Abstract:BackgroundThe structure of the fascia in upper mediastinum has already been reported from gross anatomical viewpoints by Sarrazin. But it is necessary to understand meticulous anatomy for thoracoscopic or mediastinoscopic surgery. So herein, we investigate histologically the thin membranous structure made of dense connective tissues.MethodsSemi-sequential transverse sections of the mediastinum were obtained from three cadavers. Hematoxylin and eosin staining, Elastica van Gieson staining, and Masson trichrome staining were performed to identify the presence and location of the thin membranous structure made of dense connective tissues.ResultsThe “visceral sheath” and “vascular sheath,” as previously described by Sarrazin, were observed histologically. These two thin membranous structures do not surround the esophagus and trachea cylindrically. In addition, the “visceral sheath” on the right side of the upper mediastinum was unclear in comparison to the left side. The “visceral sheath” (on the left side) gradually became unclear, and seemed to almost disappear; the esophagus was found to be very close to the thoracic duct on the caudal side of the bifurcation of the trachea. Although the left recurrent nerve was located inside the “visceral sheath” in all cadavers, the left recurrent nerve lymph nodes were located inside the “visceral sheath” in cadaver 1 and between the “visceral sheath” and “vascular sheath” in cadaver 3.ConclusionThe “visceral sheath” around the esophagus in the upper mediastinum was histologically demonstrated; however, the findings were not constant.

Abstract:The aim of this study was to find basic rules governing the fetal anatomy of the deep cervical fasciae and their connections to the mediastinal fasciae. We examined the histology of paraffin-embedded preparations of 18 mid-term fetuses (5 between 9 and 12 weeks of gestation, 3 between 15 and 18 weeks, and 10 between 20 and 25 weeks). The prevertebral lamina of the deep cervical fasciae (PLDCF) developed as an intermediate aponeurosis for the bilateral bellies of the longus colli muscles. In contrast, the alar fascia developed as a connecting band between the bilateral adventitiae of the common carotid artery. The retropharyngeal fascia became evident much later than the latter two fasciae. The fascia covering the thymus was thicker than the fascia for the strap muscles (the pretracheal lamina of the cervical fascia). The primitive suprapleural membrane, or Sibson's fascia, contained veins and fatty tissues, and was composed of the alar fascia rather than the PLDCF, tranversalis fascia, or endothoracic fascia. The prevertebral two-laminar configuration was rather evident in the early stages of development because, in the later stages, the fasciae together provided a multilaminar structure, especially in the lateral area in front of the longus colli, which suspended the cupula pleurae. To consider a continuation from the base of the neck to the upper mediastinum, the alar fascia seems to be a key structure for connecting the vascular sheath to the parietal pleura.

Abstract:Background: The incidence of aberrant thymus in the posterior mediastinum is very uncommon. It is difficult to exclude malignancy before surgical procedure.Case Presentation: In a six-month-old male coughing for two weeks prior to admission a posterior mediastinal mass was found incidentally by chest roentgenogram. Thoracotomy was performed. Histologic study revealed normal thymic tissue.Conclusion; When a mass located in the posterior mediastinum, ectopic thymus should be included in differential diagnosis. Imaging techniques may spare thoracotomy. Ectopic thymus has a benign clinical course, and surgical resection is not recommended.

Abstract:The radiographic appearance of retrothymic pneumomediastinum is quite specific. The findings include elevation of the thymus away from other mediastinal structures with increased lucency beneath it, visualization of a radiodense line extending from the inferior pole of the thymus to the midportion of the heart, and tenting of the pericardium at the point of attachment of this line. This constellation of findings will aid in the differential diagnosis of medial anterior pneumothorax, pneumopericardium, and intrapulmonary cyst in the infant with air block. The paper details the mediastinal anatomy with special emphasis on a previously undescribed layer of connective tissue that accounts for this specific radiographic appearance.

Abstract:The superficial musculoaponeurotic system (SMAS) was advocated by Mitz and Peyronie in 1976. The concept of this superficial fascia was established by surgical findings of facelift surgery and is familiar to plastic surgeons and anatomists. However, detailed characteristics of this fascia are still not widely known among head and neck surgeons. Moreover, the SMAS is generally located at the parotid and cheek regions and divides facial fat into superficial and deep layers. The SMAS connects to the superficial temporal fascia cranially and to the platysma caudally. The frontal muscle and the peripheral part of the orbicularis oculi are also in the same plane. The exact expanse of the SMAS in the face is controversial. Some authors claimed that the SMAS exists in the upper lip, whereas others denied the continuity of the SMAS to the superficial temporal fascia in a histological study. There are various other opinions regarding SMAS aside from those mentioned above. The concept of the SMAS is very important for facial soft tissue surgeries because the SMAS is a good surgical landmark to avoid facial nerve injuries. Therefore, this article summarized SMAS from an anatomical point of view.

Abstract:<section class="article-section__content"><h3 class="article-section__sub-title section1"> Introduction</h3><p>The alar fascia remains one of the most variably described fascial structure in the human body. Much disagreement persists in the literature and mainstream anatomical texts about its anatomy, function, and clinical significance. It is generally described as a coronally‐oriented fascial sheet separating the retropharyngeal space anteriorly from the danger space posteriorly. The current study aimed to confirm the presence of the alar fascia and delineate its anatomical characteristics, connections, and potential function through gross dissection and microscopic analysis. Possible clinical and surgical implications are considered.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Methods</h3><p>Twelve (12) cadaveric necks were dissected and examined histologically. Smooth muscle (αSMA), nerves (S100 protein), and myosin protein were identified immunohistologically to characterize the composition and possible functions of the alar fascia.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Results</h3><p>The alar fascia was found in all specimens spanning between the carotid sheaths. Morphologically, it was not a delamination or derivative of the prevertebral fascia. It extended from the base of the skull to the upper thoracic level (T2) where it fused with the visceral fascia. No midsagittal connection was found between the alar and visceral fasciae. Immunohistochemically, the alar fascia was positive in focal areas for αSMA and S100 proteins but negative for fast and slow myosin.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Conclusion</h3><p>The alar fascia is an independent and constant coronal fascial layer between the carotid sheaths. It contains neurovasculature and may limit the spread of retropharyngeal infections into the thorax as well as facilitate normal physiological functions of the cervical viscera.</p><p>This article is protected by copyright. Anatomy behind the Paramedian Platysmal Band: A Combined Cadaveric and Computed Tomographic Study

Abstract:Background: The muscular hypothesis explanation of the pathophysiology behind paramedian platysmal bands does not seem to provide a sufficient explanation for the clinical presentation of these platysmal bands in aged individuals with cervical soft-tissue laxity. The purpose of this study was to investigate the fascial relationships of the anterior neck to enhance appropriate surgical treatment. Methods: Retrospective analysis of computed tomographic scans of 50 Caucasian individuals (mean age, 55.84 ± 17.5 years) and anatomical dissections of 20 Caucasian and 10 Asian body donors (mean age, 75.88 ± 10.6 years) were conducted. Fascial adhesions were classified according to platysma fusion types, and platysma mobility was tested during dissection procedures. Results: Fusion between the left and right platysma muscles occurred in 64 percent in the suprahyoid region and in 20 percent at the thyroid cartilage, and in 16 percent, the platysma attached to the mandible without fusion. In the absence of muscular fibers, a fascial adhesion zone with limited mobility was observed where the superficial cervical fascia fused with the investing layer of the deep cervical fascia. Conclusions: Muscular contraction of the platysma results in elevation of the most medial platysma muscle fibers, which are not attached in the fascial adhesion zone. The presence of a fatty layer deep to the platysma enables platysmal movement and anterior and inferior gliding of the skin and platysma when cervical soft-tissue laxity exists. Surgical treatments should include transection of the fascial adhesion; this could potentially prevent the recurrence of paramedian platysmal bands.
Fascial Release of the Pectoralis Major: A Technique Used in Pectoralis Major Muscle Closure of the Mediastinum in Cases of Mediastinitis

Abstract:BACKGROUND: The authors' goal was to demonstrate a technique of gaining added width from the right pectoralis major muscle flap through muscle fascia release, with the hope of attaining sturdier, tension-free dead space closure of the debrided mediastinum in cases of mediastinitis.METHODS: The authors measured the preincisional and postincisional widths of the right pectoralis major flap following release of the fascia in nine patients who underwent mediastinal reconstruction using pectoralis major muscle flaps from 2002 to 2004 at the Detroit Medical Center.RESULTS: The average width of the nine muscles was 16.3 +/- 1.7 cm before fascia release and 22.1 +/- 1.3 cm after release. The average increase in width was 5.8 +/- 1.3 cm, with a 95 percent confidence interval of 4.8 to 6.8 cm. A value of p < 0.0001 indicated that this increase in width was statistically significant. After fascial release, the width of the muscles increased by an average of 26.1 percent, with a 95 percent confidence interval of 21.6 to 30.6 percent.CONCLUSIONS: The authors present a new technique that can be used to gain additional width from the right pectoralis major muscle in cases of mediastinal reconstruction using the pectoralis major muscle flap. With the added 26 percent of width obtained from the right pectoralis major muscle with fascia release, the authors contend that a sturdier and tension-free closure of the mediastinal dead space can be obtained, further expanding the indications for use of the pectoralis major muscle flap as the sole closure modality in even the most complicated cases of mediastinitis.

Abstract:According to current scientific standards, the fascia is a connective tissue derived from two separate germ layers, the mesoderm (trunk and limbs, part of the neck) and the ectoderm (cervical tract and skull). The fascia has the property of maintaining the shape and function of its anatomical district, but it also can adapt to mechanical-metabolic stimuli. Smooth muscle and non-voluntary striated musculature originated from the mesoderm have never been properly considered as a type of fascia. They are some of the viscera present in the mediastinum, in the abdomen and in the pelvic floor. This text represents the first article in the international scientific field that discusses the inclusion of some viscera in the context of what is considered fascia, thanks to the efforts of our committee for the definition and nomenclature of the fascial tissue of the Foundation of Osteopathic Research and Clinical Endorsement (FORCE).

Abstract:All fascia between the transversalis fascia of anterior abdominal wall and the peritoneum can be called the preperitoneal fascia clinically.According to the interfascial plane theory,the preperitoneal fascia is a multi-layer structure,and preperitoneal space was divided into two surgical planes including parietal and visceral by its membrane layers.Surgical planes of totally extraperitoneal herniorraphy (TEP) can be described as follow:for median region,surgery can be performed in parietal plane,that is avascular region filled with loose areolar tissues between the transversalis fascia and the preperitoneal fascia,which is called "holy plane" of TEP.For Doom region,surgery is transferred into viscecal plane,which is between the preperitoneal fascia and the peritoneum,due to the particularity of spermatic cord through the abdominal wall.For lateral region,the abdominal wall restores to normal structure,and surgery can be performed in parietal plane or viscecal plane.The preperitoneal fascia needs to be dissected during the transfer from parietal plane to viscecal plane.The concept of plane transformation can explain the formation mechanism of preperitoneal loop and no connection between Reztius space and Bogros space.The author has elucidated the significance of preperitoneal fascia1 anatomy of anterior abdominal wall in TEP based on clinical experience of TEP and authoritative literatures.
Applied Anatomy of the Trans-Upper-sternal Approach to the Cervicothoracic Junction

Abstract:Objective:To provide anatomic data for the trans-upper-sternal approach to the cervicothoracic junction. Methods:40 cadaveric specimens were dissected, according to the operative procedure of the trans-upper-sternal approach to the cervicothoracic junction, and then related structures were observed and measured. Results:The rate of absence of the unilateral middle thyroid vein was 48.8%. After splitting the sternum, the diameter between segments of the sternum was about 60.8±1.64 mm. The middle part of the anterior border of bilateral pleurae can be described as separated, closed, contacted and overlapped types respectively. 50.4% specimens were last two types. The distance between the left border of the esophagus and the trachea was 0.5±0.08 cm. Thoracic duct located in the triangle region of jugular vein angle all the way from the behind of carotid sheath to the point draining into the vein. For 94.5% and 59.6% of specimens, the left and right recurrent laryngeal nerve located in the esophagotracheal groove respectively and formed a complicated relationship with the inferior thyroid artery. The sternal angle located at the level of T4/5 disc, the lower 1/3 of T4 and the upper 1/3 of the T5 level (62.5%). The recurrent point of the left recurrent laryngeal nerve located at T3/4 disc, T4 level (75.6%), while the right at T1/2 disc, T2 level (82.0%). The upper border of the aortic arch located at T3 level (73.7%), and the upper border of the right subclavian artery at T1 (70.0%) level. The cephalic and caudal border of the surgical area concerning with trans-upper-sternal approach was at C3/4 disc, C4 (75.6%) level and T3, T3/4 disc level (86.8%) respectively. Conclusions:The trans-upper-sternal approach to the cervicothoracic junction is valuable for its fully exposure and easy operation during the operation.
[Fascial anatomy of ligamentous structures associated with colon cancer surgery]

Abstract:The ligamentous structures integral to the surgical management of colon cancer include the gastrocolic ligament, the phrenicocolic ligament, and the splenocolic ligament. Historically, the era of conventional open surgery was characterized by the use of large forceps for clamping and ligating these ligaments. However, the advent of fascial and mesenteric anatomy research has ushered in a paradigm shift. Aided by high-definition laparoscopy, colorectal surgeons have progressively clarified the fundamental anatomical structures, thereby refining surgical techniques in accordance with fascial and mesenteric anatomical principles. This study synthesizes the author's anatomical research findings to dissect the fascial and mesenteric anatomy of the ligaments pertinent to colon cancer surgery, thereby exploring their implications for surgical practice and oncological outcomes. The gastrocolic ligament exhibits distinct fascial and mesenteric anatomical configurations within the omental sac and extra-omental regions. Within the omental sac, the sub-omental arch pathway emerges as a viable alternative to the paracolic approach for accessing the omental sac through the gastrocolic ligament. Conversely, in the extra-omental region, the incision of the greater omentum overlaying the space between the mesogastrium and the transverse mesocolon represents a mesenteric bridge facilitating access to this area. The incidence of nodal metastasis in the gastrocolic ligament associated with transverse colon and hepatic flexure colon cancer is notably low; nevertheless, selective dissection in high-risk patients can still provide survival benefits. The splenocolic ligament is formed by the convergence of the splenic hilum region of the mesogastrium (including the pancreatic mesentery) with the mesocolon of the splenic flexure of the colon. A natural avascular plane exists within it, and dissection along this plane can avoid encountering the branches of the left gastroepiploic artery that are typically encountered in traditional dissection routes. To date, there is no compelling evidence advocating for the resection of the splenic hilum region of the mesogastrium or the lymph nodes of the gastrocolic ligament in the context of splenic flexure colon cancer.

Abstract:Objective The purpose of this study was to understand further into the composition of cardinal ligament and its relationship with pelvic extra-peritoneal spaces from the surgical anatomy point of view and to provide a new precise skeletal dissection model for radical hysterectomy (RH).Methods 10 fixed Chinese female cadavers from Education and Research Office of Anatomy in Cangzhou Medical Specialized School were involved in this study.The ligamentous structure of cardinal ligament and pelvic spaces were examined and recorded carefully.The anatomic landmarks of cardinal ligament associated with Piver classification of RH was analyzed.Results (1)The cardinal ligament was a double-layered fascia surrounding mesentery-like structure between the paravesical space and the pararectal space.In the sheath, the core was composed of the blood vessels from internal iliac artery and vein and pelvic splanchnic nerve from S3.The deep uterine vein was the borderline vessel between the blood part and the nerve part.(2)The mean width of left cardinal ligament weas (5.31±0.74)cm and the right was (5.48±0.68) cm.The means of caudal depth of cardinal ligament were (5.76±0.55)cm (left) and (5.51±0.39)cm (right) respectively.The means of cranial depth of cardinal ligament were (4.93±0.41)cm (left) and (4.81±0.48)cm (right) respectively.The pararectal space was shallower than the paravesical space and with much more blood vessels in its bottom.(3)The blood vessels in the cardinal ligament varied largely and the basic vessels were superior vesical artery, middle vesical artery, uterine artery, superficial uterine vein, deep uterine vein, middle rectal vessel and vaginal vein was the most important vessel for surgery.(4)The anatomic landmarks for Piver classification in the cardinal ligament were uterine artery different points (medial to ureter, originating point from internal iliac artery), superior vesical artery and lateral umbilical artery.Conclusion The cardinal ligament is mesentery-like structure with abundant blood vessels and nerve in it located in pelvic extra-peritoneal space.A new understanding about cardinal ligament in radical hysterectomy would lead to a new model of RH--from en bloc resection to precise skeletal dissection.

Abstract:The study aim was to explore the anatomy, histopathology, and molecular biological function of the fascias posterior to the interperitoneal colon and its mesocolon to provide information for improving complete mesocolic excision. To accomplish this aim, we performed intraoperative observations in 60 interperitoneal colon-cancer patients accepted for complete mesocolic excision and conducted local anatomy observations for five embalmed cadavers. An additional two embalmed child cadaver specimens were studied with large slices and paraffin sections. Ten of the 60 patients were examined with a lymph node tracer technique in vivo, while fresh specimens from these patients were assessed by histopathological examination and transwell cell migration assays in vitro. The anatomical and histopathological findings showed that the fascias posterior to the interperitoneal colon and its associated mesocolon were composed of two independent layers: the visceral and parietal fascias. These two fascias were primarily composed of collagen fibers, with the parietal fascia containing a small amount of muscle fiber. The in vivo test showed that the visceral fascia surrounded the colon and its associated mesocolon, including vessels and lymphatics, and that it had no lymphatic flow through it into the rear tissues. Moreover, the in vitro assays showed the visceral fascia was able to block tumor cell migration. Although many surgical scholars have known of the existence of fascia tissue posterior to the intraperitoneal colon, the detailed structure has been ignored and been unclear. As shown by our findings, the visceral and parietal fascias are truly formed structures that have not been previously reported. A thorough understanding of fascial structures and the function of the visceral fascia barrier in blocking tumor cells will facilitate surgeons when performing high-quality complete mesocolic excision procedures.

Abstract:This review provides an overview of comprehensive knowledge regarding the anatomy of the fasciae and fascial spaces of the maxillofacial and the anterior neck regions, principally from the standpoint of oral surgery, whose descriptions have long been puzzling and descriptively much too complex. The maxillofacial and the anterior neck regions are divided into four portions: the portions superficial and deep to the superficial layer of the deep cervical fascia (SfDCF) including its rostral extension to the face, the intermediate portion sandwiched by the splitting SfDCF, and the superficial portion peculiar to the face where the deep structures open on the body surface to form the oral cavity. Different fascial spaces are contained in each of the portions, although the spaces belonging to the portion of the same depth communicate freely with each other. The spaces of the superficial portions are adjacent to the oral cavity and constitute the starting point of deep infections from that cavity. The spaces of the intermediate portion lie around the mandible and occupy the position connecting the superficial and deep portions. Among these spaces, the submandibular and prestyloid spaces play an important role as relay stations conveying the infections into the deep portion. The spaces of the deep portion lie near the cervical viscera and communicate inferiorly with the superior mediastinum, among which the poststyloid space plays a role as a reception center of the infections and conveys the infections into the superior mediastinum particularly by way of the retrovisceral space and the carotid sheath.

Sternalis featuring tendomuscular fusion to the contralateral sternocleidomastoid: a cadaveric case report

Abstract:Purpose: To report the morphologic and spatial relationships of a bilateral sternalis muscle variant. Methods: Routine cadaveric dissection in an undergraduate anatomy laboratory revealed two sternalis muscles parasternal to the sternal body. Subsequent fine prosection of the anterior thoracic wall and neck was carried out to uncover the soft tissue attachments of both sternalis muscles. Positional relationship to neighboring anterior thoracic and neck structures, and geometric dimensions of the muscle bellies and tendons, were documented. Results: Both sternalis muscles were imbedded in the pectoral fascia, with their muscle fibers running obliquely to the midsternal line. The right sternalis muscle was notably larger in length, width, and thickness compared to the sternalis muscle on the left. The right sternalis muscle featured a distinct superior cord-like intermediate tendon that traversed superolateral and fused directly with the contralateral sternomastoid and sterno-occipital portions of the left sternocleidomastoid muscle. The superior tendon of the left sternalis muscle was aponeurotic and affixed to the cord-tendon of the right sternalis muscle. A distinct tendinous entheses for the sternal head for both sternocleidomastoid muscles were noted at the manubrial body. Conclusions: This case report describes a unique bilateral sternalis muscle variant with conjoined sternocleidomastoid muscle integration. This anatomical description of a sternalis-sternocleidomastoid muscle morphology may supplement radiographic interpretations and support diagnostic accuracy.
What is the identity of Gerota fascia? Histological study with cadavers

Abstract:Objectives: The advancement of laparoscopic surgery has allowed surgeons to see finer anatomical structures during surgery. As a result, several issues have arisen regarding Gerota fascia that cannot be explained by previous interpretations, such as its various forms observed during surgery. To address these issues, we histologically examined the structure of Gerota fascia. Methods: Specimens for study were prepared from kidneys with Gerota fascia from four cadavers, and the structure was studied histologically. Its thickness and collagen fiber area ratios were measured using ImageJ and compared to those of the epimysium of the rectus abdominis muscle. Results: Connective tissue that appeared to be Gerota fascia was observed in 26 specimens. Histologically, the basic structure of Gerota fascia was a sandwich-like structure with a thin layer of thick, long collagen fibers in the central layer, and small granular collagen fibers scattered at the edges. However, not all areas observed had a similar structure; eight specimens were composed only of small granular collagen fibers. The average thickness of the Gerota fascia was 466 μm, and the area ratio of collagen was 27.1%. In contrast, the epimysium was much thicker than Gerota fascia, and its collagen fibers were much thicker and denser. Conclusions: Gerota fascia, unlike the epimysium, was a very thin and fragile layer of collagen fibers, and its structure was diverse. This explains why Gerota fascia was observed in various states during surgery. It is important for surgeons to understand the properties of Gerota fascia and to treat it appropriately.

Abstract:The pleura are a composition of two serous membranes: the visceral pleura lining the lungs and the parietal pleura lining the inner chest wall, diaphragm, and mediastinum. The pulmonary hilum is composed of blood vessels, airways, nerves, and lymph nodes and denotes the meeting point between the mediastinum and the pleural cavities. This review covers disorders of the pleura, mediastinum, and hilum. Figures show a schematic diagram of normal filtration/resorption of fluid in the pleural space; comparison of a simultaneously obtained chest x-ray (CXR) and a chest computed tomographic (CT) scan of the same patient; an algorithm for evaluation of patients with pleural effusion (PE); a three-compartment model of mediastinal anatomy; photographs of a resected, well-encapsulated thymoma and a benign, multiloculated thymic cyst that were completely removed by sternotomy, a resected esophageal duplication cyst, and an esophageal leiomyoma being removed by means of a right thoracotomy; CT scans demonstrating the characteristic appearance of an invasive thymoma, an extragonadal germ cell tumor, a primary mediastinal B cell lymphoma, a goiter with extension behind the trachea, right paratracheal adenopathy in the middle mediastinal compartment, a middle mediastinal cystic mass, a large subcarinal bronchogenic cyst, a tracheal chondrosarcoma, gas in the mediastinum due to esophageal perforation, and mediastinal fibrosis; coronal imaging showing a large, smooth muscle tumor associated with the distal esophagus; a barium swallow showing a large esophageal perforation that resulted in soilage of the middle mediastinum; and posteroanterior and lateral CXRs of a posterior mediastinal neurogenic tumor accompanied by a CT scan showing the posterior mediastinal neurogenic tumor visualized in the posteroanterior radiograph. Abstract:Hernia and abdominal wall surgery has entered an era of membrane anatomy which emphasizes anatomy level and tissue embryos.According to the membrane anatomy of abdominal fascia,such as "onion skin" or "ring over ring",the fascia structure of groin area was analyzed via embryology,holistic view of anterior and posterior abdominal wall,and laparoscopic and open perspective.Attempt to clarify the anatomical terms,such as transversalis fascia,extraperitoneal fascia,conjoint fascia,so as to unify the understanding of groin anatomy,to facilitate distinguish the depth of dissection,to avoid iatrogenic organs injury,to shorten the learning curve,and to improve the homogenization level of sugery.
