Abstract:BackgroundEpicardial adipose tissue (EAT) is directly related to coronary artery disease (CAD), but little is known about its role in hemodynamically significant CAD. Therefore, our goal is to explore the impact of EAT volume on hemodynamically significant CAD.MethodsPatients who underwent coronary computed tomography angiography (CCTA) and received coronary angiography within 30 days were retrospectively included. Measurements of EAT volume and coronary artery calcium score (CACs) were performed on a semi-automatic software based on CCTA images, while quantitative flow ratio (QFR) was automatically calculated by the AngioPlus system according to coronary angiographic images.ResultsThis study included 277 patients, 112 of whom had hemodynamically significant CAD and showed higher EAT volume. In multivariate analysis, EAT volume was independently and positively correlated with hemodynamically significant CAD [per standard deviation (SD) cm3; odds ratio (OR), 2.78; 95% confidence interval (CI), 1.86–4.15; P < 0.001], but negatively associated with QFRmin (per SD cm3; β coefficient, −0.068; 95% CI, −0.109 to −0.027; P = 0.001) after adjustment for traditional risk factors and CACs. Receiver operating characteristics curve analysis demonstrated a significant improvement in predictive value for hemodynamically significant CAD with the addition of EAT volume to obstructive CAD alone (area under the curve, 0.950 vs. 0.891; P < 0.001).ConclusionIn this study, we found that EAT volume correlated substantially and positively with the existence and severity of hemodynamically significant CAD in Chinese patients with known or suspected CAD, which was independent of traditional risk factors and CACs. In combination with obstructive CAD, EAT volume significantly improved diagnostic performance for hemodynamically significant CAD, suggesting that EAT could be a reliable noninvasive indicator of hemodynamically significant CAD.
Epicardial adipose tissue characteristics, obesity and clinical outcomes in COVID-19: A post-hoc analysis of a prospective cohort study

Abstract:Background and aims: Obesity-related cardiometabolic risk factors associate with COVID-19 severity and outcomes. Epicardial adipose tissue (EAT) is associated with cardiometabolic disturbances, is a source of proinflammatory cytokines and a marker of visceral adiposity. We investigated the relation between EAT characteristics and outcomes in COVID-19 patients. Methods and results: This post-hoc analysis of a large prospective investigation included all adult patients (≥18 years) admitted to San Raffaele University Hospital in Milan, Italy, from February 25th to April 19th, 2020 with confirmed SARS-CoV-2 infection who underwent a chest computed tomography (CT) scan for COVID-19 pneumonia and had anthropometric data available for analyses. EAT volume and attenuation (EAT-At, a marker of EAT inflammation) were measured on CT scan. Primary outcome was critical illness, defined as admission to intensive care unit (ICU), invasive ventilation or death. Cox regression and regression tree analyses were used to assess the relationship between clinical variables, EAT characteristics and critical illness. One-hundred and ninety-two patients were included (median [25th-75th percentile] age 60 years [53-70], 76% men). Co-morbidities included overweight/obesity (70%), arterial hypertension (40%), and diabetes (16%). At multivariable Cox regression analysis, EAT-At (HR 1.12 [1.04-1.21]) independently predicted critical illness, while increasing PaO2/FiO2 was protective (HR 0.996 [95% CI 0.993; 1.00]). CRP, plasma glucose on admission, EAT-At and PaO2/FiO2 identified five risk groups that significantly differed with respect to time to death or admission to ICU (log-rank p < 0.0001). Conclusion: Increased EAT attenuation, a marker of EAT inflammation, but not obesity or EAT volume, predicts critical COVID-19. Trial registration: NCT04318366.

Abstract:Abstract Background Both visceral adipose tissue and epicardial adipose tissue (EAT) have pro-inflammatory properties. The former is associated with Coronavirus Disease 19 (COVID-19) severity. We aimed to investigate whether an association also exists for EAT. Material and methods We retrospectively measured EAT volume using computed tomography (CT) scans (semi-automatic software) of inpatients with COVID-19 and analyzed the correlation between EAT volume and anthropometric characteristics and comorbidities. We then analyzed the clinicobiological and radiological parameters associated with severe COVID-19 (O2 $$\ge$$ ≥ 6 l/min), intensive care unit (ICU) admission or death, and 25% or more CT lung involvement, which are three key indicators of COVID-19 severity. Results We included 100 consecutive patients; 63% were men, mean age was 61.8 ± 16.2 years, 47% were obese, 54% had hypertension, 42% diabetes, and 17.2% a cardiovascular event history. Severe COVID-19 (n = 35, 35%) was associated with EAT volume (132 ± 62 vs 104 ± 40 cm 3 , p = 0.02), age, ferritinemia, and 25% or more CT lung involvement. ICU admission or death (n = 14, 14%) was associated with EAT volume (153 ± 67 vs 108 ± 45 cm 3 , p = 0.015), hypertension and 25% or more CT lung involvement. The association between EAT volume and severe COVID-19 remained after adjustment for sex, BMI, ferritinemia and lung involvement, but not after adjustment for age. Instead, the association between EAT volume and ICU admission or death remained after adjustment for all five of these parameters. Conclusions Our results suggest that measuring EAT volume on chest CT scans at hospital admission in patients diagnosed with COVID-19 might help to assess the risk of disease aggravation.

Abstract:Recent epidemiological studies have demonstrated that common cardiovascular risk factors are strongly associated with adverse outcomes in COVID-19. Coronary artery calcium (CAC) and epicardial fat (EAT) have shown to outperform traditional risk factors in predicting cardiovascular events in the general population. We aim to determine if CAC and EAT determined by Computed Tomographic (CT) scanning can predict all-cause mortality in patients admitted with COVID-19 disease. We performed a retrospective, post-hoc analysis of all patients admitted to Montefiore Medical Center with a confirmed COVID-19 diagnosis from March 1st, 2020 to May 2nd, 2020 who had a non-contrast CT of the chest within 5 years prior to admission. We determined ordinal CAC scores and quantified the epicardial (EAT) and thoracic (TAT) fat volume and examined their relationship with inpatient mortality. A total of 493 patients were analyzed. There were 197 deaths (39.95%). Patients who died during the index admission had higher age (72, [64–80] vs 68, [57–76]; p < 0.001), CAC score (3, [0–6] vs 1, [0–4]; p < 0.001) and EAT (107, [70–152] vs 94, [64–129]; p = 0.023). On a competing risk analysis regression model, CAC ≥ 4 and EAT ≥ median (98 ml) were independent predictors of mortality with increased mortality of 63% (p = 0.003) and 43% (p = 0.032), respectively. As a composite, the group with a combination of CAC ≥ 4 and EAT ≥ 98 ml had the highest mortality. CAC and EAT measured from chest CT are strong independent predictors of inpatient mortality from COVID-19 in this high-risk cohort.

Abstract:Objectives: Epicardial adipose tissue (EAT) is a type of visceral adipose tissue with pro-inflammatory properties. We sought to examine the relationship between the EAT volume and attenuation measured on non-contrast chest computed tomography (CT), inflammation markers, and the severity of COVID-19 pneumonia.Methods: One hundred and twenty-five patients who are over 18 years old who applied to our hospital and were found to have COVID-19 polymerase chain reaction (+) on nasopharyngeal swab sample and COVID-19 pneumonia on chest CT were included in the study. At admission, C-reactive protein (CRP), procalcitonin, fibrinogen, leukocytes, neutrophil-lymphocyte ratio, platelet-lymphocyte ratio, lactate dehydrogenase (LDH), ferritin, and d-dimer were evaluated. EAT volume and attenuation were measured on chest CT. Patients who were hospitalized and discharged from the ward were categorized as Group 1, whereas patients who required intensive care admission and/or died were classified as Group 2. The primary endpoint of our study was defined as death, hospitalization in the intensive care unit, and discharge. The relationship between disease severity and EAT and other inflammatory markers was investigated.Results: One hundred and six individuals were in Group 1 and 19 patients were in Group 2. Of the 125 individuals, 46 were women and 79 were men. The mean age was 58.5±15.9 years. Group 2 patients were older. Regarding measurements of the EAT volume and attenuation; there was no statistically significant difference between the groups determined. The patients in Group 2 had statistically substantially higher values for urea, creatinine, LDH, d-dimer, troponin T, procalcitonin, CRP, and neutrophil/lymphocyte ratio in their laboratory tests. When compared to patients in Group 1, patients in Group 2 had statistically significantly lower albumin values (p<0.001). In obese patients, EAT volume was statistically significantly higher and EAT attenuation was found to be lower.Conclusion: In our study, no relationship was found between critical COVID-19 disease and EAT volume and attenuation, which is an indicator of EAT inflammation. Inflammatory markers from routine laboratory tests can be used to predict critical COVID-19 disease. No relationship was found between obesity and critical COVID-19 disease.

Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Background & Aims</h3><p>Adipose tissue is a biologically active organ with pro-immunogenic properties. We aimed to evaluate the prognostic value of epicardial adipose tissue (EAT) in COVID-19 and its correlation with other inflammatory biomarkers.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Material and Methods</h3><p>One-hundred patients with COVID-19 were enrolled. C-reactive protein (CRP), lactate dehydrogenase (LDH), neutrophil-to-lymphocyte ratio (NLR), lymphocyte-to-CRP ratio (LCR), and platelet-to-lymphocyte ratio (PLR) were evaluated on admission. EAT volume and density were measured by computed tomography. Patients were followed until death or discharge. Univariate and multivariate analysis was performed and ROC curve analysis was used to assess the ability of inflammatory markers in predicting survival. The relationship between EAT and other inflammatory markers was also investigated.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>The mean ± SD age of patients was 55.5 ± 15.2 years old; 68% were male. Univariate analysis revealed that increased lung involvement, blood urea nitrogen, LDH and NLR, and decreased platelet count were significantly associated with death. After adjustment, LDH was independently predictive of death (OR= 1.013, p-value= 0.03). Among inflammatory markers, LCR had the best ability for predicting survival with 79.7% sensitivity and 64.3% specificity at an optimal cut-off value of 20.8 (AUC=0.744, 95% CI= 0.612 - 0.876, p-value= 0.004). EAT volume demonstrated positive correlation with NLR and PLR (p=0.001 and 0.01), and a negative correlation with LCR (p=0.02). EAT density was significantly different between decedents and survivors (p=0.008).</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusion</h3><p>Routine laboratory tests that represent status of inflammation can be used as cost-effective prognostic markers of COVID-19. Also, the significant association between EAT volume and other inflammatory biomarkers might explain the more severe disease in obese patients.</p>

Abstract:Background: Many people infected with COVID-19 develop myocardial injury. Epicardial adipose tissue (EAT) is among the various risk factors contributing to coronary artery disease. However, its correlation with myocardial injury in patients diagnosed with COVID-19 remains uncertain. Methods : We examined myocardial biomarkers in population affected by COVID-19 during the period from December 2022 to January 2023. The patients without myocardial injury were referred to as group A ( n = 152) and those with myocardial injury were referred to as group B ( n = 212). Results: 1) The A group and the B group exhibitedstatistically significant differences in terms of age, TC, CRP, Cr, BUN, LDL-C, IL-6, BNP, LVEF and EAT ( p < 0.05). 2) EAT volumehad a close relationship with IL-6, LDL-C, cTnI, and CRP ( p < 0.05); the corresponding correlation coefficient values were 0.24, 0.21, 0.24, and 0.16. In contrast to those with lower EAT volume, more subjects with a higher volume of EAT had myocardial injury ( p < 0.05). Regression analysis showed that EAT, LDL-C, Age and Cr were established as independent risk variables for myocardial injury in subjects affected by COVID-19. 3) In COVID-19 patients, the likelihood of myocardial injury rised notably as EAT levels increase ( p < 0.001). Addition of EAT to the basic risk model for myocardial injury resulted in improved reclassification. (Net reclassification index: 58.17%, 95% CI: 38.35%, 77.99%, p < 0.001). Conclusion: Patients suffering from COVID-19 with higher volume EAT was prone to follow myocardial injury and EAT was an independent predictor of heart damage in these individuals.

Abstract:Objectives: To assess the prognostic significance of epicardial adipose tissue volume (EATv) and pericoronary adipose tissue attenuation (PCATa) in patients with heart failure with preserved ejection fraction (HFpEF). Methods: This retrospective study was based on HFpEF and controls who underwent coronary computed tomography angiography (CCTA) screening to rule out coronary disease. Comparisons of EATv and PCATa were made between HFpEF patients and a control group, employing statistical analyses including Kaplan-Meier and Cox regression to assess prognostic significance. Results: A total of 224 patients were retrospectively analyzed. The EATv was 56.1 cm3 ± 11.9 and PCATa in the right coronary artery (PCATa-RCA) was -74.7 HU ± 3.82 in HFpEF patients, which increased significantly compared with controls. Among them, 112 HFpEF patients (mean age: 71.9 ± 8.5 years; 40% male) were followed up for a median of 27 ± 0.6 months (range 2-47 months). EATv and PCATa-RCA were predictive of outcome with an optimal threshold of 56.29 cm3 and -71.17 HU, respectively. In Kaplan-Meier analysis, the high EATv and PCATa-RCA attenuation had significantly higher rates of composite outcomes (log-rank test, all P < 0.01). EATv and PCATa-RCA were independently predictive of outcome following adjustment for confounding variables (EATv: hazard ratio [HR] 1.03; 95% confidence interval [CI] (1.01-1.06); p < 0.01, PCTAa-RCA: HR 1.44; 95% CI 1.27-1.62; p < 0.001)). Conclusions: Increased EATv and PCATa-RCA are associated with worse clinical outcomes in HFpEF patients. Advances in knowledge: This study highlights the potential of CCTA-derived adipose tissue metrics as novel, non-invasive biomarkers for risk stratification in HFpEF.
Incremental Value of Radiomics Features of Epicardial Adipose Tissue for Detecting the Severity of COVID-19 Infection

Abstract:Introduction: Epicardial adipose tissue (EAT) is known for its pro-inflammatory properties and association with Coronavirus Disease 2019 (COVID-19) severity. However, existing detection methods for COVID-19 severity assessment often lack consideration of organs and tissues other than the lungs, which limits the accuracy and reliability of these predictive models. Material and methods: The retrospective study included data from 515 COVID-19 patients (Cohort 1, n=415; Cohort 2, n=100) from two centers (Shanghai Public Health Center and Brazil Niteroi Hospital) between January 2020 and July 2020. Firstly, a three-stage EAT segmentation method was proposed by combining object detection and segmentation networks. Lung and EAT radiomics features were then extracted, and feature selection was performed. Finally, a hybrid model, based on seven machine learning models, was built for detecting COVID-19 severity. The hybrid model's performance and uncertainty were evaluated in both internal and external validation cohorts. Results: For EAT extraction, the Dice similarity coefficients (DSC) of the two centers were 0.972 (±0.011) and 0.968 (±0.005), respectively. For severity detection, the area under the receiver operating characteristic curve (AUC), net reclassification improvement (NRI), and integrated discrimination improvement (IDI) of the hybrid model increased by 0.09 (p<0.001), 19.3 % (p<0.05), and 18.0 % (p<0.05) in the internal validation cohort, and by 0.06 (p<0.001), 18.0 % (p<0.05) and 18.0 % (p<0.05) in the external validation cohort, respectively. Uncertainty and radiomics features analysis confirmed the interpretability of increased certainty in case prediction after inclusion of EAT features. Conclusion: This study proposed a novel three-stage EAT extraction method. We demonstrated that adding EAT radiomics features to a COVID-19 severity detection model results in increased accuracy and reduced uncertainty. The value of these features was also confirmed through feature importance ranking and visualization.
Greater pericardial and visceral adiposity increase risk of hospitalization or death from SARS-CoV-2 infection in community dwelling adults: A C4R Study

Abstract:Importance: Higher body mass index (BMI) increases risk of respiratory failure and death from SARS-CoV-2 infection. BMI is imprecise and fails to account for adipose tissue distribution. Objective: To determine whether pericardial adipose tissue (PAT), abdominal visceral adipose tissue (VAT), subcutaneous adipose tissue (SAT), or hepatic adipose deposition on pre-pandemic computed tomography (CT) scans associate with increased risk of hospitalization or death from SARS-CoV-2. Design: The Collaborative Cohort of Cohorts for COVID-19 Research (C4R) ascertained SARS-CoV-2 outcomes among participants from 14 US-based cohort studies. This analysis includes participants enrolled in the Jackson Heart, CARDIA, MESA, and Framingham Heart studies. Participants: C4R attempted to enroll all cohort participants who were alive on March 1, 2020 and had not withdrawn consent for cohort participation. Exposures: Adipose depot size measured on research CT scans performed between 2000 and 2011. Outcome: Time from March 1, 2020 to hospitalization or death from SARS-CoV-2 infection ascertained by self-report or active surveillance and confirmed when available, by protocolized record review. Results: There were 8412 participants with at least one CT adipose measure, among whom 184 events occurred over a median (interquartile range) of 547 (338-609) days. Participants were 57% female with median (SD) age of 69.1 (10.4) years. In adjusted models, both higher PAT (per doubling in PAT volume, HR 1.62, 95% CI 1.25-2.09) and higher VAT (per 1-standard deviation, HR 1.41, 95% CI 1.20-1.67) were associated with greater hazards of hospitalization or death from SARS-CoV-2. Associations remained after adjustment for BMI, diabetes, cardiovascular disease, and hypertension. SAT was associated with hospitalization or death from SARS-CoV-2 but not after adjustment for BMI, diabetes, cardiovascular disease or hypertension. Hepatic adipose deposition was not associated with hospitalization or death. Associations did not vary by sex, age, race, cohort, smoking status, diabetes, or BMI. Conclusions and Relevance: Greater CT-measured pericardial and abdominal visceral adipose tissue were associated with increased hazards of hospitalization or death from SARS-CoV-2 independent of BMI or clinical cardiometabolic comorbidities. Further research should investigate use of CT adipose measures to quantify risk of severe disease from viral infections and to investigate mechanisms linking adiposity to lung injury.

Abstract:BackgroundThere appears to be an association of epicardial adipose tissue (EAT) with coronary artery disease (CAD) and its risk factors. EAT is assumed to influence CAD development by altering vasomotor tone and via toxic paracrine effects. The relationship of EAT to myocardial perfusion has not been studied.MethodsQuantification of EAT and CAC was performed on positron emission tomography/computed tomography (PET/CT) studies in 45 subjects (77% intermediate pre-test probability of CAD) with mild-moderate myocardial ischemia (5-14% perfusion defect, n = 23), severe ischemia (≥15% defect, n = 22) and a control group with no ischemia matched for CAD risk factors (n = 52).ResultsEAT volume showed a better correlation with myocardial ischemia than total CAC (r = .47 vs r = .28, P < .01). EAT volume increased significantly from the control group to subjects with mild-moderate and severe ischemia (96.9, 124.5, and 143.9 cm3, P < .01 for both ischemia groups vs controls). Total mean CAC was significantly higher in the severe ischemia group (676.3) than in control group (229.4) (P < .01). Multivariable logistic regression analyses showed that EAT volume was, but CAC was not, a significant predictor of ischemia after adjustment for age, sex, body mass index, and each other. EAT volume was a better predictor of ischemia than total CAC [area under the curve (AUC): .764 vs .6291, P = .04]. The combination of EAT + CAC (AUC = .7694) did not improve over EAT volume alone (P = .57).ConclusionsIn this study, EAT volume assessed by CT was an independent predictor of ischemia on PET, and outperformed CAC score in a CAD naïve population at intermediate pre-test probability of disease.

Abstract:Background: Myocardial injury is a life-threatening complication of coronavirus disease 2019 (COVID-19). Pre-existing health conditions and early morphological alterations may precipitate cardiac injury and dysfunction after contracting the virus. The current study aimed at assessing potential risk factors for COVID-19 cardiac complications in patients with pre-existing conditions and imaging predictors. Methods and Results: The multi-center, retrospective cohort study consecutively enrolled 400 patients with lab-confirmed COVID-19 in six Chinese hospitals remote to the Wuhan epicenter. Patients were diagnosed with or without the complication of myocardial injury by history and cardiac biomarker Troponin I/T (TnI/T) elevation above the 99th percentile upper reference limit. The majority of COVID-19 patients with myocardial injury exhibited pre-existing health conditions, such as hypertension, diabetes, hypercholesterolemia, and coronary disease. They had increased levels of the inflammatory cytokine interleukin-6 and more in-hospital adverse events (admission to an intensive care unit, invasive mechanical ventilation, or death). Chest CT scan on admission demonstrated that COVID-19 patients with myocardial injury had higher epicardial adipose tissue volume ([EATV] 139.1 (83.8–195.9) vs. 92.6 (76.2–134.4) cm 2 ; P = 0.036). The optimal EATV cut-off value (137.1 cm 2 ) served as a useful factor for assessing myocardial injury, which yielded sensitivity and specificity of 55.0% (95%CI, 32.0–76.2%) and 77.4% (95%CI, 71.6–82.3%) in adverse cardiac events, respectively. Multivariate logistic regression analysis showed that EATV over 137.1 cm 2 was a strong independent predictor for myocardial injury in patients with COVID-19 [OR 3.058, (95%CI, 1.032–9.063); P = 0.044]. Conclusions: Augmented EATV on admission chest CT scan, together with the pre-existing health conditions (hypertension, diabetes, and hyperlipidemia) and inflammatory cytokine production, is associated with increased myocardial injury and mortality in COVID-19 patients. Assessment of pre-existing conditions and chest CT scan EATV on admission may provide a threshold point potentially useful for predicting cardiovascular complications of COVID-19.

Abstract:Background: Epicardial adipose tissue (EAT) is closely related to coronary artery disease (CAD). Hemodynamically significant CAD has a worse prognosis and is more likely to benefit from revascularization. However, the specific relationship between EAT and hemodynamically significant CAD remains unclear.Methods: A total of 164 inpatients received single-photon emission computerized tomography-myocardial perfusion imaging (SPECT/MPI) and coronary angiography (CAG) between March 2018 and October 2019 at the Third Affiliated Hospital of Soochow University were enrolled in the retrospective cross-sectional study. Data on body mass index (BMI), hypertension, hyperlipidemia, diabetes mellitus (DM), active smoking, and symptoms were gathered. Epicardial fat volume (EFV) and coronary artery calcium (CAC) were quantified by noncontrast computed tomography (CT). Hemodynamically significant CAD was defined by coronary stenosis severity ≥50% with reversible perfusion defects in the corresponding areas of SPECT/MPI.Results: A total of 37.8% of patients had hemodynamically significant CAD. Age and BMI increased with tertiles of EFV (P for trend =0.009 and P<0.001). The ratios of hemodynamically significant CAD in EFV from low to high were 16.4%, 37.0%, and 60.0%, respectively (P for the trend <0.001). In univariate regression analysis, EFV was associated with hemodynamically significant CAD [odds ratio (OR) per 10 cm3 =1.36; 95% confidence interval (CI): 1.20-1.55; P<0.001]. After correcting for traditional risk factors and CAC, EFV was firmly linked to hemodynamically significant CAD (OR per 10 cm3 =1.53; 95% CI: 1.25-1.88; P<0.001). With an increasing trend in EFV for the tripartite groups, the likelihood of hemodynamically significant CAD increased significantly (P for trend <0.001). There was a saturation effect between EFV and hemodynamically significant CAD according to the generalized additive model (GAM). When EFV <134.43 cm3, EFV was linearly correlated with hemodynamically significant CAD (OR per 10 cm3 =2.06; 95% CI: 1.45-2.94; P<0.001). When EFV ≥134.43 cm3, the hemodynamically significant CAD risk was steeper and gradually reached saturation. Hypertension affected the relationship between EFV and hemodynamically significant CAD (P for the interaction =0.02) with an interaction effect.Conclusions: There was a robust relationship between EFV and hemodynamically significant CAD. After adjustment for confounders, we found that the risk of hemodynamically significant CAD onset increased nonlinearly for EFV above 134.4 cm3. This refined understanding of the relationship is helpful for the accurate clinical prediction of hemodynamically significant CAD.

Abstract:Epicardial adipose tissue (EAT) is known for its pro-inflammatory properties and association with Coronavirus Disease 2019 (COVID-19) severity. However, current EAT segmentation methods do not consider positional information. Additionally, the detection of COVID-19 severity lacks consideration for EAT radiomics features, which limits interpretability. This study investigates the use of radiomics features from EAT and lungs to detect the severity of COVID-19 infections. A retrospective analysis of 515 patients with COVID-19 (Cohort1: 415, Cohort2: 100) was conducted using a proposed three-stage deep learning approach for EAT extraction. Lung segmentation was achieved using a published method. A hybrid model for detecting the severity of COVID-19 was built in a derivation cohort, and its performance and uncertainty were evaluated in internal (125, Cohort1) and external (100, Cohort2) validation cohorts. For EAT extraction, the Dice similarity coefficients (DSC) of the two centers were 0.972 (+-0.011) and 0.968 (+-0.005), respectively. For severity detection, the hybrid model with radiomics features of both lungs and EAT showed improvements in AUC, net reclassification improvement (NRI), and integrated discrimination improvement (IDI) compared to the model with only lung radiomics features. The hybrid model exhibited an increase of 0.1 (p<0.001), 19.3%, and 18.0% respectively, in the internal validation cohort and an increase of 0.09 (p<0.001), 18.0%, and 18.0%, respectively, in the external validation cohort while outperforming existing detection methods. Uncertainty quantification and radiomics features analysis confirmed the interpretability of case prediction after inclusion of EAT features.

Abstract:Objective: The aim of this study is to assess the association between epicardial adipose tissue (EAT) and coronary artery disease (CAD) via meta−analysis. Methods: Specific searches of online databases from January 2000 to May 2022 were conducted. All observational studies evaluating the association between EAT and CAD in PubMed, Web of Science, and the Cochrane Library databases were screened. A meta-analysis was conducted following the Preferred Reporting Items for Systematic Reviews and Meta−Analyses guidelines (PRISMA). In total, 21 studies encompassing 4975 subjects met the inclusion criteria, including 2377 diagnosed and assigned as the CAD group, while the other 2598 were assigned as the non−CAD group. Subjects in the CAD group were further divided into the severe stenosis group (stenosis ≥ 50%, n = 846) and the mild/moderate stenosis group (stenosis < 50%, n = 577). Results: Both the volume and thickness of EAT in the CAD group were larger compared to the non−CAD group (p < 0.00001). In a subgroup analysis within the CAD group, the severe stenosis group had a larger volume and thickness with respect to EAT when compared to the mild/moderate group (p < 0.001). Conclusions: The enlargement of EAT presented in CAD patients with an association with CAD severity. Although limited by different CAD types and measuring methods for EAT, as well as a smaller sample size, our results suggest that EAT is a novel predictor and a potential therapeutic target for CAD.

Abstract:Introduction and objectives The epicardial adipose tissue (EAT) is a metabolically active fat depot that has shown to be implicated in the development of coronary artery disease (CAD) and the prediction of cardiovascular events. However, these findings might be influenced by body size since previous studies have demonstrated a strong correlation between the amount of epicardial fat and anthropometric measurements. Our objective was to assess the association between indexed-EAT (EAT-i) volume and the presence of CAD and cardiovascular events in a cohort of symptomatic patients. Methods Patients referred for cardiac computed tomography from March 2010 to April 2017 were included. CAD was defined as the presence of obstructive coronary stenosis or a high amount of coronary calcium. EAT was assessed using a semiautomatic software and indexed to body surface area. A regression analysis was performed to determine the relationship between relevant CAD and EAT-i, age, sex, cardiovascular risk factors, and comorbidities. The association between EAT-i and cardiovascular events was analyzed with Cox regression models. Results A total of 179 patients were recruited (56±12 years, 57.5% male). EAT-i (P =.003), age (P <.001) and male sex (P <.001) were significantly and independently associated with CAD. During follow-up (5.5 years, 2.4–6.4) patients with elevated EAT-i showed an increased risk of events (HR, 2.44; 95%CI, 1.07–5.56; P =.033). Conclusions In symptomatic patients, an increased volume of indexed epicardial fat was associated with obstructive CAD as well as with cardiovascular events.

Abstract:<section class="article-section__content"><h3 class="article-section__sub-title section1"> Objects</h3><p>To assess the association between adipose tissue distribution and severity of clinical course in patients with severe acute respiratory syndrome coronavirus 2 (SARSCoV2).</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Methods</h3><p>For this retrospective study, 143 hospitalized patients with confirmed coronavirus disease 2019(COVID‐19) who underwent un‐enhanced abdominal computed tomography (CT) scan between January 1<sup>th</sup>, 2020 and March 30<sup>th</sup>, 2020 were included. Univariate and multivariate logistic regression analyses were performed to identify the risk factors associated with the severity of COVID‐19 infection. </p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Results</h3><p>There were 45 patients who were identified as critically ill. High visceral to subcutaneous adipose tissue area ratio (VSR, called visceral adiposity) (OR: 2.47, 95CI: 1.05 to 5.98, p=0.040) and low mean attenuation of skeletal muscle (SMD, called high intramuscular fat [IMF] deposition) (OR: 11.90, 95CI: 4.50 to 36.14, p<0.001) were independent risk factors for critical illness. Furthermore, visceral adiposity or high IMF deposition increased the risk of mechanical ventilation (p=0.013, p<0.001; respectively). High IMF deposition increased the risk of death (p=0.012).</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Conclusion</h3><p>COVID‐19 patients with visceral adiposity or high IMF deposition have higher risk for critical illness. Hence, patients with abdominal obesity should be monitored more carefully when hospitalized.</p></section>

Abstract:Background and aims: A small difference in epicardial adipose tissue (EAT) attenuation measured on computed tomography (CT) imaging has been reported between patients who suffered coronary events and event-free patients. EAT consists of beige adipose tissue functionally similar to brown adipose tissue and its attenuation may be affected by seasonal temperature variations and clinical factors. Methods: We retrospectively measured EAT attenuation on cardiac CT in 597 patients submitted to cardiac CT imaging for coronary artery calcium scoring. All scans were performed on the same CT scanner during the summer (June, July, August) or winter (December, January, February) months. EAT attenuation in Hounsfield units (HU) was assessed near the proximal right coronary artery in an area free of artifacts. For comparison, subcutaneous adipose tissue (SCAT) attenuation was measure along the midaxillary line. Results: The clinical and demographic characteristics of patients scanned during the summer (N = 253) and the winter (N = 344) months were similar. One third of patients were women, one quarter used statins and anti-hypertensive drugs and 30% were obese. The EAT attenuation was significantly lower during the summer than the winter months (-98.17 ± 6.94 HUs vs -95.64 ± 7.99 HUs; p<0.001). Sex, white race, body mass index, diabetes status, treatment with statins and anti-hypertensive agents significantly modulated the seasonal variation in EAT attenuation. SCAT attenuation was not affected by season or other factors. Conclusions: The measurement of EAT attenuation is complex and is affected by season, demographic and clinical factors. These factors may hinder the utilization of EAT attenuation as a biomarker of cardiovascular risk.

Abstract:Background Epicardial Adipose Tissue (EAT) volume is associated with the risk of cardiovascular events, which can be assessed by cardiac computed tomography. However, he optimal method and their prognostic utility in patients following transcatheter aortic valve replacement (TAVR) is unknown. Methods We evaluated 258 participants, focusing on changes in EAT volume using cardiac CT enhancement. EAT volume was automatically computed as three-dimensional voxels between −190 to −30 HU on contrast-enhanced slices. Univariate and multivariable Cox regression analyses were conducted to assess the association of various clinical parameters and EAT volume indices with major adverse cardiovascular events (MACE). Results During a median follow-up of 2.0 years [IQR, 1.8–2.3 years], 34 participants (median age 73 [IQR: −13.1 to −8.3) years, 55.4 % male) experiencing MACE. The optimal cutoff values for EAT volume change fraction (EATVCF) was 15.2 %, determined by the Youden-index. Kaplan-Meier curve analysis revealed that patients with high EATVCF were at higher risk (p < .01). In Cox regression, EATVCF (hazard ratio [HR]: 0.92, 95 % CI: 0.87 to 0.97, p = .001) remained significantly associated with MACE after adjusting for clinical factors. The addition of EATVCF to the clinical model increased the net Reclassification Improvement (NRI) by 30.1 % (95 % CI: 0.07–1.16). Conclusion EAT volume change fraction emerged as a significant predictor of MACE post-TAVR, highlighting the clinical value of EAT volume assessment in cardiovascular risk stratification.
