Abstract:Background: Myotonic dystrophy type 1 (MD1) is the most common neuromuscular disease in adults. Patients may present with limb muscle impairment, respiratory pump failure (RPF) and sleep disordered breathing (SDB). Whether limb muscle impairment correlates with the respiratory function is not known. Aim: To describe the correlation between 4-limb hypotonia, as assessed by the muscular impairment rating scale (MIRS), pulmonary function impairment and the need for non-invasive home mechanical ventilation (HMV) in MD1. Methods: We retrospectively analysed the data of all 121 patients followed from 2004 to 2023 in our neuromuscular disease outpatient clinic with MD1. Results: In our sample (54% male, mean age 39±17 ys.), MIRS score at first visit was available in 95 patients. MIRS was 1 in 8%, 2 in 36%, 3 in 41% and 4 in 15% of patients. HMV was prescribed in 49.6% of patients, 23.2% for SDB and 26.4% for RPF (pCO2 <45 mmHg and/or vital capacity (VC) <70% predicted). There was no statistically significant difference between the MIRS of patients with or without indication for HMV (p=0.7, 2) and between patients ventilated for SBD or RPF (p=0.2, 2). In contrast, in the whole sample, patients with reduced VC had significantly higher MIRS (p=0.002, 2). In the RPF group, MIRS was significantly higher in reduced VC group with or without hypercapnia compared to patients with only hypercapnia (p=0.027, 2). Conclusion: A high MIRS at the first visit seems to predict the development of functional impairment, as assessed by VC reduction. However, given the prevalence of other indications for HMV besides VC reduction, all MD1 patients should be followed in the pulmonary setting regardless of MIRS.

Abstract:A series of sulfur-containing congeners have been prepared from alpha-ethyl-alpha-methyl-gamma-butyrolactone, beta-ethyl-beta-methyl-gamma-butyrolactone, and alpha,alpha,beta,beta-tetramethyl-gamma-butyrolactone as potential neuropharmacologic agents. The lactones were treated with benzyl mercaptide anion to form 4-(benzylthio)butyric acid, which, on treatment with trifluoroacetic acid, cyclized to yield thiololactones. The thiono- and dithiolactones were prepared by treating the corresponding lactones either with Lawesson's reagent or with phosphorus pentasulfide, respectively. As had been observed previously for the lactones, the beta-substituted and alpha,beta-substituted congeners were potent convulsants that caused generalized clonic and tonic seizures in mice. The alpha-substituted congeners were effective in inhibiting pentylenetetrazole-induced seizures in mice. alpha-Ethyl-alpha-methylthiolo-gamma-butyrolactone showed an increase in potency over the congeneric alpha-ethyl-alpha-methyl-gamma-butyrolactone and, additionally, was effective against maximal electroshock seizures. In no cases was a convulsant converted to an anticonvulsant or vice versa by sulfur-for-oxygen substitution.
Integrating the Kaltura Video Platform with the Photodentro Video Repository: A Case Study

Abstract:This work presents a case study for the integration of two web-based systems, the "Kaltura" video platform and the "Photodentro Video" digital repository. The integration is based on the Open Archives Initiative Protocol for Metadata Harvesting and an associated automation, transformation, and push/pull notification strategy. Kaltura is a video management platform, which allows publishers and content producers to distribute, manage, and monetize their videos and other rich-media content. Photodentro Video is the Greek Educational Video Repository for primary and secondary education, based on the DSpace open source digital repository management system. The architecture and the orchestrated workflow of the integration of Kaltura and Photodentro Video are presented, with emphasis on the addressed challenges. Furthermore, the technologies for scaling up the video service provided by Kaltura to a massive number of end-users are reviewed and discussed along with the proposed solution. Finally, the performance of the overall integrated platform is evaluated.
Age-related self-stigma of people over 65 years old: adaptation of the Internalized Stigma of Mental Illness scale (ISMI) for use in age-related self-stigma (IS65+) in a Spanish sample

Abstract:ABSTRACT Objectives: To adapt the Internalized Stigma of Mental Illness scale (ISMI) to examine self-stigma associated with aging and to study the psychometric properties of this adapted version (IS65+). Finally, self-stigma associated with age in older people is studied. Method: The IS65+ was administered to a random sample of 419 people over 65 years from Madrid (Spain) to study the psychometric properties of this adapted version. A regression model was estimated to identify the variables that best predict self-stigma associated with old age. Results: The IS65+ showed good internal consistency (α = .89) and a factorial structure of five factors. The data showed lower levels of self-stigma related to age in the sample than the levels of mental illness self-stigma in people with mental illness. The variables associated with age-related self-stigma are: high levels of perceived loneliness, low levels of coping strategies, gender (female), mental disorder, major depressive disorder, low levels of optimism and quality of life, and high levels of functional impairment. Conclusion: A new version of ISMI (IS65+) with acceptable psychometric criteria has been developed for use in people over 65 years old.

Abstract:PURPOSE: The authors determined the degree of respiratory and peripheral muscle weakness in patients with moderate to severe chronic obstructive pulmonary disease (COPD). Differences in severity of muscle weakness among muscle groups may provide treatment options, such as selective muscle training, to adapt the exercise prescription in pulmonary rehabilitation programs. In addition, this information may add to the knowledge on the mechanisms of muscle weakness.METHODS: Respiratory and peripheral muscle force were quantified in 22 healthy elderly subjects and 40 consecutive COPD patients (forced expiratory volume in 1 second, percent of predicted value [% pred] 41 +/- 19; transfer factor for carbon monoxide, % pred 47 +/- 26) admitted to a pulmonary rehabilitation program. Lung function, diffusing capacity, isometric force of four peripheral muscle groups (handgrip, elbow flexion, shoulder abduction, and knee extension), neck flexion force, and maximal inspiratory and expiratory pressures were measured.RESULTS: Patients had reduced respiratory muscle strength (mean 64% of control subjects' value [% control]) and peripheral muscle strength (mean 75% control) compared to normal subjects. Inspiratory muscle strength (59 +/- 18% control) was significantly lower than expiratory muscle strength (69 +/- 25% control) and peripheral muscle strength (P < 0.01). Neck flexion force (80 +/- 19% control) was better preserved than maximal inspiratory pressure and shoulder abduction force (70 +/- 15% control, P < 0.01). Handgrip force (78 +/- 16% control) and elbow flexion force (78 +/- 14% control) were significantly less affected than shoulder abduction force (70 +/- 15% control, P < 0.01). Finally, shoulder abduction force and knee-extension force (72 +/- 24% control) were not significantly different.CONCLUSIONS: Muscle weakness in stable COPD patients does not affect all muscles to a similar extent. Inspiratory muscle force is affected more than peripheral muscle force, whereas proximal upper limb muscle strength was impaired more than distal upper limb muscle strength.

Abstract:INTRODUCTION: Chronic obstructive pulmonary disease (CPOD) is a severe, incapacitating pathology. Inspiratory and/or expiratory muscle training may favorably impact the indicators of both specific and general improvement with regard to this disease. We are hypothesizing that when combined with bronchial decluttering, this training will have a beneficial effect on lung function and quality of life in these patients.METHOD: Fourty COPD subjects classified Gold I and Gold II and aged 60.38±8.02years were divided into four groups of 10. Three of the groups were trained with the help of Threshold(®) tools used for (1) inspiratory, (2) expiratory and (3) inspiratory and expiratory purposes; their training supplemented the decluttering and lower limb muscle exercise that the 4th group concurrently received. The patients underwent 16 rehabilitation sessions over an 8-week period. The variables consisted in: (1) forced expiratory volume in 1s (FEV1) and spirometrically measured peak expiratory and inspiratory flow rates (PEFR and PIFR); (2) fatigability, dyspnea, heart rate and walking distance evaluated during the 6-minute walk test; (3) maximum inspiratory pressure and (4) maximum expiratory pressure as assessed by the Threshold(®) tools and (5) the signs of quality of life in terms of the Saint-George's respiratory questionnaire (SGRQ) score.RESULTS: Only in group 1, there was significant improvement with regard to FEV1 and PEFR. There was no PIFR modification in any of the groups. On the other hand, signs of quality of life scores along with dyspnea, fatigability and heart rate showed significant improvement in the three experimental groups, and significant improvement in maximum inspiratory pressure was observed in groups 1 and 3.DISCUSSION: When associated with decluttering techniques, diaphragmatic rehabilitation and lower limb muscle exercise along with psychological support and educational efforts, respiratory muscle training is beneficial when compared with the usual protocols in rehabilitation of COPD patients.

Abstract:CHILDREN with intellectual and educational handicaps that are not accompanied by any clear-cut neurological deficits can present problems to both psychologist and neurologist, and there is a wide diversity of opinion as to their origin. In some respects such cases challenge the traditional dichotomy which has grown up between psychology and neurology: they do not easily fit into the conceptual model of psychiatry, with its emphasis on early instinctual and emotional development, neither are they easily assimilated into clinical neurology. Some look on these intellectual and educational handicaps as essentially an emotional affair, while others find the idea of an emotional holdup inadequate. It is certainly worth exploring the possibility that there is an organic basis to these disabilities, though the approach in this field has been limited both by speculative theories of brain function and by too little regard for the educational issues. In his talk to the Association for Child Psychology and Neurology on November 8 , on Common Problems in Child Neurology and Psychologj., Prof. 0. L. Zangwill described cases of children who have a good verbal I.Q. but a poor visual performance. At school, such children are often poor at mathematics, science and geography, though good at languages: they are clumsy without being apraxic; they are poor at practical skills; they are often left-handed; they have poor spatial judgement, and in particular they confuse left and right, which probably accounts for their poor performance in geography. These characteristics are usually not accompanied by any history of gross neurological disorders, and the EEG may be normal or show only slight abnormalities. There are also a few children who are no good at language skills, and others who have good spatial performance but a poor verbal I.Q. These children often have poor speech development. Again there may be no history of birth trauma: the EEG is often normal and they have no clear neurological disorders. The children therefore present fairly specific educational and psychological problems; they are often temperamentally instable, restless and anxious. But are these essentially psychiatric cases ? Should we not regard their defects as extreme instances of normal abilities linked to individual differences, or are we rather faced with a syndrome of personality disorder? Abstract:Plasma membrane-enriched preparations from scrapie-infected and healthy hamster brains, as well as preparations of neural retina, were sonicated, then separated by rate-zonal sedimentation in 10 to 25% Nycodenz gradients. Gradient fractions were extracted with 0.5% Triton X-100 and re-fractionated by equilibrium density centrifugation in linear 25 to 40% CsCl gradients. Infectivity was highest in a fraction having a density of 1.280 g/ml and which contained a visible band of material. Digestion of the Nycodenz fractions with proteinase K before detergent extraction and CsCl fractionation resulted in a shift in the visible band to a density of 1.235 g/ml with most of the scrapie infectivity remaining at 1.280 g/ml. When labelled with 125I after 40-fold concentration, this 1.280 g/ml CsCl fraction from the proteinase K-treated gradients contained only a single band of protein(s) having a mol. wt. near 30 000. No differences were seen between proteins in healthy or scrapie-infected preparations.
Respiratory Trajectories and Correlation with Serum Biochemical Indices in Spinal and Bulbar Muscular Atrophy

Abstract:Background/Objectives: The primary life-threatening complication in spinal–bulbar muscular atrophy (SBMA) is ventilatory failure. The present study analyzes the longitudinal patterns of respiratory function tests over a follow-up of 11 years. Methods: We collected data from 9 genetically confirmed SBMA patients. Spirometric measurements [maximum inspiratory pressure (MIP), maximum expiratory pressure (MEP), and forced vital capacity (FVC)], serum biochemical indices and SBMA functional rating scale (SBMAFRS) were collected every 6 months for 11 years. An average time curve was utilized to assess the changes in both pulmonary tests and serum biochemical indices of the patients. One-way repeated-measures ANOVA was applied to assess statistical differences. The Spearman's rank correlation coefficient was utilized to evaluate the correlations between the respiratory function tests and serum biochemical and clinical indices. Results: A progressive decrease was observed in the respiratory function tests; the slope of the linear regression was significantly non-zero (p < 0.0001) for all three time curves. A major decrease was observed for MEP (52%) and MIP (42%), while this was minor for FVC (25%). SBMAFRS score correlated with FVC (r = 0.27), MIP (r = 0.53) and MEP (r = 0.51). MIP and MEP correlated with creatine phosphokinase (r = 0.3, r = 0.25, respectively) and MIP with creatinine levels (r = 0.31). Conclusions: This longitudinal study shows a progressive decline of spirometric data throughout life in SBMA patients. The decline appears to be related to clinical deterioration and muscle denervation. Spirometric measures relative to maximal strength of the respiratory muscles (MIP and MEP) may have a better predictive value for pulmonary and muscular decline than FVC.

Respiratory function and therapeutic expectations in DMD: families experience and perspective

Abstract:Objective: The aim of this study was to use a structured questionnaire in a large cohort of Duchenne Muscular Dystrophy (DMD) patients to assess caregivers and patients views on respiratory function and to establish if their responses were related to the patients' age or level of functional impairment. Methods: Questionnaires were administered to caregivers in 205 DMD patients of age between 3 and 36 years (115 ambulant, 90 non-ambulant), and to 64 DMD patients (3 ambulant, 61 non-ambulant) older than 18 years, subdivided into groups according to age, FVC, ambulatory and ventilatory status. Results: Some differences were found in relation to FVC % values (p = 0.014), ambulatory (p = 0.043) and ventilatory status (p = 0.014). Nearly half of the caregivers expected deterioration over the next years, with the perspective of deterioration more often reported by caregivers of non-ambulant (p = 0.018) and ventilated patients (p = 0.004). Caregivers appeared to be aware of the relevance of respiratory function on quality of life (84%) showing willingness to enter possible clinical trials if these were aiming to stabilize the progression of respiratory function with a very high number of positive responses across the spectrum of age, FVC, ambulatory and ventilatory status. The boys older than 18 years showed similar results. Conclusions: Our study showed that the concern for respiratory function increases with age and with the reduction of FVC or the need for ventilation, but the need for intervention was acknowledged across the whole spectrum of age and functional status.

Respiratory muscle training in late-onset Pompe disease: Results of a sham-controlled clinical trial

Abstract:To address progressive respiratory muscle weakness in late-onset Pompe disease (LOPD), we developed a 12-week respiratory muscle training (RMT) program. In this exploratory, double-blind, randomized control trial, 22 adults with LOPD were randomized to RMT or sham-RMT. The primary outcome was maximum inspiratory pressure (MIP). Secondary and exploratory outcomes included maximum expiratory pressure (MEP), peak cough flow, diaphragm ultrasound, polysomnography, patient-reported outcomes, and measures of gross motor function. MIP increased 7.6 cmH2O (15.9) in the treatment group and 2.7 cmH2O (7.6) in the control group (P = 0.4670). MEP increased 14.0 cmH2O (25.9) in the treatment group and 0.0 cmH2O (12.0) in the control group (P = 0.1854). The only statistically significant differences in secondary/exploratory outcomes were improvements in time to climb 4 steps (P = 0.0346) and daytime sleepiness (P = 0.0160). The magnitude of changes in MIP and MEP in the treatment group were consistent with our pilot findings but did not achieve statistical significance in comparison to controls. Explanations for this include inadequate power and baseline differences in subject characteristics between groups. Additionally, control group subjects appeared to exhibit an active response to sham-RMT and therefore sham-RMT may not be an optimal control condition for RMT in LOPD.

Abstract:Background Chronic obstructive pulmonary disease (COPD) is characterized by progressive airflow obstruction and lung hyperinflation due to loss of elastic recoil and air trapping. These changes are related to an altered pattern of ventilatory muscle recruitment. Respiratory muscle training aims to alter respiratory muscle recruitment to reduce dyspnea, hyperinflation, improve respiratory muscle performance, and optimize thoracoabdominal movement. Objectives To analyze the effect of respiratory muscle training in people with Chronic Obstructive Pulmonary Disease at different stages. Methods This is a systematized integrative literature review, carried out in the PubMed, SciELO and Physiotherapy Evidence Database (PEDro) databases, using the following search strategy: “breathing exercises” AND “maximal respiratory pressures” AND “chronic obstructive pulmonary disease”. Searches were carried out limited to the years 2019 to 2023. The inclusion criteria were randomized controlled clinical trials, carried out in humans, in Portuguese and/or English, available in full and related to the proposed topic. The selection of studies was performed by two reviewers simultaneously and independently. The research was conducted following four stages: 1) reading of titles and abstracts; 2) analysis of duplicates; 3) reading the articles in full and 4) extracting the main information. The risk of bias in clinical trials was assessed using the Cochrane Risk of Bias Tool and the quality of evidence using the Grading of Recommendations Assessment, Development and Evaluation (GRADE). Results After searching the databases, 880 articles were found. Of these, 7 articles were selected to compose the present review because they met the objective and determined criteria. The respiratory muscle training protocols consisted of fast and deep breathing training with Voldyne, inspiratory and expiratory muscle training with Threshold, pursed lip breathing, respiratory muscle training with Powerbreathe, deep breathing and blowing straw in a glass of water. A total of 472 individuals, with mild to severe COPD, were analyzed. Intervention duration and weekly frequency ranged from 1 to 18 months, 3 to 7 days a week, respectively. All studies show that respiratory muscle training improved symptoms of dyspnea and fatigue, increased muscle strength, FEV1, FVC, functional capacity and improved quality of life (p<0.05). About risk of bias, studies ranged from low to moderate and the quality of evidence ranged from moderate to very low. Conclusion It was observed that respiratory muscle training significantly contributed to improving symptoms of shortness of breath, fatigue and increased muscle strength, quality of life and functional capacity. Implications Respiratory muscle training seems to be a good treatment for patients in different stages of COPD and at any level of care, whether at home, outpatient, or hospital, in addition to improving the symptoms of the disease and the patient's quality of life.

Aerobic capacities and exercise tolerance in neuromuscular diseases: a descriptive study

Abstract:Introduction: Up until now, there has been little description of the test exercises carried out in patients with neuromuscular diseases. However, prescription of physical therapy by exercise requires rigorous individualized assessment of the patient's physical endurance parameters. Objective: To describe exercise tolerance and external limitation factors in a sample group of adult patients suffering from neuromuscular diseases. Materials and methods: A descriptive retrospective study of exercise tests realized at the Reims university hospital for patients divided into three groups according to their pathologies: CMT hereditary neuropathies, muscular dystrophies, metabolic myopathies. Results: Forty-four (44) tests were analyzed. Cessation was occasioned on 63.6% of the cases by muscular symptomatology, with no possibility of maintaining the cycling frequency in 29.5% of the overall population. Insufficient lung volume recruitment was involved in 61.4% of the patients, final oxygen pulse (VO2/heart rate) was 80% lower than the theoretical pulse in 50% of the patients, and there existed an early first ventilatory threshold in 54.5% of the cases. Peripheral deconditioning was highly severe in 18% of the population and significantly greater in the subjects suffering from dystrophies (VT1 at 31% of the maximum theoretical O2, P<0.001). Discussion: The main limitation factor in exercise tests is peripheral. Ventilatory and cardiovascular limitations can nonetheless be added on; while they are absent when the patient is at rest, they are unmasked in effort situations. Exercise tests could be of great interest in screening and managing the relevant pathologies. Multicenter studies on homogeneous populations could facilitate definition of the procedures specific to exercise tests for patients suffuring from neuromuscular diseases.

Abstract:<section class="article-section__content"><h3 class="article-section__sub-title section1"> Aims</h3><p>Cardiac surgery has improved life expectancy of patients with congenital heart diseases (CHDs). Exercise capacity is an important determinant of survival in patients with CHDs. There is a lack of studies focusing on the role of resting respiratory performance in reducing exercise tolerance in these patients.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Objectives</h3><p>To determine the prevalence and severity of respiratory functional impairment in different types of corrected/palliated CHDs, and its impact on an exercise test.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Materials and Methods</h3><p>Retrospective single‐center study involving 168 corrected/palliated patients with CHD and 52 controls. Patients CHD were divided into subgroups according to the presence of native pulmonary blood flow or total cavopulmonary connection (TCPC). All subjects performed complete pulmonary function tests and gas diffusion; patients with CHD also performed cardiopulmonary exercise test (CPX).</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Results</h3><p>Mean values of lung volumes were within the normal range in all CHD groups. Comparing to controls, patients with the reduced pulmonary flow and with TCPC had the highest reduction in lung volumes. CPX was reduced in all groups, most severely in TCPC, and it was correlated to decreased dynamic volumes in all CHD groups except in TCPC. Younger age at intervention and number of surgical operations negatively affected lung volumes.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Conclusions</h3><p>Respiratory function is within the normal range in our patients with different CHDs at rest but altered in all CHDs during exercise when cardiorespiratory balance is likely to be inadequate. Comparing the different groups, patients with reduced pulmonary flow and TCPC are the most impaired.</p></section>

Abstract:Background: Although it is commonly assumed that pulmonary congestion and edema in patients with chronic heart failure (CHF) promotes peripheral airway closure, closing capacity (CC) has not been measured in CHF patients. Purposes: To measure CC and the presence or absence of airway closure and expiratory flow limitation (FL) during resting breathing in CHF patients. Methods: In 20 CHF patients and 20 control subjects, we assessed CC, FL, spirometry, blood gas levels, control of breathing, breathing pattern, and dyspnea. Results: The patients exhibited a mild restrictive pattern, but the CC was not significantly different from that in control subjects. Nevertheless, airway closure during tidal breathing (ie, CC greater than functional residual capacity [FRC]) was present in most patients but was absent in all control subjects. As a result of the maldistribution of ventilation and the concurrent impairment of gas exchange, the mean (+/- SD) alveolar-arterial oxygen pressure difference increased significantly in CHF patients (4.3 +/- 1.2 vs 2.7 +/- 0.5 kPa, respectively; p < 0.001) and correlated with systolic pulmonary artery pressure (r = 0.49; p < 0.03). Tidal FL is absent in CHF patients. Mouth occlusion pressure 100 ms after onset of inspiratory effort (P0.1) as a percentage of maximal inspiratory pressure (Pimax) together with ventilation were increased in CHF patients (p < 0.01 and p < 0.005, respectively). The increase in ventilation was due entirely to increased respiratory frequency (fR) with a concurrent decrease in Paco2. Chronic dyspnea (scored with the Medical Research Council [MRC] scale) correlated (r2= 0.61; p < 0.001) with fR and P0.1/Pimax. Conclusions: In CHF patients at rest, CC is not increased, but, as a result of decreased FRC, airway closure during tidal breathing is present, promoting the maldistribution of ventilation, ventilation-perfusion mismatch, and impaired gas exchange. The ventilation is increased as result of increased fR, and Pimax is decreased with a concurrent increase in P0.1, implying that there is a proportionately greater inspiratory effort per breath (P0.1/Pimax). These, together with the increased fR, are the only significant contributors to increases in the MRC dyspnea score.
Multiple breath washout lung function reveals ventilation inhomogeneity unresponsive to mechanical assisted cough in patients with neuromuscular disease

Abstract:Background: Respiratory involvement defines the clinical outcome of neuromuscular diseases (NMD). The lung clearance index (LCI) is a marker of lung ventilation inhomogeneity and indicates small airway disease. It is determined by mulitple breath washout lung function (MBW). The merit of LCI is undisputed for primary lung diseases like cystic fibrosis, but its role in NMD is unclear. Methods: We investigated the role of MBW in patients with NMD and the effect of two different tracer gases and cough assist devices on the LCI. Patients and controls performed MBW with nitrogen (N2) and sulfur hexafluoride (SF6), whereas the latter analysis was repeated after the use of a cough assist device in the NMD group. LCI was compared to forced vital capacity (FVC) and peak cough flow (PCF). Results: 24 NMD patients (12 Duchenne Muscular Dystrophy, 8 Spinal Muscular Atrophy, 4 other NMDs) and 15 healthy controls were enrolled. In the NMD group, overall LCI N2 was higher than LCI SF6 (9.67 ± 1.56 vs. 8.71 ± 1.47; mean ± SD; p < 0.033). In controls, LCI N2 did not differ significantly from LCI SF6 (7.03 ± 0.37 vs. 7.05 ± 0.67; p = 0.882). Both LCI N2 and LCI SF6 were significantly higher in NMD patients as in controls (9.67 ± 1.56 vs. 7.03 ± 0.37, p < 0.001, and 8.71 ± 1.478.65 vs. 7.05 ± 0.67, p < 0.001). In the NMD group, both LCI N2 and LCI SF6 showed a negative correlation to FVC (r = - 0.525; p = 0.008 and r = - 0.526; p = 0.008, respectively) and PCF (r = - 0.590; p = 0.002 and r = - 0.641; p = 0.001, respectively). LCI N2 and LCI SF6 correlated well in the NMD group. LCI SF6 did not change significantly after the use of the cough assist in NMD patients (n = 22; 8.65 ± 1.52 pre vs. 8.79 ± 2.03 post, p = 0.667). Conclusion: Lung involvement of patients with neuromuscular diseases goes beyond weakness of respiratory muscles. MBW with both N2 and SF6 is suitable to detect ventilation inhomogeneity in NMD patients with respiratory impairment. Cough assist devices with low to moderate pressure levels do not immediately improve the LCI.
A Study of Respiratory Muscle Strength in Patients with Chronic Obstructive Pulmonary Disease

Abstract:Purpose In order to assess respiratory muscle strength and central inspiratory drive and their relationship to lung function tests in patients with chronic obstructive pulmonary disease (COPD). Methods We measured in 66 patients with chronic obstructive pulmonary disease the maximum inspiratory and expiratory pressure (Pimax, Pemax, cm H2O% pred) , Inspiratory pressure after 0.1 sec. at tidal breathing (P0.1) , lung function and arterial gas analysis respectively. Results COPD patients showed significantly lower Pimax and Pemax and higher P0.1 than normal subjects. The FRC/TLC% ,RV% pred,FRC% pred and FEV1 were significantly correlated with Pimax and P0.1. Among them, Pimax was the most closely correlated with FRC/TLC% ( r = - 0.581 P 0.001). There were no relationship between FEV1, FRC/TLC% and Pemax. PaO2 and PaCO2 were only correlated with Pimax. No relationship between PaO2, PaCO2 and Pemax, P0.1.TLC% pred was not correlated with Pimax,Pemax and P0.1. Conclusions There is respiratory muscle strength dysfunction in COPD patients. Hyperinflation and airway obstruction predominantly affected inspiratory muscle strength in a group of COPD patients.
