Abstract:The COVID-19 disease, caused by a coronavirus known as SARS-CoV-2, was declared a pandemic by World Health Organization on March 11, 2020. The Canadian provinces and territories subsequently declared a state of public health emergency (e.g., Alberta on March 17 and Ontario on April 14). Public health disasters such as viral pandemics are known to result in long-lasting deleterious effects on mental health of both health-care workers and the general population. Regarding the general public, those exposed to SARS during 2003 in Hong Kong showed highly elevated symptoms of anxiety during the epidemic that declined to pre-epidemic levels after about 3 months.<a class="ref showRefEvent2" href="#">1</a> Similarly, the epidemic of MERS in Korea during 2015 was accompanied by high levels of anxiety and anger that declined substantially after 4 to 6 months.<a class="ref showRefEvent2" href="#">2</a> A recent report of Czech adults indicates increased levels of mental disorders, including anxiety and major depression, during the COVID-19 pandemic.<a class="ref showRefEvent2" href="#">3</a>
Mental Health Issues Associated With COVID-19 Among the Elderly Population: A Narrative Review

Abstract:Since its discovery in December 2019, coronavirus disease 2019 (COVID-19) has been affecting humanity in economic, social, physical, and psychological manner. During COVID-19, the older population needs special consideration since they have a greater risk of developing serious illnesses. We used a narrative review approach to identify the relevant articles. PubMed, Scopus, and Google Scholar databases were searched for the following keywords: (mental health OR mental illness(es) OR anxiety OR depression OR irritability) AND (elderly OR older people OR aged 60 years or more) AND (Covid-19 OR pandemic OR chronic diseases) AND (mitigate OR manage). In the initial search, we found 948 articles related to our search string, and only 33 studies were included in this narrative review. The results demonstrated that the elderly population is more prone to mental health issues associated with COVID-19. This narrative review also reported that loneliness, stress, depression, anxiety, sleep disturbance, and suicidal ideation symptoms are experienced by the elderly during the pandemic. Our results also demonstrate that interventions, such as community activities through social interactions, and the use of digital technologies could improve the quality of life of older people and help in the mitigation and management of the adverse effects of COVID-19.

Abstract:Older adults faced unique challenges in the pandemic due to their increased vulnerability to coronavirus disease 2019 (COVID-19) and its complications. Pandemic-related restrictions such as physical distancing, stay-at-home orders, lock-down, and mandatory face cover affected older adults in unique ways. Additionally, older adults experienced psychosocial concerns related to discrimination based on ageism and emotional distress from exposure to conflicting messages in the media. They experienced several forms of loss and associated grief and survivor guilt. Pandemic added to their loneliness and social isolation. Furthermore, older adults experienced the fear and anxiety related to COVID and the fear of contracting the disease and dying from it. Pandemic experience included events potential to generate the desire and capability for suicide. Several studies report varying symptoms such as loneliness, anxiety, and depression among older adults during the pandemic. However, during the initial months of the pandemic, there were reports on coping and resilience among this population. The impact of COVID-19 on older adults' mental health may have long-term implications. This narrative review examines the impact of COVID-19 on older adults' mental health and psychosocial wellbeing. Additionally, the review highlights various factors that affected their psychosocial wellbeing during the COVID-19 pandemic.

Abstract:The coronavirus disease 2019 (COVID-19) pandemic presents an unprecedented challenge to public health, with over 233 million confirmed cases and over 4.6 million deaths globally as of September 20211. Although many studies have reported worse mental health outcomes during the early weeks of the pandemic, some sources suggest a gradual decrease in anxiety and depressive symptoms during the lockdown2. It remains to be explained whether mental health continued to deteriorate during the initial lockdown or whether there were signs of stabilization or improvement in the mental health of community-dwelling middle-aged and older adults. Our results showed that adults had twice the odds of depressive symptoms during the pandemic compared with the prepandemic period, with subgroups characterized by lower socioeconomic status and poor health-related factors experiencing a greater impact. Over 43% of adults showed a pattern of moderate or clinically high levels of depressive symptoms at baseline that increased over time. Loneliness and COVID-19 stressors were predictors of worsening depressive symptom trajectories. The disparities and patterns in the depressive symptom trajectories suggest that the negative mental health impacts of the pandemic persist and may worsen over time. Interventions that address the pandemic stressors and alleviate their impact on the mental health of adults are needed.

Abstract:Introduction The novel coronavirus SARS‐CoV2 (COVID‐19) was declared a global pandemic in 2020 with the greatest risk to older adults. Prolonged restrictions and isolation threaten the social and emotional welfare of vulnerable groups with concerns focused on the long‐term impact of this pandemic on the health and well‐being of aging populations. Purpose Using the Socioemotional Selectivity Theory (STT) as a conceptual framework, the purpose of this literature review was to explore the impact of COVID‐19 on the psychological and social well‐being of older adults. Methods Numerous academic and healthcare‐related databases were searched to yield 24 relevant primary research articles, published during the pandemic (2020–2022), for analysis. Results/Conclusions Overall results indicated the pandemic had a significant negative affect on the psychological and social well‐being of older adults to include those with cognitive impairments and dementia despite perceptions of reduced awareness. There were significant differences between age groups with older adults reporting greater emotional stability and coping skills than younger cohorts. Although supportive services and the use of technology‐enhanced well‐being these resources were reduced during the pandemic due to lack of trained staff, funding, and other socioeconomic or political barriers. The SST proposes that feelings of satisfaction, a sense of belonging, and purpose are important for emotional well‐being but the pandemic challenged these goals and, for many, resulted in stress, anxiety, and depression. Clinical Relevance Healthcare providers and policymakers need to be aware of the negative consequences triggered by the prolonged pandemic and take measures to provide services that support the psychosocial well‐being of older adults.

Abstract:Older adults (65 years or older) are prone to various mental health disorders, which require appropriate care. Mental illnesses like depression, dementia and Alzheimer’s disease, among others, increase cognitive deterioration, which leads to dependence on normal activities in life. During pandemics like COVID-19 and Ebola, the mentally ill older adults are highly susceptible to increased morbidity, mortality rates and reduced quality and social life hence require appropriate care. Pandemic occurrence is a concern and worry for the mentally ill older adults since they have an increased vulnerability and are highly dependent on help and care. Moreover, a pandemic like COVID-19 may lead to neurological and mental health complications, which may adversely affect the mentally ill older adults. During the initial stages of the COVID-19 pandemic, older adults were more likely to be infected with the virus and have worse outcomes than their younger counterparts. Similarly, morbidity and mortality rates were highest among older adults who had co-occurring conditions such as hypertension, cardiovascular disease, diabetes, chronic respiratory disease, and chronic kidney disease. Older adults were also susceptible to loneliness and social exclusion. The risk factors that were associated with worsening of physical, psychological, and social well-being among these individuals included age, ageism, isolation, loneliness, lack of security, frailty dependency, sexism, stigma, abuse, and cognitive and sensory impairments. The pandemic also had a significant effect on the mental health of older adults.

Abstract:COVID-19 continues to have detrimental effects worldwide, especially on vulnerable populations. The burden of mental health concerns and psychological well-being resulting from social isolation and loneliness induced by COVID-19 are increasingly recognized in old age. The aim is to determine the extent of social isolation and loneliness among older adults, the methodologies used, the effect on mental health during COVID-19, and review intervention strategies and lifestyle changes to improve the current situation through the recommendations of the studies included. The articles published on PubMed, ProQuest, and Scopus databases from December 2019 to December 2021 with the following keywords ("Older Adults" "Social Isolation," "Loneliness," "Mental Health," "COVID-19") in English were included. Older adults have faced a serious burden of social isolation and adverse mental health effects during COVID-19. Anxiety, depression, stress, and insomnia are the major mental health concerns among older adults worldwide. It is suggested that spousal support and social networks, adaptive organizational change, and a responsive public sector are critical in reducing the obstacles to older adults. COVID-19 resilience among the aging population can be remarkable if adequately cared for with senior-friendly pandemic-related intervention strategies, policies and legislation.

Abstract:Introduction: Older adults are at greater risk for developing severe illness from SARS-COV2 infection and may be more vulnerable to negative mental health outcomes as a result of public health guidelines that increase social isolation. We assessed mental health outcomes in older adults with normal cognition (NC), past history of major depressive disorder (i.e., remitted; rMDD), or mild cognitive impairment (MCI) to determine the prevalence of depression, anxiety, general stress, and post-traumatic stress during the COVID-19 pandemic and the impact of diagnosis and sex. Methods: The sample included 108 older adults (37 males, mean age=72.1 years): 71 older adults with normal cognition (NC) based on normal neuropsychological test performance and no psychiatric history, 21 rMDD participants based on DSM5 criteria, and 16 MCI participants based on NIA-AA criteria. Participants completed self-report measures of depression [Patient Health Questionnaire-9 (PHQ-9)], anxiety [Patient-Reported Outcomes Measurement Information System (PROMIS)], general stress [Perceived Stress Scale (PSS)] and post-traumatic stress [Impact of Events Scale Revised (IES-R)] through video- or teleconferencing. Prevalence rates of clinically significant psychiatric symptoms were expressed as the percentage of participants with total scores that exceed the normal cut-offs. Separate MANOVAs were used to examine the effects of diagnosis and sex. Non-normally distributed data (PHQ-9 and PROMIS total scores) were rank-transformed. Results: Approximately 1/3rd of participants endorsed clinically significant symptoms based on scores exceeding the cut-off for normal: 33.7% on PHQ-9, 31.3% on PROMIS-Anxiety, 35.5% on PSS, 38.3% on IES-R. rMDD participants scored higher on all measures compared to NC participants (p's < .005) while MCI participants scored higher on the PSS compared to NC (p=.035). Women scored higher on all measures compared to men. Conclusions: These rates of approximately 1/3rd reporting clinically significant symptoms of depression, anxiety, general stress, and post-traumatic stress are higher than those described in population surveys of older adults but are comparable to prevalence rates of psychiatric symptoms in the general adult population. Abstract:The COVID-19 pandemic contributed to a severe burden on the physical and mental health of people around the globe. It contributed to fear, uncertainty, stress, isolation, and loneliness amongst the geriatric population. These feelings were often higher in people with mental health disorders. Having a prior history of psychiatric disorders can be a risk factor for increased psychological distress after going through any disaster-related traumatic experience. The present exploratory pilot study aimed to understand the adverse effects of social isolation due to the pandemic on geriatric patients’ mental health and awareness of the available resources provided by both government and private agencies, to cope with stress. The study included a questionnaire that was administered to 50 geriatric patients. Descriptive statistics were conducted using Microsoft Excel. The results showed that 56% of the participants reported that their mental health was affected due to the social isolation from the pandemic. Statistical significance (p<0.05) was observed amongst the participants who reported feeling socially isolated and lonely during the pandemic. Worsening of mood, anxiety, sleep disturbances, loss of interest, appetite changes, weight issues, and concentration difficulties were statistically significant. Statistical significance was also observed for symptom worsening compared to the pre-pandemic period and for the awareness of lack of resources. About 60% of participants reported that they were not aware of the resources to cope with stress during the pandemic. The study findings suggest that the mental health of geriatric patients was adversely affected by the social isolation from the COVID-19 pandemic. It is important to focus on the physical and mental health of vulnerable populations, such as geriatric patients, and to carefully monitor them during difficult times including pandemics and other emergency situations.

Abstract:The coronavirus disease 2019 (COVID-19) caused by the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) has rapidly spread worldwide and has had unprecedented effects in healthcare systems, economies and society. COVID-19 clinical presentation primarily affects the respiratory system causing bilateral pneumonia, but it is increasingly being recognized as a systemic disease, with neurologic manifestations reported in patients with mild symptoms but, most frequently, in those in a severe condition. Elderly individuals are at high risk of developing severe forms of COVID-19 due to factors associated with aging and a higher prevalence of medical comorbidities and, therefore, they are more vulnerable to possible lasting neuropsychiatric and cognitive impairments. Several reports have described insomnia, depressed mood, anxiety, post-traumatic stress disorder and cognitive impairment in a proportion of patients after discharge from the hospital. The potential mechanisms underlying these symptoms are not fully understood but are probably multifactorial, involving direct neurotrophic effect of SARS-CoV-2, consequences of long intensive care unit stays, the use of mechanical ventilation and sedative drugs, brain hypoxia, systemic inflammation, secondary effects of medications used to treat COVID-19 and dysfunction of peripheral organs. Chronic diseases such as dementia are a particular concern not only because they are associated with higher rates of hospitalization and mortality but also because COVID-19 further exacerbates the vulnerability of those with cognitive impairment. In patients with dementia, COVID-19 frequently has an atypical presentation with mental status changes complicating the early identification of cases. COVID-19 has had a dramatical impact in long-term care facilities, where rates of infection and mortality have been very high. Community measures implemented to slow the spread of the virus have forced to social distancing and cancelation of cognitive stimulation programs, which may have contributed to generate loneliness, behavioral symptoms and worsening of cognition in patients with dementia. COVID-19 has impacted the functioning of Memory Clinics, research programs and clinical trials in the Alzheimer's field, triggering the implementation of telemedicine. COVID-19 survivors should be periodically evaluated with comprehensive cognitive and neuropsychiatric assessments, and specific mental health and cognitive rehabilitation programs should be provided for those suffering long-term cognitive and psychiatric sequelae.

Abstract:For almost two years, populations around the globe faced precariousness and uncertainty as a result of the COVID-19 pandemic. Older adults were highly affected by the virus, and the policies meant to protect them have often resulted in ageist stereotypes and discrimination. For example, the public discourse around older adults had a paternalistic tone framing all older adults as “vulnerable”. This study aimed to measure the extent to which perceived age discrimination in the context of the COVID-19 pandemic, as well as the sense of loneliness and social isolation, fear and perception of COVID-19 risks, had a negative effect on older adults’ mental illness. To do so, a self-report questionnaire was administered to 1301 participants (average age: 77.25 years old, SD = 5.46; 56.10% females, 43.90% males). Descriptive and correlational analyses were performed, along with structural equation modelling. Results showed that perceived age discrimination in the context of the COVID-19 pandemic positively predicts loneliness and also indirectly predicts mental illness. In addition, loneliness is the strongest predictor of mental illness together with fear of COVID-19 and social isolation. Such results highlight the importance of implementing public policies and discourses that are non-discriminating, and that favour the inclusion of older people.

[The development of mental health in oldest-old individuals during the COVID-19 pandemic and the role of social support]

Abstract:Background: The elderly population is one of the high-risk groups with regard to a severe course of disease and increased mortality when infected with the coronavirus SARS-CoV‑2 (Severe Acute Raspiratory Syndrom Coronavirus 2). This group may be at higher risk for psychological strains from the COVID-19 pandemic itself but also from the health protection measures. The aim is to examine how symptoms of depressiveness, anxiety, and somatization change over the course of the pandemic and which role social support plays in that. Methods: Using two written surveys of n = 156 elderly participants in the periods May to June 2020 and March to May 2021, sociodemographic data, factors of psychological strain (depressiveness, anxiety, and somatization), as well as the perceived social support were recorded. The mean age of the respondents was 87.20 years (SD = 4.65; age range = 77.68-96.75 years) and 88.03 years (SD = 4.63; age range = 78.52-97.62 years) for 2020 and 2021, respectively. Data were analyzed using Wilcoxon t‑tests and generalized linear regression models. Results: A significant increase in the expression of psychological strain with regards to depressiveness, anxiety, and somatization can be identified. Higher scores of psychological strains in 2020 are associated with a higher psychological strain in 2021. Higher perceived social support in 2020 is associated with lower depressiveness one year later. Conclusion: An increase in psychological strain has been observed in the elderly population over the course of the COVID-19 pandemic until May 2021. This population should be supported by preventive programs to avert a further increase in symptoms. The expansion of social support could be useful, especially in the prevention of depressive symptoms.

Severity and symptoms of depression in older adults presenting to mental health services during COVID‐19 lockdown – an experience from south London region

Abstract:Background Social restrictive measures during COVID‐19 pandemic has influenced people's mental health conditions across different countries. Global studies indicated an increased prevalence of anxiety disorder and depression with varied impacts among different age groups. Most studies in the elderly have been done via surveys or internet medias missing out on people with no access to digital technology tools. We aimed to investigate the prevalence of depression among elderly people aged 65 years and above who attended mental health service for older adults (MHOA). Methods Retrospective data was collected from the South London and Maudsley NHS Foundation Trust (SLaM) Biomedical Research Centre (BRC) Case Register for MHOA. Using this large mental health care database in South London, United Kingdom, we identified referrals to MHOA during the first UK lockdown (March ‐ July 2020) and compared those to a corresponding pre‐COVID period (February ‐ July 2019). Logistic regression analysis adjusted for age, gender, ethnicity, diagnosis and living conditions were done to examine association between COVID‐19 lockdown and severity and symptoms of depression. Results 536 patients were referred and seen within the MHOA during the first UK COVID‐19 lockdown period compared to 1,455 patients during the pre‐COVID‐19 corresponding period. We found presence of COVID‐19 lockdown had no effect on overall severity of depression but significantly associated with depressive symptoms such as anhedonia (1.64, 95% CI 1.14‐2.37, p = 0.008), lack of energy (1.65. 95% CI 1.65 (1.25‐2.16), p<0.001), tearfulness (1.43, CI% 1.15‐1.78, p = 0.001), agitation (1.32, CI% 1.07‐1.63, p = 0.009), disturbed sleep (1.37, CI% 1.11‐1.70, p = 0.003) and guilt feelings (1.45, CI% 1.04‐2.03, p = 0.030) Conclusions There was an increased prevalence of certain depressive symptoms among older adults within MHOA during first COVID‐19 lockdown which implies impact of isolation and possibly physical ill‐health on mental well‐being. This needs to be explored further for potential prevention and managements.

We aim to investigate the interactive effects of stressors, including social isolation, financial stress, and fear of catching COVID-19, on mental health outcomes. Our study aims to provide insights for the development of prevention and intervention strategies to address the mental health effects of the pandemic by examining the psychological changes attributable to the pandemic and its impact.

Abstract:ObjectivesTo review the impact of social isolation during COVID-19 pandemic on mental and physical health of older people and the recommendations for patients, caregivers and health professionals.DesignNarrative review.SettingNon-institutionalized community-living people.Participants20.069 individuals from ten descriptive cross-sectional papers.MeasurementsArticles since 2019 to 2020 published on Pubmed, Scielo and Google Scholar databases with the following MeSh terms (‘COVID-19’, ‘coronavirus’, ‘aging’, ‘older people’, ‘elderly’, ‘social isolation’ and ‘quarantine’) in English, Spanish or Portuguese were included. The studies not including people over 60 were excluded. Guidelines, recommendations, and update documents from different international organizations related to mental and physical activity were also analysed.Results41 documents have been included in this narrative review, involving a total of 20.069 individuals (58% women), from Asia, Europe and America. 31 articles included recommendations and 10 addressed the impact of social distancing on mental or physical health. The main outcomes reported were anxiety, depression, poor sleep quality and physical inactivity during the isolation period. Cognitive strategies and increasing physical activity levels using apps, online videos, telehealth, are the main international recommendations.ConclusionMental and physical health in older people are negatively affected during the social distancing for COVID-19. Therefore, a multicomponent program with exercise and psychological strategies are highly recommended for this population during the confinement. Future investigations are necessary in this field.

Abstract:Background Throughout the COVID‐19 pandemic, older Canadians were the most at risk of severe physical harm, including death, and their return to post‐COVID life was expected to be especially anxiety‐provoking. A study was conducted to obtain nationally representative evidence of older Canadians' self‐perceived anxiety levels and their strategies to manage or mitigate it as public health restrictions were lifting. Materials and Methods This study had a cross‐sectional descriptive design. An e‐survey was used to collect data from 1327 Canadians aged 60+ stratified by age, sex, and education to resemble the larger general population. Participants completed the Geriatric Anxiety Scale (GAS‐10) and indicated which of the 16 Centre for Addictions and Mental Health's (2022) Coping with Stress and Anxiety strategies they were using to manage or mitigate their anxiety when social distancing was lifting. Descriptive statistics, Pearson correlation, and multiple regression analysis were used to explore the data. Results While, on average, older Canadians were mildly anxious, nearly one‐quarter of responders rated their anxiety as severe. Age, sex, perceived health, were statistically significant correlates of anxiety. Six coping strategies were associated with significantly lower anxiety scores. Moreover, accepting some fear and anxiety as normal, challenging worries and anxious thoughts, and practising relaxation/meditation appeared to increase anxiety. Conclusion Older Canadians used multiple strategies to manage or mitigate their anxiety, and seemingly, with more successful than detrimental tries. Practitioners have much to learn and understand about older people's mental health promotion efforts after social distancing, now and for future pandemics.

Abstract:The coronavirus disease 2019 (COVID-19) pandemic has significantly altered the lives and lifestyles of several older populations in the United Kingdom. It was important to note how it has affected their physical, mental, and social health and well-being during the first wave of the Covid-19 pandemic. To study the impact of the Covid-19 pandemic and the imposed restrictions on the day-to-day lives of the elderly population in the United Kingdom. A review of the published literature on the first wave of the Covid-19 pandemic and its consequences on the older population in the United Kingdom is done. Search engines used for medical databases were Pubmed, Google Scholar, and Internet Explorer. It was found that physical as well as mental well-being was affected in the elderly citizens of the United Kingdom. Mental health studies noted an obvious increase in anxiety and depressive symptoms. Social isolation and reduced access to healthcare services had a deteriorating impact on their social health. Covid-19-related lockdown and pandemic-associated physical, mental, and social well-being effects have been evident in the elderly population in the United Kingdom. The reasons identified for such findings are lack of physical activity, poor social interactions, social isolation, the perceived threat of a pandemic, and poor access to healthcare facilities.
