Abstract:Objective To explore the current status and influencing factors of oral health knowledge and behavior among the elderly living in Gongshu District of Hangzhou City.Methods A total of 600 elderly individuals were selected by multi-stage stratified cluster random sampling and interviewed with a self-designed questionnaire which included demographic characteristics and oral health knowledge and behavior.The status and influencing factors of oral health behavior were analyzed by single and multiplefactor analysis methods.Results A questionnaire survey was conducted among 600 elderly residents,with a response rate of 99.3％ (596/600) and an effective response rate of 94.1％ (561/600).The rate of good oral hygiene was 50.4％.Those elderly with different age,education level,medical insurance and oral hygiene showed significant difference in qualified rate of oral health behavior (x2 values were 10.79,21.32,5.72 and 16.33,respectively; all P＜0.05).In Logistic regression model,education level was positively correlated with oral health behavior among the elderly,and the qualified rate of oral health behavior of the elderly with education level of junior school or above was 2.69 times higher than that of illiterate elderly (x2=10.53,P=0.001).Conclusion The awareness rate of oral health knowledge among the elderly living in Gongshu District of Hangzhou City is at a relatively higher level,though oral health behavior is moderate.Age,education level,medical insurance and oral hygiene could be impacting factors of oral health behavior.
Investigation of tooth loss and prosthetic conditions on 986 elderly outpatients

Abstract:Objective To investigate the tooth loss,prosthesis hygiene and abutment conditions as well. Methods 986 elderly outpatients,aged from 60 to 88 years old, were interviewed and clinically examined by the same two dentists following the unified criteria. Results The tooth loss rate was 93.5% . Statistical analyses indicated the tooth loss percentage of male outpatients(92.0%) was significant different from that of female ones(95.6%)(P<0.05).60.3% of elderly outpatients were found to have dental prostheses. It had positive correlation with age. In over denture cases, the percentages of root cavities and gingivitis were 27.3% and 38.4%, respectively. Conclusion It is important to give preventive oral care and health education to the public. The prosthetic design should accord with the tooth loss characteristics of elderly people.

Abstract:Objective To assess dental knowledge and behavior of teachers in Shaoxing.Methods The questionnaire and the standard on oral health survey authorized by WHO were employed in this study.Results 84.6% of the investigated teachers brush teeth twice or three times per day,12.9% of whom brush teeth over three minutes each time.Only 38.7% of the teachers acknowledged that the pit and fissure sealing treatment can prevent dental caries.31.7% did not have periodic oral health checks,and 82.1% did not visit the dentist until they had toothache.Conclusion The findings suggested that we should emphasize oral health education among teachers in order to promote school-based oral health education program.

Abstract:Objective The purpose of this study was to assess the associations between the self-reported oral health-related quality of life (OHRQoL) of older Chinese people and their socio-economic status and oral health using data collected in the 4th National Oral Health Survey. Methods After multistage stratified cluster sampling, 4332 adults aged 65-74 years participated in the study. After a clinical examination, the participants completed a structured questionnaire. A Mandarin version of the General Oral Health Assessment Index (GOHAI) was included in the questionnaire. Clinical examinations were carried out using the criteria recommended by the World Health Organization. Multivariate analysis was used to evaluate the associations of the clinical and socio-demographic variables with the GOHAI scores of the participants. Results The weighted mean (SE) GOHAI score was 51.6 (0.0). Female sex and having more than 10 missing teeth, unrestored tooth spaces, more than 10 decayed teeth and a periodontal pocket >= 6 mm were negatively associated with the GOHAI score, while having a higher education level and living in the Eastern region of China were positively associated with the GOHAI score. Conclusion After adjustment for confounders, the OHRQoL of older Chinese individuals was mainly affected by untreated dental caries and the loss of teeth. Older Chinese individuals who were male, had a higher education level, were from the Eastern region, had no unrestored tooth spaces or deep periodontal pockets and had fewer decayed or missing teeth had higher GOHAI scores, indicating better OHRQoL.

Abstract:OBJECTIVE:To investigate the dental caries status and related factors in Chinese elderly people, using data from the 4th National Oral Health Survey of China.METHODS:In this cross-sectional study, a multistage, cluster strategy was used to recruit 4,431participants (2,222 male and 2,209 female) aged 65 to 74 years from all 31 provinces, autonomous regions and municipalities of the mainland of China. The survey was performed according to the diagnostic standard proposed by the World Health Organization (WHO). Socio-demographic information was collected with a closed questionnaire.RESULTS:The caries prevalence in 65 to 74-year-olds was rather high; the report shows it was 98.0% (DMFT ≥ 1) among elderly people in China. The mean DMFT, DT, MT and FT was 13.33 ± 9.32, 3.33 ± 4.17, 9.50 ± 8.66 and 0.49 ± 1.41, respectively. The filling rate was very low, assessed as 12.8%. The number of DMFT related to gender, residential district type, educational level, level of annual household income, toothbrushing frequency, consumption frequency of desserts, attitudes to oral health and oral health-related knowledge.CONCLUSION:The prevalence of dental caries in Chinese elderly people is rather high and a lot of decayed teeth still need to be filled and suitable prevention and treatment for this group is urgently needed.

Abstract:The role of health behaviors in oral health conditions in individuals of extremely old age remains understudied. This study included 185 participants aged 100 years or older from the Nanjing Centenarians Study (NCS) to examine the associations between health behaviors and oral health and investigate the potential moderating role of education and living arrangements in such relationships. The oral health status as an outcome included the self-reported oral health status and edentulous status. Health behavior variables included smoking, eating fruits, eating vegetables, participating in leisure activities, and practicing oral hygiene behaviors. Sociodemographic characteristics and health status were considered as confounders. Descriptive statistics, ordinal regression, and logistic regression models were used to address the research questions. Results showed that better oral health was reported by centenarians who were non-smokers, participated in more leisure activities, and practiced higher frequency of oral hygiene behaviors. Those who ate fruits daily and practiced more frequently oral hygiene behaviors were more likely to be dentate. The positive association of oral hygiene behaviors was stronger for centenarians who were formally educated and co-resided with family members. The results suggest that effective interventions should consider health behaviors and living arrangements in this growing population to improve their oral health status.

Abstract:OBJECTIVES:To describe oral health behaviour, illness behaviour, oral health knowledge and attitudes among 35-44 and 65-74-year-old Chinese; to analyse the oral health behaviour profile of the two age groups in relation to province and urbanisation, and to assess the relative effect of socio-behavioural risk factors on dental caries experience.METHODS:A total number of 4,398 35-44-year-olds and 4,399 65-74-year-olds were selected by multistage stratified cluster random sampling which involved 11 provinces in China. Data were collected by self-administered structured questionnaires and clinical examinations (WHO criteria).RESULTS:32% of the 35-44-year-olds and 23% of the 65-74-year-olds brushed at least twice a day but only 5% used fluoridated toothpaste; 30% and 17% respectively performed 'Love-Teeth-Day' recommended methods of tooth brushing. A dental visit within the previous 12 months was reported by 25% of all participants and 6% had a dental check-up during the past two years. Nearly 15% of the subjects would visit a dentist if they experienced bleeding from gums; about 60% of the subjects paid no attention to signs of caries if there was no pain. Two thirds of the urban residents and one fifth of the rural participants had economic support for their dental treatment from a third party, either totally or partially. Significant variations in oral health practices were found according to urbanisation and province. At age 35-44 years 43% of participants had daily consumption of sweets against 28% at age 65-74 years. Dental caries experience was affected by urbanisation, gender, frequency, time spent on and method of tooth brushing. Knowledge of causes and prevention of dental diseases was low with somewhat negative attitudes to prevention observed.CONCLUSION:Systematic community-based oral health promotion should be strengthened and preventive-oriented oral health care systems are needed, including promotion of further self-care practices and the use of fluoridated toothpaste.
The relationship between oral status and healthy aging in Chinese older adults: A community cohort study

Abstract:Objectives In recent years, there has been a growing academic interest in investigating the determinants of healthy aging. The main objective of this study was to investigate the association between oral status, as defined by tooth loss and denture use, and healthy aging among Chinese older adults over a four‐year duration. Methods The data were derived from the 2014 and 2018 waves of the Chinese Longitudinal Healthy Longevity Survey. Descriptive statistics and multivariate logistic regression analysis were employed for statistical analysis. Results The results suggest that older adults with ≥20 natural teeth have a higher likelihood of experiencing healthy aging compared to those with fewer than 20 natural teeth. In the fully adjusted model, only older adults with ≥20 natural teeth demonstrated a higher rate of healthy aging compared to those with 0 natural teeth, with an OR (95% CI): 2.09 (1.54–2.84). Conclusions These results highlight the importance of maintaining ≥20 teeth for promoting healthy aging among Chinese older adults. Furthermore, wearing dentures can, to varying degrees, mitigate the adverse effects of tooth loss on healthy aging. Rehabilitative interventions, such as the utilization of dentures and other clinical strategies, play a pivotal role in enhancing the oral health status of older individuals.

Abstract:OBJECTIVE:To explore the clinical and socio-demographic factors inﬂuencing oral health-related quality of life (OHRQoL) of Chinese adults in the 4th National Oral Health Survey.METHODS:Multistage stratified cluster sampling and PPS method were used in sampling and 4720 adults aged 35 to 44 years were recruited. The study subjects completed a structured questionnaire in an interview and underwent a clinical examination. The questionnaire was a Putonghua version of the General Oral Health Assessment Index (GOHAI) and was completed by the interviewer on the site of the 4th National Oral Health Survey. Clinical examination was performed using the criteria recommended by the World Health Organization (WHO).RESULTS:The mean GOHAI score of the subjects was 54.42 (SD 6.01). Result of Poisson regression showed that subjects with a higher household income per capita, had lower DMFT, fewer missing teeth, had no unrepaired missing teeth, or not wearing a partial denture had higher GOHAI scores indicating better OHRQoL.CONCLUSION:The OHRQoL of the adults in China was fair and was mainly inﬂuenced by dental caries, integrity of dentition and restoration of lost teeth. With limited resources and dental manpower in China, higher priority should be given to the prevention and treatment of the main cause leading to losing teeth.

Abstract:Abstract This study examined the relationship between social cohesion (i.e., sense of community and neighborhood cohesion) and self-reported number of oral health problems and further investigated the potential moderating role of cognitive function in such a relationship among U.S. older Chinese adults. Data came from baseline of the Population Study of Chinese Elderly in Chicago between 2011 and 2013 (N = 3,157). Stepwise negative binomial regression models with interaction terms were used. Individuals with a stronger sense of community had 1% less risk of having oral health problems (RR = .99; 95% CI = .98, .99; p < .001). Individuals experiencing a stronger neighborhood cohesion had a 11% reduction in risk of having oral health problems (RR = .89; 95% CI = .86, .92; p < .001). To promote optimal oral health, interventions need to account for individuals’ perception and actual integration with their neighborhood and communities.

Abstract:Objective To assess oral health knowledge, beliefs and practices among community-dwelling older adults in Shanghai, China. Background China is ageing faster than other developing countries. Four National Oral Health Surveys have been carried out in China between 1983 and 2015, focused on clinical oral examinations. Materials and methods A cross-sectional study was conducted among 983 participants aged 60-93 years old, drawn from two community health service centres of Pudong New Area in Shanghai. Data were collected using a Chinese language questionnaire of oral health knowledge, beliefs and practices based on the Knowledge, Attitudes (beliefs) and Practices (KAP) model. The scale ranges were knowledge 0-19, beliefs 0-17 and practices 0-55, where a higher score indicated a more accurate perception. Multivariate analysis explored the related factors using IBM SPSS Statistics version 22.0. Results Mean (SD) scores of this questionnaire were oral health knowledge 13.0 (3.6), beliefs 13.0 (3.2) and practices 31.2 (11.2). Geographic location, age group, employment status, education and monthly household income showed significant statistical association with these scores (P < .001). Only three sociodemographic variables (education, age group and employment status) remained significant after further multiple regression analysis (P < .001). Conclusions The level of oral health knowledge, beliefs and practices among older adults in Shanghai, China, was not high. Younger and working individuals with higher level of education showed good oral health knowledge, beliefs and practices. Community-based oral health strategies are needed to increase oral health knowledge, in particular of oral daily cleaning, and improve beliefs and practices in regular dental visits.
Association between self-reported oral health and clinical examination outcomes among older adults

Abstract:Objective To examine the association between self-reported oral health status and clinical examination outcomes among older adults in three provinces,northeastern region of China.Methods Data were drawn from an epidemiological study that included both clinical oral health examinations and survey questions.A total of 1084 participants(age 65-75)were included in the analyses.Self-reported oral health status included the condition of teeth,gum,and oral hygiene status which was measured separately by a 3-point Likert scale (poor,fair,and good).Clinical status was measured including the number of decayed teeth (DT),missing teeth (MT),bleeding on probing,depth of pocket,and attachment loss.Multinomial logistic regressions were used to examine the association adjusting for clinical examination outcomes and self-reported oral health status.Results After adjusting for covariates,a higher number of DT and MT were significantly correlated with poorer self-reported oral health status.Similarly,good gum status and oral hygiene status was associated with less number of bleeding on probing.Conclusion A significant association was found between self-reported oral health conditions and clinical outcomes from the examination.This study suggests that self-reported measures are good indicators for overall oral health status.It is possible that self-reported measures could be used as a screening measure of oral health status among Chinese older adults.

Effect modification by denture use was observed (Pfor interaction <0.001).WHAT ARE THE IMPLICATIONS FOR PUBLIC HEALTH PRACTICE?:Measures should be taken to help Chinese older adults promote oral health and strengthen the care of dentures by expanding basic health insurance coverage to include dental prosthodontics or providing affordable dental insurance for seniors.

Abstract:AbstractBackgroundThe purpose of the study is to examine the association between tooth loss and hypertension among older community residents in urban China.MethodsThis study included 3677 participants aged ≥50 years from the Shanghai Aging Study. We determined the number of teeth missing from questionnaires. Hypertension was defined as the mean of two measurements of systolic blood pressure (SBP) (140 mmHg or higher), diastolic blood pressure (DBP) (90 mmHg or higher) or physician-diagnosed hypertension confirmed from medical records. A multivariable logistic regression model was used to investigate the association between tooth loss and hypertension.ResultsThe average number of missing teeth among study participants was 9.67. Among them, participants with hypertension had lost an average of 10.88 teeth, significantly higher than those without hypertension (8.95) (p < 0.0001). After adjusting for covariates (socio-demographic characteristics, health behaviors and other chronic conditions), teeth lost (15 or more) was significantly associated with grade III hypertension, with OR = 1.55(95% CI 1.09–2.20).ConclusionsSignificant tooth loss maybe associated with severe hypertension among older Chinese adults. Prevention of tooth loss is important to the overall health of this population.

Abstract:Abstract The purpose of this study was to examine the associations between tooth/gums symptoms and changes in cognitive function. We used data from the Population Study of Chinese Elderly in Chicago, a two-wave epidemiological study of 2,713 U.S. Chinese older adults. We selected self-reported oral (tooth and gum) symptoms as independent variables. We measured global function and three cognitive domains: episodic memory, executive function and working memory. Adjusting for sociodemographic and health-related characteristics, participants who reported having teeth symptoms at baseline, experienced their global cognition and episodic memory decrease (both p<0.05). Participants who reported having teeth symptoms at baseline, experienced a faster rate of decline in global cognition for every additional year. However, this effect disappeared once we adjusted for all covariates. We found no significant relationship between baseline gum symptoms and change of cognitive function. Future research directions, clinical and policy implications will be discussed.

Abstract:Background: This study aimed to explore how intergenerational support affects the oral healthcare or treatment-seeking behaviors of older Chinese adults and provide evidence for improving the oral health of the older adults in an aging society. Methods: Data from a cross-sectional survey, the 2015 China Health and Retirement Longitudinal Study, were used to explore the relationship between oral healthcare-seeking behavior in older adults and various independent variables, such as marital status, number of children, offspring's education duration, parent–offspring interaction frequency, and financial/material support provided by children. The chi-square test and binary logistic regression were used. Results: According to the results of data analysis, age, sex, marital status, cohabitation status, number of children, children's education level, and financial support from children affected older adults' oral healthcare-seeking behavior. Interviewees living with a partner and those who had 1–2 or 3–4 children showed different likelihoods of seeking oral healthcare. Moreover, interviewees whose children had higher education duration and those who received more financial/material support from their children were more likely to seek dental treatment. Conclusion: Regarding the study's outcome, financial and emotional support, as well as practical assistance from family members can significantly promote oral health-seeking behavior among older adult people. Intergenerational support can serve as a crucial mechanism for promoting oral health behaviors among the older adults or act as a valuable complement to social medical assistance, warranting increased attention.

Abstract:AbstractBackgroundThe prevalence of tooth loss varies across the globe among oldest-old individuals. The presence of fewer than 20 teeth in old age was associated with a decrease in people’s health and quality of life. This paper explored the association between socioeconomics, health-related factors, and tooth loss among the population over the age of 80 in China.MethodsThe tooth loss status of older Chinese adults was collected with a structured questionnaire from the 8thwave of the Chinese Longitudinal Healthy Longevity Survey (CLHLS). A total of 6716 individuals aged 80 years and above were included. Logistic regression was used to assess the association between socioeconomic statuses, dietary intake at approximately 60 years old, health-related factors, and tooth loss.ResultsOf the 6716 individuals aged 80 years and above, the composition of the group with fewer teeth for both men and women was statistically significant in many ways. Multivariate logistic regression analyses show that for men, being older than 90 years and being ADL disabled (adjusted OR: 1.71, 95% CI: 1.01–2.89) are factors that are significantly and consistently associated with a higher risk of having fewer than 20 teeth, while having a higher household income per capita (adjusted OR: 0.56, 95% CI: 0.32–0.99) decreases the risk. For women, an age of above 95 years, brushing teeth less than once per day (adjusted OR: 1.96, 95% CI: 1.26–3.03), consuming sugar some of the time as opposed to less than once per month at approximately 60 years old (adjusted OR = 1.74, 95% CI: 1.15–2.62), and being ADL disabled (adjusted OR: 1.70, 95% CI: 1.04–2.77) are factors that are significantly associated with the risk of having fewer than 20 teeth.ConclusionThe analysis suggests that socioeconomic status, dietary intake in early old age, and ADL capacity are associated with the risk of having fewer teeth for the population aged 80 years and above, and the risk factors vary between sexes.
Association between oral health and cognitive function among Chinese older adults: the Taizhou imaging study

Abstract:Background: We aimed to investigate the association between oral health and cognitive function in a sample of older adults from a Chinese rural community. Methods: The cross-sectional cognitive function of 677 individuals were assessed by Mini-Mental State Examination (MMSE) and Montreal Cognitive Assessment (MoCA). A comprehensive profile of the oral health status was evaluated by questionnaire and clinical examination. Results: Multiple covariates-adjusted regression models demonstrated decayed teeth (DT) and decayed/missing/filled teeth (DMFT) were negatively associated with MoCA score (all p < 0.05). Calculus index (CI) and clinical attachment loss (CAL) were significantly associated with the lower MoCA, short-term memory and executive function score, respectively (all p < 0.05). Additionally, participants with missing teeth unrestored tend to get lower MMSE and MoCA scores (p < 0.05). The results also showed that increased DT and CI were modestly associated with higher odds of cognitive impairment (p < 0.05). Conclusions: There is an association between oral health and global cognition. Poor periodontal status was strongly associated with worse global cognition performance, especially in the short-term memory and executive domain for the aging population.
