Abstract:Objective Violence has had adverse effects on the physical and psychological dimensions of pregnant women. This study was conducted with the aim of determining the relationship of domestic violence with pregnancy symptoms and pregnancy experience. Design In this cross-sectional study, pregnant women were selected through two-stage cluster sampling method. Data were collected using the domestic violence questionnaire of the WHO, pregnancy symptoms inventory (PSI) and pregnancy experience scale (PES). To determine the relationship between domestic violence with pregnancy symptoms and pregnancy experience, adjusted general linear model was used in multivariate analysis. Setting Health centres in Urmia—Iran in 2022. Participants 415 pregnant women. Results The frequency of emotional, physical and sexual violence was 86.0%, 67.7% and 79.5%, respectively. The mean (SD) of PSI was 49.45 (14.38) with attainable score of 0–123 and PES including happiness and worry was 14.32 (6.48) and 16.21 (2.51) with attainable score of 0–30, respectively. Based on the adjusted general linear model, the mean score of PSI in women who experienced physical violence (mild (p<0.001) and moderate (p<0.001)); sexual violence (mild (p<0.001), moderate (p<0.001) and severe (p<0.001)); and emotional violence (mild (p<0.001), moderate (p=0.002) and severe (p<0.001)) was significantly higher than women without experiencing violence. The mean score of happiness during pregnancy in women who experienced physical violence (moderate (p=0.011)) and emotional violence (mild (p<0.001), moderate (p=0.002) and severe (p<0.001)) was significantly lower than women without experience of violence. Also, the mean score of worry scores in women with experience of sexual violence (mild (p=0.001) and moderate (p=0.012)) and emotional violence (mild (p<0.001), moderate (p<0.001)) and severe (p<0.001)) was significantly higher than women without experiencing violence. Conclusion Considering the relationship between violence and pregnancy symptoms and pregnancy experiences, it is necessary to use appropriate strategies to prevent violence in pregnant women.

Abstract:Background: Domestic violence is an important health, which has serious impacts on women's health. The study aimed to discover the protective and risk factors of domestic violence among pregnant adolescents.Materials and Methods: In the analytical cross-sectional study, 255 eligible pregnant women aged 14-20 years who have beenreferred to Al-Zahra Hospital, in northern Iran between September 2020 and March 2022 participated. Demographical characteristicswere recorded by a questionnaire. Domestic violence was assessed using the instrument of violence against women. Descriptive statistics were used to describe the basic features of the data.Results: The frequency of physical, emotional, and sexual violence was respectively 13.33%, 23.52%, and 9.01%. The risk factors included the spouse’s substance use (OR 2.41, 95% CI 1.25–4.62), spouse’s low education (OR 1.41, 95% CI 1.12–3.52), spouse’s unemployment (OR 1.14, 95% CI 1.03–1.57) and domestic violence exposure in childhood (OR 1.85, 95% CI 1.46–2.51). Higher education for women was a protective factor for domestic violence (OR 0.70, 95% CI 0.45–0.83).Conclusions: The education level is a protective factor against domestic violence among pregnant adolescents. These results can help to design the most appropriate prevention programs to reduce the risk factors for violence among pregnant adolescents.

The effect of family-based counseling on domestic violence in pregnant women referring to health centers in Sahneh city, Iran, 2018

Abstract:Background: Domestic violence is a term that shows aggressive behavior with different physical, psychological, economic, and social dimensions. This concept is considered as one of the health priorities regarding its significant effects on pregnancy, postpartum, and the outcome of pregnancy. The present study was conducted to investigate the effect of family-based counseling on domestic violence against pregnant women. Methods: An intervention trial was conducted on 92 pregnant women exposed to domestic violence, selected among 274 pregnant women accessible in Sahneh, Iran. Data collection was performed using the standard questionnaire of domestic violence against women of Mohseni Tabrizi. The pregnant women exposed to violence were randomly assigned to intervention and control groups (in two groups of 45 people). In the intervention group, three 45-min individual counseling sessions were held for the pregnant women and their spouses according to Greeting, Ask, Tell, Help, Explain, and Refer (GATHER) principles. Four weeks after the end of the sessions, the two groups again completed the questionnaire. Results: Family-based counseling intervention reduced the mean score of domestic violence in the intervention group from 68.58 ± 9.21 before the intervention to 49.56 ± 8.83 after intervention. Also, various domains of violence including mental, verbal, financial, physical, sexual, and social violence were significantly declined in the intervention group (P < 0.001); however, there was no significant decrease in emotional violence score (P > 0.001). Conclusion: Family-based counseling plays a significant role in reducing the various types of violence against women through increasing the awareness of couples and by improving the relationship between couples during pregnancy. It will reduce the incidence of violence against a pregnant woman and consequently reduce complications on the mother and pregnancy outcomes. Family-oriented counseling played a significant role in deterring all forms of violence against women by increasing awareness of couples and improving their relationship during pregnancy. Moreover, family-oriented counseling reduced the incidence of violence against pregnant women and thus deterred maternal complications associated with pregnancy.

Abstract:Background: Domestic Violence (DV) against women is a global issue and is recognized by United Nations as any act of gender-based violence against women. It is estimated that 736 million women are globally subject to such violence in their lifetime. Objective: To determine the frequency of domestic violence among pregnant women and their adverse maternal outcomes in Pakistan. Methods: All pregnant women aged 15-49 years in the third trimester attending Jinnah Postgraduate Medical Centre, Karachi were enrolled in the study after informed consent. Selected women were interviewed and followed till delivery. The interview encompassed close-ended questionnaires in 5 sections with yes or no options. Items comprised questions on physical, emotional/verbal, financial, psychological, and sexual abuse to assess domestic violence. Pregnancy intendedness was evaluated, and the adverse maternal outcome was assessed at delivery. Results: A total of 383 women were included with a mean age of 26.73 ±5.28 years. 175 (45.7%) were subjected to some form of DV during pregnancy. Parity (p<0.001), marital status (p<0.001), ethnicity (p=0.005), substance abuse by husband (p<0.001) and monthly income of husband (p=0.002) were significantly different among women with and without DV. Among the 175 women who suffered from DV, 112 (64.0%) experienced adverse events, but among women who did not suffer from DV, 116 (55.8%) had adverse events (p=0.102). Adverse events including antepartum and postpartum haemorrhage (p=0.004), unintended pregnancy (p=0.013), pre-term labour (p 4000 g (p<0.001) were significantly different among women reporting DV and not reporting DV. Conclusion: Violence during pregnancy is common in our society and verbal, emotional and psychological violence are the comments type. A high frequency of adverse maternal and foetal outcomes was noted in these women.
He hurts her womb: Physical-sexual violence and pregnancy complications among women in Afghanistan

Abstract:Background: Though some studies show the association between intimate partner violence and pregnancy complications in developed countries, the association remains understudied in less developed and low-income settings. This study examines the association of physical and sexual violence with pregnancy complications among women in Afghanistan. Methods: This study used the data from the 2015 Afghanistan Demographic and Health Survey (AfDHS). The analysis included 7229 women aged between 15 and 49 and used logistic regression to show the association of physical and sexual violence with pregnancy compilations. The analysis controlled for some potential variables and followed complex survey design factors such as strata, clusters, and survey weights. Results: Fully adjusted regression model shows that the women who experienced physical violence were 21% (adjusted odds ratio [OR]=1.21; confidence interval [CI]=0.98, 1.50; P <0.1) more likely to endure pregnancy complications compared to those who did not face the violence. Likewise, the women experiencing sexual violence were 89% (adjusted OR=1.89; CI=1.37, 2.62; P <0.01) higher to face pregnancy complications than those who did not face any of sexual violence. More specifically, physically and sexually violated women were highly prey to the complications that increased with the increment of the violence. Conclusion: This study adds that policymakers may formulate policies for ensuring 3C (caring couple counselling) by readdressing couple relationships, raising gender rights and awareness, providing reproductive health literacy, and increasing mental health awareness during pregnancy.

Abstract:Introduction: Domestic Violence (DV) during pregnancy is a serious public health issue which threatens maternal and foetal health outcomes. Routine antenatal care provides an opportunity for identifying women experiencing violence during pregnancy. Aim: To evaluate the prevalence of DV among pregnant women so that foetal complications can be prevented and adequate measures can be taken to protect mothers from DV. Materials and Methods: This cross-sectional study was conducted from 1st October to 30th November 2016 at the Outpatient Department (OPD) of Obstetrics and Gynaecology of Mahatma Gandhi Medical College and Research Institute, Puducherry, India. A validated modified version of Abuse Assessment Screen questionnaire was given to 200 pregnant women at their first hospital visit. A statistical analysis was conducted using Chi-square test and Fisher’s-Exact Test in MS Excel 2007, version 12.0. A p-value of less than 0.05 was considered significant. Results: A total of 200 consecutive pregnant women at the first visit to hospital were enrolled in the study. Prevalence of DV was reported to be 6.5%. Out of which maximum (2.5%) reported verbal abuse. There was no reporting of sexual abuse. Educational status showed influence on DV. Economic status too had great influence on DV which was more prevalent among proverty strucken people (19.4%). There were 7.9% incidences for DV in the age group <25 year (p-value=0.3). Prevalence was slightly more in rural areas (8.7%) than in urban areas (4.2%) (p-value=0.2). With respect to education, women with high school and above had lower incidence of DV (4.8%). Conclusion: The pregnant woman should be continually provided with a non judgemental, sensitive and supportive service during their pregnancies. The study establishes that women in the present environment experience DV during pregnancy and more in younger age group. This is also more common in women with lower literacy and with lower socio-economic status.

Abstract:Background: Violence against women is one of the most widespread, persistent and detrimental violations of human rights in today's world, which has not been reported in most cases due to impunity, silence, stigma and shame, even in the age of social communication. Domestic violence against women harms individuals, families, and society. The objective of this study was to investigate the prevalence and experiences of domestic violence against women in Semnan. Methods: This study was conducted as mixed research (cross-sectional descriptive and phenomenological qualitative methods) to investigate domestic violence against women, and some related factors (quantitative) and experiences of such violence (qualitative) simultaneously in Semnan. In quantitative study, cluster sampling was conducted based on the areas covered by health centers from married women living in Semnan since March 2021 to March 2022 using Domestic Violence Questionnaire. Then, the obtained data were analyzed by descriptive and inferential statistics. In qualitative study by phenomenological approach and purposive sampling until data saturation, 9 women were selected who had referred to the counseling units of Semnan health centers due to domestic violence, since March 2021 to March 2022 and in-depth and semi-structured interviews were conducted. The conducted interviews were analyzed using Colaizzi's 7-step method. Results: In qualitative study, seven themes were found including "Facilitators", "Role failure", "Repressors", "Efforts to preserve the family", "Inappropriate solving of family conflicts", "Consequences", and "Inefficient supportive systems". In quantitative study, the variables of age, age difference and number of years of marriage had a positive and significant relationship, and the variable of the number of children had a negative and significant relationship with the total score and all fields of the questionnaire (p < 0.05). Also, increasing the level of female education and income both independently showed a significant relationship with increasing the score of violence. Conclusions: Some of the variables of violence against women are known and the need for prevention and plans to take action before their occurrence is well felt. Also, supportive mechanisms with objective and taboo-breaking results should be implemented to minimize harm to women, and their children and families seriously.
The effect of solution-focused counseling on violence rate and quality of life of pregnant women at risk of domestic violence: a randomized controlled trial

Abstract:Background: Domestic violence during pregnancy as one of the most common social problems and major challenges of health systems can affect the health of the mother and fetus. The study aimed to compare the two groups of intervention based on solution-focused counseling and control in terms of violence and quality of life amounts in women who had experienced domestic violence. Methods: A randomized controlled trial on 90 pregnant women was blocked into two intervention groups (n = 45) and a control (n = 45). The intervention group received six counseling sessions with a solution-focused approach. Study tool included conflict tactics scale (CTS- 2) and short form health survey (SF-36). The tools were completed once before the intervention and again 6 weeks after the end of the counseling sessions by the participants of both groups. The P-value less than 0.05 was considered statistically significant. Results: A total of 82/90 pregnant women were analyzed. The results showed that median and interquartile range (IQR) of physical, psychological and sexual violence significantly decreased in the intervention group than the control group (Ps = 0.001). Moreover, quality of life scores significantly improved in the intervention group compared to the control group (P = 0.001). Conclusion: Solution-focused counseling could be an effective approach to reduce the amount of violence and increase the quality of life in women exposed to domestic violence. Abstract:Introduction Domestic violence (DV) in the form of verbal abuse is very common among women, especially pregnant women, posing as a serious public health issue that could lead to complications in pregnancy and threaten maternal and fetal outcomes. Studies have determined that domestic verbal abuse (DVA) in pregnancy was more common in women less than 25 years of age as well as in those with low education levels. This study determined the overall prevalence of verbal abuse in pregnant women, in a semi-urban population and is unique in that the verbal abuse in pregnant women with a previous girl child was also determined. This study helps healthcare providers identify the potential causes of DVA in pregnancy and provide timely interventions in the form of counseling for pregnant women and families. Objective This observational study was carried out to assess the prevalence of DVA among pregnant women, to determine the trimester of occurrence of DVA among pregnant women, and to explore the associations of DVA with age, employment status, parity gestational age, and birth weight. Materials and methods This was a six-month hospital-based observational study conducted at Dr D. Y. Patil Medical College's in-patient department (IPD) of Obstetrics and Gynecology in Pimpri, Pune. Consent was obtained from 200 pregnant women who received admission for delivery and provided a validated modified copy of a DV assessment screening questionnaire. A statistical analysis was performed using GraphPad Prism 10. A Chi-square test was employed wherever required, and a p-value of less than 0.05 was considered significant. Results The study included 200 pregnant women, who were admitted to the hospital for delivery. The prevalence of DV in the form of verbal abuse was noted to be 74 out of 200 (37%). The working status of the woman showed an influence on DVA. There was also a significant increase in verbal abuse (68%) among the age group between 18 and 23 years. The previous delivery of a female child also had a significant impact on DVA, which turned out to be more prevalent, particularly if two female children were born previously (80%). The study also noted higher rates of preterm deliveries in pregnant women with DVA being 57%. Conclusion The study demonstrates that women, even in modern times, experience DVA during pregnancy, especially among the younger age group. It has also been found that it is more common among women who are financially dependent due to maternal unemployment. As a result, there is a need to routinely screen pregnant women for DVA to avoid potentially detrimental pregnancy outcomes and to prevent ongoing abuse.
The cause and consequence of domestic violence on pregnant women in India

Abstract:The objective of this study is to evaluate and elucidate the impact of domestic violence on the health and pregnancy outcomes of women. Data were extracted from literature through the MEDLINE database for years 2000-2011. Domestic violence occurs in every society, irrespective of class, creed, religion and country. Women attending antenatal clinics in Delhi reported experience of 26.9% physical, 29% mental and 6.2% sexual abuse, irrespective of their age. The spouse was the perpetrator of abuse in 47% cases and his family members were responsible for 31%. Pregnant women were hit by their husbands on the back and abdomen, sometimes repeatedly, besides psychological abuse. Incidence of domestic violence was more when the male spouse was less educated or in the habit of consuming alcohol, opium or tobacco. Illiteracy, poverty, family status and uncaring attitude of community about spousal violence were the causes of domestic violence. Women having experience of violence were less likely to receive antenatal care or home visits by health workers and had a risk of perinatal and neonatal mortality of 2.59 and 2.37 times higher, respectively, than women having no violence during pregnancy. The survey indicated that 4.5% of abused women required hospitalisation and 3.8% needed medical care. Women's education, economic autonomy and empowerment may reduce the incidence of domestic violence among Indian women.

Abstract:Domestic violence during pregnancy is a critical health issue due to its significant implications for both maternal and fetal well-being. Women in South Asian countries are particularly at a higher risk of experiencing domestic violence. The objective of this study was to identify factors associated with domestic violence among pregnant women attending tertiary care antenatal services in Pakistan. The secondary aim was to evaluate the impact of domestic violence on maternal and perinatal outcomes. The study participants were 681 pregnant women registered for antenatal care and delivery at a tertiary care hospital in Islamabad, Pakistan. Data were collected through in-person surveys and medical chart reviews. Associations between domestic violence and study variables were examined by logistic regression analysis. Among the study participants, 31.4%experienced some form of domestic violence during pregnancy, with psychological abuse (16.4%) being the most prevalent followed by sexual abuse (9.5%) and physical violence (1.8%). Women whose husbands were unemployed were three times more likely to experience domestic violence (aOR; 3.3, 95%CI: 1.6, 7.0). Similarly, husband's substance abuse was significantly associated with domestic violence during pregnancy (aOR: 120.8, 95%CI: 57.9, 252.2). Domestic violence was a significant predictor of adverse maternal outcomes, including anemia, preterm labor, and recurrent abdominal pain. Similarly, domestic violence was a significant predictor of NICU admission (aOR: 20.6, 95%CI: 12.6, 33.8) and low birthweight (aOR: 24.4, 95%CI: 10.2, 58.4). The study findings emphasize the urgent need for comprehensive interventions targeting domestic violence among women and preventing its cascading effects on pregnancies.

Abstract:Domestic violence is a serious threat to the health of women in the world and derives from several factors. Therefore, due to the importance of this issue, this study aimed to determine domestic violence against women in four educational hospitals in Iran as a Muslim country. The study employed a cross-sectional design and was conducted in four educational hospitals supervised by the Kerman University of Medical Sciences in 2016. Using a researcher-made questionnaire, we assessed factors associated with domestic violence in female employees using a census method ( N = 400). Data were analyzed using descriptive statistics including mean and SD and analytic statistics such as Kolmogorov–Smirnov, ANOVA, t test, and Pearson and multivariate regression tests using SPSS 16 and p ≤ .05. This study showed that most common types of violence against women are psychological/verbal (58%), physical (29.25%), and sexual (10%), respectively. There was a significant relationship between couples’ age gap, forced marriage, husband addiction, income, and history of violence experienced by the husband with domestic violence against women. This study examines the basic prevalence of partner violence victimization among Iranian women who work in hospitals in southeast Iran. Findings suggest that national and local policies in Iran may need to examine factors that contribute to violence against women as well as focusing on how to reduce partner violence.

Abstract:Background: Intimate partner violence in pregnancy is a significant public health issue that has several detrimental effects. Pregnant women subjected to intimate partner violence (IPV) have a higher risk for adverse pregnancy outcomes. Objective: The aim of the study was to determine the prevalence, patterns and factors associated with intimate partner violence among pregnant women attending a primary care antenatal clinic. Methods: A quantitative cross-sectional study design was employed to study intimate partner violence among 269 pregnant women in Accra, Ghana between July and October 2021. Participants were selected by systematic sampling technique. The self-reported Composite Abuse Scale was used to assess and classify intimate partner violence. Socio-demographic, clinical (obstetric) and behavioural characteristics were obtained with a structured questionnaire. Associations were determined between independent and dependent variables using the chi-squared test, and logistic regression with adjusted odds ratio (AOR). The statistical significance level was set at a p-value ≤ 0.05. Results: The prevalence of IPV was 11.2%. The prevalence of emotional/psychological abuse, harassment/controlling behaviour, physical abuse, sexual abuse and severe combined abuse were 12.3%, 13.0%, 8.2%, 3.3% and 8.9% respectively. Pregnant women who were employed had reduced odds of experiencing IPV [AOR = 0.16 (95%CI: 0.05-0.47), p = 0.001], however, the past experience of violence [AOR = 4.9 (95%CI: 1.06-22.96), p = 0.042], alcohol use by women [AOR = 7.8 (95%CI: 1.63-37.42), p = 0.01], and partners' alcohol consumption [AOR = 10.0 (95%CI: 3.22-31.26), p<0.001] were associated with increased odds of IPV. Conclusions: There is a high prevalence of IPV among pregnant women in this study from a resource-limited setting. The factors found to be associated with IPV in pregnancy were the employment status of women, alcohol consumption by women or their partners and a previous history of violence. Healthcare providers in primary care need to recognize IPV as a medical condition that can occur in pregnancy and be ready to assist and manage the victims when it is detected.
Violence against women by their intimate partners in Shahroud in northeastern region of Iran

Abstract:Background: Violence against women is one of the worst consequences of cultural, political, and socio-economic inequalities between men and women. Intimate Partner Violence (IPV) has been identified as an important cause of morbidity from multiple mental, physical, sexual, and reproductive health outcomes. Nonetheless, the prevalence and related factors of this international problem have not been investigated extensively in some parts of the world. The aims of this research were to determine the prevalence of physical and mental violence perpetrated by men against their intimate partners and to assess the associated factors of partner violence among women in Shahroud in northeastern region of Iran in 2010. Methods: This Cross-Sectional study was conducted in Shahroud, in northeast of Iran in 2010. Cluster sampling was done from primary health service institutions, universities, public schools and governmental organizations throughout the city and six hundred married women completed the study. A structured questionnaire with 34 items was designed in three parts to assess the physically (10 items) and mentally (15 items) violent acts by a current intimate male partner and identify collative behaviors (9 items) of victims. The Logistic regression analysis was applied to determine the net effect of background variables on the IPV occurrence within the past year. Results: About 20% of the participants experienced at least one type of physical violence. Increased risk of physical violence was positively associated with the younger age of the couple (OR=3.08, P<0.05), lower education (OR=2.28, P<0.01) and having a semi-manual skilled occupation of husband (OR=3.62, P<0.05), husband's heavy cigarette smoking (OR=2.62, P<0.01), and his drug abuse (OR=2.1, P<0.05). About 85% of the women had experienced mental harassment within the past twelve months. Logistic Regression Analysis found that lower education (OR=3.06, P<0.01) and having semi-manual skilled occupation (OR=3.8, P<0.05) of husband, increasing years of marriage (OR=2.8, P<0.01), husband's heavy cigarette smoking (OR=2.3, P<0.01) and his abusing the use of drugs (OR=3.4, P<0.01) had significant associations with women's experience of mental violence. Conclusions: Some socioeconomic characteristics such as educational level, occupational status of men, heavy smoking and drug abusing are associated with the occurrence of violence against one's intimate partner. Since IPV is usually unreported, healthcare providers should be aware of the risk factors associated with domestic violence to be able to design preventive measures against its negative health outcomes in women.

Abstract:Abstract Background Some women avoid sexual intercourse during pregnancy due to the physiological changes they undergo during this period as well as their fear of causing harm to the fetus and to themselves, which can lead to problems in sexual health. The aim of the present study was to investigate the effects of a sexual health education package on the dimensions of sexual health in pregnant women. Methods This randomized, longitudinal, clinical trial was carried out in 2018–2019 on 154 pregnant women in early to late pregnancy who presented to comprehensive health centers in Rasht, Iran, and were divided into three groups: Group A or the training group (50 participants), Group B or the self-training group (53 participants), and Group C or the control group (51 participants). The study tools included the Pregnancy Sexual Response Inventory (PSRI), the Sexual Quality of Life-Female (SQOL-F) and the Sexual Violence Questionnaire. The dimensions of sexual health were examined before beginning each intervention in each trimester of pregnancy and then at the end of pregnancy using these questionnaires. The collected data were analyzed using statistical tests, namely the Chi-square test, one-way ANOVA, Cochrane’s test, and the repeated measures ANOVA at a significance level of P < 0.05. Results There was no statistically significant difference in the mean total scores of SQOL-F and PSRI in the three groups at baseline. As for the intergroup results, there was a statistically significant difference in the mean score of SQOL-F and PSRI at the end of pregnancy. The mean scores of PSRI and SQOL-F in the training group (Group A) increased from the beginning to the end of pregnancy compared to the control and self-training groups. As for the intergroup comparisons, there was no statistically significant difference in the mean total scores of sexual violence among the pregnant women in the different groups in the third trimester of pregnancy and at the end of the third trimester. Although sexual violence was not statistically significant, the number of sexually-violated women in the training group decreased during the training period compared to the self-training and control groups. Conclusion The results obtained in the intervention group compared to the control group revealed the effectiveness of the sexual health education package in terms of improvement in the dimensions of sexual health. According to the results, in order to maintain and promote the sexual health of pregnant women, health care providers are recommended to offer sexual health training during pregnancy along with other health care services. Abstract:Journal of Interpersonal Violence, Ahead of Print. Violence against women is considered as one of the most serious social problems in all the societies. It is common that the abused women experience physical, psychological, and health problems, as well as reproductive health issues. Domestic violence affects women's health behaviors and ability to seek any form of health care. This study aimed to investigate the relationship between health-promoting behaviors and reproductive health needs in women experienced domestic violence. This cross-sectional study was conducted from May 5 to September 21, 2021, on 380 abused women. Cluster sampling was performed in health centers of Karaj. Data were collected using demographic survey questions, Domestic Violence Survey, Reproductive Health Needs of Domestic Violated Women scale, and health-promoting behaviors questionnaire. The mean (standard deviation) scores for reproductive health needs and health-promoting behaviors were 158.88 (20.24) and 131.08 (20.53), respectively. Psychological violence had the highest (69.5%) prevalence among all types and 37.6% of women reported severe violence. The results of Spearman's rank correlation coefficient test indicated that all dimensions of the reproductive health needs of abused women (men's participation, self-care, support and health services, and sexual and marital relationships) positively and significantly related to the total score and various dimensions of health-promoting behaviors (interpersonal relationship, health responsibility, physical activity, spiritual growth, nutrition, and stress management). The dimensions of health-promoting behaviors together can explain 21.6% of the changes in reproductive health needs based on linear regression. Violence is a global public concern and paying attention to the various health dimensions in abused women is an important aspect of the health policies. Developing health-promoting behaviors in abused women improves their reproductive health state and society as a whole.

Physical violence during pregnancy in sub-Saharan Africa: why it matters and who are most susceptible?

Abstract:Objective The study assessed the prevalence of physical violence against pregnant women and its associated factors in sub-Saharan Africa (SSA). Design We analysed cross-sectional data of 108971 women in sexual unions from the most recent Demographic and Health Surveys of 26 countries in SSA. The predictors of physical violence were examined using a multilevel binary logistic regression. All the results were presented as adjusted odds ratios (aORs) with their corresponding 95% confidence intervals (CIs). Setting Twenty-six countries in SSA. Participants 108 971 women who had ever been pregnant. Outcome measure Physical violence during pregnancy. Results Physical violence was identified in 6.0% of pregnant women in SSA. The highest prevalence (14.0%) was reported in South Africa, while Burkina Faso recorded the lowest (2.1%). Women who had primary (aOR=1.26, 95% CI=1.15, 1.38) and secondary education (aOR=1.15, 95% CI=1.01,1.32); those who were cohabiting (aOR=1.21, 95% CI=1.11, 1.32); those who were working (aOR=1.17, 95% CI=1.08, 1.28); and those whose partners had primary (aOR=1.15, 95% CI=1.04, 1.28) and secondary education (aOR=1.14, 95% CI=1.01, 1.28) were more likely to experience physical violence during pregnancy compared with those who had no formal education; those who were married; those who were not working, and those whose partners had no formal education, respectively. Moreover, women whose partners consumed alcohol (aOR=2.37, 95% CI=2.20, 2.56); those who had parity of four or more (aOR=2.06, 95% CI=1.57, 2.72); and those who perceived intimate partner violence (IPV) as a culturally accepted norm (aOR=1.55, 95% CI=1.44, 1.67) had higher odds of experiencing physical violence during pregnancy compared to those whose partners did not consume alcohol, those with parity zero, and those who did not perceive IPV as culturally accepted, respectively. On the contrary, women who were aged 35-39, those who were of the richest wealth index, and those in rural areas had reduced odds of experiencing physical violence during pregnancy. Conclusion Based on the findings, community leaders are encouraged to liaise with law enforcement agencies to strictly enforce laws on gender-based violence by prosecuting perpetrators of IPV against pregnant women as a deterrent. Also, intensifying education on what constitutes IPV and the potential consequences on the health of pregnant women, their children, and their families will be laudable. Improving the socioeconomic status of women may also help to eliminate IPV perpetration against women at their pregnancy stage.

Abstract:Introduction: Intimate partner violence (IPV) against women is a serious public health issue and refers to physically, sexually and psychologically harmful behaviors as well as emotionally controlling behaviors and financial abuse that occur in the form of marriage or cohabitation. Knowing the current situation of the IPV prevalence against women and high-risk areas in the Zanjan city, Iran, can help policymakers to establish better health programs for risk reduction. Methods: This population-based cross-sectional study consisted of married women aged 18–55 years living in Zanjan city in 2021. 760 married women covered by 19 urban comprehensive health service centers (UCHSCs) were selected by the stratified systematic random sampling method. The prevalence of IPV against women was measured in four types: psychological, physical, sexual, and economic. Results: Mean (SD) age of the women was 35.49 (8.76) years. 606 women (79.7%) experienced one type of IPV. The highest and lowest IPV prevalence against women were psychological (76.6%) and economic (12%), respectively. The highest and lowest prevalence of psychological violence were observed in CUHSCs 2 and 17, physical violence in CUHSCs 1 and 14, sexual violence in CUHSCs 2 and 17, and economic violence in CUHSCs 2 and 8, respectively. The severity of violence was higher among self-employment or workers husbands, with low monthly household income, and among younger women. Discussion: The IPV rate in the target population is high, and the highest rate is related to psychological violence. These results highlight the need to intervention in the society and high-risk women for policymakers of the health system.
