Abstract:INTRODUCTION: Because most data on placental abruption are derived from retrospective studies, multiple sources of bias may have affected the results. Thus, we aimed to characterize risk factors and outcomes for placental abruption in a large prospective cohort of nulliparous women.METHODS: This was a secondary analysis of women enrolled in the Nulliparous Pregnancy Outcomes Study Monitoring-to-be (nuMom2b) study, a prospective observational cohort. Participants were recruited in their first trimester of pregnancy from 8 sites and had 4 study visits, including at delivery. Placental abruption was defined by confirmed clinical criteria. The primary analysis was restricted to abruption identified antepartum and intrapartum. As a secondary analysis, we examined antepartum and intrapartum abruptions separately. We compared risk factors (maternal demographic and clinical characteristics) and outcomes in women with and without placental abruption using univariable and multivariable analyses as appropriate.RESULTS: 9450 women were included in the primary analysis. Abruption was identified in 0.66% (n = 62), of which 35 (56%) were antepartum and 27 (44%) intrapartum. For women with abruption, the mean gestational age at delivery was 35.6 ± 4.4 weeks and 38.8 ± 2.2 weeks for women without abruption. Gravidity was associated with abruption (OR 3.1, 95% CI: 1.6-6.0). In univariate analysis, abruption was associated with cesarean delivery (OR 3.7, 95% CI: 2.2-6.0), blood transfusion (OR 3.8, 95% CI: 1.4-10.7), PPROM (OR 9.0, 95% CI: 5.4-15.1), preterm birth (OR 8.5, 95% CI: 5.1-14.2), SGA (OR 4.0, 95% CI: 2.3-6.95), RDS (OR 5.5, 95% CI: 2.6-11.2), IVH 20.2 (OR 20.2, 95% CI: 5.9-68.8) and ROP (OR 12.2, 95% CI: 2.8-52.6). However, after adjustment for confounders including gestational age, abruption was only associated with increased odds of cesarean delivery and blood transfusion. Results were similar when restricted to antepartum and intrapartum abruptions.CONCLUSION: Abruption was identified in <1% of nulliparous women. However, few maternal risk factors were identified. Neonatal morbidities were associated with an abruption and were primarily driven by gestational age due to preterm birth.

Abstract:Preterm placental abruption is a multifactorial pregnancy complication, the etiology of which remains uncertain, and the risk of maternal morbidity and perinatal mortality is high. Over the past decade, the profile of obstetric patients has changed, as well as the approaches to medical care, and some risk factors for preterm placental abruption have also been revised. Objective. To summarize current knowledge about the epidemiology and causes of preterm placental abruption. Materials and methods. The search for publications was carried out in the electronic databases PubMed, Web of Science, SCOPUS, Google Scholar by four reviewers independently from January 1, 2017 to March 31, 2020. Clinical case reports, conference abstracts and studies with no comparison group were excluded. Results. Thirty-four studies involving 20,833,192 patients met the inclusion criteria, of whom 193,114 were diagnosed with preterm placental abruption, with an incidence of 0.93% (after excluding five studies with cross-sectional design, the overall incidence of preterm placental abruption decreased to 0.83%). According to the results obtained, among the undoubted risk factors for preterm placental abruption were smoking (odds ratio (OR) 1.72–13.0), anemia (OR 1.36–2.44), chronic kidney disease (OR 3.72– 5.51), the Negroid race (OR 1.47–3.59), uterine scar (OR 1.53–6.33) and uterine anomalies (OR 3.35–4.69). Contradictory information was received about the negative impact of the following factors: maternal age, assisted reproductive technologies, gynecological diseases, alcohol, various air pollutants and hot weather. Obesity was among the risk factors that did not affect preterm placental abruption (OR 0.81–0.74). Conclusion. The risk factors presented in the review were divided into groups. In the first two, timely correction of lifestyle and/or prescription of medicines can reduce the probability of preterm placental abruption. The third group is represented by risk factors, most of which are beyond regulation. However, to fully understand the etiology of preterm placental abruption, further qualitative studies with their more frequent systematization are required. Abstract:To evaluate the effects of changes in risk factors between the first two pregnancies on the occurrence of placental abruption (PA) in the same woman.Routinely collected obstetric data from Aberdeen Maternity and Neonatal Databank, the Maltese National Obstetric Information System and the Finnish Medical Birth Register were aggregated. Records of the first two singleton pregnancies from women who had PA in one pregnancy but not the other, were identified from this pooled dataset. A case-crossover study design was used; cases were pregnancies with abruption and matched controls were pregnancies without abruption in the same woman. Conditional logistic regression was used to investigate changes in risk factors for placental abruption in pregnancies with and without abruption.A total of 2,991 women were included in the study. Of these 1,506 (50.4%) had PA in their first pregnancy and 1,485 (49.6%) in a second pregnancy. Pregnancies complicated by preeclampsia {194 (6.5%) versus 115 (3.8%) adj OR 1.69; (95% CI 1.23–2.33)}, antepartum haemorrhage of unknown origin {556 (18.6%) versus 69 (2.3%) adjOR 27.05; 95% CI 16.61–44.03)} and placenta praevia {80 (2.7%) versus 21 (0.7%) (adjOR 3.05; 95% CI 1.74–5.36)} were associated with PA. Compared to 20 to 25 years, maternal age of 35–39 years {365 (12.2) versus 323 (10.8) (adjOR 1.32; 95% CI 1.01–1.73) and single marital status (adjOR 1.36; 95% CI 1.04–1.76) were independently associated with PA. Maternal smoking, BMI and fetal gender were not associated with PA.Advanced maternal age, pregnancies complicated with unexplained bleeding in pregnancy, placenta praevia and preeclampsia were independently associated with a higher risk of placental abruption.

Abstract:Objective To investigate the high risk factors of placental abruption and pregnancy outcome. Methods Placental ab-ruption group included 72 puerpera who were diagnosed with placental abruption in our hospital in recent 3 years,while the control group con-sisted of pregnant women with normal childbirth over the same period. The relationship between placental abruption and maternal age,history of cesarean section,preeclampsia,maternal,premature rupture of membranes,abortion,male fetuses,as well as the pregnancy outcomes were retrospectively analysed. Results In the study group,14 cases had a history of cesarean section,25 cases preeclampsia and 33 cases the maternal;compared with the control group,the difference was statistically significant(P < 0. 05). Also in the study group,there were 20 cases of stillbirth and neonatal death,29 cases of acute hypoxia fetus and 43 cases of preterm low weight children;compared with the control group,the difference was statistically significant(P < 0. 05). Conclusion Previous history of cesarean section,preeclampsia are high risk factors of maternal placental abruption;there is high incidence of stillbirths and neonatal deaths and acute hypoxia fetus and preterm low birth weight children in puerpera with maternal placental abruption. Attention should be paid to various risk factors for placental abruption;early detection,diagnosis and treatment,individualized treatment are helpful for the improvement of maternal outcome.
Maternal, Labor, Delivery, and Perinatal Outcomes Associated with Placental Abruption: A Systematic Review

Abstract:Objective Risk factors for placental abruption have changed, but there has not been an updated systematic review investigating outcomes. Methods We searched PubMed, EMBASE, Web of Science, SCOPUS, and CINAHL for publications from January 1, 2005 through December 31, 2016. We reviewed English-language publications reporting estimated incidence and/or risk factors for maternal, labor, delivery, and perinatal outcomes associated with abruption. We excluded case studies, conference abstracts, and studies that lacked a referent/comparison group or did not clearly characterize placental abruption. Results A total of 123 studies were included. Abruption was associated with elevated risk of cesarean delivery, postpartum hemorrhage and transfusion, preterm birth, intrauterine growth restriction or low birth weight, perinatal mortality, and cerebral palsy. Additional maternal outcomes included relaparotomy, hysterectomy, sepsis, amniotic fluid embolism, venous thromboembolism, acute kidney injury, and maternal intensive care unit admission. Additional perinatal outcomes included acidosis, encephalopathy, severe respiratory disorders, necrotizing enterocolitis, acute kidney injury, need for resuscitation, chronic lung disease, infant death, and epilepsy. Conclusion Few studies examined outcomes beyond the initial birth period, but there is evidence that both mother and child are at risk of additional adverse outcomes. There was also considerable variation in, or absence of, the reporting of abruption definitions.
The environmental risk factors prior to conception associated with placental abruption: an umbrella review

Abstract:Abstract Background The present umbrella review evaluated risk factors prior to conception associated with placental abruption based on meta-analyses and systematic reviews. Methods We searched PubMed, Scopus, and Web of Science until June 25, 2021. All meta-analyses that had focused on assessing the risk factors associated with placental abruption were included. We calculated summary effect estimates, 95% CI, heterogeneity I 2 , 95% prediction interval, small-study effects, excess significance biases, and sensitive analysis. The quality of the meta-analyses was evaluated with A Measurement Tool to Assess Systematic Reviews 2 (AMSTAR 2). Results There was no risk factor in the present umbrella review with the high level of evidence (class I or II). Eight risk factors including maternal asthma (RR 1.29 95% CI 1.14, 1.47), prior cesarean section (RR 1.38, 95% CI 1.35–1.42), cocaine using (RR 4.55, 95% CI 1.78–6.50), endometriosis (OR 1.40, 95% CI 1.12–1.76), chronic hypertension (OR 3.13, 95% CI 2.04–4.80), advanced maternal age (OR 1.44, 95% CI 1.35–1.54), maternal smoking (OR 1.80, 95% CI 1.75–1.85) (RR 1.65, 95% CI 1.51–1.80), and use of assisted reproductive techniques (ART) (OR 1.87, 95% CI 1.70–2.06) were graded as suggestive evidence (class III). The other four risk factors including pre-pregnancy underweight (OR 1.38, 95% CI 1.12–1.70), preeclampsia (OR 1.73, 95% CI 1.47–2.04), uterine leiomyoma (OR 2.63, 95% CI 1.38–3.88), and marijuana use (OR 1.78, 95% CI 1.32–2.40) were graded as risk factors with weak evidence (class IV). Conclusion Maternal asthma, prior cesarean section, cocaine use, endometriosis, chronic hypertension, advanced maternal age, maternal smoking, and use of ART, pre-pregnancy underweight, preeclampsia, uterine leiomyoma, and marijuana use were risk factors associated with placental abruption. Although factors associated with placental abruption have been investigated, the current meta-analytic associations cannot disentangle the complex etiology of placental abruption mainly due to their low quality of evidence.

Multivariate Analysis of Pregnancy Outcomes and Investigation of Precaution in Placental Abruption.

Abstract:Objective To investigate the influence factors of obstetrics and neonatal outcomes in placental abruption and to investigate the clinical precaution against placental abruption.Method The clinical data of 89 cases with placental abruption were reviewed during Jan 1994 to Dec 2008 in Peking University Third Hospital.There were 52 cases complicated with preeclampsia and 37 cases of unexplained placental abruption.Logistic regression analysis was used to analyze the correlation between relevant factors to obstetrics and neonatal outcomes.Results Initial clinical warning signs with abnormalities in ultrasound as a first clinical sign was significantly more in the preeclamptic group than in the unexplained placental abruption group(P=0.039).Abdominal pain as a initial clinical sign was significantly more in the severe cases group than in the mild cases group(P=0.010).The severity of placental abruption:Preeclampsia(P=0.003)and the interval from the onset of the initial clinical sign to delivery(P=0.008)were associated with the severity of placental abruption.Postpartum hemorrhage was associated with the interval from the onset of the initial clinical sign to delivery(P=0.040).Fetal demise was associated with preeclampsia(P=0.016),severe placental abruption(P=0.016)and the interval from the onset of the first clinical sign to delivery(P=0.003).Neonatal asphyxia was associated with the interval from the onset of the first initial clinical sign to delivery(P=0.031).Conclusion Preeclampsia and the interval from the onset of the initial clinical sign to delivery were associated with the severity of placental abruption.Shrinking time interval from the onset of the initial clinical sign to delivery may be able to improve obstetric and neonatal outcomes.

Abstract:Our case-control study, aiming at identifying the risk factors for placental abruption (PA), looked at variables including maternal age, daily transportation means, folic acid consumption, smoking, past-history of infertility, number of antenatal visits done, presence of pre-eclampsia and umbilical cord insertion. Significant risk factors for PA were frequent transportation by motorbikes, consumption of haematinic preparation not containing folic acid, passive smoking, past-history of infertility, pre-eclampsia and marginal cord insertion. Pregnant women should be counselled about the above-mentioned risk factors.
[Multivariate Analysis of Risk Factors with Placental Abruption in Preeclampsia].

Abstract:OBJECTIVE:To investigate the risk factors with placental abruption in patients with preeclampsia.METHODS:Retrospective analysis on medical documents of 219 patients treated in Peking University Third Hospital from Jan. 1994 to Dec. 2008. Patients were divided into 3 groups, including 52 cases with severe preeclampsia terminated following placental abruption, 130 cases only with severe preeclampsia and 37 cases with unexplained placental abruption without preeclampsia. One hundred and seventy-eight normal pregnancies were selected as the control group (1:2 case-control). Univariate and multivariate regression analysis were used to identify the risk factors for placental abruption in patients with preeclampsia.RESULTS:(1) When compared with those in control group, univariate analysis showed that gravidity, parity, history of preeclampsia, second and third trimester pregnancy loss, history of autoimmune disease, chronic hypertension disease, lack of regular antenatal care, fetal growth restriction (FGR) and raises of umbilical artery Doppler resistance were risk factors associated with placental abruption. Logistic regression analysis showed that lack of a regular antenatal care (OR = 45.348, 95%CI: 17.096 - 120.288, P = 0.000), FGR (OR = 27.087, 95%CI: 5.585 - 131.363, P = 0.000) and second/third trimester pregnancy loss (OR = 16.068, 95%CI: 1.698 - 152.029, P = 0.015) were independent risk factors with placental abruption. (2) When compared with those in preeclampsia patients without placental abruption, the history of preeclampsia (OR = 3.715, 95%CI: 1.096 - 12.596, P = 0.035) and lack of a regular antenatal care (OR = 2.509, 95%CI: 1.173 - 5.370, P = 0.018) were risk factors for placental abruption in preeclampsia.CONCLUSION:Lack of regular antenatal care, FGR, history of preeclampsia and second/third trimester pregnancy loss were risk factors associated with placental abruption in patients with preeclampsia.

Abstract:Background and Objectives: Risk factors for neonatal/maternal morbidity and mortality in placental abruption have been incompletely studied in the current literature. Most of the research overlooked the African American population as mostly Caucasian populations are selected. We aimed to find which risk factor influence the neonatal and maternal outcome in cases of placental abruption occurring in African American pregnant women in an inner-city urban setting. Materials and Methods: We performed a retrospective cohort study at St. Joseph’s Regional Medical Center, NJ United States of America (USA), between 1986 and 1996. Inclusion criteria were African American race, singleton pregnancy with gestational age over 20 weeks and placental abruption. Maternal age, gravidity, parity, gestational age at delivery/occurrence of placental abruption and mode of delivery were collected. Risk factors for placental abruption such as placenta previa, hypertensive disorders of pregnancy, cigarette smoking, crack/cocaine and alcohol use, mechanical trauma, preterm premature rupture of membranes (PPROM), and premature rupture of membranes (PROM) were recorded. Poor neonatal outcome was considered when anyone of the following occurred: 1st and 5th minute Apgar score lower than 7, intrauterine fetal demise (IUFD), perinatal death, and neonatal arterial umbilical cord pH less than 7.15. Poor maternal outcome was considered if any of the following presented at delivery: hemorrhagic shock, disseminated intravascular coagulation (DIC), hysterectomy, postpartum hemorrhage (PPH), maternal intensive care unit (ICU) admission, and maternal death. Results: A population of 271 singleton African American pregnant women was included in the study. Lower gestational age at delivery and cesarean section were statistically significantly correlated with poor neonatal outcomes (p = 0.018; p < 0.001; p = 0.015) in the univariate analysis; only lower gestational age at delivery remained significant in the multivariate analysis (p = < 0.001). Crack/cocaine use was statistically significantly associated with poor maternal outcome (p = 0.033) in the univariate analysis, while in the multivariate analysis, hemolysis, elevated enzymes, low platelet (HELLP) syndrome, crack/cocaine use and previous cesarean section resulted significantly associated with poor maternal outcome (p = 0.029, p = 0.017, p = 0.015, p = 0.047). PROM was associated with better neonatal outcome in the univariate analysis, and preeclampsia was associated with a better maternal outcome in the multivariate analysis. Conclusions: Lower gestational age at delivery is the most important risk factor for poor neonatal outcome in African American women with placental abruption. Poor maternal outcome correlated with HELLP syndrome, crack/cocaine use and previous cesarean section. More research in this understudied population is needed to establish reliable risk factors and coordinate preventive interventions.
Placental Histopathology and Pregnancy Outcomes in “Early” vs. “Late” Placental Abruption

Abstract:Placenta-associated pregnancy complications (fetal growth restriction and preeclampsia) are traditionally classified as "early" and "late" due to their different pathophysiology, histopathology, and pregnancy outcomes. As placental abruption (PA) represents another placenta-associated complication, we aimed to study if this categorization can be applied to PA as well. Pregnancy and placental reports of all pregnancies complicated by PA between November 2008 and January 2019 were reviewed. Maternal background, pregnancy outcomes, and placental histopathology were compared between cases of PA < 34 weeks (early PA group) vs. > 34 weeks (late PA group). Placental lesions were classified according to the "Amsterdam" criteria. The primary outcome was severe neonatal morbidity (≥ 1 severe neonatal complications: seizures, IVH, HIE, PVL, blood transfusion, NEC, or death). Included were 305 cases of PA, 71 (23.3%) in the early group and 234 (76.7%) in the late group. The early PA group was characterized by higher rates of vaginal bleeding upon presentation (p = 0.003), DIC (p = 0.018), and severe neonatal morbidity (p < 0.001). The late PA group was characterized by a higher rate of urgent Cesarean deliveries (p < 0.001). The early PA group was characterized by higher rates of placental maternal vascular malperfusion (MVM) lesions (p < 0.001), maternal inflammatory response (MIR) lesions (p < 0.001), placental hemorrhage (p < 0.001), and a lower feto-placental ratio (p < 0.001). Using regression analysis, we found that severe neonatal morbidity was independently associated with early abruption (aOR = 5.3, 95% CI = 3.9–7.6), placental MVM (aOR = 1.5, 95% CI = 1.2–1.9), placental MIR (aOR = 1.9, 95% CI = 1.4–2.3), and inversely associated with antenatal corticosteroids (aOR = 0.9, 95% CI = 0.6–0.98). "Early" and "late" PA significantly differ in their presentation, placental pathology, and pregnancy outcomes.

Analysis of misssed diagnosis and misdiagnosis of 1212 cases with placental abruption

Abstract:Objective To investigate the risk factors and clinical manifestations of placental abruption, and to analyze the causes of missed diagnosis and misdiagnosis. Methods A retrospective analysis was conducted in 135584 women who delivered in Women′s Hospital, School of Medicine, Zhejiang University from January 2005 to December 2015. The diagnosis of placental abruption was made in 1212 cases. According to the consistency of prenatal and postnatal diagnosis, they were divided into 3 groups.(1) The diagnosis was consistent prenatally and postnatally in 715 cases(58.99%,715/1212) as the diagnosis group.(2)In 312 cases (25.74%,312/1212), the diagnosis was made after birth as the missed diagnosis group.(3)In 185 cases (15.26%,185/1212), the diagnosis was made prenatally but excluded after birth as the misdiagnosis group. The disease classification was made, and the risk factors, clinical manifestations, lab results, the time of termination and perinatal outcomes were recorded in the 3 groups. The reasons of missed diagnosis and misdiagnosis were analyzed. Results (1) In the 1212 cases, the diagnosis of placental abruption was confirmed in 1027 cases, with the incidence of 0.76%(1027/135584). The rate of missed diagnosis was 30.38%(312/1027), and the rate of misdiagnosis was 0.14%(185/134557). (2) There were significant differences in the degree of placental abruption among the 3 groups (P＜0.05). (3)Significant differences were found among the 3 groups regarding the ratio of hypertensive disorders, trauma, induced labor and advanced maternal age (all P＜0.05). (4) There were statistically significant differences among the 3 groups regarding the incidence of vaginal bleeding, persistent abdominal pain and uterine tenderness, bloody amniotic fluid, increased uterine tension and stillbirth (all P＜0.05). (5) There was no significant difference in the rate of abnormal fetal heart rate mornitoring among the 3 groups (P=0.22). The differences were statistically significant among the 3 groups when regarding the incidence of abnormal ultrasound finding and abnormal blood coagulation (P＜0.01), with the highest incidence of abnormal ultrasound in the diagnosis group (68.1%) and the highest incidence of abnormal coagulation in the misdiagnosis group (24.9%). (6)There was statistically significant difference among the 3 groups when comparing the ratio of termination of pregnancy within 24 hours (P=0.01). (7) There were statistically significant differences among the 3 groups when the ratios of postpartum hemorrhage, DIC, neonatal asphyxia and perinatal death were compared (all P＜0.05). The highest incidence of postpartum hemorrhage was in the diagnosis group (17.9%) and the lowest was in the misdiagnosis group (5.4%). The highest incidence of DIC was in the diagnosis group (3.9%) and the lowest was in the misdiagnosis group (0). The highest incidence of neonatal asphyxia was in the diagnosis group (30.6%) and the lowest was in the misdiagnosis group (7.6%). And for perinatal death, the highest incidence was in the diagnosis group (12.6%), the lowest was in the misdiagnosis group (2.2%). Conclusions Placental abruption could be misdiagnosed when depending on risk factors, such as trauma. And it could be missed diagnosis during the induction of labor. Uterine contraction, abnormal fetal heart rate mornitoring, abnormal ultrasound and abnormal coagulation function are important in the diagnosis of placental abruption.
Prevalence and Its Feto-Maternal Outcome in Placental Abruption: A Retrospective Study for 5 Years from Dubai Hospital

Abstract:Introduction: Placental abruption (PA) is a major obstetric complication leading to increased risk of maternal and neonatal morbidity and mortality globally. This study aims to determine the prevalence and its feto-maternal outcome in cases of placental abruption in our population. Methods: A retrospective descriptive case study was conducted from January 2013 till December 2017 in Dubai Hospital, Dubai, UAE. All patients admitted with suspicion of placental abruption at 28 weeks of gestation and beyond were included in the study. The clinical information like maternal age, parity, antenatal risk factors, gestational age at delivery, mode of delivery, postpartum complications and perinatal outcome were studied. Result: A total of 15,079 deliveries occurred during the study period; PA accounted for 0.61%. 62% of the women had revealed abruption and 38% had concealed abruption. The antenatal risk factors associated with abruption were diabetes (26%), chronic hypertension (2.2%), pre-eclampsia (19.5%), previous cesarean section (26%), and multiple pregnancy (9.8%). Cesarean section was the mode of delivery in 78% of abruption cases. 33% had postpartum hemorrhage and 20% had received blood products. Adverse fetal outcomes were as follows: 51% of the cases had preterm delivery, 47% of the babies had a birth weight of less than 2.5 kg, 8 cases died of intrauterine fetal death and there was 1 neonatal death. Conclusion: Even when being aware of the risk factors, placental abruption still remains unpredictable or unpreventable. Having better equipped obstetric and neonatal units with multidisciplinary management can improve both maternal and perinatal outcome in cases of placental abruption.

Abstract:Introduction: Placental abruption (PA) is a major obstetric complication associated with worse maternal and neonatal outcomes. Though ultrasound findings may support the diagnosis of PA, the association of such findings to the severity of PA and maternal and neonatal outcomes is not yet clear. We aimed to assess the maternal and neonatal outcomes of PA cases with vs. without related sonographic findings. Methods: In this retrospective cohort study, all deliveries complicated by PA between 2009 and 2022 were included. Placental histopathology, obstetric, and neonatal outcomes were compared between cases of PA with vs. without supporting sonographic findings. A composite of severe neonatal morbidity was compared between the groups, including ≥1 of the following: seizures, intraventricular hemorrhage, hypoxic-ischemic encephalopathy, periventricular leukomalacia, respiratory-distress syndrome, sepsis, anemia, blood transfusion or death. Results: Of the 420 cases with PA eligible for the study, 50 patients (12 %) were in the PA with sonographic features group and 370 (88 %) were in the PA without sonographic features group. The PA with sonographic features group was characterized by significantly higher rates of prematurity (p < 0.001), severe composite adverse neonatal outcome (p < 0.01), and a composite maternal vascular malperfusion lesions in placental histopathology (p = 0.001) In multivariable regression analyses, preterm birth was independently associated with the presence of sonographic features (aOR = 8.79, 95 % CI 2.41-31.93, p < 0.001). Discussion: PA with supporting sonographic features is associated with higher rates of adverse obstetric and neonatal outcomes and placental lesions. These findings emphasize the importance of sonographic evaluation for every case of PA before deciding upon management.

Abstract:Placental abruption is a rare complication of pregnancy with a high risk of maternal and neonatal morbidity and mortality. Preeclampsia (PE) is often mentioned among the possible causes of placental abruption. However, the combined effect of PE and placental abruption on maternal and neonatal outcomes has been little studied. Objective. To conduct a comparative analysis of somatic and obstetric histories, pregnancy course, maternal and neonatal outcomes in placental abruption in pregnant women with and without PE. Patients and methods. A total of 1594 cases of placental abruption collected from 22 medical centers in 16 regions of the Russian Federation were included in the study. The number of patients with PE was 509 (31.9% out of the total sample). The study group comprised 509 pregnant women with placental abruption and PE, the control group – 1085 pregnant women with placental abruption without PE. Results. Overweight and obesity (OR = 2.03; 95% CI: 1.64–2.52), chronic arterial hypertension (OR = 6.90; 95% CI: 4.78–9.95), chronic kidney disease (OR = 1.55; 95% CI: 1.07–2.25) and chronic anemia (OR = 1.33; 95% CI: 1.07–1.66) were statistically significantly more common in the group of patients with PE. An association between obstetric and gynecological history and risk of PE was established only for parity of 2 or less (OR = 1.37; 95% CI: 1.11–1.70) and PE in a previous pregnancy (OR = 4.83; 95% CI: 2.59–9.01). During the current pregnancy, patients in the study group were almost 4 times more likely to be diagnosed with intrahepatic cholestasis (OR = 3.88; 95% CI: 1.89–7.95) and about 2 times more likely to have fetal growth restriction (OR = 1.85; 95% CI: 1.48–2.31), low birth weight (OR = 1.72; 95% CI: 1.19–2.48), and anemia of pregnancy (OR = 1.77; 95% CI: 1.43–2.19) compared to patients without PE. The study group was about twice as likely to have severe maternal morbidity (OR = 2.12; 95% CI: 1.41–3.17) and require hysterectomy (OR = 1.82; 95% CI: 1.01–3.29) despite the same mean blood loss (800 mL) in both groups. Patients with PE were almost 3 times more likely to be diagnosed with intrauterine fetal demise (OR = 2.80; 95% CI: 1.92–4.08) and about twice as likely to have hypotrophy (OR = 1.81; 95% CI: 1.45–2.26) and asphyxia (OR = 1.81; 95% CI: 1.44–2.26) compared to controls. Conclusion. It was demonstrated that patients with PE and placental abruption had statistically and clinically significant differences compared to the control group. Abstract:Placental abruption is the separation of the placenta from the lining of the uterus before childbirth. It is an infrequent perinatal complication with serious after-effects and a marked risk of maternal and fetal mortality. Despite the fact that numerous placental abruption risk factors are known, the pathophysiology of this issue is multifactorial and not entirely clear. The aim of this review was to examine the current state of knowledge concerning the molecular changes on the maternal–fetal interface occurring in placental abruption. Only original research articles describing studies published in English until the 15 March 2021 were considered eligible. Reviews, book chapters, case studies, conference papers and opinions were excluded. The systematic literature search of PubMed/MEDLINE and Scopus databases identified 708 articles, 22 of which were analyzed. The available evidence indicates that the disruption of the immunological processes on the maternal–fetal interface plays a crucial role in the pathophysiology of placental abruption. The features of chronic non-infectious inflammation and augmented immunological cytotoxic response were found to be present in placental abruption samples in the reviewed studies. Various molecules participate in this process, with only a few being examined. More advanced research is needed to fully explain this complicated process.

Abstract:Introduction: Two possible causative pathways have been suggested to participate in the development of placental abruption (PA), an acute inflammatory pathway and placental vascular derived, a chronic pathway. We aimed to study the impact of the inflammatory pathway on maternal and neonatal outcome. Methods: The computerized medical files and placental reports of all pregnancies diagnosed with PA, between 11/2008-1/2019, at 24-42 weeks, were reviewed. Placental lesions were classified according to "Amsterdam" criteria into maternal and fetal vascular malperfusion lesions, acute inflammatory responses and chronic villitis. Composite neonatal morbidity included ≥1 of the following: seizures, intra-ventricular hemorrhage (IVH), hypoxic-ischemic encephalopathy, periventricular leukomalacia (PVL), blood transfusion, necrotizing enterocolitis (NEC), neonatal sepsis, respiratory distress syndrome, or neonatal death. Maternal and neonatal outcome were compared between PA with and without histologic chorioamnionitis (HC). Results: As compared to the PA without HC group (n = 267), the PA with HC group (n = 77) was characterized by lower gestational age (GA) at delivery (32.9 ± 5.5 vs. 35.6 ± 4.1 weeks, p < 0.001), higher rates of oligohydramnios (p < 0.001), bloody amniotic fluid at labor (p < 0.001), maternal postpartum fever (p < 0.001), longer maternal hospitalization (<0.001), and increased composite adverse neonatal morbidity (41.6% vs. 22.8%, p = 0.002). By multivariate analysis, GA and HC were found to be independently associated with adverse neonatal outcome, aOR 0.63 95% CI 0.43-0.78, p < 0.001, and aOR1.12, 95% CI 1.02-3.87, p = 0.04, respectively. Discussion: The involvement of the inflammatory causative pathway in the development of placental abruption, is associated with increased maternal and neonatal morbidity.

Abstract:Placental abruption (PA) is a concern for maternal and neonatal morbidity. Adverse neonatal outcomes in the setting of PA include higher risk of prematurity. Placental pathologies include maternal vascular malperfusion (MVM), fetal vascular malperfusion (FVM), acute chorioamnionitis, and villitis of unknown etiology (VUE). We aimed to investigate how placental pathology contributes to acute neonatal outcome in PA. A retrospective cohort study of all placentas with PA were identified. Exposures were MVM, FVM, acute chorioamnionitis and VUE. The primary outcome was NICU admission and the secondary outcomes included adverse base deficit and Apgar scores, need for resuscitation, and small-for-gestational age. A total of 287 placentas were identified. There were 160 (59.9%) of placentas with PA alone vs 107 (40.1%) with PA and additional placental pathologies. Odds of NICU admission were more than two times higher in pregnancies with placental pathologies (OR = 2.37, 95% CI 1.28–4.52). These estimates were in large part mediated by prematurity and birthweight, indirect effect acting through prematurity was OR 1.79 (95% CI 1.12–2.75) and through birthweight OR 2.12 (95% CI 1.40–3.18). Odds of Apgar score ≤ 5 was more than four times higher among pregnancies with placental pathologies (OR = 4.56, 95% CI 1.28–21.26). Coexisting placental pathology may impact Apgar scores in pregnancies complicated by PA. This knowledge could be used by neonatal teams to mobilize resources in anticipation of the need for neonatal resuscitation.

Abstract:Objective: To investigate the diagnosis, treatment, and maternal and fetal outcomes of placental abruption.Materials and methods: We recruited 585 cases of placental abruption from the Women and Children's Hospital Affiliated to Xiamen University between January 2012 and December 2017. Cases were categorized into four groups (class 0–III) according to the clinical guidelines published by the Obstetrics and Gynecology Branch of the Chinese Medical Association. We then compared clinical data and auxiliary examinations across the four groups.Results: The differences were statistically significant (p < .01) among the four groups of placental abruption with regard to the incidence of an abnormal ultrasound finding. Positive ultrasound signs were evident in 6.4% of the patients categorized as class 0 and 100.0% of patients categorized as class III. Monitoring showed that fetal heart rate (FHR) was abnormal in class II patients with placental abruption; patients in class III showed no fetal heart sounds. Cesarean section was carried out for 26.6%, 75.1%, 65.2%, and 47.1% of patients in classes 0, I, II, and III, respectively. The rate of cesarean section for classes I and II was the highest, while the lowest rate occurred for class 0. Postpartum hemorrhage occurred in 2.5%, 9.3%, 15.2%, and 29.4% of patients across the four groups, DIC occurred in 0.0%, 1.3%, 2.3%, and 23.5% of cases, and perinatal death occurred in 1.0%, 1.3%, 7.6%, and 100.0% of cases, respectively. The highest incidence of postpartum hemorrhage was in class III (29.4%) and the lowest was in class 0 (2.5%). The highest incidence of DIC was in class III (23.5%) and the lowest was in class 0 (0.0%). The highest incidence of neonatal asphyxia was in class II (34.1%) and the lowest was in class 0 (10.1%). Regarding perinatal death, the highest incidence was in class III (100.0%) and the lowest was in class 0 (1.0%). These data showed significance differences when compared across the four groups of patients (p < .01).Conclusions: We recommend that the diagnosis of placental abruption should consider risk factors, clinical features, FHR monitoring, and dynamic ultrasound monitoring. Early diagnosis and treatment can improve maternal and infant prognosis.

Abstract:INTRODUCTION:Placental abruption is a serious pregnancy complication that causes maternal and neonatal mortality and morbidity. Whether maternal and neonatal outcomes differ between patients who concurrently presented with preeclampsia and those who did not, have not been fully investigated.METHODS:A total number of 158 patients with placental abruption were included. Of them, 66 concurrently had preeclampsia. Maternal and neonatal characteristics including delivery weeks, time of onset and birthweight as well as the grade of placental abruption were collected and analysed.RESULTS:The time at diagnosis of placental abruption in patients who concurrently presented preeclampsia was significantly earlier than that in patients who did not. The number of patients with grade III placental abruption was significantly higher in patients who concurrently presented with preeclampsia, compared to patients who did not. The odds ratio of an increase in grade III placental abruption in patients who concurrently presented preeclampsia was 5.27 (95%CL: 2.346, 12.41), compared to patients who did not. The numbers of infants who were born pre-term and the numbers of stillbirth/neonatal deaths as well as the number of fetal distress were significantly higher in patients who concurrently presented with preeclampsia, compared to patients who did not. The fetal birthweight was significantly lower in patients who concurrently presented with preeclampsia compared to patients who did not.DISCUSSION:Our study demonstrates that women with preeclampsia experiencing placental abruption had worse maternal, fetal and neonatal outcomes, compared to women experiencing placental abruption alone.
