Abstract:Ashruf and colleagues 1 present retrospective data from the electronic medical records of more than 17 000 pediatric patients who experienced community-acquired acute kidney injury (CA-AKI) over a 20-year study period. This work raises awareness of 2 critically important yet understudied topics in pediatric AKI and sets the stage for future study in these areas. Pediatric CA-AKI has historically been difficult to define owing to the paucity of baseline serum creatinine (SCr) data in children. 2 The large database used for this study contains information from 21 million pediatric patients with a CA-AKI prevalence of less than 0.1%. A database of this size provides the opportunity to examine important questions about CA-AKI in a way that cannot be done with the small sample sizes derived from single-center or multicenter studies. Using propensity matching, the authors 1 compared the risk of 1-year adverse outcomes, including emergency department (ED) visits, intensive care unit admissions, mechanical ventilation, and death, in patients after CA-AKI. Patients were stratified by race (Asian, Hispanic, non-Hispanic Black, and non-Hispanic White) and age (0-9 years vs 10-18 years) according to electronic medical record reporting. CA-AKI diagnoses were based on billing codes for most patients. For 24% of the patients, a CA-AKI diagnosis was assigned on the basis of an increase in SCr more than 0.3 mg/dL above baseline (although determination of baseline SCr is not reported). 1 The authors report that Black and Hispanic children who had a CA-AKI diagnosis had a higher risk of composite and individual outcomes compared with White children. Asian children had a greater likelihood of mechanical ventilation, but overall had outcomes similar to those of White children. Black and Hispanic children aged 0 to 9 years had the highest risk of adverse outcomes by the end of the study period. The authors 1 posit that these disparate outcomes after an episode of CA-AKI could be the result of one or more factors, such as "disease burden, progression, clinical care, or social determinants of health," all of which are complicated, nuanced issues that are challenging to frame and study. Similarly, the suggested interventions—education, advocacy, and more accurate diagnostic tools for the detection of AKI—are all worthwhile and critical for the mission, yet resource intensive in terms of both development and implementation. Given the disruptive and detrimental outcomes to the children described in this analysis, both CA-AKI and health care disparities deserve ongoing investigation and efforts to understand and mitigate their impact. We must consider, are we asking the right questions about sociodemographic disparities? Racial disparities have been reported in the incidence and outcome of AKI in both adults and children. 3 -5 However, these are largely understood to be associated with social inequities and racism, rather than race itself, which is a social construct and not representative of inherent biological differences. 6 It is not enough to identify differences in outcomes by race without a deeper dive into the factors underlying those differences in the social constructs. In adults, both the risk of AKI and the risk of mortality after AKI are highest among those with the greatest inequities in income, employment, education, health and disability, crime, barriers to housing, and living environment. 7 In a report of more than 10 000 adults in the Atherosclerosis Risk in Communities study, 5 Black participants were at a greater risk of AKI compared with White participants, and this was hypothesized to be related to greater inequities in income and insurance. Ashruf et al 1 do not report details of the social determinant inequities in this cohort. However, they do report potential health hazards related to socioeconomic and psychosocial concerns and acknowledge that more research is needed to determine whether specific social factors may be impacting the racial disparities in pediatric CA-AKI. Ashruf et al 1 also report higher rates of ED visits for Black and Hispanic children with CA-AKI compared with White children with CA-AKI. They did not include patients without CA-AKI, so it is not known whether the increased ED visits and worse outcomes are related to race and social determinant inequities or CA-AKI. Black and Hispanic patients are more likely to experience greater acute care utilization, longer wait times, and worse outcomes in the ED, related to previously discussed social determinants of inequities. 7 Thus, future research building upon this study should evaluate children with and without CA-AKI along with more detailed social determinants to address these questions.
