Abstract:Introduction: In the big data era, where corporations commodify health data, non-fungible tokens (NFTs) present a transformative avenue for patient empowerment and control. NFTs are unique digital assets on the blockchain, representing ownership of digital objects, including health data. By minting their data as NFTs, patients can track access, monetize its use, and build secure, private health information systems. However, research on NFTs in healthcare is in its infancy, warranting a comprehensive review. Methods: This study conducted a systematic literature review and thematic analysis of NFTs in healthcare to identify use cases, design models, and key challenges. Five multidisciplinary research databases (Scopus, Web of Science, Google Scholar, IEEE Explore, Elsevier Science Direct) were searched. The approach involved four stages: paper collection, inclusion/exclusion criteria application, screening, full-text reading, and quality assessment. A classification and coding framework was employed. Thematic analysis followed six steps: data familiarization, initial code generation, theme searching, theme review, theme definition/naming, and report production. Results: Analysis of 19 selected papers revealed three primary use cases: patient-centric data management, supply chain management for data provenance, and digital twin development. Notably, most solutions were prototypes or frameworks without real-world implementations. Four overarching themes emerged: data governance (ownership, tracking, privacy), data monetization (commercialization, incentivization, sharing), data protection, and data storage. The focus lies on user-controlled, private, and secure health data solutions. Additionally, data commodification is explored, with mechanisms proposed to incentivize data maintenance and sharing. NFTs are also suggested for tracking medical products in supply chains, ensuring data integrity and provenance. Ethereum and similar platforms dominate NFT minting, while compact NFT storage options are being explored for faster data access. Conclusion: NFTs offer significant potential for secure, traceable, decentralized healthcare data exchange systems. However, challenges exist, including dependence on blockchain, interoperability issues, and associated costs. The review identified research gaps, such as developing dual ownership models and data pricing strategies. Building an open standard for interoperability and adoption is crucial. The scalability, security, and privacy of NFT-backed healthcare applications require further investigation. Thus, this study proposes a research agenda for adopting NFTs in healthcare, focusing on governance, storage models, and perceptions.

Abstract:In recent years we have seen the adoption of distributed ledger technology (DLT), originally the mechanism underpinning the operation of the Bitcoin crypto currency, across a wider range of technology sectors including healthcare. DLT allows for the design of informatics systems with the properties of immutability, security, and decentralization. One recent innovation in the space has been the specification and development of Non-Fungible Tokens (NFTs). NFTs are decentralized DLT-based records that represent ownership of a unique digital asset. The predominant current use case for NFTs has been in the representation and sale of digital artwork, however the features offered by NFTs, unique-ness, immutability, transferability, and verifiability, are directly applicable to the design of health informatics systems. In this paper we explore these properties and describe a reference architecture for using NFTs as a means of representing and transferring records of patient's consent for medical data use.

Abstract:The integration of nonfungible tokens (NFTs) in health care, particularly in cardiovascular medicine, represents a disruptive shift toward enhancing the security and interconnection of electronic health data around the patient. NFTs, unique digital certificates stored on a blockchain network, bind various sources of health data to their owner (the patient) and delineate the access rights for stakeholders (providers, researchers) using smart contracts. Data sources might include electronic medical records from different hospitals, clinics, pharmacies, test centres, and mHealth devices. Accordingly, patients and their providers benefit from seamless visibility of diagnoses, medications, electrocardiograms, imaging, home blood pressure logs, and artificial intelligence-enabled insights from these aggregated data. Rather than being stored on proprietary servers, data are encrypted and stored on decentralized networks with a unified point of access and immutable proof of ownership, making them more robust to theft or tampering. As custodians of their NFTs, patients are incentivized to actively partake in their health monitoring and self-driven research that aligns with their needs using innovative marketplaces that allow them to browse studies, document their informed consent, and monetize their contributions. Furthermore, they are empowered to educate themselves and seek care across siloes in traditional settings or virtual platforms such as the metaverse, where NFTs serve as digital passports. Despite these exciting prospects, adoption within the health care sector remains in its infancy, with ethical and technical limitations still being addressed. In this article we explore the multifaceted applications and key players in the field, and outline use-cases for patient-centred cardiovascular care featuring NFTs.

Abstract:Amid global health challenges, resilient health systems require continuous innovation and progress. Stakeholders highlight the critical role of digital technologies in accelerating this progress. However, the digital health field faces significant challenges, including the sensitivity of health data, the absence of evidence-based standards, data governance issues, and a lack of evidence on the impact of digital health strategies. Overcoming these challenges is crucial to unlocking the full potential of digital health innovations in enhancing healthcare delivery and outcomes. Prioritizing security and privacy is essential in developing digital health solutions that are transparent, accessible, and effective. Non-fungible tokens (NFTs) have gained widespread attention, including in healthcare, offering innovative solutions and addressing challenges through blockchain technology. This paper addresses the gap in systematic-level studies on NFT applications in healthcare, aiming to comprehensively analyze use cases and associated research challenges. The search included primary studies published between 2014 and November 2023, searching in a balanced set of databases compiling articles from different fields. A review was conducted following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) framework and strictly focusing on research articles related to NFT applications in the healthcare sector. The electronic search retrieved 1902 articles, ultimately resulting in 15 articles for data extraction. These articles span applications of NFTs in medical devices, pathology exams, diagnosis, pharmaceuticals, and other healthcare domains, highlighting their potential to eliminate centralized trust sources in health informatics. The review emphasizes the adaptability and versatility of NFT-based solutions, indicating their broader applicability across various healthcare stages and expansion into diverse industries. Given their role in addressing challenges associated with enhancing data integrity, availability, non-repudiation, and authentication, NFTs remain a promising avenue for future research within digital health solutions.

Abstract:Introduction: Non-Fungible Tokens (NFTs) are digital assets that are verified using blockchain technology to ensure authenticity and ownership. NFTs have the potential to revolutionize healthcare by addressing various issues in the industry. Method: The goal of this study was to identify the applications of NFTs in healthcare. Our scoping review was conducted in 2023. We searched the Scopus, IEEE, PubMed, Web of Science, Science Direct, and Cochrane scientific databases using related keywords. The article selection process was based on Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA). Results: After applying inclusion and exclusion criteria, a total of 13 articles were chosen. Then extracted data was summarized and reported. The most common application of NFTs in healthcare was found to be in health data management with 46% frequency, followed by supply chain management with 31% frequency. Furthermore, Ethereum is the main blockchain platform that is applied in NFTs in healthcare with 70%. Discussion: The findings from this review indicate that the NFTs that are currently used in healthcare could transform it. Also, it appears that researchers have not yet investigated the numerous potentials uses of NFTs in the healthcare field, which could be utilized in the future.

Abstract:CryptoKitties, a trendy game on Ethereum that is an open-source public blockchain platform with a smart contract function, brought nonfungible tokens (NFTs) into the public eye in 2017. NFTs are popular because of their nonfungible properties and their unique and irreplaceable nature in the real world. The embryonic form of NFTs can be traced back to a P2P network protocol improved based on Bitcoin in 2012 that can realize decentralized digital asset transactions. NFTs have recently gained much attention and have shown an unprecedented explosive growth trend. Herein, the concept of digital asset NFTs is introduced into the medical and health field to conduct a subversive discussion on biobank operations. By converting biomedical data into NFTs, the collection and circulation of samples can be accelerated, and the transformation of resources can be promoted. In conclusion, the biobank can achieve sustainable development through "decentralization."
Self-sovereign Identity Empowered Non-Fungible Patient Tokenization for Health Information Exchange Using Blockchain Technology.

Abstract:Background: Patient tokenization is a novel approach that allows anonymous patient-level linkage across healthcare facilities, minimizing the risk of breaching protected health information in health information ex-change (HIE). Most patient tokenization is the centralized approach that is unable to address data security concerns fundamentally. Non-Fungible Tokens (NFT), which are non-transferable cryptographic assets on the blockchain, have the potential to provide secure, decentralized, and trustworthy patient tokenization. Self -Sovereign Identity (SSI) is a user-centric approach to verify the ownership of NFTs in a decentralized manner.Methods: We have developed a blockchain architecture that contains four modules: (1) Creation module for NFTs creation, (2) Linkage module to link the local patients' accounts to their NFTs, (3) Authentication module that allows patients to permit healthcare providers to access their token, and (4) Exchange module, which involves the HIE process and the validation of the legitimacy of the token through SSI.Results: A case study has been conducted on the proposed architecture. Over 3 million transactions have been completed successfully with a blockchain validation and written time of 1.17 s on average. A stability test has also been conducted with a higher throughput of 200 transactions per second running for an hour with an average transaction processing time of 1.42 s.Conclusions: This study proposed a blockchain architecture that achieves SSI-enabled NFT-based patient toke-nization. Our architecture design, implementation, and case studies have demonstrated the feasibility and po-tential of NFT with SSI to establish a secure, transparent, and patient-centric identity management and HIE.

Abstract:There is increasing recognition about health-oriented datasets that could be regarded as intangible assets: distinct assets with future economic benefits but without physical properties. While health-oriented datasets - particularly health records - are ascribed monetary value on the black market, there are few established methods for assessing the value for legitimate research and business purposes. The emergence of blockchain has created new commercial opportunities for transferring assets without intermediaries. Therefore, blockchain is proposed as a medium by which research datasets could be transacted to provide future value. For authorized individuals to verify their transactions, blockchain methodologies offer security, auditability, and transparency. The authors share data valuation methodologies consistent with accounting principles and include discussions of black market valuation of health data. Furthermore, this article describes blockchain-based methods of managing real-time payment/micropayment strategies.

Abstract:Non-Fungible Tokens (NFTs) are a new development in blockchain technology. News around NFTs is surrounded by skepticism because unrealistically high prices are being paid online for these NFTs which are in the form of apparently simple digital arts and photographs. It is not clear if this is a trend, a hype, a bubble, or a legitimate novel way of holding and trading value. A literature review of peer-reviewed scholarly studies, performed in the context of business and management, is presented here. Moreover, we also discuss open issues, and challenges, and present future research directions. Analysis of these studies reveal that schools of thoughts are divided on the validity of this form of digital tokens. On one hand, there is a lot of criticism but on the other hand, we can find novel business models and applications of NFTs especially the feature of smart contracts. It can, therefore, be concluded that NFTs, even if not in their current form, are here to stay and may promise new ways of protecting digital assets in an immutable and easily traceable form.

Abstract:Currently, most of the personal health data (PHD) are managed and stored separately by individual medical institutions. When these data need to be shared, they must be transferred to a trusted management center and approved by data owners through the third-party endorsement technology. Therefore, it is difficult for personal health data to be shared and circulated over multiple medical institutions. On the other hand, the use of directly exchanging and sharing the original data has become inconsistent with the data rapid growth of medical institutions because of the need of massive data transferring across agencies. In order to secure sharing and managing the mass personal health data generated by various medical institutions, a federal personal health data management framework (PHDMF, ) has been developed, which had the following advantages: 1) the blockchain technology was used to establish a data consortium over multiple medical institutions, which could provide a flexible and scalable technical solution for member extension and solve the problem of third-party endorsement during data sharing; 2) using data distributed storage technology, personal health data could be majorly stored in their original medical institutions, and the massive data transferring process was of no further use, which could match up with the data rapid growth of these institutions; 3) the distributed ledger technology was utilized to record the hash value of data, given the anti-tampering feature of the technology, malicious modification of data could be identified by comparing the hash value; 4) the smart contract technology was introduced to manage users' access and operation of data, which made the data transaction process traceable and solved the problem of data provenance; and 5) a trusted computing environment was provided for meta-analysis with statistic information instead of original data, the trusted computing environment could be further applied to more health data, such as genome sequencing data, protein expression data, and metabolic profile data through combining the federated learning and blockchain technology. In summary, the framework provides a convenient, secure, and trusted environment for health data supervision and circulation, which facilitate the consortium establish over medical institutions and help achieve the value of data sharing and mining.

Abstract:INTRODUCTION: Patients with diabetes often generate large amounts of data specifically related to the disease and to their general health. Cross-institutional sharing of patient health care data is complex, and as a consequence, data are not always available to the health care provider treating the patient. Accommodating this challenge could lead to better clinical effectiveness and improve clinical research. This work aims to present an approach for a blockchain-based platform for sharing health care data. The approach considers privacy concerns, data sharing, and patients as the center for governing their own data.METHODS: The concept of this blockchain-based platform consists of using the NEM multi-signature blockchain contracts for access control of data management and the sharing and encryption of data to allow privacy and control of health care data. The architecture is built around cryptography, tokens, and multi-signature contracts. The multi-signature contract enables several entities to administrate the activity of an account and control the assets of one account. Multi-signature generates a contract that assigns the rights and powers of a certain account to other accounts; this contract can be edited to allow or remove entities.DISCUSSION: Using blockchain could lead to improvements in diabetes data management. In the coming years, this technology should be implemented in existing small-scale diabetes health care system to explore its real-world benefits and challenges.CONCLUSION: This new approach could potentially lead to more efficient sharing of data between institutions and utilization of new types of data and research possibilities.
Efficient and Secure Management of Medical Data Sharing Based on Blockchain Technology

Abstract:In the current landscape of medical data management, processing data across diverse institutions and maximizing their value are paramount. However, traditional methods lack a secure and efficient mechanism for end-to-end traceability and supervision, posing challenges in distributed scenarios lacking mutual trust. Leveraging blockchain’s decentralized, tamper-proof, and traceable features, this paper introduces a blockchain-based medical data management platform. This platform enables full-process management of raw data, operational behaviors, intermediate data, and final data, meeting the needs of trusted storage and supervision of data. We propose two methods, namely, naive method and DAG-based method, to realize forward tracking and backward tracing of medical data stored on the blockchain, respectively. We validated and analyzed the storage and query performance of the medical data management platform on real medical data, and we also conducted experimental analyses on the efficiency of the proposed traceability algorithm under different data scales and processing path lengths. The results demonstrate that our platform and traceability methods effectively meet the management needs of medical data distributed across institutions.

Abstract:With the assistance of the Internet of Things, the fast developing Healthcare Internet of Things (H-IoT) has promoted the healthcare ecosystem into the era of Health 5.0 and enables many promising medical applications, such as remote healthcare that is crucial in pandemic (e.g., coronavirus disease 2019). Healthcare participants can make accurate diagnosis, treatment, and research based on the shared personal health records (PHRs) sensed from remote H-IoT devices. However, current H-IoT systems fall short of a secure and trustworthy PHR sharing service in remote healthcare, which is able to prevent user privacy leakage and PHR integrity violation together with high efficiency in key distribution alongside efficient data retrieval and fine-grained access control. In response, we present a blockchain-based hierarchical data sharing framework (BHDSF) to provide fine-grained access control and efficient retrieval over encrypted PHRs with low consumed hierarchical key distribution and key leakage resistance. Compared with the existing solutions, the BHDSF takes both untrusted cloud and malicious auditor into consideration simultaneously and achieves trustworthy PHR integrity auditing and metadata verification by leveraging the blockchain technique. Besides, the BHDSF enables efficiently aggregative authentication for the trustworthiness of source records from H-IoT devices, which is lacked in most of the existing data sharing frameworks. Finally, we demonstrate the feasibility of the BHDSF by conducting extensive empirical tests over a real-world dataset.

Abstract:Healthcare data are a valuable source of healthcare intelligence. Sharing of healthcare data is one essential step to make healthcare system smarter and improve the quality of healthcare service. Healthcare data, one personal asset of patient, should be owned and controlled by patient, instead of being scattered in different healthcare systems, which prevents data sharing and puts patient privacy at risks. Blockchain is demonstrated in the financial field that trusted, auditable computing is possible using a decentralized network of peers accompanied by a public ledger. In this paper, we proposed an App (called Healthcare Data Gateway (HGD)) architecture based on blockchain to enable patient to own, control and share their own data easily and securely without violating privacy, which provides a new potential way to improve the intelligence of healthcare systems while keeping patient data private. Our proposed purpose-centric access model ensures patient own and control their healthcare data; simple unified Indicator-Centric Schema (ICS) makes it possible to organize all kinds of personal healthcare data practically and easily. We also point out that MPC (Secure Multi-Party Computing) is one promising solution to enable untrusted third-party to conduct computation over patient data without violating privacy.

Abstract:Nonfungible tokens (NFTs) have recently drawn considerable attention, highlighted by a digital art piece that sold for $69M USD in early 2021. Though they have only just started receiving coverage by traditional media outlets and interest from casual observers, the foundations of NFT technology date back to advances in computer science in the late 1970s. In this article, we examine the emergence of NFTs, from their technical origins, the introduction of blockchain technologies and the first token-based collectibles that led to modern day NFT products. We categorize the current use cases for NFTs, introduce their potential future applications, and highlight the challenges managers face in incorporating them into their existing workflows. By presenting our NFT adoption framework, we offer managers strategies for evaluating the risks and benefits of NFTs.

Abstract:Non-fungible tokens or NFTs are the digital assets on a blockchain. NFTs are unique and they cannot be divided like cryptocurrencies. NFTs could store digital ownership of an artwork or collections or can be fan tokens or tickets for clubs. NFTs are based on a smart contract on a blockchain network which supports them, such as Ethereum, Cardano or Polkadot. Most of the NFTs are now minted on Ethereum (ERC-20) network, but it has some main issues like high transaction fees and low speed. There are lots of domains which can be benefited from NFT technology such as art, music, gaming, sport and wildlife conservation. NFTs could be also bought or sold on lots of NFT marketplaces such as OpenSea and Chiliz. The trend is in a huge hype because the market cap and popularity of NFTs are growing significantly.

Abstract:The rapid development of wearable sensors and the 5G network empowers traditional medical treatment with the ability to collect patients’ information remotely for monitoring and diagnosing purposes. Meanwhile, the health-related mobile apps and devices also generate a large amount of medical data, which is critical for promoting disease research and diagnosis. However, medical data is too sensitive to share, which is also a common issue for IoT (Internet of Things) data. The traditional centralized cloud-based medical data sharing schemes have to rely on a single trusted third party. Therefore, the schemes suffer from single-point failure and lack of privacy protection and access control for the data. Blockchain is an emerging technique to provide an approach for managing data in a decentralized manner. Especially, the blockchain-based smart contract technique enables the programmability for participants to access the data. All the interactions are authenticated and recorded by the other participants of the blockchain network, which is tamper resistant. In this paper, we leverage the K-anonymity and searchable encryption techniques and propose a blockchain-based privacy-preserving scheme for medical data sharing among medical institutions and data users. To be specific, the consortium blockchain, Hyperledger Fabric, is adopted to allow data users to search for encrypted medical data records. The smart contract, i.e., the chaincode, implements the attribute-based access control mechanisms to guarantee that the data can only be accessed by the user with proper attributes. The K-anonymity and searchable encryption ensure that the medical data is shared without privacy leaking, i.e., figuring out an individual patient from queries. We implement a prototype system using the chaincode of Hyperledger Fabric. From the functional perspective, security analysis shows that the proposed scheme satisfies security goals and precedes others. From the performance perspective, we conduct experiments by simulating different numbers of medical institutions. The experimental results demonstrate that the scalability and performance of our scheme are practical.

Abstract:Healthcare blockchains provide an innovative way to store healthcare information, execute healthcare transactions, and build trust for healthcare data sharing and data integration in a decentralized open healthcare network environment. Although the healthcare blockchain technology has attracted broad interests and attention in industry, government and academia, the security and privacy concerns remain the focus of debate when deploying blockchains for information sharing in the healthcare sector from business operation to research collaboration. This article focuses on the security and privacy requirements for medical data sharing using blockchain, and provides a comprehensive analysis of the security and privacy risks and requirements, accompanied by technical solution techniques and strategies. First, we discuss the security and privacy requirements and attributes required for electronic medical data sharing by deploying the healthcare blockchain. Second, we categorize existing efforts into three reference blockchain usage scenarios for electronic medical data sharing, and discuss the technologies for implementing these security and privacy properties in the three categories of usage scenarios for healthcare blockchain, such as anonymous signatures, attribute-based encryption, zero-knowledge proofs, verification techniques for smart contract security. Finally, we discuss other potential blockchain application scenarios in healthcare sector. We conjecture that this survey will help healthcare professionals, decision makers, and healthcare service developers to gain technical and intuitive insights into the security and privacy of healthcare blockchains in terms of concepts, risks, requirements, development and deployment technologies and systems.

Abstract:In the ever-evolving healthcare sector, the widespread adoption of Internet of Things and wearable technologies facilitates remote patient monitoring. However, the existing client/server infrastructure poses significant security and privacy challenges, necessitating strict adherence to healthcare data regulations. To combat these issues, a decentralized approach is imperative, and blockchain technology emerges as a compelling solution for strengthening Internet of Things and medical systems security. This paper introduces HNMblock, a model that elevates the realms of epidemiological monitoring, medical system security, and wellness enhancement. By harnessing the transparency and immutability inherent in blockchain, HNMblock empowers real-time, tamper-proof tracking of epidemiological data, enabling swift responses to disease outbreaks. Furthermore, it fortifies the security of medical systems through advanced cryptographic techniques and smart contracts, with a paramount focus on safeguarding patient privacy. HNMblock also fosters personalized healthcare, encouraging patient involvement and data-informed decision-making. The integration of blockchain within the healthcare domain, as exemplified by HNMblock, holds the potential to revolutionize data management, epidemiological surveillance, and wellness, as meticulously explored in this research article.
