Abstract:OBJECTIVE:To study the social determinants and health-related quality of life among the Chinese 'empty nest' (lonely) elderly.METHODS:A total of 4995 elderly, aged 60 or over were recruited in rural and urban areas of Zhejiang province, under a cluster sampling method. Stepwise logistic regression analysis was performed to analyze the social determinants on the 'empty nest' elderly. Health-related quality of life was assessed by SF-36 and compared between the 'empty nest' and 'non-empty nest' elderly, using multiple linear regression analysis after adjusting for confounding factors.RESULTS:Comparing with the 'non-empty nest' elderly, the characteristics of the empty nest population were as follows: age at 70 to 79 (P = 0.0417), having higher education level (P < 0.0001), living in rural area (P < 0.0001), lower income level (P = 0.0178, P = 0.0049), with higher level of satisfaction of life (P = 0.0070, P = 0.0035) and lower level of depressive mood (P = 0.0371). 'Empty nest' elderly people had lower scores of SF-36 in BP (P = 0.0032)dimension of physical function, and RE (P = 0.0033) dimension of psychological function as well as MCS (P = 0.0342) after adjusting for age, gender, marriage status, education level, residential areas, personal income, scores for satisfaction of life and depressive mood.CONCLUSION:The 'empty nest' elderly people with the characteristics such as higher education level, lower income level and living in the rural areas should be under greater concern, especially on those people with lower health-related quality of life or poorer psychological function.

Abstract:Background: In China, rapid economic growth and increasing social problems constitute the two basic characteristics underlying contemporary social change. With dramatic social change, loneliness in older adults may have changed across birth cohorts, thus altering older adults' mental health. The present study aims to identify birth cohort changes in Chinese older adults' loneliness and the social indicators underlying these changes.Methods: Cross-temporal meta-analysis was utilized to investigate changes in Chinese older adults' loneliness from 1995 to 2011. We analyzed 25 studies (N = 13,280 adults; age >= 60 years) employing the University of California at Los Angeles Loneliness Scale. We correlated loneliness scores with social indicators and matched these correlations for three periods: ten years before the data collection, five years before data collection, and during the year of data collection.Results: Loneliness levels in Chinese older adults have increased by 1.02 standard deviations from 1995 to 2011. Social indicators such as increased urbanization level, personal medical expenditure, divorce rate, the Gini coefficient, and unemployment rate significantly predicted loneliness in Chinese older adults. Decrease in social connectedness and increase in levels of health threat may be responsible for the observed increase in levels of loneliness.Conclusions: Cross-temporal meta-analysis revealed a birth cohort increase in loneliness among Chinese older adults. We conclude that changes in social connectedness and levels of health threat likely play an important role in predicting loneliness in the population of Chinese elderly adults.
Are China’s Oldest-Old Living Longer with Less Disability? A Longitudinal Modeling Analysis of Birth Cohorts Born 10 Years Apart

Abstract:China has transitioned from being one of the fastest-growing populations to among the most rapidly aging countries worldwide. In particular, the population of oldest-old individuals, those aged 80+, is projected to quadruple by 2050. The oldest-old represent a uniquely important group—they have high demand for personal assistance and the highest healthcare costs of any age group. Understanding trends in disability and longevity among the oldest-old—that is, whether successive generations are living longer and with less disability—is of great importance for policy and planning purposes. We utilized data from successive birth cohorts (n = 20,520) of the Chinese oldest-old born 10 years apart (the earlier cohort was interviewed in 1998 and the later cohort in 2008). Disability was defined as needing personal assistance in performing one or more of five essential activities (bathing, transferring, dressing, eating, and toileting) or being incontinent. Participants were followed for age-specific disability transitions and mortality (in 2000 and 2002 for the earlier cohort and 2011 and 2014 for the later cohort), which were then used to generate microsimulation-based multistate life tables to estimate partial life expectancy (LE) and disability-free LE (DFLE), stratified by sex and age groups (octogenarians, nonagenarians, and centenarians). We additionally explored sociodemographic heterogeneity in LE and DFLE by urban/rural residence and educational attainment. More recently born Chinese octogenarians (born 1919–1928) had a longer partial LE between ages 80 and 89 than octogenarians born 1909–1918, and octogenarian women experienced an increase in partial DFLE of 0.32 years (P = 0.004) across the two birth cohorts. Although no increases in partial LE were observed among nonagenarians or centenarians, partial DFLE increased across birth cohorts, with a gain of 0.41 years (P < 0.001) among nonagenarians and 0.07 years (P = 0.050) among centenarians. Subgroup analyses revealed that gains in partial LE and DFLE primarily occurred among the urban resident population. Successive generations of China’s oldest-old are living with less disability as a whole, and LE is expanding among octogenarians. However, we found a widening urban-rural disparity in longevity and disability, highlighting the need to improve policies to alleviate health inequality throughout the population.
Impact of Social Changes and Birth Cohort on Subjective Well-Being in Chinese Older Adults: A Cross-Temporal Meta-analysis, 1990–2010

Abstract:Over the past several decades, the proportion of older adults in the Chinese population has steadily increased during a period of substantial social change; the effect of these changes on older adults warrants examination. The present study explored changes in subjective well-being (SWB) in older adults by birth cohort and the social factors influential in these changes. We performed a cross-temporal meta-analysis using data from 20,713 adults over 60 years of age; data were obtained from 61 studies that used the Memorial University of Newfoundland Scale of Happiness. The dynamics of SWB in older adults were evaluated as a function of time. The mean SWB score decreased by 4.98 over the measured period, indicating a decrease of .52 standard deviations between 1990 and 2010. In addition, SWB in older adults was significantly correlated with high social connectedness and low overall threat. Specifically, urbanization level, Gini coefficient, personal medical expenditure, and birth rate were strong predictors of SWB. Our research demonstrates that social connectedness and overall threat contribute to SWB in Chinese older adults. As SWB is significantly related to the mental and physical health of older adults, the present study may serve as a useful reference for health care agencies and policy-makers.

Abstract:We use data from the 2014 China Longitudinal Aging Social Survey to examine the effect of attitudes toward aging on quality of life among older adults in China. We find that older people who experience psychosocial loss may be less satisfied with their life, and have more feelings of depression and loneliness. Compared to older women, older men have lower life satisfaction, and more depression and loneliness, though psychosocial loss has less of an effect on them. Older women who experience psychosocial loss are more likely to be dissatisfied with their life and feel more depressive and lonelier than their male counterparts. Among those who experience psychosocial loss, older people living in rural communities have a higher level of life satisfaction, while living in urban areas may counteract some of the positive impact of psychological growth on reducing the frequency of feeling depressed and lonely. Social policy can play a role in shaping constructive social/community environments to build more positive attitudes toward aging.

Abstract:In China, rapid economic growth and increasing social problems constitute two basic characteristics of contemporary social change. During the process of dramatic social change, an emerging question is how adolescents' mental health has changed across birth cohorts. The present paper reviews four studies of crosstemporal meta-analysis conducted by us. By meta-analysis of previous literature, we examined changes in mean scores on mental health measures over time (from the early 1990s to the mid-2000s). It was found that since the early 1990s, Chinese adolescents' mental health deteriorated across birth cohorts, shown in increased scores on the negative indicators of mental health (e.g. mental problems, anxiety, and depression), whereas self-esteem as a positive trait decreased. The dropping trend in Chinese adolescents' mental health could be attributed to social change, especially increasing social problems. Therefore, adequate attention must be paid to potential influences of social change on individuals' psychological development.

Abstract:Objectives To investigate the effects of age, period, and cohort on the trends of depression; and to examine the influence of these three temporal effects on residential disparities in depression.Methods Using data from the China Health and Retirement Longitudinal Study (CHARLS) during 2011 to 2020, involving 77,703 respondents aged 45 years old and above. The measurement of depressive symptoms was the score of 10-question version of the Center for Epidemiologic Studies Depression Scale (CES-D 10). The hierarchical age-period-cohort cross-classified random effects models were conducted to examine trends in depressive symptoms related to age, period and cohort.Results CES-D scores increased with age and slightly decreased at older age. The cohort trends mostly increased except for a downward trend among those born in 1950s. As for the period effect, CES-D scores decreased gradually from 2011 to 2013 followed by a upward trend. Rural residents were associated with higher level of depression than those live in urban area. These residence gaps in depression enlarged before the age of 80, and then narrowed. The urban-rural disparities in CES-D scores gradually diminished across cohorts, while the corresponding period-based change in urban-rural gaps was not significant.Conclusion When age, period, cohort factors are considered, the age effects on depression dominated, and the period and cohort variations were relatively small. The residence disparities in depression reduced with successive cohorts, more attention should be paid to the worsening depression condition of younger cohorts in urban areas.

Abstract:This study aimed to uncover the trend of mental health in China over the years and the influence of related social factors. This study employed a cross-temporal meta-analysis (CTMA). The study analyzed 182 studies from 1993 to 2017. A total of 83,603 participants were included in 182 studies. Symptom Checklist-90 (SCL-90) Scale. From 1993 to 2001 and from 2004 to 2017, the scores for each dimension for mental health of older adults were relatively stable, and the absolute value of the effect size was between 0.02 and 0.62. From 2001 to 2003 and from 2003 to 2004, the scores of each dimension of older adults changed greatly. From 2001 to 2003, the total mean score of SCL-90 and the scores of different dimensions in older adults showed a rapidly decreasing trend, with the d ranging from −0.40 to −0.93. From 2003 to 2004, the scores of all dimensions in SCL-90 showed a trend of sharp increase, and the d of each dimension was 0.40 to 0.89. The mental health status of the older adults is declining as a whole and has a large fluctuation from 2001 to 2004 various social development indicators, including Gross Domestic Product (GDP), per capita, divorce rate, unemployment rate, and Engel’s coefficient, demonstrated a significant correlation with mental health status

Abstract:Objectives This study was performed to describe the health status of Chinese oldest-old individuals aged ≥80 years from 1998 to 2014 and explore differences in their health status based on demographic characteristics and socioeconomic status. Methods Data were obtained from the seven waves of the Chinese Longitudinal Healthy Longevity Survey. The Mini Mental State Examination, Katz Index of Independence in Activities of Daily Living, and chair stand test were used to examine cognitive and physical function. Logistic regression was used to evaluate the changes in health outcomes. Results Regression results showed increasing cognitive impairment trends and decreasing disability trends, indicating that the oldest-old individuals had worse body function in more recent years. Older people and women had a weaker health status. Rural elders had better physical performance but worse cognitive function. Participants with an education had better overall functioning. Individuals with severe disability had the shortest survival time among the study population. Conclusions From 1998 to 2014, the health status of oldest-old individuals was relatively stable in China. These findings indicate the need for specific and continuing attention to the oldest-old population to improve their health condition.

Abstract:China has been successful in getting her people to live longer. But, merely adding years to life and not life to years poses immense socio-economic challenges. However, not much is known about the effects of government policy and program interventions on changes in how well the elderly live over the years. Accordingly, we cross-temporally meta-analyzed 45 research reports (N = 36,352) that utilized the health-related quality of life (HRQoL) scale (SF-36) from 2000 to 2020. We found that: (1) the bodily pain, general health, vitality, and mental health of the elderly deteriorated over time; however, their physical and emotional roles, as well as their social functioning, improved with time; (2) the rising dependency ratio impoverished the HRQoL of the elderly; (3) the HRQoL indicators of the elderly revealed positive gains under the home-based care model whilst they showed a downward trend under the institutional pension model; (4) the HRQoL indicators of the elderly in economically developed areas produced mixed results; but, they all worsened over the years in economically underdeveloped areas. Thus, more investment efforts from the government and private entities are needed to reduce the dependency ratio and to improve the lives of the elderly under institutional care and/or in economically underdeveloped areas.

Abstract:BACKGROUND:The oldest-old (those aged ≥80 years) are the most rapidly growing age group globally, and are most in need of health care and assistance. We aimed to assess changes in mortality, disability in activities of daily living, and physical and cognitive functioning among oldest-old individuals between 1998 and 2008.METHODS:We used data from the Chinese Longitudinal Healthy Longevity Study. Three pairs of cohorts aged 80-89 years, 90-99 years, and 100-105 years (in total, 19 528 oldest-old participants) were examined; the two cohorts in each pair were born 10 years apart, with the same age at the time of the assessment in the 1998 and 2008 surveys. Four health outcomes were investigated: annual death rate, Activities of Daily Living (ADL), physical performance in three tests and cognitive function measured by Mini-Mental State Examination (MMSE). We used different tests and multivariate regression analyses to examine the cohort differences.FINDINGS:Controlling for various confounding factors, we noted that annual mortality among oldest-old individuals was substantially reduced between 0·2% and 1·3% in 1998-2008 compared with individuals of the same age born 10 years previously, and that disability according to activities of daily living had significantly reduced annually between 0·8% and 2·8%. However, cognitive impairment in the later cohorts increased annually between 0·7% and 2·2% and objective physical performance capacity (standing up from a chair, picking up a book from the floor, and turning around 360°) decreased anually between 0·4% and 3·8%. We also noted that female mortality was substantially lower than male mortality among the oldest-old, but that women's functional capacities in activities of daily living, cognition, and physical performance were worse than their male counterparts.INTERPRETATION:Advances in medications, lifestyle, and socioeconomics might compress activities of daily living disability, that is, benefits of success, but lifespan extension might expand disability of physical and cognitive functioning as more frail, elderly individuals survive with health problems, that is, costs of success.FUNDING:National Natural Science Foundation of China, National Institute on Aging/National Institutes of Health, United Nations Funds for Population Activities.

Abstract:Background Population aging is an inevitable trend and previous studies have showed the relationship between social health related factors and health-related quality of life (HR-QOL) in the elderly. The objective of this study is to investigate the association of social health status with HR-QOL among community-dwelling elderly in Zhejiang. Methods This cross-sectional study was based on community-dwelling elderly individuals from July 2018 to September 2018 in Zhejiang, China. HR-QOL was measured by the 12-item Short-Form Health Survey (SF-12). Social health status was estimated by the long-form of the Social Health Scale for the Elderly (SHSE-L) and classified into three categories (poor, moderate and good). Multivariable linear regression models were conducted to evaluate the association between social health status and HR-QOL (PCS, MCS and SF-12 total score). Results A total of 2952 elderly participants were included in this study. The mean age was 70.68 ± 7.75 years (mean ± SD); of the eligible participants, more than half (50.4%) were females; the mean scores were 48.10 ± 8.49, 47.70 ± 7.09 and 47.90 ± 5.86 for PCS, MCS and SF-12 total score, separately. Results from the multivariable models showed that social health status was positively related to HR-QOL after adjusting for covariates. Compared with individuals with a poor social health status, those who had a moderate or good social health status were more likely to report better HR-QOL (for moderate social health status: β = 1.90(95%CI: 1.09, 2.71) for PCS, β = 1.78(1.08, 2.48) for MCS, β = 1.84(1.29, 2.39) for SF-12 total score; for good social health status: β = 3.29(2.24, 4.34) for PCS, β = 3.10(2.12, 4.01) for MCS, β = 3.20(2.48, 3.91) for SF-12 total score). Conclusion In our study, we found that social health status was positively associated with HR-QOL among the elderly in Zhejiang. Our findings could provide valuable information for decision-makers to develop interventions to improve the HR-QOL of the elderly.
Trends and Disparities in Quality of Life Among Older Adults From 1998 to 2018 in China: A National Observational Study

Abstract:Objective To investigate 20-year trends and disparities in quality of life among older adults in China from 1998 to 2018. Methods Our study was based on eight representative nationwide health surveys among older adults conducted in China from 1998 to 2018. Quality of life data were obtained from 91,993 individuals aged 65 years or above. All surveys included identical indicators of self-reported quality of life, demographic factors, socioeconomic status, lifestyle habits, and health status. The trends in the standardized prevalence of poor quality of life from 1998 to 2018 were examined by locally weighted scatterplot smoothing regression (LOWESS) analysis. We assessed the prevalence of poor quality of life and its related factors by logistic regression models after adjusting for potential confounders. Results The prevalence of poor quality of life was 38.2% (95% CI : 37.9–38.5%). The trends of poor quality of life showed an inverted “U” shape, that the prevalence increased from 27.8% in 1998 to 43.6% in 2008, and then decreased from 39.2% in 2011 to 32.1% in 2018. Disparities in the prevalence of poor quality of life were exacerbating among participants with low or moderate household income per capita and participants with high household income per capita from 1998 to 2018. After controlling potential confounders, living in rural areas, aged below 80 years, unmarried, living alone, low household income, current smoker, poor dietary diversity, never participating in organized social activities, with chronic diseases, functional disability, poor self-reported health, and unhealthy psychological status were risk factors related with poor quality of life in the multivariate model (all p < 0.05). Conclusion During the past two decades, poor quality of life in Chinese older adults showed an inverted “U” trend from 1998 to 2018 that the prevalence of poor quality of life peaked in 2008 and declined since China's deepening health system reform in 2009. However, disparities in the poor quality of life were exacerbating among participants with different socioeconomic statuses. Strengthening the health system is of great importance in improving the quality of life. More efforts are needed to reduce the disparities in the quality of life among the population at the different socioeconomic levels.

Trend analysis and influencing factors of healthy aging in middle-aged population in China: a longitudinal study based on the China Health and Retirement Longitudinal Study

Abstract:Objectives This study aimed to assess the trends of healthy aging and investigate its determinants in the middle-aged population. Study design This was a longitudinal study. Methods The sample comprised 3043 participants aged 45–59 years from the China Longitudinal Study of Health and Retirement 2011–2018. We plotted the prevalence across four waves and used ordered logistic models to investigate the determinants of cumulative times of healthy aging. Results We enrolled 3043 middle-aged people in our study. The prevalence of healthy aging is 28.2% at baseline but subsequently decreased to 19.72% at wave 4. Active socializing consistently ranked the lowest among the five dimensions. Participants with older age (odds ratio [OR] = 0.95, 95% confidence interval [CI] = 0.94–0.97), low monthly income (OR = 0.82, 95% CI: 0.69–0.97) or lived in urban (OR = 0.81, 95% CI: 0.70–0.94) were less likely to have per time increase in healthy aging. Participants with more than primary school degree (OR = 1.79, 95% CI: 1.31–2.46), high life satisfaction (OR = 2.38, 95% CI: 1.86–3.06), and good self-report health (OR = 1.97, 95% CI: 1.66–2.34) were more likely to have healthy aging. Conclusion The number of middle-aged individuals in China who achieved healthy aging is declining and eventually less than one in five, which was far from ideal. Particular attention should be paid to older, women, urban dwellers, individuals with low income, low life satisfaction or poor self-report health. It is urgent to develop public health policies to improve the health and well-being of the middle-aged population

Two Decades of Change in Living arrangements and Oldest Old Health in China, 1998-2018

Abstract:Abstract Living arrangement has been documented to be influential on older adults’ health. However, there is hardly comprehensive temporal model for understanding the effects of time and cohort on oldest old’s health in various household contexts. This study intended to fill the voids of prior literature by studying Chinese oldest old through analyzing data from eight (1998, 2000, 2002, 2005, 2008, 2011-12, 2014 and 2018) waves of the Chinese Longitudinal Healthy Longevity Survey (CLHLS). The results showed changing patterns of Chinese oldest old’s living arrangements by time period and across birth cohorts. Chinese oldest old were more inclined to live with a spouse or live alone. Living in nursing homes was also chosen by more oldest old in recent years. Younger cohorts were more inclined to live with a spouse, live both with a spouse and adult children/grandchildren or live alone than older birth cohorts. Oldest old who lived with a spouse only, and those living in multigenerational families tended to have better physical and mental health. Respondents who lived in nursing homes and stayed with others showed the worst physical and mental health. Oldest old who lived alone had better physical health but suffered from loneliness and reported the worst subjective wellbeing. Significant period and cohort variations were also found when linking living arrangements to oldest old health. Cohort variations mainly occurred among subgroups living alone, staying in nursing homes and living with others. Period effects were more prominent than cohort effects, especially in times of economic recessions and natural disasters that had massive social impacts. The study emphasized strong period effects and highlighted that special attention should be given to more vulnerable groups to promote their health, including those living alone, living in nursing homes and living with others.

Abstract:: Due to the dramatic social change, Chinese older people may have experienced increasing loneliness in recent decades. Wecomprehensively reviewed 25 studies using cross-temporal meta-analysis which were published from 1995 to 2011 and involved 13,280 old people. Results showed that (1) a positive relationship between loneliness and the year was found with 0.95 SD increased from 1995 to 2011; (2) loneliness scores for married and non-married groups had been increasing significantly,also to male and female groups; (3) the difference was significant betweennon-married and married people, but no difference was found between males and females.
The effect of community environment on the trajectory of depressive symptoms and cohort differences among middle-aged and older Chinese adults

Abstract:Background: Urbanization has changed the living environment of middle-aged and older Chinese adults, but it also brings certain mental pressure to them. Few studies have explored the effect of community environment on the development trajectory and the cohort differences of depressive symptoms in Chinese individuals. Methods: Based on the longitudinal data of the China Health and Retirement Longitudinal Study (CHARLS) from 2011 to 2020, using three-level hierarchical linear growth model, this study examined the effect of community environment on the trajectory of depressive symptoms and cohort differences among middle-aged and older Chinese adults in five cohorts from 1920 to 1929, 1930–1939, 1940–1949, 1950–1959, and 1960–1966. Results: The results of this study showed that middle-aged and older adult individuals who lived in neighborhoods with better community physical and social environments had a protective effect on depressive symptoms. There was a cohort difference in the development of depressive symptoms among middle-aged and older Chinese adults. The baseline depressive symptoms in the later birth cohorts were higher than those in the earliest cohort, and the development rate were also significantly higher. The role of community environment in the development rate of depressive symptoms among middle-aged and older adult individuals varied across the cohort. A better community social environment had a more significant moderating effect on the development rate of depressive symptoms in the early birth cohort (1930–1939), and a better physical environment had a more significant moderating effect on the development rate of depressive symptoms in the 1940–1949 cohort. Conclusion: Under the strategic background of healthy aging in China, the construction and improvement of community environment should become an important part of coping with the realistic challenges of the aging population, such as the expanding scale of depressed population among the middle-aged and older adult individual.

Influences of public health emergency and social isolation on older adults' wellbeing: evidence from a longitudinal study

Abstract:Previous research has identified social isolation as a significant detriment to the wellbeing of older adults. However, studies that consider endogenous issues are scarce. The present paper examines the impact of the recent exogenous shock, the COVID-19 pandemic on the wellbeing of the older adult population using a longitudinal dataset from China for the period 2016-2020. The results of this study indicate that the life satisfaction of Chinese older adults was negatively affected, e particularly in regions where social distancing measures were more strictly enforced. Declines in physical and mental health were found to be attributable to declines in life satisfaction. Those who experienced greater exposure to the pandemic were more likely to suffer from chronic disease, illness, and insomnia, and many found it challenging to complete tasks during the lockdown. Furthermore, heterogeneity estimation shows that these effects are stronger among the rural older adult, females, those without a spouse, and those with less education.

Abstract:Purpose:Psychosocial factors have been found to profoundly impact mental health of older adults, but the main focus in the current literature has been on one particular aspect of these factors. This study aimed to identify latent classes of older adults based on four psychosocial factors (loneliness, social isolation, perceived social support, and social capital) and the transition of classes over 6 months. We also sought to assess the predictive role of changes in these classes in relation to depression, anxiety, and stress at 18-month follow-up. Methods:We analyzed longitudinal data from 581 community-dwelling older adults in Shanghai, China. The data were collected at baseline (T0), 6-month follow-up (T1) and 18-month follow-up (T2) between March 2021 and April 2023. Using latent class analysis, we identified three underlying classes (Social Connectors, Subjective Social Isolates, and Social Isolates) of the sample. We also established five transition categories from T0 to T1 (Social Connectors T0-T1, Subjective Social Isolates T0-T1, Social Isolates T0-T1, Good Transition, and Bad Transition) using latent transition analysis. Logistic regression was employed to examine the temporal relationships between these transition categories and subsequent symptoms of depression, anxiety and stress, adjusting for age, sex, education, marital status, family income level, sleep quality, health status and outcome variables at T0. Results:Multivariable associations revealed that compared to older adults with persistent good social environment (Social Connectors T0-T1), those with persistent high levels of loneliness and social isolation and low levels of perceived social support and social capital (Social Isolates T0-T1), and those who shifted towards a poorer social environment (Bad Transition) were more likely to experience depression, anxiety and stress at T2. Sustained subjective social isolation (Subjective Social Isolates T0-T1) was associated with more severe depressive symptoms at T2. Conclusion:Our study indicated that adverse psychosocial environment worsened mental health in older adults. These findings highlight the importance of early identification of older individuals at long-term psychosocial risk and development of tailored interventions to improve their social environment and mental health.
