Abstract:The aim of the present study was to identify a novel strategy that predicts the metastatic status of lymph nodes (LNs) in patients diagnosed with colorectal cancer, using detailed characteristics of contrast-enhanced CT scan images. A total of 284 preoperative CT scans derived from patients diagnosed with colorectal cancer at Second Affiliated Hospital, Zhejiang University School of Medicine between January 2013 and July 2018 were retrospectively reviewed. A total of 794 LNs were assessed for size, margins, morphology and subtle internal enhancements in the equilibrium phase. Imaging features were analyzed by two abdominal radiologists (Department of Radiology, Second Affiliated Hospital, Zhejiang University School of Medicine and Departments of Radiology; Shaoxing Second Hospital Departments of Radiology, Shaoxing Second Hospital) in a blind manner. If the conclusions were not concordant, the final score was determined by a senior radiologist who specialized in abdominal radiology for ≥30 years. According to the histopathology results, 27.3% (217/794) of LNs were metastatic (LN+). In addition, LNs >10 mm in size demonstrated sensitivity, specificity, positive predictive values (PPVs) and negative predictive values (NPVs) of 47.0, 80.9, 48.1 and 80.2%, respectively [odds ratio (OR), 3.77; 95% confidence interval (CI), 2.69-5.28]. LNs in the shape of a kidney bean (middle fat depression like kidney) and/or those with an oblong shape were more likely to be metastasis negative LNs (LN-), while lobulated and irregular LNs were more likely to be LN+. In magnified images, internal enhancement characteristics of LN- were defined as homogeneous, spotted, striped and core enhancing. By contrast, rim and heterogeneity enhancement features for LN+ demonstrated sensitivity, specificity, PPVs and NPVs of 46.5, 89.9, 63.5 and 81.7%, respectively (OR, 7.79; 95% CI, 5.33-11.40). The results demonstrated that the internal enhancement features of LNs may be used as a predictor of metastasis. The detailed benign characteristics, such as homogeneity, spotted, striped and core enhancement of LNs may facilitate the identification of LN- in patients with colorectal cancer.

Abstract:OBJECTIVE:To assess the diagnostic value of dual energy CT for lymph node metastasis in patients with non-small cell lung cancer (NSCLC).METHODS:Forty NSCLC patients, including 15 cases of squamous cell carcinoma and 25 cases of adenocarcinoma, underwent dual energy CT examination in pre-contrast and venous phase contrast scans, then the CT attenuation value of the lung cancer lesions and 85 mediastinal enlarged lymph nodes (the short diameter ≥ 5 mm, 53 metastatic and 32 non-metastatic) were measured at different energy levels (40-190 keV, spacing 10 keV) in venous phase contrast. CT spectral curves of the lung cancer lesions, hilus pulmonis and mediastinal enlarged lymph nodes were produced automatically, through comparing their CT spectral curves slope to judge whether or not the lymph nodes were metastatic. Receiver operating characteristic (ROC) curve was used to evaluate the efficiency of CT spectral curve in diagnosis of lymph node metastasis.RESULTS:The CT spectral curves slopes of the lung cancer, metastatic lymph nodes and non-metastatic lymph nodes were 1.10±0.11, 1.08±0.07 and 1.54±0.17, respectively. There was no significant difference in curve slope between metastatic lymph nodes and lung cancer (t=-1.32,P>0.05); while there was significant difference between non-metastatic lymph nodes and lung cancer (t=-2.58,P<0.05). The CT spectral curve slope ratios of metastatic and non-metastatic lymph nodes to lung cancer were 0.98±0.05 and 1.40±0.12, respectively (t=-2.86,P<0.05). ROC curve showed that taking CT spectral curve slope ratio of 1.15 as cut-off value for the diagnosis of metastatic lymph nodes, the sensitivity, specificity, positive predictive value, negative predictive value and accuracy were 81.1%, 87.5%, 91.5%, 73.7% and 83.5%, respectively.CONCLUSIONS:Dual energy CT is of value in improving the diagnostic accuracy of lymph node metastasis in NSCLC patients before treatment.

Abstract:Objective To evaluate the value of energy spectrum CT with multimodal quantitative parameters in diagnosis of the metastatic lymph nodes of gastric adenocarcinoma compared with the conventional CT.Methods 37 patients with gastric adenocarcinoma diagnosed by gastroscopy underwent enhanced CT scan with gemstone spectral imaging mode.We positioned and marked the stomach lymph nodes on CT images and compared with postoperative pathologic results one to one.ROC was used to identify the critical quantitative index of energy spectrum CT in distinguishing the metastasis from benign lymph node,and to evaluate the diagnostic effect compared with CT features.Results The 40-80 keV single energy spectrum curve slope (λHU )and normalized iodine concentration (NIC)in metastasis group were lower than those in the benign one in the arterial phase with statistical differences (P <0.05 ),but without statistical differences in venous phase.The combination of arterialλHU and NIC showed significant advantages compared with CT features(P <0.01), with the sensitivity,specificity and accuracy of 84.3% vs 61.4 %,88.4% vs 81.3 % and 86.8% vs 73.6 %.Conclusion The energy spectrum CT can provide quantitative analysis for differential diagnosis of gastric cancer metastasis from benign lymph nodes,and significantly improve the diagnostic accuracy of lymph node metastasis of the cancer.
Spectral CT for preoperative diagnosis of N2 station lymph node metastasis in solid T1 non-small cell lung cancer

Abstract:Purpose To evaluate the diagnostic value of spectral CT for the preoperative diagnosis of N2 station lymph nodes metastasis in solid T1 non-small cell lung cancer (NSCLC). Method For this retrospective study, dual-phase contrast agent-enhanced CT was performed in patients with NSCLC from September 2019 to June 2023. Quantitative spectral CT parameters measurements were performed by 2 radiologists independently. Logistic regression analysis and Delong test were performed. Results 60 NSCLC patients (mean age, 62.85 years ± 8.49, 44men) were evaluated. A total of 121 lymph nodes (38 with metastasis) were enrolled. There was no significant difference in the slope of the spectral Hounsfield unit curve ( λ Hu ) on arterial phase (AP) or venous phase (VP) between primary lesions and metastatic lymph nodes ( P > 0.05), but significant difference in VP λ Hu between primary lesions and non-metastatic lymph nodes ( P < 0.001). The CT 40KeV , λ Hu , normalized iodine concentration (nIC), normalized effective atomic number (nZ eff ) measured during both AP and VP were lower in metastatic lymph nodes than in non-metastatic lymph nodes (all P < 0.05). Short-axis diameter (S) of metastatic lymph nodes was higher than non-metastatic lymph nodes ( P < 0.001). Area under the curve (AUC) for S performed the highest (0.788) in diagnosing metastatic lymph nodes. When combined with VP λ Hu , VP nZ eff , AUC increased to 0.871. Conclusion Spectral CT is a complementary means for the preoperative diagnosis of N2 station lymph nodes metastasis in solid T1 NSCLC. The combined parameters have higher diagnostic efficiency.

Abstract:Background Traditional computed tomography (CT) can predict the lymph node metastasis of gastric cancers with moderate accuracy; however, investigation of spectral CT imaging in this field is still limited. Purpose To explore the application of spectral CT imaging in evaluating lymph node metastasis in patients with gastric cancers. Material and Methods Twenty-four patients with gastric cancers prospectively underwent spectral CT imaging in the arterial phase. The short and long diameters, material concentrations, and CT values were measured and compared between lymph nodes with and without metastasis. The diagnostic performance of the CT index in identifying metastatic lymph nodes was analyzed with receiver operating characteristic (ROC) analysis. Results A total of 102 lymph nodes (77 metastatic, 25 non-metastatic) were detected on spectral CT imaging with the reference of postoperative pathologic exanimation. The short and long diameters, water/fat concentrations, CT value, and ratio between lymph nodes vs. tumors of metastatic lymph nodes were significantly higher than those of non-metastatic ones (all P < 0.05). With a cut-off of 0.785, the CT ratio of lymph node/tumor on 70-keV monochromatic images yielded an accuracy of 81.4% in differentiating lymph nodes with and without metastasis. Conclusion Spectral CT imaging detects lymph nodes more clearly, and the CT ratio of lymph node/tumor on 70-keV monochromatic images holds great potential in differentiating lymph nodes with and without metastasis, which is more accurate than size measurement.

Abstract:BACKGROUND: Lymph node (LN) metastasis is an important factor affecting the treatment of lung cancer. The purpose of this article was to investigate the benefits of dual-layer spectral detector computed tomography (SDCT) for the evaluation of metastatic LNs in lung cancer.METHODS: Data from 93 patients with lung cancer who underwent dual-phase enhanced scanning with SDCT were retrospectively analyzed. According to the pathological findings, 166 LNs were grouped as metastatic (n=80) or non-metastatic (n=86). LNs in station 4 (n=80) and station 7 (n=35) accounted for the majority of the LNs (approximately 69.23%). The short-axis diameter of the LN, arterial enhancement fraction (AEF), normalized iodine concentration (NIC), and the slope of the spectral Hounsfield unit curve (λHU) during the arterial phase (AP) and venous phase (VP) were measured. The Mann-Whitney U test was used to statistically compare these quantitative parameters. Receiver operating characteristic (ROC) curves were plotted to identify the cutoff values, and decision curve analysis (DCA) was performed to determine the net benefit of each parameter. The diagnostic performance, obtained by combining the short-axis diameter with each of the above parameters, was also studied.RESULTS: The short-axis LN diameter, AEF, NIC, and λHU during the AP and VP all showed significant differences between the metastatic and non-metastatic groups (P<0.05). Of the parameters, the AEF had the greatest diagnostic efficiency for metastatic LNs [area under the ROC curve (AUC)AEF =0.885] with a threshold of 86.40%. The sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV), and 95% confidence interval were 90.00%, 89.53%, 88.89%, 90.59%, and 0.830-0.944, respectively. When the quantitative parameters were combined with the short-axis diameter, the AUCs of the parameters, except the AEF, were significantly improved (P<0.05).CONCLUSIONS: The iodine quantitative parameters from SDCT, such as the AEF, demonstrated high diagnostic performances in the differentiation of metastatic and non-metastatic LNs.

Abstract:To develop a clinical-radiomics nomogram based on spectral CT multi-parameter images for predicting lymph node metastasis in colorectal cancer. A total of 76 patients with colorectal cancer and 156 lymph nodes were included. The clinical data of the patients were collected, including gender, age, tumor location and size, preoperative tumor markers, etc. Three sets of conventional images in the arterial, venous, and delayed phases were obtained, and six sets of spectral images were reconstructed using the arterial phase spectral data, including virtual monoenergetic images (40 keV, 70 keV, 100 keV), iodine density maps, iodine no water maps, and virtual non-contrast images. Radiomics features of lymph nodes were extracted from the above images, respectively. Univariate analysis and least absolute shrinkage and selection operator (LASSO) regression were used to select features. A clinical model was constructed based on age and carcinoembryonic antigen (CEA) levels. The radiomics features selected were used to generate a composed radiomics signature (Com-RS). A nomogram was developed using age, CEA, and the Com-RS. The models' prediction efficiency, calibration, and clinical application value were evaluated by the area under the receiver operating characteristic curve (AUC), calibration curve, and decision curve analysis, respectively. The nomogram outperforms the clinical model and the Com-RS (AUC = 0.879, 0.824). It is well calibrated and has great clinical application value. This study developed a clinical-radiomics nomogram based on spectral CT multi-parameter images, which can be used as an effective tool for preoperative personalized prediction of lymph node metastasis in colorectal cancer.

Application of Spectral CT in the Differential Diagnosis of Benign and Malignant Mediastinal Lymph Nodes

Abstract:Objective To compare the differences of the spectral CT parameters between benign and malignant mediastina lymph nodes and to investigate the value of spectral imaging in the differential diagnosis. Methods According to the follow-up and pathology results, a total of 73 patients were enrolled in this study. One experienced radiologist determined the locations of lymph nodes on CT images corresponding to the surgical pathological ones. A total of 47 malignant nodes and 65 benign nodes were found. The longest and shortest diameters of lymph nodes in mediastina window were measured .The vHU, IC, WC, Zef and the CT values of normal lung, normal muscle and lymph nodes at 40, 70 and 140 keV were measured. The indices were compared between the benign nodes and malignant nodes by independent-samples t test. The Spectral HU curves were drawn. Results The long and short diameter of malignant lymph nodes were (28.8 ± 10.5),(20.3 ± 9.1)cm , while they were were (14.0 ± 4.6),(7.5 ± 1.8)cm for benign lymph nodes. Malignant nodes were significantly larger than benign nodes (t= 6.54, 7.13, respectively, P<0.05), The differences of the CT values at 40, 70 and 140 keV and the vHU, IC, WC, Zef for the normal lung, normal muscle and lymph nodes between the two groups had no statistical significance (P>0.05). The two groups showed similar spectral HU curve slopes (1.86 values 1.73). Conclusion Spectral imaging may not be helpful in the differential diagnosis of malignant and benign lymph nodes.
Manifest and meaning of lymph node metastases in gastric cancer with muit-slice spiral computed tomography

Abstract:Objective To study the manifestation characteristics of spiral computed tomography(SCT)for lymph nodes metastases in gastric cancer,and to provide a appropriate method for judgement of lymph nodes metastases.Methods Analysis the characteristics of spiral computed tomography(SCT) of lymph nodes in 35 gastric cancer patients with or without metastasis,and control study with by pathology.Results A total of 379 lymph nodes(positive 173,negative 206) were detected by SCT and confirmed by pathology in metastasis-positive or metastasis-negative patients.The positive rate with diameter of lymph nodes≥10mm is 62.7%.The positive rate with irregular and uneven potentialize lymph nodes are 96.3% and 89.4%.If the attenuation values,more than or equal to 25 Hu in plain scan or 70 Hu in arterial phase or 80 Hu in venous phase,were used as the threshold to detect the metastasis-positive lymph nodes,the positive rate are 55.7%,56.3%,67.8% respectively.Conclusion SCT is valuable in judging the metastasis in gastric cancer.The application of diameter≥10mm,and remark the shape and the attenuation values can dramatically improve the diagnosis for lymph node metastasis in gastric cancer.
Clinical Study of Multislice Spiral CT Enterography in the Diagnosis of Small Bowel Lymphoma

Abstract:Objective To evaluate the value of double-phase enhanced multislice spiral CT enterography(MSCTE) in the assessment of small bowel lymphoma. Methods Twenty-two patients with pathologically proven small bowel lymphoma were examined by double-phase dynamic enhanced MSCTE scan. The images were processed by using Advantage workstation 4. 2( ADW4. 2). The locations, shape, enhancement pattern .visualization of regional lymphadenopathy and feeding vessels of the lesions were analysed. Results In all 22 cases,the tumors located at duodenum in 4, jejunum in 6,ileum in 12 and terminal ileum in 6. The average CT values increased up to 32 HU in portal venous phase. The tumors appeared as infiltrative ulcer type in 12 cases, aneurysm like dilated in 8 cases and polypoid type in 4 cases. The feeding vessels of the lesions were seen in 6 cases, 15 cases had regional mesenteric lymphadenopathy. The multiple planar reformation images showed feeding vessels in 6 cases. The diagnostic accuracy in localization and qualitative diagnosis of the lesions by MSCTE were 95. 5% and 91% , respectively. Conclusion MSCTE is helpful in the diagnosis and differential diagnosis of lymphoma,which can improve the detective rate of the lesions.

Abstract:Objective To discuss and evaluate the preoperative CT examination in the diagnosis and staging of colorectal carcinoma.Methods The preoperative CT imagings of 60 patients with colorectal carcinoma were analyzed retrospectively by comparing its surgical pathologic findings with Thoeni CT staging and the modiefied Dukes' classification.Results The spiral CT showed the changes of lumen and the wall and the relationship between the lesions and adjacent tissues.It could also detect the invasion of adjacent tissues and distant metastasis.Conclusion The preoperative CT examination is useful in the diagnosis and staging of colorectal carcinoma,and in determining its surgical eligibility and its prognosis.

Abstract:Objective To investigate the value of multi-slice spiral CT (MSCT) in diagnosis of colorectal carcinoma. Methods MSCT images of 42 cases of colorectal carcinoma proved by surgery and pathology were retrospectively analyzed. Multiplanar reconstruction (MPR) in all cases, and curved planar reconstruction (CPR), shaded surface display (SSD), CT virtual colonoscopy (CTVC) and Raysum were performed in some of the cases. MSCT findings were analyzed in comparison with operative and pathologic features. Results The detecting rate and quatitative rate of colorectal carcinomas with MSCT were 100%,respectively.The accurate rates were 82.9%(29/35) and 100%(7/7) in detection of invasion of surrounding structures and metastasis in abdominal organs respectively,the sensitivity and specificity for detetion of lymph node metastasis were 71%(22/31) and 100% with MSCT imaging.Conclusion MSCT scan with multiform reconstruction techniques can make definite diagnosis of colorectal carcinoma, which is helpful for treatment plan.
The Diagnostic Value of Spiral CT Enhanced Scan in Central Type of Lung Cancer

Abstract:Objective To explore the application value of spiral CT (SCT) enhanced scanning in the diagnosis of central lung cancer.Methods Eighty-four cases who were pathologically confirmed central lung cancer from December 2013 to December 2015 in our hospital were chosen as the object of study, and they were divided into four groups according to tumor stage. Except for a few patients examined by plain scan, the rest had spiral CT enhanced scan, the results of it were compared to and pathologic diagnosis after conventional surgery.Results Spiral CT can effectively distinguish central lung mass from atelectasis, while the coincidence rate of surgical pathology in each group was 83% ,81%, 80% and 71.4%, and total coincidence rate was 78.3%. The misdiagnosed cases are all patients who did not receive enhanced CT scan.Conclusion Spiral CT enhanced scan can show hila structures, pleural assault clearly. It has great significance to help the diagnosis of central lung cancer before operation.

Abstract:Purpose: To evaluate the application of plain scan with energy spectral CT in non-small cell lung cancer and inflammatory mass. Methods: One hundred and seven cases of lung mass, which were performed the energy spectrum CT from October 2016 to March 2018, were retrospectively analyzed. These cases included 55 adenocarcinomas, 29 squamous cell carcinomas and 23 inflammatory masses without tuberculosis. Energy spectral analysis software (GSI Viewer) was used to measure the water (calcium) concentration, effective atomic number value and CT value at 40-140 keV (10 keV interval) for each of the three groups. After that, the slopes of K40-90 keV (absolute value) of the energy spectrum curve of these three groups of data were calculated. Finally, statistical analysis was performed to compare the differences of parameters among each group. Results: ① The K40-90 ke V, Eff-z and water (calcium) concentration in the inflammatory group were (1.52±0.13) , (6.82±0.19) , (1065.79±8.58) , respectively; ② The K40-90 kev, Eff-z and water (calcium) in the adenocarcinoma group concentration were (0.67±0.18) , (7.93±0.13) , (1026.04±10.10) , respectively; ③ The K40-90 kev, Eff-z and water (calcium) concentrations in the squamous cell carcinoma group were (0.40±0.15) , (7.38±0.11) , (1053.14±8.82) , respectively. Inflammatory masses had the largest standard deviation value of the slope of energy spectral curve, followed by the adenocarcinoma, and the adenocarcinoma had the smallest one. The standard deviation value of the effective atomic number of adenocarcinoma, squamous cell carcinoma and inflammatory masses were in descending order. The standard deviation value of the water (calcium) concentration of adenocarcinoma, squamous cell carcinoma and inflammatory masses were in ascending order. There were statistical significant differences between each group (P<0.05). Conclusion:These three groups have different quantitative parameters in plain scans of energy spectral CT which have important application value.
Evaluation of Dual Energy Spectral CT in Differentiating Metastatic from Non-Metastatic Lymph Nodes in Rectal Cancer: Initial Experience

Abstract:Objectives: To investigate the value of dual energy spectral CT (DEsCT) imaging in differentiating metastatic from non-metastatic lymph nodes in rectal cancer.Methods: Fifty-five patients with rectal cancer underwent the arterial phase (AP) and portal venous phase (PP) contrast-enhanced DEsCT imaging. The virtual monochromatic images and iodine-based material decomposition images derived from DEsCT imaging were interpreted for lymph nodes (LNs) measurement. The short axis diameter and the normalized iodine concentration (nIC) of metastatic and non-metastatic LNs were measured. The two-sample t test was used to compare the short axis diameters and nIC values of metastatic and non-metastatic LNs. ROC analysis was performed to assess the diagnostic performance.Results: One hundred and fifty two LNs including 92 non-metastatic LNs and 60 metastatic LNs were matched using the radiological-pathological correlation. The mean short axis diameter of metastatic LNs was significantly larger than that of the non-metastatic LNs ( 7.28 +/- 2.28 mm vs. 4.90 +/- 1.64 mm, P < 0.001). The mean nIC value for metastatic LNs was significantly lower than that of non-metastatic LNs (0.24 +/- 0.08 vs. 0.34 +/- 0.21, P=0.001 in AP; 0.47 +/- 0.18 vs. 0.64 +/- 0.17, P < 0.001 in PP). Combining nIC (PP) with the short axis diameter, the overall accuracy could be improved to 82.9%.Conclusions: With the combination of nIC value in PP and conventional size criterion, dual energy spectral imaging may be used to differentiate metastatic from non-metastatic lymph nodes in rectal cancer. Abstract:Purpose:To investigate the value of CT spectral imaging in differential diagnosis of gastrointestinal stromal tumor(GIST) and small bowel lymphoma.Methods:Fifteen patients with GIST and 13 patients with lymphoma confirmed by surgery from Aug.2010 to Sep.2011 underwent dual phase spectral CT imaging.Quantitative values(CT attenuation values of GIST and lymphoma in the monochromatic images on 11 sets of keV levels(40～140keV,10keV increment) and iodine concentration and water concentration in the arterial phase and portal venous phase) were analyzed with t test.Results:For GIST and lymphoma,the iodine concentration was(0.394±0.147)g/L vs.(0.089±0.012)g/L,P=0.000,in arterial phase;the CT values on 40keV to 100keV(with 10 keV step) in arterial phase were(204±122) HU vs(.158±44)HU,(145±78) HU vs.(112±30)HU(,110±50)HU vs.(84±25)HU(,88±34)HU vs(.68±21 HU)(,74±23)HU vs.(58±19)HU,(65±19)HU vs(.51±18)HU and(59±18)HU vs.(46±18) HU respectively;and the CT values on 40keV to 60keV(with 10keV step) in portal venous phase were(225±86) HU vs(.170±45)HU(,156±56)HU vs(.121±34)HU and(116±38)HU vs(.91±27)HU respectively.The iodine concentration was higher in GIST than those in lymphoma.Conclusion:CT spectral imaging features of GIST and small bowel lymphoma are distinctly different.The iodine concentration and lower level keV on the monochromatic images can effectively differentiate them.

Abstract:OBJECTIVE:To investigate the diagnostic and application value of pre-operative 64-slice spiral CT evaluation in operation selection for esophagus cancer.METHODS:Multi-slice computed tomography (MSCT) was conducted in 50 patients with esophageal cancer before operation, using the work station for after-treatment to get CT virtual endoscopy (CTVE), multiplanar reconstruction (MRP), shaded surface display (SSD) and Raysum images, and combined with the transect images to record the preoperative MSCT staging and to predict the operation scheme, and compared with the postoperative pathological staging and the actual operational plan.RESULTS:The diagnostic sensitivity of MSCT for preoperative T staging was 100.0% (50/50), while the N staging was 80.0% (16/20). According to the MSCT prediction, the resection rate of esophageal cancer was 96.0% (48/50). Through the preoperative MSCT evaluation, the accuracy of CT-TNM stage was 90.0%, highly consistent with the pathological TNM stage (Kappa = 0.811, P < 0.05).CONCLUSIONS:MSCT can effectively display the shape, size and position of the tumor, determine the tumor invasion range, lymph node metastasis and distant metastasis, etc., make preoperative evaluation for esophagus cancer patients and provide evidence for clinicians to predict the operation scheme for esophagus cancer.
Priliminary Study of Sentinel Lymph Node Biopsy in Colorectal Cancer and Detection of Micrometastasis in Sentinel Lymph Node

Abstract:Objective:To evaluate the feasibility of sentinel lymph node(SLN) biopsy with carbon nanoparticles(CNP) and 99mTc sulfur colloid(TSC) for colorectal cancer.To evaluate the value of immunohistochemistry(IHC) and quantitative Real Time PCR(QRT-PCR) for detection micrometastasis in SLN.Methods: Twenty-two patients who underwent radical resection of a primary colorectal cancer were enrolled.During operation,SLN mapping was performed.All lymph nodes were stained with hematoxylin eosin(HE) staining.SLNs,when negative by routione HE staining,were further analyzed with CK20 IHC staining.SLN tissue from 20 patients were analyzed with qRT-PCR.Results: SLNs were detected in 20 of 22 patients with a successful detection rate of 90.91%.The number of SLNs ranged from 1 to 4,with a mean value of 1.75±0.97 per case.Of 10 patients with SLNs being negative by HE staining,there were 2 patients detected micrometatastases by CK20 IHC.Five of twenty patients were found high level CK20-mRNA in SLN tissue by qRT-PCR.Conclusions: Combined-agent SLN mapping is a potential diagnostic method for predicting local lymph node metastasis in the patients with CRC.IHC and qRT-PCR can increase detection rate of micrometastasis in SLN.But the clinical value of difference of CK20-mRNA in SLN needs to be further studied.

Abstract:UNLABELLED:The aim of this study was to evaluate the incremental value provided by single-photon emission computed tomography (SPECT)/spiral computed tomography (CT) over SPECT alone for the evaluation of equivocal bone lesions in patients with hepatocellular carcinoma (HCC).PATIENTS, METHODS:This was a retrospective study of 65 patients (61 men and 4 women; mean age, 53.1 ± 10.1 years; age range, 31-78 years) with HCC confirmed by pathology who underwent planar whole body scintigraphy and SPECT/CT. Each lesion was scored visually using a 5-point scale (0, negative; 1, benign; 2, likely benign; 3, likely bone metastasis; 4, bone metastasis). Lesions in which diagnostic confidence was scored as 3 or 4 were categorized into metastases.The final diagnosis of each lesion was based on pathological confirmation or follow-up by whole body scintigraphy and SPECT/spiral CT conducted two or more times. The diagnostic accuracy, sensitivity, specificity, positive predictive value (PPV) and negative predictive value (NPV) of SPECT alone and SPECT/CT were computed.RESULTS:The diagnostic accuracy, sensitivity, specificity, PPV, and NPV of SPECT alone were 78.4, 70.9%, 94.9%, 96.8% and 59.7%, respectively. Moreover, for agreement of SPECT alone and the final results, the kappa value was 0.567 (p < 0.001), which was considered to be moderate agreement. The diagnostic accuracy, sensitivity, specificity, PPV, and NPV of SPECT/CT were 99.2%, 100%, 97.4%, 98.9% and 100 %, respectively. Furthermore, for agreement of SPECT/CT and the final results, the kappa value was 0.981 (p < 0.001), which was considered to be nearly perfect agreement.CONCLUSIONS:SPECT/spiral CT is more accurate and valuable than SPECT alone in the differential diagnosis of equivocal bone lesions in patients with HCC.
