Abstract:Dissolving microneedles have been employed as a safe and convenient transdermal delivery system for drugs and vaccines. To improve effective drug delivery, a multilayered pyramidal dissolving microneedle patch, composed of silk fibroin tips with the ability of robust mechanical strength, rapid dissolution and drug release supported on a flexible polyvinyl alcohol (PVA) pedestal is reported. To show the utility of this approach the ability of the fabricated microneedles to deliver insulin is demonstrated. The dissolving microneedles have sufficient mechanical strength to be inserted into abdomen skin of mice to a depth of approximately 150μm, and release their encapsulated insulin into the skin to cause a hypoglycemic effect. The fabrication of microneedles avoids high temperature which benefits storage stability at room temperature for 20d. This result indicates >99.4% of insulin remained in the microneedles. In comparison to traditional needle-based administration, the proposed multilayered pyramidal dissolving microneedle patches enable self-administration, miniaturization, pain-free administration, drug delivery and drug stability, all being important features in needle free drug delivery.

Abstract:To reduce the pain caused by subcutaneous injections, microneedle patches as the new transdermal drug delivery method are gaining increased attention. In this study, we fabricated a composite insulin-loaded microneedle patch. Silk fibroin, a natural polymer material, was used as the raw material. The tip of the microneedle had good dissolving property and was able to dissolve rapidly to promote the release of insulin. The pedestal had the property of swelling without dissolving and was carrying insulin as a drug store. The insulin carried by the pedestal could release continuously through the micropore channels created by the microneedles. This kind of microneedle could achieve a sustained release effect. It was observed that the insulin had good storage stability in this kind of microneedle, and it maintained more than 90% of its biological activity after 30 days. The results of transdermal delivery to diabetic rats showed that the microneedle patches displayed an apparent hypoglycemic effect and indicated a sustained release effect. These drug-loaded silk microneedle patches may act as potential delivery systems for the treatment of diabetes.

Abstract:Insulin administration at mealtimes for the control of postprandial glucose is a major part of basal-bolus insulin therapy; however, painful subcutaneous (SC) injections lead to poor patient compliance. The microneedle (MN) patch, which allows painless transdermal drug delivery, is a promising substitute; however, it remains a big challenge to deliver insulin as rapidly as by SC injection. Here a novel MN patch is designed in which the MNs are coated with insulin/poly-l-glutamic acid (PGA) layer-by-layer (LBL) films at pH 3.0. This coating is pH-sensitive because the net charge of insulin turns from positive to negative when the pH increases from 3.0 to 7.4. As a result, when transferred to pH 7.4 media, e.g., when inserted into skin, the coating dissociates instantly and releases insulin rapidly. A brief epidermal application (<1 min) of the coated MNs is enough for complete film dissociation. More importantly, the coated MN patch exhibits a pharmacokinetic and a pharmacodynamic profile comparable to that of insulin administrated by SC injection, suggesting the coated MN patch can deliver insulin as rapidly as the SC injection. In addition, the patch exhibits excellent biocompatibility and storage stability. The new MN patch is expected to become a painless, convenient method for the control of postprandial glucose.

Abstract:Aim: Insulin therapy require self-administration of subcutaneous injection leading to painful and inconvenient drug therapy. The aim is to fabricate nanoemulsion (NE) based insulin loaded microneedles with improved bioavailability and patient compliance.Materials & methods: Different ratios of polyvinyl alcohol and polyvinylpyrrolidone as polymers were prepared through micro-molding technique for microneedles. Characterization of were performed using scanning electron microscope, differential scanning calorimetry, Fourier-transform infrared spectroscopy and circular dichroism. Mechanical strength, hygroscopicity and pain perception of these microneedles were also evaluated. In vitro release, permeation and in vivo PK/PD study of NE-based microneedles were conducted.Results: NE-based microneedles of insulin have improved bioavailability and quick response.Conclusion: Microneedles loaded with insulin can be effectively delivered insulin transdermally to treat diabetes with increased convenience and patient compliance.

Abstract:To reduce the inconvenient and painful of subcutaneous needle injection, the polymer microneedle patches that fabricated from modified alginate and hyaluronate were prepared for transdermal delivery of insulin. The as-prepared microneedles (MNs) exhibited excellent mechanical strength to penetrate the skin and good degradability to release loaded insulin. In vitro skin insertion capability was determined by staining with tissue-marking dye after insertion, and the real-time penetration depth was monitored using optical coherence tomography. Confocal microscopy images revealed that the rhodamine B and fluorescein isothiocyanate-labeled insulin (FITC-insulin) can gradually diffuse from the puncture sites to deeper tissue. In vivo and pharmacodynamic studies were then conducted to estimate the feasibility of the administration of insulin-loaded microneedle patches on diabetic mice for glucose regulation. The relative pharmacologic availability (RPA) and relative bioavailability (RBA) of insulin from microneedle patches were 90.5±6.8% and 92.9±7%, respectively. These results suggests the MNs developed in this study have a promising application in diabetes treatment via transdermal delivery.

Abstract:Severely diabetic patients need insulin input to maintain the body's glycemic balance. However, traditional injection methods are often associated with poor adherence and an increased risk of hypoglycemia. Microneedle technology offers a promising solution by minimizing pain and trauma during insulin administration. Nonetheless, achieving prolonged glycemic control by microneedle with high insulin loading remains a significant challenge. Herein, we introduce an innovative microneedle patch that draws inspiration from the elegant lightinduced blooming of water lily petals. The patch features a glucose-responsive hydrogel network crafted from two modified polysaccharide polymers, which enables the delivery of long-acting insulin without depending on glucose oxidase. By incorporating phenylboronic acid-modified sodium alginate, quaternary ammonium chitosan, and polyvinyl alcohol into a hydrogel matrix, we have created a microneedle system that harbors dynamic borate ester linkages and electrostatic attractions, resulting in heightened sensitivity to blood glucose levels. The electrostatic interaction acts as a relatively stable crosslinking point, balancing the dynamic reproducibility response based on the borate ester bond. This self-adaptive hydrogel can regulate insulin-controlled release by responding to changes in glucose concentration. Herein, we achieved massive insulin loading (20 IU) with long lasting glycaemic control (48 h) in a single treatment of diabetic SD rats.
"Smart" Matrix Microneedle Patch Made of Self‐Crosslinkable and Multifunctional Polymers for Delivering Insulin On‐Demand

Abstract:A self‐crosslinkable and multifunctional polymer‐based matrix microneedle (mMN) patch is developed, which is capable of stabilizing highly concentrated insulin and delivering insulin on demand as demonstrated by in vitro analyses and in vivo efficacy studies. The introduced crosslinking strategy eliminates conventionally used harsh conditions for making crosslinked microneedle (MN) patches and thus enables patch fabrication with encapsulated susceptible protein in one step. A transdermal patch that delivers insulin at high glucose concentrations can offer tremendous advantages to ease the concern of safety and improve the quality of life for people with diabetes. Herein, a novel self‐crosslinkable and glucose‐responsive polymer‐based microneedle patch (MN) is designed to deliver insulin at hyperglycemia. The microneedle patch is made of hyaluronic acid polymers functionalized with dopamine and 4‐amino‐3‐fluorophenylboronic acid (AFBA) that can be quickly crosslinked upon mixing of the polymer solutions in the absence of any chemicalcrosslinking agents or organic solvents. The catechol groups in the dopamine (DA) units form covalent crosslinkages among themselves by auto‐oxidation and dynamic crosslink with phenylboronic acid (PBA) via complexation. The reversible crosslinkages between catechol and boronate decrease with increasing glucose concentration leading to higher swelling and faster insulin release at hyperglycemia as compared to euglycemia. Such superior glucose‐responsive properties are demonstrated by in vitro analyses and in vivo efficacy studies. The hydrogel polymers also preserve native structure and bioactivity of insulin, attributable to the interaction of hyaluronic acid (HA) with insulin molecules, as revealed by experiments and molecular dynamics simulations. The simplicity in the design and fabrication process, and glucose‐responsiveness in insulin delivery impart the matrix microneedle (mMN) patch great potential for clinical translation.

Abstract:Utilizing the unique virtue of polyvinyl alcohol (PVA) of forming microcrystalline domains as the cross-linking junctions, a new microneedle system, phase-transition microneedle (PTM) patch, is invented, which enables highly efficient transdermal delivery of insulin without depositing the needle tip materials to the skin. PTM, formed of biocompatible PVA as the main component, is sufficiently strong for its needle tip to penetrate the epidermis in the dry state, release preloaded cargos by absorbing body fluid in the dermis layer nearly as fast as subcutaneous injection, and retain mechanical toughness in the hydrated state to ensure complete removal from the skin. The microcrystalline cross-linking enables a protein-friendly fabrication process free of hazardous cross-linking agents required for chemical and ionic cross-linking. Pharmacokinetic and efficacy studies of insulin-loaded PTM using pig models indicate a transdermal bioavailability over 20%, similar deviations and peak width, only 18 min behind Tmax, and lower glycated hemoglobin (HbA(1c)) as compared with injection pens. The complete removability of hydrated needle tips may endow PTM with an additional safety insurance, terminating medication whenever hypoglycemia becomes a concern. PTM patch is practically applicable to a variety of protein/peptide medicines requiring frequent dosing by offering painless administration, freedom of refrigeration, and minimal safety concerns.
Fabrication and Evaluation of Transdermal Delivery of Carbamazepine Dissolving Microneedles

Abstract:This project aims to prepare hydrogel microneedle patches (MNs) as a painless method to deliver carbamazepine transdermally. This can be used as a sustained release system that offers the advantages of lower gastrointestinal side effects and avoids the first-pass metabolism of the drug. MNs were composed of two medicated layers, a microneedle layer and a base layer. MNs were fabricated using polyvinyl alcohol with or without polyvinylpyrrolidone Kollidon 30 as a matrix polymer and in the presence of selected solubilizing agent (polyethylene glycol 400, Tween 80, or α-tocopherol polyethylene glycol). Freezing–thawing cycle was evaluated as one of the processing parameters that may affect the drug release. The MNs were evaluated for their weight variation, base thickness, and content uniformity. The physicochemical compatibility between carbamazepine and the polymers was estimated by Fourier transform infrared spectroscopy, differential scanning calorimetry, thermogravimetric analysis, and X-ray powder diffraction. Evaluation for the in vitro release studies and ex vivo permeation studies was performed. The prepared MNs were flexible, clear, and uniform in weight, base thickness, and drug content. Physicochemical characterizations showed that carbamazepine was amorphous in most of the MNs. In vitro release and ex vivo permeation studies of carbamazepine were significantly higher for MNs containing a combination of 1:1 w/w of PEG 400 and Tween 80 as solubilizing agents where the release was extended over 96 h, with the release of 85.2% and 59.6% permeation percentage compared to other MNs. A significant effect of the freezing–thawing cycle on the release profile of the drug was observed. The hydrogel MNs are shown to be stable under the studied storage conditions.Graphical

Abstract:In this study, we propose a novel method for the preparation of dissolvable microneedle (DMN) patch directly attached on a medical adhesive tape for transdermal drug delivery. The fabrication process is simple and time-saving without second curing procedures and the fabricated DMN patch exhibits high flexibility and dosage accuracy owing to the property of the medical adhesive tape. Sodium carboxymethyl cellulose (SCMC) was adopted as matrix material due to its biodegradability and mechanical hardness. The MN patch shows great ability to penetrate skin ex vivo and in vivo by utilizing porcine skin and the living mouse. Most importantly, it possesses good capacity to carry drugs and locally release drug molecules into mouse skin by adopting Rhodamine B as the drug analog.

Abstract:Microneedles are primarily designed for enhancing transdermal drug delivery in a minimally invasive manner. In particular, coated microneedles have attracted increasing attention due to the resulting mode of rapid and highly efficient drug administration. In this study, we developed a novel gelatin/sucrose film-coated poly(ethylene glycol)diacrylate (PEGDA) microneedle patch, which can effectively improve the skin permeability of therapeutic drugs and realize convenient and efficient drug administration. A simple and reliable technique was proposed to produce uniform film coatings on microneedles at room temperature. After introducing a prepolymer solution into the mold cavity, a film-coated microneedle patch was fabricated via a photo-induced polymerization process. Four categories of molecular models, including rhodamine B (RhB), bovine serum albumin (BSA), doxorubicin (DOX) and indocyanine green (ICG), were encapsulated into the gelatin/sucrose film to evaluate their transdermal delivery and therapeutic effects both in vitro and in vivo. The presented methodology represents a facile technique for rapid formation of film-coated microneedles for efficient transdermal delivery of small molecular drugs and large proteins.
Fabrication of Composite Microneedles Integrated with Insulin-Loaded CaCO3 Microparticles and PVP for Transdermal Delivery in Diabetic Rats.

Abstract:Dissolving microneedles (MNs) display high efficiency, safety and painless in transdermal delivery for poorly permeable drugs. Here, a dissolving composite MNs prepared from insulin-loaded CaCO3 microparticles (INS-CaCO3 MPs) and poly(vinyl pyrrolidone) (PVP) as matrix were fabricated with two-step centrifuging and molding process to immobilize insulin in the MNs. The as-prepared INS-CaCO3/PVP MNs exhibited remarkable mechanical strength and slower solubility properties when compared with pure PVP MNs. In vitro skin insertion capability was determined by staining with FITC-labeled insulin which was revealed after insertion, gradually diffusing from the puncture sites to deeper tissues. In vivo pharmacodynamic studies were then conducted to estimate the feasibility of the administration of insulin-loaded dissolving MNs on diabetic rats for glucose regulation. The relative pharmacological availability (RPA) and relative bioavailability (RBA) of insulin from MNs were 98.2 and 96.6%. Thus, this study suggests that the use of INS-CaCO3/PVP MNs achieved both high efficiency and constant release of insulin in comparison with the traditional subcutaneous injection approach and presented a promising device to delivery insulin for diabetic therapy.
Development and in vitro tuning of empagliflozin-containing dissolvable microneedle patch for enhanced transdermal delivery

Abstract:This painless method allows drugs to penetrate the outer skin layer, offering several advantages over alternative administration routes, including ease of use and the ability to bypass enterohepatic circulation. Among transdermal drug delivery systems (TDDS), microneedle patches (MNPs) are emerging as an innovative approach for minimally invasive drug delivery, enhancing the skin permeation of substances ranging from macro to micro sizes. This study explores dissolvable microneedle patches (dMNPs) as a novel method to improve the systemic delivery of empagliflozin, an SGLT-2 inhibitor, commonly used to manage type 2 diabetes mellitus (T2DM). However, its oral administration poses challenges due to rapid absorption, variable bioavailability, and a moderate half-life that necessitates frequent dosing. The microneedle patches were manufactured using a mold-based solvent casting technique, utilizing a polymer-drug combination that dissolves upon skin application. The development of the dMNPs involved polyvinylpyrrolidone and polyvinyl alcohol. Characterization of the formulated dMNPs included scanning electron microscopy (SEM), Fourier transform infrared spectroscopy (FTIR), differential scanning calorimetry (DSC), thermogravimetric analysis (TGA), powder X-ray diffraction (PXRD), dissolution testing, tensile strength analysis, percentage elongation measurement, mechanical strength assessment, and skin irritation studies. The optimized dMNP formulation (MP6) exhibited notable characteristics such as sharp, uniform, pyramid-shaped needles, stability at elevated temperatures, crystalline structures of the drug and polymers, controlled weight loss upon heating, effective drug dissolution, optimal tensile strength, penetration depth, moisture content, elongation capability, and a favorable release rate. In vitro release demonstrated the enhanced properties of the dissolvable microneedle patches, with zero-order kinetics identified as the most suitable model for MP6 based on regression coefficient analysis. Overall, the characterization studies, in vitro skin irritation evaluation, confirmed the stability and biocompatibility of the optimized dMNPs, making them suitable for transdermal application.

Abstract:The traditional method of injecting insulin requires multiple injections to stimulate the insulin secretion pattern physiologically, and injections have compliance and comfort issues. Microneedle s can reduce pain and are a patient-friendly technique. In this study, silk fibroin was utilized as a drug delivery system, which exhibited an excellent therapeutic effect by maintaining proper insulin activity. It makes a noninvasive alternative to traditional insulin administration by injection a reality and improves insulin use safety. The swelling capacity of the needle controls the release of insulin to stabilize the blood glucose concentration and achieve a long-term effect of lowering blood sugar. The relative effectiveness and bioavailability of the insulin-loaded microneedle patches were 69.4 +/- 7.2% and 64.6 +/- 9.5%, respectively, in hypoglycemic therapy in diabetic rats. Silk microneedle patches provide a new idea for solving the problem of cold chain transportation and bring new hope for the treatment of diabetes.

Abstract:Most reported microneedle patches still suffered from a complicated preparation process and insufficient loading capacity for the glucose-responsive delivery system . Herein, based on the reversibility of the boronate ester bond, a novel gel was formed by mixing phenylboronic acid grafted sodium hyaluronate and polyvinyl alcohol. The insulin-loaded microneedle patch could be directly obtained by simply blending high-dose insulin and gel into the mold. The introduction of cellulose nanofiber was also found to be in favor of mechanical strength. The glucose responsibility could be achieved when the boronate ester bond was broken by glucose, leading to the change of crosslinking density and further controlling insulin release. In the hypoglycemic experiment of diabetic rats, the microneedle patches could effectively puncture the skin and maintain normal blood glucose levels for an extended period. The facile preparation process and high loading capacity give this microneedle patch a great chance in mass production.

Abstract:Microneedles offer minimally invasive, user-friendly, and subcutaneously accessible transdermal drug delivery and have been widely investigated as an effective transdermal delivery system. Ibuprofen is a common anti-inflammatory drug to treat chronic inflammation. It is crucial to develop microneedle patches capable of efficiently delivering ibuprofen through the skin for the effective treatment of arthritis patients requiring repeated medication. In this study, the mechanical properties of a new type of polymer microneedle were studied by finite element analysis, and the experimental results showed that the microneedle could effectively deliver drugs through the skin. In addition, a high ibuprofen-loaded microneedle patch was successfully prepared by micromolding and subjected to evaluation of its infrared spectrum morphology and dissolve degree. The morphology of microneedles was characterized by scanning electron microscopy, and the mechanical properties were assessed using a built linear stretching system. In the in-vitro diffusion cell drug release test, the microneedle released 85.2 ± 1.52% (210 ± 3.7 μg) ibuprofen in the modified Franz diffusion within 4 h, exhibiting a higher drug release compared to other drug delivery methods. This study provides a portable, safe and efficient treatment approach for arthritis patients requiring daily repeated medication.

Abstract:In this study, we constructed a novel powder-laden core-shell crosslinked chitosan microneedle patch for high-dose and controllable delivery of various drugs, including both macromolecular biological drugs and small-molecule chemical drugs. Direct loading of drug powders greatly improved drug loading capacity and minimized degradation. The results of the in vitro drug release study suggested that the release behaviors of the most tested drugs (both macromolecular drugs and small-molecule drugs) can be tuned by adjusting the crosslink density of the microneedle shell to achieve either rapid or sustained release of the loaded drug. The in vivo hypoglycemic efficacy test in streptozotocin-induced diabetic mice further proved that the onset and duration of the insulin-laden patch can be customized by adjusting the crosslink density. Furthermore, a combination of microneedle patches with different crosslink densities not only rapidly reduced blood glucose levels to normoglycemic levels (within 1 h) but also maintained normoglycemia for up to 36 h. The insulin loaded in the patch also showed good stability during storage at 40 °C for 6 months. Our results suggest that this powder-laden patch represents a strong candidate for addressing the multiple challenges in the preparation and application of polymeric microneedles and shows promise in clinical applications.
Assisted 3D Printing of Microneedle Patches for Minimally Invasive Glucose Control in Diabetes.

Abstract:Recently, microneedle systems have become an attractive solution for drug delivery. Traditionally, microneedle systems were fabricated via template molding methods. In this study, we present a novel method for the fabrication of a microneedle patch system. We used extrusion-based 3D printing and post stretching to fabricate a microneedle patch system for minimally invasive and glucose-responsive insulin delivery for diabetes treatment. First, we investigated the printability of various bioinks composed of alginate with hydroxyapatite as an additive. After printing the substrate and a cylindrical array of the patch, we stretched the top surface of the cylindrical array to form needle-like tips. The prepared microneedle patch contained 6 × 6 microneedles, and only the microneedles performed glucose-responsive release of insulin. Each microneedle was conical in shape, with a base diameter of 601 μm, a tip diameter of 24 μm, and a height of 643 μm. The fabricated microneedles exhibited sufficient mechanical strength to penetrate the skin of mice and responsively released insulin according to the glucose levels both in glucose solution and in type 1 diabetic mice. When transdermal application was performed only once on the skin of mice, the microneedle patches regulated the blood glucose levels of diabetic mice in normoglycemic ranges for up to 40 h and alleviated the diabetic symptoms of the mice. Our study proposed a method for the fabrication of microneedle patch systems, which can be potentially applied for transdermal drug delivery.
Innovative freeze-drying technique in the fabrication of dissolving microneedle patch: Enhancing transdermal drug delivery efficiency

Abstract:Microneedle patch (MNP) has become a hot research topic in the field of transdermal drug delivery due to its ability to overcome the stratum corneum barrier. Among the various types of microneedles, dissolving microneedles represent one of the most promising transdermal delivery methods. However, the most used method for preparing dissolving microneedles, namely microfabrication, suffers from issues such as long drying time, susceptibility to humidity, and large batch-to-batch variability, which limit the development of dissolving microneedles. In this study, we report for the first time a method for preparing dissolving microneedles using freeze-drying technology. We screened substrates suitable for freeze-dried microneedle patch (FD-MNP) and used coating technology to enhance the mechanical strength of FD-MNP, allowing them to meet the requirements for skin penetration. We successfully prepared FD-MNP using hyaluronic acid as the substrate and insulin as the model drug. Scanning electron microscopy revealed that the microneedles had a porous structure. After coating, the mechanical strength of the microneedles was 0.61 N/Needle, and skin penetration rate was 97%, with a penetration depth of 215 μm. The tips of the FD-MNP dissolved completely within approximately 60 s after skin penetration, which is much faster than conventional MNP (180 s). In vitro transdermal experiments showed that the FD-MNP shortened the lag time for transdermal delivery of rhodamine 123 and insulin compared to conventional MNP, indicating a faster transdermal delivery rate. Pharmacological experiments showed that the FD-MNP lowered mouse blood glucose levels more effectively than conventional MNP, with a relative pharmacological availability of 96.59 ± 2.84%, higher than that of conventional MNP (84.34 ± 3.87%), P = 0.0095. After storage under 40°C for two months, the insulin content within the FD-MNP remained high at 95.27 ± 4.46%, which was much higher than that of conventional MNP (58.73 ± 3.71%), P < 0.0001. In conclusion, freeze-drying technology is a highly valuable method for preparing dissolving microneedles with potential applications in transdermal drug delivery.

Abstract:A novel glucose‐responsive core–shell microneedle patch is prepared from biocompatible materials. The microneedle patch features high insulin loading capacity (36 IU cm−2), high puncture efficiency (92%), and conformability to skin. Moreover, the microneedle patch reveals good hypoglycemic effect in diabetic rats, achieves a hypoglycemic rate similar to that of direct injection, and works effectively to prevent excess insulin transmission. Developing glucose‐responsive microneedle patches with features of simple preparation, high insulin loading, and safety has a great contribution to the treatment of diabetic patients. In this work, a novel glucose‐responsive core–shell microneedle patch is prepared from biocompatible materials by a facile process. The exterior glucose‐responsive shell is composed of polyvinyl alcohol (PVA) and ɛ‐polylysine modified with phenylboronic acid , and the inner flexible core is prepared by pure PVA loaded with insulin. The shell shows superior glucose‐induced swelling and controlled insulin release ability due to the decomposition of borate ester bonds under the synergistic effect of glucose and water. The microneedle patch also features flexibility and high insulin loading capacity (36 IU cm−2). It shows high puncture efficiency (92%) and maintains a relatively complete morphology after being removed, enabling to reduce material residues in the skin. Moreover, the microneedle patch reveals a good hypoglycemic effect in diabetic rats, achieves a hypoglycemic rate similar to that of direct injection after swelling, and works effectively to prevent excess insulin transmission. The high glucose‐responsive ability, simple preparation, high loading capacity, rapid hypoglycemic speed, high conformability to skin, and low component dissolution make this microneedle patch a suitable candidate for diabetic treatment.
