Abstract:Endoscopic ultrasound (EUS) is one of the most important modalities for the diagnosis of digestive tract diseases. EUS has been evolving ever since it was introduced. New techniques such as elastography and contrast enhancement have emerged, increasing the accuracy, sensitivity and specificity of EUS for the diagnosis of digestive tract diseases including pancreatic masses and lymphadenopathy. EUS-elastography evaluates tissue elasticity and therefore, can be used to differentiate various lesions. Contrast-enhanced EUS can distinguish benign from malignant pancreatic lesions and lymphadenopathy using the intravenous injection of contrast agents. This review discusses the principles and types of these new techniques, as well as their clinical applications and limitations.

Abstract:Photoacoustic (PA) imaging is able to provide extremely high molecular contrast while maintaining the superior imaging depth of ultrasound (US) imaging. Conventional microscopic PA imaging has limited access to deeper tissue due to strong light scattering and attenuation. Endoscopic PA technology enables direct delivery of excitation light into the interior of a hollow organ or cavity of the body for functional and molecular PA imaging of target tissue. Various endoscopic PA probes have been developed for different applications, including the intravascular imaging of lipids in atherosclerotic plaque and endoscopic imaging of colon cancer. In this paper, the authors review representative probe configurations and corresponding preclinical applications. In addition, the potential challenges and future directions of endoscopic PA imaging are discussed.

In addition, Utilizing multi-wavelength laser excitation, a number of important physiological parameters, such as the total hemoglobin concentration, oxygen saturation of hemoglobin, and micro-hemodynamic flux, can be obtained by PAE. Such information directly correlates with vascular angiogenesis and abnormal metabolisms and thus reflects the formation and progression of many neoplasms.

Abstract:Photoacoustic imaging and optical coherence tomography are two emerging imaging modalities which provide complementary optical absorption and scattering contrasts for biological tissues. While photoacoustic imaging provides tissue vasculature information, optical coherence tomography offers micron-scale morphological imaging with penetration depths of 1 similar to 3 mms. Pulse-echo ultrasound is readily available from photoacoustic system and it provides tissue structure information at deeper depths with resolutions scalable with the transducer frequency. We present a prototype endoscope that consists of a ball lensed optical coherence tomography probe, a right-angled multimode fiber for delivering the laser beam for photoacoustic imaging, and a high frequency ultrasound transducer of 35 MHz center frequency. The overall diameter is 5mms. Porcine ovaries were imaged ex vivo to demonstrate the capability of this new combined endoscopy. The microvascular and high resolution structural images at subsurface and deeper tissue range demonstrate the synergy of the combined endoscopy over each modality alone.

Abstract:Endoscopy is an essential clinical tool for the diagnosis of gastrointestinal (GI) tract cancer. A photoacoustic system that elegantly combines optical and ultrasound endoscopy advantages by providing high‐sensitivity functional information and large imaging depth is a potentially powerful tool for GI tract imaging. Recently, several photoacoustic endoscopic imaging systems have been proposed and developed. However, the relatively large size and rigid length of the catheter make it difficult to translate them into wide clinical applications; while the existing system of a relatively small catheter, capable of in vivo animal imaging, is unable to acquire full (360°) field‐of‐view cross‐section images. In this study, we developed a photoacoustic/ultrasonic dual‐modality endoscopic system and a corresponding miniaturized, encapsulated imaging catheter, which provides a full 360° field‐of‐view. The diameter of the catheter is 2.5 mm, which is compatible with the 2.8‐mm instrumental channel of a conventional clinical optical endoscope. Using this system, we demonstrate in vivo 3‐dimensional endoscopic photoacoustic/ultrasonic imaging of the colorectum of a healthy Sprague Dawley rat, by depicting vasculature and morphology of the GI tract. The significantly improved imaging field of view, reduced catheter size, high‐quality imaging results suggest that the developed photoacoustic/ultrasonic dual‐modality endoscopy has a great potential to be translated into a broad range of clinical applications in gastroenterology.

Abstract:The rapid progress in biomedical imaging technology has generated considerable interest in new non-invasive photoacoustic endoscopy imaging techniques. This emerging technology offers significant benefits, including high spectral specificity, strong tissue penetration, and real-time multidimensional high-resolution imaging capabilities, which enhance clinical diagnosis and treatment of prostate cancer. This paper delivers a thorough review of current prostate cancer screening techniques, the core principles of photoacoustic endoscopy imaging, and the latest research on its use in detecting prostate cancer. Additionally, the limitations of this technology in prostate cancer detection are discussed, and future development trends are anticipated.

Abstract:Photoacoustic tomography (PAT) is an emerging biomedical imaging modality that combines optical and ultrasonic imaging, providing overlapping fields of view. This hybrid approach allows for a natural integration of PAT and ultrasound (US) imaging in a single platform. Due to the similarities in signal acquisition and processing, the combination of PAT and US imaging creates a new hybrid imaging for novel clinical applications. Over the recent years, particular attention is paid to the development of PAT/US dual-modal systems highlighting mutual benefits in clinical cases, with an aim of substantially improving the specificity and sensitivity for diagnosis of diseases. The demonstrated feasibility and accuracy in these efforts open an avenue of translating PAT/US imaging to practical clinical applications. In this review, the current PAT/US dual-modal imaging systems are discussed in detail, and their promising clinical applications are presented and compared systematically. Finally, this review describes the potential impacts of these combined systems in the coming future.

Abstract:Photoacoustic imaging (PAI) is an emerging biomedical imaging technique that combines the advantages of optical and ultrasound imaging, enabling the generation of images with both optical resolution and acoustic penetration depth. By leveraging similar signal acquisition and processing methods, the integration of photoacoustic and ultrasound imaging has introduced a novel hybrid imaging modality suitable for clinical applications. Photoacoustic-ultrasound imaging allows for non-invasive, high-resolution, and deep-penetrating imaging, providing a wealth of image information. In recent years, with the deepening research and the expanding biomedical application scenarios of photoacoustic-ultrasound bimodal systems, the immense potential of photoacoustic-ultrasound bimodal imaging in basic research and clinical applications has been demonstrated, with some research achievements already commercialized. In this review, we introduce the principles, technical advantages, and biomedical applications of photoacoustic-ultrasound bimodal imaging techniques, specifically focusing on tomographic, microscopic, and endoscopic imaging modalities. Furthermore, we discuss the future directions of photoacoustic-ultrasound bimodal imaging technology.

Abstract:Ultrasound/photoacoustic dual-modality imaging technology has greatly promoted the clinical application and photoacoustic imaging technology because it integrates the advantages of high-resolution structural imaging of ultrasound and high-contrast functional imaging of photoacoustic imaging. Traditional ultrasound/photoacoustic dual-modality imaging is based mainly on the array probe used in ultrasound imaging to collect photoacoustic signals at the same time. The system has a compact structure and easy operation. However, this kind of equipment utilizes array probes and multi-channel data acquisition system, which makes it expensive. And the imaging quality can be affected by the difference in channel consistency. In this paper, an ultrasound/photoacoustic dual-modality imaging method based on an acoustic scanning galvanometer is proposed. In this system, a single ultrasonic transducer combined with a one-dimensional acoustic scanning galvanometer is used for fast acoustic beam scanning to realize ultrasound/photoacoustic dual-modality imaging. It is a compact, low-cost and fast dual-modality imaging technology. The experimental results show that the effective imaging range of the system is 15.6 mm, and the temporal resolution of ultrasound and photoacoustic imaging are 1.0 and 0.1 s –1 (B scan), respectively (the temporal resolution of photoacoustic imaging is limited mainly by the laser repetition rate). Based on the proposed technology research, it is helpful to further promote the clinical transformation and popularization of ultrasound/photoacoustic dual-modality imaging. It also provides a low-cost, miniaturized and fast scheme for multimodal imaging technology which is based on ultrasound signal detection.

Abstract:Endoscopy, enabling high-resolution imaging of deep tissues and internal organs, plays an important role in basic research and clinical practice. Recent advances in photoacoustic microscopy (PAM), demonstrating excellent capabilities in high-resolution functional imaging, have sparked significant interest in its integration into the field of endoscopy. However, there are challenges in achieving functional PAM in the endoscopic setting. This Perspective article discusses current progress in the development of endoscopic PAM and the challenges related to functional measurements. Then, it points out potential directions to advance endoscopic PAM for functional imaging by leveraging fiber optics, microfabrication, optical engineering, and computational approaches. Finally, it highlights emerging opportunities for functional endoscopic PAM in basic and translational biomedicine.

Abstract:A miniature endoscope capable of imaging multiple tissue contrasts in high resolution is highly attractive, because it can provide complementary and detailed tissue information of internal organs. Here we present a photoacoustic (PA)-fluorescence (FL) endoscope for optical-resolution PA microscopy (PAM) and FL microscopy (FLM). The endoscope with a diameter of 2.8 mm achieves high lateral resolutions of 5.5 and 6.3 mu m for PAM and FLM modes, respectively. In vivo imaging of zebrafish larvae and a mouse ear is conducted, and high-quality images are obtained. Additionally, in vivo endoscopic imaging of a rat rectum is demonstrated, showing the endoscopic imaging capability of our endoscope. By providing dual contrasts with high resolution, the endoscope may open up new opportunities for clinical endoscopic imaging applications. (C) 2021 Optical Society of America

Abstract:We developed a miniaturized, simple and full field-of-view photoacoustic/ultrasonic endoscopy system, and used a flexible coil to transmit the rotational torque from the rotary stage, which enables a 360o field-of-view imaging in vivo. The developed imaging catheter was fully encapsulated by a single-use protective polyamide tube. A B-scan rate up to 5 Hz (200 A-lines/B-scan) was achieved. Three-dimensional photoacoustic and ultrasound images of the rectum from a SD rat were acquired in vivo. It suggests that this PAE system can be of great interest for clinical translation for a variety of endoscopic applications.

Abstract:Photoacoustic imaging is a novel imaging modality that utilizes pulsed laser to illuminate samples and ultrasonic transducers for detection of the laser absorption generated ultrasonic/photoacoustic waves. Benefited from its high optical contrast and acoustic resolution at depth, as well as the capability of acquiring optical spectral information, photoacoustic imaging allows for structural and functional imaging of the biological relevant information such as hemoglobin concentration, blood oxygen metabolism, and lipid with high specificity, and is able to capture the corresponding physiological feature dynamics with high sensitivity. In terms of the imaging system, photoacoustic imaging manifests a variety of embodiments catering the multi-scale biomedical imaging needs. The above mentioned features and capabilities enable extensive biomedical application potentials of this imaging modality in both clinical diagnosis and pathological analysis, such as malignant tumor detection, atherosclerotic plaque assessment, and microcirculation abnormality diagnosis. Therefore, it is attracting more and more attention and interests in a wide range of biomedical research areas. This article presented the recent progress of photoacoustic technology development and application studies in our group in the fields of microvascular imaging, intravascular imaging, gastrointestinal endoscopic imaging, and sentinel lymph node imaging.

Abstract:Photoacoustic imaging has drawn a significant amount of attention due to its unique capacity for functional, metabolic, and molecular imaging, which is achieved by the combination of optical excitation and acoustic detection. With both strengths of light and ultrasound, photoacoustic images can provide strong optical contrast at high ultrasound resolution in deep tissue. As photoacoustic imaging can be used to visualize complementary information to ultrasound imaging using the same data acquisition process, several studies have been conducted on combining photoacoustic imaging with existing clinical ultrasound systems. This review highlights our development of a photoacoustic/ultrasound dual-modal imaging system, various features and functionalities implemented for clinical translation, and preclinical/clinical studies performed by using the systems.

Abstract:There has been a longstanding expectation that the optical resolution embodiment of photoacoustic tomography could have a substantial impact on gastrointestinal endoscopy by enabling microscopic visualization of the vasculature based on the endogenous contrast mechanism. Although multiple studies have demonstrated the in vivo imaging capability of a developed imaging device over the last decade, the implementation of such an endoscopic system that can be applied immediately when necessary via the instrument channel of a video endoscope has been a challenge. In this study, we developed a 3.38 mm diameter catheter based, integrated optical resolution photoacoustic and ultrasonic miniprobe system and successfully demonstrated its intrainstrument channel workability for the standard 3.7 mm diameter instrument channel of a clinical video endoscope based on a swine model. Through the instrument channel, we acquired the first in vivo photoacoustic and ultrasonic dual mode endoscopic images from the esophagogastric junction, which is one of the most frequently cited anatomical sites in the gastrointestinal tract in relation to Barretts esophagus. Moreover, from a rat colorectum in vivo imaging experiment, we visualized hierarchically developed mesh like capillary networks with a hole size as small as 50 microns, which suggests the potential level of image details that could be photoacoustically provided in clinical settings in the future.

Abstract:Endoscopic ultrasound (EUS) is now well established as an important tool in clinical practice. From purely diagnostic imaging, it has progressed to include tissue acquisition, which provided the basis for therapeutic procedures. Even as interventional EUS developed, there has been ongoing progress in EUS diagnostic capabilities due to improved imaging systems, better needles for tissue acquisition and development of enhanced imaging functions such as contrast harmonic EUS (CHEUS) and EUS elastography. EUS is well established for differentiation of subepithelial lesions, for T-staging of luminal gastrointestinal and pancreaticobiliary malignancies, for differentiation of benign pancreaticobiliary disorders and for diagnostic tissue acquisition, which can be achieved by EUS-guided fine needle aspiration or by EUS-guided fine needle biopsy using dedicated biopsy needles. This review briefly describes the technique of performing EUS and then discusses its clinical utility in terms of gastrointestinal cancer staging, the evaluation of pancreaticobiliary disorders and tissue acquisition. Enhanced imaging techniques such as CHEUS and EUS elastography are briefly reviewed.
