Abstract:The author reconsiders Freud's "The Sexual Aberrations," the first of his Three Essays on the Theory of Sexuality (1905), in light of contemporary psychoanalytic theory. Are the concepts of sexual aberration and norm still viable? The author argues that they are necessary but insufficient elements in current theory. He then presents a competing model in which sexuality can be reduced to a more elemental level of disturbance and wish, where it is an expression of a nonsexual wish--for example, to possess or control the object to eliminate separateness. The author presents clinical material to demonstrate this alternative model.
[Phenomenology and history of the origin of exhibitionism]

Abstract:Exhibitionism should not be misunderstood as a throw back into phylogenetically highly primitive "sexual advertising". The issue is also missed if one shares the widespread belief that at least some of the cases could be explained by an abnormally increased sexual drive. This sexual deviation mostly appears for the first time at the turn of the second to the third decade of life. The relatively close association with age is not conspicuous when we take a look at first police reports or at the sum total of culpable criminal offences. Only about one-tenth of the cases concern late delinquency beyond 35 or 40 years of age. More than 75% of the adult delinquents had married. Marriage practically never implied a turning-point within the sexual maldevelopment, perhaps also largely due to the specific choice of the spouse. It is more likely that the original conflicts are even aggravated as a result of the confrontation with the marriage partner. There is a sharp contrast between the general tendency on the part of the exhibitionists to follow a set behavioural pattern which is stereotyped and rather mechanical, on the one hand, and the wide variety of "victims" selected by many of these delinquents on the other. There is also a noticeable affinity to theft and misappropriation of property--a criminologically interesting aspect. Among the factors contributing to a lack of self-concept and deficient feeling of male identity, the following have been determined to act in combination with other negative influences: The delinquents grow up in a large family comprising many brothers and sisters, a highly significant factor being a predominance of boys among the siblings; often, there is no positive male reference person as would be represented by the father; or the patient is himself an illegitimate offspring. There may often be a physical malformation or handicap acting as a kind of social stigma. This can result in amplifying the subjective feeling of rejection or non-acceptance. Conflicts of the type of a dysmorphophobia were in far more than one-third of the examined 40 patients contributory to the prerequisites of sexual anomaly. Lack of confidence in one's own capacities is one of the main factors in preventing the delinquents from being professionally as successful as their fathers--which would offer a chance of raising their self-esteem. Self-limitation because of strong aggressive inhibition seems to be the main reason for the albeit slight deterioration in status-determined behaviour and hence social status.(ABSTRACT TRUNCATED AT 400 WORDS)

Abstract:Freud was an astute observer, but his understanding of female psychology was distorted by centration on biological forces, the adult neurosis, libido theory, the Oedipus complex, and male psychology. His views on the role of penis envy, narcissism, and masochism in women, and on the female superego, currently are being updated on the basis of new data emerging out of psychoanalytic investigation and direct observational studies. It has become clear, for example, that core feminine gender identity develops very early and that it is mediated in large part by cognitive development and learning rather than by the impact of the observation of genital differences between the sexes. The little girl's level of cognitive organization does not permit her at first to construct logical, consistent categories of female and male, predicated or coordination of criteria of inclusion and exclusion. She puts girls and women into one group out of intuitive recognition that they "go together" and builds up a view of herself as female on the basis of what she feels as her mother's daughter and what she perceives of the way her parents and other significant object see her. Her mother's feelings about her, about herself, and about females in general influence her greatly, and her father's responses to and expectations of her have an important impact on her self-image and self-esteem. Her representation of herself as female at first does not necessarily take genital differences between the sexes into account. The discovery, some time in the second or third year, that the genitals of boys and girls are different from one another presents the girl with a number of problems, however. Her cognitive organization makes it extremely difficult for her to apprehend her complex, incompletely demarcated, and to a large extent internal, unobservable, and impalpable sexual organs. Her initial conclusion is that she lacks desirable body parts which are possessed by other beings. Because of the narcissistic-exhibitionistic libidinal organization of that period of life, this represents a painful narcissistic injury. The intensity of the girl's castration reaction and the degree to which she will be able to overcome it in the course of further development are determined by multiple internal and external developmental factors. Masochism, narcissistic vulnerability, and penis envy are neither limited to the female sex nor normal outcomes of female development. Where they are prominent features in the psychopathology of female patients, it is insufficient to interpret them as emanating from penis envy as a bedrock, primal cause. Detailed clinical examples are presented to illustrate the thesis that through exploration and explication of the multiple sources of such symptomatology in a female analysand is the most effective way of relieving the symptomatology and releasing the potential for emotional growth.

Abstract:A deficit or problematic achievement of female orgasm is often classified as a sexual disorder that creates complications in the sex life of couples. This assumption is generally accepted, even though vaginal anorgasmia is an accepted statistical norm and non-coital methods of generating female orgasm are not as easy as they are for men. Female orgasms manifest themselves in different ways; they are variable and can be dependent on a number of variable factors. Some theories suggest a high degree of correlation between the capacity for orgasmic experience, sexual attitudes and behaviour, but also with reproductive potential or the stability of the given couple's relationship. Female orgasm is often seen as a discriminatory mechanism influencing attitudes towards sexuality or even as a kind of fertility catalyst. There is no consensus on the importance of female orgasm. The results of some relevant studies refute theories about the female orgasm's positive influence on adaptive functions of the couple's relationship, as well as its influence on fertility. The orgasm in women is most likely an evolutionary by-product of its male variant, since the clitoris and penis have an identical embryonic basis of development. Female anorgasmia should not be considered an unquestioned diagnosis, let alone a psychiatric construct leading to a paradigm in which anorgasmia is categorically the cause of frustration or other problems.

Abstract:In this article, we discuss a function of the “perversion” which, although there are in the basic concepts of psychoanalysis, from Freud to Lacan, but in the continues of article it “perverse” from that path to expand and develop from within and articulate itself with some concepts from other realms and look for an alternative approach to explain and intervene in the theoretical and practical world. This approach is a montage of the concepts of psychoanalysis, philosophy, politics and sociology, that the main problematic of which is obtain a critical understanding of our current social situation in the context of the transformations of contemporary history and finally to think about the “politics of the future”. For this reason, wherever we talk about “perversion”, we should absolutely not rely on its pathological meaning, but should see it as a pure form and process. With this perspective, we will first discuss Freud’s formulation of fetishism and we trace it in Lacan’s works and the distinction he makes between desire and drive. In the following, we try to keep going on our argument by a pluralistic point of view, in accordance with the plurality of drives, instead of a unitarian point of view to man and society. For this purpose, we go beyond psychoanalysis and discuss the concept of the drive according to Leibniz and Nietzsche, as well as the concept of “Becoming-Woman” by Deleuze and Guattari and we fold it again on the concept of “feminine jouissance’s Lacan to measure that’s relationship with queer feminism and its social and political implications. The central question of this article is what effect does the decline of patriarchal functions and Oedipal structures in the late modernity have on the dynamism of desire and drive and various social and political morphogenesis based on it? The political claim of this article is that theology, patriarchy, and capitalism are historically integrated in the nation-state form and then by looking at the experience of the revolutionary movement of “woman, life, freedom” in Iran, we seek to provide a new analytical and discursive framework to think about anti-authority social movements which seeks to establish a horizontal and egalitarian social geometry instead of the vertical and oppressive ruling geometry.

Abstract:The universal phenomenon of climacterium in women has received little attention from psychiatrists, except in the case of extreme emotional reactions, the “involutional psychoses”. This study was intended to explore the responses of a broad range of women to the changes of climacterium. As a pilot study, it was guided only by the general expectation that a woman's response to menopause would be influenced by her response to earlier psychosexual events. Fifty-five women of diverse ethnic origins participated in semistructured psychiatric interviews. The interview focused on the subject's attitude toward her femininity, her psychosexual history, menopause, and toward the family and social problems associated with this age.Response to menopause was coded on a three-point ordinal scale, ranging from “mixed-positive” to “mixed-negative”. The association of 11 independent variables to the response to menopause was tested for the population as a whole and controlling for ethnicity. Of the 11 independent variables, only one was associated with a positive response to menopause: a lack of desire for additional children, among the Oriental-Arab group. Categories in 9 of the 11 independent variables tend to be associated with a negative attitude toward menopause; but the pattern of association does not support our general expectation that a history of successful response to earlier psychosexual experiences is predictive of a positive response to menopause. Our findings, though drawn from a small sample, indicate that this relationship is more complex than is generally suggested.

Narcissism and female gender identity: a reformulation

Abstract:The theory of narcissism and the study of the psychological development of the female child have both been subjects of considerable controversy within psychoanalysis. Some issues central to these controversies have been discussed and evaluated with attention to the contributions made by advances in each area upon the other. From a historical perspective, Freud's illuminating introduction of the concept of narcissism opened the door toward explaining a variety of puzzling clinical phenomena. Deutsch then elaborated upon Freud's contribution as well as upon his specific bias in her description of the psychology of women. Later, Jacobson addressed herself to a reformulation of then existing views of female psychological development. Kohut's innovative clinical observations and Stolorow's functional definition of narcissism provided a unique vantage point from which to offer a critical restatement of narcissistic factors in the development of the female child. Specifically, narcissism was freed from its singular relationship to female development (the emphasis on castration shock and penis envy as rock bottom factors in female character formation) and related to structuralization of the representational world--self- and object-representations. Sexual differences can then be understood as most relevant for the content of these structures. In this context, the work of Stoller was cited. Finally, three topics that grew out of these considerations and elaborations were considered: the role of the little girl's attachment to and separation from her mother, the role of the psychosexual phases and an evaluation of penis envy, and the role of the father as an ideal. The reciprocal relationship between psychosexual development and the differentiation, integration and consolidation of self and object images has specific relevance for the manner in which femininity as the content of the self-representation of the developing girl enhances and is enhanced by the structuralization of the representations. The unfolding of the psychosexual phases with their associated imagery can be viewed as the gradual unfolding of more complex, differentiated and articulated self and object representations. Conversely, the gradual separation, integration and consolidation of self- and object-representations makes possible the development of increasingly more advanced forms of psychosexual experiences. Thus, the original question of the relationship between narcissism and female development has led to much broader issues. Narcissism concerns the structuralization and maintenance of the self. However, while narcissistic activities may promote structuralization, they may also serve to shore up a precarious sense of gender identity.

Abstract:OBJECTIVE To explore the feasibility of endoscopic surgery for primary trigeminal neuralgia, and to evaluate its advantages and disadvantages. METHODS Fifteen patients diagnosed as primary trigeminal neuralgia were included in this study. All of them had maxillary neuralgia, concurrently with 8 ophthalmic neuralgia and 2 mandibular neuralgia. The median course of disease was 4 years. The surgeries were performed by transnasal endoscope, through sphenopalatine foramen, into the pterygopalatine fossa, to find rotundum foramen, and then coagulated and cut the maxillary nerve. Post-operative evaluation was done based on Brisman R' s way. The post-operative improvement of symptom was compared with preoperative symptom, and the complications of this operation were observed. RESULTS The follow-up time was 6 months to 16 months, with the median time of 13 months. Thirteen patients were cured, 2 patients had effective outcome. Seven months after operation, 1 patient appeared supraorbital neuralgia. After the radiofrequency operation, the pain was improved. All of the patients had no serious complications and no subjective discomfort of nose and eyes. CONCLUSIONS The surgery for primary maxillary neuralgia under transnasal endoscope had a direct way to rotundum foramen, with clear operative vision. It is a minimally invasive surgery, it can minimize the serious complications. The primary curative effect is confirmed.
Examining Masochistic Personality Traits in Terms of Freud and Fairbairn's Theories

Abstract:Masochism is under the heading of sexual perversions in widely used diagnosis and classification systems such as DSM (Diagnostic and Statistical Manual of Mental Disorders) and ICD (International Classification of Diseases). In the psychoanalytic literature, on the other hand, masochism is described more comprehensively with personality traits that work against itself. It is seen as a personality organization that is characterized by symptoms such as creating obstacles to oneself, triggering rejecting and aggressive behaviors in other people, making unfortunate decisions, not being able to enjoy themselves, wasting their own efforts, suffering, having intense feelings of guilt, punishing oneself, preparing the ground for failure. Although it is a common situation in psychotherapy, research on masochism is much less compared to other character organizations. Especially when the studies published in Turkey are reviewed, it is striking that there is almost no comprehensive compilation study or research on masochism. The aim of this article is to discuss the main symptoms, clinical presentation and developmental basis of masochism from a psychoanalytic perspective. First of all, a descriptive review of the characteristics of masochism was made, then Freud's death drive and repetition compulsion concepts and moral defense and split in Fairbairn's object relations model were used to explain the etiology of masochism. Finally, the psychotherapy process carried out with the case of Mr. A, who exhibits personality patterns that work against himself, is formulated using the theoretical approaches of Freud and Fairbairn.

Abstract:The histological and ultrastructural features of a case of malakoplakia affecting the testis and the epididymis of a 53-year-old man are described. Light microscopy shows a granulomatous inflammatory process, characterized by the accumulation of large mononuclear cells with abundant granular cytoplasm and PAS-positive calcific intracytoplasmic inclusions (so-called Michaelis-Gutman bodies). On E.M., such inclusions appear as concentric lamellated structures with a mineralized core. Although the pathogenesis of malakoplakia is still unclear, an abnormality of the bacteria-digesting function of tissue macrophages, leading to persistence of phagolysosomes, has been postulated. Malakoplakia of testis is a benign entity, which must be differentiated from neoplastic lesions, especially germ cell tumours, and granulomatous disease such as histoplasmosis, mycobacteriosis and idiopathic granulomatous orchitis.
A regressive formula of perversity: Wertham and the women of comics

Abstract:This essay serves as a close reading of psychiatrist Fredric Wertham's 1950s descriptions and critiques of female comics characters. I draw on archival sources as well as relevant texts in the study of gender and sexuality. Additionally, I integrate ideas from two of his close associates, folklorist Gershon Legman and psychiatrist Hilde Mosse, whose contributions to Seduction of the Innocent are woven-often invisibly-throughout that text. The character of Wonder Woman serves as a touchstone, but other provocative female figures that Wertham found objectionable in mid-twentieth-century comics accompany her. My goal is to illuminate Wertham's understanding of what he viewed as the "regressive formula of perversity" enacted through women's representations in comics, particularly those female characters who played active roles. Although not all of these concerns are tied to lesbian identity, they do all touch on broader notions of queerness vis-à-vis deviant sexual and gender expression.
Agonistic approaches to sexuality: A critical analysis of the conservative mindset

Abstract:Why do so many people attack sexualities they do not desire and which do not harm them independently of their negative emotions? This is a question to conservative audiences whose anxious reactions this study aims to explain. I introduce seven principles to clarify the situation. The Foundational Principle sketches the basic normative rules. The Hyperbolic Principle recognizes the ever-present exaggeration of sex narratives. According to the Imaginary Principle, extreme imaginings will become too painful to bear. The Principle of de se Reading of Sexual Thought explains contagious emotions and vicarious arousal. The Principle of Motivation argues that sexual images are intrinsically arousing and thus prima facie motivating. According to the Principle of Safety, an unacceptable sexual desire feels unsafe and risky. The Exclusionary Principle says, if a person fails to recognize a given sexual desire, she rejects it. This is the principle that characterizes a conservative reaction to unaccepted sexuality. These negative socio-psychological facts have not attracted adequate attention in sexuality studies. Yet, anxious conservative audiences have the political power to jeopardize liberal policy formation, sex education, and LGBT/BDSM lifestyles. The method of this study is conceptual and philosophical. Its premises and result should lead to empirical verification. At the same time, I suggest a more sophisticated conceptual and linguistic framework for advancing the critical discussion of moral tradition, scientific sexuality studies, and diagnostic art.

Abstract:Libido theory and the idea of an innate matrix of bisexuality are an inadequate basis on which to understand female psychology. Rather, it is the mutual influence of sexuality, aggression, object relations, and a superego that inherits the object-related conflicts of early childhood, intertwined as these are with gender issues, that determine the way a woman experiences herself A woman can have a basically confident, narcissistically valued sense of femaleness and at the same time be troubled by intense feelings of inadequacy and deprivation. Instead of arbitrarily attributing these feelings to penis envy and a sense of castration, based on the idea of bisexuality as bedrock, this paper suggests that we consider primary femininity as bedrock, but that the superego, as heir to unresolved preoedipal and oedipal object-related conflicts, functions to maintain these painful emotions. Clinical material illustrates the utility of these ideas.

Abstract:Women’s capacity for sexual fluidity is at least as interesting a phenomenon from the point of view of evolutionary biology and behavioral endocrinology as exclusively homosexual orientation. Evolutionary hypotheses for female nonheterosexuality have failed to fully account for the existence of these different categories of nonheterosexual women, while also overlooking broader data on the causal mechanisms, physiology, ontogeny, and phylogeny of female nonheterosexuality. We review the evolutionary-developmental origins of various phenotypes in the female sexual orientation spectrum using the synergistic approach of Tinbergen’s four questions. We also present femme-specific and butch-specific hypotheses at proximate and ultimate levels of analysis. This review article indicates that various nonheterosexual female phenotypes emerge from and contribute to hormonally mediated fast life history strategies. Life history theory provides a biobehavioral explanatory framework for nonheterosexual women’s masculinized body morphology, psychological dispositions, and their elevated likelihood of experiencing violence, substance use, obesity, teenage pregnancy, and lower general health. This pattern of life outcomes can create a feedback loop of environmental unpredictability and harshness which destabilizes intrauterine hormonal conditions in mothers, leading to a greater likelihood of fast life history strategies, global health problems, and nonheterosexual preferences in female offspring. We further explore the potential of female nonheterosexuality to function as an alloparental buffer that enables masculinizing alleles to execute their characteristic fast life history strategies as they appear in the female and the male phenotype. Synthesizing life history theory with the female sexual orientation spectrum enriches existing scientific knowledge on the evolutionary-developmental mechanisms of human sex differences.

Borderline Personality Disorder and Emotion Dysregulation

Abstract:In Part 1 of this paper, we discussed emerging evidence suggesting that a general psychopathology or ‘p’ factor underlying the various forms of psychopathology should be conceptualized in terms of the absence of resilience, that is, the absence of positive reappraisal mechanisms when faced with adversity. These impairments in the capacity for positive reappraisal seem to provide a comprehensive explanation for the association between the p factor and comorbidity, future caseness, and the ‘hard-to-reach’ character of many patients with severe personality pathology, most notably borderline personality disorder (BPD). In this, the second part of the paper, we trace the development of the absence of resilience to disruptions in the emergence of human social communication, based on recent evolutionary and developmental psychopathology accounts. We argue that BPD and related disorders may be reconceptualized as a form of social understanding in which epistemic hypervigilance, distrust or outright epistemic freezing is an adaptive consequence of the social learning environment. Negative appraisal mechanisms become overriding, particularly in situations of attachment stress. This constitutes a shift towards a more socially oriented perspective on personality psychopathology in which the absence of psychological resilience is seen as a learned response to the transmission of social knowledge. This shift in our views has also forced us to reconsider the role of attachment in BPD. The implications for prevention and intervention of this novel approach are discussed.

Abstract:The sudden appearance of the term "desexualization" in The Ego and the Id is considered as a marker of the subtle, almost unnoticeable changes that occurred in Freud's thinking after 1920. The strict dichotomy between life and death drives posed a series of new problems that force Freud to invoke a "desexualized libido" in order to restore some fluidity in the psychic apparatus. But the mechanism of desexualization was difficult to describe and Freud seems to resort to a circular explanation. In the end, the restored dialectics between Eros and the death drive, thanks to desexualization, force Freud to invoke a split in the ego itself.

Abstract:BACKGROUND The equivalent uniform dose (EUD) radiobiological model can be applied for lung cancer treatment plans to estimate the tumor control probability (TCP) and the normal tissue complication probability (NTCP) using different dose calculation models. Then, based on the different calculated doses, the quality adjusted life years (QALY) score can be assessed versus the uncomplicated tumor control probability (UTCP) concept in order to predict the overall outcome of the different treatment plans. METHODS Nine lung cancer cases were included in this study. For the each patient, two treatments plans were generated. The doses were calculated respectively from pencil beam model, as pencil beam convolution (PBC) turning on 1D density correction with Modified Batho's (MB) method, and point kernel model as anisotropic analytical algorithm (AAA) using exactly the same prescribed dose, normalized to 100% at isocentre point inside the target and beam arrangements. The radiotherapy outcomes and QALY were compared. The bootstrap method was used to improve the 95% confidence intervals (95% CI) estimation. Wilcoxon paired test was used to calculate P value. RESULTS Compared to AAA considered as more realistic, the PBCMB overestimated the TCP while underestimating NTCP, P<0.05. Thus the UTCP and the QALY score were also overestimated. CONCLUSIONS To correlate measured QALY's obtained from the follow-up of the patients with calculated QALY from DVH metrics, the more accurate dose calculation models should be first integrated in clinical use. Second, clinically measured outcomes are necessary to tune the parameters of the NTCP model used to link the treatment outcome with the QALY. Only after these two steps, the comparison and the ranking of different radiotherapy plans would be possible, avoiding over/under estimation of QALY and any other clinic-biological estimates.

Abstract:Networked vehicle systems are composed of nodes possessing distinct physical and computational capabilities and interact with each other over a time-varying communications topology. The resulting dynamic structure delivers novel system-level properties that can not be found in the constituent nodes. In this paper we document a series of technological achievements which allowed the concretization of resilient, flexible and very capable networks of heterogeneous vehicles that can adapt to environment conditions and collaborate in order to attain user-provided objectives safely and robustly. We also report results from several field deployments where these approaches have been successfully applied for biology, oceanography and military purposes.
