Predicting Prognostic Factors of Breast Cancer Using Shear Wave Elastography

Abstract:The purpose of the study described here was to investigate the correlation between histologic factors, including immunohistochemical factors, related to the prognosis of breast cancer and shear wave elastography (SWE) measurements. One hundred twenty-two breast cancers from 116 women were subjected to sonoelastography. Of the SWE features, mean and maximum elasticity and SWE ratio were extracted. The SWE ratio was calculated as the ratio of the stiffness of a portion of the lesion to that of a similar region of interest in fatty tissue. High ratios indicate stiffer lesions. The Mann-Whitney U-test, Kruskal-Wallis test and receiver operating characteristic (ROC) curve were used for statistical analysis. Estrogen receptor negativity, progesterone receptor negativity, p53 positivity, Ki-67 positivity, high nuclear grade, high histologic grade and large tumor (invasive) size were associated with a significantly high SWE ratio (p < 0.05). ROC curve analysis yielded SWE ratio cutoff values of 2.74-3.69 for significant immunohistochemical factors and 4.21 for the basal-like subtype by maximizing specificity while ensuring more than 80% sensitivity. Breast cancers with aggressive histologic features had high SWE ratios. Shear wave elastography may provide useful information for determining prognosis.

Abstract:Objective To explore the feasibility of shear wave elastography (SWE) parameters for assessing the biological behavior of breast cancer. Materials and Methods In this prospective study, 224 breast cancer lesions in 216 female patients were examined by B-mode ultrasound and shear wave elastography in sequence. The maximum size (S max ) of the lesion was measured by B-mode ultrasound, and then shear wave elastography was performed on this section to obtain relevant parameters, including maximum elasticity (E max ), mean elasticity (E mean ), standard deviation of elasticity (SD), and the area ratio of shear wave elastography to B-mode ultrasound (AR). The relationship between SWE parameters and pathological type, histopathological classification, histological grade, lymphovascular invasion status (LVI), axillary lymph node status (ALN), and immunohistochemistry of breast cancer lesions was performed according to postoperative pathology. Results In the univariate analysis, the pathological type and histopathological classification of breast cancer were not significantly associated with SWE parameters; with an increase in the histological grade of invasive ductal carcinoma (IDC), SD ( p = 0.016) and S max ( p = 0.000) values increased. In the ALN-positive group, S max ( p = 0.004) was significantly greater than in the ALN-negative group; S max ( p = 0.003), E max ( p = 0.034), and SD ( p = 0.045) were significantly higher in the LVI-positive group than in the LVI-negative group; SD ( p = 0.043, p = 0.047) and S max ( p = 0.000, p = 0.000) were significantly lower in the ER + and PR + groups than in the ER - and PR - groups, respectively; AR ( p = 0.032) was significantly higher in the ER + groups than in the ER - groups, and S max ( p = 0.002) of the HER2 + group showed higher values than that of the HER2 - group; S max ( p = 0.000), SD ( p = 0.006), and E max ( p = 0.004) of the Ki-67 high-expression group showed significantly higher values than those of the Ki-67 low-expression group. In the multivariate analysis, Ki-67 was an independent factor of S max ( p = 0.005), E max ( p = 0.004), and SD ( p = 0.006); ER was an independent influencing factor of S max ( p = 0.000) and AR ( p = 0.032). LVI independently influences S max ( p = 0.006). Conclusions The SWE parameters E max , SD, and AR can be used to evaluate the biological behavior of breast cancer.

Abstract:The purpose of our study is to explore the efficiency of Shear Wave Elastography (SWE) in predicting malignancy Breast Cancer (BC). 63 patients with normal mammary glands and 61 patients with invasive carcinoma were detected by Real-time Tissue Elastography (RTE). Two SWE images of breast hyperplasia with fibroadenoma and invasive breast carcinoma were also obtained. For the SWE characteristics, maximum, minimum, mean elasticity and SWE ratio were acquired. The SWE ratio was counted to reflect the stiffness of lesions, and a high ratio demonstrated a stiff lesion. Kruskal-Wallis one-way ANOVA, Receiver Operating Characteristic (ROC) curve, and Z-Test were used for statistical analysis. Our results indicated that SWE in patients with normal mammary glands showed a light-blue coloured lesion and the SWE values (E-mean) was 7.1 kPa. SWE in patients with invasive carcinoma revealed a yellow-to-red coloured mass, and the SWE values (E-mean) was 243.6 kPa. The cut-off values of the ROC curve analysis at lesion (E-max) and gland ratio (E-rat) were 55.8 kpa and 3.45. The Z value was 0.915, and the P value was 0.36 for ROC curve between E-max and E-rat. The SWE value (E-mean) in front of the fat layer was 12.6 kPa in patients with breast hyperplasia accompanying fibroadenoma. The SWE values (E-mean) in front of the fat layer was 23.9 kPa in patients with invasive carcinoma. Breast cancers with invasive characteristics had high SWE ratios. Therefore, SWE may provide potential application value for predicting prognosis.

Abstract:Shear wave elastography (SWE) shows promise as an adjunct to greyscale ultrasound examination in assessing breast masses. In breast cancer, higher lesion stiffness on SWE has been shown to be associated with features of poor prognosis. The purpose of this study was to assess whether lesion stiffness at SWE is an independent predictor of lymph node involvement. Patients with invasive breast cancer treated by primary surgery, who had undergone SWE examination were eligible. Data were retrospectively analysed from 396 consecutive patients. The mean stiffness values were obtained using the Aixplorer® ultrasound machine from SuperSonic Imagine Ltd. Measurements were taken from a region of interest positioned over the stiffest part of the abnormality. The average of the mean stiffness value obtained from each of two orthogonal image planes was used for analysis. Associations between lymph node involvement and mean lesion stiffness, invasive cancer size, histologic grade, tumour type, ER expression, HER-2 status and vascular invasion were assessed using univariate and multivariate logistic regression. At univariate analysis, invasive size, histologic grade, HER-2 status, vascular invasion, tumour type and mean stiffness were significantly associated with nodal involvement. Nodal involvement rates ranged from 7 % for tumours with mean stiffness <50 kPa to 41 % for tumours with a mean stiffness of >150 kPa. At multivariate analysis, invasive size, tumour type, vascular invasion, and mean stiffness maintained independent significance. Mean stiffness at SWE is an independent predictor of lymph node metastasis and thus can confer prognostic information additional to that provided by conventional preoperative tumour assessment and staging.

Abstract:Abstract Background Early prediction of tumor response to neoadjuvant chemotherapy (NACT) is crucial for optimal treatment and improved outcome in breast cancer patients. The purpose of this study is to investigate the role of shear wave elastography (SWE) for early assessment of response to NACT in patients with invasive breast cancer. Methods In a prospective study, 62 patients with biopsy-proven invasive breast cancer were enrolled. Three SWE studies were conducted on each patient: before, at mid-course, and after NACT but before surgery. A new parameter, mass characteristic frequency ( f mass ), along with SWE measurements and mass size was obtained from each SWE study visit. The clinical biomarkers were acquired from the pre-NACT core-needle biopsy. The efficacy of different models, generated with the leave-one-out cross-validation, in predicting response to NACT was shown by the area under the receiver operating characteristic curve and the corresponding sensitivity and specificity. Results A significant difference was found for SWE parameters measured before, at mid-course, and after NACT between the responders and non-responders. The combination of E mean2 and mass size ( s 2 ) gave an AUC of 0.75 (0.95 CI 0.62–0.88). For the ER+ tumors, the combination of E mean_ratio1 , s 1 , and Ki-67 index gave an improved AUC of 0.84 (0.95 CI 0.65–0.96). For responders, f mass was significantly higher during the third visit. Conclusions Our study findings highlight the value of SWE estimation in the mid-course of NACT for the early prediction of treatment response. For ER+ tumors, the addition of Ki-67improves the predictive power of SWE. Moreover, f mass is presented as a new marker in predicting the endpoint of NACT in responders.

Abstract:Objectives Shear wave elastography (SWE) is increasingly used in breast cancer diagnostics. However, large, prospective, multicenter data evaluating the reliability of SWE is missing. We evaluated the intra‐ and interobserver reliability of SWE in patients with breast lesions categorized as BIRADS 3 or 4. Methods We used data of 1288 women at 12 institutions in 7 countries with breast lesions categorized as BIRADS 3 to 4 who underwent conventional B‐mode ultrasound and SWE. 1243 (96.5%) women had three repetitive conventional B‐mode ultrasounds as well as SWE measurements performed by a board‐certified senior physician. 375 of 1288 (29.1%) women received an additional ultrasound examination with B‐mode and SWE by a second physician. Intraclass correlation coefficients (ICC) were calculated to examine intra‐ and interobserver reliability. Results ICC for intraobserver reliability showed an excellent correlation with ICC >0.9, while interobserver reliability was moderate with ICC of 0.7. There were no clinically significant differences in intraobserver reliability when SWE was performed in lesions categorized as BI‐RADS 3 or 4 as well as in histopathologically benign or malignant lesions. Conclusion Reliability of additional SWE was evaluated on a study cohort consisting of 1288 breast lesions categorized as BI‐RADS 3 and 4. SWE shows an excellent intraobserver reliability and a moderate interobserver reliability in the evaluation of solid breast masses.

Abstract:Background: Preoperative evaluation of axillary lymph node status is crucial for the selection of both systemic and surgical treatment in early breast cancer. This study assessed the particular role of additional shear wave elastography (SWE) in axillary staging in patients undergoing initial breast cancer diagnostics. Methods: One hundred patients undergoing axillary lymph node biopsy due to a sonographically suspicious axillary lymph node were prospectively evaluated with SWE using virtual touch tissue imaging quantification (VTIQ). Mean values of tissue stiffness for axillary tissue and lymph node tissue were measured prior to core-cut biopsy of the lymph node. All lymph nodes were clip-marked during the biopsy. Cut-off values to differentiate between malignant and benign lymph nodes were defined using Youden's index. Results: Lymph nodes with evidence of malignant tumor cells in the final pathological examination showed a significantly higher velocity as measured by SWE, with a mean velocity of 3.48 ± 1.58 m/s compared to 2.33 ± 0.62 m/s of benign lymph nodes (p < 0.0001). The statistically optimal cutoff to differentiate between malignant and benign lymph nodes was 2.66 m/s with a sensitivity of 69.8% and a specificity of 87.5%. Conclusions: Lymph node metastases assessed with SWE showed significantly higher elasticity values compared to benign lymph nodes. Thus, SWE provides an additional useful and quantifiable parameter for the sonographic assessment of suspicious axillary lymph nodes in the context of pre-therapeutic axillary staging in order to differentiate between benign and metastatic processes and support the guidance of definitive biopsy work-up. Relationship of Shear-wave Elastography Findings with Breast Cancer Molecular Subtypes and Comparison with Other Radiological Imaging Techniques

Abstract:Purpose: We aimed to evaluate the shear-wave elastography (SWE) findings of breast cancer and to compare their differences with other imaging techniques and to determine the rela-tionship between molecular subtypes. Methods: Radiological findings and histopathology results of patients who underwent biopsy due to breast mass and diagnosed as malignant were evaluated retrospectively. SWE findings, ultrasonography (US), mammography and magnetic resonance imaging (MRI) findings were evaluated. The histo-pathology results of the tru-cut biopsy performed under the guidance of US-SWE were analyzed. The findings of SWE and other radiological techniques were compared statistically and relationship with molecular subtypes was evaluated. Results: We had 51 patients with a median age of 58 years. In SWE, the mean tumor size was 20 mm, and the tumor elasticity was 105 kPa. Statistically signi-ficant correlation was found between the tumor size differences measured by B mode US-SWE and the tumor elasticity value. No correlation was found between SWE findings and molecular subtypes. SWE had significantly high positive correlation with mamography and MRI in terms of lesion size, with US very high positive correlation and with ER positivity weak negative correlation. Conclusion: In addition to conventional radiological imaging techniques the use of SWE provides useful insight to evaluation of breast cancer.
Second-Look Ultrasound Using Shear-Wave Elastography in MRI-Suspected Locoregional Recurrence of Breast Carcinoma

Abstract:Abstract Purpose To investigate if second-look US using shear-wave elastography (SWE) can help to differentiate between benign and malignant changes in the postoperative breast after surgical treatment of breast carcinoma. Materials and Methods SWE and related sonographic features were reviewed in 90 female patients with a history of surgical treatment of breast carcinoma and a suspicious lesion detected on a follow-up MRI scan. A single experienced radiologist performed all second-look US exams with SWE measurements placing a circular region of interest measuring 2 mm in diameter over the stiffest part of the lesion. Tissue samples for histopathological analysis were obtained during the same US examination via core-needle biopsy. Results Out of 90 lesions, 39 were proven malignant on histopathological analysis. 50 % of malignant lesions had Elmax values ranging from 128 to 199 kPa, and 50 % of benign lesions had Elmax values ranging from 65 to 169 kPa. The cut-off value of 171.2 kPa for Elmax shows a sensitivity of 59 % and specificity of 78.4 % for carcinoma recurrence, area under the curve 0.706 (CI95 % 0.6–0.81), P = 0.001. In univariate logistic models, restricted diffusion and stiffness on SWE, Elmax > 171.2 kPa, were shown as significant recurrence predictors. In the multivariate model, restricted diffusion remains significant independent recurrence predictor. With a recurrence prevalence of 43 %, the test sensitivity is 95 % (CI95 % 81–99 %) and the specificity is 75 % (CI95 % 60–85 %). Conclusion Stiffer lesions should be considered suspicious on second-look US in the postoperative breast and SWE can be a helpful tool in identifying malignant lesions, especially if this is related to restricted diffusion on MRI exam. Lesion stiffness, however, should not be considered as an independent predictor of lesion malignancy in the postoperative breast, because of benign changes that can appear stiff on SWE, as well as carcinoma recurrences that may appear soft.
Shear Wave Elastography Combined with Molecular Subtype in Early Predicting Response to Neoadjuvant Chemotherapy for Breast Cancer: A Prospective Case-control Study

Abstract:Abstract Background: This study was designed to investigate the performance of quantitative shear wave elastography (SWE) and the accuracy of SWE with the molecular subtype for early prediction of pathological response of breast cancer to neoadjuvant chemotherapy (NAC). Methods: In this prospective case-control study, 102 patients were screened from September 2016 to August 2020. Characteristics of conventional ultrasonography (US), SWE and contrast-enhanced magnetic resonance imaging (CE-MRI), were recorded, and the changes were compared to the pre-NAC baseline data. The pathological response was classified according to the Miller Payne grading system. Multivariate logistic regression was used to develop a predictive model for the response to NAC. Results: Significant differences related to changes in SWE characteristics of breast lesions between between pathological response groups were observed earlier than size on the conventional US and MRI images. According to the multivariate predictive model, the best parameter for predicting the pathological response after the first cycle of NAC was the molecular subtype of the tumor [area under the curve (AUC) = 0.83] with low sensitivity (66.04%). Better predictive performance was achieved when △A E/B and the molecular subtype were applied in combination after the second cycle of NAC (AUC = 0.92) with a higher sensitivity (86.79%). The predictive performance of molecular subtype combined with △A E/B and △SWV mean after the third and fourth cycles of NAC were improved (AUC = 0.94). Conclusion : The SWE can be an early predictor of the pathological response to NAC for breast cancer. The combination of SWE and the molecular subtype may be a preferred method for the clinical evaluation of NAC to facilitate the personalization of treatment regimens for breast cancer.
Anisotropy of Shear Wave Elastography in Breast Lesions and Its Correlation with Histopathology

Abstract:Objective To retrospectively study the anisotropy of shear wave elastography (SWE)quantitative parameters of breast lesions,and the correlation with histopathology.Methods A total of 281 consecutive women with 281 breast lesions (179 malignant,102 benign) were included,who underwent conventional ultrasound (US) and 2D SWE before surgical excision.Three acquisitions each for transverse and longitudinal planes were obtained,and maximum elasticity (Emax),mean elasticity (Emean),standard deviation (Esd) of the whole lesion and ratio between the elasticity in the mass and the fatty tissue (Eratio)were recorded.Anisotropic difference (AD) and anisotropy factors (AF) were calculated,and correlation with histopathology was analyzed.Results The average Emax,Emean and Esd of transverse planes were significantly higher than those of longitudinal planes.AF showed positive correlation with quantitative elasticity (Emax,Emean,Esd and Eratio) (P =0.000),and was significantly higher in malignant lesions than that in benign besions (P =0.000).AUC of AF was significantly higher than that of AD (P ＜0.001).AF was significantly higher in invasive ductal carcinoma than that in ductal carcinoma in situ.Higher AF was associated with higher histopathological grades of invasive ductal carcinoma (P =0.000),and correlated with ER/PR(+).Conclusions Anisotropy of SWE is an indicator of malignancy of breast lesions,and is of predictive value for prognosis in breast cancer.
Clinical application of shear wave elastography (SWE) in the diagnosis of benign and malignant breast diseases

Abstract:Shear wave elastography (SWE) is an emerging technique which can obtain quantitative elasticity values in breast disease. We therefore evaluated the diagnostic performance of SWE for the differentiation of breast masses compared with conventional ultrasound (US). Conventional US and SWE were performed by three experienced radiologists for 158 consecutive women who had been scheduled for US-guided core biopsy or surgical excision in 182 breast masses (89 malignancies and 93 benign; mean size, 1.76 cm). For each lesion, quantitative elasticity was measured in terms of the Young’s modulus (in kilopascals, kPa) with SWE, and BI-RADS final categories were assessed with conventional US. The mean elasticity values were significantly higher in malignant masses (153.3 kPa ± 58.1) than in benign masses (46.1 kPa ± 42.9), (P < 0.0001). The average mean elasticity values of invasive ductal (157.5 ± 57.07) or invasive lobular (169.5 ± 61.06) carcinomas were higher than those of ductal carcinoma in situ (117.8 kPa ± 54.72). The average mean value was 49.58 ± 43.51 for fibroadenoma, 35.3 ± 31.2 for fibrocystic changes, 69.5 ± 63.2 for intraductal papilloma, and 149.5 ± 132.4 for adenosis or stromal fibrosis. The optimal cut-off value, yielding the maximal sum of sensitivity and specificity, was 80.17 kPa, and the sensitivity and specificity of SWE were 88.8% (79 of 89) and 84.9% (79 of 93). The area under the ROC curve (Az value) was 0.898 for conventional US, 0.932 for SWE, and 0.982 for combined data. In conclusion, there were significant differences in the elasticity values of benign and malignant masses as well as invasive and intraductal cancers with SWE. Our results suggest that SWE has the potential to aid in the differentiation of benign and malignant breast lesions.

Value of shear wave elasticity in predicting the efficacy of neoadjuvant chemotherapy in different molecular types

Abstract:Purpose The purpose of this study was to evaluate the response to neoadjuvant chemotherapy (NAC) of different molecular subtypes of breast cancer using shear wave ultrasound elastography (SWE). Methods Ninety-eight patients with final diagnoses of breast cancer prior to NAC were examined with SWE and B-mode ultrasound. These parameters were compared with the response evaluation criteria in solid tumors (RECIST) index and pathological diagnoses. Then, we recorded the area under the receiver operating characteristic (ROC) curve. Immunohistochemical markers, including estrogen receptor (ER), progesterone receptor (PR), Ki67 index, and human epidermal growth factor receptor 2 (HER2) score, were examined before neoadjuvant treatment. Then, the diagnostic efficacy of SWE in different molecular subtypes was evaluated. Results One-way analysis of variance revealed that age, tumor margin, the change in tumor size after chemotherapy, and the average (Emean), minimum (Emin), and maximum (Emax) values from shear wave elastography ultrasound before chemotherapy were related to the RECIST index. Multivariate regression revealed that age, tumor margin, the change in tumor size after chemotherapy, and Emax were independently correlated with the RECIST index. According to the ROC curve, the area under the curve (AUC) of Emax was 0.773. The AUC of Emean, Emin, and the ratio between tumor tissue and normal tissue (EI) were 0.630, 0.617, and 0.510, respectively. The Emax, Emin, Emean, and EI before and after NAC were significantly different ( p ≤ 0.05). There was statistical significance in Luminal B type and HER2 enriched type in correlation results of the Emax values and RECIST indexes of different molecular subtypes. However, there was no significant difference in the Luminal A type and Triple negative types. There was no significant difference in the Emax values before NAC between the pathologic complete response (pCR) group and the non-pCR group ( p > 0.05). Conclusion Emax can be used to predict the response to NAC in women with invasive breast cancer, especially those with the Luminal B type and HER2 enriched type, but it cannot be used to predict pCR to NAC.

Correlation analysis between shear-wave elastography and pathological profiles in breast cancer

Abstract:PurposeTo explore the correlation between shear-wave elastography (SWE) parameters and pathological profiles of invasive breast cancer.MethodsA total of 197 invasive breast cancers undergoing preoperative SWE and primary surgical treatment were included. Maximum elastic modulus (Emax), mean elastic modulus (Emean), and elastic modulus standard deviation (Esd) were calculated by SWE. Pathological profile was gold standard according to postoperative pathology. The relationship between SWE parameters and pathological factors were analyzed using univariate and multivariate analysis.ResultsIn univariate analysis, large cancers showed significantly higher Emax, Emean and Esd (all P < 0.001). Emax and Esd in the group of histological grade III were higher than those in the group of grade I (both P < 0.05). Invasive lobular carcinomas (ILC) showed higher Emean than invasive ductal carcinoma (IDC) (P < 0.001). Lymphovascular invasion (LVI) group showed higher Emax values than negative group (P < 0.05). Emax, Emean and Esd of the Ki-67 positive group presented higher values than negative group (all P < 0.05). Androgen receptor (AR) positive lesions had lower Esd than AR negative lesions (P < 0.05). In multivariate analysis, invasive size independently influenced Emax (P < 0.001). Invasive size and pathological type both independently influenced Emean (both P < 0.001). Invasive size and AR status were both independently influenced Esd (both P < 0.05).ConclusionSWE parameters correlated with pathological profiles of invasive breast cancer.In particular, AR positive group showed significantly low Esd than negative group.

Abstract:Abstract Background Elastography is a promising way to evaluate tissue differences regarding stiffness, and the stiffness of the malignant breast lesions increased at the lesion margin. However, there is a lack of data on the value of the shear wave elastography (SWE) parameters of the surrounding tissue (shell) of different diameter on the diagnosis of benign and malignant breast lesions. Therefore, the purpose of our study was to evaluate the diagnostic performance of shell elasticity in the diagnosis of benign and malignant breast lesions using SWE. Methods Between September 2016 and June 2017, women with breast lesions underwent both conventional ultrasound (US) and SWE. Elastic values of the lesions peripheral tissue were determined according to the shell size, which was automatically drawn along the edge of the lesion using the following software guidelines: (1): 1 mm; (2): 2 mm; and (3): 3 mm. Quantitative elastographic features of the inner lesions and shell, including the elasticity mean (E mean ), elasticity maximum (E max ), and elasticity minimum (E min ), were calculated using an online-available software. The receiver operating characteristic curves (ROCs) of the elastographic features was analyzed to assess the diagnostic performance, and the area under curve (AUC) of each elastographic feature was obtained. Logistic regression analysis was used to predict significant factors of malignancy, permitting the design of predictive models. Results This prospective study included 63 breast lesions of 63 women. Of the 63 lesions, 33 were malignant and 30 were benign. The diagnostic performance of E max-3shell was the highest (AUC = 0.76) with a sensitivity of 60.6% and a specificity of 83.3%. According to stepwise logistic regression analysis, the E max-3shell and the E min-3shell were significant predictors of malignancy ( p < 0.05). The AUC of the predictive equation was 0.86. Conclusions SWE features, particularly the combination of E max-3shell and E min-3shell can improve the diagnosis of breast lesions.

Abstract:PURPOSETo analyze the diagnostic performance of shear wave elastography (SWE) in differentiating between benign and malignant breast lesions, with special emphasis on the value of the "stiff rim" sign, as compared with conventional ultrasonography (US).MATERIALS AND METHODSFor this ethics committee-approved retrospective study, all patients provided verbal informed consent for the analysis of their imaging data. A total of 193 consecutive women (age range, 18-82 years; mean age, 46 years) with 193 breast lesions (56 malignant, 137 benign) were included. The stiff rim sign, a qualitative SWE feature, was evaluated at the display setting of less than 180 kPa and at 180 kPa. The quantitative SWE features were assessed. Sensitivity, specificity, the area under the receiver operating characteristic curve (Az), and positive and negative likelihood ratios were calculated for conventional US features, for SWE features, and for combined conventional US and SWE features.RESULTSAmong all qualitative and quantitative SWE features, the stiff rim sign at the display setting (<180 kPa) showed the highest Az (0.918; 95% confidence interval [CI]: 0.870, 0.953), which was comparable to that for conventional US (0.891; 95% CI: 0.838, 0.931) (P = .40). The combination of the stiff rim sign at less than 180 kPa and conventional US features, with a positive likelihood ratio of 12.23 (95% CI: 11.5, 13.0) and a negative likelihood ratio of 0.02 (95% CI: 0.003, 0.1), yielded the highest Az (0.982; 95% CI: 0.951, 0.995)-higher than those for conventional US only and for any single SWE feature alone (P < .001 for all)-and yielded higher sensitivity (98.2% [55 of 56]; 95% CI: 90.4%, 100.0%) and similar specificity (92.0% [126 of 137]; 95% CI: 86.1%, 95.9%) compared with conventional US (P < .001 and P = .58, respectively). With use of this combination, the rate of recommendation of benign lesions for interventional procedures would decrease from 56.9% (78 of 137) (on the basis of conventional US features) to 15.3% (21 of 137), and 100.0% (56 of 56) (95% CI: 93.6%, 100.0%) malignant lesions would be correctly selected for biopsy.CONCLUSIONAdding SWE features, especially the stiff rim sign at the display setting (<180 kPa), to conventional US has the potential to improve the differentiation of breast lesions.

Abstract:The objective of this study was to evaluate the inter-operator reproducibility of strain elastography (SE) and shear wave elastography (SWE) in three groups: all lesions, benign lesions and malignant lesions. Ninety-one lesions from ninety-one women were examined by SE and SWE from January 2017 to December 2017 by two radiologists. The reproducibility of elastic score, SE strain ratio and SWE Young's modulus between operators was prospectively evaluated. There was good agreement on elasticity score, with κ values of 0.711, 0.640 and 0.766. The intra-class correlation coefficients of the strain ratio, mean elastic modulus (Emean), maximum elastic modulus (Emax) and elastic modulus standard deviation (Esd) ranged from 0.723–0.876, which indicated good and excellent agreement. We concluded that both SE and SWE had good reproducibility among different operators using the same probe in the same ultrasound instrument. Strain elasticity score was more consistent among operators in malignant breast tumors. There was better agreement on strain elastic ratio and shear wave elasticity among operators in benign breast lesions.
The Utility of Quantitative Parameters of Shear-Wave Elastography to Predict Prognostic Histologic Features of Breast Cancer

Abstract:ABSTRACT: In this study, we aimed to investigate the correlation of stiffness values of shear-wave elastography (SWE) and histopathological prognostic factors in patients with breast cancer. Between January 2021 and June 2022, SWE images of 138 core-biopsy proven breast cancer lesions from 132 patients were retrospectively reviewed. Histopathogic prognostic factors, including tumor size, histologic grade, histologic type, hormone receptor positivity, human epidermal growth factor receptor (HER2) status, immunohistochemical subtype and Ki-67 index were documented. Elasticity values including mean and maximum elasticity ( Emean and Emax ) and lesion-to-fat ratio ( Eratio ) were recorded. The association between histopathological prognostic factors and elasticity values were assessed using Mann-Whitney U and Kruskal-Wallis test, and multiple linear regression analysis. Tumor size, histological grade, and Ki-67 index were significantly associated with the Eratio ( P < 0.05). Larger tumor size and higher Ki-67 index also showed significantly higher Emean and Emax values ( P < 0.05). However, hormone receptor positivity, HER2 status, and immunohistochemical subtype were not significantly associated with elasticity values ( P > 0.05). Multivariate logistic regression analysis revealed that tumor size was significantly associated with Emean , Emax , and Eratio values ( P < 0.05). A high Ki-67 index was also significantly associated with high Eratio values. Larger tumor size and higher Ki-67 index are independently associated with high Eratio values. Preoperative SWE may improve the performance of conventional ultrasound in predicting prognosis and treatment planning.
