Abstract:Virtual surgical planning enables precise surgical planning and translation of this planning into the operating room. Preoperative maxillofacial computed tomography scans are compared to a reference skull to identify desired surgical changes. In facial feminization surgery, these include forehead recontouring/frontal table setback, gonial angle reduction, and possible chin repositioning/reshaping, while in facial masculinization surgery, this includes forehead augmentation and gonial angle/chin augmentation. Cutting and recontouring guides as well as custom implants are then custom manufactured. Common guides include osteotomy guides, depth drilling guides, ostectomy guides, and guides for one/two-piece genioplasty or chin burring. Common implants include mandibular and chin implants.

Volumetric Perceptions in Midfacial Aging with Altered Priorities for Rejuvenation

Abstract:The history of facial rejuvenation surgery has largely involved the manipulation of facial soft tissues under tension within the two-dimensional confinements of a facial plane. However, the youthful human face is not planar; it presents as a complex geometric solid with a curvilinear profile on the oblique view that forms an architectural ogee. Restoration of this shape by sculptural manipulation of the facial soft tissues is deemed the highest priority in midfacial rejuvenation, whereas improvement of the periorbital and perioral environments is deemed second. Effacement of the nasolabial fold is relegated to a third level of importance in this philosophy of altered priorities for midfacial rejuvenation. The importance of early patient photographs is stressed in operative planning to direct facial changes toward a past intrinsic facial personality. Postoperative results are presented of patients who have undergone facial rejuvenation by a new midfacial technique that targets these reordered goals. Comparisons with photographs taken earlier in life and traditional preoperative photographs can then be made.

CSP has enhanced the precision and efficiency of surgeries for craniofacial dysostoses and hypertelorism, resulting in improved outcomes. This review delves into the importance of understanding orbital anatomy and the crucial bony and soft tissue landmarks essential to the application of CSP in frontofacial procedures, encompassing Le Fort III and monobloc advancements, as well as the correction of hypertelorism. Abstract:Nasal reconstruction provides a challenging task for any facial plastic surgeon. In performing reconstructive surgery on the nose, one must balance both the functional aspects of the nasal airway with the aesthetic responsibilities of being the central fixture of the face. Before performing surgery, the surgeon must grasp the importance of the anatomic nasal relationships and subunits, the physiology of the skin, the flaps or grafts used to reconstruct these areas, and the basic steps in wound healing. Accurate preoperative planning and analysis of the nasal defect are also vital to producing both an aesthetic and functional nose.

Abstract:Transgender individuals face significant health disparities including deficiencies in physician education, knowledge, and comfort with care. The objective of this study was to determine the perceptions, practice patterns, and familiarity of facial plastic surgeons with transgender health care. An anonymous questionnaire was sent to facial plastic surgeons within the American Academy of Facial Plastic and Reconstructive Surgery list-serve. Primary datapoints included participant characteristics, transgender-related experience, and educational goals. Of the 66 facial plastic surgeons surveyed, 49% had treated 1 to 10 transgender patients during their career, and 70% were actively treating at least 1 transgender patient. The number of patients treated and surgeries performed was significantly associated with self-perceived competence, comfort counseling on gender-affirming surgeries, discussing gender identity, asking preferred pronouns, and a desire to learn more about transgender care. Most participants (61%) obtained transgender care training through real-world experience, with only 18% receiving formal training in residency or fellowship. In total, 50% of respondents believe transgender care training among facial plastic surgeons is inadequate and 60% support its incorporation into residency/fellowship curricula. Increased awareness is needed to address the disparities experienced by transgender patients. Many facial plastic surgeons desire to learn more and support incorporating transgender care into training. Understanding the current state of transgender care can assist the facial plastic community in promoting education that strengthens physicians' ability to deliver competent care that addresses the inequities faced by this diverse group. Abstract:The topic of facial rejuvenation should be expected to be a recurring theme in a journal named Facial Plastic Surgery . I invite you to ponder that statement. The reason for this periodic reappearance of this topic is because of the amazing aspect of current human experience that is continually benefited from ongoing advances in science and medicine. These advances have not only resulted in the elimination of many illnesses and maladies but have also prolonged the active human lifespan. Peoples' health is considerably better than it was just a few decades ago. These advances continue and among such are frank endeavors to further longevity. As a result of this, as evidenced by the patients entering our practices, there is an enlarging and more sophisticated group of patients seeking physical and physiological rejuvenation. Their desire for rejuvenation of one's facial appearance is indeed part of the evolution of patients' desire for good health, youthfulness, longevity, and vitality. The field of facial rejuvenation has undergone parallel advances, like other aspect of medicine, with changes in surgical techniques and new technologies. Patients' growing desire for rejuvenation has been catalyzed by the internet and social media. Patients are continually presented with not only what is possible, but what is possible for them personally. The intermittent recurrence of the topic of facial rejuvenation in Facial Plastic Surgery , therefore, is not only understandable but necessary. As pertinent advances and practices change, it is essential that these changes are communicated to the clinicians who are involved in facial rejuvenation. That is what we present here in the Facial Rejuvenation: Comprehensive 2025 Experts' Update. I have been most privileged to assemble this group of leading experts who are immersed in the practice of facial rejuvenation and keenly represent the benefits of newer surgical techniques and technologies. Colleagues from around the globe have been so kind to share their expertise and contribute to this edition. I know you will find it informative, enlightening, and enjoyable. Abstract:Enhanced Recovery After Surgery, or ERAS, was described by Kehlet in the 1990s. At first these programs focus on addressing preoperative, intraoperative, and postoperative factors to improve surgical outcomes in cardiac and colorectal procedures. Over the ensueing decades, many other surgical specialties within orthopedic surgery, gynecology, and surgical oncology have adopted the ERAS concepts and developed ERAS protocols. The ERAS approach is evidence-based, data-driven, leveraging group experience, and group intelligence. Recognizing the lack of ERAS protocols in craniomaxillofacial surgery, efforts for their development begun in 2019, jointly between members of the American Society of Craniofacial Surgeons (ASCFS) and the American Society of Maxillofacial Surgeons (ASMS). To date, ERAS protocols for the following procedures have been developed and released during national meetings: repair of cleft lip, palatoplasty, secondary alveolarbone graft, orthognathic surgery, and fronto-orbital advancement. The ERAS protocols address 4 phases: pre-hospital, pre-operative, intra-operative, and post-operative. These are living documents in that there is a closed feedback loop with interative evaluation followed by modification. As technology improves and more data become available, better treatments should replace existing ones. These protocols are developed by craniofacial surgeons for craniofacial units to improve patient outcomes.

Abstract:Background Facial feminization surgery (FFS) is an emerging practice that falls under the broader umbrella of gender-affirming surgery. Various approaches exist to feminize the face, yet few published articles describe in detail the techniques of each component procedure. Considering the diversity of interventions employed, the objective of this manuscript is to highlight FFS techniques utilized by the senior author and create a corresponding media gallery. Methods All patients with the diagnosis of gender dysphoria that were referred to the senior author for FFS consultation between June 2017 and August 2022 were reviewed. Data were retrospectively collected from electronic medical records according to the institutional review board (IRB)–approved study protocol. Data collected and analyzed included demographics, operative documentation, and postoperative follow-up. Multimedia material was collected intraoperatively and postoperatively. Results A total of 231 patients underwent 262 operations with a total of 1224 FFS procedures. The average follow-up time was 7.7 ± 11 months. Out of the 262 operations, 24 (9.2%) patients experienced minor complications, including 3 (1.1%) with wound dehiscence, 13 (5.0%) with hematomas, and 14 (5.3%) with postoperative infection requiring antibiotics. Of those, 3 (1.1%) required a return to the operating room for washout or removal of malar implants. Conclusion Although there is a consensus on the fundamental surgical principles to achieve adequate feminization of the facial architecture, the specific techniques to do so differ according to individual practices. As techniques diverge, so do their risk profiles and outcomes; techniques must, thus, align with patients' interventional goals. The material presented here is one of many that can support trainees and junior surgeons as they build a gender-affirming practice.

Abstract:Objectives Surgeons experience a risk for physical strain and injury secondary to physical demands in the workplace. To minimize injury and maximize career longevity, physicians should be aware of ergonomics pitfalls and postural correction methods. This study investigates ergonomic trends in Facial Plastic and Reconstructive Surgery (FPRS) clinic by quantifying surgeons' and trainees' cervicothoracic spine posture. Methods Participants completed a 22‐item questionnaire to evaluate current ergonomic practices. A lightweight device was calibrated and attached to the mid‐scapular region of participants, providing real‐time posture feedback. The percentage of time in upright posture was recorded during clinical and operative workdays. Upright posture was defined as neutral spine positioning with acceptable mild to moderate deviations. Results Two FPRS attending surgeons, 1 FPRS fellow, and 11 otolaryngology residents participated over 12 months. Discomfort was most commonly reported in the neck, shoulders, and upper back during clinic. Symptoms were self‐treated by changing body position, wearing specialized footwear, adjusting height of the chair or examination table, or ignoring discomfort. Abstract:Our role as aesthetic surgeons demands individualized surgical planning that maximizes patient input and understanding. The value of such shared decision-making (SDM) in aesthetic surgery is becoming increasingly appreciated. This is particularly true for potential patients seeking surgical rejuvenation of the face, where the volume of "educational" information available on the internet, and through various social medial channels, may be overwhelming and even misleading. Presented is a "3-Level approach to facelift planning" named for the facial subregions targeted. This novel paradigm maximizes SDM with its simplicity and reproducibility, serves as an invaluable educational tool for patients, novice and seasoned surgeons alike, and facilitates communication between senior surgeons through its descriptive standardization.
