Abstract:OBJECTIVETo evaluate the safety and efficacy of laparoscopic splenectomy (LS) and pericardial devascularization in treatment of portal hypertension due to liver cirrhosis.METHODSTwenty three cases with hepatitis B and schistosoma cirrhosis and portal hypertension underwent LS and paraesophagogastric devascularization performed by one treatment team. Follow-up was conducted for 9 months.RESULTSLS combined with pericardial devascularization was successfully performed on these 20 cases. Three cases were converted to open surgery due to intra-operative bleeding. The mean operative time was 235 min (180 - 350), and mean intra-operative blood loss was 520 ml (200 - 1600 ml). All patients were treated with plasma transfusion, antibiotics, and abdominal drainage post-operatively. Peristalsis of stomach and intestine recovered 24 - 72 hours after operation. The mean hospitalization time was 8.5 days (6 - 17 days). The peri-operative complication included plural effusion in 3 cases and subphrenic abscess in one case, .mild ascites in two cases, and wound dehiscence in one case. No mortality occurred. Rebleeding rare was 0%.CONCLUSIONLS combined with pericardial devascularization is relatively safe and effective in treatment of portal hypertension due to liver cirrhosis. The keys to success include experienced laparoscopic skills, use of harmonic scalpel and careful manipulation.

Abstract:Splenic embolization is a technique that can be used alone or in conjunction with other treatments for the mitigation of portal hypertension and associated physiological effects of portal hypertension. This technique can be used safely when total embolization volume is ~50% and the procedural and periprocedural time periods are covered with antibiotics. In this patient population, partial splenic embolization can decrease the incidence of variceal bleeding, and protection can persist for at least a year. Additionally, liver function tests and serum cell counts can be expected to improve. Although not frequently used as primary therapy for patients with portal hypertension, splenic embolization can often be helpful as an alternative or adjunctive procedure.

Clinical Application of Gastroesophageal Varices Embolization Combined with Partial Splenic Embolization in Portal Hypertension

Abstract:Objective:To investigate the feasibility and efficacy of percutaneous transhepatic embolization of gastroesophageal varices combined with partial splenic embolization in gastroesophageal varices bleeding.Methods:Percutaneous transhepatic embolization of gastroesophageal varices combined with partial splenic embolization was performed in 16 cases.Results:All procedures were successfully done and gastroesophageal varices were occluded effectively in all 16 cases. In the follow up period of 3～12 months, variceal bleeding recurred only in one case in 6 months after embolization.2 cases died of hepatic function failure.Conclusion:The treatment is reasonable,safe,and convenient with few complication.It is an effective treatment for the control of gastroesophageal variceal bleeding.This treatment can replaced the operation of splenectomy plus gastric coronary veins embolization.
Acute Pancreatitis Associated Left-Sided Portal Hypertension with Severe Gastrointestinal Bleeding Treated by Transcatheter Splenic Artery Embolization: a Case Report and Literature Review

Abstract:Left-sided portal hypertension (LSPH) followed by acute pancreatitis is a rare condition with most patients being asymptomatic. In cases where gastrointestinal (GI) bleeding is present, however, the condition is more complicated and the mortality is very high because of the difficulty in diagnosing and selecting optimal treatment. A successfully treated case with severe GI bleeding by transcatheter splenic artery embolization is reported in this article. The patient exhibited severe uncontrollable GI bleeding and was confirmed as gastric varices secondary to LSPH by enhanced computed tomography (CT) scan and CT-angiography. After embolization, the bleeding stopped and stabilized for the entire follow-up period without any severe complications. In conclusion, embolization of the splenic artery is a simple, safe, and effective method of controlling gastric variceal bleeding caused by LSPH in acute pancreatitis.
Splenic embolization in treatment of portal vein occlusion deferring liver transplantation

Abstract:The number of liver transplantations performed in this country is limited to the availability of cadaver liver donors, which are always short of demand. Deterioration of patient's clinical condition during the long wait for a compatible donor usually casts a poor prognosis on the patient. We report a 6-year-old girl who underwent splenic artery embolization as an alternative procedure due to a life-threatening clinical condition while liver graft was not immediately available. She was a case of idiopathic portal hypertension with portal vein occlusion that had resulted in splenomegaly, pancytopenia, gastric and esophageal varices. Living graft transplantation was contraindicated due to portal vein lesion. She was listed as high-urgency for liver transplantation as she developed repeated esophageal variceal bleeding requiring frequent sclerotherapy and admission to the intensive care unit. Prevention of the ongoing worsening clinical complications, improvement of hematological disorders and correction of hypersplenism was achieved right after splenic artery embolization. The patient resumed normal daily life and has been successfully eliminated from the waiting list of liver transplantation. We conclude that this is a safe and effective alternative to splenectomy that might call of further liver transplantation.

Abstract:Portal hypertension (PH) commonly occurs in the setting of underlying liver disease and can precipitate variceal bleeding and ascites among other complications. Traditionally, PH is managed with medication or lifestyle changes with refractory cases treated with transjugular intrahepatic portosystemic shunts or splenectomy. Partial splenic artery embolization (PSE) is an alternative treatment which may reduce PH by decreasing splenic vein inflow into the portal vein. In this article, we review the efficacy of PSE and present illustrative cases with a focus on technical and clinical considerations for embolization. We queried the PubMed database from August 1976 to September 2021 for clinical studies of PSE for the treatment of portal hypertension. For each article, the author, year, title, study design, number of patients, age, sex, liver function, technical considerations, adverse effects, and outcomes were recorded. We also review two successful and unsuccessful cases from our institution. A total of 12 studies were identified with 258 patients undergoing 290 PSE treatments. The mean age of patients was 44.1 years (range: 6–77). Most patients had well-compensated liver disease (Child–Pugh (CP) A; 35.1%) or significant functional compromise (CP-B; 44.4%), but a few had decompensated disease (CP-C; 8.9%). The majority (91.7%) of procedures were performed distally within the splenic artery, and on average, 59.5% (20–100) of the spleen underwent infarction. Common complications included symptoms of postembolization syndrome such as abdominal pain (58.5%) and fever (53.2%) as well as atelectasis (9.0%). Five PSE procedures (1.7%) resulted in death secondary to pleuropneumonia, splenic abscess, sepsis, intracranial hemorrhage, and pulmonary embolism, respectively. PSE is an effective but often underutilized treatment for PH. These benefits have risk for minor complications including postembolization syndrome and major complications such as splenic abscess formation. This article will introduce the reader to important technical and clinical considerations regarding PSE. Abstract:BACKGROUND: Portal hypertension is one of the most important clinical conditions that cause intraoperative intensive hemorrhage in cirrhotic patients undergoing liver transplantation. Pre-transplant portal decompression may reduce the intraoperative bleeding during liver transplantation.METHODS: Splenic artery trunk embolization (SATE) was performed one month prior to liver transplantation. Platelet count, prealbumin, international normalized ratio, and blood flow in the portal vein and hepatic artery were monitored before and one month after SATE. The measurements above were collected on admission and before surgery in the non-SATE patients, who served as controls. We also recorded the intraoperative blood loss, operating time, required transfusion, post-transplant ascites, and complications within three months after operation in all patients.RESULTS: SATE significantly reduced portal venous blood flow, increased hepatic arterial blood flow, normalized platelet count, and improved prealbumin and international normalized ratio in the patients before liver transplantation. Compared to the non-SATE patients, the pre-transplant SATE significantly decreased the operating time, intraoperative bleeding, post-transplant ascites and severe surgical complications.CONCLUSION: Pre-transplant SATE decreases portal pressure, improves liver function reserve, and reduces the surgical risk of liver transplantation effectively in patients with severe portal hypertension.

Splenic artery embolization (SAE) has been increasingly used for treatment of splenomegaly/hypersplenism. However, few studies focused on the severe complications after embolization. In this paper, we reported 16 cases of severe complications after SAE for portal hypertension. Methods: From February 2011 to January 2019, 16 cirrhotic patients were diagnosed with severe complications after embolization. The clinical symptoms, therapies and prognosis were analyzed. Results: In these patients, the splenic embolization volume was between 50% and 70%. Among 16 cases, 7 patients were diagnosed with splenic abscess and/or left subphrenic abscess, suffered from the insistent fever, and eventually underwent the surgical therapy; 6 patients with left refractory pleural effusion, suffered from severe chest distress, and underwent repeated thoracocentesis or thoracic closed drainage; 3 patients with thrombosis in portal vein needed oral warfarin therapy. Conclusion: In cirrhotic patients, the severe complications after SAE are not uncommon after the procedure. It causes more suffering, prolonged hospital stay and more medical cost in these patients. It was debatable whether this technique was an appropriate technique for portal hypertension due to liver cirrhosis, patients contraindicated for the treatment should be carefully identified to avoid the complications.

Abstract:Objective This study was performed to investigate the efficacy of proximal splenic artery embolization using detachable balloons for patients with hypersplenism and portal hypertension. Methods Twelve patients diagnosed with hypersplenism with thrombocytopenia or leukocytopenia caused by portal hypertension were treated by proximal splenic artery embolization with detachable balloons and metallic fibered coils. All patients were followed for up to 6 months. Blood parameters, coagulation factors, and liver function indicators were measured. Enhanced computed tomography and abdominal ultrasonography examinations were also performed in advance to confirm the infarction area and evaluate the changes in spleen size. Results Postoperative angiography demonstrated complete embolization of the proximal splenic artery in all 12 patients. Thrombocyte and leukocyte counts rose significantly in all patients in 2 weeks and stayed significantly higher than those before embolization throughout the 6-month follow-up. The total bilirubin concentration and prothrombin activity recovered significantly and returned to normal levels 6 months later. Computed tomography revealed partial infarction and liquefaction of the splenic parenchyma in nine patients. Conclusions Proximal splenic artery embolization using detachable balloons could be considered a safe and effective therapeutic modality in alleviating hypersplenism secondary to portal hypertension.

Abstract:Objective: To sum up the value that partial splenic embolization(PSE) treats portal hypertension complicated with malignant tumors and other diseases. Methods:The clinical data of 64 cases with portal hypertension and other diseases were analyzed in recent three years. Sixty-four patients included 51males and 13 females with media age of 56 (29~82) years. Fifty-ifve cases with cirrhosis were caused by hepatitis B virus and 6 by hepatitis C virus and 3 by non hepatitis virus. Thrombcyte counts were 11×109/L~ 63×109/L and the average was (36.4±14.5)×109/L. Seven cases had small to moderate amount of hydrothorax and/or ascites. Sixty-three patients suffered from complicated malignant tumors and one had uremia. Splenic artery embolization was used with sponge pieces. Results:The highest thrombcyte counts of all patients were 83×109/L~ 453×109/L and the average was (176.7±80.8)×109/L, the difference was obvious (P<0.001) before and after PSE. The complication of pain occurred in 60 cases, fever in 37 cases, new occurrences of ascites in 6 cases, and ascites became serious in 4 cases. All of the symptoms disappeared after treatment. Sixty-three cases with malignant tumors were treated with radiotherapy and chemotherapy-based anti-cancer therapy after thromobcyte count rising. Catheter was successfully placed in one patient with uremic so that hemodialysis could be maintained. Conclusion:PSE can improve thrombocytopenia and effusion resulting from portal hypertension, which makes the malignant tumors and other diseases be treated.
Treatment of Transcatheter Arterial Embolization in a Patient with Arterioportal Shunt

Abstract:A 68-year-old female was admitted to the hospital because of abdominal distention for 20 days and upper right abdominal pain for a week. The symptoms started without any known causes and no HBV infection/alcohol abuse history. Abdominal ultrasound showed splenomegaly and large volume of ascites while abdominal CT scan showed liver cirrhosis, splenomegaly, esophageal and gastric fundus varices, ascites, cholecystolithiasis, right-side pneumonia, and heart enlargement. Ascites fluid examination confirmed transudate (SAAG 29.2 g/L >11 g/L). Ascites thus was as result of portal hypertension. The patient responded poorly to diuretics as the abdominal distension was not relieved at all after 1-week diuretics therapy. Furthermore, hepatic artery CTA/portal vein CTV/hepatic veins CTV/vena cava CTV was performed, which showed the diagnosis of arterioportal shunt (APS). The angiography showed portal vein main trunk slightly broaden, no marked filling defect found, crumby high density image located between left and right lobes can be seen in the hepatic artery phase, and nearby portal vein branches are developed. The patient underwent transcatheter arterial embolization (TAE). Successful embolization of abnormal right and middle hepatic artery branches was done and the early display of portal vein branches disappeared. Ultrasound showed significantly decreased ascites 3 days after the surgery compared with when admitted. The patient’s symptoms were remarkably relieved and checked out soon.
Two-step complete splenic artery embolization for the management of symptomatic sinistral portal hypertension

Abstract:Sinistral portal hypertension (SPH) is a rare clinical syndrome. The purpose of this study was to assess the clinical outcomes and safety of splenic artery embolization (SAE) in the treatment of SPH. This retrospective study included 39 SPH patients who underwent SAE treatment between August 2009 and May 2021. The cases had esophageal, gastric, or ectopic varices detected by endoscopy or enhanced CT, with symptoms or signs of upper gastrointestinal (GI) bleeding and/or postprandial fullness. Clinical symptom improvement rate, complications, and symptomatic recurrence rate were observed and analyzed after treatment. All the procedures were performed successfully. Of all patients, 17 received the 2-step complete SAE procedure, 19 received only the first step of the 2-step protocol (i.e., partial splenic embolization [PSE] procedure), and the remaining three received the 1-step complete SAE procedure. After the procedures, the symptoms completely disappeared in all patients, and the main complications were post-embolization syndromes, with 27 patients (69.2%) developing a low-grade fever, 24 (61.5%) developing abdominal pain and 4 (10.3%) developing nausea or mild vomiting. During the prolonged follow-up, varicose veins were gradually reduced as detected by enhanced CT; liver function parameters and platelet count remained in the normal range. Only one patient who underwent PSE treatment developed upper GI rebleeding 7 months after the procedure. Two-step complete SAE is a safe and feasible procedure for the treatment of symptomatic SPH.

Abstract:Abstract Sinistral or left-sided portal hypertension is a localized form of portal hypertension usually due to isolated obstruction of splenic vein. Most commonly, it is secondary to pancreatitis. Rarely this can present as life-threatening gastric variceal bleeding. In such patients, splenectomy is traditionally considered as the treatment of choice to relieve venous hypertension. Unfortunately, a surgical operation may not be safe in most of the patients because of the unfavorable operative field. Splenic artery embolization (SAE) is an effective method, theoretically akin to splenectomy, blocking the direct arterial inflow to the spleen and thereby reducing the outflow venous pressure. The authors demonstrate a case of a 58-year-old man who presented with severe gastric variceal hemorrhage due to sinistral portal hypertension (SPH) secondary to an episode of pancreatitis, which he had 1 month back. He was successfully managed by SAE and remains symptom-free. The authors bring to the fore the potential curability of gastric variceal hemorrhage secondary to SPH using SAE, which is a safe and effective interventional radiologic procedure.

Abstract:It is well known that splenectomy is the standard of care in the management of clinically significant hypersplenism; however, some patients are found to be unacceptably high risk to tolerate open or even laparoscopic surgery. We present a 62-year-old female with significant comorbidities who was declared a very high risk for any open surgical intervention. She underwent splenic artery embolization with remarkable improvement of her platelet count. Her postoperative course was uneventful and the patient was discharged from the hospital on the fifth hospital day.

Among 16 cases, 7 patients were diagnosed with splenic abscess and/or left subphrenic abscess, suffered from the insistent fever, and eventually underwent the surgical therapy; 6 patients with left refractory pleural effusion, suffered from severe chest distress, and underwent repeated thoracocentesis or thoracic closed drainage; 3 patients with thrombosis in portal vein needed oral warfarin therapy. Conclusion: In cirrhotic patients, the severe complications after SAE are not uncommon after the procedure. It causes more suffering, prolonged hospital stay and more medical cost in these patients. It was debatable whether this technique was an appropriate technique for portal hypertension due to liver cirrhosis, patients contraindicated for the treatment should be carefully identified to avoid the complications.
Total Embolization of the Main Splenic Artery As a Supplemental Treatment Modality for Hypersplenism

Abstract:AIM:To study the safety and feasibility of total embolization of the main splenic artery as a supplemental treatment modality for hypersplenism with thrombocytopenia or leukocytopenia accompanying liver cirrhosis.METHODS:Fifteen consecutive patients with hypersplenism due to cirrhosis were enrolled in this study from January 2006 to June 2010. All patients underwent total embolization of the main splenic artery. Clinical symptoms, white blood cell (WBC) and platelet (PLT) counts, splenic volume, and complications of the patients were recorded. The patients were followed up for 1 and 6 mo, and 1, 2, 3 years, respectively, after operation.RESULTS:Total embolization of the main splenic artery was technically successful in all patients. Minor complications occurred in 13 patients after the procedure, but no major complications were found. The WBC and PLT counts were significantly higher and the residual splenic volume was significantly lower 1 and 6 mo, and 1, 2, 3 years after the procedure than before the procedure (P < 0.01). Moreover, the residual splenic volume increased very slowly with the time after embolization. All patients were alive during the follow-up period.CONCLUSION:Total embolization of the main splenic artery is a safe and feasible procedure and may serve as a supplemental treatment modality for hypersplenism with thrombocytopenia or leukocytopenia accompanying liver cirrhosis.
