Abstract:Sir: A positive association between maternal cigarette smoking and nonsyndromic orofacial clefts has been observed in many studies.1,2 No study has ever been performed regarding the relationship between smoking history and orofacial clefts. This study was conducted in northeast China in retrospective fashion. Cigarette smoke exposure was evaluated in 303 case and 453 control mothers in a hospital-based case-control study conducted between 2006 and 2008 in the hospital of the Stomatology College of Harbin Medical University, Harbin, China. We excluded cases with all other associated congenital deformities to eliminate syndromic cases with a high genetic component.3 Clinical examination included classification of the cleft type by a clinical geneticist according to the classification proposed by Spina et al.4 Information on parental smoking exposure was obtained from a questionnaire (http://dir.niehs.nih.gov/direb/norwayclefts/home_norwayclefts.htm).5 Data regarding parental smoking history were obtained for the period from 6 months before pregnancy to conception. Detailed information was elicited about the parent's average number of cigarettes smoked per day. We interviewed mothers of all cases and controls. Unconditional logistic regression was used to estimate odds ratios and 95 percent confidence intervals for the effect of smoking on each case group. Our results showed that parental smoking was associated with a significantly increased risk of orofacial clefts. The association remained statistically significant even after adjusting for potential confounding variables. The study provides the evidence for a dose-response effect between orofacial clefts and tobacco smoke history. For maternal smoking, the magnitude of risk was modest for cleft lip and cleft lip–cleft palate, but not for cleft palate only. Paternal smoking in the period was strongly associated with all subtypes of orofacial clefts. The risk of paternal smoking (one to 10 cigarettes per day) is more critical than maternal smoking. Some uncertain risks in most of the high-exposure groups may be an artifact attributable to the small sample sizes of these groups or may indicate that high levels of exposure are so toxic to the fetus as to result in fetal death (Table 1).Table 1: Crude and Multivariate Adjusted Odds Ratios and 95% Confidence Intervals for Oral Clefts According to Registration in the Period from 6 Months before Pregnancy to ConceptionIn the case-control studies, choice of control infants who constituted a random sample of live births from the same region as the cases excludes the selection bias. Recall bias can lead to spurious inferences. Newborns with other congenital anomalies as controls that have a high genetic component did not significantly affect recall of maternal cigarette exposure on oral clefting.1 The smoking history in the study was determined by the smoking questions posed to each mother to account for parental smoking history during the 6 months before pregnancy when the teratogenic effect of tobacco that poses a risk for developing oral clefts is beginning. In summary, we found a statistically significant risk of parents who smoked during pregnancy having a newborn with orofacial clefts. Although it is modest, the potential risk can impact thousands of children, posing a serious public health concern. Young parents of child-bearing age should be advised to cease tobacco smoking.
