Analysis of etiology and prognostic factors of community acquired pneumonia in elderly patients

Abstract:Objective To analyze the etiology and prognostic factors of community acquired pneumonia in elderly patients.Methods Totally 572 patients with CAP from June 2008 to October 2014 were retrospectively analyzed.The patients were divided into survival group (481 cases) and death group (91 cases).The pathogen,vital signs and medical history were compared between the two groups,and the prognostic factors were analyzed.Results In 572 patients,466 patients (81.5%) had single infection,94 patients (16.4%) had mixed infection and no pathogens were detected in 12 patients (2.1%).In survival group,no pathogens were detected in 9 patients (1.9%),not significantly different from death group [3 patients (3.3%)] (P > 0.05).The single infection rate,mixed infection rate,respiration rate,systolic blood pressure,diastolic blood pressure,proportions of unconsciousness,cardiovascular disease,neurological disease,long term immobilization and aspiration were significantly different between the two groups [86.3% (415/481) vs 56.0% (51/91),11.9% (57/481) vs 40.7% (37/91),(22 ±5)times/min vs (28 ±5) times/min,(116 ± 13) mmHg vs (104 ± 10) mmHg,(71 ± 7) mmHgvs (63 ±5) mmHg,6.4% (31/481) vs 17.6% (16/91),7.7% (37/481) vs23.1% (21/91),6.7% (32/481)vs 16.5% (15/91),3.5% (17/481)vs 12.1% (11/91)] (P<0.05);the body temperature,pulse,history of smoking and drinking,digestive system diseases had no significant difference between the two groups (P > 0.05).The gender,body temperature,pulse,smoking and alcohol history,digestive diseases had no significant correlations with prognosis (P > 0.05);age,pathogen,respiratory rate,blood pressure,conscious state,cardiovascular and cerebrovascular diseases,nervous system diseases,long-term bed and aspiration were significant related with prognosis (P < 0.05).Conclusion The clinical features of CAP in elderly patients are complexed;the prognosis depends on different risk factors.
