Abstract:The introduction of nickel-titanium (NiTi) as material for endodontic instruments about 15 years ago opened many new perspectives. Many dentists and scientists see a benefit in using NiFi files. Initial problems such as frequent fractures and the uncertainty of the best way to use them have been solved. Other challenges such as enhancing the cutting ability or optimizing the speed, torque, and fatigue are currently being addressed. Some clinicians are skeptical because they see this approach as too mechanical. Nevertheless, the combination of anatomic, biologic, and pathophysiologic knowledge with the use of NiTi instruments is a large step forward in optimizing the quality of root canal treatment worldwide.

Abstract:Nickel-titanium (NiTi) instruments have become the backbone of endodontics due to their exceptional properties, superelasticity, and shape memory. However, challenges such as unexpected breakage, poor cutting efficiency, and corrosion have prompted researchers to explore innovative surface modifications to enhance their performance. This comprehensive review discusses the latest advancements in NiTi metallurgy and their impact on rotary NiTi file systems. Various surface treatment techniques, including ion implantation, cryogenic treatment (CT), thermal nitridation, electropolishing, and physical or chemical vapor deposition, have been investigated to minimize defects, boost surface hardness, and improve cyclic fatigue resistance. Ion implantation has shown promise by increasing wear resistance and cutting efficiency through nitrogen ion incorporation. Thermal nitridation has successfully formed titanium nitride (TiN) coatings, resulting in improved corrosion resistance and cutting efficiency. CT has demonstrated increased cutting efficiency and overall strength by creating a martensite transformation and finer carbide particles. Electropolishing has yielded mixed results, providing smoother surfaces but varying impacts on fatigue resistance. Physical or chemical vapor deposition has proven effective in forming TiN coatings, enhancing hardness and wear resistance. Furthermore, the concept of surface functionalization with silver ions for antibacterial properties has been explored. These advancements present an exciting future for endodontic procedures, offering the potential for enhanced NiTi instruments with improved performance, durability, and patient outcomes.

Abstract:Various engine-driven NiTi endodontic files have been indispensable and efficient tools in cleaning and shaping of root canals for practitioners. In this review, we introduce the relative terms and conceptions of NiTi file, including crystal phase composition, the design of the cutting part, types of separation. This review also analysis the main improvement and evolution of different generations of engine-driven nickel-titanium instruments in the past 20 years in the geometric design, manufacturing surface treatment such as electropolishing, thermal treatment, metallurgy. And the variety of motion modes of NiTi files to improve resistance to torsional failure were also discussed. Continuous advancements by the designers, provide better balance between shaping efficiency and resistance to of NiTi systems. In clinical practice an appropriate system should be selected based on the anatomy of the root canal, instrument characteristics, and operators' experience.

Abstract:This narrative review will focus on the evolution, present and future of engine-driven root canal preparation. Root canal preparation changed drastically when Walia in 1988 introduced the use of nickel-titanium (NiTi) alloys in Endodontics. In 2013, five generations of NiTi endodontic instruments had been established based on their metallurgical, mechanical properties and design features. Since then, manufacturers have been introducing further major changes in instrument design and characteristics that have not been translated in new recognized generations of instruments. In general, those changes have demonstrated enhanced instrument properties, but it is not clear yet if all those improvements are directly translated to an improvement in clinical success. This narrative review attempts to address the present status of engine-driven instruments in terms of both evidence from laboratory-based studies and clinical data, to identify potential further generations of instruments, and last to anticipate future directions for research and development.

Abstract:Introduction The performance and mechanical properties of nickel-titanium (NiTi) instruments are influenced by factors such as cross-section, flute design, raw material, and manufacturing processes. Many improvements have been proposed by manufacturers during the past decade to provide clinicians with safer and more efficient instruments. Methods The mechanical performance of NiTi alloys is sensitive to their microstructure and associated thermomechanical treatment history. Heat treatment or thermal processing is one of the most fundamental approaches toward adjusting the transition temperature in NiTi alloy, which affects the fatigue resistance of NiTi endodontic files. The newly developed NiTi instruments made from controlled memory wire, M-Wire (Dentsply Tulsa Dental Specialties, Tulsa, OK), or R-phase wire represent the next generation of NiTi alloys with improved flexibility and fatigue resistance. The advantages of NiTi files for canal cleaning and shaping are decreased canal transportation and ledging, a reduced risk of file fracture, and faster and more efficient instrumentation. The clinician must understand the nature of different NiTi raw materials and their impact on instrument performance because many new instruments are introduced on a regular basis. Results This review summarizes the metallurgical properties of next-generation NiTi instruments, the impact of thermomechanical treatment on instrument flexibility, and the resistance to cyclic fatigue and torsion. Conclusions The aim of this review was to provide clinicians with the knowledge necessary for evidence-based practices, maximizing the benefits from the selection and application of NiTi rotary instruments for root canal treatment.

Abstract:This article on endodontic instrumentation includes endodontic instruments and techniques of root-canal preparation of teeth in veterinary patients. Familiarity with instruments covered in this article and with advances in veterinary endodontics has become necessary for the veterinary dental practitioner who wishes to practice dentistry using current techniques. Veterinary endodontic techniques that help to retain the teeth longer are increasing in demand as an option to extracting damaged teeth. Endodontic treatment involves removal of the irreversibly damaged pulp, followed by cleaning and shaping of the root canal space using endodontic instruments and subsequent filling, or obturation, with a semisolid material and a sealer.

Abstract:The mechanical behavior of a rotary nickel-titanium endodontic instrument was evaluated through finite element models. Essential geometric features of blank taper and variable pitch were modeled and their impact on the mechanical performance of instruments was investigated. Simulation results showed that better bending flexibility and higher torsional stiffness can both be obtained on an instrument with a large helix configuration. The results also illustrated that the finite element method can be used to compute the critical load or deformation for the instrument which prompts the sustainable use of the instrument in clinical practices.
Differential scanning calorimetric studies of nickel titanium rotary endodontic instruments

Abstract:Differential scanning calorimetric (DSC) analyses were performed between -130 degrees and 100 degrees C on specimens prepared from nickel-titanium (NiTi) rotary endodontic instruments: ProFile (n = 5), Lightspeed (n = 4), and Quantec (n = 3). The ProFile and Lightspeed instruments were in the as-received condition, whereas the Quantec instruments were randomly selected from a dental clinic and had unknown history. The DSC plots showed that the ProFile and Lightspeed instruments analyzed had the superelastic NiTi property, with an austenite-finish (Af) temperature of approximately 25 degrees C. Differences in DSC plots for the ProFile instruments and the starting wire blanks (n = 2) were attributed to the manufacturing process. The phase transformation behavior when the specimens were heated and cooled between -130 degrees and 100 degrees C, the temperature ranges for the phase transformations, and the resulting enthalpy changes were similar to those previously reported for nickel-titanium orthodontic wires having superelastic characteristics or shape memory behavior in the oral environment. The experiments demonstrated that DSC is a powerful tool for materials characterization of these rotary instruments, providing direct information not readily available from other analytical techniques about the NiTi phases present, which are fundamentally responsible for their clinical behavior.

Abstract:Knowledge and thorough understanding of the characteristics of endodontic nickel–titanium (NiTi) files is paramount for dentists performing root canal treatments to patients. Understanding the behavior of the NiTi files guides the clinicians in choosing the correct instruments for different clinical and anatomical situations. This review focuses on the metallurgical properties of endodontic NiTi files, with a special emphasis on recent developments and improvements in metallurgy and the effects of heat treatment and surface treatment. In this study, the impact that such developments have on the properties of endodontic NiTi files is discussed.

Abstract:Superelastic and thermally activated characteristics are two distinct features of TiNi alloys. Besides its good biocompatibility, wear and corrosion resistances, TiNi alloy products played an attractive role in the medical field in the past thirty years. Yet the increasing demand for highly advanced and precise therapy pushed the material scientists to improve their processing technologies from time to time. In this paper, we demonstrated our orthodontics products and endodontic instruments with special treatments, including bending and twisting resistance treatment, surface coated treatment, flawless edges and mirror like finishing. As the resulting products, the dental archwires have outstanding properties of springiness range and also good strength, become more easy to settle in the slots, exhibit remarkable alignment and leveling effect, and make patient comfortable; whereas the root canal files and reamers show high flexibility to adapt the complicated root canal structure, own the sharp edges with high cutting efficiency, and have the long usage life.
Nickel-Titanium Rotary Instruments: An In Vitro Comparison (Torsional Resistance of Two Heat-treated Reciprocating Files)

Abstract:Aim and objective: The present study aims to evaluate the difference in torsional resistance of two reciprocating nickel-titanium (Ni-Ti) rotary files: WaveOne Gold and EdgeOne Fire. Materials and methods: A total of 40 nickel-titanium rotary instruments (n = 40): 20 WaveOne Gold Small (WOGS) and 20 EdgeOne Fire Small (EOFS) were divided into two groups. Each instrument was tested using a torsional resistance device already validated in previous studies to evaluate and compare torsional resistance. The static torsional test was implemented by blocking each instrument at 3 mm from the tip and rotating it until fracture with a reciprocating motion. Torque to fracture (TtF) and fragment length (FL) were measured and statistically analyzed. Results: Statistical analysis of TtF found significant differences between the two groups (p<0.05). The EOFS showed higher TtF if compared to WOGS, with a mean value and a standard deviation of 3.05 ± 0.07 (N cm) against 2.97 ± 0.08 (N cm). Data for FL showed no significant differences (p>0.05) between the two groups. Conclusion: According to the results of this study, it is reasonable to assert that EOFS instruments showed a higher torsional resistance if compared to the WOGS. Clinical significance: As evidenced by this study, EOFS should be considered as a safer solution, in terms of torsional resistance, if compared to WOGS, reducing the risk of intracanal separation due to excessive torsional load.
A retrospective study of endodontic treatment outcome between nickel-titanium rotary and stainless steel hand filing techniques

Abstract:Aim: The purpose of this study was to compare the periapical healing of molar root canal treatment using two instrumentation techniques. Methods: A total of 225 maxillary and mandibular first and second permanent molars endodontically treated by undergraduate or postgraduate students were randomly selected from a computerized hospital database of which 110 molars had been prepared using a hybrid rotary technique with nickel-titanium instruments (group NR) and 115 with hand stainless steel files (group HF). Patients were recalled and the teeth were examined both clinically and radiographically for signs of periapical inflammation. Results: Some 19% and 39% of teeth in the NR and HF group, respectively, were judged to have some form of procedural errors. A higher rate of periapical healing was noted for NR (77%) than the HF group (60%) (p < 0.05). Factors contributing favorably to treatment outcome included the use of rotary technique, maxillary molar, experienced operator, and absence of preoperative radiolucent lesion. Conclusion: There was a higher incidence of procedural errors and a lower success rate for primary root canal treatment of teeth prepared with stainless steel files compared with the use of NiTi instruments in a continuous reaming action.
Shaping Efficiency of Rotary and Reciprocating Kinematics of Engine-driven Nickel-Titanium Instruments in Moderate and Severely curved Root Canals Using Microcomputed Tomography: A Systematic Review of Ex Vivo Studies

Abstract:Introduction: This systematic review aimed to compare reciprocating with continuous rotary instrumentation kinematics, by means of microcomputed tomography evaluations, in extracted human permanent teeth with moderate and severe canal curvatures. Methods: The research protocol was registered in the International Prospective Register of Systematic Reviews and given the reference number CRD42023404035. An electronic search was undertaken in MEDLINE (PubMed), EBSCO, Scopus, Web of Science databases until December 2021. Manual screening of issues in endodontic journals and references of relevant articles were assessed individually. The risk of bias (RoB) of the included articles was evaluated with the QUIN tool (Quality Assessment Tool for In Vitro Studies). Results: Among 1640 retrieved articles, 49 were included in the qualitative synthesis. Fifteen articles had low RoB, 33 articles had medium RoB, and only 1 study was at high RoB. Continuous rotary systems had better centering ability in both moderate and severe canal curvatures and resulted in less apical transportation in severely curved root canals. None of the kinematic systems was capable of instrumenting the entire canal surface area. Reciprocating kinematics systems tended to provide higher increase in surface area of severely curved canals and produced fewer dentinal microcracks in moderately curved canals. Conclusions: The evidence presented in this review suggests that continuous rotary system seems to be better than reciprocating system in solving the major issues encountered during root canal instrumentation of extracted teeth with moderate and severe curvatures.
Electrochemical Properties of Nickel-Titanium Rotary Endodontic Instruments

Abstract:Introduction Nickel-titanium (NiTi) rotary endodontic files have been commercially available for decades, but more recent innovations have introduced heat-treated and surface-treated files. This study investigated the corrosion properties of various NiTi files in normal saline and sodium hypochlorite (NaOCl). Methods Ten different file brands of size 40 with a 0.04 taper were subjected to electrochemical testing in 0.9% NaCl (saline) and 5.25% NaOCl at room temperature. The Open Circuit Potential (OCP) was observed for one hour followed by a cyclic polarization test from -300 to 700 mV and back to -300 mV (versus OCP). Non-parametric ANOVA and a pairwise comparison (p<0.05) were used for statistical analysis of the OCP at 1 hour and the corrosion current (Icorr) obtained via the cyclic polarization test. Results Significant differences (p<0.05) were found between files with respect to OCP and Icorr in both solutions. Nine files exhibited significantly greater (p<0.05) corrosion currents in NaOCl than in saline. Conversely, pitting corrosion was observed in the saline solution but not NaOCl. Weak and/or moderate correlations existed between OCP and Icorr measures in the two solutions. Conclusion Significant differences in electrochemical properties were observed among the ten brands of files. Overall, there was not a clear trend between conventional, heat-treated, or surface-treated files among open circuit potential or corrosion current in either solution.

Abstract:This study aims to compare the cyclic fatigue resistance of nickel–titanium (NiTi) endodontic instruments, focusing on the impact of various alloy treatments and manufacturing processes across different generations of these instruments; Twenty instrumentation systems from different generations, comprising both continuous and reciprocating motion designs, were tested. Four hundred instruments underwent cyclic fatigue testing using an INSTRON machine, with the time and number of cycles to fracture (NCF) recorded for each instrument. Statistical analyses were performed to compare the fatigue resistance between systems, generations, and motion types; Instruments treated with advanced thermal processing, such as Excalibur, Reciproc Blue, and TruNatomy, demonstrated superior resistance to fracture, whereas systems like Protaper Universal, K3XF, and 2Shape showed the lowest resistance. Reciprocating instruments generally exhibited higher cyclic fatigue resistance than continuously rotating instruments; Technological advancements in NiTi instrument design, especially the implementation of heat-treated alloys, have improved cyclic fatigue resistance, enhancing the safety and efficiency of endodontic treatments. Reciprocating systems, in particular, exhibit superior fracture resistance, suggesting their greater utility in challenging clinical conditions.

Abstract:Numerous systematic reviews (SRs) have produced conflicting findings on engine‐driven nickel–titanium reciprocating instruments (reciprocating instruments) since Yared's seminal study 15 years ago. This umbrella review analysed SRs examining the clinical and laboratory evidence regarding reciprocating instruments for root canal treatment. SRs that evaluated qualitatively and/or quantitatively the outcomes postoperative pain, oral health‐related quality of life (OHRQoL), shaping ability, debris extrusion, microbial load, endotoxins reduction, cyclic fatigue, file fracture, dentinal cracks and root canal filling removal were included. The AMSTAR 2 tool was used to evaluate SRs quality, while the ROBIS tool to assess risk of bias (RoB). Forty SRs were included. The SRs revealed predominantly 'high' RoB and 'critically low' quality. Most focused on technical outcomes, exhibiting significant methodological and statistical heterogeneity. Findings suggest comparable efficacy between reciprocating and rotary instruments. However, due to the scarcity of high‐quality evidence, future well‐designed studies and reviews considering core outcome measures are needed.

The trends of dental biomaterials research and future directions: A mapping review

Abstract:Objective: This literature research aimed to compare, contrast and quantify the innovations in the most commonly used dental biomaterials. Methodology: Original research articles based on experimental dental biomaterials published between 2007 and 2019 were retrieved and reviewed. A search of electronic databases, PubMed, Scopus, and Web of Science indexed dental/biomaterials journals, has been conducted. The inclusion criteria in this research were: synthesis of experimental dental materials, whereas commercial dental materials, review articles, and clinical trials (case reports) were excluded. Results: It was found that the amount of publications related to dental subgingival implants, computer-aided modeling ceramics, aesthetic restorative materials, adhesives cements, ceramics, bioceramics, endodontic materials, bioactive scaffolds, stem cells, and guided-tissue membranes had increased significantly from 2007. At the same time, the number of publications related to dental cements, silver amalgam, and dental alloys has decreased. For characterization of dental materials it was noted that mechanical properties were tested mostly for restorative materials. On the other hand, biological properties were most assessed for dental subgingival implants and endodontic materials, however, physical properties predominantly for bioceramics. Conclusion: It is concluded that to meet clinical demands there was more focus on restorative materials that provided better aesthetics, including resin composites, adhesive resin composites (luting cements), zirconia, and other ceramics. The boost in laboratory and animal research related to bioceramics was attributed to their regenerative potential. This current literature study will help growing researchers to consider and judge the direction to which research might be guided in order to plan prospective research projects.
