Abstract:Effective infection prevention and control measures are required in the intensive care units of hospitals, as these areas have a high incidence of nosocomial infections. Adopting proper ventilation pattern that is efficient in pollutant removal is important for controlling the spread of infectious diseases. In this study, the computational fluid dynamics (CFD) simulation approach is used to investigate the airflow pattern, fates of respiratory particles, and patients' intake fraction in a four-bed model intensive care unit under three types of ventilation: impinging jet ventilation and two forms of mixing ventilation. The results show that in contrast to mixing ventilation, the airflow trend of impinging jet ventilation is consistent with the thermal plume of the human body. Accordingly, impinging jet ventilation can efficiently remove pollutants at the ceiling level, thereby reducing cross-infection. Our results also show that fine particles are greatly affected by the ventilation pattern, whereas large particles are primarily removed by surface deposition. The maximum intake fraction of susceptible patients under the three types of ventilation are 8.24 x 10-5 (impinging jet ventilation); 4.5 x 10-4 (ceiling air ventilation; a type of mixing ventilation) and 5.3 x 10-4 (side air ventilation; a type of mixing ventilation). Impinging jet ventilation appears to be the most effective ventilation system for removing particles and reducing susceptible patients' intake fraction under our investigated scenarios, and exhibits great potential as a ventilation method in intensive care units.

Abstract:Indoor and Built Environment, Ahead of Print. The high concentration of viral bioaerosols within the negative pressure isolation wards could pose a challenge to preventing potential cross-infection amongst healthcare workers (HCWs) and patients. Using the Euler–Lagrange methodology, this study numerically simulated the spatial and temporal distribution characteristics of bioaerosols in a typical negative pressure isolation ward as well as determined the interaction of ventilation mode and patient posture on ward ventilation performance. The removal effect of particle groups produced by two respiratory behaviours (breathing and coughing) was quantitatively analyzed, and the effect of exhaust air ratio and air exchange rate on particle distribution was discussed. The results showed that the migration characteristics of bioaerosol particles were sensitive to both the ventilation pattern and patient posture, which showed significant interactions. On this basis, the ventilation pattern with the best ventilation performance was evaluated, showing a particle removal effect of 70–85%. Due to the initial momentum difference, the diffusion behaviour of cough and breath particles was not consistent, but optimizing the airflow distribution near the exhaust outlet could improve their removal efficiency in the meantime. Further studies found that equal exhaust air velocity ratio facilitated the removal of aerosol particles, and an appropriate increase in the air exchange rate could also reduce the particle content.

Abstract:This study, that is practice-based learning in a real hospital construction project, has evaluated the ventilation performance of three strategies in the protection of health care workers and HVAC control for airborne infectious diseases induced by contaminated exhaled air from patients in a negative pressure isolation room. This paper examines air flow path and airborne pollutant distribution by computational fluid dynamics modeling and field measurement. In hospitals, the risk of virus diffusion mainly depends on air flow behavior and changes in direction caused by supply air and exhaust air locations. An improved isolation room ventilation strategy has been suggested, and is found to be the most efficient in removing contaminants based on the observations and simulation results from three ventilation systems. The results show that ventilation systems utilizing the "low-level extraction" technique are very effective at removing pollutants in the human breathing zone. A new clean isolation room ventilation strategy has been developed that employs two exhaust air grilles on the wall behind the bed at low floor level, coupled with a fan filter unit, and is found to have the highest pollutant removal efficiency.

Abstract:Efficient ventilation in hospital airborne isolation rooms is important vis-à-vis decreasing the risk of cross infection and reducing energy consumption. This paper analyses the suitability of using a displacement ventilation strategy in airborne infection isolation rooms, focusing on health care worker exposure to pathogens exhaled by infected patients. The analysis is mainly based on numerical simulation results obtained with the support of a 3-D transient numerical model validated using experimental data. A thermal breathing manikin lying on a bed represents the source patient and another thermal breathing manikin represents the exposed individual standing beside the bed and facing the patient. A radiant wall represents an external wall exposed to solar radiation. The air change efficiency index and contaminant removal effectiveness indices and inhalation by the health care worker of contaminants exhaled by the patient are considered in a typical airborne infection isolation room set up with three air renewal rates (6 h-1, 9 h-1 and 12 h-1), two exhaust opening positions and two health care worker positions. Results show that the radiant wall significantly affects the air flow pattern and contaminant dispersion. The lockup phenomenon occurs at the inhalation height of the standing manikin. Displacement ventilation renews the air of the airborne isolation room and eliminates the exhaled pollutants efficiently, but is at a disadvantage compared to other ventilation strategies when the risk of exposure is taken into account.

Abstract:In recent years, a large number of respiratory infectious diseases (especially COVID-19) have broken out worldwide. Respiratory infectious viruses may be released in the air, resulting in cross-infection between patients and medical workers. Indoor ventilation systems can be adjusted to affect fine particles containing viruses. This study was aimed at performing a series of experiments to evaluate the ventilation performance and assess the exposure of healthcare workers (HW) to virus-laden particles released by patients in a confined experimental chamber. In a typical ward setting, four categories (top supply and exhaust, side supply and exhaust) were evaluated, encompassing 16 different air distribution patterns. The maximum reduction in the cumulative exposure level for HW was 70.8% in ventilation strategy D (upper diffusers on the sidewall supply and lower diffusers on the same sidewall return). The minimum value of the cumulative exposure level for a patient close to the source of the contamination pertained to Strategy E (upper diffusers on the sidewall supply and lower diffusers on the opposite sidewall return). Lateral ventilation strategies can provide significant guidance for ward operation to minimizing the airborne virus contamination. This study can provide a reference for sustainable buildings to construct a healthy indoor environment.

Numerical Investigation of Airborne Infection in Naturally Ventilated Hospital Wards with Central-Corridor Type

Abstract:Natural ventilation is believed to control airborne infection due to high ventilation rates while an undesired flow pattern may cause infection transmission in hospital wards. A computational fluid dynamics simulation was carried out in this study to investigate the impact of airflow pattern on cross infection in a real central-corridor hospital ward with natural ventilation in Nanjing, China. The simulation results demonstrate that the predicted infection risks of the downstream cubicle are up to 10.48% and 11.59% as the index patient is located in the corridor and in the opposite upstream cubicle, respectively. Under this circumstance, the downstream cubicle should be listed on the high-risk list and the central-corridor type is not recommended in a naturally ventilated ward. Measures such as keeping cubicle doors closed should be taken in order to cut off the transmission route. The results not only give direct evidence to strongly support World Health Organization's recommendation but also suggest required amendment of the Chinese standard GB 51039-2014 to improve ventilation arrangement in general hospital wards in China. Our findings are useful for improving the future design of general hospital wards for airborne infection control.

Abstract:The Centers for Disease Control and Prevention has recommended the use of downward ventilation systems in isolation rooms to reduce the risk of cross-infection from airborne transmissible diseases. The expected airflow pattern of a downward ventilation design would supply cooler and slightly heavier clean air from a ceiling diffuser to push down contaminants, which would then be removed via outlets at floor level. A “laminar” (strictly speaking, unidirectional) flow is expected to be produced to avoid flow mixing and thus reduce cross-infection risk. Experiments were carried out in a full-scale experimental hospital ward with a downward ventilation system to investigate the possibility of applying downward ventilation in a general hospital ward. Two life-sized breathing thermal manikins were used to simulate a source patient and a receiving patient. Computation fluid dynamics was also used to investigate the airflow pattern and pollutant dispersion in the test ward. Based on both experimental and numerical results, the laminar airflow pattern was shown to be impossible to achieve due to turbulent flow mixing and flow entrainment into the supply air stream. The thermal plumes produced above people were found to induce flow mixing. We also studied the effects of the locations of the supply and extraction openings on both the flow pattern and pollutant exposure level in the occupied zone. A number of practical recommendations are suggested.

Abstract:This study investigated the effectiveness of integrating desk partitions and personal exhaust ventilation to mitigate airborne infection risks in areas where people sit closely together. Experiments were conducted in a test chamber equipped with different air distribution systems, including two different mixing and one displacement ventilation. N 2 O tracer gas was utilized to study airborne transmission between occupants. Results showed effectiveness of preventive measures varied depending on air distributions. In the absence of preventive measures, mixing ventilation systems exhibited higher infection risk compared to displacement ventilation. After the introduction of physical barriers, N 2 O concentration increased at two measurement points in presence of one of the mixing ventilation systems. In contrast, the other mixing ventilation and displacement ventilation showed a reduction in N 2 O concentration, up to 63% and 43%, respectively, depending on measurement points. Combination of physical barriers and personal exhaust ventilation consistently reduced N 2 O concentration by 34% to 83%, depending on measurement point's location and type of air distribution. Furthermore, higher airflow rates of personal exhaust increased efficiency of proposed strategy in limiting infection risk. This study supports employing physical barriers and personal exhaust ventilation to reduce airborne infection risks. Tailoring preventive measures to specific air distribution system is crucial.

Abstract:Indoor and Built Environment, Ahead of Print. This study used numerical modelling to analyze air velocity, cough particle distribution and infection risks in an isolation room. It investigated air change rates, inlet/outlet vent positioning and assessed various ventilation rates and outlet configurations for reducing infection risks. Quantitative assessments revealed different particle escape timings. In Case 1, smaller particles (2–4 μm) took 8.2 s to escape, while in Case 2, this time extended to 22.7 s. At 48 ACH, there were significant improvements in removing particles of various sizes, particularly those sized 2–4 μm, 16–24 μm and 40–50 μm, reducing the infection risk. The use of the Wells-Riley model highlighted considerable reductions in infection probabilities with higher ACH. Specifically, infection risks were reduced to 5% in Case 1 and 17% in Case 2, underscoring the marked advantage of Case 1 in reducing infection probabilities, particularly for smaller particles. Furthermore, escalated ACH values consistently correlated with decreased infection probabilities across all particle sizes, highlighting the pivotal role of ventilation rates in mitigating infection risks. The study comprehensively investigated the distribution of air velocity, dynamics of cough particles and infection risk associated with different ventilation strategies in isolation rooms.

Full-scale experimental and numerical study of bioaerosol characteristics against cross-infection in a two-bed hospital ward

Abstract:The transmission and deposition of pathogenic bioaerosols and the subsequent contamination of the air and surfaces is well recognized as a potential route of hospital cross-infection. A full-scale experiment using Bacillus subtilis and computational fluid dynamics were utilized to model the bioaerosol characteristics in a two-bed hospital ward with a constant air change rate (12 ACH). The results indicated that the bioaerosol removal efficiency of unilateral downward ventilation was 50% higher than that of bilateral downward ventilation. Additionally, health care workers (HCWs) and nearby patients had lower breathing zone concentrations in the ward with unilateral downward ventilation. Furthermore, a partition played a positive role in protecting patients by reducing the amount of bioaerosol exposure. However, no obvious protective effect was observed with respect to the HCWs. Only 10% of the bioaerosol was deposited on the surfaces in the ward with unilateral downward ventilation, while up to 35% of the bioaerosol was deposited on the surfaces in the ward with bilateral downward ventilation during the 900 s. The main deposition locations of the bioaerosols were near the wall on the same side of the room as the patient's head in all cases. This study could provide scientific evidence for controlling cross-infection in hospital wards, as well as several guidelines for the disinfection of hospital wards.

Abstract:BACKGROUND: Although the merits of ventilating operating theatres and isolation rooms are well known, the clinical benefits derived from ventilating hospital wards and patient rooms are unclear. This is because relatively little research work has been done in the ventilation of these areas compared with that done in operating theatres and isolation rooms. Consequently, there is a paucity of good quality data from which to make important decisions regarding hospital infrastructure. This review evaluates the role of general ward ventilation to assess whether or not it affects the transmission of infection.METHODS: A critical review was undertaken of guidelines in the United Kingdom and United States governing the design of ventilation systems for hospital wards and other multibed rooms. In addition, an analytical computational fluid dynamics (CFD) study was performed to evaluate the effectiveness of various ventilation strategies in removing airborne pathogens from ward spaces.RESULTS: The CFD simulation showed the bioaerosol concentration in the study room to be substantially lower (2467 cfu/m(3)) when air was supplied and extracted through the ceiling compared with other simulated ventilations strategies, which achieved bioaerosol concentrations of 12487 and 10601 cfu/m(3), respectively.CONCLUSIONS: There is a growing body of evidence that the aerial dispersion of some nosocomial pathogens can seed widespread environmental contamination, and that this may be contributing to the spread infection in hospital wards. Acinetobacter spp in particular appear to conform to this model, with numerous outbreaks attributed to aerial dissemination. This suggests that the clinical role of general ward ventilation may have been underestimated and that through improved ward ventilation, it may be possible to reduce environmental contamination and thus reduce nosocomial infection rates.

Abstract:Controlling the spread of respiratory infections in healthcare settings is important to prevent nosocomial infections. To assess the effectiveness of commonly used techniques in the hospital for evaluating the performance of building retrofit to reducing the risk of nosocomial infection, we employed computational fluid dynamics (CFD) simulation, real-time carbon dioxide (CO 2 ) monitoring, microorganism culturing, and microorganism sequencing. These techniques can quantitatively assess the direct and indirect inhalation exposure risk of healthcare workers (HCWs) to infectious respiratory particles (IRPs) exhaled by patients in a hospital lung function room under two ventilation configurations. Originally, the ventilation system provided 2 air changes per hour (ACH) for clean air and 2 ACH for recirculated air. After retrofitting, the ventilation system was modified to increase clean air to 6 ACH and remove recirculated air. However, the assessment techniques yielded conflicting results regarding the percentage differences in exposure risk before and after the retrofit. The findings indicated that the retrofit system reduced indirect inhalation exposure risk, as estimated by real-time CO 2 monitoring and microorganism culture, by over 50.00%. However, the CFD simulation indicated an increase in inhalation exposure risk when 6 ACH of clean air was supplied. These results suggest that removing recirculated air and increasing the air change rate are effective in reducing long-range indirect inhalation exposure in the lung function room. However, they are insufficient to minimize the direct inhalation exposure of patients exhaling IRPs in close proximity. Given the conflicting results obtained from the commonly accepted techniques used in our study, it is necessary to provide a better understanding of infection control associated with the ventilation dilution of IRPs for hospital architects, hospital healthcare workers, and built-environment researchers.

Reducing the exposure risk in hospital wards by applying stratum ventilation system

Abstract:In order to improve the ventilation design for hospital wards and protect the healthcare workers from respiratory infections, stratum ventilation is proposed to reduce the exposure risk. A numerical (CFD) approach is applied to investigate contaminant distribution in a two-bed hospital ward with two patients and one healthcare worker under stratum ventilation, mixing ventilation, downward ventilation and displacement ventilation. Tracer gas (CO2) is also applied to simulate the exhaled and coughed contaminants by patients. The patients are in positions of lying or sitting in bed. The contaminant concentration distributions and contaminant removal effectiveness under the different air distributions are compared. The results show that under stratum ventilation, with two patients breathing, the contaminant concentration in the breathing zone (1.3–1.7 m above the floor) is comparably lower, the contaminant removal effectiveness is relatively higher. The contaminant concentration at different moments after a patient's cough is also compared. The results show that the coughed contaminant is diluted quickly under stratum ventilation, and the high concentration spot is substantially reduced. The results show that stratum ventilation minimises the exposure risk of healthcare workers in hospital wards.

Abstract:Due to the serious global harm caused by the outbreak of various viral infectious diseases, how to improve indoor air quality and contain the spread of infectious bioaerosols has become a popular research subject. Negative pressure isolation ward is a key place to prevent the spread of aerosol particles. However, there is still limited knowledge available regarding airflow patterns and bioaerosol diffusion behavior in the ward, which is not conducive to reducing the risk of cross-infection between health care workers (HCWs) and patients. In addition, ventilation layout and patient posture have important effects on aerosol distribution. In this study, the spatial and temporal characteristics as well as dispersion patterns of bioaerosols under different ventilation patterns in the ward were investigated using the computational fluid dynamics (CFD) technique. It is concluded that changes in the location of droplet release source due to different body positions of the patient have a significant effect on the bioaerosol distribution. After optimizing the layout arrangements of exhaust air, the aerosol concentration in the ward with the patient in both supine and sitting positions is significantly reduced with particle removal efficiencies exceeding 95%, that is, the ventilation performance is improved. Meanwhile, the proportion of aerosol deposition on all surfaces of the ward is decreased, especially the deposition on both the patient's body and the bed is less than 1%, implying that the risk of HCWs being infected through direct contact is reduced.
CO2 Concentration Assessment for Infection Monitoring and Occupancy Analysis in Tanzanian COVID-19 Isolation Centers

Abstract:Monitoring of IAQ is one of the foundations of the preventative actions prompted by the worldwide recognition of COVID-19 transmission. The measurement of CO2 has emerged as one of the most popular, dependable, and easy ways to indirectly evaluate the state of indoor air renewal. Reducing the risk of respiratory diseases transmitted by aerosols is attainable through implementing and validating prevention measures made possible by CO2 control. Isolation centers are like health facilities in that they are linked to IAQ, and the presence of natural ventilation can significantly improve the circulation of fresh air, which speeds up the removal of contaminants. This is true even though healthcare facilities are among the environments with the highest rate of COVID-19 propagation. Our investigation revealed, however, that no substantial critical data on air quality in Tanzanian isolation centers is presently available. The process of metabolic CO2 creation and accumulation within health isolation center cubicles was investigated in this study. Crucially, we suggest comparing settings under various conditions using the indicator ppm/patient. In this research, we experimentally assessed the value of changing a few HVAC system characteristics. We looked at the data to see how well the filtration system worked concerning the submicron particle concentration. Study recommendations for CO2 detectors and ways to reduce infection risk in shared isolation center cubicles are provided. We also show the correlation between particle size and CO2 concentration, the correlation between CO2 concentration derivatives and air volume presented per patient in isolation cubicles, the correlation between patient occupancy and CO2 concentration levels in isolation cubicles, and how to improve air quality by adjusting the patient's bed position. The study also found that for exposure lengths of two to three hours, a typical hospital cubicle with fifty to one hundred people should have an average interior CO2 value of less than 900 ppm. Carers' length of stay in the hospital substantially impacted the permissible CO2 concentration. By establishing a connection between indoor air monitoring and healthcare goals, this study will aid in determining the feasibility of establishing regulations for interior CO2 content depending on occupancy settings, strengthening preventive efforts against COVID-19. In the post-pandemic era, it will be essential to find ways to make health facilities air cleaner so that infectious diseases cannot spread in the future.

Abstract:To better understand the transport of airborne particulate matter (PM) in hospital environments when surge control strategies are implemented, tests were conducted in a recently decommissioned hospital during a one-week period. An aerosol was released within a patient room and concentrations measured in the room and hallway with and without surge control ventilation system modifications. The average hallway protection efficiencies were high (>98%) both for the baseline ventilation configuration and when the ventilation system was modified for whole floor negative pressure, indicating very little PM reached the hallway. During entry/exit events through the patient room door into the hallway, the average minimum hallway protection efficiencies were lower during the modified ventilation operation (93-94%) than for the baseline operation (98-99%). These lower hallway protection efficiencies may be explained by the 52% reduction in the outdoor air ventilation being supplied to the hallway during the modified operation mode. This suggests that patient room doors should remain closed to control PM movement into the hallway. In addition, if there is concern about airborne infection transmission, an anteroom may be used to further reduce the transport of particles from the patient rooms to the hallways of the ward.

Abstract:This study employs computational fluid dynamics (CFD) simulations to evaluate the risk of airborne transmission of COVID-19 in low-ceiling rooms, such as elevator cabins, under mechanical displacement ventilation. The simulations take into account the effects of the human body's thermal environment and respiratory jet dynamics on the transmission of pathogens. The results of the study are used to propose a potential mitigation strategy based on ventilation thermal control to reduce the risk of airborne transmission in these types of enclosed indoor spaces. Our findings demonstrate that as the ventilation rate (Qv) increases, the efficiency of removing airborne particles (Ep) initially increases rapidly, reaches a plateau (Ep,c) at a critical ventilation rate (Qc), and subsequently increases at a slower rate beyond Qc. The Qc for low-ceiling rooms is lower compared to high-ceiling rooms due to the increased interaction between the thermal plume generated by the occupants or infectors and the ventilation. Further analysis of the flow and temperature fields reveals that Ep is closely linked to the thermal stratification fields, as characterized by the thermal interface height, the height of the temperature isosurface, and temperature gradient. The simulations also indicate that the location of infector relative to ventilation inlet/outlet affects Qc and Ep,c with higher Qc and lower Ep,c observed when infector is in a corner due to potential formation of a local hot spot of high infection risk when infector is near the ventilation inlet. In conclusion, based on the simulations, we propose a ventilation thermal control strategy, by increasing the ventilation temperature, to reduce the risk of airborne transmission in low-ceiling rooms. Our findings indicate that the thermal environment plays a critical role in the transmission of airborne diseases confined spaces.

Abstract:Negative pressure isolation wards (NPIWs) play a crucial role in providing care to COVID-19 patients during a pandemic. However, the exhalation of patients releases aerosols containing pathogens, posing a potential infection risk for healthcare workers (HCWs). While robust ventilation systems within NPIWs can help mitigate this risk, they may also lead to increased energy consumption and reduced thermal comfort for patients. Therefore, it is imperative to strike a delicate balance between HCW safety, patient comfort, and energy efficiency when optimizing airflow within NPIWs. This study introduces TARGETING, a novel index designed to assess the efficiency of aerosols removal of ventilation systems in NPIWs. The primary goal of TARGETING is to enhance the precision of ventilation systems, consequently reducing the required air changes per hour (ACH) and minimizing energy consumption. The research delves into the impact of various supply air parameters, including ACH and temperature, as well as different ventilation modes. Through a comprehensive evaluation utilizing an array of metrics such as the Air Diffusion Performance Index (ADPI), Predicted Mean Vote (PMV), TARGETING, infection risk assessment, and energy consumption estimation measured by cooling load (L cool ), this study employs an Analytic Hierarchy Process to assess the aforementioned indices. The findings highlight that air curtain ventilation (ACV) emerges as the most favorable choice, delivering comprehensive benefits, including an infection risk of less than 0.03%, an ADPI score of 90.2%, a PMV close to zero, TARGETING at 1.3, and energy savings ranging from 35.1% to 57.3%.

Abstract:This paper investigates the airflow and pollutant distribution patterns in a “negative pressure” isolation room by means of objective measurement and computational fluid dynamics (CFD) modeling based on three ventilation strategies. An effective ventilation system is crucial to protect doctors, nurses and other health-care workers from patients with infectious disease. In the preliminary study with Strategy 1, the isolation room has two air supply diffusers and two extract grilles mounted on the ceiling. Strategy 2 retains the air supply diffusers in Strategy 1 but relocates the two extract grilles to the wall behind the bed at 0.3m above the floor level. Strategy 3 has the same layout as Strategy 2 except the ceiling diffusers are replaced by supply grilles and relocated closer to the wall behind the bed.The results show that the low-level extraction technique adopted in Strategies 2 and 3 is very effective in removing pollutant at the human breathing zone as compared to extraction at ceiling level in Strategy 1. It is found that Strategy 3 has the best pollutant removal efficiency with the supply air grilles delivering a laminar flow of outside air to the occupant with minimal entrainment of the air in the room. It is observed that the ventilation strategies and furniture layout have great influence on the airflow and pollutant distribution patterns in the isolation room. Design strategies of isolation room are formulated based on the results and observations made in this study.

Abstract:A negative pressure airborne infectious isolation room (AIIR) is the primary healthcare air contamination control system used for the treatment of severe respiratory infectious patients. Effects of the ventilation system configuration and conditions on airflow pattern, aerosol distribution and ventilation performance were investigated using computational fluid dynamics (CFD). The field measurement by SARS-CoV-2 environmental surface test was also conducted. The cycle threshold values from transcription polymerase chain reaction (RT-PCR) method showed inverse relation to the simulated number of particles trapped on the surfaces indicating a good agreement. Modification of the present AIIR to have alignment between air inlet and outlet where the aspect ratio of the air outlet, Width (W): Height (H) = 1:1 (Improved case: IC#1) showed a 78 % reduction of aerosol concentration in healthcare workers (HCWs) zones. Aerosol concentrations were increased when the openings of the air outlet were enlarged. Addition of air outlet led to large swirling air, resulting in more aerosols being trapped and suspended in the air. Results suggested that AIIR with alignment air inlet on the ceiling and air outlet at the wall over the patient's head with W:H = 1:1 be the most suitable configuration to maximize the ventilation performance and minimize exposure risk to aerosolized viral infection for HCWs. Air change rate plays a more important role than the differential pressure on the removal efficiency. The differential pressure value should be at least −2.5 Pa and the air supply rate 12 ACH for effective protection of HCWs in the negative pressure AIIR.
