Abstract:To the editor: It is well known that outgrowth of collateral sprouts is a natural process during regeneration of the peripheral nervous system. However, current knowledge about this phenomenon was that aberrant growth of axonal sprouts may lead to the formation of persistent neuromas.1,2 From our previous work3-5 verified effectively in clinical environment for nerve repairing and related experiments by small gap sleeve neurorrhaphy, we made a possible inference for peripheral nerve collateral regeneration and functional repairing; the hypothesis of using these collateral sprouts as an auto graft resource to recover the injured nerve. This study focuses on the multiplication limit of the collaterals that could be maintained during peripheral nerve regeneration, and the possibility of using partial axons from an intact nerve to repair a nearby nerve with little influence on the donor nerve disfunction. Adult rats were used in this study. The tibial nerve was excised. The proximal residue of the tibial nerve segment, with variable number of axons serves as donor nerves, bridges to the distal stump of the same tibial nerve segment using biodegradable chitin conduit. The number of myelinated axons was calculated after 12 weeks. An increasing ratio of regenerative myelinated axon numbers to proximal donor axon numbers of 0.98, 1.51, 2.39, 2.89, respectively, was found with an estimated maximum value of approximately 3.3 (Figure 1 A-1C). It was the first time we got direct evidence that the sprouting collaterals could develop to full-grown neural fibers.3-5Figure 1.: Collateral multiplication limit of peripheral nerve. Histological sections through the reconstructed nerves 6 months after surgery (F).We also investigated whether delayed repair might affect the multiplication capacity of the proximal donor nerve when distal receptor nerves were degenerated, which was like the conditions of clinical cases. The distal tibial nerve stumps as receptor nerve were denervated for 1, 2, 4, and 8 weeks before being repaired with partial freshly transected proximal common peroneal nerve stumps. The multiplication ratio was 2.33, 2.32, 2.25, and 1.45, respectively (Figure 1 D). It suggested that starting repair at 8 weeks after degeneration could decrease peripheral nerve multiplication capacity.8 Rhesus monkeys were used as our research subjects. The anterior pronator teres nerve (the donor) was used to repair the ulnar never (the receptor). Six months later, the motor nerve conduction velocity was (22.63±6.34) m/s, and the fiber number of donor and receptor nerve was 1657±652 and 2661±843 respectivey. The multiplication ration was 1.61 (Figure 1E and 1F). These studies show an important biological value of collateral sprouting in peripheral nerve regeneration. Yet it is still ignored. From these study results, the collateral sprouts might be a new donor nerve resource in clinical nerve repair. Also, based on this study, it might be practical to use a small nerve to repair a big nerve trunk. Moreover, it is possible to use one nerve to repair several injured nerves simultaneously. These choices will undoubtedly improve the repair methods of peripheral nerve injury. However, there are still many unknown questions to our findings. For example, what are the changes of the proximal neuron body when it sprouts many collaterals in its distal axon? How could a neuron receive a message and send impulses through such an 1-axon trunk several-collateral model? How the brain and spinal cord remodel when multiplication happened? All of these questions need to be researched in the future.
