Abstract:Background: The large number of mildly disabled elderly people in China has resulted in the occupation of a large amount of public social resources. They have also imposed a serious social burden on the society. This study predicts that changes in the level coefficients of physical functioning, motor participation, and external environment are associated with quality of life (QoL) among the elderly with mild disabilities in nursing homes. Methods: A simulation model was created using the system dynamics method. Meanwhile, a 50-day simulation was used to predict the subsequent changes in the level of QoL of the elderly. This study was completed in the nursing homes of Chengdu, China, using random sampling and 85 elderly subjects participated to obtain the primary data. The data were analyzed using the Vensim software. Results: The results showed that the level of physical functioning (pain value of 0.241, autonomy of 0.163, and chronic disease of 0.192), the level of motor participation (education value of 0.186, and rehabilitation partner of 0.21), and the level of external environment (medical care value of 0.26, and rehabilitation environment value of 0.184) lead to subsequent changes in the level of QoL of mildly disabled elderly in nursing homes in 30 days. Conclusion: Considering the results of this simulation study, changes in the level of physical functioning, exercise participation, and external environment could forecast the QoL of mildly disabled elderly in the nursing homes of Chania, China. It is necessary to make the required changes in the nursing homes to promote the QoL of these groups.

Abstract:Nursing homes (NHs) are critical facilities for caring frail older adults with around-the-clock formal care and personal assistance. To ensure quality of care for NH residents, an adequate staffing level is of great importance. Current NH staffing practice is mainly based on experience and regulation. The objective of this paper is to investigate the viability of experience-based and regulation-based strategies, as well as alternative staffing strategies to meet the heterogeneous service demand of NH residents at reduced labor cost under various scenarios of census compositions. We propose a predictive analytics integrated computer simulation model to characterize the heterogeneous service demand of NH residents, and further evaluate and identify promising staffing strategies at the facility level. Specifically, we propose a predictive model based on latent survival analysis to characterize diverse length-of-stay (LOS) with multiple discharge dispositions among NH residents. Further, we develop a simulation model with the incorporation of predictive analytics and domain knowledge to characterize the heterogeneous service demand of NH residents on different types of caregivers over time. Based on the simulation model, we develop a graphical user interface for the simulator to evaluate different staffing strategies at the facility level and inform NH administrators about promising strategies. We use real NH data to validate the proposed model and demonstrate its effectiveness. The proposed predictive LOS model considering multiple discharge dispositions exhibits superior prediction performance and offers better staffing decisions at reduced costs than those without the consideration. With the improved modeling fidelity via integrating predictive analytics with computer simulation, the proposed model is flexible to evaluate various staffing strategies using total labor cost as a performance metric, and can identify promising staffing strategies to meet the service demand of NH residents. Promising staffing strategies with the suggested staff-to-resident (SR) ratio can significantly reduce the total labor cost of multiple types of caregivers, as compared to the benchmark strategies, such as the SR ratios based on industrial practice or minimum requirement of state regulation. Moreover, we construct multiple scenarios of different census compositions of NH residents to demonstrate the capability of the proposed model. Our proposed model can facilitate NH staffing decision making to meet the heterogeneous service demand of NH residents at reduced labor costs.
“Medical-and-care Wisdom Linkage” Pension Model Research and Exploration

Abstract:China has become an aging society faced with difficult challenges. However, the country’s endowment system is still in the infant stage, and the pension problem has become one of the major problems that need to be solved promptly. Based on China’s national policies, this paper proposes a “Medical-and-Care Wisdom Linkage” pension system with a large hospital for diagnosis and treatment as the centerpiece. This system can connect institutional pension models, community pension models, and family pension models by means of the Internet. At the same time, the system uses new cloud-based artificial intelligence for diagnosis and rehabilitation. Related practice has proven that using the Internet, advanced artificial intelligence, and other effective comprehensive treatment technologies to solve the current problems of China?s pension industry has important research value. These are also very promising efforts that are in line with national development strategies and relevant policies.

Abstract:The experience of various countries proves that the hierarchical diagnosis and treatment model can lead to reasonable resources allocation and utilization, however, the waste of China''s disordered treatment model has not been estimated.Nowadays, it is very important to establish a hierarchical diagnosis and treatment system during China''s healthcare system reform.Therefore, it is necessary to estimate the benefit the reform may bring.We take Zhejiang Province as an example, and adopted the system dynamics model to build the health delivery system, to emulate the hierarchical diagnosis and treatment model and the disordered model, and to evaluate the benefit.Results showed that if first option at the primary health institutions increased 10%, it could save 0.251 billion yuan, and if increased 15%, it could save 0.39 billion yuan.If the first option at the primary health institution reached 72.35% and the treatment in county reached 90%, the outpatient in tertiary hospital could reduce by 13 million, and those in primary healthcare institutions could increase by 74 million yuan.The direct healthcare cost could reduce 3.016 billion Yuan.

Abstract:With the background of aging, ensuring the deep integration of pension and medical services and effectively integrating pension resources and medical resources are hot issues that must be addressed in the current mode of integrating health and care services for older people. Thus, we use game theory to construct the utility model of resource allocation between pension and medical institutions. We apply this model to explore how pension institutions and medical institutions invest resources into the integration of health and care services, analysis of influencing factors, and conducting incentive mechanism research by using MATLAB 2016b software. Through theoretical deduction and experimental analysis, the following conclusions are drawn. First, the income distribution coefficient of pension institutions is positively correlated with the level of labor input, and its growth rate has a marginal diminishing effect on the level of labor. Second, in early investment, the income distribution coefficient of pension institutions is positively correlated with fixed asset investment regardless of the different effort coefficients between medical institutions and pension institutions. With a high income distribution coefficient, pension institutions are negatively correlated and marginally decrease. Third, in early investment, the income distribution coefficient of pension institutions is positively correlated with medical institutions’ labor input level. When the income distribution coefficient of pension institutions reaches a certain value, it is negatively correlated with the labor input level of medical institutions, thereby showing a marginal diminishing effect.

Abstract:With the rapid growth of the elderly population of China in recent years, the service demands of older Chinese people continue to increase. The increasingly severe situation with respect to the elderly population is an important social problem that China will face for a long time into the future. It is urgent to solve the problem of how to scientifically carry out allocation planning of service resources for the aged and guide the effective supply of service resources. This paper analyzes the factors affecting service resources for the aged, divides China’s service resource supply and demand system into a supply subsystem, a demand subsystem, and a population and economy subsystem. Using system dynamics methods to analyze the causal relationship between variables and the state space method to build a mathematical model and perform simulation analysis, we research the the current situation of China’s service resources supply and demand balance for the aged. In addition, we put forward resource configuration optimization measures for the future allocation of service resources for the aged, providing a practical basis for future decision-making.

Abstract:Objective:The research intended to apply the method of system dynamics(SD)to build a model of total expenditure on health(TEH)in Shanghai and to simulate internal structures and functions as well as dynamic behaviors. Methods:The dynamics model was constructed by the Stella 8.1 computer simulation platform,based on the SD method. Results:Historical data from 2011 to 2013 were analyzed and relative errors between simulated and observed values on TEH,TEH per capita and ratio of TEH to GDP were 1.81%,2.24%and 0.24%,respectively. Simulating results confirmed the SD model was feasible and effective. Conclusion:Based on the SD model,various influence factors on TEH in Shanghai can be considered. Key factors and development trend can be identified by the simulation platform.

Abstract:In China's new healthcare reform, the pilot local governments explore the practice of establishing a new model of hierarchical diagnosis and treatment system.Zhejiang Province has adopted a special policy of "effectively allocating hospital resources and human resources, and efficiently improving primary healthcare institution capability and patient satisfaction"(hereinafter referred to as "double allocation, double improvement"), focusing on the implementation of the 'Healthcare Talents Project', in order to fill a vacancy of human resources in primary healthcare institutions.This paper uses system dynamics modeling and the WISN method of WHO to estimate the gap in physician supply in primary healthcare institutions.After building the system dynamics model of 'Healthcare Talents Project', this paper simulates the influence of the policy on the vacancy of doctors in primary healthcare institutions and analyzes the sensitivity of regulatory factors.The simulation results show that, there are a big gap in physician supply of about 14,000 to build the hierarchical diagnosis and treatment system.The project can gradually increase the number of primary doctors, and the policy may fill the vacancy by 2021.However, if the efficiency of the hospital doctors who give assistance to primary institutions is increased by 10%, the targeted training and recruitment 100% achieve the policy plans and objectives, the project goal may be achieved by 2020.Therefore, this project can effectively adjust the human resources structure quickly and reasonably, and it can be used as reference for the reform of hierarchical diagnosis and treatment system.

Abstract:The study creates a simplified game model to propose a suitable policy to foster a win-win scenario between care institutions and families of the disabled elderly , and to give a reference basis for enhancing the welfare level of the disabled elderly as well as the commercial performance of care institutions. First, we study and synthesise the experiences of contemporary Chinese long-term care pilot cities to offer data for subsequent numerical analysis; second, we create an evolutionary game model to depict the conflicts and evolving patterns of conflicts between the disabled elderly and care facilities in China; and third, we use numerical analysis to investigate the effects of internal factors (cost of care, price of care) and policy assistance (government subsidies). Finally, we utilise numerical analysis to investigate how internal factors (cost of care, price of care) and policy assistance (government subsidy) affect the combination of solutions. The study reveals that (1) the two-sided strategic choices of care institutions and disabled families make the game unstable. (2) The government can influence the choice of care institutions and disabled families by tax rates and subsidies, implying a stability policy. (3) The presence of an evolutionary stabilization strategy implies that government control may have a desirable limit. When government engagement in this market is limited, "professional care, home care" finally becomes the evolutionary stability method.

Abstract:This paper investigates the improvement of the hospitalisation admission process of elective inpatients by delay announcement to free patients from blind and long wait, a phenomenon often observed in top Chinese hospitals. We propose a formal simulation model in which relevant resource requirements during the hospitalisation of a patient are represented by an original multistage treatment model'. Other key components of the formal model include impatience of patients, resource capacities and key performance indicators such as the original concept of waiting time visibility. Through sensitivity analysis of the simulation model, we demonstrate the benefits of delay announcement and the value of our multistage treatment model'.
The Impact of Long-Term Care Policy on the Percentage of Older Adults With Disabilities Cared for by Family Members in China: A System Dynamics Simulation

Abstract:This study examined the impact of current and future long-term care (LTC) policies on the family caregiving burden in China. System dynamics (SD) methodology was used to construct an LTC delivery system model that simulates the demand of LTC, living options, and LTC service use for disabled older adults. The model was based on three policy variables including the proportion of payment from LTC insurance, the growth rate of beds in LTC institutions, and the time to adjusting the capacity of community-based care. Results showed that the percentage of older adults with disabilities cared for by family members was projected to increase from 92.6% in 2015 to 97.8% in 2035, assuming no policy changes; under the mixed policy scenario, this percentage would reduce significantly to 63.8% in 2035. These findings illustrate that changes in LTC policy and delivery system have a significant impact on family care.

Abstract:Objective:To construct a system dynamics model for health professional allocation in public medical institutions of Pudong New Area,and to simulate and forecast the number of health professionals so as to provide evidence for decision making of health human resources allocation and development. Methods:The data from health statistics reports and previous survey of human resources in Pudong New Area were collected,and expert consultation was used to estimate the status of some variables. Vensim DSS was used to construct the system dynamics model for health professional allocation in public medical institutions of Pudong New Area to simulate and forecast. Results:Setting the goal to meet the healthcare demands under the condition of the stable policy,the system dynamics model for health professionals in public medical institutions of Pudong New Area was constructed. It was predicted that the demands for health professionals in community health service centers and public hospitals would increase by 54.08%and 34.36%,respectively in 2020. Conclusion:Health professionals in community health service centers of Pudong New Area would be significantly deficient in 2020 and it was suggested that some encouraging policies be carried out to increase the success rate of recruitment.

Abstract:With the increasingly serious population aging, economic system transformation, and social transformation, the elderly have an increasingly strong demand for the elderly care service industry, but the development of China's elderly care service industry started late, and there are still many problems. For how to build a better elderly care service model, under the current situation of shortage of elderly care resources, community elderly care may become a solution. By analyzing the status quo, existing problems and development trends of home-based care services in Hangzhou's community, and learning from foreign experience, it explores the establishment of a "integrated physical, medical, and nursing care" community health management model for the elderly and provides a perfect solution for the development of Hangzhou's community elderly care services countermeasures and suggestions for mode operation. Based on the evaluation, problem analysis, and empirical research on the current situation of home care services in Hangzhou communities, this paper draws on relatively mature community care practices, proposes to build a community elderly health management model that combines medical care and care, and improves and innovates the overall design and medical care of the elderly service model measures for the health management of the elderly in the integrated community. The experimental results of this study show that, according to the demand for elderly care services, elderly people in need of care account for 86.6%, 79.5%, and 68.4% of the elderly population, especially in the areas of medical care, life care, and housekeeping services.
Transformation of Potential Medical Demand in China: A System Dynamics Simulation Model

Abstract:Objectives: The increasing of potential medical demand in China has threatened the health of the population, the medical equity, accessibility to medical services, and has impeded the development of Chinese health delivery system. This study aims to understand the mechanism of the increasing potential medical demand and find some solutions.Methods: We constructed a system dynamics model to analyze and simulate this problem, to predict the influences of health policies on the actual percentage of patients not seeking medical care (adjusting the quantity structure of hospitals and community health systems (CHSs), adjusting outpatient prices, and adjusting the level of health insurance).Results: Decreasing the number of hospitals, increasing the number of CHSs, and raising the proportion of health insurance compensation would effectively increase the transformation of potential medical demand. But currently, changes of the outpatient prices didn't play a role in the transformation of potential medical demand.Conclusions: Combined with validation analysis and model simulation, we suggest some possible solutions. The main factors causing potential medical demand are accessibility to medical services and proportion of health insurance compensation. Thus, adjusting the number of hospitals and CHSs and increasing the proportion of health insurance compensation should decrease the actual percentage of patients not seeking medical care and accelerate the transformation of potential medical demand, which deserved being concerned in policymaking. Health performance assessment modeling and its application to compact medical communities in China

Abstract:Some regions in China have already implemented capitation payment or capitation budget management for medical insurance funds. However, there remains a shortage of adequate tools and methodologies to accurately quantify differences in population health risks. Therefore, this paper constructs a health performance assessment model that comprises four steps. The first step is to categorize all participants into health risk groups based on whether they have contracted with a family doctor, their age, sex, and the type of consultation. The second step is to categorize health risk groups based on differences in healthcare resource utilization. The third step is to analyze health performance by examining healthcare resource utilization year over year. The fourth step is to apply the assessment results to assist local finance bureaus and medical insurance bureaus in developing incentive schemes. According to cost weights, the health risk groups are split into six classes: insured residents without health care visits, healthy insured person, slightly ill insured patients, ill insured patients, more seriously disease patients, and severely ill insured patients. We evaluate one compact medical community's health performance by examining changes in the proportion of resource usage group size and expense. From 2019 to 2021, both the proportion of patients with severe and ultra-severe diseases and the proportion of costs in the sample increased, according to changes in resource utilization levels. This result indicates that the population's overall health has not improved and that the compact medical community is still primarily focused on treating diseases, with poor implementation of health maintenance measures and minimal improvement in health performance.
A SBM-DEA based performance evaluation and optimization for social organizations participating in community and home-based elderly care services

Abstract:The community and home-based elderly care service system has been proved an effective pattern to mitigate the elderly care dilemma under the background of accelerating aging in China. In particular, the participation of social organizations in community and home-based elderly care service has powerfully fueled the multi-supply of elderly care. As the industry of the elderly care service is in the ascendant, the management lags behind, resulting in the waste of significant social resources. Therefore, performance evaluation is proposed to resolve this problem. However, a systematic framework for evaluating performance of community and home-based elderly care service centers (CECSCs) is absent. To overcome this limitation, the SBM-DEA model is introduced in this paper to evaluate the performance of CECSCs. 186 social organizations in Nanjing were employed as an empirical study to develop the systematic framework for performance evaluation. Through holistic analysis of previous studies and interviews with experts, a systematic framework with 33 indicators of six dimensions (i.e., financial management, hardware facilities, team building, service management, service object and organization construction) was developed. Then, Sensitivity Analysis is used to screen the direction of performance optimization and specific suggestions were put forward for government, industrial associations and CECSCs to implement. The empirical study shows the proposed framework using SBM-DEA and sensitivity analysis is viable for conducting performance evaluation and improvement of CECSCs, which is conducive to the sustainable development of CECSCs.

Abstract:The increasing number of geriatric patients is one of the most important problems for the next years. This kind of patients is often dependent and does not tolerate environment changes so that hospitalization can strongly deteriorate the elderly health state. This paper focuses on two services of the geriatric services: the Short Stay (Acute Care) and Rehabilitative Care (to prepare the return at home). We use Discrete Event Simulation to compare two different configurations (integrated care vs separated) based on two French hospitals case studies. In the integrated configuration both services are located in the same department and in the separated configuration services are independent. We measure the impact of these organizations on the occupancy, admissions, waiting and the total length of stay. We also study the economic impact based on the French funding system taking into account the Diagnosis Related Group of the patient.

Abstract:This research investigates and evaluates China's urban medical care usage for non-working residents using microsimulation techniques. It focuses on modelling medical services usage and simulating medical expenses on hospitalization treatments as well as clinic services for serious illness in an urban area for the period of 2008-2010. A static microsimulation model was created to project the impact of the medical insurance scheme. Four kinds of achievements have been made. For three different scenarios, the model predicted the hospitalization services costs and payments, as well as the balance of the social pool fund and the medical burden on families.
Performance Evaluation of Respite Care Services Through Multi-Agent Based Simulation

Abstract:Caregivers of patients with chronic diseases are undergoing a daily burnout in their lives. Although respite care seems a promising solution, no quantitative analysis has yet been provided to demonstrate its positive impact. In this article, we propose (i) a new model of caregivers' burnout evolution based on Markov chain and machine learning to model health state evolution, and (ii) a multi-agent based simulation approach to describe the burnout evolution of caregivers and the impact of respite structures on the system. Optimal capacity of respite structures is obtained through a design of experiment. Several management strategies are also tested (collaboration between structures, reservation of beds for emergent cases). Key performance indicators considered are quality of service and costs. Results show a positive impact of respite services on both quality of service and costs. The model also show a trade-off between quality of service and costs when bed reservation policies are used.
