Abstract:Sjogren's syndrome (SS),named as dry impediment in Chinese medicine,has complex clinical symptoms.It is mostly caused by constitutional insufficiency,emotional disorder and improper diet combining external pathogen attack,resulting in dysfunction and interaction of five zang-organs,overconsumption and distribution dysfunction of yin,blood and fluid-humor,and failing to arrive at all limbs,bones and orifices.The paper elucidates the pathogenesis and diagnostic and therapeutic characteristics of dry impediment,and indicates the disease is related to all five zang-organs (heart,liver,spleen,lung,kidney) while most closely related to kidney,spleen and liver.The pathogenesis of the disease is based on deficiency of kidney yin and essence,and mainly dominated by dysfunction of spleen in transportation and block of liver's free activity.The treatment of the disease,based on pattern differentiation,commonly use therapies of nourishing yin and tonifying kidney,fortifying spleen and tonifying qi,soothing liver and activating blood,clearing lung-heat and moistening dryness,and nourishing heart and tranquilizing mind,meanwhile matched with nourishing blood and engendering fluid,freeing collateral vessels and relieving pain,and clearing heat and eliminating dampness.Among these therapies,nourishing yin,tonifying qi and activating blood are keys in treatment.

Abstract:This article introduces the philosophy of traditional Chinese medicine (TCM) and discusses the concepts of yin-yang balance, qi (energy or life force), and the five elements—wood, fire, earth, metal and water—in relation to health and healing. In addition to qi, an overview of jing (essence) and shen (spirit) are presented in this article. The effects of one’s emotions on health and specific organs are detailed within this article, identifying the emotion and its correlative effect on the organ(s) involved and TCM approaches to harmonizing the effects of these emotions on health.

Abstract:In traditional medical practices, music therapy has proven effective in treating various psychological and physiological ailments. Particularly in Eastern traditions, the Five Elements Music Therapy (FEMT), rooted in traditional Chinese medicine, possesses profound cultural significance and unique therapeutic philosophies. With the rapid advancement of Information Technology and Artificial Intelligence, applying these modern technologies to FEMT could enhance the personalization and cultural relevance of the therapy and potentially improve therapeutic outcomes. In this article, we developed a music therapy system for the first time by applying the theory of the five elements in music therapy to practice. This innovative approach integrates advanced Information Technology and Artificial Intelligence with Five-Element Music Therapy (FEMT) to enhance personalized music therapy practices. As traditional music therapy predominantly follows Western methodologies, the unique aspects of Eastern practices, specifically the Five-Element theory from traditional Chinese medicine, should be considered. This system aims to bridge this gap by utilizing computational technologies to provide a more personalized, culturally relevant, and therapeutically effective music therapy experience.

Abstract:In recent years, music has been regarded as a promising non-pharmacological intervention for a number of physical and mental conditions. Five-elements music therapy—based on the five-element theory—is a unique non-pharmacological therapy of East Asian traditional medicine. It has the potential to effectively provide individualized music therapy to individuals with illness. However, one limitation of this music therapy is that the classification of the five elements and its application is mainly based on subjective judgment. The development of artificial intelligence (AI) has enabled the acoustic analysis of multi-factor sound sources. This can develop five-element music therapy. Here, we discussed the challenges proposed by the future combination of five-element music therapy and AI. Further, we hypothesized that AI may promote its use in the medical field.

Theoretic exploration of music therapy in traditional Chinese medicine based on five-tone theory in Inner Canon of Huangdi

Abstract:Music has been used as a healing force for thousands of years. Records of treating with music in China can be traced back to the Spring and Autumn. Ancient music therapy ideas were presented in Inner Canon of Huangdfiwith a core of five Zang linked to five-tone. Music can improve mood closely related to Zang according to five elements, in which way music brings benefits to health. Due to the lack of adequate records and evidence, the explanation of five-tone treatment by descendants is not objective and accurate. There is much debate on what the nature of five-tone is and how it should be used. Based on a quantity of literature, and combined with the author's own views, this article points out the misunderstanding of traditional Chinese music therapy in Inner Canon of Huangdfiat present and of ers some new styles of development.

Application of Psychotherapy Based on Five-Element Theory in Traditional Chinese Medicine in Improving the Mental States of Patients with Post-Stroke Depression

Abstract:Background: This study aims to explore the clinical effect of psychotherapy based on five-element theory in traditional Chinese medicine (TCM) in improving the mental states of patients with post-stroke depression (PSD), to provide reference and guidance for clinical treatment. Methods: A retrospective study was conducted on 163 PSD cases treated in The Second Affiliated Hospital, Hengyang Medical School, University of South China from January 2022 to January 2023. The patients treated with conventional treatment were included in the control group (n = 84), and patients receiving psychotherapy based on five-element theory in TCM and conventional treatment were classified as the observation group (n = 79). The anxiety levels, depression levels, TCM syndrome scores and sleep quality were compared in both groups. The enumeration data were tested by χ2 test. Kolmogorov-Smirnov method tested whether the measurement data met normal distribution, and those did not meet normal distribution were tested by Mann-Whitney method. The difference was statistically significant at p < 0.05. Results: After 4-week and 8-week treatments, the observation group had significantly lower scores of Hamilton Anxiety Scale (z = -4.562, -6.765; p < 0.001), Hamilton Depression Scale (z = -7.588, -8.023; p < 0.001), TCM syndrome (z = -7.138, -10.946; p < 0.001), and Pittsburgh sleep quality index (z = -6.819, -8.240; p < 0.001) than the control group. Conclusion: Psychotherapy based on five-element theory in TCM is beneficial to patients with PSD and can bring a certain clinical reference value.

Abstract:Focusing on the original text record in Huangdi Neijing (Inner Canon of Yellow Emperor), the relevant theories of "Tianyou (TE 16) and five regions" are explored, e.g. acupoint names, meridians and acupoint features, and the clinical application of these acupoints has been analyzed. It is discovered that "Tianyou (TE 16) and five regions" are mainly used in treatment of the disorders in the nervous system, five sensory organs and motor system. Besides, in terms of the relevant theories, "Tianyou (TE 16) and five regions" has been compared with "root and knot" and "twelve divergent meridians". It is found that "Tianyou (TE 16) and five regions" communicates the externally-internally related meridians and is applicable in treatment of the disorders with both exterior and interior involved. It is the essential acupoint composition of the human body.

Abstract:“Yu controlled the flood”and“five-element”category in the Great Norms belong to the original five-element theory, the two are mutually external and internal. During the Yin and Zhou dynasties, bringing soil and water under control or complying with the five-element was the precondition and justification of the holy king to rule over the world. The purpose of this was to“stabilize the constant principle”or“achieve the harmony and stability of the world”. Even though it has multiple meanings,“peace”is always in the core. Moreover, the original five-element theory also contains the germ of the theory of“the harmony of five-elements generating vitality”, which includes an old interpretation put forward by the former king and a new one put forward by Shi Bo. The essential difference between the old and new interpretation is that the former takes“earth”as the core element, while the latter does not. The new interpretation is more popular and closer to practical experience and common sense in daily life. Both interpretations take“generating vitality”as the fundamental direction. Besides, the theory of“the harmony of five-elements generating vitality”is widely used. Yan Ying’s“the difference between harmony and uniformity”, “the compatibility between water and fire”, “the compatibility between feasibility and infeasibility”and Confucius’ s“harmony in diversity”are all deductions of this new interpretation. In addition, the early five-element theory is characterized by political thinking, which starts from politics and serves politics at the same time.

Abstract:With the development of biomedicine, and significant success achieved by targeted therapy in the treatment of complex and refractory diseases such as cancers, clinical treatment and drug discovery based on molecular targets become the main direction of modern medical research. Guided by holistic concept, traditional Chinese medicine (TCM) has a unique advantage with regards to the prevention and treatment of complex diseases through its comprehensive effect. However, at present, TCM research is insufficient and problems such as inaccurate clinical findings, lack of dominant diseases, unclear evaluation of efficacy, and unclear mechanisms, cannot be solved easily. Therefore, the authors believe that the development of TCM should adopt an innovative approach that integrates the advantages of TCM and western medicine, and positively influences the development of TCM and even future medicine. This article proposes Target-combined Holistic Treatment (THT), a new concept based on the integration of TCM and western medicine. While being guided by the holistic and systematic perspective of TCM, THT also factors in the latest advances in both TCM and modern biomedical research, and uses macro-micro combination, target-syndrome combination, and internal and external codivision, as the primary strategies to establish a clinical diagnosis model that combines disease differentiation, syndrome differentiation, and target differentiation. It also uses drug ingredients/components to enable comprehensive interventions against multiple targets linked to a disease, and establishes target medication and research strategy of TCM, so as to improve its pertinence, efficacy, and safety, and ensure precise and effective medication. The theories and methods of THT fully integrate the advantages of TCM and western medicine, factoring in both macro-control and micro intervention, and also systematic confrontation, precision treatment, and embracing TCM theory and modern technology. This can guide the further development of TCM, western medicine, as well as the combination of TCM and western medicine. It will therefore help promote the integration and innovation of both TCM and western medicine, and the in-depth development of TCM. The theories and methods of THT can be used to guide not only the whole process of clinical diagnosis but also the development of innovative drugs. THT has been used to guide the formulation of TCM diagnosis and treatment for COVID-19 and has shown remarkable results. We have developed a Chinese herbal prescription "Keguan-1", to prevent and treat COVID-19 based on the theory and method of THT, and use the doctrine of "resist foreign aggression and pacify the interior" of TCM against COVID-19. Using the findings of research in modern medicine, THT can innovate and develop TCM based treatments, and thus aid in the treatment of various diseases, and also help promote the integration and development of TCM and western medicine. © 2021, Science Press. Abstract:"Syndrome"and"Treating different diseases with same method",which are important concepts in Traditional Chinese Medicine ( TCM) ,have profound meanings and are widely used in clinical practice. Chronic inflammatory diseases such as asthma,chronic obstructive pulmonary disease and atopic dermatitis have some similarities in terms of pathogenesis and intervention methods. However, current treatments for these diseases have limitations. According to the theory of TCM,these diseases are usually identified as syndrome "Kidney deficiency"or"Qi deficiency",therefore,treated with the method"strengthening Kidney and replenishing Qi". This is in line with the thought of"Use similar method to treat different diseases as identical syndrome". Strengthening Kidney and replenishing Qi is proved to be effective treatment,based on our preliminary work,this is because strengthening kidney have an impact on HPAA and replenishing Qi on immunity system,and correct abnormal relations. The research proved a synergistic effect between strengthening Shen and replenishing Qi herbs.

Abstract:This paper investigates the relationship of medical practitioners based on the concepts of Naturalism and Power of natural treatment(自然治癒力) in Taoism. It focuses on how the concept of Naturalism has been applied and evolved in 『Laozi』, 『Zhuangzi』, 『Huainanzi』, and 『Huangdi-Neijing』. The characteristic of the Naturalism in 『Laozi』 is to minimize artificial acts(人爲) and preserve the essence(精) and spirit(神) of the body, achieving maximum effects with minimal involvement and effort. 『Laozi』 emphasizes emptying desires and delusions, achieving inner stability, behaving with gentleness and humility, practicing moderation in vital energy(精氣) and conduct, and cultivating physical essence. The teaching of 『Laozi』 aims to let the state and the body operate naturally without artificial acts. Furthermore, 『Laozi』 suggests that even when problems occur in any process, the body has an inherent self-healing power that resolves the health problems and restores its original state. 『Zhuangzi』 depicts the ideal of living with empty mind without greed for external things, practicing deep breathing, and cultivating the union of heaven and human(天人合一). It emphasizes the possibilities of the universal system and the motive power of medicine and science within the concept of Naturalism. Interpreted through Guo Xiang(郭象)'s theory of self-becoming(獨化論), Taoist philosophy of 『Zhuangzi』 enhances its natural scientific characteristics. If the meaning of self-perception(自得) in 『Zhuangzi』 is interpreted through Guo Xiang's theory of inner sage and outer ruler(內聖外王論), it can be summarized as the perception of governing the external world through the inactive attitude(無爲). The theory of Naturalism in 『Zhuangzi』 respects the principles of nature and individual nature and personality, aiming to govern all affairs in accordance with the will of heaven(天命) and prolong the lifespan of individuals and communities. In 『Huainanzi』, the concept of Naturalism is supplemented with the notion of assisting the realization of one's innate nature, emphasizing the necessity of active efforts and education. According to 『Huainanzi』, there are conditions for natural healing in the spirit(神) and body(形). If the body follows the spirit, it leads to survival and healing, while if the spirit follows the body, it leads to death. The theory of mutual response(感應論) and the relation of heaven and men(天人相通) in 『Huainanzi』 control the fundamental aspects to the terminal symptoms through the power of natural healing. 『Huainanzi』 emphasizes the importance of active actions based on one's nature. In the context of 『Huainanzi』, Naturalism as an inactive attitude, amplifies the realization of all possibilities by adding human actions, embodying an active and intentional philosophy. The characteristic of 『Huainanzi』 is respecting the natural disposition while considering the importance of human actions. 『Huangdi-Neijing』 is a medical theory that applies the Taoist concept of Naturalism. In 『Huangdi-Neijing』, the role of medical techniques based on the power of natural healing is crucial. The direction of life and death is determined by the principles of direct motion(順行) and retrogression(逆行) respectively. The treatment approach focuses on balancing and promoting circulation. The human body possesses dynamism. It is important to regulate the balance between Yin and Yang and the fullness and emptiness of Qi(氣) within the appropriate therapeutic period. By addressing the problem within the progressing time period, one can resolve a problem and restore the body to its original healthy state. The emphasis here lies in the timing propriety of treatment. The logic of cause and effect(本末) from Taoism has been specialized into the medical theory of 『Huangdi-Neijing』. In essence, diseases have both root causes and peripheral symptoms, and the relationship between diseases and treatments involves addressing both the root cause and the peripheral symptoms.

Abstract:The theory of “analogous patterns in the same disease” is important in the development of disease differentiation and treatment in traditional Chinese medicine (TCM). Syndrome differentiation and treatment were considered the characteristics of TCM. However, disease differentiation and treatment, which were included in the theory of “formula corresponds to the syndrome complex” by Dr Zhang Zhongjing, were neglected. In the modern technological environment, disease differentiation and treatment is the expansion and extension of syndrome differentiation and treatment. It is also an important way of promoting TCM development. Composite Denshen Pilulae (CDP) successfully passed Phase-II clinical trials of the Food and Drug Administration in the United States. One of the important reasons for this occurrence is that CDP adopted chronic myocardial ischemia of coronary heart disease as its indication, which met the international and modern practice of medicine. Clinical practice calls for enrichment of disease differentiation and treatment on the basis of further development of TCM syndrome differentiation and treatment. A favorable atmosphere needs to be created for innovative thought on this subject.
Biological Network-Based Exploration of the Principles of Traditional Chinese Medicine

Abstract:The clinical application of traditional Chinese medicine (TCM) relies on herbal formulas as the main form of prescription. The holistic effect of TCM treatment is a significant advantage in treating complex diseases, but also makes comprehension of the scientific principles of TCM challenging. There is an urgent need to establish new theories and methods in line with the holistic characteristics of TCM to address the needs of research regarding the complex relationship between the chemical constituents of TCM formulas and biological systems. The biological basis of TCM theory and the mechanism of action of TCM formula are not well understood. For a long time, there was a lack of holistic research methods and theories in TCM. The development of TCM has been severely hampered by these factors. Recently, emerging disciplines such as informatics and systems biology have made significant progress. Conducting intersecting research between TCM and cutting-edge science and technology, such as artificial intelligence, big data, and informatics, based on biological network, is becoming increasingly important. Multidisciplinary cross-research has become an important way to explore the scientific principles of TCM. Biological network-based "network target" theory breaks through the gap between macro- and micro-intrinsic correlation analysis, achieving organic integration of reductionism and holism. This brings new possibilities to explore the principles of TCM from a holistic perspective and bridge modern medicine with TCM. "Cold" and "Hot" are the core diagnostic and therapeutic concepts of TCM, and "treating pre-disease" is a distinctive theory thereof. The scientific principles of these TCM theories need to be explored by modern science and technology. Recently, biological network-based techniques, such as graphical neural networks, have been applied to the elucidation of the biological basis of TCM syndrome, the analysis of the mechanism of action of TCM formulas, and the discovery of biomarkers. Precision prevention and treatment in TCM and precision research and development of TCM formulae have made remarkable progress due to the further elucidation of the principles of TCM. We aimed to review the theory and methodology of TCM research based on biological network, and its application in the interpretation of the scientific basis of disease-syndrome combination of the distinctive TCM theories such as "Cold/Hot" and "treating pre-disease". The elucidation of TCM principles from a biological network perspective requires high-quality TCM big data. In the future, research on the principles of TCM based on the biological network should first obtain high-quality TCM big data, combined with constant development and updating of algorithms to build more accurate prediction models. The established methods should be tested in experiments and clinics, enabling the development of artificial intelligence and big data to truly enhance the scientific and translational research of TCM. Based on the research progress from Shao Li's lab, this review provides an up-to-date overview of the use modern science and technology to interpret the principles of TCM and promote the modernization of TCM from the perspective of the biological network.

Abstract:Pattern complex is 'analogous patterns in the same disease' which means that the patients who suffer from the same disease manifest the common basic pattern complex in spite of slight differences in their accompanying patterns.So we can use a basic Chinese herb formula to treat these patients through slightly modifying the prescription for the sake of their accompanying patterns when giving the medicine to each patient.This thesis clarifies ideological connotation and clinical guiding significance of 'analogous patterns in the same disease' in accordance with several medical records of metabolic syndrome under the guidance of 'analogous patterns in the same disease'.

Abstract:Objective: To observe the effect of five-element music therapy of traditional Chinese medicine (TCM) on the clinical symptoms and the quality of life in the patients with suboptimal health status (SHS) of liver stagnation and spleen deficiency and explore the corresponding specificity changes in the temperature of acupoints when zangfu functions are of dysfunction and recovered to be balanced, separately. Methods: Sixty patients with SHS of liver stagnation and spleen deficiency were randomized into an observation group and a control group, 30 cases in each one. In the control group, the conventional health education was provided. In the observation group, on the base of the therapeutic regimen as the control group, the patients received the five-element music therapy to pacify the liver qi and strengthen the spleen functions, once every two days, 30 min each time, 3 treatments a week. The course of treatment consisted of 4 weeks. Before and after treatment, the TCM syndrome score and the MOS 36-item short form healthy survey (SF-36) score were compared between the two groups and the clinical therapeutic effect was evaluated. Using infrared thermal imaging, the temperature at the acupoints of the affected organs (liver, spleen), the related organs (gallbladder, stomach) and the other non-related zangfu organs (pericardium, lung) was detected before and after treatment in the two groups. Results: After treatment, the TCM syndrome scores were reduced when compared with those before treatment in the two groups (P<0.01, P<0.05); the reduction in the observation group was larger than that of the control group (P<0.01). The score of each domain for the SF-36 in the observation group and the score of role-emotional domain in the control group were all increased when compared with the scores before treatment (P<0.01, P<0.05); and the score of each domain for the SF-36 in the observation group was higher than that in the control group (P<0.01). The total effective rate was 66.7% (20/30) in the observation group, which was higher than 10.0% in the control group (3/30, P<0.05). In the observation group, the temperature of the yuan-primary point, the back-shu point and the front-mu point related to the liver, as well as those related to the gallbladder after treatment was reduced when compared with the temperature before treatment; and the changes were larger than those of the control group (P<0.01). The temperature of the yuan-primary point, the back-shu point and the front-mu point related to the spleen, as well as the back-shu point and the front-mu point related to the stomach in the observation group was increased when compared with the temperature before treatment (P<0.01); and the changes were larger than those of the control group (P<0.01). For the temperature of the non-specific points related to the liver and spleen, as well as the yuan-primary point, the back-shu point and the front-mu point related to the pericardium and the lung, there was no significant differences when compared with the temperature at the above-mentioned acupoints before and after treatment (P>0.05). Conclusion: TCM five-element music therapy associated with the conventional health education may effectively relieve the clinical symptoms and improve the quality of life in the patients with suboptimal health status of liver stagnation and sleep deficiency; and the therapeutic effect is better than the simple health education. The changes in the temperature of acupoints may reflect the functional regulation of the related zangfu organs in the body.
