Abstract:Handball is a team sport involving a great physical demand from its practitioners in which a high number of injuries occur, affecting individual and collective performance. Knowledge of the injuries is of great importance for their prevention. The objective of the present study was to identify, locate and compare the most frequent injuries and injury mechanisms in handball practice. It was carried out following the Preferred Informed Item for Systematic Reviews and Meta-analysis (PRISMA) guidelines. The source of data collection was direct consultation of the PubMed and Medline databases. Several keywords were used for the documentary retrieval, and the quality of the studies that were selected was evaluated. Of the 707 studies retrieved, only 27 were considered appropriate for the review, and quality scores were obtained that ranged from 10 to 26 points, out of a maximum of 28. The most frequent injuries in handball players are located in the lower limbs (thigh, knee and ankle), and in the shoulder in the upper limbs. Regarding the playing position, the players who play over the 6-m line are the most affected by injuries, while the women players have a higher probability of injury. Most injuries occur during competition.
Risk Factors of Overuse Shoulder Injuries in Overhead Athletes: A Systematic Review

Abstract:Context: Shoulder injuries are highly prevalent in sports involving the upper extremity. Some risk factors have been identified in the literature, but consensus is still lacking. Objectives: To identify risk factors of overuse shoulder injury in overhead athletes, as described in the literature. Data sources: A systematic review of the literature from the years 1970 to 2018 was performed using 2 electronic databases: PubMed and Scopus. Study selection: Prospective studies, written in English, that described at least 1 risk factor associated with overuse shoulder injuries in overhead sports (volleyball, handball, basketball, swimming, water polo, badminton, baseball, and tennis) were considered for analysis. Study design: Systematic review. Level of evidence: Level 3. Data extraction: Data were extracted from 25 studies. Study methodology quality was evaluated using the Modified Coleman Methodology Score. Results: Intrinsic factors, previous injury, range of motion (lack or excess), and rotator cuff weakness (isometric and isokinetic) highly increase the risk of future injuries. Additionally, years of athletic practice, body mass index, sex, age, and level of play seem to have modest influence. As for the effect of scapular dysfunction on shoulder injuries, it is still controversial, though these are typically linked. Extrinsic factors, field position, condition of practice (match/training), time of season, and training load also have influence on the occurrence of shoulder injuries. Conclusion: Range of motion, rotator cuff muscle weakness, and training load are important modifiable factors associated with shoulder injuries. Scapular dysfunction may also have influence. The preventive approach for shoulder injury should focus on these factors.

Abstract:Handball is characterised by a high rate of injuries. Recent epidemiologic studies, which take the characteristics of handball into account, demonstrate a clear picture of injury patterns and injury risks for handball athletes. While more than half of all athletes can continue playing handball after medical treatment, the other half of injuries is accompanied by loss of training and match time. Although the sustained injuries are associated with a contact mechanism, they are not related to a high rate of foul play in the league setting. This shows that there is a high potential for prevention. Handball medicine strongly aims to primarily reduce and thus prevent injuries in this sport. The existing evidence for the potential of prevention of severe knee injuries must now be implemented into daily training. Due to the high prevalence of overuse injuries to the throwing shoulder, here, too, the first insights have been implemented in a prevention programme. This is described in detail in this article.

Abstract:Context: Sports involving overhead motions put substantial biomechanical demands on the shoulder and may result in injuries. Objective: To determine risk factors (RFs) for shoulder injuries in female athletes who play overhead sports and evaluate strategies to reduce shoulder injuries in these athletes. Data sources: A systematic electronic search was performed according to Preferred Reporting Items for Systematic Review and Meta-Analysis (PRISMA) guidelines. Databases included were PubMed, Tulane Matas Library Search Engine, and Google Scholar, with search terms: "Overhead injuries/Shoulder AND female athletes AND Risk Factors." Study selection: Of the initial 1574 studies identified, 314 were evaluated for eligibility by full-text review and 291 studies were excluded. Overall, 23 studies were included in this study. Studies were published from 2000 to 2021, subject age range was 15 to 35 years, with documented prevalence of shoulder injuries in female athletes playing overhead sports. Study design: Systematic review. Level of evidence: Level 2. Data extraction: Two independent researchers completed abstract and full-text review. Data extraction used the Covidence and Cochrane Consumer guide template. Results: Volleyball was the most common sport with shoulder injuries (6/23; 26%) followed by softball 5/23 (22%), swimming 5/23 (22%), gymnastics 4/23 (17%), tennis 3/23 (13%), water polo 2/23 (8%), and basketball 1/23 (4%). Six RFs (dominant shoulder, volume/overuse, time in sport, older age at time of injury, past injury, and multidirectional instability) were described. Of the 23 studies, 9 (39%) identified the dominant shoulder as a RF for sustaining injury (mean risk ratio [RR], 2.04), while 6 (26%) cited volume of repetition and overuse as a prominent RF (RR, 1.45). Conclusion: This systematic review demonstrates important RFs for shoulder injuries in female athletes associated with playing overhead sports. Multiple prevention strategies are described. Prevention programs are helpful in reducing the risk of reinjury.

Abstract:Studies in tennis injuries have successfully identified the incident rate, the location, and the type of the injury. The majority of the studies have multiple perspectives (epidemiology, biomechanics, performance), however only a few studies were able to identify risk factors or mechanisms that contribute to tennis injuries. Until now, there has not been a systematic literature review that identifies risk factors for tennis injuries. The objective of this review was to identify and critically appraise the evidence related to risk factors for upper limb injury in tennis players. A systematic review was conducted in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) framework, using a research question developed by the Patient Problem, (or Population) Intervention, Comparison or Control, and Outcome (PICO) methodology. The quality of the studies included was moderate to low, indicating prolonged tennis (exposure to tennis), scapular dyskinesis, muscle fatigue, scapulothoracic properties, shoulder kinetics or kinematics, skill level, and technique as risk factors for upper limb injury in tennis players. In this review, it is evidenced that the majority of tennis injuries are associated with overuse and a chronic time course, however, tennis injuries do not arise from a linear combination of isolated and predictive factors. Therefore, the multifactorial and complex nature of tennis injuries has to be further examined. The necessity of more randomized control trial studies is highly recommended.

Abstract:Soccer is a global widespread sport with a high injury rate, mostly to the lower limbs. Even though upper limb injuries are much less common among soccer players, their prevalence has increased in recent years. This is due to several reasons/factors, including new and more aggressive tactics, a rise in the number of younger players with different levels of skeletal maturation, and due to the growth of women’s participation in sports, which is in line with the fast progression in the degree of professionalism in women’s soccer. This review paper aims to (a) describe the most common injuries in the shoulder, arm, and elbow in soccer players and (b) to show the role of different imaging modalities in diagnosing upper extremity injuries that can occur in this sport. Familiarity with these injuries and their mechanisms will lead to a quicker diagnosis and correct reporting of imaging of soccer players.

Abstract:Lifestyles advocating for proper health and fitness have been trending in recent years, and as such, sports like weightlifting have become very popular worldwide. While these sports improve physical fitness and cardiovascular health, they carry an inherent risk for physical injuries, mainly to the shoulder. In this review, we aimed to explore the epidemiology of shoulder injuries in weightlifting using a systematic search of the literature. The databases PubMed, Google Scholar (pages 1–20), Embase, and SPORTDiscus were queried using relevant search terms to extrapolate all studies pertaining to shoulder injuries in these two sports. Shoulder injuries turned out to be common in both sports with varying incidence rates and were shown to occur to athletes independent of gender and age. Anterior instability and overuse injuries were the most common injury types, and presentation varied with regards to symptoms and severity. Both intrinsic and extrinsic factors can contribute to shoulder injuries in the setting of these two sports, including incorrect implementation of techniques, age, vulnerable positioning of the shoulder during the lift, and overtraining which leads to overuse injuries.

Abstract:Background: Shoulder injuries are prevalent in adolescent water polo (WP) players. Study aimed to determine whether preseason shoulder characteristics (range of motion [ROM], flexibility, and strength) and core endurance can identify athletes at risk of future shoulder injuries. Hypothesis: Shoulder characteristics, including changes in ROM (internal rotation [IR], external rotation [ER], and total), strength (IR and ER), pectoralis minor flexibility, shoulder capsule flexibility, and core endurance, would be risk factors for shoulder reinjury in athletes with previous overuse injuries compared with noninjured athletes. Study design: Prospective cohort study. Level of evidence: Level 3. Methods: At baseline, 53 male youth WP players (mean age, 16.6 ± 3.5 years) were assigned to Group 1 (with previous shoulder injuries [G1, n = 26]) and Group 2 (without previous shoulder injuries [G2, n = 27]). ROM, flexibility, strength, and core muscle endurance were assessed preseason. After a 12-month follow-up, players were again divided into those who developed new shoulder injuries (G3, n = 27) and those who remained healthy (G4, n = 26). Results: Total of 26 players (49%) had previous shoulder injuries at baseline. At baseline, decreased pectoralis minor flexibility, IR, total ROM, and core endurance were found in players with a previous shoulder injury compared with players without a previous shoulder injury (P < 0.05). Conclusion: At baseline, a significant difference was present in proposed risk factors (shoulder and core endurance parameters) between players with and without a previous shoulder injury. Shoulder IR ROM and years of experience were significant predictors of shoulder injury. Early detection of modifiable proposed risk factors may help prevent reinjury in young athletes. Clinical relevance: Screening at an early age can help identify and address pre-existing injuries, support youth athletes' return to sport after a shoulder injury, prevent new injuries, and improve performance.
Injuries in Japanese university handball: a study among 1017 players

Abstract:The aim of this study was to examine the injury profile of Japanese university handball players. In this cross-sectional study, a total of 1017 participants who played in the 2018 Japanese National University Handball Championship were followed. The incidence within the previous year was 0.59 [95% CI, confidence interval: 0.56-0.62] per player per year. The ankle (33.3%), knee (23.6%), and shoulder/clavicle (12.6%) were the body regions most affected by traumatic mechanisms, while the lumbar spine/lower back (26.0%), knee (15.7%), and shoulder (15.0%) were the body regions most affected by overuse mechanisms. Sprain (30.7%), ligamentous rupture (23.2%), and fracture (11.8%) were the main types of traumatic injuries, and stress fracture (25.0%) and lesion of meniscus or cartilage (25.0%) were the main types of overuse injuries. Female players were 1.5 times more likely (OR, odds ratio: 1.55 [95% CI: 1.20-2.01]) to experience an injury than their male counterparts, and line players were also 1.5 times more likely (OR: 1.49 [95% CI: 1.00-2.21]) to experience an injury than their wing counterparts. This study showed that there is a need for increased injury prevention efforts in Japanese handball, especially among female players.

Abstract:Water polo players' shoulders are exposed to repeated overhead and throwing motions as well as direct and indirect traumas. Shoulder injuries account for over half of all injuries sustained by water polo players. This is a monocentric descriptive epidemiological study on the clinical and radiological presentation of a consecutive series of water polo players from January 2002 to September 2022. All patients underwent clinical and physical examinations and an MRI arthrogram. A total of 92 water polo players were included in this study. Fifty-three patients (57.6%) reported at least one previous shoulder instability episode; 100% of patients in this group were diagnosed with a capsulolabral complex lesion, and 88.7% of these players complained of subjective symptoms of shoulder instability (RR: 4.4). A total of 39 out of 92 patients (42.4%) did not report previous shoulder dislocation episodes; the mean age at presentation in this group was significantly higher than the mean age of the patients who experienced previous instability episodes (p < 0.01), and the throwing arm was affected in 79.5% of patients (RR = 1.41).

Abstract:Background: Hand injuries are common in sports and associated with high dropout rates and costs. Hence, efforts should strive for further risk prevention measures in order to increase safety in sports. This implies knowledge of sports injury risk profiles. So far, major surveillance programs exist mainly in Anglo-American countries, reflecting the specific concerns of sports in this part of the world. Data on sports injuries within Europe are scarce. As sports behaviour appears to vary demographically, we hypothesised that risk injury profiles differ as well. Methods: To assess whether the described sports injuries of the hand are applicable to the German population, we performed a five-year retrospective, single-centre analysis of sports-related hand injuries, using data from the Enterprise Clinical Research Data Warehouse of the Hannover Medical School. Results: Notable differences in comparison to other data were observed. Ball sports, cycling and equestrian sports caused most of the recorded hand injuries, which were predominantly fractures of the wrist and hand. Hand injuries in equestrian sports were associated with significantly higher operation and hospitalisation rates as well as a significantly longer inpatient treatment. Conclusion: Risk profiles for sports-related hand injuries appear to differ not only in terms of age- and sex, but also geographically. Nation- and Europe-wide hand trauma registries as well as a broad registry participation are necessary in order to accurately assess the risk patterns in Europe; henceforth reducing hand injuries and their sequelae.
Incidence, prevalence, and risk factors for elbow and shoulder overuse injuries in youth athletes: A systematic review

Abstract:In order to create sport‐specific preventive measures to enhance healthy sport participation in youth athletes, valid data on incidence, prevalence, and etiology of overuse injuries have to be identified. This systematic review aims to provide an update on incidence, prevalence, and risk factors for overuse injuries of the elbow and shoulder in youth athletes participating in overhead sports. MEDLINE and EMBASE were searched from inception until January 19, 2018, for studies that reported incidence, prevalence, or risk factors for overuse injuries or pain of the elbow and shoulder in youth overhead athletes. In total, 36 studies were included. Incidence rates of overuse injuries of the elbow and shoulder were highest in tennis, baseball, and softball. Incidence and prevalence of elbow pain in baseball were 12%‐58% and 17%‐35%, respectively. Incidence of shoulder pain was 8%‐32% for baseball, 47% for basketball, 63% for handball, and 52% for volleyball. Prevalence of shoulder pain was 9%‐16% for baseball, 41% for basketball, 32%‐49% for handball, and 40% for volleyball. Risk factors for pain were almost exclusively described for youth baseball players and included playing position, throwing technique, and intensity and duration of training. Incidence and prevalence of overuse injuries and pain of the elbow and shoulder are high in youth overhead athletes. Interpretation is challenging as different definitions are used to report overuse injuries and pain. Risk factors in youth baseball are largely covered by preventive pitch‐count restrictions; knowledge on risk factors in other overhead sports is currently lacking.
Video analysis of acute injuries and referee decisions during the 24th Men's Handball World Championship 2015 in Qatar

Abstract:Although handball is a contact sport with a high risk of acute match injuries, their mechanisms have not yet been investigated. We aimed to describe the mechanisms of acute match injuries in elite male handball and evaluate referee performance in injury situations. Based on injury surveillance from the 24th Men's Handball World Championship 2015 in Qatar, injury situations and the referee decisions were identified on video footage. A total of 55 injury situations and 37 referee decisions were included for analysis. The injury situations were analyzed individually by five handball experts, followed by a consensus meeting. An expert referee panel performed individual blinded evaluation of the referee decisions, followed by an online consensus meeting. Injuries were evenly distributed among attackers (n = 29) and defenders (n = 26). The most frequent injury cause was contact trauma due to a tackle (n = 27). At the time of injury, attackers were most frequently performing a jump shot (n = 9), while defenders were completing a tackle (n = 10). Defenders most commonly tackled the throwing arm (n = 7) or toward the head/face region (n = 6) of injured attackers, while attackers most frequently hit injured defenders with the knee during jump shots (n = 5). Agreement between the referees and the expert panel was weak (kappa: 0.22, 95% CI 0.07 to 0.36), with substantially more lenient rule interpretation by the referees. Our results suggest that stricter refereeing and rule amendments should be considered to prevent acute match injuries in elite handball, especially in relation to tackling episodes when an attacker is performing a jump shot.
Ligament Injuries in Professional Male Handball Players: A 6-Year Longitudinal Study

Abstract:Handball is a sport that involves high-intensity actions throughout the game, such as sprints, jumps, landings, and high-speed, repeated throws. This, along with competitive and tactical factors, congested schedules, and the need to maintain a high level of performance throughout the season, contributes to a high injury rate. This study aimed to analyse ligament injuries in a professional handball team over six consecutive seasons. A total of 68 elite male Spanish handball players participated, with 54 time-loss injuries (i.e., injuries involving at least one day of absence) observed during this study period. Ligament injury information was recorded following the International Olympic Committee consensus statement. The overall incidence was 0.89 ligament injuries per 1000 h of exposure. Additionally, a higher incidence and burden of ligament injuries was observed during match-play compared to training. Most ligament injuries were classified as minor or moderate (i.e., 79.63% of the total), and 46.29% were reinjuries. A significantly higher incidence of ligament injuries was suffered in the lower limbs compared to the upper limbs (0.81 vs. 0.08 ligament injuries per 1000 h; p < 0.001). Specifically, the highest incidence was observed in the anterior talofibular ligament of the ankle (0.57 injuries per 1000 h of exposure), while the greatest burden was related to the anterior cruciate ligament (24.08 absence days per 1000 h of exposure). This study provides an overview of ligament injuries among professional handball players, highlighting the need to implement strategies with positive effects during competition (e.g., specific activation strategies or training programmes based on strength and balance) and to reduce injury recurrences.

Abstract:Handball is a contact sport with a high prevalence of acute injuries. Injuries often occur during high-speed movements involving several players, making it difficult to describe the exact mechanism of injury. Describe the mechanisms of injury in elite male handball. Prospective video analysis. During the men´s handball world championship 2015, 132 injuries were reported through on-site injury surveillance by team medical staff. Based on the injury reports, two operators accessed the videos of each game and identified the specific injury situations using video analysis software (ProzoneHANDBALL v.1.0.0.14, ProzoneSports, Leeds, UK). A total of 97 injury videos were identified; 42 were excluded, as the injury situation was not visible. Five handball experts performed an individual analysis of the 55 included videos based on a specific form developed to describe the injury situations and causes, followed by a consensus meeting. The form included variables related to the playing situation, landing situation, action of attacker and defender at time of injury and localization of tackle/hit. The most common injury location was the head and face (n=17, 31%), followed by the ankle (n=6, 11%). The most frequent injury cause was opponent contact (n=27, 49%), followed by landing trauma subsequent to opponent contact (n=8, 15%). The injuries were equally distributed between attackers (n=29, 53%) and defenders (n=26, 47%), but time-loss injuries (49%) were more common among attackers (68%). The most frequent playing action performed by injured attackers was a jump shot (n=9, 31%); tackles were most often to their throwing arm (n=7, 30%). Acute injuries in elite male handball are equally distributed between attackers and defenders. However, attackers sustain more severe injuries. The most frequent injury causes are direct opponent contact and landing trauma after opponent contact.

Abstract:The incidence of first‐time shoulder dislocation in professional football is increasing and currently amounts to 9.1 per 1000 matches played. A conservative treatment resulted in a quicker return to competition, while frequency and time to redislocation were similar regardless of treatment. The increasing incidence urges an analysis of injury mechanisms and the implementation of preventive measures. Purpose Injuries exert a detrimental impact on the team performance in professional football. Shoulder dislocations are known for their severity and often cause substantial lay‐off times. However, there is a notable gap in the general understanding regarding the frequency and convalescence of these events in professional football. Thus, the aims of this study were (1) to determine the incidence, (2) to evaluate return to competition (RTC) and redislocation events (RDEs) and (3) to identify relevant player‐, match‐ and injury‐related parameters. Methods All shoulder dislocations within the highest three German football leagues (1.–3. Bundesliga) during the seasons 2012/2013 until 2022/2023 were identified. Player‐, injury‐ and match‐related data were obtained by performing a media analysis. Data were analysed using descriptive statistics. p Value was set at 0.05. Abstract:Aim We aimed to identify patterns and mechanisms of injury situations in men’s professional handball by means of video match analysis. Methods Moderate and severe injuries (absence of >7 days) sustained in competition in one of six seasons (2010 to 2013 and 2014 to 2017) in men’s professional handball were prospectively analysed with a newly developed standardised observation form. Season 2013 to 2014 was excluded because of missing video material. Results 580 injuries were identified: 298 (51.4%) contact injuries, 151 (26.0%) indirect contact injuries and 131 (22.6%) non-contact injuries. Head (87.5%), hand (83.8%), shoulder (70.2%) and ankle (62.9%) injuries were mainly sustained during direct contact. Typical contact injuries included collision with an opponent’s upper extremity or torso, and ankle injuries mainly consisted of foot-to-foot collisions. A large proportion (41.7%) of knee injuries were caused by indirect contact, whereas thigh injuries mainly occurred (56.4%) through non-contact mechanism. Wing (56.9%) and pivot (58.4%) players had the highest proportion of contact injuries, whereas backcourt players had a high proportion of indirect contact injuries (31.5%) and goalkeepers of non-contact injuries (48.9%). The injury proportion of foul play was 28.4%. Most injuries occurred in the central zone between the 6-metre and 9-metre lines (26.1%) and during the last 10 min of each match half (OR 1.71, p=0.016). Conclusions In men’s professional handball in a league setting, contact — but not foul play — was the most common mechanism associated with moderate and severe injuries. Head, hand, shoulder and ankle injury were mainly sustained during direct contact.

Shoulder Internal Rotator Strength as Risk Factor for Shoulder Pain in Volleyball Players

Abstract:The aim of this study was to identify the intrinsic factors that could increase risk of shoulder pain in adolescent volleyball players. Twenty-eight young male volleyball players (between 14 and 18 years old) participated in this study. Athletes were submitted to: isokinetic muscle strength assessment of shoulder rotator muscles, ball service speed assessment, anterior and posterior drawer test, apprehension test, groove sign and scapular dyskinesia test. Athletes were followed for 16 weeks to monitor the presence of shoulder pain. All athletes were submitted to the same training protocol. During the 16 weeks, 28.5% of the athletes (n=8) experienced shoulder pain in the dominant limb higher than 3 according to Numerical Rating Scale criteria; 71.5% of the athletes (n=20) did not experience pain, or pain equal or lower than 3. The main result of our study was that the odds of feeling pain higher than 3 was significantly higher among players who presented higher values for internal rotation peak torque (OR=1.113, CI 95%=1.006 to 1.232 and p=0.038). The odds of feeling pain increased by 11% for every N·m of the internal rotator muscles. Pre-season isokinetic rotator strength assessments can help identify adolescent volleyball players at increased risk of a shoulder injury. Handball-specific loading acutely reduces the acromiohumeral distance in experienced handball players and in non-handball experienced athletes

Abstract:Context: When playing handball, the preservation of the subacromial space, which can be quantified by the acromiohumeral distance (AHD), plays a crucial role for shoulder health of handball players. Acute effects of handball-specific loading on the subacromial space with consideration of individual adaptions resulting from long-term handball-specific loading experience have yet to be determined in order to prevent injuries such as e. g. an impingement of the supraspinatus tendon. Objective: To (1) assess the acute effects of handball-specific loading on the AHD in healthy experienced handball players (HB) and non-handball experienced athletes (CG) and (2) to assess the AHD behavior in relation to individual intrinsic factors to identify possible risk factors and the effect of handball-specific experience associated adaptations. Participants: 20 HB (10m; 10f) and 20 CG (10m; 10f); 24 ± 5 years. Intervention: Handball-specific loading protocol. Main outcome measures: The AHD was measured by ultrasonography at 0° and 60° abduction pre and post intervention. Isometric shoulder strength was measured with hand-held dynamometry. Shoulder range of motion (ROM) was measured with goniometry. Results: Handball-specific loading led to significantly reduced AHD in the dominant shoulder in the 60° abducted position in both groups (HB: -1.7 ± 2.0 mm; p = 0.001, d = 0.69; CG: -1.1 ± 2.0 mm; p = 0.024, d = 0.37) and in the non-dominant shoulder in 0° (-0.7 ± 1.5 mm; p = 0.038, d = 0.35) and 60° abducted position (-1.3 ± 1.8 mm; p = 0.004, d = 0.69) in HB only. Handball-specific loading enhanced AHD reduction when elevating the shoulder from 0° to 60° in both groups and arms. Larger shoulder abduction strength affected the maintenance of the AHD positively. HB demonstrated less shoulder strength compared to CG, while ROM did not differ. Conclusions: Handball-specific loading can affect the ability to preserve the subacromial space which might put handball players at risk for shoulder injuries. Poor shoulder strength can aggravate this mechanism. Therefore, implementation of strengthening exercises of the external rotator and abductor muscles in the training schedule may improve shoulder health of handball players.
Prevalence of sport injuries in Olympic combat sports: a cross-sectional study examining one Olympic period

Abstract:Background: Due to full contact impacts in combat sports, the risk of injuries is elevated. The aim of this study is to report severe injuries among athletes in Olympic combat sports. Specific injury types, time loss, and the performance level after injury are examined. Methods: Survey to investigate injuries in Olympic combat sports during the time period from 2012-2016. Reported injuries were analyzed by anatomical location, injury type, gender, time loss, and performance level. Results: The three injuries resulting in the longest time loss (ACL rupture: judo= 37 weeks; karate = 49 weeks; shoulder dislocation: wrestling = 41 weeks; shoulder rotator cuff injury: wrestling = 32 weeks) also accounted for the largest proportion of athletes with career-ending injuries (ACL rupture: judo = 28%; karate = 67%; shoulder dislocation: wrestling = 40%; shoulder rotator cuff injury: wrestling = 50%). Taekwondo and fencing had the shortest time loss (<12 weeks) among all combat sports. More injuries occurred during training (58%) as compared to competition (42%). Injury prevalence of competitive athletes was significantly higher as compared to recreational athletes. Male athletes suffered significantly more anterior cruciate ligament injuries (72% vs. 56%; P<0.05), unspecific shoulder injuries (89% vs. 47%; P<0.01), and elbow ligament injuries (57% vs. 30%; P<0.05) during training. Conclusions: The study shows that there are important differences between "punching and kicking" and "throwing" martial arts in terms of specific injury types. In judo and wrestling, the injuries are more likely to affect the joints (knee and shoulder). Therefore, injury prevention these sports should focus on strength training of the muscles surrounding the joints and on defensive reactionary movements to avoid dangerous biomechanical joint angles. In "punching and kicking" sports, injuries of the hands and feet, due to the large impact forces of strikes and kicks, could be reduced by improved protective equipment on hands and feet.
