Abstract:The direct anterior approach (DAA) is a minimally invasive operative approach that has recently gained populari-ty in patients undergoing total hip arthroplasty (THA). Many studies have reported that the DAA in hip hemiarthroplasty provided benefits for patients in terms of muscle protection, blood loss, post-operative pain and early mobilization. Clinical efficacy of direct anterior approach vs. other surgical approaches for total hip arthroplasty: A systematic review and meta-analysis based on RCTs

Abstract:Background: Direct anterior approach (DAA) is an accurate technique for total hip arthroplasty (THA) through the muscle gap. Physicians who apply DAA believe that it accelerates patient recovery and results in lower rates of postoperative dislocation. However, the traditional surgical approach adherents believe that it is shorter and has fewer complications than DAA. Methods: We use the method of META analysis to organize and analyze the data of the randomized controlled studies (RCT) obtained after our screening. To compare the clinical efficacy of DAA approach and other surgical approaches for THA. Results: After rigorous screening, 15 RCT studies were included in our study, and data were extracted. The study included 1,450 patients from 15 RCTs, with a mean age of 63 years and a distribution of 52-67 years. Six weeks after the operation, the Harris hip score of the DAA approach improved by an average of 4.06 points (95% confidence interval (CI) 2.54 -5.59, P < 0.01, I-2 = 45%, which can significantly improve the clinical efficacy of patients. However, the 0.61 points [95% confidence interval (CI) -1.13 -2.34, P > 0.01, I-2 = 0%] at 3 months and 1.49 points [95% confidence interval (CI) -1.65 -2.25, P > 0.01, I-2 = 0%] at 12 months postoperatively. In terms of dislocation rate, results show that the use of DAAs does not reduce Dislocation Rate with significant statistical heterogeneity among study groups (95% CI 0.18-2.94 P > 0.001, I-2 = 0%). Conclusion: The hip function of DAA was superior to posterolateral approach (PLA) and latera approach (LA) in the early days after hip replacement, especially within six weeks. However, at six months or more after surgery, the difference was not significant. The DAA did not show a lower rate of dislocation than other surgical approaches.

Abstract:Introduction: The direct anterior approach (DAA) has its origins in the first and oldest approach for hip replacement in the literature, but at the same time it would not be fanciful to suggest its increasing popularity as the latest approach for hip replacement procedures, especially among younger surgeons. However, in a geographical context, the DAA is not considered the major approach in most countries. Moreover, the term DAA encompasses numerous variations in terms of technique. Areas covered: In this narrative review, we describe our recent experience of advances in the DAA in terms of improved techniques and devices, along with some of its disadvantages. Also, we express our perspective on its future application. Expert opinions: The DAA is established as one of exemplary approaches to THA. The use of fluoroscopy, the traction table, and appropriate soft tissue management has become essential in the DAA for a safe and trouble-free procedure with adequate patient comfort. With the combination of recent technologies such as robotics, three-dimensional preoperative planning, and artificial intelligence (AI)-based surgeon assist systems, we can look forward to the DAA being performed more efficiently in the future.
Efficacy of direct anterior approach versus posterolateral approach in total hip arthroplasty: a systematic review and meta-analysis

Abstract:Background: To compare the efficacy of the direct anterior approach (DAA) versus the posterolateral approach (PLA) in total hip arthroplasty (THA) in terms of operation time, incision length, intraoperative blood loss, postoperative pain, and incision infection rate. Methods: We systematically searched databases including China National Knowledge Infrastructure (CNKI), Wanfang Data, VIP Chinese sci-tech journals, Chinese Biomedical Literature Database (CBM), PubMed, and Cochrane Library up to December 2023. We included randomized controlled trials (RCTs) that compared DAA with PLA in THA, with a minimum sample size of 80 and a follow-up of at least 6 months. Studies were screened by two independent researchers, following PRISMA guidelines. Data were extracted using a pre-established feature table, capturing study design, sample size, patient demographics, and outcomes of interest. Meta-analysis was performed using RevMan 5.4.1 software. Heterogeneity was assessed using the Q-value statistical test and I² test. The fixed-effects model was used when heterogeneity was low; otherwise, the random-effects model was applied. Results: A total of 19 RCTs met the inclusion criteria. The Meta-analysis revealed that DAA was associated with a longer operation time [MD = 5.89, 95%CI(2.26 to 9.51), P = 0.001] but resulted in a smaller incision length [MD = -2.99, 95%CI(-3.76 to -2.22), P < 0.00001], less intraoperative blood loss [MD=-108.36, 95%CI(-131.10 to -85.62), P < 0.00001], lower incidence of postoperative incision infection [OR = 0.39, 95%CI(0.19 to 0.83), P = 0.01], and reduced hip Visual Analog Scale (VAS) scores on the 1st and 3rd days postoperatively [MD=-0.85, 95%CI(-0.96 to -0.74), P < 0.00001; MD=-0.60, 95%CI(-1.13 to -0.07), P = 0.03]. No significant difference was observed in VAS scores on the 7th postoperative day. Conclusion: The DAA for THA offers advantages over PLA, including reduced incision size, blood loss, and postoperative pain, albeit with a longer operation time. These findings should guide clinical decision-making, considering the benefits and potential increased complexity of the DAA.

Abstract:To investigate the outcomes of the direct anterior approach (DAA) in total hip arthroplasty (THA) and its impact on improving hip joint function. This retrospective analysis included 94 patients who underwent THA between December 2017 and December 2020 at Dongguan Hospital, Guangzhou University of Chinese Medicine. The study group comprised 50 patients who received the DAA, while the control group comprised 44 patients who received the postero-lateral approach (PA). The follow-up period was 12 months. A comparison was made between the 2 groups based on perioperative indicators (operation time, intraoperative blood loss, hospitalization time, bed rest time, incision length, pain score), duration of walker use, incidence of postoperative complications, hip joint function (Harris score), quality of life (SF-36), and activities of daily living (ADL). The treatment effectiveness rate was higher in the study group (P < .05). The study group had a longer operation time, lower intraoperative blood loss, shorter hospitalization and bed rest time, smaller incision length, and lower visual analog scale (VAS) score after treatment, with statistically significant differences (P < .05). The study group also had a shorter duration of walker use after surgery (P < .05). The Harris score after treatment was higher in the study group compared to the control group (P < .05). Additionally, the study group had higher SF-36 scores and ADL scores after treatment (P .05). The DAA in THA resulted in reduced pain and intraoperative blood loss, contributing to the promotion of postoperative recovery in patients with good short-term outcomes. This procedure warrants further promotion.

Abstract:BackgroundTotal hip arthroplasty (THA) using the direct anterior approach (DAA) is becoming increasingly popular due to its potential benefits over the posterolateral approach (PLA). However, few studies have compared the efficacies of these two surgical approaches in hip fusion treatment. This study compared early clinical direct anterior and posterolateral THA outcomes in hip fusion treatment.MethodsHere, 127 hips (65 DAA, 62 PLA) were retrospectively evaluated. Early postoperative functional outcomes of DAA and PLA groups were assessed using Harris score and Oxford Hip Score (OHS) and standard anteroposterior hip radiographs. Surgical characteristics, perioperative results, and complications within 6 months postoperatively were recorded.ResultsThough baseline values were similar, Harris and OHS scores were better in the DAA group than in the PLA group at 1 and 3 months postoperatively. The average cup anteversion angle was significantly greater in the DAA group than in the PLA group (12.7° vs. 11.1°). More hips undergoing DAA were successfully orientated in both inclination and anteversion angles (46 vs. 32). Early postoperative hip function predictors were preoperative fused hip position, surgical approach, and range of motion. DAA was associated with reduced postoperative blood loss and shorter hospital stays. Furthermore, 14 vs. 8 complications occurred in the DAA vs. PLA group. Lateral femoral cutaneous nerve injuries were observed in eight hips (12.3%) of the DAA group.ConclusionFor fused or ankylosed hips, THA using DAA in the lateral decubitus position may result in excellent prosthesis positioning and faster postoperative recovery throughout early follow-up vs. PLA.

Abstract:Background:The efficacy and safety of direct anterior approach (DAA) for total hip arthroplasty (THA) is still unclear. Objective:To conduct a systematic review and meta-analysis of published studies comparing DAA and posterior ap-proach (PA) for THA. Methods:Controlled trials about comparison between DAA and PA for THA were searched in major databases. Suitable studies were selected according to inclusion and exclusion criteria. The data were extracted and the quali-ty of included studies was evaluated by two reviewers. Meta analysis was conducted using RevMan5.2 software, and the studies that could not be combined were analyzed descriptively. Results:Eleven literatures were included in this study, and a total of 1612 patients were involved. There were 817 patients in DAA group and 795 patients in PA group. All surgeons who performed DAA THA had already completed their learning curves. The patients in DAA group were able to walk earlier without the need for an assistive device than PA group (WMD=-11.05, 95%CI:-17.79,-4.31, P=0.001), and DAA was su-perior to PA in early postoperative function rehabilitation and activities. There was no statistical difference in the incidences of intraoperative and postoperative complications (OR=1.48, 95%CI:0.69, 3.20, P=0.32), intraoperative fractures (OR=1.31, 95%CI: 0.50, 3.45, P=0.58), postoperative dislocations (OR=0.34, 95%CI: 0.09, 1.28, P=0.11), heterotopic ossification (OR=1.01, 95%CI:0.26, 3.94, P=0.99) or groin pain (OR=2.73, 95%CI:0.62, 12.06, P=0.19) between two groups. Neither were the operative time (WMD=10.25, 95%CI:-6.33, 26.83, P=0.23), length of hospital stay (WMD=-0.34, 95%CI:-0.76, 0.07, P=0.10) or the percentage of acetabular cups placed within the Lewinnek safe zone (OR=2.08, 95%CI:0.65, 6.72, P=0.22). DAA could avoid postoperative dislocations as compared with PA and lead to a lower incidence of abnormality in ace-tabular cup position when using intraoperative fluoroscopy during DAA THA. Conclusions DAA is a superior approach for THA when surgeons make themselves master on the procedure of DAA THA.

Direct Anterior Approach in Lateral Position Achieves Superior Cup Orientation in Total Hip Arthroplasty: a Radiological Comparative Study of Two Consecutive Series

Abstract:Introduction Although the advantages of total hip arthroplasty (THA) using the direct anterior approach (DAA) have been well documented, DAA in the supine position is limited by the difficulty in exposing the acetabulum and femur, which may result in implant malpositioning and intra-operative complications. Recent studies have reported that DAA-THA in the lateral position can achieve satisfactory clinical and radiographic outcomes. The aim of this study was to investigate the difference in implant alignment between two patient cohorts that underwent primary THA by DAA and the traditional posterolateral approach (PLA) in the lateral position. Methods A total of 208 THAs (110 using DAA and 98 using PLA) were retrospectively analyzed. Results The cup anteversion angle was closer to the target anteversion angle, and the accuracy of cup orientation was superior in the DAA group. A total of 82 (74.5%) DAA-THA and 56 (57.1%) PLA-THA cases showed successful inclination and anteversion angles (p = 0.008) of the implant. In addition, 90 (81.8%) DAA-THA and 67 (68.4%) PLA-THA cases had successful anteversion angle (p = 0.024). There was no significant difference between the two groups in terms of successful inclination angle (98 vs. 81, p = 0.181), as well as in the stem alignment in both coronal and sagittal planes. Conclusion THA using DAA in the lateral position can achieve superior cup orientation compared to PLA while maintaining the stem alignment.

Abstract:Objective To compare the early rehabilitation effects of total hip arthroplasty (THA) with the direct anterior approach (DAA)versusthe posterior approach (PA). Methods We searched PubMed, Embase, Web of Science, the Cochrane Library, and Google databases from inception to June 2019 to select studies that compared the DAA and PA for THA. Only randomized controlled trials (RCT) were included. Two researchers independently screened studies for inclusion, extracted data, and assessed the methodological quality. A meta-analysis was conducted using RevMan 5.3 software provided by Cochrane Assisted Network. Results A total of 932 patients underwent THA. There were 467 cases in group DAA and 465 cases in group PA. There was a significant difference in the incidence of lateral femoral cutaneous nerve injury between DAA and PA groups (RR = 38.97, 95% CI: 7.89-192.57,P < 0.05). DAA was associated with less pain compared with PA [WMD = -0.65, 95% CI (-0.91-0.38),P < 0.05]. There was no significant difference in operation time, hospitalization stay, and intraoperative bleeding volume. Moreover, in supplementary data, the number of acetabular prostheses in Lewinnek's safety zones in DAA was more than that in the PA group (RR = 1.20, 95% CI [1.04-1.39],P < 0.05), and the time of discontinuation of walking aids in the DAA group was earlier than that in the PA group (WMD = -11.05, 95% CI [-17.79-4.31],P < 0.05). Conclusion The DAA total hip arthroplasty has comparable results with PA, with earlier postoperative functional recovery, less postoperative pain scores, and higher incidence of lateral femoral cutaneous nerve injury. The results need to be validated by large-sample, high-quality RCT studies, and long-term follow-up of complications.

Abstract:The direct anterior approach (DAA) to total hip arthroplasty has been promoted as a minimally invasive alternative to the lateral approach, which we sought to verify by systematically reviewing and meta-analyzing the literature comparing clinical, radiographic, and surgical outcomes.Two reviewers independently searched PubMed, OVID, and Web of Science databases for randomized controlled trials (RCTs) and cohort studies comparing the DAA and lateral approach for total hip arthroplasty. Quality of RCTs was assessed using the Jadad scoring system, quality of cohort studies, using the Minors system.Data were extracted and meta-analyzed or qualitatively synthesized for primary outcomes (function, complications, and hospitalization time) and several secondary outcomes. Data were extracted from 12 trials involving 4901 arthroplasty procedures. Meta-analysis showed that DAA was associated with significantly shorter hospitalization than the lateral approach, as well as greater functional rehabilitation and lower perceived pain during the early postoperative period. On the other hand, DAA was associated with longer surgery time. The 2 approaches were associated with similar rates of perioperative surgical complications and transfusions, as well as similar radiographic analysis results.Although DAA may provide shorter hospitalization and faster recovery during the early postoperative period, the available evidence is still insufficient to conclude whether the DAA or lateral approach is superior for total hip arthroplasty. More high-quality studies and subsequent meta-analyses are needed.
Improved surgical exposure and early clinical outcomes using a femoral-release-first technique in direct anterior approach during total hip arthroplasty

Abstract:Background: Total hip arthroplasty (THA) performed using the direct anterior approach (DAA) has demonstrated favourable early-, mid-, and long-term outcomes. However, the traditional femoral release technique remains technically demanding and is associated with challenges and a heightened risk of complications. This study aimed to compare the clinical outcomes of patients who underwent THA with DAA performed using either the femoral-release-first (FRF) or the traditional approach (TA) strategy. Methods: A retrospective analysis of demographics, clinical and radiological outcomes, and occurrence of complications was performed using data from 106 patients between 2018 and 2019. The patients were categorised into two groups: FRF (44 hips) and TA (69 hips). Results: The FRF group showed a reduced operative time, haemoglobin (Hb) drop, postoperative hospital stay, and more optimal acetabular cup anteversion angles. Furthermore, during the first 2 months postoperatively, the FRF group demonstrated superior visual analogue scale, Harris Hip, and Oxford Hip scores. In the TA group, two hips experienced greater trochanter fractures, and one experienced delayed incision healing. Conclusions: Compared with the TA, employing the FRF strategy during THA with DAA resulted in improved outcomes within the first 2 months postoperatively and comparable functional recovery beyond this period. The FRF method exhibited advantages such as favourable acetabular exposure and alignment and a reduced risk of complications. Therefore, the FRF strategy may be a favourable option.

Abstract:Background: Total hip arthroplasty (THA) has become one of the most successful operations in joint surgery in recent years. With the progress of surgical technology, people want to find a more suitable surgical method for enhanced re-covery after surgery (ERAS). Direct anterior approach (DAA) for THA has become a popular choice. Objective: To explore the short-term efficacy of DAA and posterolateral approach in THA. Methods: Two hundred patients scheduled for THA from June 2015 to December 2016 were selected as objects of the study. During THA, DAA was used in 100 patients (obser-vation group), and posterolateral approach was applied in the other 100 patients (control group). All the patients were fol-lowed up for one month. The operation time, intraoperative blood loss, length of incision, time and length of hospital stay, anteversion angle of acetabulum, abduction angle of acetabulum, acetabulum prosthesis on average diameter, Harris hip score, complication rates, and the quality of life were compared between the two groups. Results: There was no significant difference in incision length, Harris score, anteversion angle of acetabulum, abduction angle of acetabulum, acetabulum prosthesis on average diameter or incidence of complications between groups (P>0.05). The operation time,amount of bleeding,ambulation time and hospitalization time in observation group were significantly shorter (less) than those in con-trol group (P<0.05). The SF-36 score in observation group was significantly higher than that in control group (P<0.05). Con-clusions: Compared with posterolateral approach, direct anterior approach can significantly shorten operation time, bed ac-tivity time and hospitalization time, and improve the quality of life of patients undergoing THA.
[Surgical technique and early clinical outcomes of direct anterior approach to total hip arthroplasty]

Abstract:Objective: To describe the surgical technique of direct anterior approach to total hip arthroplasty and to report the early clinical outcomes. Methods: A series of 100 consecutive, unselected patients who had 116 primary total hip arthroplasty surgeries (16 bilateral) done through direct anterior approach from March 11 2015 to June 21 2016 was reviewed. There were 50 male patients and 50 female patients. The average patient age was 51 years, and the average body mass index was 24.69 kg/m2. The preoperative diagnosis included avascular necrosis of femoral head, hip osteoarthritis, osteoarthritis secondary to acetabular dysplasia, sequelae of hip old infection, ankylosing spondylitis, rheumatoid arthritis and avascular necrosis of femoral head after cannulated screws fixation of femoral neck fracture. There were 7 hips which had surgical history prior to the index hip arthroplasty, including 3 cases with bone graft treatment for avascular necrosis of femoral head through Smith-Peterson approach, 2 cases with acetabular shelf procedures for acetabular dysplasia through Smith-Peterson approach, and 2 cases with cannulated screws fixation for femoral neck fracture (internal fixation residual). All were uncemented hips. The stems used in this study included 67 Triloc stems (DePuy company, USA), 45 Corail stems (DePuy company, USA), 2 Accolade stems (Stryker company, USA), 1 Synergy stem (Smith-Nephew company, USA) and 1 Polarstem (Smith-Nephew company, USA). Results: The average follow up period was 8.5 months, the average incision scar length was 10 cm, and the average postoperative Harris score was 93.62. There was 95% postoperative leg length discrepancy within 3 mm. The average cup inclination angle was 38.7° with 94.8% in the range of 30° to 50°. The average cup anteversion angle was 14.3° with 94.2% within the target range of 5° to 25°.The were 15 (12.9%) operative complications, including two femoral perforations (changing stem from Triloc to Corail), three calcar fractures (treated with cerclage wires), four greater trochanter fractures (2 were treated wire tension band, and 2 nondisplaced fractures untreated), one deep infection (debridement and retaining of the prothesis), one superficial infection (debridement), one hematoma and three wound healing complications (debridement). All the complications were successfully treated without any sequelae at the end of the latest follow-up. There was no postoperative dislocation. There was no major nerve and vascular injuries. There were 35 cases (30.2%) reporting symptoms of lateral femoral cutaneous nerve palsy. Conclusion: Direct anterior approach to total hip arthroplasty allows accurate and reproducible cup orientation positioning and leg length restoration and decreases the risk of postoperative dislocation, which is helpful for early rapid postoperative recovery.

Abstract:In general, the literature makes numerous positive claims regarding the direct anterior approach with a fracture table for total hip arthroplasty (THA), including quicker recovery and return to unassisted ambulation, along with reduced soft tissue damage, surgery time, pain, and risk of dislocation with early elimination of hip precautions. The benefits of the direct anterior approach are mostly due from muscle preservation rather than muscle splitting, which occurs with the more traditional approaches. With the use of the muscle-preserving direct anterior approach for THA, there is less muscle damage and earlier return to function, and postoperative precautions are not needed. The most significant improvements in THA have been allowing patients to be immediately weight bearing as tolerated after THA, incorporating a multimodal pain management protocol, and now using the direct anterior approach. There is a learning curve, and I strongly recommend that people attend cadaver-based learning centers as well as surgeon visitations. We must always remember the oath we took to "do no harm," especially when embarking on a new procedure such as the direct anterior approach in THA or any other new procedure or technology. My position in the debate is not whether we should embrace this technique or other new techniques, but rather how they should be introduced.

Abstract:Contrary to recent claims that the direct anterior approach is an innovative and novel surgical technique, this approach has been used in hip surgery or arthroplasty for more than 140 years. The direct anterior approach was first described in 1881 by Carl Hueter in the textbook, Grundriss der Chirurgie [ 44 ] and subsequently popularized by Smith-Peterson in 1917 and then by the Judet brothers and O'Brien in the 1950s [ 22, 39, 51 ]. The approach fell out of favor over the next few decades due to the increased risk of femoral complications and the growing popularity of Charnley's transtrochanteric osteotomy [ 10 ]. The direct anterior approach had a resurgence after Light and Keggi [ 27 ] reported their experience with this approach in 1980 and Judet and Judet [ 21 ] published their outcomes with this approach using an orthopaedic operating table in 1985. The basic premise of the direct anterior approach is that it utilizes the internervous and intermuscular intervals between the tensor fascia lata and the sartorius muscles [ 30 ]. Conceptually, using this interval avoids detachment of the muscles from bone and thereby limits the trauma of surgery. It was concluded in an earlier review that "the orthopaedic community will embrace this technique" and that it should "be introduced into routine practice" [ 36 ]. Currently, a growing number of surgeons are adopting the direct anterior approach in THA. The 2018 survey by the American Association of Hip and Knee Surgeons showed that 56% of respondents used the direct anterior approach in THA, and those who performed the direct anterior approach reported increased patient market share [ 40 ].

Abstract:Background: The direct anterior approach (DAA) for total hip arthroplasty (THA) is typically performed in the supine position using a specially designed operating room table, which makes this approach more accessible to orthopedic surgeons. We attempted to perform this procedure in the lateral decubitus position on an ordinary operation table to avoid dependence on a special operating room table. There is an obvious absence of literature regarding this subject. Methods: A total of 248 patients (295 hips) were recruited for primary THAs from July 1, 2014 to December 31, 2014. In total, 126 hips (42.7%) underwent THAs using the DAA in the lateral decubitus position. The technical feasibility and early results were evaluated. Results: The orientation of the acetabular component was 16.5 degrees +/- 4.9 degrees anteversion and 43.3 degrees +/- 3.5 degrees abduction. Intraoperative proximal femoral fracture occurred in one hip. The superficial wound complications occurred in 2 hips and the hematoma in one hip while in hospital. The lateral femoral cutaneous nerve injury was noted in 43 hips. The early dislocation occurred in 2 hips. Heterotopic ossification was Brooker class I in 5 hips and class II in 1 hip. No aseptic loosening, postoperative periprosthetic fracture, and deep infection occurred in our series. Conclusion: The DAA for THA in the lateral decubitus position may be a valuable alternative if the DAA in the supine position is difficult to implement owing to absence of a special operating room table. This technique also seems to provide satisfactory clinical and radiographic outcomes with an acceptable complication in our early follow-up. (C) 2016 Elsevier Inc. All rights reserved.

Abstract:[Objective] The concept of fast track surgery,also termed as enhanced recovery after surgery (ERAS) was first introduced by Professor Kehlet in 1997,aiming to reduce postoperative morbidity,hospital stay and costs.ERAS has already demonstrated benefits to the surgical patient in colorectal,gastrointestinal and orthopaedic surgeries.Direct anterior approach (DAA) for total hip arthroplasty,as a minimally invasive,muscle sparing,internervous approach,is a ERAS concept based technique and has gained further popularity in the last few years.The possible advantages include early functional recovery,shorter hospital stay and less blood lost.This paper reviews the concept of ERAS and the procedure and superiority of DAA.

Abstract:Objective To retrospectively analyze and compare the short-term clinical outcomes of direct anterior approach and minimal invasive posterolateral approach for total hip arthroplasty. Methods From October 2013 to November 2014,the results of 42 patients receiving primary regular total hip arthroplasty( THA) in department of orthopaedics of the Union Hospital,were enrolled. Complicated cases,revisions,infections and tumor patients were excluded. There were 13 males and eight females in the direct anterior approach( DAA) group,aged from 36 to 68 years,( 55. 3 ± 11. 3) years on average;while the minimal invasive posterolateral approach( MPA) group consisted of 11 males and 10 females,aged from 41 to 70 years,( 57. 4 ± 7. 5) years on average. Operation time,length of incision,intraoperative blood loss and hospital stay were recorded routinely. Clinical outcomes were evaluated by Harris hip score( HHS),arthritis index of Western Ontario and Mc Master Universities( WOMAC),visual analogue scale( VAS) pre-operatively and one week,one month and three months postoperatively. Range of motion,usage of analgesics and assistive mobility aids were also noted for comparison. Chi-square test and t test were applied for data analysis. Results The DAA group had significantly increased operation time( t = 8. 50,P < 0. 01) and reduced hospital length of stays( t = 9. 99,P < 0. 01) compared with the MPA group. The quantified joint grading systems including HHS( t = 2. 19,P < 0. 05),WOMAC( t = 2. 45,P <0. 05) and VAS( t = 6. 93,P < 0. 01) were more favorable in the DAA group than the MPA group at oneweek postoperatively but the differences faded thereafter. In the DAA group,there was significantly less usage of analgesics( 2= 13. 75,P < 0. 01) and assistive devices( 2= 10. 10,P < 0. 01) four weeks after the operation than in the MPA group. Range of motion of the hip joint was significantly increased in the DAA group than in the MPA group during the whole follow-up period( week 1 t = 23. 63,week 4 t =26. 71,3rdmonth t = 24. 14,all P < 0. 01). Conclusion DAA for primary THA could achieve more favorable early clinical outcomes and would allow for more superior recovery of the patients than MPA,which may diminish the consumption of social health medical resources.

Abstract:Two familiar surgical methods, posterior approach (PA) and direct anterior approach (DAA), have been extensively utilized in the treatment of total hip arthroplasty (THA) with similar long-term rates of success. The sufficient sample size and a good clinical trial are urgently needed. Considering the above factors, we implemented a retrospective research to compare the prognosis of patients with primary THA receiving the techniques of PA or DAA.This is an observational retrospective research that prospectively collected information via several surgeons at a center utilizing the 2 above treatment methods for unilateral primary total hip arthroplasty. A review of primary THA performed with DAA or PA between February 2017 and February 2019 was conducted in our hospital. The inclusion criteria contained the degenerative changes in end-stage of hip owing to the rheumatoid arthritis, inflammatory arthritis, and osteoarthritis, as well as the Crowe I and II dysplasia that did not require the enhancement. The primary endpoint was the Harris hip score. The measures of secondary outcome contained the operation time, length of incision, hospital stay, the complications after operation, as well as patient satisfaction. The Statistical Package for Social Sciences version 20.0 was utilizing for the statistical analysis (IBM Corporation, Armonk, NY).We assumed that the 2 treatment methods possess similar results.This study protocol was registered in Research Registry (researchregistry6008).
