Abstract:Adopted adolescents create identity narratives conceptualizing their connections to their families of adoption and birth. Previous work with a sample of adoptive adolescents identified a sub-group who reported negative experiences regarding adoption as part of their navigating of adoptive identity processes (the "Unsettled" group). The current study examined interviews with adolescents in the "Unsettled" group to elucidate these negative experiences, specifically through identifying the relationship challenges linked to adoption. Participants included 30 adopted adolescents (18 females, 12 males) from a longitudinal study of adoptive families. All the adolescents (M age = 15.2 years) were domestically adopted in infancy by heterosexual couples who were the same race as the adolescents (29 White, 1 Mexican American). Thematic analysis revealed six themes reflecting adolescents' relationship challenges as related to adoption, both in terms of interpersonal interactions and how relational experiences influenced adolescents' thoughts and feelings of past, present, and future selves: (a) Negative experiences in relationships with adoptive family members, (b) Negative experiences in relationships with birth family members, (c) Difficulties in the adoptive kinship network, (d) Negative thoughts and feelings toward the self as an adopted person, (e) Negative views toward adoption as a form of building a family, and (f) Negative connections between adoption and future relationships. Multiple subthemes were also identified that built upon topics within the adoption and family systems literature, such as communication among family members, navigation of birth family contact, and adopted adolescents' perceptions of loss. Also identified were four profiles across themes. Implications for mental health providers and adoption professionals are discussed.
The Perceptions of New Zealand Lawyers and Social Workers About Children Being Adopted by Gay Couples and Lesbian Couples

Abstract:Global trends increasingly appear to be legitimizing same-gender relationships, yet international research shows that despite statutory rights to marry-and by extension, adopt children-same-gender couples continue to experience difficulties when trying to adopt. Primary among these barriers are the persistent heteronormative beliefs, which strongly underpin the unfounded myths about parenting abilities of same-gender couples. Such biased beliefs are perpetuated by some adoption professionals who oppose placing children with lesbian or gay couples. In 2013, New Zealand passed the Marriage Equality Act, making it possible for same-gender couples to legally marry-and by extension, adopt. This provided an opportunity to investigate the perceptions of New Zealand professionals about children being placed with same-gender couples, in a country often perceived to be more tolerant of LGBT people. New Zealand social workers and lawyers (an under-studied group)-the professions most likely involved in adoption-were recruited via professional bodies. Because studying perceptions and beliefs on socially sensitive topics are highly susceptible to social desirability, we designed an instrument utilizing multiple methods to assess and corroborate participants' views about placing children for adoption with couples of the same gender. Administered online and anonymously, the survey included demographic questions, evaluation of negative-meaning and positive-meaning statements, and used a scenario describing a prospective adoptive couple whose gender was ambiguous, in the context of adopting children of varying needs. Overall, the study found that while New Zealand lawyers and social workers (N = 314) had generally favorable views of gay and lesbian adoption, they still reported a preference to see children adopted by heterosexual couples over same-gender couples, within which lesbian and gay couples were preferred equally. Moreover, being religious and politically conservative were characteristics associated with more negative views toward placing children with same-gender couples. We conclude that, despite winning the rights to marry (and adopt as couples), such legislative wins might be merely the first hurdle to be overcome; normalizing same-gender parenting is what needs to happen next. Our study adds to the research focused on adoption professionals in various countries, with the ultimate aim to inform practices and policies supportive of families headed by same-gender couples and formed through adoption.
Unconscious phantasy as a structural principle and organizer of mental life: The evolution of a concept from Freud to Klein and some of her successors

Abstract:Starting with Freud's discovery of unconscious phantasy as a means of accessing his patients' internal world, the author discusses the evolution of the concept in the work of Melanie Klein and some of her successors. Whereas Freud sees phantasy as a wish fulfilling imagination, dominated by primary process functioning and kept apart from reality testing, Klein understands phantasies as a structural function and organizer of mental life. From their very beginnings they involve object relations and gradually evolve from primitive body-near experiences to images and symbolic representations. With her concept of projective identification in particular, Klein anticipates the communicative function of unconscious phantasies. They are at the basis of processes of symbolization, but may also be put into the service of complex defensive operations. The author traces the further evolution of the concept from the contributions of S. Isaacs, the theories of thinking proposed by W.R. Bion and R. Money-Kyrle, Hanna Segal's ideas on symbolization and reparation all the way to the latest approaches by R. Britton, J. Steiner and others, including the understanding of transference and counter-transference as a 'total situation'. Points of contact with Freud are to be found particularly in connection with his concept of 'primal phantasies'. In the author's view, the idea of the transmission and communicative potential of unconscious phantasies enabled these authors to overcome the solipsistic origins of drive theory in favour of a notion in which unconscious phantasies both set down the coordinates of the inner world and form and reflect the matrix of inter-subjective relations.

The Psychoanalytic Study of the Child

Abstract:This paper is a summary of findings from the "Adoption and Attachment Representations" study which was a collaboration between the Anna Freud Centre, Great Ormond Street, and Coram that focused on the nature and development of parent and child attachment representations in an adoption context. The study began with Adult Attachment Interview assessments of both mothers and fathers who would go on to adopt a group of 58 previously maltreated children aged four to eight years. There was also a comparison group of 42 children, also aged four to eight years but who had been adopted in their infancy. The children were assessed early in their adoptive placement, with the Story Stem Assessment Profile in order to focus on their attachment representations along with a range of cognitive and psychological functioning assessments. The parents were interviewed using the Parent Development Interview to access their attachment representations of their children. These assessments were repeated one year and two years later with subsequent follow-ups in early adolescence and a recently completed phase in the adoptees' adulthood. The study is unique in exploring aspects of intergenerational patterns of attachment in nonbiologically related parents and children in a longitudinal design. The findings suggest that parental attachment states of mind are associated with their children's attachment representations and the "intervention" of adoption is powerful with "late placed" children adopting more secure themes in their story stem narratives over time while also retaining some less optimal features such as avoidance and disorganization. The paper makes links with previous developmental and psychoanalytic work in the field of adoption and explores the potential policy implications of the study.

Abstract:Journal of Adolescent Research, Ahead of Print. There is limited research on stigma experiences and disclosure practices among adolescents who: (a) are adopted, (b) who have LGBTQ+ parents, and (c) who are sexual or gender minorities themselves. At a time when LGBTQ+ identities are both increasingly visible and also publicly debated, we conducted interviews with 50 adolescents (M age = 14.86 years) in 12 two-father, 20 two-mother, and 18 father/mother families. Following protocols approved by Clark University's Institutional Review Board and through the frameworks of sexual stigma, microaggressions, and communication privacy management, we used thematic analysis to explore themes of disclosure practices, peer responses to disclosure, and parent responses to sexual and gender identity disclosure. Adolescents described various disclosure decisions around their adoptive status, LGBTQ+ parent family structure, and their own sexual and gender identities, ranging from rare to reactive to proactive disclosure. Such decisions were in some cases shaped by the intersections among participants' race, gender, and family structure. Participants often selectively disclosed because of concerns related to privacy and negative peer reactions. Many adolescents reported instances of microaggressions from peers around their identities. Parent reactions to their children's sexual and gender identity disclosure were more complex than peers' reactions. Findings have implications for therapists and other professionals working with adolescents and their families.

Filial Trauma: The Experience of Adoptive Parents of Children with Complex Behavioral and Relational Problems

Abstract:Children adopted from out-of-home care may present symptoms of developmental trauma that affect their functioning. This puts their adoptive parents at risk of experiencing secondary trauma. The purpose of this research was to understand the experience of adoptive parents who self-report symptoms of secondary trauma in relation to their child's symptoms of developmental trauma. Individual semi-structured interviews were conducted, transcribed, and analyzed using Braun and Clarke's (Qualitative Research in Psychology, 3(2), 77-101, 2006) thematic analysis method. Ten adoptive parents were recruited from four community organizations providing support for adoptive parents or parents of children with attachment disorder. All participants reported feeling strong emotions related to secondary trauma, as well as physical and mental health issues that appeared after the child's arrival in the family. Participants reported experiencing aggression from their child, which greatly disturbed the parent-child relationship and led to the child being placed in residential care for half of the families involved in the study. Secondary trauma was insufficient to fully capture the experience of these parents as it failed to account for the suffering and distress linked to their efforts to establish a parent-child relationship. Instead, filial trauma was used to describe this experience. The authors conclude that a better understanding of filial trauma can contribute to improving post-adoption services for parents struggling to fulfill one of the most important roles of their lives: being the parent of their child.
Depression and Anxiety Symptoms of British Adoptive Parents: A Prospective Four-Wave Longitudinal Study

Abstract:The mental health of birth parents has gained attention due to the serious negative consequences for personal, family, and child outcomes, but depression and anxiety in adoptive parents remains under-recognized. Using a prospective, longitudinal design, we investigated anxiety and depression symptoms in 96 British adoptive parents over four time points in the first four years of an adoptive placement. Depression and anxiety symptom scores were relatively stable across time. Growth curve analysis showed that higher child internalizing scores and lower parental sense of competency at five months post-placement were associated with higher initial levels of parental depressive symptoms. Lower parental sense of competency was also associated with higher initial levels of parental anxiety symptoms. Parents of older children and those with higher levels of parental anxiety and sense of competency at five months post-placement had a steeper decrease in depressive symptoms over time. Support for adoptive families primarily focuses on child adjustment. Our findings suggest that professional awareness of parental mental health post-placement may be necessary, and interventions aimed at improving parents' sense of competency may be beneficial.
Unconscious phantasy: Some historical and conceptual dimensions

Abstract:This paper seeks firstly to grasp both conceptually and historically the different phenomenologies that are captured by the term 'Unconscious Phantasy'. The term is shown to refer to a number of distinct though overlapping conceptual domains. These include: phantasy as scene, phantasy as representation of drive, phantasy as representation of wish as its fulfilment, phantasy as split off activity of the mind functioning under the aegis of the pleasure principle; phantasy as representation of the minds own activities (which Wollheim calls' the way "the mind represents its own activities to itself''). Lastly unconscious phantasy is understood as being the basic foundation of all mental life, including drives, impulses, all anxiety situations and defences. Having mapped out this territory through following the development of the concept in the work of Freud and Klein, the author draws on the work of the philosopher Richard Wollheim who, the author contends, has made a fundamental contribution to our conceptual understanding of unconscious phantasy. In the last section of the paper, the author draws a distinction between what he terms 'objects' (namely psychic objects) and what he terms 'facts'. It is suggested that this distinction, though implicit in much of our work, benefits from being made explicit and that in so doing an important dimension of analytic work is illuminated. We aim to help the patient to discover what he is like, to understand the ways in which he conceives and misconceives himself, to unravel the fact-ness of himself and his world from its 'object qualities', to differentiate between unconscious phantasy and reality.

Abstract:This article presents the findings of an ongoing supervision group (founded in 1999) researching the after-effects of the Nazi period on people in psychotherapy in Germany today. The unacknowledged collective shadow hidden behind half-truths, prevarications, and silence itself prevents a genuine working through of the Nazi past. Patients' lack of knowledge concerning their families' own past leads to unconscious guilt, which often then leads to psychosomatic disturbances. But this is not only a problem in Germany. Unacknowledged collective shadows are prevalent in many countries worldwide. Psychological difficulties on the individual and societal levels result.

Abstract:The concept of unconscious phantasy has played - and still does play - a central role in psychoanalytic thinking. The author discusses the various forms by which unconscious phantasies manifest themselves in the analytic session as they are lived out and enacted in the transference relationship. This paper also aims at expanding the kleinian theory of symbol formation by exploring the impact that emotional aspects connected to early "raw', "pre-symbolic' phantasies have in the analysis and how their corporeal elements interlock with the signifying process. The author follows the expressive forms of primitive unconscious phantasies as they appear in a psychoanalytic session and proposes that the emotional effect that can be experienced in the communication between patient and analyst depends in great measure on "semiotic' aspects linked to primitive phantasies that are felt and lived out in embodied ways. Rather than a move from unconscious phantasies that typify symbolic equations to those showing proper symbolization, these can coexist and simultaneously find their way to what is communicated to the analyst. As early phantasies bear an intimate connection to the body and to unprocessed emotions when they are projected into the analyst they can produce a powerful resonance, sometimes also experienced in a physical way and forming an integral part of the analyst's counter-transference.
Implicit memory and early unrepressed unconscious: their role in the therapeutic process (how the neurosciences can contribute to psychoanalysis)

Abstract:The author discusses memory from the point of view of the neurosciences and molecular biology, proposing an integration with the psychoanalytic theory of the unconscious. The discovery of the implicit memory has extended the concept of the unconscious and supports the hypothesis that this is where the emotional and affective--sometimes traumatic--presymbolic and preverbal experiences of the primary mother-infant relations are stored. They could form the ground structure of an early unrepressed unconscious nucleus of the self. Identifying the unconscious with the memory leads to a theory about its morpho-functional organization. The unrepressed unconscious can be brought to the surface in analysis through the 'musical dimension' of the transference, characterized by the voice (its intonation and rhythm) and the prosody of the language. Dreams can symbolically transform pre-symbolic and preverbal experiences, so that they can be put into words and thought about even without their recollection. Dreams can also create images to fi ll the gap of the absence of representation which characterize the unrepressed unconscious. The description of a segment of analysis of a patient suffering from death anxiety provides a clinical illustration of the theories discussed. The interpretation of her voice and of the prosody of her language, besides the work on dreams, reproduced the emotional essence of the analysand's traumatic childhood experiences. This reconstruction enabled her to speak and think about them even without the actual recollection.

Abstract:Adoptive parents often face stigma related to "non-traditional" family structures. Lesbian and gay (LG) adoptive parents often face additional stigmatization based on sexual identity, which in turn may negatively affect parents' mental health. Despite controversy about LG parenting, research demonstrates that family processes are more strongly associated with individual outcomes than family structure. Thus, family systems and minority stress theories provided our conceptual foundation in examining how adoptive LG parents' stigma experiences were associated with mental health, parenting competence, and parent–child relationships. Participating families (N = 106; n = 56 LG parent families) were originally recruited from five US domestic private infant adoption agencies and completed two waves of data collection (W1, W2; 91% retention) when children were preschool-age (Mage = 3.01 years) and school-age (Mage = 8.36 years), respectively. Data for the current study are largely drawn from W2. Via Qualtrics, parents completed assessments of mental health symptoms, adoption stigma, and perceived childcare competence. LG parents also reported on their experiences of homonegative microaggressions, and children responded to a measure about their relationships with parents. No significant differences emerged as a function of parental sexual orientation and gender except that lesbian mothers, heterosexual mothers, and gay fathers all reported higher parenting competence than heterosexual fathers. Although parents' mental health did not significantly predict parent–child relationship quality, parents' perceived competence and LG parents' current homonegative microaggression experiences did (e.g., greater competence, greater closeness; more microaggressions, lower closeness). Consistent with our conceptual framework, our results—derived from parent and child reports—demonstrate that although adoptive and LG parent families experience stigma, family processes (rather than structure) are most associated with individual outcomes. Researchers, policy makers, and practitioners should work together to employ identity-affirming practices to reduce stigma and support adoptive family functioning and well-being.

Abstract:The article shows the peculiarities of the unconscious sphere and their importance in the formation of the spiritual sphere of the personality of adolescent and teenage years on the basis of instinct, archetypes and unconscious attitudes. The problem of instincts must be approached from the perspective of methodology, understanding it as a mental, unconscious phenomenon. The psychological side is currently central and important for youngsters and teenagers. Fantasies, images, desires, inducements, which can be based on archetypes and unconscious mindsets, play a special role in the formation of the spiritual sphere of the personality in adolescent and teenage years. The sexual instinct of teenagers should be treated as an unconscious phenomenon with enormous potential. The factor of ‘humanizing’ instincts in general and sexual instinct in particular, is the formation of personality archetypes, which include fantasies, images, desires, inducements of teenagers and their connection with the archetype, as a psychological determinant being the main function. In adolescence and youth, mental energy sublimates. Psychological determinants of the unconscious sphere at teenage and junior age can cause imagination, creativity, to define it as subject in a social situation of development, identification of the personality, to assist formation of personal qualities, interaction in a group of people and the ability to achieve goals. In prepubescent years and adolescent age there are appear a desire for freedom, an effort to find meaning, creative activity, a desire for integrity, love, value, an appeal to and unity with the spiritual, higher divine world, which are manifested as signs of the archetype ‘selfhood’.
Thinking About the Birth Father: Loss, Longing, Ambivalence, and Indifference Among Adopted Adolescents With Lesbian Mothers

Abstract:Adopted youth often have contact with or at least information about birth family members—but such relationships or knowledge rarely extend to birth fathers. The current study explores ideas, feelings, and questions about birth fathers among youth raised by two mothers, including whether or not they desire contact. Interviews were conducted with 25 adolescents, ages 13–19 years. Thematic analysis of narratives revealed seven groups of respondents who displayed a range of interests, feelings, and contact desires in relation to birth fathers. Some adolescents had limited information about or interest in their birth fathers, others showed ambivalence and uncertainty, and still others voiced curiosity and strong interest. A few articulated a sense of a fathers 'absence' and a longing for a fatherly presence. The data are interpreted in the context of ambiguous loss, uncertainty management, and gender perspectives. Implications for future research, adoption practice, and adoptive parenting are considered.

Abstract:From its inception, psychoanalysis has tended to idealize the curative value of insight while devaluing the mutative significance of the analytic relationship. This paper argues that applying the construct of the "relational unconscious" in clinical practice offers a possible resolution of this "mind-relationship" rift. From this perspective, transference and countertransference provoke a continual intersubjective/interpersonal enactment of a co-created infantile drama emanating from the internal worlds of analyst and analysand in which a vital form of parental and/or self loving is at stake. Case vignettes demonstrate how to apply the relational unconscious in clinical practice.

Abstract:In this theoretical paper the authors explore the connections between BDSM (i.e., practices involving bondage, discipline, dominance, submission, sadism, masochism) and CSA (childhood sexual abuse) in order to investigate the potential unconscious mechanisms at play and the therapeutic functions of BDSM practices among CSA victims. Drawing on the embodiment framework, the authors address how BDSM may serve as a form of unconscious repetition of traumatic experiences for certain CSA victims, with the aim of processing trauma and healing. A review of the empirical evidence regarding the links between BDSM and CSA trauma, along with the potential of BDSM to trigger trauma and elicit dissociation, guilt, or shame, is conducted. Finally, BDSM practices are reviewed through the concept of trauma-play, which involves deliberate rescripting. In short, the complex relationship between BDSM and CSA is highlighted, as well as its implications for understanding and potentially addressing trauma experiences in therapy.

Abstract:Adoption is an act of kindness and an expression of the most exalted of human morality. It fulfills the needs of both the adoptive parents and the adopted child. Muslim religious law has rejected the concept of adoption as it exists in the western world and presents several alternatives including personal liability, declaration of guardianship, bestowing a gift and leaving a last will and testament. Nevertheless, over the past 20 years, the Arab-Muslim population in Israel has developed a certain acceptance of the more typical concept of adoption and the willingness to accept the civil legislation that is applied in domestic courts in Israel. This gradual integration into Israeli society as well as the very act of adoption, which remains controversial, often creates the need for treatment, consultation and guidance for the adoptive family and the adopted child. The traditional, collective characteristics of the Arab-Muslim society have a significant impact on the child's emotional state and behavior and, of course, effect the adoptive family's social standing as well. In such situations, it is imperative to discuss the interaction and the often difficult and complex relationship that develops between the therapist or counselor and the patient.

Abstract:Many of the most significant changes in concepts of personality development and maldevelopment derive simply from belated appreciation that the child grows up in a family; and that the child requires considerable positive input from those who raise him to grow into a reasonably integrated person. Only recently have psychiatrists and psychoanalysts begun to focus on the influence of the family transactions throughout the formative years. The paper briefly presents the requisites a family must provide to assure the integrated development of its offspring and groups them under four headings: the parental nurturant functions that change with each phase of the offspring's development; the influence of the dynamic organization of the family on the offspring's intrapsychic structure, affecting self-boundaries, gender identity, superego directives, and so forth; conveying through family transactions the basic social roles and societal institutions to the child; transmitting the instrumental techniques of the culture, and particularly its language with its system of meanings and logic upon which virtually all ego functioning depends. The paper considers the evidence that the "choice of the neurosis" -- or more correctly, the choice of the psychiatric syndrome -- does not depend only upon fixation at various phases of pre-genital development, but also, and perhaps primarily, upon the panphasic influence of the intrafamilial environment. It is of particular importance to recognize that there is a very direct relationship between the family transactions, the separation-individuation process, bounary formation, the attainment of various degrees of object constancy, problems of splitting, superego directives, and what can be conscious and what need be repressed into the unconscious. The failure to recognize the import of the family has interfered with the formation of an integrated developmental theory that can unite various aspects of the developmental process that have remained more or less isolated from one another -- drive theory, object relations, ego psychology, separation-individuation, and cognitive development. Finally the question is raised if in providing guidance to parents we have been amiss in not focusing on such matters as who the parents are, how they relate to one another as well as to the child, the values they communicate by their behaviour -- matters that are fundamental and to which almost all else is secondary, but about which relatively little has been said or taught.
Dysfunctional Schemas from Preadolescence as One Major Avenue by Which Meaning Has Impact on Mental Health

Abstract:A main way by which meaning influences mental health is by the formation of interpersonal schemas that specify what to expect from others and how to treat them. Particularly during preadolescence (a developmental phase focused on interpersonal skills), young people living in a stressful or hurtful environment can form atypical schemas that can help them survive but that produce serious problems when later applied to newly forming adult relationships. We provide three case studies illustrating this process. A boy learned to cope by withdrawing from social interaction and excelling in schoolwork. A girl learned to cope by denying her own needs and sacrificing herself for the welfare of others. Another girl coped by pervasive distrust of others and by becoming assertively independent. These children learned well enough to adapt to these dysfunctional relationships so as to suffer as little as possible, and they even developed some personal skills and resources. However, the rigid schemas had a destructive impact on their adult relationships. Proposals for interventions to change meaning and behaviors are discussed.

Dr. Lanes is a certified psychotherapist. The main area of research in which the Lanes deals is "differentiation of self" among couples and individuals. Lanes is an invited lecture in training courses for psychotherapy and couple therapy. In the past,he served as chairman of the Association for Family and Couple Therapy and as head of the Magid School of Psychotherapy.Ateret Gewirtz-Meydan is an Associate Professor at the School of Social Work at the University of Haifa and the director of The Science of Sex Research Lab. Dr. Gewirtz-Meydan studies intimate relationship problems,sexual distress and dysfunction,and sexuality following trauma. Dr. Gewirtz-Meydan is also a research fellow at the Crimes against Children Research Center (CCRC),the Haruv Institute,and The Interdisciplinary Research Centre on Intimate Relationship Problems and Sexual Abuse (CRIPCAS). In practice,Dr. Gewirtz-Meydan is a certified sex therapist and is an invited lecture in training courses for sex counseling and therapy.Lee Reuveni,has a MSW in social work,and another MA in gender studies. Lee is also a certified supervisor and sex therapist working with couples in a private clinic as an EFT therapist. Reuveni is the author of the book "mommy tummy mommy heart" that explains how children are conceived in a family that has two mothers,and the book "twin bed for teen" – a guide for teenager's sexuality.

Abstract:The central aim of the current study is to further the understanding of how therapists identify and understand erotic transference in therapy, and how they perceive it to influence the therapeutic processes and alliance. Answers were analyzed thematically using an open-coding analysis. A total of 116 therapists participated in the study reflecting on how erotic transference is experienced in the therapeutic setting. Data analysis yielded three main themes: (1) threat; (2) boundaries; and (3) growth. The threat theme included two subthemes: Threat to the therapeutic process and threat to the therapist's professional-esteem. Growth also consisted of two subthemes: development of the therapist and opportunity for insight (into the inner world of the patient). The present study subscribes to the idea that erotic transference is a significant aspect of the therapeutic setting and, if addressed appropriately and more comprehensively, can have a great impact on the patient's healing process.
