Abstract:Despite vaccination, there were more than 100,000 annual cases of varicella in the United States in 2013–2014. Individuals at highest risk of developing severe or complicated varicella include immunocompromised people, preterm infants, and pregnant women. Varicella zoster immune globulin (human) (VARIZIG) is recommended by the CDC for postexposure prophylaxis to prevent or attenuate varicella-zoster virus infection in high-risk individuals. Contemporary information on administration of VARIZIG is limited.This open-label, expanded-access program provided VARIZIG to physician-identified, high-risk participants exposed to varicella. Participants included immunocompromised children/adults, infants (preterm, newborns whose mothers had varicella onset within 5 days before or 2 days after delivery, and those aged <1 year), and pregnant women. VARIZIG (125 IU/10 kg [up to 625 IU]) was administered intramuscularly, ideally within 96 hours, but up to 10 days, postexposure. Incidence of varicella rash and severity (>100 pox, pneumonia, or encephalitis) were assessed up to 42 days after administration.The varicella outcome population (n = 507) included 263 immunocompromised participants (32 adults, 231 children), 137 pregnant women, 105 infants, and 2 healthy adults with no history of varicella. Varicella incidence was 4.5% in immunocompromised participants, 7.3% in pregnant women, and 11.5% in infants. The incidence of varicella was similar when comparing VARIZIG administration ≤ 96 hours vs > 96 hours (up to 10 days) postexposure in the entire population (6.2% vs. 9.4%, respectively), and also in each subgroup. Of 34 participants with varicella, 5 developed > 100 pox and 1 developed pneumonia and encephalitis. There were no product-related deaths and only 1 serious adverse event (serum sickness) considered probably related to VARIZIG.Postexposure administration of VARIZIG was associated with low rates of varicella in high-risk participants, regardless of when administered within 10 days postexposure. VARIZIG was well-tolerated and safe in high-risk participants.

Abstract:Abstract Background Immunocompromised children and adults are at increased risk for severe disease and death following varicella zoster virus infection. Varicella zoster immune globulin (human) (VARIZIG) is recommended for post-exposure prophylaxis to prevent or attenuate varicella infection in high-risk individuals. Methods An open-label, expanded-access program provided VARIZIG to high-risk individuals exposed to varicella or herpes zoster. Immunocompromised participants were stratified by type of immunocompromising condition (“oncologic immunodeficiency”, “primary immunodeficiency”, “solid organ transplant” [SOT], “hematopoietic cell transplant” [HCT], and “other”). Patient characteristics, type of exposure and varicella outcome, and safety data were assessed. Results This analysis included 40 adults (primary [ n = 6] or oncologic [ n = 10] immunodeficiencies, history of SOT [ n = 5] or HCT [ n = 6], and other [ n = 13]), and 263 children (primary [ n = 13] or oncologic [ n = 152] immunodeficiencies, history of SOT [ n = 36] or HCT [ n = 17], and other [ n = 45]). Among adults and children, 48% vs 72% were exposed to varicella, 38% vs 16% were exposed to herpes zoster, and 15% vs 12% had an unspecified exposure. Overall incidence of varicella infection in adults after VARIZIG use was 6%; incidence of varicella infection in children after VARIZIG use was 7%. Similar rates were noted in each subgroup. Most cases of varicella were mild, with two children developing > 100 lesions and no cases of varicella-related pneumonia or encephalitis. Varicella-related hospitalizations occurred primarily in children with oncologic immunodeficiencies. One serious adverse event (serum sickness) was considered related to VARIZIG and occurred in a child with oncologic immunodeficiency. There were no varicella- or VARIZIG-related deaths. Conclusions These data indicate that VARIZIG may reduce severity of varicella in immunocompromised children and adults. Abstract:Post-exposure prophylaxis (PEP) with varicella zoster immunoglobulins (VZIG) should be administered as soon as possible after exposure to the virus, but always within ten days; in the previous guidelines this was within 96 hours. In cases of perinatal exposure, PEP with VZIG should be administered to neonates if the mother develops clinical chickenpox between seven days before delivery and seven days after delivery; in the previous guidelines this was between five days before delivery and two days after delivery. A new chapter on the treatment of chickenpox has been added to the guidelines.

Abstract:Abstract Background Despite universal childhood vaccination, Varicella-Zoster virus (VZV) remains a cause of severe infection following varicella exposure for immunocompromised children and high-risk infants. Post-exposure prophylaxis (PEP) is recommended to minimize this risk; however, providers may not consistently administer Varicella-specific immune globulin (VariZIG) as recommended in guidelines or may choose alternative regimens. The purpose of this study was to determine patterns of PEP for Varicella-Zoster Virus (VZV) at children’s hospitals. Method Using the Pediatric Health Information Systems database, clinical encounters with the ICD-9/10 codes for VZV exposure from 2009-2019 were reviewed. Only the first VZV exposure encounter for a given patient in the study period was included. Patients receiving prophylaxis or intravenous immunoglobulin (IVIG) within 30 days of the index encounter were also excluded. Presence of chronic illness or conditions at risk of progression to severe varicella disease was identified by reviewing ICD-9/10 codes for encounters in the prior 12 months. Choice and timing of PEP regimen exposure were identified, classified into the following categories: VZIG, intravenous immunoglobulin (IVIG), acyclovir, or combination prophylaxis. Clinical encounters for subsequent varicella disease within 30 days of exposure were recorded. Results There were 1770 children with first clinical encounters for VZV during the study period of which, 66 were excluded due to recent prior receipt of anti-viral therapy or IVIG. Of these 1704 children, 509 (29.9%) were prescribed PEP after VZV exposure, while 65 (3.8%) ultimately developed VZV disease. Of patients who received PEP, acyclovir was most frequently prescribed (n=195, 38.3%), divided between administration between1-4 days (n=132), 5-10 days (n=19), and both time periods (n=44), post-exposure. The proportion who received the other regimens was VariZIG (n=146, 28.7%), IVIG (n=115, 22.6%), and combination therapy (n=53, 10.4%). From 2009-2019, the proportion of children receiving acyclovir declined from 56% to 36%, VariZIG increased from 10 to 15%, IVIG decreased from 32 to 7%, while a similar percentage (8-9%) received a combination of Acyclovir and IG. The proportion of patients who subsequently had clinical encounters for varicella disease was highest for Acyclovir (30/195, 15.4%) followed by VariZIG (5/146, 3.4%), combination therapy (2/53, 3.8%), and IVIG alone (0/115). (p<0.0001). Of the 65 patients that developed VZV disease, 24/65 (36.9%) had at least one high-risk condition. Conclusion Varicella PEP in high-risk children was highly varied among children’s hospitals and often diverged from guideline recommendations. Use of acyclovir was associated with a higher rate of breakthrough infection. More consistent use of guideline-recommended agents like VariZIG may lead to less varicella disease and complications in high-risk children.

Abstract:Varicella-zoster virus causes both varicella (chickenpox) and herpes zoster (shingles). Although varicella incidence has dramatically declined since introduction of the live-attenuated varicella vaccine, vaccination rates are suboptimal, and outbreaks still occur. Additionally, herpes zoster incidence continues to rise. Severe or fatal complications may result from varicella transmission to at-risk individuals who are exposed to either varicella or herpes zoster. An increasing number of children and adults are receiving immunosuppressive therapies and are at high risk for severe varicella and other complications if exposed to the virus. Clinical management of individuals exposed to varicella-zoster virus should take into consideration the type of exposure, evidence of immunity, and host-immune status with regard to ability to receive varicella vaccination safely. Post-exposure varicella vaccination may prevent infection or mitigate disease severity in persons eligible for vaccination. Post-exposure prophylaxis with varicella zoster immune globulin is indicated for populations ineligible for vaccination, including immunocompromised children and adults, pregnant women, newborns of mothers with varicella shortly before or after delivery, and premature infants. Appropriate post-exposure management of individuals exposed to either varicella or herpes zoster-including assessment of immune status and rapid provision of optimal prophylaxis-can help avoid potentially devastating complications of varicella-zoster virus infection.

Abstract:Varicella infection during pregnancy is associated with serious maternal and fetal complications such as congenital varicella syndrome, maternal pneumonia and neonatal varicella. Pregnant women are ineligible to receive the varicella vaccination, thus women who lack evidence of immunity to varicella are at an increased risk for developing a varicella infection if exposed to a contagious individual. Presently, post-exposure prophylaxis involves the administration of a varicella zoster immunoglobulin (VariZIG™) to prevent or reduce the severity of an infection. The US FDA recently approved VariZIG for licensure and recommend that it be administered as soon as possible following VZV exposure, ideally within 96 h for greatest effectiveness. The following review critically examines the role of VariZIG in post-exposure prophylaxis of varicella during pregnancy.

Abstract:Background and objectives: Since the introduction of an effective vaccine in 1995, the incidence of primary varicella zoster virus (VZV) has greatly decreased. However, newborns and immunocompromised patients remain at risk for serious disease. Currently, varicella-specific immunoglobulin is recommended for treatment of nonimmune, exposed, high-risk patients with varicella-specific immunoglobulin. However, product inavailability has led to substitution of intravenous immunoglobulin (IVIg) for such prophylaxis on the basis of studies from the preimmunization era. No studies in the post-vaccine era have shown that IVIg contains adequate varicella-specific antibodies to protect patients at high risk. The overall effect of vaccination on varicella-specific immunoglobulin G (IgG) levels in donor-pooled IVIg products is unknown. We compared the varicella-specific IgG levels in prevaccine and current IVIg products. Methods: We used stored historic IVIg samples and current samples from our inpatient pharmacy. All samples were tested for varicella-specific IgG levels by enzyme-linked immunosorbent assay. Results: Ten historic lots and 24 current lots were tested. The overall mean value of varicella-specific IgG in the historic lots was 3.07 (SD: 0.70); the current lots had a mean of 3.83 (SD: 0.58). The postvaccine IVIg contained higher levels of antibody than the prevaccine lots. Conclusions: We found that current IVIg preparations continue to have high levels of varicella-specific IgG despite the changing epidemiology of how immunity has been obtained. Given the results of this study, it is reasonable for physicians to comfortably substitute IVIg for varicella-specific immunoglobulin preparations when treating high-risk patients exposed to VZV.
Safety and Varicella Outcomes in In Utero–Exposed Newborns and Preterm Infants Treated With Varicella Zoster Immune Globulin (VARIZIG): A Subgroup Analysis of an Expanded-Access Program

Abstract:Abstract Background Infants exposed to varicella zoster virus (VZV) in utero ≤5 days before or ≤48 hours after delivery and preterm infants are at high risk for varicella complications. An expanded-access program assessed varicella outcomes after administration of varicella zoster immune globulin (human) (VARIZIG) in a real-world setting. Methods In this open-label, expanded-access program, high-risk infants received ≤125 IU/10 kg of VARIZIG (NCT00338442). VZV outcomes and safety were assessed. Results There were 43 newborns exposed to VZV in utero and 80 preterm infants exposed to VZV; >80% received VARIZIG within 96 hours of reported exposure. When varicella outcomes were available, varicella occurred in 7 of 38 (18%) in utero–exposed newborns and zero of 65 preterm infants. Varicella-related complications were reported in 3 in utero–exposed newborns (3 with >100 lesions, 1 each with encephalitis and pneumonia). Adverse events were reported for 16% of in utero–exposed newborns and 25% of preterm infants, but few were considered related to VARIZIG. There were no deaths attributable to varicella or VARIZIG. Conclusions Varicella incidence and morbidity were low in in utero–exposed infants and zero in preterm infants who received prophylactic VARIZIG. There were few VARIZIG-related safety concerns.

Abstract:Varicella-zoster virus (VZV) infection causes two distinct clinical conditions. Primary varicella infection results in chickenpox, a contagious rash illness typically seen among children. VZV can reactivate years after the initial infection to cause herpes zoster (HZ) and lead to post-herpetic neuralgia, a common complication resulting in persistent pain that may last for years after the zoster rash resolves. A person's risk of having longer lasting and more severe pain associated with HZ increases with age. Since the introduction of VZV vaccines, the rates of infection, hospitalizations, and mortality have declined. In this review, we discuss in detail current VZV vaccines available for the prevention of VZV and HZ infections. Varilrix (GSK Biologicals, UK), Varivax (Merck, USA) and the combined measles, mumps, rubella, and varicella (MMRV) vaccine contain the live attenuated Oka strain of VZV for routine varicella vaccination. While Zostavax is the only HZ vaccine currently approved for use in the United States and the European Union [EMEA, 2011], a subunit vaccine candidate called HZ/su has recently shown improved efficacy for zoster prevention in two clinical trial phase III studies. VariZIG, a post-exposure prophylactic, uses zoster immune globulin to prevent VZV infection in those who have recently been in contact with VZV but lack evidence of varicella immunity and are contraindicated to receive the varicella vaccine. Further, we discuss the skin tropic and neurotropic factor VZV ORF7 gene and its involvement in varicella infection, reactivation and latency in ganglia. Ultimately, these studies can contribute to the development of a neuroattenuated vaccine candidate against varicella or a vector for delivery of other virus antigens.

Abstract:Objectives: Although administration of a second dose of varicella vaccine (2nd-dose VarV) to individuals who have previously received one-dose VarV has been recommended as a post-exposure prophylaxis (PEP) strategy for outbreak control, the effectiveness of this strategy remains unclear. We evaluated the vaccine effectiveness (VE) of 2nd-dose VarV as PEP among students involved in 129 varicella outbreaks in Shanghai, China from 2013 to 2016. Methods: Students who had received one-dose VarV more than 5 years prior to varicella exposure were eligible to receive 2nd-dose VarV as PEP. We evaluated the VE using the following formula: VE = (1 - hazard ratio (HR)) x 100%. Results: A total of 6762 students were eligible for 2nd-dose VarV, of whom 58.6% accepted PEP after varicella exposure. The adjusted VE of 2nd-dose VarV as PEP was 77% (95% confidence interval 64-85%). In addition, the adjusted VE of 2nd-dose VarV as PEP in affected classrooms with high vaccine uptake was higher than that in classrooms with lower vaccine uptake (87% vs. 69%). The adjusted VE was also higher in students who received 2nd-dose VarV within 3 days of exposure than those who received it more than 3 days post exposure (77% vs. 64%). Conclusions: These Results suggest that administration of 2nd-dose VarV as PEP is an appropriate intervention for outbreak control in countries where two-dose VarV has not been adopted. Implementation of Hospital Policy for Healthcare Workers and Patients Exposed to Varicella-Zoster Virus

Abstract:Varicella-zoster virus (VZV) causes a highly contagious and generally benign, self-limited disease. However, in high-risk populations including immunocompromised patients, pregnant women, and neonates, VZV infection can be associated with significant morbidity and mortality. Healthcare-associated transmission of VZV occurs among healthcare workers (HCWs) and patients by airborne transmission or by direct contact with the index case. To minimize the risk of transmission in healthcare settings, all VZV-susceptible HCWs should be encouraged strongly to be immunized with the varicella vaccine. For post-exposure management, active immunization (varicella vaccine), passive immunization (varicella-zoster immune globulin) and/or antiviral agents, and isolation could be used in specific situations. To prevent the transmission of VZV infection in the hospital settings, the development and implementation of hospital policies for appropriate infection control is also warranted. This article reviews the general information and healthcare-associated transmission of VZV and summarizes the recommendations for the pre- and post-exposure management of HCWs and patients, in hospital settings.

Abstract:Objective: To determine the likely rate of patient randomisation and to facilitate sample size calculation for a full-scale phase III trial of varicella zoster immunoglobulin (VZIG) and aciclovir as postexposure prophylaxis against chickenpox in children with cancer. Design: Multicentre pilot randomised controlled trial of VZIG and oral aciclovir. Setting: England, UK. Patients: Children under 16 years of age with a diagnosis of cancer: currently or within 6 months of receiving cancer treatment and with negative varicella zoster virus (VZV) serostatus at diagnosis or within the last 3 months. Interventions: Study participants who have a significant VZV exposure were randomised to receive PEP in the form of VZIG or aciclovir after the exposure. Main outcome measures: Number of patients registered and randomised within 12 months of the trial opening to recruitment and incidence of breakthrough varicella. Results: The study opened in six sites over a 13-month period. 482 patients were screened for eligibility, 32 patients were registered and 3 patients were randomised following VZV exposure. All three were randomised to receive aciclovir and there were no cases of breakthrough varicella. Conclusions: Given the limited recruitment to the PEPtalk2 pilot, it is unlikely that the necessary sample size would be achievable using this strategy in a full-scale trial. The study identified factors that could be used to modify the design of a definitive trial but other options for defining the best means to protect such children against VZV should be explored. Abstract:Varicella, or chickenpox, results from primary infection caused by the varicella-zoster virus (VZV). This virus is a member of the human herpes virus family which is highly contagious and infects most individuals during the childhood years. Recent efforts toward improving the clinical management of primary VZV infection have focused on evaluating the potential efficacy of oral acyclovir for the treatment of acute varicella in otherwise healthy children and adults. The feasibility of a preventive approach to VZV infection has been addressed in studies of the administration of the live attenuated varicella vaccine to susceptible children and adults. The information derived from these investigations of antiviral and vaccine efficacy has important implications for the clinical approach to the treatment and prevention of VZV infections.

Abstract:Varicella vaccine was first licensed in Japan and South Korea in 1989 for use in healthy children and was introduced in US in 1995. So far, 29 countries have adopted varicella vaccine in their universal immunization program (UIP). No Asian country, India included, has adopted the varicella vaccine as part of their UIP. The extra-cutaneous sites for VZV diseases are central nervous system and gastrointestinal tract, the expanded disease spectrum includes vasculopathy, myelitis, inflammatory bowel disease, perforated ulcers, and gastritis. The actual disease burden of varicella is not known as most of the infected individuals may not visit the physician. The amplifiable VZV DNA will not always be detectable in cerebrospinal fluid (CSF) samples in protracted illnesses such as vasculopathies, but demonstrable anti-VZV IgG in CSF has diagnostic value. The World Health Organization (WHO) position paper 2014 recommends two doses of varicella and zoster vaccines in targeted population. In India, varicella vaccine is not included in the UIP due to the cost and the belief that lifelong immunity occurs following primary infection. The expanded spectrum of VZV disease and the mounting body of evidence, however, suggest the need for both varicella and zoster vaccines in routine immunization schedule.

Abstract:Varicella vaccination programs introduced in 1995 significantly reduced morbidity and mortality of the disease in the United States. Initiated as a single-dose vaccine, studies evaluating efficacy identified periodic reports of outbreaks prompting the implementation of the 2-dose varicella vaccination in 2007. The single-vaccine dose period, 1995 to 2007, produced a gap of 2-dose coverage for many individuals, now in their late teens to late 30s. Those in the vaccine gap are likely unaware of possible compromised immunity and may be at risk. Primary care providers should screen patients in this gap population, administer a second vaccine dose as needed, as well as be alerted to recognize symptoms of breakthrough disease in this cohort.

Abstract:Varicella caused by varicella-zoster virus (VZV) is a common childhood disease and is generally mild and self-limiting. Abstract:Varicella-zoster virus (VZV), a common and ubiquitous human-restricted pathogen, causes a primary infection (varicella or chickenpox) followed by establishment of latency in sensory ganglia. The virus can reactivate, causing herpes zoster (HZ, shingles) and leading to significant morbidity but rarely mortality, although in immunocompromised hosts, VZV can cause severe disseminated and occasionally fatal disease. We discuss VZV diseases and the decrease in their incidence due to the introduction of live-attenuated vaccines to prevent varicella or HZ. We also focus on acyclovir, valacyclovir, and famciclovir (FDA approved drugs to treat VZV infections), brivudine (used in some European countries) and amenamevir (a helicase-primase inhibitor, approved in Japan) that augur the beginning of a new era of anti-VZV therapy. Valnivudine hydrochloride (FV-100) and valomaciclovir stearate (in advanced stage of development) and several new molecules potentially good as anti-VZV candidates described during the last year are examined. We reflect on the role of antiviral agents in the treatment of VZV-associated diseases, as a large percentage of the at-risk population is not immunized, and on the limitations of currently FDA-approved anti-VZV drugs. Their low efficacy in controlling HZ pain and post-herpetic neuralgia development, and the need of multiple dosing regimens requiring daily dose adaptation for patients with renal failure urges the development of novel anti-VZV drugs.

Abstract:Management of the exposure of pediatric oncology patients to varicella zoster virus (VZV) is controversial. We report the exposure of 56 patients to a single child with chicken pox at a pediatric cancer housing facility and describe our strategic approach for their management. We reviewed the immune and clinical status of 56 children with cancer receiving ongoing treatment at Memorial Sloan Kettering Cancer Center (MSK) who, while living at a pediatric cancer housing facility, were exposed to the index patient. The management of patients exposed included: (1) determination of immune status, (2) availability of vaccination history or VZV disease prophylaxis, (3) exposure status and subsequent isolation during the period of incubation, and (4) VZV disease prophylaxis. In addition to the 56 patients exposed to the index case, eight children with cancer treated at other facilities and 11 healthy siblings living in the facility were exposed. Of the 56 MSK patients, 21 were classified as immunosuppressed and received varicella zoster immune globulin (human), intravenous standard immune globulin, or acyclovir based on serostatus and immune function. The cohort was followed for 4 weeks after the exposure and no secondary infections were diagnosed. We performed a risk assessment and created a management plan to control and prevent further exposure and development of disease. No secondary cases developed. This strategic approach could serve as a model for the management of VZV exposure for other pediatric oncology centers.
