Abstract:The cathedral ceiling located in the entrance hall of the Montreal Neurological Institute, planned by its founder Wilder Penfield, has intrigued visitors since it was erected in 1934. Central to its charm is a cryptic comment by the ancient physician Galen of Pergamum, which refutes a dire Hippocratic aphorism about prognosis in brain injury. Galen's optimism, shared by Penfield, is curious from a fellow ancient. In this article, we use primary sources in Ancient Greek as well as secondary sources to not only examine the origins of Galen's epistemology but also, using a methodology in classics scholarship known as reception studies, illustrate how an awareness of this ancient debate can illuminate contemporary clinical contexts. While Galen based his prognostications on direct clinical observations like the Hippocratics, he also engaged in experimental and anatomic work in both animals and humans, which informed his views on neurologic states and outcomes. Penfield's memorialization of Galen is representative of the evolution of the neurosciences and the ongoing importance of evidence-based prognostication in severe brain injury.

Abstract:Wilder Penfield is known for his contributions to the structure-function relationship of the brain and for the surgical treatment of focal epilepsy. Less well known are his contributions to the study of glial cells and his investigation of their role in human neuropathology. Penfield learned the gold and silver methods for staining neurons, glial cells, and their projections from Charles Sherrington and Pío del Río-Hortega. He and his colleague William Cone established a laboratory for the study of glial cells and human neuropathology using metallic stains, initially at the Presbyterian Hospital in New York City in 1925, and then at the Montreal Neurological Institute in 1928. Penfield, Cone, and their research fellows, building on the findings of Río-Hortega, confirmed the existence of oligodendrocytes and their relationship with myelin, and investigated the putative mesodermal origin of microglia. They discovered the reaction of oligodendrocytes to pathological stressors, and the phagocytic activity of microglia in human gliomas. In this article, we argue that Penfield's studies of astrocytes, oligodendrocytes, and microglia, and their responses to craniocerebral trauma, epilepsy, malignant brain tumors, and other pathologies of the central nervous system inaugurated a new era in clinical neurocytology and neuropathology.

Abstract:The paper explores Lionel Penrose's scientific work. Penrose investigated the causes of mental disorders from clinical and genetic points of view. His investigations on phenylketonuria and Down syndrome helped to demonstrate the heterogenous character of mental disorders, whose causes can range from genetic with high penetrance, to largely environmental. He was specifically selected by JBS Haldane to become University College London's third Galton Chair as a result of his Colchester survey investigations. He became the first Galton Chair who had medical training. He never concealed his distaste for anything related to eugenics. As well as using his scientific work to reject eugenic ideas such as suggestions on sterilisation measures or the existence of a social problem group, he campaigned successfully to rename the Department of Eugenics, Biometry and Genetics to Department of Human Genetics and Biometry. With his work, he discredited prejudiced eugenic ideas on mental disorders and became an advocate for those with mental disabilities.

Abstract:In the 1930s and 1940s, neurosurgeons and clinical neurologists engaged in a fierce exchange on the scope of their specialties. Neurosurgeon Wilder Penfield's rhetoric of therapeutic superiority had a strong impact both on the Rockefeller Foundation's support for his institute and on the self-fashioning of neurologists. Neurologists articulated their identity in spirited performances at the meetings of specialist societies, their response shifting from a combative approach to a focus on internal organization. In light of the neurosurgeons' discourse, by the 1950s a new generation of neurologists created a revisionist narrative that inaccurately portrayed the clinical neurologists of the past as having been uninterested in therapeutics.

Abstract:This chapter examines the importance of visual materials for studying the brain in health and in disease. Surveying historical representations, this research confirms that images of the brain's form and function have long served as teaching tools and as historical reference points for neurological events. The research is divided into five sections: the first section, Early History to Printing Technology considers prehistoric and ancient imagery, pre-Renaissance thinking about the brain, and the impact of printing and printmaking on neurological research. The second section, Renaissance Illustration, focuses on Leonardo da Vinci, Andreas Vesalius, and other contributors who produced images of the brain as dissection restrictions eased. The third section, which turns to Early Modern and Modern Illustration, highlights the work of Thomas Willis, Charles Bell, and other scientists (throughout the 19th century) who demonstrated the value of a visual component within brain studies. The fourth section presents examples of Neurologically-Descriptive Illustrations, with the final section considering Historical Illustration and Contemporary Research.

Abstract:The A.A. insert the life and work of the naturalist and chemist Felice Fontana, born in Pomarolo (Trentino), in the frame of 18th century sciences, beside other great names of that century like Carolus Linnaeus, Réaumur, von Haller, Spallanzani, Morgagni, Priestley and Lavoisier. In the field of general biology, the discovery of nucleus and nucleolus and consequently the discovery of the eukaryotic cell, as we say in our days, in his, as well as the one of anabiosis. The A.A. enucleate and analyse the contributions of Fontana to the neurosciences; he has discovered the axon and the myelinic sheath half century before Remak and Purknije; he found out that the white matter of the brain is made of fibres alike those of nerves and the grey matter is made of globules (i.e. cells) mixed up with fibres; he discovered in the retina a part of coming out from the brain; he described the transversal bands of fibres of the skeletal muscles; he was the first to introduce into physiology the law of "all and nothing"; he attributed the irritability to the whole animal life; he identified the pupillar reflexes to the light, the reflex of accommodation, the consensual reflex, the psycho-emotive mydriasis and at last the myosis of sleep. He made experimental searches about nerves and recognised their regeneration, he enumerated various pathological intracranial masses, he made an important anatomopathological research about hydatid cyst in the brain of the sheep affected by "capostorno" and madness, he demonstrated their parasitical nature (he said that the hydated cysts were covered inside by small animals), he come out to formulate the hypothesis that some neuropsychiatric diseases of man can depend from similar aetiology. He declared that passions may have pathological effects (psyco-somatic aetiology), but he has also drawned the attention against the danager of aprioristical generalisation of neurogenical causes in all diseases. The A.A. give to Fontana the palm of precursor of modern neurosciences and of neurology intended as synthesis and unified corpus of these sciences.

Abstract:The skull of René Descartes is held in the National Museum of Natural History since the 19th c. Up to date, only anthropological examinations were carried out, focusing on the cranial capacity and phrenological interpretation of the skull morphology. Using CT-scan based 3D technology, a reconstruction of the endocast was performed, allowing for its first complete description and inter-disciplinary analysis: assessment of metrical and non-metrical features, retrospective diagnosis of anatomical anomalies, and confrontation with neuro-psychological abilities of this well-identified individual.

Abstract:This paper describes the scientific figure of Roger Sperry as a maverick researcher, an original thinker who arrived at definitive notions about the working of the brain mostly by distancing himself from the prevalent views of his peers. After solving the riddle of the functions of the corpus callosum, he won a Nobel prize in physiology or medicine for identifying the different cognitive abilities of the disconnected right and left hemispheres of the human brain. He could have won another Nobel prize for his work on the prenatal formation of behavioral neuronal networks and their growth and development after birth. In the last part of his life, he fought a courageous but inconclusive battle for demonstrating that mental and spiritual factors can direct brain activity and behavior without violating the laws of orthodox neurophysiology. Some nodal points in his scientific career and some sources of inspirations for his thinking are identified and discussed within the historical background of the neurosciences of the twentieth century.

Abstract:Purpose Most ophthalmologists in the US recognize the name "Bascom Palmer" but few are familiar with the individual for whom the Eye Institute is named. This article reviews the biography of Bascom Headen Palmer, Jr., MD (1889-1954). Design Historical review. Methods Historical documents were reviewed, including the archives of the University of Miami, the Miami Lighthouse for the Blind and Visually Impaired, and other sources. Results Palmer, born in Lake City, FL in 1889, was the younger son of a Florida politician. He graduated from what was then called the College of Medicine of the Tulane University of Louisiana (now the Tulane University School of Medicine) in 1914, followed by a two-year internship at Touro Infirmary, service in the army during World War I, and finally a post-graduate course in ophthalmology at the University of Pennsylvania in 1923. He then moved to Miami, where he became influential in two new organizations: the University of Miami, founded in 1925, and the Florida Association of Workers for the Blind (now the Miami Lighthouse for the Blind and Visually Impaired), founded in 1930. He forged collaborations between these two organizations which eventually led to the foundation of the eponymous Eye Institute. Conclusions The decades-long collaboration between the Miami Lighthouse and the University of Miami, facilitated by Palmer, ultimately led to the founding of the Eye Institute.

Abstract:Jean-Martin Charcot (1825-1893), the "father of neurology" in France and much beyond, was also the man who established academic psychiatry in Paris, differentiating it from clinical alienism. In his teaching, he used artistic representations from previous centuries to illustrate the historical developments of hysteria, mainly with the help of his pupil Paul Richer. Charcot liked to draw portraits (in particular, sketches of colleagues during boring faculty meetings and students' examinations), caricatures of himself and others, church sculptures, landscapes, soldiers, etc. He also used this skill in his clinical and scientific work; he drew histological or anatomic specimens, as well as patients' features and demeanor. His most daring artistic experiments were drawing under the influence of hashish. Charcot's tastes in art were conservative; he displayed no affinity for the avant-gardes of his time, including impressionism, or for contemporary musicians, such as César Franck or Hector Berlioz. Léon Daudet, son of Charcot's former friend and famous writer Alphonse Daudet, described Charcot's home as a pseudo-gothic kitsch accumulation of heteroclite pieces of furniture and materials. However, as Henry Meige wrote a few years after his mentor's death, Charcot the artist remains "inseparable from Charcot the physician."
Raphaël Blanchard, parasitology, and the positioning of medical entomology in Paris

Abstract:The histories of medical entomology and parasitology are entwined. Raphaël Blanchard (1857-1919), Chair of Medical Natural History and Parasitology at the Faculty of Medicine in Paris, organized the teaching of medical entomology and civilian colonial medicine. He also founded and edited the journal Archives de Parasitologie and started the Institute de Médecine Coloniale where he mentored many foreign students and researchers. Additionally, Blanchard is important for his scientific internationalism and medical historical work on the cultural location of parasitology and for training the future professors of parasitology Jules Guiart, Emile Brumpt, and Charles Joyeux.

Abstract:We present here a reproduction of a daguerreotype of Phineas Gage that came into our possession more than 30 years ago. It is, as far as we know, the only image of this famous patient. We describe how we identified the subject in the image, describe how daguerreotypes are made and set out our comparisons of the image with the Phineas Gage life mask and tamping iron held in the Warren Anatomical Museum, Francis A. Countway Library of Medicine.
Alfred Vulpian and Jean-Martin Charcot in each other's shadow? From Castor and Pollux at La Salpêtrière to neurology forever

Abstract:While Alfred Vulpian (1826-1887) is not completely forgotten, he cannot match the uninterrupted celebrity which Jean-Martin Charcot (1825-1893) still enjoys today. After becoming interne (residents) at the same institute in 1848, both were involved in shaping the cradle of what would become modern neurology. Both started work as chiefs at a La Salpêtrière service on January 1, 1862, making common rounds and studies, with several common publications. While their friendship remained 'for life', as stated by Charcot at Vulpian's funeral, their career paths differed. Vulpian progressed quicker and higher, being appointed full professor and elected at the Académie Nationale de Médecine and the Académie des Sciences several years before Charcot, as well as becoming dean of the Paris Faculty of Medicine. These positions also enabled him to support his friend Charcot in getting appointed full clinical professor and becoming the first holder of the chair of Clinique des Maladies du Système Nerveux in 1882. Before studying medicine, Vulpian had worked in physiology with Pierre Flourens, and his career always remained balanced between physiology and neurology, with remarkable papers. He introduced Charcot to optic microscopy during their La Salpêtrière years, indirectly helping him to become his successor to the chair of pathological anatomy in 1872. While Vulpian succeeded so well in local medical affairs, Charcot spent his time building up a huge clinical service and a teaching 'school' at La Salpêtrière, which he never left for over 31 years until his death. This 'school' progressively became synonymous with clinical neurology itself and perpetuated the master's memory for decades. Vulpian never had such support, although Jules Déjerine was his pupil and Joseph Babinski was his interne before becoming Charcot's chef de clinique (chief of staff) in 1885. This unusual switch in Parisian medicine contributed to Charcot's unaltered celebrity over more than a century, while Vulpian was progressively relegated to the studies of historians. However, Vulpian and Charcot remain inseparable in the memory of a lifelong friendship which gave birth to neurology.

Abstract:A preeminent quest of nineteenth-century visual neuroscience was to identify the anatomical elements of the retina that respond to light. A major breakthrough came in 1854, when Carl Bergmann discovered through direct observation that the human fovea contains only rods and cones. On this basis, he argued that these must necessarily be the light-sensitive elements (i.e., photoreceptors) that initiate vision. Bergmann also argued that Henle's fibers form part of the necessary anatomical link between these distal elements and the proximal ganglion cells, which transmit visual signals to the brain via the optic nerve. However, despite his achievement, Heinrich Müller, not Bergmann, is remembered as the discoverer of human photoreceptors in the literature. This article seeks to correct the record. It situates Bergmann's work alongside that of his contemporaries, sets out his arguments and the critique he received using archival documents, and makes this history more accessible for current readers by comparing what was said to what we know now. We argue that Bergmann's arguments are at least as compelling as those of Müller, and that he should be recognized as a codiscoverer of the anatomic site in the retina where vision is initiated.

Abstract:Few original portraits exist of René Descartes, yet his theories of vision were central to Enlightenment thought. French philosophers combined his emphasis on sight with the English approach of insisting that ideas are not innate, but must be built up from experience. In particular, Denis Diderot criticised Descartes's views by describing how Nicholas Saunderson--a blind physics professor at Cambridge--relied on touch. Diderot also made Saunderson the mouthpiece for some heretical arguments against the existence of God.

Chapter 6: after Galen Late Antiquity and the Islamic world

Abstract:It is usually assumed that after Galen there was nothing new until the Renaissance. Contrary to this view, there were significant modifications of the inherited legacy in Late Antiquity, followed by fundamental changes within the Arabic/Islamic world. Their formative influence extends from the medieval period of transmission to the Renaissance and the 17th century. The increasing emphasis on the primacy of the brain initiated the beginnings of ventricular localization of function in Late Antiquity, which was subsequently developed into a theory and transmitted to the West via Arabic. Following the unprecedented translation movement in 9th-century Baghdad, the cumulative Greek and Hellenistic knowledge of the brain, nerves, and the senses from diverse sources were brought together in the systematic, logically unified Arabic medical compendia of encyclopedic proportions, which embody divergence from accepted views and new diagnostic observations. Their Latin versions became standard texts in medical schools. The oldest extant schematic diagrams relevant to neurology (the eye, the ventricles, the visual system, and the nerves) date from this period, and served as models for the medieval Latin West. The development of coherent descriptions of the motor and sensory systems, and related clinical disorders, by analogy with the mechanisms of hydraulic automata, foreshadows some of the explanatory methods associated with the 17th century. Furthermore, an entirely new approach resulted in a paradigm shift in theory and methodology through the experimental studies on the physics of light and vision of Ibn al-Haytham (d. 1040), who showed that what is sensed is not the object itself, but a punctate optical "image" due to light reflected from its surface to the eye. This revolutionary approach to vision destroyed the viability of the Greek tradition of holistic forms and tactile sensory impressions. Ibn al-Haytham's theory of point-to-point correspondence formed the basis of subsequent attempts to find a topological representation of external objects in the eye (the retinal surface with Kepler) and in the brain (the pineal gland with Descartes and ultimately in the cortex with Munk). His serial re-projection of the punctate image in the eye, chiasma, and the brain can be regarded as dramatic precursors of the principles in neurology of hierarchical anatomical organization of sensory information from simple to complex. Due to Ibn al-Haytham, an understanding of vision increasingly required a synthesis of anatomy with the physics of light. Subsequently the visual enquiry shifted from the global question of "how do we perceive the external world by the sense of sight?" to specific concerns arising from the implications of Ibn al-Haytham's "optical" image in the eye: (a) the preservation of a point-to-point correspondence between object and image; (b) image inversion and the veridical (upright) perception of the object; (c) the unity of perception, or the binocular fusion of the two separate images, one from each eye; and (d) the anatomical projection of the retinal maps to various brain centers. The fact that these became central issues extending to Descartes and beyond underscores the unique legacy in neurology of Ibn al-Haytham's conception of a punctate sensory map, which was re-projected to the brain for complex sensory functions.
