Abstract:OBJECTIVE:To investigate the antimicrobial resistance among the nosocomial gram-negative pathogens from 15 teaching hospitals located in different areas in China in 2005.METHODS:A total of 1927 non-repetitive nosocomial gram-negative pathogens were collected from 15 teaching hospitals in different areas in China and sent to the central lab for reidentification and susceptibility testing. The levels of minimal inhibitory concentration (MIC) of 18 antimicrobial agents were determined by agar dilution method. WHONET 5.4 software was used to analyze the data.RESULTS:The strains of Escherichia coli, Klebsiella pneumoniae, and Proteous mirabilis isolates that did not produce extended spectrum beta lactamases (ESBLs) showed high sensitivity to beta-lactams. The antibiotics with a susceptibility rates over 80% against the strains of Entorobacter cloacae, Enterobacter aerogene, Citrobacter spp, Serratia spp, and Proteous vulgaris producing AmpC enzyme included meropenem, imipenem, and piperacillin-tazobactam, and these 3 drugs showed a susceptibility rate of more than 80% against the ESBL-producing strains of Escherichia coli and Klebsiella pneumoniae. Other antimicrobial agents showing a relatively high activity against Enterobacter spp, Citrobacter spp, Serratia spp and Proteous vulgaris included cefepime (67.3% - 100%), amikacin (67.3% - 95.2%), ceftazidime (52.9% - 100%) and cefoperazone-sulbactam (51.9% - 100%). The susceptibility rate of fluoroquinolones was 34.8% - 36.1% against non-ESBL-producing Escherichia coli and was 13.4% - 17.1% against ESBL-producing isolates. The most active agent against Pseudomonas aeruginosa was polymyxin B (95.6%). The agents with the activity rates of 70% - 80% included meropenem, imipenem, amikacin, and piperacillin-tazobactam. The antibiotic with a high susceptible rate against Acinetobacter baumannii was polymyxin B (98.3%), followed by imipenem (80.8%), meropenem (76.2%), and minocycline (67.4%). The susceptible rates of other agents were all below 60%. The agents with relatively high activity against Stenotrophomonas maltophilia included minocycline (85%), levofloxacin (82.5%), and trimethoprim-sulfamethoxazole (77.5%). The agents with a relatively high activity against Burkholderia cepacia included minocycline (77.2%) and meropenem (61.4%).CONCLUSION:Carbapenem, piperacillin-tazobactam, amikacin and cefepime remained relatively high activity against nosocomial Enterobacteriaceae, Non-fermenting pathogens have lower susceptibility to the antimicrobial agents than before.

Abstract:ABSTRACT To evaluate the antimicrobial susceptibility of Gram-negative bacilli that caused hospital-acquired and community-acquired intra-abdominal infections (IAIs) in China between 2012 and 2013, we determined the susceptibilities to 12 antimicrobials and the extended-spectrum β-lactamase (ESBL) statuses of 3,540 IAI isolates from seven geographic areas in China in a central laboratory using CLSI broth microdilution and interpretive standards. Most infections were caused by Escherichia coli (46.3%) and Klebsiella pneumoniae (19.7%). Rates of ESBL-producing E. coli ( P = 0.031), K. pneumoniae ( P = 0.017), and Proteus mirabilis ( P = 0.004) were higher in hospital-acquired IAIs than in community-acquired IAIs. Susceptibilities of enterobacteriaceae to ertapenem, amikacin, piperacillin-tazobactam, and imipenem were 71.3% to 100%, 81.3% to 100%, 64.7% to 100%, and 83.1% to 100%, respectively, but imipenem was ineffective against P. mirabilis (<20%). Although most ESBL-positive hospital-acquired isolates were resistant to third- and fourth-generation cephalosporins, the majority were susceptible to cefoxitin (47.9% to 83.9%). Susceptibilities of ESBL-positive isolates to ampicillin-sulbactam (<10%) were low, whereas susceptibilities to ciprofloxacin (0% to 54.6%) and levofloxacin (0% to 63.6%) varied substantially. The prevalences of cephalosporin-susceptible E. coli and K. pneumoniae were higher in the northeastern and southern regions than in the central and eastern regions, reflecting the ESBL-positive rates in these areas, and were lowest in the Jiangsu-Zhejiang (Jiang-Zhe) area where the rates of carbapenem resistance were also highest. Ertapenem, amikacin, piperacillin-tazobactam, and imipenem are the most efficacious antibiotics for treating IAIs in China, especially those caused by E. coli or K. pneumoniae . Resistance to cephalosporins and carbapenems is more common in the Jiang-Zhe area than in other regions in China.

Abstract:The purpose of this study was to survey antibacterial resistance in outpatients of Chinese county hospitals. A total of 31 county hospitals were selected and samples continuously collected from August 2010 to August 2011. Drug sensitivity testing was conducted in a central laboratory. A total of 2946 unique isolates were collected, including 634 strains of Escherichia coli, 606 Klebsiella pneumoniae, 476 Staphylococcus aureus, 308 Streptococcus pneumoniae, and 160 Haemophilus influenzae. Extended-spectrum β-lactamases were detected in E. coli (42.3% strains), K. pneumoniae (31.7%), and Proteus mirabilis (39.0%). Ciprofloxacin-resistance was detected in 51.0% of E. coli strains. Salmonella spp. and Shigella spp. were sensitive to most antibacterial agents. Less than 8.0% of Pseudomonas aeruginosa isolates were resistant to carbapenem. For S. aureus strains, 15.3% were resistant to methicillin, and some strains of S. pneumoniae showed resistance to penicillin (1.6%), ceftriaxone (13.0%), and erythromycin (96.4%). β-lactamase was produced by 96.5% of Moraxella catarrhalis strains, and 36.2% of H. influenzae isolates were resistant to ampicillin. Azithromycin-resistant H. influenzae, imipenem-resistant but meropenem-sensitive Proteus, and ceftriaxone- and carbapenem non-sensitive M. catarrhalis were recorded. In conclusion, cephalosporin- and quinolone-resistant strains of E. coli and Klebsiella pneumonia and macrolide-resistant Gram-positive cocci were relatively prominent in county hospitals. The antibacterial resistance profiles of isolates from different geographical locations varied significantly, with proportions in county hospitals lower than those in their tertiary counterparts in the central cities, although the difference is diminishing.
Comparison of antimicrobial activity in vitro of 7 β-lactams against 280 isolates from patients with nosocomial infections

Abstract:Objective. To analyse the resistance of 280 clinical isolates from patients with nosocomial infection to 7 β-lactams [cefoperazone (CPZ), ceftazidime (CAZ), imipenem (IPM), ceftriaxone (CRO), cefotaxime (CTX), aztreonam (AZ) and cefoperazone/sulbactam (CPZ/SB)]. Methods. MICs of 7 β- lactams against clinical isolates were detected by the method of MH-broth dilution, β-lactamases were detected by hydrolysis of penicillin G and nitrocefin. Result. (1) All of the 7 β-lactams demonstrated high resistance to isolates of Staphylococci and Enterococci, except for imipenem to enterococci (with resistant rates of 21.0%). (2) To most of gram-negative microorganisms, imipenem showed quite low resistant rate (0~18.1% to E. coli, K. pneumoniae, Enterobacter cloacae, Acinetobacter and P. aeruginosa), but highly resistant to X. maltophillia (95.5%), gram-negative microorganisms isolates showed higher resistant to another 6 β-lactams. Conclusion. The resistant rates of IPM, CPZ/SB and CAZ are the lowest. The β-lactamase producing strains of the gram-negative isolates are cross-resistant to AZ with the other third generation cephalosporins and it has no activity against gram-positive cocci.
Analysis of trends in antimicrobial resistance patterns among clinical isolates of Bacteroides fragilis group species from 1990 to 1994

Abstract:Antimicrobial resistance, including plasmid-mediated resistance, among Bacteroides fragilis group species is well documented. A 5-year (1990-1994) prospective, eight-center survey of 3,177 clinical isolates of Bacteroides species was undertaken to review trends in resistance, using the breakpoints for full and intermediate susceptibility established by the National Committee for Clinical Laboratory Standards. No documented resistance to either metronidazole or chloramphenicol was found in this survey. Among B. fragilis isolates virtually no resistance was seen to imipenem, meropenem, ampicillin/sulbactam, piperacillin/tazobactam, or ticarcillin/clavulanate. Significant increases in resistance among B. fragilis isolates to cefotetan, ceftizoxime, and clindamycin (p < .01) were noted. Resistance to cefoxitin remained unchanged. Among the non-fragilis species of the B. fragilis group, there was virtually no resistance to imipenem, meropenem, chloramphenicol, or metronidazole. The three beta-lactamase inhibitors had increasing levels of resistance, although 95%-98% of strains were susceptible (p < .05). There was a significant decline in cefoxitin, cefmetazole, and clindamycin activity over time against these strains (p <.01). There was a significant (P < .001) increase in geometric mean minimum inhibitory concentration for most drugs and species tested from 1990 to 1994. Clusters in the eight institutions could not account for this rise in resistance. This survey demonstrates that rates of resistance of B. fragilis and non-fragilis species of B. fragilis group are increasing.
Geographic patterns of antimicrobial susceptibilities for Bacteroides species worldwide: results from the Antimicrobial Testing Leadership and Surveillance (ATLAS) program, 2007-2020

Abstract:Antimicrobial susceptibilities of 4,973 Bacteroides species isolates recovered from various sources of patients from 12 countries (99.6% from European countries) in the Antimicrobial Testing Leadership and Surveillance (ATLAS) program, 2007-2020, were investigated. The minimum inhibitory concentrations (MICs) of the isolates with six commonly used agents were determined using the agar dilution method. Among the isolates, ten Bacteroides species were included: B. fragilis (n = 3,180, 64.0%) was most frequently encountered, followed by B. thetaiotaomicron (n = 675) and B. ovatus (n = 409). During the 14 years, the proportion of B. fragilis declined, but the proportion of non- fragilis Bacteroides species increased. More than 90% of the isolates tested were susceptible to piperacillin-tazobactam, meropenem, and tigecycline. Significantly lower susceptibility rates to cefoxitin ( p <0.001), clindamycin ( p <0.001), piperacillin/tazobactam ( p <0.001), and tigecycline ( p = 0.006) were observed among non- fragilis Bacteroides species isolates than among B. fragilis isolates. Moreover, the susceptibility rates to clindamycin ( p = 0.003) and tigecycline ( p = 0.044) decreased significantly among non- fragilis Bacteroides species over time. Clindamycin susceptibility rates higher than 80% were found in Greece (100%), Sweden (86.3%), and the United Kingdom (80.7%), and the lowest susceptibility rates were found in the United States (42.9%) and Japan (53.9%). In conclusion, the susceptibility of Bacteroides species to commonly used antibiotics varied geographically. Empirical antibiotic therapy for suspected anaerobic infections with clindamycin and cefoxitin should be avoided due to high resistance rates. Piperacillin-tazobactam, meropenem, metronidazole, and tigecycline could be considered favourable options for the treatment of infections caused by Bacteroides species.

[Analgic effect of indomethacin suppositories on postoperative pain].

Abstract:A double-blind study to investigate the analgesic effect of indomethacin was performed on postoperative patients undergone upper abdominal surgery. Pain relief was assessed by scoring pain and by modified spirometric technique of Bromage who reported that postoperative pain reduced the vital capacity of patients and the degree of its improvement was paralleled with analgesia. Indomethacin suppositories were given at doses of 100mg each upon the termination of operation and 6 hours later, and at 50mg 20 hours later after operation.The indomethacin-treated group exhibited a lower pain score and a decreased requirement of narcotics than the control. The spirometry revealed a significant improvement of both postoperative vital capacity and peak flow rate in the indo methacin-treated group. Modified respiratory restoration factor was 7.5% at 2 hours, 8.9% at 6 hours and 15.8% at 24 hours after surgery.

Abstract:Objective: To investigate the bacterial resistance of Gram negative bacilli isolated from eleven Shanghai hospitals. Methods: Antimicrobial susceptibility of clinical isolates were tested by Kirby Bauer method. ESBLs of these isolates were detected by method recommended by NCCLS 1999. Results: Of 13?934 Gram negative bacilli, 8?686(62.3%) were Enterobacteriaceae, 5?248(37.7%) were non fermentative bacilli. The most common species were E. coli , Klebsiella spp, P. aeruginosa , Acinetobacter spp and Enterobacter spp. etc. Among them, strains of non fermentative bacilli especially strains of S. maltophilia were increasing in recent years. The resistant rates of different species were slightly higher than those of last year. The antibacterial activity of antimicrobial agents tested against Enterobacteriaceae strains were IMP, MEMFEPCPZ SBT, PIP TAZ, AMKCAZCIP, while the activity against nonfermenters were IMP, MEMCPZ SBT, PIP TAZCAZ, CIPFEP,AMK. IMP is as active against Gram negative bacilli as MEM. Strains of Enterbacteriaceae were as susceptible to CPZ SBT as to PIP TAZ, the susceptibility of non fermentative Gram negative organisms to CPZ SBT was similar to that of PIP TAZ, however, the number of strains resistant to PIP TAZ were more than those resistant to CPZ SBT. Conclusion: Our data may have great significance in the empirical use of antimicrobial agents for the treatment of infections caused by Gram negative bacilli.
Surveillance of antibiotic resistance among the bacterial strains isolated from hospitals in Shanghai: results of 2016

Abstract:Objective To investigate the resistance profile of clinical bacterial isolates to antibiotics in Shanghai during 2016.Methods Antimicobial susceptibility testing was carried out for the clinical isolates collected from 47 hospitals according to a unified protocol using Kirby-Bauer method or automated systems.The participating hospitals included 28 tertiary hospitals and 19 secondary hospitals across Shanghai.Results were analyzed according to CLSI 2016 breakpoints.Results A total of 122 548 clinical isolates were collected,including 35 522 (29.0％) strains of gram positive cocci and 87 026 (71.0％) strains of gram negative bacilli.Overall,28.9％ of the isolates were from secondary hospitals and 71.1％ from tertiary hospitals.Gram positive and gram negative isolates accounted for 25.8％ and 74.2％ in secondary hospitals,30.3％ and 69.7％ in tertiary hospitals,respectively.The overall prevalence of MRSA in Staphylococcus aureus was 48.7％ and 77.2％ of MRCNS in coagulase-negative Staphylococcus.The average prevalence of MRSA and MRCNS was 55.9％ and 73.3％ in secondary hospitals,45.9％ and 78.6％ in tertiary hospitals.No strains were found resistant to vancomycin in Staphylococcus.About 77.4％ of the 1 111 strains of non-meningitis S.pneumoniae isolated from children were penicillin-susceptible (PSSP),13.2％ were penicillin-intermediate (PISP) and 9.4％ were penicillinresistant (PRSP).The prevalence of PSSP,PISE and PRSP was 97.8％,2.2％,and 0 in secondary hospitals,76.5％,13.7％,and 9.8％ in tertiary hospitals.Of the 285 strains isolated from adults,94.0％,4.2％ and 1.8％ were PSSP,P1SP and PRSP,respectively.The prevalence of PSSP,PISP and PRSP among the isolates from adults was 93.7％,5.3％,and 1.0％ in secondary hospitals,94.2％,3.7％,and 2.1％ in tertiary hospitals.Overall,37 strains of vacomycin-resistant E.feacium (14 from secondary hospitals and 23 from tertiary hospitals) and 25 strains of vacomycin-resistant E.feacalis (all from tertiary hospitals) were identified.PCR and sequencing analysis indicated that most of these resistant strains were vanA type.The overall prevalence of ESBLs-producing srains was 52.2％ in E.coli,30.9％ in Klebsiella pneumoniae and 29.8％ in Proteus mirabilis.Specifically,the corresponding prevalence of such strains was 55.1％,33.6％ and 34.0％ in secondary hospitals,51.0％,29.7％ and 28.0％ in tertiary hospitals,respectively.Enterobacteriaceae strains were still highly susceptible to carbapenem antibiotics.Overall,8.9％ and 9.1％ of the Enterobacteriaceae strains were resistant to imipenem and meropenem,respectively.The figure was 6.6％ and 7.1％ in secondary hospitals,9.9％ and 10.0％ in tertiary hospitals.Extensively drug-resistant strains were identified in A.baumannii,K.pneumoniae,P.aeruginosa,and E.coli,specifically,223,63,10,and 4 strains in secondary hospitals;224,201,22,and 9 strains in tertiary hospitals.Conclusions Antibicotic resistance is still very serious in the common clinical strains,which poses a critical threat to healthcare facilities.This issue should be taken seriously and effective infection control measures must be put in place.
Five years prospective survey of antibiotic resistance in Bacteroides and Parabacteroides isolated from inpatients of clinical hospital in Warsaw, Poland

Abstract:Objectives: Diagnostics of anaerobic bacterial infections and determination of drug susceptibility are technically difficult and time-consuming; therefore, the number of studies on Anaerobic Gram-negative bacilli is significantly limited, especially in Europe. The aim of the study was to analyze the antibiotic susceptibility of clinically important anaerobic bacteria Bacteroides spp. and Parabacteroides distasonis. Strains were isolated from infections of patients hospitalized at one Polish hospital as a result of routine microbiological diagnostics. Material and methods: Clinical isolates were identified with MALDI-TOF MS. Antimicrobial susceptibility of 276 strains was carried out by E-test gradient strip to commonly used antibiotics i.e. benzylpenicillin, amoxicillin with clavulanic acid, imipenem, clindamycin and metronidazole. MIC values were determined. The interpretation of antimicrobial susceptibility tests were conducted in accordance with The European Committee on Antimicrobial Susceptibility Testing (EUCAST) recommendations. Results: Susceptibility tests of all isolates yielded the following rates of resistance to the evaluated β-lactam antibiotics: benzylpenicillin (96%), amoxicillin/clavulanic acid (7.6%), imipenem (2.1%). In presented study 38.8% of clindamycin-resistant strains were isolated, among them 18.3% of B. fragilis and 53.85% P. distasonis. All strain were susceptible to metronidazole. Conclusions: Obtained results and analysis of the results of other researchers convinces us that it is necessary to routinely or at least periodically monitor drug susceptibility of clinical isolates of anaerobic bacteria and use targeted therapy based on the result of the antibiogram. Although high percentage of the tested Bacteroides and Parabacteroides strains remained susceptible to metronidazole and β-lactam antibiotics the use of clindamycin in empirical therapy may not be efficacious. Antibiogram results should be consult with the staff responsible for patient treatment and hospital antibiotic policies.

Abstract:OBJECTIVE:To investigate the pathogen profile of nosocomial infection in China, and to survey the susceptibility rates of these pathogens to the clinical common antibiotics.METHODS:The non-repetitive nosocomial pathogens isolated from bloodstream infection (BSI), hospital acquired pneumonia (HAP) and intra-abdominal infection (IAI) and the case data were collected from 13 teaching hospitals in different areas of China and sent to a central laboratory for re-identification and susceptibility testing. The levels of minimal inhibitory concentration (MIC) of the common antibiotics were determined by agar dilution method. The data were analyzed by WHONET 5.6 software.RESULTS:A total of 2103 clinical isolates were collected from January to December 2011, of which gram positive cocci and gram negative organisms accounted for 23.2% and 76.8% respectively. The top three pathogens of BSI were E. coli (31.0%, 243/784), K. pneumoniae (14.8%, 116/784) and S. aureus (10.6%, 83/784). The top three pathogens of HAP were A. baumannii (24.2%, 158/652), P. aeruginosa (23.0%, 150/652) and K. pneumoniae (16.4%, 107/652). The top three pathogens of IAI were E. coli (34.3%, 229/667), E. faecium (13.3%, 89/667) and K. pneumoniae (9.6%, 64/667). Methicillin-resistant S. aureus (MRSA) and coagulase negative Staphylococcus (MRCNS) accounted for 64.4% and 78.1% respectively. The susceptibility rates of Staphylococcus species to tigecycline, vancomycin, teicoplanin and linezolid were all 100%. The prevalence of MRSA in HAP was significantly higher than that in BSI or IAI. The susceptibility rates of Enterococcus species to tigecycline, teicoplanin and linezolid were all 100%. The prevalence of extended-spectrum β-lactamases (ESBL) was 64.3% in E. coli and 38.3% in K. pneumonia. Against Enterobacteriaceae, the most active agents were as following in order: tigecycline (92.3% - 100%) [except P.mirabilis], meropenem (87.5% - 100%), imipenem (87.5% - 100%) [except M. morganii], amikacin (87.5% - 100%), polymyxin B (75% - 100%) [except S. marcescens, P. mirabilis and M morganii], cefepime (67.8% - 100%), cefoperazone-sulbactam (66.6% - 100%), piperacillin-tazobactam (61.5% - 100%). Carbapenem-resistance Enterobacteriaceae strains emerged. The susceptibility rates of P. aeruginosa to imipenem and meropenem were 66.2% and 72.2%, respectively. The susceptibility rates of A. baumannii to imipenem and meropenem were 27.7% and 25.9%, respectively. The most active agents against A. baumannii were polymyxin B (100%), followed by tigecycline (79.8%) and minocycline (50.4%). The susceptibility rates of P.aeruginosa to antibiotics in BSI were higher than those in HAP and IAI. Susceptibility rates of S. maltophilia to trimethoprim-sulfamethoxazole, minocycline and levofloxacin were about 90% or above. Susceptibility rates of B. cepacia to trimethoprim-sulfamethoxazole, ceftazidime and meropenem were all 100%. Several P.aeruginosa and A. baumannii strains were resistant to all tested antibiotics except polymyxin B.CONCLUSIONS:The pathogen profile is different in different types of infection. The prevalence of multi-drug resistant A. baumannii is high, which is still a key problem of nosocomial infection. Tigecycline remains relatively high activity against gram-positive cocci and gram-negative bacteria (except P. aeruginosa and P. mirabilis) in vitro.

Abstract:ObjectiveTo investigate the bacterial resistance of clinical isolates from hospitals in Shanghai in 2003.MethodsAntimicrobial susceptibility testing of clinical isolates were performed using Kirby-Bauer methods and the results were analysed according to NCCLS(2003).ResultsOf 22 158 clinical isolates,gram positive organisms accounted for 33.6% (7 449/ 22 158),Gram negative organisms for 66.4% ( 14 709/ 22 158).In Staphylococcus aureus(MRSA) and coagulase negative Staphylococcus,MRSA and MRCNS accounted for 59.8% and 80.3% respectively.Penicillin-nonsusceptible S.pneumonine (PISP+PRSP) strains were 62.8% and 13.6% from children and adult respectively.ESBLs-producing strains accounted for 33.6% and 44.2% in E.coli and Klebsiella spp..Vancomycin and teicoplanin are the most potent antimicrobial agent against gram positive cocci.No strain of Staphylococcus spp.and Enterococcus spp.resistant to both antimicrobial agents has been found yet.Imipenem and meropenem are the most potent antimicrobial agents tested against strains of Enterobacteriaceae and Acinetobacter spp.,resistant rates of which were the lower,but resistant rates of P.aerogenosa were 24.5% and 16.9%. S.maltophilia and Burkholderia spp.to the both of imipenem and meropenem were very high resistant,but sensitive to cefoperazone-sulbactam,ciprofloxacin,ticarcillin-clavulanic acid and SMZ-TMP.ConclusionsThe data will be useful for reasonably choosing antimicrobial agents in the treatment of infections caused by pathogenic bacteria.

Abstract:Objective To investigate antimicrobial resistance among gram-positive cocci in China in 2006.Methods From Jun 2006 to Dec 2006,674 consecutive and non-repetitive gram-positive bacteria were collected from 7 teaching hospitals.The minimal inhibitory concentration(MICs)of antibacterial agents were determined by agar dilution method.Results The prevalence of penicillin.resistant(ease)and pemcllhn. intermediate S.pneumoniae(PISP)among 100 isolates was l%and 19%,respectively.Teicoplanin and vancomycin were the most active agents against S.pneumoniae.97% and 98% S.pneumoniae isolates were susceptible to levofloxacin and moxifloxacin,respectively.The susceptibility of amoxicillin/clavulanic acid, ceftriaxone and chloramphenicol are 96%,87% and 73%,respectively.The susceptible rates of penicillin. susceptible S.pneumoniae(PSSP)to cefprozil and cefaclor were 62% and 55.7%,respectively.All the PISP and PRSP isolates were resistance to the two antibiotics.The susceptibility to macrolides,trimethoprim/ sulfamethoxazole and tetracycline was lower than 35%.The prevalence of methicillin-resistant Staphylococcus aureus(MRSA)and methicillin.resistant coagulase-negative Staphylococci(MRSCON)was 48%(33%-84%)and 81%(69%-94%),respectively.The susceptible rates of MRSA to trimethoprim/ sulfamethoxazole,chloramphenicol,rifampin,and the other antibiotics in this study were 72%,66%and 45%,respectively.The susceptible rate of MRSA to marcrolides,aminoglycosides,tetracyclines and quinolones were not more than 18%.56%(30%-86%)of E.faecalis and 80%(50%.100%)of E.faecium were highly resistant to gentamicin.The susceptibility of E.faecalis to all the antibiotics except chloramphenicol and tetracycline were higher than E.faecium.All isolates of S.aureus,CoNS and E.faecalis tested were susceptible to vacomycin and teicoplanin.There were two vacomycin.resistant E.faecium strains isolated from Hangzhou.Conclusions Antimicrobial resistance patterns of gram.positive cocci differed in different regions.The resistance of gram-positive cocci to the antibiotics in this study this year was a little higher than the data of the year of 2005.Teicoplanin and vancomycin remained very high activity to gram-positive cocci.

Abstract:Objeciive To investigate the antibiotic resistance of clnical isolates in Shanghai hospitals from January to December in 2010. Methods Antimicrobial susceptibility testing was carried out for 41 326 finical isolates according to an agreed protocol using Kirby-Bauer (KB) method. Results were analyzed according to the breakpoints of CLSI 2010. Results Of the 41 326 clinical isolates, gram negative ba-11X11 and gram positive cocci accounted for 69.7% and 30.3%, respectively. The prevalence of MRSA in S. aureus were 57.9%, and 78.9% of the coagulase negative Staphylococcus isolates were methicillin resistant (MRCNS). No staphylococcus isolate was found resistant to vancomycin, teicoplanin or linezolid. Seven of the 656 strains of S. pneumoniae were isolated from cerebrospinal fluid (meningitis strain). The remaining were nonmeningitis strains. The prevalence of penicillin-susceptible S. pneumoniae (PSSP) was 71.2% in the nonmeningitis strains from children. Penicillin intermediate S. pneumonia (PISP) and penicillin-resistant strains accounted for 17.0% and 11.8%, respectively. The prevalence of PSSP and PISP were 97.0% and 3% in the nonmeningitis strains from adults. Fifteen strains of E. faecium and 6 strains of E. faecalis were found resistant to vancomycin. Eleven of the 15 E. faecium strains and 5 of the 6 E. faecalis strains showed van A-type resistance. The remaining strains were van B-type resistance. ESBLs production was confirmed in 56.8% of E. coli, 40.8% of Klebsiella spp. (K. pneumoniae and K. oxytoca) and 19.7% of P. mirabilis isolates. Enterobacteriaceca isolates were still highly sensitive to carbapenem antibiotics. The percentage of the strains resistant to imipenem or meropenem was <5%. The resistance rate of Acinetobacter spp. to imipenem and meropenem was increasing. About 49.6% and 51.2% of the strains were resistant to imipenem and meropenem, respectively. Compared with the data of year 2009, the prevalence of pan-drug resistant K. pneumoniae and A. baumannii strains increased markedly. Conclusions Bactrial resistance is still increasing, which poses a serious threat to lineal practice. It is urgent to take effective infection control measures.

Abstract:Abstract Background To investigate the current state of antibacterial resistance of clinical isolates from hospitals in Shanghai, China. Methods Antimicrobial susceptibility testing (AST) was carried out for the clinical isolates from 50 hospitals (including 30 grade A tertiary hospitals and 20 grade B tertiary hospitals/grade A secondary hospitals, and there were 3 children hospitals among them) according to a unified protocol using Kirby–Bauer(KB) method or automated AST systems. Results were analyzed according to CLSI 2018 breakpoints. Results Of the 144373 clinical isolates, Gram-positive cocci and Gram-negative bacilli accounted for 29.6% and 70.4%, respectively. The overall prevalence of MRSA in Staphylococcus aureus was 45.9% and 78.4% for MRCNS in coagulase-negative Staphylococcus. No strains were found resistant to vancomycin in Staphylococcus spp. 84.1% of the 1204 strains of non-meningitis S. pneumoniae isolated from children were penicillin-susceptible (PSSP), 15.9% were penicillin-nonsusceptible, including penicillin-intermediate (PISP, 10.5%) and penicillin-resistant (PRSP, 5.4%) strains. Of the 361 strains isolated from adults, 94.5%, 3.0% and 2.5% were PSSP, PISP, and PRSP, respectively. Vacomycin-resistance E. feacium was 0.7% and no vacomycin-resistant E. feacalis were identified. According to PCR results, most of these resistant strains were vanA genotype. The prevalence linezolid-nonsusceptible E. faecalis was about 1.6%, few E. feacium was resistant to Linezolid. The overall prevalence of ESBL-producing strains was 54.0% in E. coli, 35.0% in Klebsiella pneumoniae and 47.1% in Proteus mirabilis. Enterobacteriaceae isolates were still mainly susceptible to carbapenems. Overall, 11.7% and 11.2% of the Enterobacteriaceae isolates were resistant to imipenem and meropenem, respectively. The predominant organism of CRE isolates was K. pneumoniae. The prevalence of CRAB and CRPA were 62.5% and 28.7%, respectively. Conclusion Antimicrobial resistance remains to be a problematic issue in healthcare settings, especially in Gram-negative bacilli, effective infection-control measures should be promoted to tackle this critical threat. Abstract:Objective To investigate the resistance of clinical isolates in Shanghai hospitals from January to December in 2007.Methods Antimicrobial susceptibility testing of 35 907 clinical isolates were carried out according to an agreed protocol using Kirby-Bauer (K-B). Results were analyzed according to CLSI 2007. Results Of the 35907 clinical isolates,gram negative bacilli and gram positive cocci accounted for 63.0% and 37.0% respectively. The prevalence of MRSA was 61.1% in S. aureus. The prevalence of MRCNS was 75.9% in coagulase negative Staphylococcus. No vancomycin-intermediate S. aureus (VISA) or vancomycin-resistant S. aureus (VRSA) was identified. About 11.7% of the S. pneumoniae isolates from children were penicillin-susceptible (PSSP). The remaining 88.3% (PISP 73.9% and PRSP 14.4%) of S. pneumoniae strains were nonsusceptible to penicillin. Two strains of vancomycin-resistant Enterococcus (VRE) were identified in E. faecium. ESBLs-producing strains accounted for 58.0%,53.9% and 14.7% in E. coli,Klebsiella spp. and P. mirabilis,respectively. About 26.7% and 18.8% of the P. aeruginosa isolates were resistant to imipenem and meropenem respectively. And 21.0% and 22.5% of the Acinetobacter isolates were resistant to imipenem and meropenem respectively. Some pan-resistant isolates were found in P. aeruginosa (2.8%),A. baumannii (5.5%),C. freundii (6.0%) and K. pneumoniae (0.1%).Conclusions Bacterial resistance is still on the rise. It is important to strengthen the detection of resistant bacteria in routine work,screen carriers of resistant bacteria,and take further steps to conduct research on the mechanism of antibiotic resistance,which are useful for rational use of antimicrobial agents and containment of bacterial resistance.

Abstract:OBJECTIVE:To explore the antimicrobial resistance of nosocomial Gram-negative bacilli across China. METHODS:A total of 1247 consecutive and non-repetitive Gram-negative bacilli were isolated from 13 Chinese teaching hospitals from March to August 2012. All isolates were sent to a central laboratory for reidentification and susceptibility testing. The minimal inhibitory concentration (MICs) of meropenem and other antibacterial agents were determined by agar dilution method. And the data were analyzed with WHONET-5.6 software. RESULTS:The activity of antimicrobial agents against Enterobacteriaceae was in the following descending order of susceptibility rate: meropenem (97.5%, 849/871) , amikacin (94.5%, 823/871) , imipenem (93.6%, 815/871) , ertapenem (92.9%, 809/871) , piperacillin/tazobactam (89.9%, 783/871) , cefoperazone/sulbactam (83.5%, 727/871) , cefepime (78.1%, 680/871) , polymyxin B (77.0%, 670/871) , cefiazidime (69.6%, 606/871) , levofloxacin (69.2%, 603/871) , ciprofloxacin (63.6%, 554/871) , minocyline (63.1%, 550/871) , ceftriaxone (55.7%, 485/871) , cefotaxime (54.2%, 472/871) and cefoxitin (51.4%, 448/871) . The prevalence of extended-spectrum beta-lactamase (ESBL) was 64.3% (117/182) in Escherichia coli (E. coli) and 32.1% (60/187) in Klebsiella pneumonia (K. pneumoniae) . The sensitivities of E. coli to meropenem and imipenem were 100%. And over 90% of E. coli was sensitive to ertapenem, amikacin, piperacillin/tazobactam and polymyxin B. However, over 60% of E. coli was resistant to ciprofloxacin, levofloxacin, ceftriaxone and cefotaxime. The susceptibility of K. pneumoniae to meropenem, imipenem, amikacin and polymyxin B maintained at over 90%. The activities of antimicrobial agents against E. cloacae, E. aerogenes and Citrobacter freundii were in the following descending order of susceptibility rate: meropenem (96.0%-100%) , imipenem (96.0%-100%) , polymyxin B (95.8%-100%) , amikacin (92.2%-100%) , ertapenem (85.6%-93.3%) , cefepime (77.8%-93.3%) , cefoperazone/sulbactam (78.4%-90.0%) and piperacillin/tazobactam (65.0%-89.8%) . The most susceptible agent against Acinetobacter baumannii (A. baumannii) was polymyxin B (100%) . The susceptibilities of A.baumannii to imipenem, meropenem and minocyline were 37.8% (65/172) , 36.0% (62/172) and 62.8% (108/172) respectively. The most active agents against Pseudomonas aeruginosa (P. aeruginosa) were polymyxin B (97.2%, 173/178) , followed by amikacin (89.3%, 159/178) and cefiazidime (83.7%, 149/178) . Clinical and Laboratory Standards Institute revised P.aeruginosa susceptibility standard in 2012. The sensitivity of piperacillin/tazobactam changed from 83.7% (149/178) to 77.5% (138/178) . The sensitivity of meropenem decreased from 78.1% ( 139/178 ) to 71.3% ( 127/178 ) while that of imipenem declined from 69.7% (124/178) to 59.6% (106/178) . The prevalence of multi-drug resistant A. baumannii and P. aeruginosa were 65.7% (113/172) and 9.0% (16/178) respectively. CONCLUSIONS:Carbapenems remain highly active against Enterobacteriaceae. Increasing resistance of A. baumannii to all antimicrobial agents is noted. New breakpoint to P.aeruginosa has obvious effects on antimicrobial sensitivity.
Variation in resistance of beta-lactam antibiotics against bacterial pathogens during 2001～2005

Abstract:Objective To investigate the changing profile of resistance of beta-lactam antibiotics against pathogenic bacteria isolated from inpatients in Peking University Third Hospital during 2001～2005.Methods All the pathogenic bacteria from 2001 to 2005 were analyzed retrospectively.Antibiotic susceptibility was tested by Kirby-Bauer method and interpreted according to NCLLS recommendations.Results A total of 10600 clinical isolates were collected,including 6728(63.5%) strains of Gram-negative bacilli and 554(33.5%) strains of Gram-positive coccus.Methicillin-resistance Staphylococcus aureus(MRSA) was susceptible to vancomycin.Three common ESBLs-producing enterobacters were susceptible to imipenem.Pseudomonas aeruginosa,Acinetobacter baumanii and Stenotrophomonas maltophilla were only susceptible to cefoperazone-sulbactam and ceftazidime,but resistant to other beta-lactam antibiotics.Conclusions Antibiotic resistance became more prevalent in some major pathogens.Rational use of antibiotics was important for the containment of bacterial resistance.

Abstract:Antimicrobial resistance (R) surveillance across Asia and especially in China has documented unique patterns and mechanisms. This 2011 study reports results for 2278 isolates from 12 hospitals in China (94–216 strains/site); most from bacteremia (20.4%), pneumonias (29.1%), or skin and skin structure infections (20.9%). Samples were tested by reference broth microdilution methods, interpreted by published susceptibility (S) breakpoints. The most common species were Staphylococcus aureus (343, 45.8% MRSA), Escherichia coli (EC; 288), Pseudomonas aeruginosa (PSA; 221), Klebsiella spp. (KSP; 208), acinetobacters (ACB; 178), enterobacters (155), Streptococcus pneumoniae (SPN; 154, 46.8% penicillin-S), and enterococci (ENT; 137). Among 849 Gram-positive (GP) cocci, linezolid, tigecycline (TIG), daptomycin, and vancomycin provided best antimicrobial coverage (≥99.7% S). Resistance patterns of concern were 0.3% VISA, 15.4% teicoplanin non-S coagulase-negative staphylococci, 1.5% vancomycin-R ENT (all Enterococcus faecium), 1.9% levofloxacin-R β-haemolytic streptococci, and 35.1 and 12.7% ceftriaxone-non-S rates for SPN and viridans group streptococci, respectively. For Gram-negative bacilli, R among Enterobacteriaceae was highest against β-lactams (extended spectrum β-lactamase-phenotype strains at 73.6 and 42.8% in EC and KSP, respectively; carbapenem-R was only 2.1–4.3% with KPC and IMP type enzymes detected in KSP). The widest spectrum agents were cefoperazone/sulbactam (79.5–86.1%), piperacillin/tazobactam (88.9–92.0%), TIG (98.6–100%), amikacin (AMK; 91.8–93.7%), and meropenem (95.7–97.1% S). PSA was most inhibited by AMK (90.5% S) and colistin (COL; 99.5%), with cefepime (67.9%) best among the tested β-lactams. Only COL (100% S) and TIG (MIC90, 2 μg/mL) showed significant potencies against ACB. In conclusion, R among pathogens from 12 Chinese hospitals illustrates several agents active against GP pathogens, but more serious R problems were noted among Enterobacteriaceae, PSA, and ACB. Combination treatment for the latter multidrug-R strains appears necessary, guided by local antibiograms and national surveillance results applying reference methods.

Abstract:Objective To investigate the resistance of clinical isolates from 14 Shanghai hospitals in 2004. Methods Antimicrobial susceptibility testing of (26 890) clinical isolates was carried out by means of Kirby-bauer(KB) and results were analysed according to NCCLS(2004). Results Of 26 890 clinical isolates, gram negative bacilli and gram positive cocci was accounted for (64.7)% and 35.3% respectively. Methicillin-resistant Staphylococci (MRSA and MRCNS) were still a major problem the detection rates of MRS being 63.9% in S.aureus ,and 82.9% in coagulase negative Staphylococcus. Penicillin-nonsusceptible S.pneumoniae (PISP+PRSP) strains accounted for 70.1% in children isolates, but only (8.8)% in adult isolates. No strains of gram positive cocci were resistant to vancomycin and teicoplanin. No VISA, VRSA or VRE were found. In Enterobacteriaceae, ESBLs-producing strains accounted for 36.5% and 45.0% in E.coli and Klebsiella spp. respectively. (24.0)% strains of P.mirabilis also produced ESBLs. ESBLs-producing strains were resistant to most β-lactams and some other antimicrobial agents. Imipenem and meropenem are the most potent antimicrobial agents. The antimicrobial activity of imipenem and meropenem against Enterobacteriaceae strains were better than non-fermantive against gram negative organisms. Conclusions Detection rates of MRSA and MRCNS were high, as well as PISP and PRSP in S.pneumoniae from children isolates and ESBLs in E.coli and Klebsiella spp.. No strains of gram positive cocci resistant to glycopeptides were found. Our data will be useful for the selection of antimicrobial agents for the treatment of bacterial infections.
