Abstract:Summary Background In 2019, the Global Burden of Diseases, Injuries, and Risk Factors Study attributed 0·98 million deaths to ambient air pollution in India based on potentially inappropriate exposure–response functions from countries with low air pollution levels. Instead, using data from India, we investigated long-term exposure to PM 2·5 and all-cause mortality with a causal inference method. Methods We collected national counts of annual mortality from 2009 to 2019 from the Civil Registration System at the district level to calculate annual district-level mortality rate as our main outcome and obtained annual PM 2·5 concentrations from a high-resolution spatiotemporal model. We applied an extended version of the difference-in-differences design by use of generalised additive models with quasi-Poisson distribution, including indicator variables and separate time trends for spatial administrative divisions. PM 2·5 concentrations obtained at 1 km × 1 km spatial resolution across the country were used to calculate annual district-level mean PM 2·5 concentrations. Similarly, we collected confounders at the district level, such as mean and SD of quarterly temperatures, gross domestic product per capita, population aged 60 years or older, clean cooking fuel usage, literacy in women, and median age. The spatial unit of analysis was administrative division. Findings The annual median population-weighted PM 2·5 was 38·9 μg/m 3 (5–95th percentile 19·7–71·8 μg/m 3 ). The full population lived in areas with PM 2·5 concentrations exceeding the 5 μg/m 3 annual mean recommended in the WHO guidelines, and 1·1 billion of 1·4 billion (81·9% of the total population) lived in areas above the Indian National Ambient Air Quality Standards for annual mean PM 2·5 not exceeding 40 μg/m 3 . A 10 μg/m 3 increase in annual PM 2·5 concentration was associated with an 8·6% (95% CI 6·4–10·8) higher annual mortality. Based on the Indian National Ambient Air Quality Standards, a total of 3·8 million (95% CI 2·9–4·9) deaths between 2009 and 2019 were attributable to PM 2·5 , amounting to 5·0% (3·8–6·4) of total mortality. Based on the WHO guidelines, a total of 16·6 million (13·0–21·8) deaths were attributable to PM 2·5 , amounting to 24·9% (19·5–32·5) of total mortality. Interpretation Our difference-in-differences approach allowed us to assess the full extent of registered deaths in the most populated country in the world, which has high levels of air pollution. We provide new evidence of increased mortality risk from long-term PM 2·5 , which emphasises the need for tighter regulatory standards to potentially substantially reduce mortality across India. Abstract:Air pollution in India is a foremost environmental risk factor that affects human health. This study first investigates the geographical distribution of ambient and household air pollution (HAP) and then examines the associated mortality risk. Data on fine particulate matter (PM2.5) concentration has been extracted from the Greenhouse Gas Air Pollution Interactions and Synergies (GAINS) model. HAP, mortality and socio‐demographic data were extracted from the National Family and Health Survey‐5, India, 2019–2021. Regression models were applied to see the difference in age‐group mortality by different pollution parameters. The districts with PM2.5 concentration above the National Ambient Air Quality Standard (NAAQS) level of 40 μg/m3 show a higher risk of neonatal (OR‐1.86, CI 1.418–2.433), postneonatal (OR‐2.04, CI 1.399–2.971), child (OR‐2.19, CI 0.999–4.803) and adult death (OR‐1.13, CI 1.060–1.208). The absence of a separate kitchen shows a higher probability of neonatal (OR: 1.18, CI 1.074–1.306) and adult death (OR‐1.06, CI 1.027–1.088). The interaction between PM2.5 levels above NAAQS and HAP leads to a substantial rise in mortality observed for neonatal (OR 1.19 CI 1.051–1.337), child (OR 1.17 CI 1.054–1.289), and adult (OR 1.13 CI 1.096–1.168) age groups. This study advocates that there is a strong positive association between ambient and HAP and mortality risk. PM2.5 pollution significantly contributes to the mortality risk in all age groups. Children are more vulnerable to HAP than adults. In India, policymakers should focus on reducing the anthropogenic PM2.5 emission at least to reach the NAAQS, which can substantially reduce disease burden and, more precisely, mortality.

Abstract:Ambient particulate matter of aerodynamic diameter less than 2.5 microns PM2.5) levels in Delhi routinely exceed World Health Organization (WHO) guidelines and Indian National Ambient Air Quality Standards (NAAQS) for acceptable levels of daily exposure. Only a handful of studies have examined the short-term mortality effects of PM in India, with none from Delhi examining the contribution of PM2.5.OBJECTIVES: We aimed to analyze the association between short-term PM2.5 exposures and daily nonaccidental mortality in Delhi, India.METHODS: Using generalized additive Poisson regression models, we examined the association between daily PM2.5 exposures and nonaccidental mortality between June 2010 and December 2016. Daily exposures to PM2.5 were estimated using an ensemble averaging technique developed by our research group, and mortality data were obtained from the Municipal Corporations of Delhi and the New Delhi Municipal Council.RESULTS: Median exposures to PM2.5 were 91.1 µg/m3 (interquartile range = 68.9, 126.2), with minimum and maximum exposures of 21.4 µg/m3 and 276.7 µg/m3, respectively. Total nonaccidental deaths recorded in Delhi during the study period were 700,512. Each 25 µg/m3 increment in exposure was associated with a 0.8% (95% confidence intervals [CI] = 0.3, 1.3%) increase in daily nonaccidental mortality in the study population and a 1.5% (95% CI = 0.8, 2.2%) increase in mortality among those with 60 years of age or over. The exposure-response relationship was nonlinear in nature, with relative risk rising rapidly before tapering off above 125 µg/m3. Meeting WHO guidelines for acceptable levels of exposure over the study period would have likely averted 17,526 (95% CI = 6,837, 25,589) premature deaths, with older and male populations disproportionately affected.DISCUSSION: This study provides robust evidence of the impact of short-term exposure to PM2.5 on nonaccidental mortality with important considerations for various stakeholders including policymakers and physicians. Most importantly, we find that reducing exposures significantly below current levels would substantially decrease the mortality burden associated with PM2.5.

Part 1. Short-term effects of air pollution on mortality: results from a time-series analysis in Chennai, India

Abstract:This report describes the results of a time-series analysis of the effect of short-term exposure to particulate matter with an aerodynamic diameter < or = 10 pm (PM10) on mortality in metropolitan Chennai, India (formerly Madras). This was one of three sites in India chosen by HEI as part of its Public Health and Air Pollution in Asia (PAPA) initiative. The study involved integration and analysis of retrospective data for the years 2002 through 2004. The data were obtained from relevant government agencies in charge of routine data collection. Data on meteorologic confounders (including temperature, relative humidity, and dew point) were available on all days of the study period. Data on mortality were also available on all days, but information on cause-of-death (including accidental deaths) could not be reliably ascertained. Hence, only all-cause daily mortality was used as the major outcome for the time-series analyses. Data on PM10, nitrogen dioxide (NO2), and sulfur dioxide (SO2) were limited to a much smaller number of days, but spanned the full study period. Data limitations resulting from low sensitivity of gaseous pollutant measurements led to using only PM10 in the main analysis. Of the eight operational ambient air quality monitor (AQM) stations in the city, seven met the selection criteria set forth in the common protocol developed for the three PAPA studies in India. In addition, all raw data used in the analysis were subjected to additional quality assurance (QA) and quality control (QC) criteria to ensure the validity of the measurements. Two salient features of the PM10 data set in Chennai were a high percentage of missing readings and a low correlation among daily data recorded by the AQMs. The latter resulted partly because each AQM had a small footprint (approximate area over which the air pollutant measurements recorded in the AQM are considered valid), and partly because of differences in source profiles among the 10 zones within the city. The zones were defined by the Chennai Corporation based on population density. Alternative exposure series were developed to control for these data features. We first developed exposure series based on data from single AQMs and multiple AQMs. Because neither was found to satisfactorily represent population exposures, we subsequently developed an exposure series that disaggregated pollutant data to individual zones within the city boundary. The zonal series, despite some uncertainties, was found to best represent population exposures among other available choices. The core model was thus a zonal model developed using disaggregated mortality and pollutant data from individual zones. We used quasi-Poisson generalized additive models (GAMs) with smooth functions of time, temperature, and relative humidity modeled using penalized splines. The degrees of freedom (df) for these confounders were selected to maximize the precision with which the relative risk for PM10 was estimated. This is a deviation from the traditional approaches to degrees of freedom selection, which usually aim to optimize overall model fit. Our approach led to the use of 8 df/year for time, 6 df/year for temperature, and 5 df/year for relative humidity. The core model estimated a 0.44% (95% confidence interval [CI] = 0.17 to 0.71) increase in daily all-cause mortality per 10-pg/m3 increase in daily average PM10 concentrations. Extensive sensitivity analyses compared models constructed using alternative exposure series and contributions of model parameters to the core model with regard to confounder degrees of freedom, alternative lags for exposure and meteorologic confounders, inclusion of outliers, seasonality, inclusion of multiple pollutants, and stratification by sex and age. The sensitivity analyses showed that our estimates were robust to a range of specifications and were also comparable to estimates reported in previous time-series studies: PAPA, the National Morbidity, Mortality, and Air Pollution Study (NMMAPS), Air Pollution and Health: A European Approach (APHEA), and Air Pollution and Health: A European and North American Approach (APHENA). While the approaches developed in previous studies served as the basis for our model development, the present study has new refinements that have allowed us to address specific data limitations (such as missing measurements and small footprints of air pollution monitors). The methods developed in the study may allow better use of routine data for time-series analysis in a broad range of settings where similar exposure and data-related issues prevail. We hope that the estimates derived in this study, although somewhat tentative, will facilitate local environmental management initiatives and spur future studies.
Ambient air pollution and daily mortality in ten cities of India: a causal modelling study

Abstract:Background: The evidence for acute effects of air pollution on mortality in India is scarce, despite the extreme concentrations of air pollution observed. This is the first multi-city study in India that examines the association between short-term exposure to PM2·5 and daily mortality using causal methods that highlight the importance of locally generated air pollution. Methods: We applied a time-series analysis to ten cities in India between 2008 and 2019. We assessed city-wide daily PM2·5 concentrations using a novel hybrid nationwide spatiotemporal model and estimated city-specific effects of PM2·5 using a generalised additive Poisson regression model. City-specific results were then meta-analysed. We applied an instrumental variable causal approach (including planetary boundary layer height, wind speed, and atmospheric pressure) to evaluate the causal effect of locally generated air pollution on mortality. We obtained an integrated exposure-response curve through a multivariate meta-regression of the city-specific exposure-response curve and calculated the fraction of deaths attributable to air pollution concentrations exceeding the current WHO 24 h ambient PM2·5 guideline of 15 μg/m3. To explore the shape of the exposure-response curve at lower exposures, we further limited the analyses to days with concentrations lower than the current Indian standard (60 μg/m3). Findings: We observed that a 10 μg/m3 increase in 2-day moving average of PM2·5 was associated with 1·4% (95% CI 0·7-2·2) higher daily mortality. In our causal instrumental variable analyses representing the effect of locally generated air pollution, we observed a stronger association with daily mortality (3·6% [2·1-5·0]) than our overall estimate. Our integrated exposure-response curve suggested steeper slopes at lower levels of exposure and an attenuation of the slope at high exposure levels. We observed two times higher risk of death per 10 μg/m3 increase when restricting our analyses to observations below the Indian air quality standard (2·7% [1·7-3·6]). Using the integrated exposure-response curve, we observed that 7·2% (4·2%-10·1%) of all daily deaths were attributed to PM2·5 concentrations higher than the WHO guidelines. Interpretation: Short-term PM2·5 exposure was associated with a high risk of death in India, even at concentrations well below the current Indian PM2·5 standard. These associations were stronger for locally generated air pollutants quantified through causal modelling methods than conventional time-series analysis, further supporting a plausible causal link. Abstract:Forecasting alterations in ambient air pollution and the consequent health implications is crucial for safeguarding public health, advancing environmental sustainability, informing economic decision making, and promoting appropriate policy and regulatory action. However, predicting such changes poses a substantial challenge, requiring accurate data, sophisticated modeling methodologies, and a meticulous evaluation of multiple drivers. In this study, we calculate premature deaths due to ambient fine particulate matter (PM2.5) exposure in India from the 2020s (2016-2020) to the 2100s (2095-2100) under four different socioeconomic and climate scenarios (SSPs) based on four CMIP6 models. PM2.5 concentrations decreased in all SSP scenarios except for SSP3-7.0, with the lowest concentration observed in SSP1-2.6. The results indicate an upward trend in the five-year average number of deaths across all scenarios, ranging from 1.01 million in the 2020s to 4.12-5.44 million in the 2100s. Further analysis revealed that the benefits of reducing PM2.5 concentrations under all scenarios are largely mitigated by population aging and growth. These findings underscore the importance of proactive measures and an integrated approach in India to improve atmospheric quality and reduce vulnerability to aging under changing climate conditions.
Long-term exposure to particulate matter and all-cause and cause-specific mortality in an analysis of multiple Asian cohorts

Abstract:Background: Exposure to ambient air pollution is associated with a significant number of deaths. Much of the evidence associating air pollution with adverse effects is from North American and Europe, partially due to incomplete data in other regions limiting location specific examinations. The aim of the current paper is to leverage satellite derived air quality data to examine the relationship between ambient particulate matter and all-cause and cause-specific mortality in Asia. Methods: Six cohorts from the Asia Cohort Consortium provided residential information for participants, recruited between 1991 and 2008, across six countries (Bangladesh, India, Iran, Japan, South Korea, and Taiwan). Ambient particulate material (PM2·5) levels for the year of enrolment (or 1998 if enrolled earlier) were assigned utilizing satellite and sensor-based maps. Cox proportional models were used to examine the association between ambient air pollution and all-cause and cause-specific mortality (all cancer, lung cancer, cardiovascular and lung disease). Models were additionally adjusted for urbanicity (representing urban and built characteristics) and stratified by smoking status in secondary analyses. Country-specific findings were pooled via random-effects meta-analysis. Findings: More than 300,000 participants across six cohorts were included, representing more than 4-million-person years. A positive relationship was observed between a 5 µg/m (Dockery et al., 1993) increase in PM2·5 and cardiovascular mortality (HR: 1·06, 95 % CI: 0.99, 1·13). The additional adjustment for urbanicity resulted in increased associations between PM2.5 and mortality outcomes, including all-cause mortality (1·04, 95 % CI: 0·97, 1·11). Results were generally similar regardless of whether one was a current, never, or ex-smoker. Interpretation: Using satellite and remote sensing technology we showed that associations between PM2.5 and all-cause and cause-specific Hazard Ratios estimated are similar to those reported for U.S. and European cohorts. Health Risk Associated with Potential Source Regions of PM2.5 in Indian Cities

Abstract:This paper estimates the regional contribution of high PM2.5 concentration and associated mortality using HYSPLIT back trajectory analysis in eight Indian cities during 2015–2016. Health risk and mortality estimation were carried out using the Integrated Exposure Response function (IER) which was verified using our previous time series study in Delhi. Risk estimates from IER were observed to be slightly over-predicted (2.14%) when compared to health risk from time series study in Delhi. Health risk in the eight cities across the four seasons indicated higher chronic obstructive pulmonary disease (COPD), lung cancer (LC), ischemic heart disease (IHD), and stroke in the northern (COPD = 1.35, LC = 1.50, IHD = 1.39, Stroke = 2.06) and eastern cities (COPD = 1.27, LC = 1.38, IHD = 1.35, Stroke = 1.93) as compared to in southern or western cities. Risk of stroke was observed to be the highest: North = 1.37–1.52, South = 1.20–1.31, East = 1.40–1.52, and West = 1.24–1.35 times to that of other diseases. Uttar Pradesh was observed to be a major contributor to premature mortality in Delhi, Lucknow, and Patna accounting for 30, 71, and 42% of total premature death due to high PM2.5 concentration during winter. Similarly, high PM2.5 concentration from West Bengal and Bangladesh was responsible for 52% of total premature mortality in Kolkata while the Indian Ocean was a major contributor to premature mortality in western and southern cities during winter. Reduction of both local and regional pollution is required to yield a significant reduction in pollution of all cities except Delhi and Lucknow where regional and local sources respectively are dominant.

However, little is known about the factors responsible for these inter-regional differences. In this contribution, we used data from a multi-country multi-city study to identify contextual factors that modify short-term mortality risks of PM. Methods: We collected daily series of mortality counts and air pollution data (PM10, PM2.5) for 327 cities in 25 countries in the period 1985-2015. City-level standardized measures of demographic, socioeconomic, and environmental indicators were derived from the OECD Regional/Metropolitan Database, in addition to climatological variables from the weather series. We applied a two-stage time-series design. First, for each city we modelled the linear associations of two-day moving average PM10, and PM2.5 with total mortality using quasi-Poisson regression and distributed lag nonlinear models. Second, we used meta-regression models to evaluate the effect modification of each indicator. Results: In preliminary analyses, the pooled percent mortality change for a 10 μg/m3 increase in PM10 and PM2.5 concentrations were 0.20% 95%CI(0.14%; 0.25%), and 0.31% 95%CI(0.21%; 0.41%) respectively. For PM10, significant changes of the figures above, for an IQR increase in each factor, were found for Gini index (0.12% to 0.29%, p=0.015), average temperature (0.13% to 0.27%, p=0.001), and Sprawl index (0.14% to 0.26%, p=0.033). For PM2.5, a tendency to increase the mortality risk was found for GDP (0.24% to 0.38%, p=0.073), while cities with an extended green area have a lower percent mortality change (0.41% to 0.20%, p=0.054). Conclusion: This represents the largest study assessing effect modification of ambient particulate air pollution and mortality relationships, performed by comparing different populations across the world. Preliminary findings suggest larger PM-mortality risks, in urban communities characterised by a higher level of inequalities, warmer climate, and less green area.
Estimates of Global Mortality Burden Associated with Short-Term Exposure to Fine Particulate Matter (PM2·5)

Abstract:Background The acute health effects of short-term (hours to days) exposure to fine particulate matter (PM2.5) have been well documented; however, the global mortality burden attributable to this exposure has not been estimated. We aimed to estimate the global, regional, and urban mortality burden associated with short-term exposure to PPM2.5 and the spatiotemporal variations in this burden from 2000 to 2019. Methods We combined estimated global daily PM2.5 concentrations, annual population counts, country-level mortality rates, and epidemiologically derived exposure-response functions to estimate the mortality attributable to short-term PM2.5 exposure from 2000 to 2019, in the continental regions and in 13 189 urban centres worldwide at a spatial resolution of 0.1 degrees x 0.1 degrees. We tested the robustness of our mortality estimates with different theoretical minimum risk exposure levels, lag effects, and exposure-response functions. Findings Approximately 1 million (95% CI 690 000-1.3 million) premature deaths per year from 2000 to 2019 were attributable to short-term PM2.5 exposure, representing 2.08% (1.41-2.75) of total global deaths or 17 (11-22) premature deaths per 100 000 population. Annually, 0.23 million (0.15 million-0.30 million) deaths attributable to short-term PM2.5 exposure were in urban areas, constituting 22.74% of the total global deaths attributable to this cause and accounting for 2.30% (1.56-3.05) of total global deaths in urban areas. The sensitivity analyses showed that our worldwide estimates of mortality attributed to short-term PM2.5 exposure were robust. Interpretation Short-term exposure to PM2.5 contributes a substantial global mortality burden, particularly in Asia and Africa, as well as in global urban areas. Our results highlight the importance of mitigation strategies to reduce short-term exposure to air pollution and its adverse effects on human health. Copyright (c) 2024 The Author(s). Abstract:BACKGROUND: Ambient fine particulate matter [PM <math><mrow><mo>≤</mo><mn>2.5</mn><mspace></mspace><mi>μ</mi><mi>m</mi></mrow></math> in aerodynamic diameter (<math><mrow><msub><mrow><mrow><mi>PM</mi></mrow></mrow><mrow><mrow><mn>2.5</mn></mrow></mrow></msub></mrow></math>)] is a major health risk for children, particularly in South Asia, which currently experiences the highest <math><mrow><msub><mrow><mrow><mi>PM</mi></mrow></mrow><mrow><mrow><mn>2.5</mn></mrow></mrow></msub></mrow></math> levels globally. Nevertheless, there is comparatively little epidemiological evidence from this region to quantify the effects of <math><mrow><msub><mrow><mrow><mi>PM</mi></mrow></mrow><mrow><mrow><mn>2.5</mn></mrow></mrow></msub></mrow></math> on child survival.OBJECTIVES: We estimated the association between <math><mrow><msub><mrow><mrow><mi>PM</mi></mrow></mrow><mrow><mrow><mn>2.5</mn></mrow></mrow></msub></mrow></math> exposure and child survival in India.METHODS: We constructed a large, retrospective, and nationally representative cohort of children <math><mrow><mo><</mo><mn>5</mn><mtext> years of age</mtext></mrow></math>, born between 2009-2016, from the publicly available, cross-sectional 2015-2016 Demographic Health Surveys in India. In utero and post-delivery lifetime average ambient <math><mrow><msub><mrow><mrow><mi>PM</mi></mrow></mrow><mrow><mrow><mn>2.5</mn></mrow></mrow></msub></mrow></math> exposures were estimated with data from satellite remote sensing, meteorology, and land use information (model <math><mrow><msup><mrow><mi>R</mi></mrow><mrow><mn>2</mn></mrow></msup><mo>=</mo></mrow></math> 0.82). We used Cox proportional hazards regression to estimate the association between both average in utero and post-delivery lifetime <math><mrow><msub><mrow><mrow><mi>PM</mi></mrow></mrow><mrow><mrow><mn>2.5</mn></mrow></mrow></msub></mrow></math> and all-cause child mortality, controlling for individual- and household-level covariates, seasonality, location, and meteorology.RESULTS: Over 7,447,724 child-months of follow-up, there were 11,559 deaths at <math><mrow><mo><</mo><mn>5</mn><mtext> years of age</mtext></mrow></math> reported by the children's mothers. Abstract:Introduction Air pollution presents a major public health threat to India, affecting more than three quarters of the country's population. In the current project, GEOHealth Health Effects of Selected Environmental Exposomes Across the Life CourSe–India, we aim to study the effect of environmental exposomes—fine particulate matter (PM 2.5 ), nitrogen dioxide (NO 2 ), ozone (O 3 ) and extremes of temperature—on multiple health outcomes using a modified life course approach. The associated training grant aims to build capacity in India to address the unique environmental health problems. Methods and analysis The project aims to (A) Develop exposure assessments in seven cities, namely Delhi, Chennai, Sonipat, Vizag, Pune, Hyderabad and Bikaner, for: (1) A fine-scale spatiotemporal model for multiple pollutants (PM 2.5 , NO 2 , O 3, temperature); (2) Combined ground monitoring and modelling for major chemical species of ambient PM 2.5 at seven cities; and (3) Personal exposure assessment in a subsample from the six cities, except Pune, and (B) Conduct health association studies covering a range of chronic non-communicable diseases and their risk factors leveraging a unique approach using interdigitating cohorts. We have assembled existing pregnancy, child, adolescent, adult and older adult cohorts across India to explore health effects of exposomes using causal analyses. We propose to use Bayesian kernel machine regression to assess the effects of mixtures of all pollutants including species of PM 2.5 on health while accounting for potential non-linearities and interactions between exposures. This builds on earlier work that constructed a fine spatiotemporal model for PM 2.5 exposure to study health outcomes in two Indian cities. Ethics and dissemination Ethical clearance for conduct of the study was obtained from the Institutional Ethics Committee (IEC) of the Centre for Chronic Disease Control, and all the participating institutes and organisations. National-level permission was provided by the Indian Council of Medical Research. The research findings will be disseminated through peer-reviewed publications, policy briefs, print and social media, and communicating with the participating communities and stakeholders. Training of Indian scientists will build the capacity to undertake research on selected adverse environmental exposures on population health in India.

Abstract:Rationale: The associations between ambient coarse particulate matter (PM2.5-10) and daily mortality are not fully understood on a global scale. Objectives: To evaluate the short-term associations between PM2.5-10 and total, cardiovascular, and respiratory mortality across multiple countries/regions worldwide. Methods: We collected daily mortality (total, cardiovascular, and respiratory) and air pollution data from 205 cities in 20 countries/regions. Concentrations of PM2.5-10 were computed as the difference between inhalable and fine PM. A two-stage time-series analytic approach was applied, with overdispersed generalized linear models and multilevel meta-analysis. We fitted two-pollutant models to test the independent effect of PM2.5-10 from copollutants (fine PM, nitrogen dioxide, sulfur dioxide, ozone, and carbon monoxide). Exposure-response relationship curves were pooled, and regional analyses were conducted. Measurements and Main Results: A 10 μg/m3 increase in PM2.5-10 concentration on lag 0-1 day was associated with increments of 0.51% (95% confidence interval [CI], 0.18%-0.84%), 0.43% (95% CI, 0.15%-0.71%), and 0.41% (95% CI, 0.06%-0.77%) in total, cardiovascular, and respiratory mortality, respectively. The associations varied by country and region. These associations were robust to adjustment by all copollutants in two-pollutant models, especially for PM2.5. The exposure-response curves for total, cardiovascular, and respiratory mortality were positive, with steeper slopes at lower exposure ranges and without discernible thresholds. Conclusions: This study provides novel global evidence on the robust and independent associations between short-term exposure to ambient PM2.5-10 and total, cardiovascular, and respiratory mortality, suggesting the need to establish a unique guideline or regulatory limit for daily concentrations of PM2.5-10.

Abstract:Abstract. Anthropogenic and biomass burning emissions are the major sources of ambient air pollution. India has experienced a dramatic deterioration in air quality over the past few decades, but no systematic assessment has been made to investigate the individual contributions of anthropogenic and biomass burning emissions. In this study, we conducted a pioneering comprehensive analysis of the long-term trends of particulate matter with aerodynamic diameters < 2.5 μm (PM2.5) and ozone (O3) in India and their mortality burden changes from 1995 to 2014, using a state-of-the-art high-resolution global chemical transport model (CAM-chem). Our simulations revealed a substantial nationwide increase in annual mean PM2.5 (6.71 μg m-3 decade-1) and O3 (7.08 ppbv decade-1), with the Indo-Gangetic Plain (IGP) and eastern central India as hotspots for PM2.5 and O3 trend changes individually. Noteworthy substantial O3 decreases were observed in the northern IGP which were potentially linked to NO titration due to a surge in NOx emissions. Sensitivity analyses highlighted anthropogenic emissions as primary contributors to rising PM2.5 and O3, while biomass burning played a prominent role in winter and spring. In years with high biomass burning activity, the contributions from BB on both PM2.5 and O3 changes were comparable with or even exceeding anthropogenic emissions in specific areas. The elevated air pollutants were associated with increased premature mortality attributable to PM2.5 and O3, leading to 97.83 K and 73.91 K per decade. Despite a per capita decrease in the IGP region, the increased population offset its effectiveness.

Abstract:BACKGROUND: The association between fine particulate matter (PM2.5) and mortality widely differs between as well as within countries. Differences in PM2.5 composition can play a role in modifying the effect estimates, but there is little evidence about which components have higher impacts on mortality.METHODS: We applied a 2-stage analysis on data collected from 210 locations in 16 countries. In the first stage, we estimated location-specific relative risks (RR) for mortality associated with daily total PM2.5 through time series regression analysis. We then pooled these estimates in a meta-regression model that included city-specific logratio-transformed proportions of seven PM2.5 components as well as meta-predictors derived from city-specific socio-economic and environmental indicators.RESULTS: We found associations between RR and several PM2.5 components. Increasing the ammonium (NH4+) proportion from 1% to 22%, while keeping a relative average proportion of other components, increased the RR from 1.0063 (95% confidence interval [95% CI] = 1.0030, 1.0097) to 1.0102 (95% CI = 1.0070, 1.0135). Conversely, an increase in nitrate (NO3-) from 1% to 71% resulted in a reduced RR, from 1.0100 (95% CI = 1.0067, 1.0133) to 1.0037 (95% CI = 0.9998, 1.0077). Differences in composition explained a substantial part of the heterogeneity in PM2.5 risk.CONCLUSIONS: These findings contribute to the identification of more hazardous emission sources. Further work is needed to understand the health impacts of PM2.5 components and sources given the overlapping sources and correlations among many components.

Abstract:Air pollution, characterized by high levels of particulate matter (PM), poses the greatest environmental threat to human health, causing an estimated 7 million deaths annually and accounting for 5 % of the global gross domestic product (GDP). While the health impacts of PM are influenced by the toxicity of its individual chemical constituents, the mortality burden of PM is solely based on its total mass concentration. This is because of a lack of large-scale, high-resolution data on PM chemical composition, needed for epidemiological assessments. Identifying which PM constituents are harmful to health has been the "holy grail" of atmospheric science since the landmark 1993 study on six US cities established a definitive link between PM and mortality. Ever since, atmospheric scientists have focused on understanding aerosol composition, emission sources, and formation pathways, while longitudinal epidemiological studies have required individual-level exposure data, employing land use regression models for the prediction of exposures at fine resolutions. In this opinion article, we argue that the time has come to shift the focus towards incorporating PM chemical composition into epidemiological health assessments, laying the foundation for the development of new regulatory metrics. This shift will enable the creation of targeted guidelines and subsequent regulations, prioritizing mitigation efforts against the most harmful anthropogenic emissions. Central to this shift is the availability of global, long-term, high-resolution data on PM chemical composition that are obtained through field observations and modelling outputs. In the article, we underscore key milestones within aerosol science that have been integral for advancing this foundational shift. Specifically, we examine emerging modelling tools for estimating exposure to individual PM components, present the type of ambient observations needed for model developments, identify key gaps in our fundamental understanding of emissions and their atmospheric transformation, and propose advancing cross-disciplinary collaboration between aerosol scientists and epidemiologists to understand the health impacts of individual PM components. We contend that aerosol science has now reached a pivotal moment in elucidating the differential health impacts of PM components, representing a first step towards their incorporation into air quality guidelines.
