Abstract:Dural repair products are evolving from animal tissue-derived materials to synthetic materials as well as from inert to absorbable features; most of them lack functional and structural characteristics compared with the natural dura mater. In the present study, we evaluated the properties and tissue repair performance of a new dural repair product with biomimetic design. The biomimetic patch exhibits unique three-dimensional nonwoven microfiber structure with good mechanical strength and biocompatibility. The animal study showed that the biomimetic patch and commercially synthetic material group presented new subdural regeneration at 90 days, with low level inflammatory response and minimal to no adhesion formation detected at each stage. In the biological material group, no new subdural regeneration was observed and severe adhesion between the implant and the cortex occurred at each stage. In clinical case study, there was no cerebrospinal fluid leakage, and all the postoperation observations were normal. The biomimetic structure and proper rate of degradation of the new absorbable dura substitute can guide the meaningful reconstruction of the dura mater, which may provide a novel approach for dural defect repair.

Abstract:Objective: Biomimetic design will significantly improve growth and regeneration of dural cells and tissue for better repairing effects and fewer complications in repairing the native dura. This study designed a novel composite, biomimetic substitute based on the characteristics of native dura extracellular matrix. Methods and results: This substitute is expected to rapidly induce cell adhesion, migration, and fast regeneration of neotissue. The material characteristics (contact angle, surface charge, and zeta potential were evaluated), in vitro biological characteristics (cell stretch, connections between cells, cell proliferation) and in vivo tissue regeneration capability of this substitute were evaluated, compared to those of collagen dura substitute, the mostly used dura substitute. The results showed that the surface properties of this composite substitute were more biomimetic to native extracellular matrix than collagen substitute did, together with better cytocompatibility, tissue ingrowth, and neoangiogenesis. This composite substitute further demonstrated in clinical case study its ideal repair effect with no CSF leakage or other adverse reactions. Conclusion: In conclusion, the new biomimetic composite substitute provides alternative substitute for dura repairing.

Abstract:The dura mater, as the crucial outermost protective layer of the meninges, plays a vital role in safeguarding the underlying brain tissue. Neurosurgeons face significant challenges in dealing with trauma or large defects in the dura mater, as they must address the potential complications, such as wound infections, pseudomeningocele formation, cerebrospinal fluid leakage, and cerebral herniation. Therefore, the development of dural substitutes for repairing or reconstructing the damaged dura mater holds clinical significance. In this review we highlight the progress in the development of dural substitutes, encompassing autologous, allogeneic, and xenogeneic replacements, as well as the polymeric-based dural substitutes fabricated through various scaffolding techniques. In particular, we explore the development of composite materials that exhibit improved physical and biological properties for advanced dural substitutes. Furthermore, we address the challenges and prospects associated with developing clinically relevant alternatives to the dura mater.

Biomechanical Study of Human Dura and Its Substitutes.

Abstract:OBJECTIVE:To investigate the biomechanical characters of human dura and its substitutes and to establish guidelines for selection of optimal repair materials for reconstruction of skull base defects.METHODS:A measurement of creep properties of human dura and its substitutes were conducted using biomechanical tests. The dynamic changes of biomechanical properties of canine dura obtained from skull base defects were observed as well.RESULTS:The creep properties of human dura presented a linear-relationship between initial strain and stress, and the creep strain increased slowly with time. The creep compliance formula for human dura and its substitutes was as follows: J (t) = J(0) + Kf (t). The initial compliance of canine dura in skull base defects was reduced by 35%, 46% and 50%, respectively 1, 3 and 6 months after surgery.CONCLUSION:The optimal material for the repair of skull base defects can be estimated using creep compliance analysis. The less the compliance, the better the property of anti-protrusion. Fresh human dura is the least compliant and then in increasing order are lyophilized human dura, fresh human pericranium, Terylene and silicon membranes. The pattern of biomechanical characteristics of the dura mater at skull base defects shows that the strain ability of the dura mater decreases distinctly for 1 - 3 months and then remains almost unchanged for 3 - 6 months after surgery.

Abstract:The dura mater protects underlying tissues and cerebrospinal fluid, but dural abnormalities can cause leaking and meningitis and wound infections. This review emphasizes the relevance of dural repair and the progress of dural reconstruction materials, focusing on biological scaffolds. A comprehensive assessment of dural substitution literature focused on composite, acellular, natural, synthetic, and homologous materials. Decellularized dura scaffolds as viable natural biomaterials for tissue engineering are explored in this overview of these materials' properties, effectiveness, and therapeutic uses. The evaluation also compares materials and assesses host tissue response in preclinical and clinical studies. The review highlights various dural substitution findings. There is no gold standard for dural repair, despite the variety of materials used. Replicating natural dura mater's mechanical and structural qualities is difficult. Recent research suggests that decellularized dura scaffolds can regenerate tissue while reducing inflammation and transplant rejection. Furthermore, the movement toward personalized medicine in this sector suggests that bespoke alternatives might be chosen depending on patient characteristics, improving treatment effects. Synthesizing dural replacement research and clinical uses in this review adds to knowledge. It tackles standard materials' shortcomings and shows how biological scaffolds might improve dural repair. The analysis opens the door to personalized dural repair research that might enhance patient outcomes and advance neurosurgery. The insights presented herein highlight the intriguing prospects for appropriate dural restorative materials, seeking to improve dural defect treatment.

Abstract:Biomimetics is the science of imitating nature's designs and processes to create innovative solutions for various fields, including dentistry and craniofacial reconstruction. In these areas, biomimetics involves drawing inspiration from living organisms/systems to develop new materials, techniques, and devices that closely resemble natural tissue structures and enhance functionality. This field has successfully demonstrated its potential to revolutionize craniofacial procedures, significantly improving patient outcomes. In dentistry, biomimetics offers exciting possibilities for the advancement of new dental materials, restorative techniques, and regenerative potential. By analyzing the structure/composition of natural teeth and the surrounding tissues, researchers have developed restorative materials that mimic the properties of teeth, as well as regenerative techniques that might assist in repairing enamel, dentin, pulp, cementum, periodontal ligament, and bone. In craniofacial reconstruction, biomimetics plays a vital role in developing innovative solutions for facial trauma, congenital defects, and various conditions affecting the maxillofacial region. By studying the intricate composition and mechanical properties of the skull and facial bones, clinicians and engineers have been able to replicate natural structures leveraging computer-aided design and manufacturing (CAD/CAM) and 3D printing. This has allowed for the creation of patient-specific scaffolds, implants, and prostheses that accurately fit a patient's anatomy. Systematic review and meta-analysis of the biomechanical properties of the human dura mater applicable in computational human head models

Abstract:Accurate biomechanical properties of the human dura mater are required for computational models and to fabricate artificial substitutes for transplantation and surgical training purposes. Here, a systematic literature review was performed to summarize the biomechanical properties of the human dura mater that are reported in the literature. Furthermore, anthropometric data, information regarding the mechanically tested samples, and specifications with respect to the used mechanical testing setup were extracted. A meta-analysis was performed to obtain the pooled mean estimate for the elastic modulus, ultimate tensile strength, and strain at maximum force. A total of 17 studies were deemed eligible, which focused on human cranial and spinal dura mater in 13 and 4 cases, respectively. Pooled mean estimates for the elastic modulus (n = 448), the ultimate tensile strength (n = 448), and the strain at maximum force (n = 431) of 68.1 MPa, 7.3 MPa and 14.4% were observed for native cranial dura mater. Gaps in the literature related to the extracted data were identified and future directions for mechanical characterizations of human dura mater were formulated. The main conclusion is that the most commonly used elastic modulus value of 31.5 MPa for the simulation of the human cranial dura mater in computational head models is likely an underestimation and an oversimplification given the morphological diversity of the tissue in different brain regions. Based on the here provided meta-analysis, a stiffer linear elastic modulus of 68 MPa was observed instead. However, further experimental data are essential to confirm its validity.

Abstract:<p>Dura mater defect and subsequent cerebrospinal fluid (CSF) leakage usually appear in trauma or neurosurgical procedures and are followed by a series of serious complications and even death. The use of a qualified dura mater substitute with multifunction of leakage blockade, adhesion prevention, and dura reconstruction is one of the promising treatment methods. However, even though some products have been used in the clinic, none of the substitutes achieved the required multifunction. In this study, we aimed to design and fabricate a dura repair composite with the ideal multifunction. By biomimicking the structure and component of natural dura, we applied L-polylactic acid (PLLA), chitosan (CS), gelatin, and acellular small intestinal submucosa (SIS) powders to successfully prepare a triple-layered composite. Then, a series of specific devices and techniques were developed to investigate the performance. The results revealed that satisfactory structural stability could be realized under good synergistic interactions among the components. In addition, all the findings suggested that the bionic triple-layered composite showed satisfactory multifunction of leakage blockade, adhesion prevention, antibacterial property, and dura reconstruction potential, and thus, it might be a promising candidate for dura repair.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Statement of Significance</h3><p>: Developing qualified dura mater substitutes with multifunction of leakage blockade, adhesion prevention, and dura reconstruction is crucial for treating dura mater defect and subsequent cerebrospinal fluid (CSF) leakage that appear in trauma or neurosurgical procedures. In this study, we designed and fabricated a triple-layered dura repair biocomposite with satisfactory structural stability and desired multifunction based on biomimicking of the structure and component of natural dura. Moreover, a series of specific devices and techniques were developed to investigate the relevant performance. Overall, the developed hydrogel electrospinning system exhibited excellent advantages in achieving multifunction and could be applied widely in the future to achieve multifunctional tissue repair materials.</p>

Abstract:Biological nanocomposites, such as bone, tooth, shell, and wood, exhibit exceptional mechanical properties. Much recent effort has been directed at exploring the basic mechanical principles behind the microstructures of these natural materials to provide guidelines for the development of novel man-made nanocomposites. This article reviews some of the recent studies on mechanical properties of biological nanocomposites, including their stiffness, strength, toughness, interface properties, and elastic stability. The discussion is focused on the mechanical principles of biological nanocomposites, including the generic nanostructure of hard-mineral crystals embedded in a soft protein matrix, the flaw-tolerant design of the hard phase, the role of the soft matrix, the hybrid interface between protein and mineral, and the structural hierarchy. The review concludes with sonic discussion of and outlook on the development of biomimicking synthetic materials guided by the principles found in biological nanocomposites.

Abstract:Dura substitutes are applied in duraplasty to repair lost or damaged dura. Collagen-based dura substitutes are mainstream products in both the US and Chinese markets. In this study, dura substitute devices with potential dura regeneration ability are evaluated. The dura substitutes are composed of fibrous type I collagen that were purified from bovine tendon. Physical and chemical characterization demonstrated that the tested dura substitute has desirable porous scaffolding structures and is composed of highly purified type I collagen. The collagen dura substitutes were further investigated in vivo with a rabbit model for 6 months to evaluate their safety and performance to repair and regenerate dura. No inflammation or infection was observed during the course of in vivo study. The integration of the collagen dura substitutes with surrounding tissue was normal as compared to native tissue. The macroscopic and microscopic histological assessments of the sampled animal tissue showed that the damaged dura were regenerated. The collagen dura substitutes were resorbed between 3 and 6 months along with newly regenerated dura. Both tissue adhesion and dura repair was the worst in blank control group as compared to those in the collagen dura substitutes. Taken together, regenerative collagen dura substitutes demonstrated with suitable physicochemical properties. The in vivo evaluation in a rabbit model further demonstrated the safety and performance of such substitutes for dura repair and regeneration.

Abstract:Dural defects are a common problem in neurosurgical procedures and should be repaired to avoid complications such as cerebrospinal fluid leakage, brain swelling, epilepsy, intracranial infection, and so on. Various types of dural substitutes have been prepared and used for the treatment of dural defects. In recent years, electrospun nanofibers have been applied for various biomedical applications, including dural regeneration, due to their interesting properties such as a large surface area to volume ratio, porosity, superior mechanical properties, ease of surface modification, and, most importantly, similarity with the extracellular matrix (ECM). Despite continuous efforts, the development of suitable dura mater substrates has had limited success. This review summarizes the investigation and development of electrospun nanofibers with particular emphasis on dura mater regeneration. The objective of this mini-review article is to give readers a quick overview of the recent advances in electrospinning for dura mater repair.

Abstract:Craniofacial tissue-engineered techniques have significantly improved over the past 20 years as a result of developments in engineering and in material science. The regeneration of the craniofacial tissue is frequently complicated due to the craniofacial region's complexity, which includes bone, cartilage, soft tissue, and neurovascular bundles. It is now possible to construct tissues in the lab using scaffolds, cells, and physiologically active chemicals. For bone repair/augmentation, the biomaterials are classified into natural like “collagen, fibrin, alginate, silk, hyaluronate, chitosan” and synthetic like “polyethyleneglycol, poly-e-caprolactone, polyglycolic acid” and some bioceramics “tricalcium phosphate, hydroxyapatite, biphasic calcium phosphate, and the bioactive glasses” along with metals certain (Titanium and Zirconia ) and as this is part of advanced tissue engineering in dentistry there are some bioactive restorative materials like mineral trioxide aggregate and biodentine. The newer advanced techniques like 3D printed templates present a framework for achieving the three pillars of tissue engineering: healing, rebuilding and rejuvenation. The field of tissue engineering has recently become interested in 3D printing, also known as “Additive Manufacturing”, which is a ground-breaking technique that allows for the printing of patient-specific scaffolds, medical devices, multiscale, biomimetic/intricate cytoarchitecture/function-structure hierarchies and multicellular tissues in complex microenvironments. Biopolymers use is dependent on meeting the criteria for various scaffolds, including mechanical integrity, thermal stability, chemical composition, along with biological properties. Researchers have developed a revolutionary 4D bioprinting technique using cell traction forces and they are used to develop intricate dynamic structures, smart medical devices, or complex human organs.
Evaluation of efficacy and biocompatibility of a new absorbable synthetic substitute as a dural onlay graft in a large animal model

Abstract:Background: Numerous dura substitutes are commercially available, but no absorbable synthetic dura repair product has been used for both onlay and suture applications. Objective: The safety and effectiveness of a new absorbable synthetic substitute composed of Poly-L-lactide microfibers as onlay dural graft were evaluated. Methods: Physical properties and performance of the microfibrous synthetic dural substitute implanted as an onlay or suturable grafts were compared with these commercial products, including CODMAN ETHISORB™ Dura Patch and DuraGen™ Dural Graft Matrix, in a canine duraplasty model. The cerebrospinal fluid (CSF) leakage, macroscopic and microscopic observation at 30 and 90 days after implantation were investigated. Results: The absorbable synthetic dural substitute exhibited good wettability and conformability. When implanted as an onlay graft, it can prevent CSF leakage and integrate with the surrounding tissue to repair dural defects, indicating its good efficacy and biocompatibility as an onlay graft. Conclusion: Based on the excellent physical properties and performances mentioned above, the new absorbable synthetic substitute can be applied as a dural graft for both onlay and suturable applications.

Abstract:Nature's inspiration is a promising tool to design new biomaterials especially for frontier technological areas such as tissue engineering and nanomedicine. Polyurethanes (PURs) are a flexible platform of materials that can be designed to fit the requirements imposed by their final applications. The choice of their building blocks (which are used in the synthesis as macrodiols, diisocyanates, and chain extenders) can be implemented to obtain biomimetic constructs, which can mimic the native tissue in terms of mechanical, morphological and surface properties. In bone tissue engineering, elastomeric PURs avoid shear forces at the interface between bone and the implant, supporting the proliferation of osteogenic cells. Soft tissues can be engineered equally efficiently by PURs, which have been reported to be reliable candidates in the fabrication of muscle constructs (including heart, blood vessels, cartilage and peripheral nerve regeneration). This review summarizes the recent progress in the biomedical applications of polyurethanes. After introducing the concept of biomimetics (paragraph 2), the use of PURs in the engineering of hard tissues (para. 3.1), soft tissues (para. 3.2) and in nanomedicine (para. 4) is reported. Taken collectively, reports in the literature clearly indicate the potential of PURs to complement or substitute alternative, FDA approved, degradable polymers, such as those belonging to the polyester family, in the replacement of damaged tissues or organs, as well as in the emerging field of nanomedicine, where they might show superior drug encapsulation efficiency and enhanced capability to target specific tissue compartments.

The paper provides an introduction to biomaterials covered and their available types, such as metal, ceramic, polymer, and composites. Biomaterials are structures, devices, or materials that may repair or implant tissue. Biomaterials are biocompatible that have high mechanical strength, and need a long time to break down. A biomaterial is any natural or synthetic material that regularly touches the human body or is utilized to replace or restore biological tissue—metallic systems in the body and numerous ceramic and polymeric implant materials and systems. Polymers as biomaterials have had a significant impact on medical technology development. Furthermore, features of biomaterials such as mechanical, physical, and chemical qualities have been detailed to assist users in selecting the best biomaterial for their needs based on these properties. While these materials do not replace human tissues' function, they have good physical and mechanical qualities that allow them to be used as body tissue substitutes. Apart from this, biomaterials are applied in various fields, including medicine and pharmacy. Biodegradable polymeric biomaterials, in particular, offer the advantage of being able to be broken down and removed once their purpose is completed. The most common biomaterials are used in tissue engineering, medicine delivery systems, orthopedic, orthodontic, wound healing, and cardiovascular applications. The conclusion summarizes that biomaterials are the most advantageous in medical science. Biomaterial and cellular biology researchers have been working to make biomanufacturing technologies more broadly available for a long time. The main area of focus is medicine, where technology is critical in studying and preventing uncommon diseases.

However, in the last three decades, new alternatives have emerged, such as polymers that could replace metallic materials. The aim of this research work is to demonstrate the structural effects caused by the fatigue phenomenon and the comparison with polymeric materials that may be biomechanically viable by reducing the stress shielding effect at the bone–implant interface. A numerical simulation was performed using the finite element method. Variables such as Young's modulus, Poisson's coefficient, density, yield strength, ultimate strength, and the S-N curve were included. Prior to the simulation, a representative digital model of both a dental implant and the bone was developed. A maximum load of 550 N was applied, and the analysis was considered linear, homogeneous, and isotropic. The results obtained allowed us to observe the mechanical behavior of the dental implant by means of displacements and von Mises forces. They also show the critical areas where the implant tends to fail due to fatigue. Finally, this type of non-destructive analysis proves to be versatile, avoids experimentation on people and/or animals, and reduces costs, and the iteration is unlimited in evaluating various structural parameters (geometry, materials, properties, etc.).

Abstract:Most of the polymers used as biomaterials for scaffolds are naturally occurring, synthetic biodegradable, and synthetic non-biodegradable polymers. Since synthetic polymers can be adapted for obtaining singular desired characteristics by applying various fabrication techniques, their use has increased in the biomedical field, in dentistry in particular. The manufacturing methods of these new structures include many processes, such as electrospinning, 3D printing, or the use of computer-aided design/computer-aided manufacturing (CAD/CAM). Synthetic polymers show several drawbacks that can limit their use in clinical applications, such as the lack of cellular recognition, biodegradability, and biocompatibility. Moreover, concerning biodegradable polymers, the time for matrix resorption is not predictable, and non-resorbable matrices are preferred for soft tissue augmentation in the oral cavity. This review aimed to determine a new biomaterial to offset the present shortcomings in the oral environment. Researchers have recently proposed a novel non-resorbable composite membrane manufactured via electrospinning that has allowed obtaining remarkable in vivo outcomes concerning angiogenesis and immunomodulation throughout the polarization of macrophages. A prototype of the protocol for in vitro and in vivo experimentation with hydrogels is explained in order to encourage innovation into the development of promising biomaterials for soft tissue augmentation in the near future.

Abstract:PURPOSE: To evaluate the in vivo efficiency of commercial polymeric membranes for guided bone regeneration.METHODS: Rat calvarial critical size defects was treated with LuminaCoat (LC), Surgitime PTFE (SP), GenDerm (GD), Pratix (PR), Techgraft (TG) or control (C-) and histomorphometric analysis determined the percentage of new bone, connective tissue and biomaterial at 1 or 3 months. Statistical analysis used ANOVA with Tukey's post-test for means at same experimental time and the paired Student's t test between the two periods, considering p < 0.05.RESULTS: New bone at 1 month was higher for SP, TG and C-, at 3 months there were no differences, and between 1 and 3 months PR had greater increase growthing. Connective tissue at 1 month was higher for C-, at 3 months for PR, TG and C-, and between 1 and 3 months C- had sharp decline. Biomaterial at 1 month was higher for LC, in 3 months for SP and TG, and between 1 and 3 months, LC, GD and TG had more decreasing mean.CONCLUSIONS: SP had greater osteopromotive capacity and limitation of connective ingrowth, but did not exhibit degradation. PR and TG had favorable osteopromotion, LC less connective tissue and GD more accelerated biodegradation.
