended. The patient underwent a left lateral thoracotomy and based on frozen section findings. A left lower lobectomy was performed. The final pathological diagnosis revealed sclerosing pneumocytoma. This is an atypical patient demographic considering the propensity of the disease for middle-aged Asian women. The case presentation and work-up highlight this critical differential diagnosis for incidental pulmonary nodules increasingly being noted due to widespread use of imaging for screening and routine care in the current medical climate. There are no specific imaging criteria to diagnose this condition. The final diagnosis is made only after surgical biopsy and histopathology. No additional treatment is needed following the diagnosis.
Pulmonary Sclerosing Pneumocytomas Mimicking Lung Cancer

Abstract:Pulmonary sclerosing pneumocytomas are benign tumors. These tumors are often found incidentally and can be challenging to distinguish from lung malignancies. Here, we describe the case of a 31-year-old woman who presented with an incidental finding of a lung nodule in the lingula. She was asymptomatic and had no history of cancer. Positron emission tomography showed [18F] fluorodeoxyglucose (FDG) uptake in the nodule but no FDG-avid mediastinal lymphadenopathy. In view of these findings, a bronchoscopy was performed, and biopsy samples were taken. The final pathological diagnosis revealed a sclerosing pneumocytoma.

Abstract:Pulmonary sclerosing pneumocytoma, formally named pulmonary sclerosing hemangioma, is a rare benign tumor with malignant potential often identified as solid pulmonary nodules. Sarcoidosis is an inflammatory, multisystemic disease of unknown cause with a wide range of clinical manifestations. The disorder is characterized by the formation of noncaseating granulomas in virtually any organ in the body. We present a case of a patient presenting with fever, weight loss, and respiratory symptoms found to have both a sclerosing pneumocytoma and pulmonary sarcoidosis. A diagnosis was made following the lobectomy. The patient was followed for two years with stable lymphadenopathy while remaining asymptomatic.

Abstract:Pulmonary sclerosing pneumocytoma (PSP) is a rare and interesting pulmonary tumour that presents a diagnostic challenge to the surgeon due to its bland symptomatology, indolent course, non-specific radiological features and ambiguous histopathological findings, which could lead to errors in management and prognostication. Abstract:Pulmonary sclerosing pneumocytoma (PSP), a rare benign lung tumor of primordial epithelial origin, commonly effects middle-aged Asian females. Diagnosis of this entity is challenging because of the non-specific radiological characteristics that resemble malignancies and its histological heterogeneity. Main differential diagnoses considered are adenocarcinoma lung and carcinoid tumor. In this case report, we discuss our experience of diagnosing a case of pulmonary sclerosing pneumocytoma, which showed increased SUV uptake in PET-CT indicating towards a malignancy and was also misdiagnosed as adenocarcinoma in CT-guided FNAC. The histology showed variable morphological features and there was a differential staining pattern of TTF1 and napsin A in the cells. We have highlighted the differential diagnosis and the challenges faced for diagnosing this benign, rare entity.

Abstract:Aspects of histogenesis and nomenclature of so called "sclerosing hemangioma" of the lung (WHO 1999) are discussed and compared with immunohistochemical findings in eight examined operation specimen. The lesion is characterised by the presence of typical surface cells, which can be related to type II pneumocytes. Progesterone-receptor positive stromal cells may derive from primitive mesenchymal cells. Endothelial proveniance of tumor cells could not be confirmed by immunohistochemistry. Therefore, this rare usually benign pulmonary neoplasm should be entitled "pneumocytoma" analogous to the suggestion of several other authors.

Abstract:Background: Pulmonary sclerosing pneumocytoma is a kind of rare benign pulmonary tumor with potential malignancy. The clinical features, risk factors for prognosis, and optimal treatment have not been identified yet. This study aimed to investigate the clinical features and prognosis of pulmonary sclerosing pneumocytoma. Methods: We retrospectively performed a review of pulmonary sclerosing pneumocytoma patients in West China Hospital from 2009 to 2019. The basic characteristics, treatment regimens, operation detail, postoperative variables, and follow-up time were recorded for each case. Differences in features between patients undergoing lobectomy and segmentectomy were compared. We also performed a case review and summarized reported clinical features in former studies. Results: Altogether 61 pulmonary sclerosing pneumocytoma patients were retrospectively reviewed. Fifty-six patients were female and 5 were male. The patients' median age was 51 (23-73). Seven (11.48%) patients had smoking history. Twenty tumors were located in the right lung [upper lobe (n = 7), middle (n = 2), and lower (n = 11)] and 41 in the left [upper (n = 12) and lower (n = 29)]. The median tumor size was 2 (0.9-7) cm. Thirty-six (59.02%) patients underwent sublobectomy (segmentectomy or wedge resection) whereas 25 (40.98%) underwent lobectomy. All patients recovered uneventfully, and no perioperative mortality was identified. Sublobectomy showed a trend towards reduced chest tube duration and shorter postoperative hospital stays compared with lobectomy. Conclusions: The findings showed good prognosis of pulmonary sclerosing pneumocytoma and proved its benign characteristics. Sublobectomy showed advanced efficacy regarding chest tube duration and postoperative hospital stay compared with lobectomy.
Multiple sclerosing pneumocytomas: a review

Abstract:Sclerosing pneumocytoma (SP) is a rare benign low-grade tumour of the lung, and typically presents as single discrete coin lesions on imaging. Multiple SP is an exceedingly rare entity and thus reported sparingly. We review the literature on multiple SP, their clinical presentations, histopathology, relevant differential diagnoses and molecular histogenesis of this entity. SP has a predilection for East Asian origin females who have never smoked. Patients are either asymptomatic or have symptoms such as cough, haemoptysis that may be persistent, chest pain if involving the pleura and presents as discrete coin lesion on chest X-ray. Histologically, they are papillary, solid, angiomatoid or sclerotic, or combinations of these four basic patterns. Multiple lesions have the same or slightly different histological patterns. They can be distributed in either lung, in any lobe and can even be bilateral. AKT-1 molecular pathways are pivotal in their molecular pathogenesis. In this review, we further propose a classification based on five types of distribution of multiple SP.

Abstract:Pulmonary sclerosing pneumocytoma is a rare, benign lung tumor that usually occurs in middle-aged women. It usually manifests as isolated pulmonary nodules and is often confused with other solitary, benign nodules. We present a case of sclerosing pneumocytoma with clustered lung nodules and surrounding ground-glass opacity in a single lobe in a young man. Progressive growth was seen on serial computed tomography observations, and the nodules appeared similar to those seen in active pulmonary tuberculosis.
Comprehensive Genomic and Transcriptomic Analysis of Sclerosing Pneumocytoma

Abstract:Sclerosing pneumocytoma is a rare and distinct lung neoplasm whose histogenesis and molecular alterations are the subject of ongoing research. Our recent study revealed that AKT1 internal tandem duplications (ITD), point mutations, and short indels were present in almost all tested sclerosing pneumocytomas, suggesting that AKT1 mutations are a major driving oncogenic event in this tumor. Although the pathogenic role of AKT1 point mutations is well established, the significance of AKT1 ITD in oncogenesis remains largely unexplored. We conducted comprehensive genomic and transcriptomic analyses of sclerosing pneumocytoma to address this knowledge gap. RNA-sequencing data from 23 tumors and whole-exome sequencing data from 44 tumors were used to obtain insights into their genetic and transcriptomic profiles. Our analysis revealed a high degree of genetic and transcriptomic similarity between tumors carrying AKT1 ITD and those with AKT1 point mutations. Mutational signature analysis revealed COSMIC signatures 1 and 5 as the prevailing signatures of sclerosing pneumocytoma, associated with the spontaneous deamination of 5-methylcytosine and an unknown etiology, respectively. RNA-sequencing data analysis revealed that the sclerosing pneumocytoma gene expression profile is characterized by activation of the PI3K/AKT/mTOR pathway, which exhibits significant similarity between tumors harboring AKT1 ITD and those with AKT1 point mutations. Notably, an upregulation of SOX9, a transcription factor known for its involvement in fetal lung development, was observed in sclerosing pneumocytoma. Specifically, SOX9 expression was prominent in the round cell component, whereas it was relatively lower in the surface cell component of the tumor. To the best of our knowledge, this is the first comprehensive investigation of the genomic and transcriptomic characteristics of sclerosing pneumocytoma. Results of the present study provide insights into the molecular attributes of sclerosing pneumocytoma and a basis for future studies of this enigmatic tumor.

Abstract:Introduction and importance: Pulmonary sclerosing pneumocytoma (PSP) is a rare non-cancerous lung tumor that is usually asymptomatic, but may cause respiratory distress if it becomes large. PSPs are often detected incidentally because of their slow growth, lack of symptoms, characteristic radiographic features, and increased use of imaging studies. Although it is not a malignant tumor, it can mimic malignancy on imaging and histology, leading to misdiagnosis and unnecessary surgery. Case presentation: A 23-year-old asymptomatic female was incidentally diagnosed with PSP during evaluation for a breast fibroadenoma. A chest CT revealed a 3 cm lobulated mass in the left upper lobe. Cytology showed malignant cells with necrotic debris. Immunohistochemistry was positive for TTF-1 and EMA, negative for p63 and AE1/AE3. Histopathology confirmed a well-circumscribed benign neoplasm, consistent with pulmonary sclerosing pneumocytoma. There was no mediastinal lymph node invasion, and the post-surgery prognosis was good. Clinical discussion: PSP is a slow-growing tumor that is often asymptomatic until it reaches a significant size. Owing to their well-circumscribed margins and the presence of calcifications, they are often detected incidentally during imaging studies, such as routine chest radiography or CT scans for unrelated conditions. Although these tumors are often incidental, it is important to diagnose and treat them appropriately to prevent potential complications and malignant transformation. Conclusion: The findings of this study contribute to the existing literature, increase awareness of this rare tumor, and provide insights into its diagnosis, treatment, and follow-up.

Abstract:Pulmonary sclerosing pneumocytoma (PSP) is a quite rare tumor outside Eastern countries. This rarity, together with a wide histological appearance, makes its correct identification a diagnostic challenge for pathologists under the microscope. Historically, PSP was considered a vascular-derived neoplasm (sclerosing hemangioma), but its immunohistochemical profile clearly supports its epithelial origin. No specific molecular fingerprint has been detected so far. This short narrative revisits the clinical, histological, immunohistochemical, and molecular aspects of this tumor, paying special attention to some controversial points still not well clarified, i.e., clinical aggressiveness and metastatic spread, multifocality, the supposed development of sarcomatoid change in a subset of cases, and tumor associations with lung adenocarcinoma and/or well-differentiated neuroendocrine hyperplasia/tumors. The specific diagnostic difficulties on fine-needle aspiration cytology/biopsy and perioperative frozen sections are also highlighted. Finally, a teaching case of tumor concurrence of lung adenocarcinoma, neuroendocrine lesions, and PSP, paradigmatic of tumor association in this context, is also presented.
[Analysis of Clinical Characteristics of 35 Cases of Pulmonary Sclerosing Pneumocytoma]

Abstract:Background: Pulmonary sclerosing pneumocytoma (PSP) is rare benign lung tumor which usually develops in middle-aged women without typical clinical and imaging findings. PSP consists of two basic cell types (surface cubic epithelial cells and round mesenchymal cells) and four histological types (hemorrhagic, sclerotic, solid and papillary). It grows slowly, but it can metastasize to distant organs. The pathology before surgery is easily misdiagnosed. This study aims to improve clinicians' understanding of PSP by discussing the clinical characteristics of the disease. Methods: This represents a retrospective study of thirty-five patients diagnosed with pulmonary sclerosing pneumocytoma by pathological examination from January 2011 to December 2019. Results: A total of 35 patients in this study, 12 cases were male and 23 cases were female, the average age is 51 years old. 7 cases were discovered accidentally by physical examination or routine chest computed tomography (CT), and 28 cases were found due to symptoms such as cough, sputum, hemoptysis and chest pain. The imaging changes is mainly featured with isolated or clear circular or round-like single nodule and lump in the lungs. In this group, 12 cases underwent percutaneous lung biopsy, only 7 cases were diagnosed with PSP. A total of 28 patients underwent surgery, 24 cases underwent rapid frozen pathological biopsy, only 5 cases diagnosed with PSP. Postoperative pathological examination results shows that 1 case was diagnosed with keratotic squamous cell carcinoma with partial PSP, and the rest were diagnosed with PSP. The surgical and non-surgical patients were followed up for 1 to 8 years after discharge, and the overall recovery was good. The patients were no recurrence and metastasis on chest CT review. Conclusions: PSP is a clinically rare benign lung tumor, which is more common in middle-aged women. The clinical manifestations and imaging features are lack of significance. Percutaneous lung puncture pathological examination and intraoperative rapid frozen pathological sections often leads to misdiagnosis. Final diagnosis relies on postoperative pathological work-up for most cases.
Sclerosing Pneumocytoma: A Host for a Typical Carcinoid With Pleural Metastasis-A Wolf in Sheep`s Clothing

Abstract:Sclerosing pneumocytoma (SP) is a rare primary lung tumor. Typical carcinoids (TCs) count for 2% of lung malignancies. A description of a combined neoplasm of SP with a nodal and pleural metastasized TC has, to our knowledge, never been published. A 57-year-old actively smoking woman received a diagnosis of a lesion in the left lower lobe via a screening CT scan for rheumatoid arthritis. A fluorodeoxyglucose-PET scan confirmed a 21 × 26 × 16 mm (standardized uptake maximum value, 3.0), well-circumscribed round lesion with calcification, which was thought to be most probably benign. No mediastinal lymph node enlargement or fluorodeoxyglucose uptake was detected. The results of routine laboratory tests, respiratory function tests, and physical examination were unremarkable. In diagnostic thoracoscopy pleural, diaphragmatic, and pericardial lesions were discovered and biopsied in addition to a wedge resection. After diagnosis of a pleural metastasized TC mixed with SP, radical resection and systemic lymph node dissection were performed. The patient is in remission after 36 months of follow-up.

Abstract:Sclerosing pneumocytoma is an uncommon pulmonary tumor which generally behaves benignly and occurs predominately in women. Rarely, it is associated with neuroendocrine proliferations such as hyperplasia, tumorlets and carcinoid tumors, which may be observed in relation to the tumor or in the distant lung parenchyma; the mechanism underlying this neuroendocrine differentiation is not clear. We present a case of a 33 year-old male with sclerosing pnemocytoma with coexistent neuroendocrine hyperplasia and combined carcinoid tumorlets. Taking into account the pluripotentiality of the round cells present in the sclerosing pneumocytoma, with positive staining for stem cells markers, it is possible that the different components of this neoplasia share a common origin, in accordance with previously reported findings.
Pneumocytoma: a Case Report and Review of Literature

Abstract:Pneumocytoma is a rare benign neoplasm which was described over 50 years ago. Over the years its nomenclature and histogenesis has been a subject of much debate and controversy. We would like to report a case of a lady who presented with complaints of cough and hemoptysis. X-ray chest and computerized tomography (CT) scan showed a solitary pulmonary nodule. Biopsy was inconclusive. She underwent a left lower lobectomy. Her postoperative period was uneventful. Her final diagnosis was of pneumocytoma. She is on regular follow-up and is asymptomatic at present.

Abstract:Introduction Sclerosing pneumocytoma (SP) is a rare benign tumor and a potential diagnostic pitfall. Our aim was to review the cytologic features of our surgically diagnosed SP cases including the clinical, immunohistochemical and available molecular findings. Materials and Methods A computerized search from 2013 to 2020 for surgical cases with corresponding cytology specimens diagnosed as SP was performed. The clinical data, cytology, and surgical specimens were collated for analysis. Results Six cytology specimens were collected. All were female (mean age = 35). Three have incidental lung nodules and three with cough. Cytologic findings showed variable architectural pattern (papillary, solid, singly scattered, acinar/rosette‐like) and cellular heterogeneity (surface, stromal, epithelioid, plasmacytoid cells). Atypia was identified in 4/6 cases. The original cytology diagnoses were negative = 1, SP = 2 and adenocarcinoma = 3. The latter diagnoses were amended to SP after review of the surgical specimens. The three false positive cases on review have cytologic features mimicking adenocarcinoma. Immunohistochemical stains showed tumor cells (surface and stromal) were positive for TTF‐1, and EMA with only the surface cells positive for pancytokeratin and Napsin A. Though two cases sent for molecular testing were negative for AKT1 or CTNNB1 exon 3 mutation, our panel did not evaluate AKT1 exon 4. Conclusions SP is a diagnostic pitfall with 50% initially misdiagnosed as adenocarcinoma. Integrating the clinical/radiologic findings, cytologic features, and performance of immunohistochemistry on cell block are helpful in avoiding misdiagnosis.
