Abstract:Abstract Because of the exacerbation of population ageing, more family members are involved in the informal care-giving for older adults with disabilities to cater to their long-term care needs. As informal care-givers are associated with the mental health of the care recipient, this study, based on the theoretical framework of the Actor–Partner Effect, utilises a mixed method composed of quantitative and qualitative strategies to identify the relationship among gender and type of both care-giver and care recipient, and mental health of the recipient and explain its internal mechanism. In the quantitative research, ordinary least squares (OLS) regression results based on the basis of the Chinese Longitudinal Healthy Longevity Survey (CLHLS) data corroborate the existence of actor and partner effects within the care-giving dyad. The qualitative analysis further explains the socially constructed differences in gender role, trust relationship and perception towards filial piety of adult care-givers playing their role in the emergence of the Actor–Partner Effect. Finally, this study proposes a developed Actor–Partner Effect analysis framework, advocating to destigmatise disability and construct social support networks for the older adults with disabilities who receive informal care to promote their mental health based on the reflection of socially constructed gender difference.

Abstract:Long-term care (LTC) policy in China is in its infancy, and it is highly decentralised. Where policy structures exist, they are poorly resourced. Although China's demography is still young by developed country standards, it is ageing very rapidly, and by mid-century will have "caught up" with many countries in the developed world with respect to population ageing. LTC policy development, therefore, is becoming a priority in China. We argue that it should be formulated with population ageing as a framework.Policy designs, which take account of and encourage, informal care provision, will be critical to the fiscally sustainable delivery of LTC. In China, informal care is sometimes seen as very scarce because of the one child policy. With only one child, it is argued, there will be less informal care offered than in societies with larger families.This paper uses the recently developed China Health and Retirement Longitudinal Study (CHARLS) dataset to analyse the current patterns of disability and informal care availability. In particular, and contrary to expectation, we find that fertility change is not the main driver for reducing informal care. Education levels, living standards, urbanization and co-residency are much more important. This suggests that current policy, which targets those with one child families, may be misguided, and also that mechanical extrapolations of future demand for care may be misleading.

Abstract:Many countries face policy challenges related to the well-being of their aging populations, and China, with the world's largest aging population, is no different. Over the past 40 years, China has experienced demographic transition toward an “aging society.” According to the National Bureau of Statistics in China (NBSC), the number of people aged 60 years and older in China has risen to 185 million, with rural areas aging more rapidly during China's demographic transition (National Bureau of Statistics of China, 2013). In 1982, the proportion of population aged 60 years and older was 7.8% in rural China and 7.1% in urban China. However, by 2011 these proportions had risen to 12.6% and 15.4%, respectively, with a national average of 13.26% (National Bureau of Statistics of China, 2013). Difference between rural and urban areas in the proportion of the population aged 65 years and older in China has shown a similar trend over time. Rapid economic reforms since the 1980s, associated with increased rural-to-urban migration, especially the outflow of rural young population, have resulted in a larger proportion of elderly adults in rural areas (Cai et al ., 2012; National Bureau of Statistics of China, 2013). These same social forces have resulted in relatively fewer people available to take care of the growing population of older adults as well. The “elderly dependency ratio” (the ratio of older adults in China to those in the population of working age) has increased from 8.0% in 1982 to 12.7% in 2012 (National Bureau of Statistics of China, 2013). The economic and social impact on caregiving will be profound.
Burdens and Coping Strategies of Formal Caregivers Caring for the Elderly with Dementia:A Perspective of Resilience

Abstract:As the country with the largest number of older adults with dementia, China is faced with unprecedented challenges in dementia care. In consideration of the complexity, professionality, and high dependency of caring for the elderly with dementia, the burden outweighs the capacity of informal caregivers which leads to the corresponding increment of demand in caring in institutions. While formal caregivers caring for older adults with dementia are exposed to mounting caregiving burdens, the characteristics of resilience composed of “risk factors” and “protective factors” influence the origin of the burden and their coping strategies. The characteristics of caregivers and care recipients and their interactive relationship form the internal burden, while the trust between caregivers and relatives of the elderly as well as the management within institutions are two major elements resulting in the outer burden. Personal characteristics, spiritual beliefs, behavioral strategies, and received family support of formal caregivers together shape their burden adaptation and coping strategies. In the construction of caring for the older adults with dementia in institutions, it is an urgent need to reflect on the “feminization of care”, reconstruct the significance of trust in resilience strategies, and build up a comprehensive social support system for formal caregivers at the same time. Relieving the burden of formal caregivers and responding actively is an important guarantee for stabilizing the team of caregivers and improving the quality of care for the elderly with dementia.

Abstract:Objective: To describe the characteristics of oldest-old Chinese with disability and their adult-child caregivers, and the extent to which these characteristics were associated with caregiver burden. Methods: The study was based on 168 pairs of disabled oldest-old adults and their adult-child caregivers, derived from the Chinese Longitudinal Healthy Longevity Survey. Descriptive analyses of care recipients' and caregivers' characteristics were conducted respectively, in reference to caregiver burden. Statistically significant characteristics identified in these bivariate analyses were then jointly evaluated in multiple linear regression models with caregiver burden as the outcome. Results: Care recipients positive emotion status [(beta =-0.227 (-0.412,-0.042)], multiple chronic disease [(beta = 0.513 (0.081, 0.945)], and caregivers spent more caregiving time [(beta = 0.225 (0.061, 0.389)] were main factors associated with caregiver burden. Conclusion: Adult-children caregivers perceived heavier burden if care recipients had low positive emotions, had multiple chronic diseases, and caregivers spent more time caregiving. Abstract:This paper explores the content and extent of the burden of caregiving for Chinese families in transition. It sets out to understand how Chinese families manage to balance family caregiving responsibilities with employment, the impact of the existing social institutions on family caregiving practices, and the risks that caregivers have to face. Data were collected from a sample of 214 workers from 14 manufacturing companies in an industrialized city in central China in 2013. Analysis revealed that common types of eldercare were assisting with activities of daily living and medical related care; middle aged employed respondents were most likely to be the caregivers to older family members; financial and time demands of care were challenging for caregivers, but women with more education and a secure job responded to the pressure of care giving better than those with less education and insecure jobs. An absence of workplace policies to support family caregivers was reported to create insecure employment conditions among middle aged workers. The findings imply an urgent need for legislative action and workplace policy that support family caregiving in China.

Abstract:Introduction: The impact of intergenerational caregiving on the mental health of providers remains a controversial topic, especially in countries like China where it is prevalent. Given the country's aging population and recent liberalization of the two-child policy, understanding the effects of intergenerational caregiving on the mental health of middle-aged and older adult(s) individuals is crucial. This study aimed to explore the impact of intergenerational caregiving on mental health among middle-aged and older adult(s) individuals. Methods: We analyzed data from the China Health and Aging Tracking Survey (CHARLS) 2013, consisting of 6602 participants finally. Personal information, family structure, financial support, health status, and physical measurements were selected for analysis. Correlation and regression analyses were used for relationships among variables controlling for potential confounding variables. Mental health status was evaluated using the depression self-rating scale. Results: There is a significant positive effect of intergenerational care on the mental health of middle-aged and older adult(s) people. Additionally, we re-profiled intergenerational care variables by considering the number and length of intergenerational caregivers, and found that the effects remained significant. Furthermore, the effects of intergenerational care vary across subgroups based on gender, age, nature of usual residence, marital status, and physical health status. Finally, we identified two mechanisms through which intergenerational caregiving positively affects mental health: intergenerational financial support and intergenerational spiritual support. Discussion: These findings have important implications for policymakers, healthcare professionals, and family members in promoting the mental health of middle-aged and older adult(s) individuals in China.

Abstract:The International Journal of Aging and Human Development, Ahead of Print. The rate of usage of community elderly care services in China is low, and past studies about the effects of these services on caregiver burden domestically are few. This study used a large sample of Beijing census data (n = 55,634) to examine the impact of these services on caregiver burden. Logistic regression and propensity score matching were used to estimate the effects. The results showed that meal assistance, respite care, and spiritual comfort are all significantly associated with a lower likelihood of perceived emotional burden among caregivers, while only respite care is very significantly associated with a lower likelihood of feeling physically burdened. However, the effects of these services on caregiver burden also depend on the activities of daily living performance and location of residence of the care recipients. The practical implications for policy makers are discussed.

Abstract:Abstract With acceleration of the ageing population globally, more and more governments are concerned about the potential increase in elder abuse and neglect (EA/N). Recently the National Office for Ageing and the provincial offices for ageing conducted a survey of 224,352 Chinese over the age of 60 years using household interviews to assess economy, health, service, social participation, culture, rights protection, livable environment, etc. Author’s analysis of this data shows that 54% of the elderly people interviewed experienced physical and mental abuse or intimidation, and 6.95% of them felt that their legal rights were violated. Data also supports that the occurrence of EA/N was significantly correlated to self-care ability, economic status, and urban and rural regions of the elders. The researcher will discuss the practice and policy implications for the prevention of EA/N.
The Role and Wellbeing of Female Family Caregivers in the Provision of Aged Care in China

Abstract:There has been inadequate understanding in the literature of the role of family members in providing aged care in a population experiencing rapid ageing, and the subsequent effects on caregiving family members. Using the data of the China Health and Nutrition Survey, this study examines the involvement in aged care provision at home of Chinese women and investigates the effect of care provision on the wellbeing of female family caregivers. The results indicate that Chinese women are increasingly and more intensively involved in caring for elderly parents at home. Based on the instrumental variable method, this study also demonstrates that caring for parents/parents-in-law at home exerts a clear negative effect on female family caregivers’ wellbeing, though no effect of caregiving on the life satisfaction of female family caregivers was detected. The study suggests that, although provision of formal aged care has been increasing rapidly in recent years in China, the burden of caring for parents on Chinese families has not been relieved. Comprehensive policy support is needed for family caregivers to enhance their wellbeing and improve conditions in caregiving, given that demand for aged care services—family-based or otherwise—will be overwhelming in the coming decades.

Abstract:Purpose This study aimed to explore the impact of informal care on the health of older adults, providing data to inform the development of informal care systems and social support policies for long‐term care services in developing countries. Methods Based on data from the 2018 Chinese Longitudinal Healthy Longevity Survey (CLHLS), this study employed the Propensity Score Matching (PSM) method to empirically analyze the effects of informal care on older adults' self‐rated health, depression, and anxiety. Results A total of 4790 older adults were included in the study sample, with 3194 receiving informal care and 1596 not receiving informal care. The results indicated that older adults who received informal care reported better self‐rated health and had lower depression and anxiety scores compared to those who did not receive informal care. Among different groups, the positive effects of informal care on physical and mental health were more significant for very old adults, rural residents, and those without a spouse. Informal care had a greater positive impact on the physical health of women and the mental health of men. Conclusion Government agencies should promote effective coordination between informal and formal care through financial support, awareness campaigns, and policy incentives. This will help to accelerate the improvement of the long‐term care system and effectively meet the long‐term care needs of older adults.

Abstract:Our study examines the impact of informal care on healthcare utilization, focusing on caregiver types, urban-rural, and gender differences. Analyzing data from the China Health and Retirement Longitudinal Study and using fixed effects models, we discovered complementary effects between informal care and healthcare. Specifically, spousal care increased inpatient care use, adult child care boosted both inpatient and outpatient use, and dual care from children and spouses showed the most significant impact on healthcare use. The association between informal care and healthcare use varied across gender or urban-rural residence. Our findings highlight the importance of caregivers in accessing healthcare services.

Abstract:Easing the pressure on family caregiving and addressing the shortage of manpower for family caregiving are significant challenges that China faces in responding to its aging population. This study utilizes data from the 2015 and 2020 China Health and Retirement Longitudinal Study (CHARLS) and employs a Difference-in-Difference method to investigate the impact of pilot policies that integration of medical and caregiving for aging individuals. The findings reveal that these pilot programs are successful in reducing the amount of time spent on family caregiving and the financial burden placed on families, effectively relieving the pressure associated with family caregiving. However, the effects of these programs differ depending on the level of disability, household registration, and the geographical location of the participants. Further analysis suggests that these pilot programs achieve these positive outcomes by increasing government investment in health and wellness funds and providing in-family medical and caregiving services.

Abstract:The aim of the current study was to review the current state and characteristics of the elderly population in China in the context of aging, difficulties and challenges faced by older people, and efforts of the current Chinese Government in this area. The process of population aging in China began to accelerate in the late 1970s and has continued to increase at a rate of about 3.2% per year since then. This process took more than 45 years in developed countries, while it took only about 27 years in China, and aging may continue to increase for a long time. China is now moving toward a superannuated society due to declining fertility rates and increasing life expectancy. There is a great need for care due to the high disease burden among older people. However, more than 1 million "families have lost their only child", and this number is increasing annually by about 76,000; moreover, there are a large number of "deficient families [with an injured family member]" in China. These families face greater difficulties due to aging and need to rely on society for more support given the lack of care provided by their children or spouses. The current study has focused on improving the quality of life of older people, helping them achieve healthy aging, and to assist the country in further providing care for the elderly.

Abstract:As a result of rapid socio-economic change, a steep decline in the fertility rate and an ageing population, the care regime in China faces challenges. Using existing data (primarily the National Surveys on Social Support to Older People in Rural and Urban China, conducted by the China Research Centre on Ageing in 2000 and 2006), we examine the change in China's care regime, focusing on the infrastructure (services and financing) and the distribution of provision among four sectors: family, state, community and the market. We show that the care regime in China remains traditional, relying heavily on the family. Although facing challenges, the state is hesitant to assume more responsibility for funding and provision.

Abstract:Background: The rapid population aging in China, characterized by a higher prevalence of illnesses, earlier onset of diseases, and longer durations of living with ailments, substantially engenders challenges within the domain of older adults' healthcare. Community home-based elderly care services (CHECS) are a feasible solution to solve the problem of older adults' care and protect older adults' health. The aim of this study is to investigate the relationship, heterogeneity effects and influential mechanisms between older adults' use of CHECS and their self- reported health. Methods: The study employs the Instrumental Variable technique and empirically investigates the relationship, heterogeneity effects and influential mechanisms between older adults using CHECS and their self-reported health using data from the China Longitudinal Aging Social Survey from 2018. Results: The findings indicate, firstly, that using CHECS considerably improves older adults' self-reported health. Secondly, the heterogeneity test reveals that the effect is more pronounced for older adults who are under the age of 80, have functional disabilities, are free of chronic diseases, have never attended school, reside in lower-income households, are single, rarely interact with their children, and live in central urban or city/county regions. Thirdly, the mechanism test reveals that the "social network effect" and "family care effect" are the key influence channels of using CHECS. Conclusion: An empirical foundation for the policy reform of community home-based care for seniors is provided by this study with the limitations to discuss the other socioeconomic aspects such as government health expenditure and discuss the specific services aspects such as health care. The findings carry substantial implications for improving the health of older individuals and provide suggestions for establishing a socialized aged care system in China.

Abstract:Abstract Over the years, the Chinese elderly care policy has undergone significant changes, leading to the systematic development of a comprehensive elderly care service system. This study explores the pivotal policies formulated by central policy-making bodies and examines the historical and current state of the elderly care system in China. The findings demonstrate a consistent alignment between the system’s evolutionary trajectory, the practical expectations of the population, and the Communist Party of China’s (CPC) objective of fostering a unified and balanced society. The study also highlights inherent issues concerning governance mechanisms and role-position within the system. Notably, we highlight the successful protection of older adults during the COVID-19 pandemic as an exemplary care of care service provision and draw lessons from international experience.

Abstract:SAGE Open, Volume 13, Issue 4, October-December 2023. The economic and social transformation of rural areas, coupled with the decline of family-based elderly care and increasing health risks, necessitates in-depth research on the effectiveness of informal care for rural elderly adults. Utilizing 10-year, four-period follow-up data from the Chinese Longitudinal Healthy Longevity Survey (CLHLS) between 2008 and 2018, this study empirically examines the changing trends and heterogeneity of informal care. In addition, this paper identifies factors impacting on the satisfaction of rural informal care services, providing a comprehensive understanding of the effectiveness of these services for different segments of the rural elderly adults. The results indicate that the changing trends of informal care for rural elderly adults follows a differential pattern, with over 60% relying on their children, and 7.79% to 13.27% on their spouses. Only a small fraction of elderly individuals relies on institutional care and market-oriented care, with percentages below 2% and 3%, respectively. While over 95% of rural elderly adults express satisfaction with their informal care services, less than 50% are fully satisfied, and the proportion of dissatisfied elderly adults is increasing annually. Key variables impacting on the effectiveness of informal care services include marital status, health status, living arrangement, care intensity, care expenditures, pension insurance, and daily community care support. Policy options such as financial support for informal care, enhancement of the old-age security system, and the establishment socialized formal care can help alleviate the burden of informal care for rural elderly adults.
