Abstract:The article by Karlsson and colleagues 1 provides a sobering look at the high prevalence of young children aged 6 to 23 months in low- and middle-income countries who rely exclusively on breast milk or other fluids (eg, broth, juice, tea, and coffee) as their sole source of nutrition. The authors used the novel metric of zero food to classify children who did not consume any solid or semisolid food items during the 24 hours before each survey to characterize the magnitude of this problem. While exclusive breastfeeding or formula is well established as the most important feeding practice for newborns and infants in the first 6 months of life to promote optimal growth and immunocompetence protecting children against diseases including diarrhea and upper respiratory infections, 2 alone, they do not meet the dietary needs of a growing child older than 6 months. At older ages, infants and young children increase their physiological requirements for energy, protein, and micronutrients as they are increasingly exposed to infectious pathogens. Thus, it is imperative that their diet be complemented by an array of nutritious foods in addition to breastfeeding or formula, ideally a combination of foods from a diversity of food groups: grains, beans or other legumes, dairy products, meats, eggs, and vegetables and fruits, particularly those rich in vitamin A. The consumption of foods from at least 5 of these food groups constitutes a minimum dietary diversity and is associated with micronutrient sufficiency for infants and young children aged 6 to 23 months. 3 The finding that 1 in 10 children aged 6 to 23 months (10.4%) from 92 low- and middle-income countries consumed nothing but breast milk 1 indicates a type of infant starvation during this critical period of rapid physical growth and cognitive development, placing these children at increased risk of stunting and underweight and infectious diseases. Improper infant feeding and undernutrition are associated with developmental delays, cognitive impairment, as well as a heightened chance of developing chronic noncommunicable disorders later in life such as allergic diseases. This figure represents an estimated 13.9 million infants with direct consequences, not only for individual children and their families but for entire societies, as it constrains future economic growth and human capital potential. 4 The study also reveals the important geographic and cultural differences that are associated with diets, as the prevalence of zero-food diets ranged from 0.4% in Tunisia to 21.8% in Guinea and was highest in West and Central Africa (10.5%) and India (19.3%), 1 which corresponds to estimates on the prevalence of acute malnutrition, or wasting. 5 In addition, significant intracountry inequities in infant feeding exist, as differences in zero-food prevalences among the 36 states of India using National Family Health Surveys have been reported, which were associated with rates of low maternal education and household poverty. 6 It should be noted that in that study, 6 virtually all of the nationally representative surveys on child feeding included in the analysis were conducted prior to 2021 and did not fully reflect the adverse impact that the global COVID-19 pandemic and the Russia-Ukraine war may have had on food supplies and childhood malnutrition. 7 The article does a good job to cite some of the limitations of the study, including the use of national aggregate estimates that may have masked disparities and inequities among subgroups within populations, but did not mention the importance of sex, which is likely a major driver of dietary inadequacy and feeding practices in many countries. (Sex was not a significant indicator in the study in India. 6 ) The article amplified the need to identify effective strategies to improve access and affordability of nutritious foods for complementary feeding of young children. While poverty is a key driver of poor diets, it is imperative that countries prioritize efforts to strengthen social behavior change and communications to increase awareness among parents and caregivers of the importance of complementary foods along with culturally specific guidelines on the preparation of nutritious, complementary foods. While achievement of a minimum dietary diversity may be beyond the means of some families, there are ways to increase the nutrient density of diets, such as adding critical vitamins and minerals through fortification of staple foods like wheat, maize, rice, and other cereals that constitute the main source of dietary energy in many low-income households. 8 Similarly, use of iodized salt is a low-cost way to ensure optimal intake of iodine.
