Abstract:OBJECTIVES: Bloating and distention are often attributed to dietary factors by patients with irritable bowel syndrome (IBS). This study examined the effects of gas production and visceral hypersensitivity on digestive symptoms after lactose ingestion in a population with lactase deficiency.METHODS: IBS patients (n = 277) and healthy controls (HCs, n = 64) underwent a 20-g lactose hydrogen breath test (LHBT) with evaluation of hydrogen gas production and lactose intolerance (LI) symptoms. Abdominal distention (199 IBS, 40 HCs) was measured during LHBT. Rectal sensitivity (74 IBS, 64 HCs) was assessed by barostat studies.RESULTS: Hydrogen production and distention were similar in IBS patients and HCs during LHBT; however, LI was more frequent in IBS (53.8 vs. 28.1%, P < 0.001), especially bloating (39.0% vs. 14.1%, P < 0.001) and borborygmi (39.0 vs. 21.9%, P = 0.010). Only 59.0% of patients with bloating had distention. No correlation was observed between girth increment and bloating (P = 0.585). IBS patients had lower rectal sensory thresholds (P = 0.001). Multivariate analysis indicated that hydrogen production increased bloating (odds ratio (OR) 2.19, 95% confidence interval (CI) 1.09-4.39, P = 0.028) and borborygmi (OR 12.37, 95% CI 3.34-45.83, P < 0.001) but not distention (P = 0.673). Visceral hypersensitivity was associated with bloating (OR 6.61, 95% CI 1.75-25.00, P = 0.005) and total symptom score (OR 3.78, 95% CI 1.30-10.99, P = 0.014).CONCLUSIONS: Gas production and visceral hypersensitivity both contribute to digestive symptoms, especially bloating and borborygmi, in IBS patients after lactose ingestion. Objective abdominal distention is not correlated with subjective bloating.
Bloating in Irritable Bowel Syndrome Is Associated with Symptoms Severity, Psychological Factors, and Comorbidities

Abstract:BackgroundBloating is one of the most bothersome symptoms of irritable bowel syndrome (IBS), but its association with other symptoms is not well described.AimsWe investigated the association between symptoms of abdominal bloating, other IBS symptoms, psychological distress, and comorbid pain conditions.MethodsWe conducted a cross-sectional study on a large cohort of IBS patients with and without symptoms of abdominal bloating and healthy controls. Subjects were assessed for IBS and its subtypes, pain severity, symptoms severity, psychological disturbances, comorbidities, and dietary restrictions of three fluid groups.ResultsA total of 484 subjects were investigated. Compared with IBS − B, IBS + B subjects had higher rates of constipation (30% vs. 15%, p = 0.191) and lower rates of diarrhea, (70% vs. 85%, p = 0.191) although these were not statistically significant. Bloating severity correlated with IBS symptoms severity (r = 0.397, p = 0.000), pain severity (r = 0.364, p = 0.000), and both anxiety and somatization scores (r = 0.167, p = 0.015 and r = 0.219, p = 0.001, respectively). Prevalence of fibromyalgia and depression and somatization scores was significantly higher in IBS with bloating than in IBS without bloating. IBS patients with bloating reported more dietary restriction of three fluid groups to control their symptoms compared with healthy controls and IBS patients without bloating.ConclusionsAbdominal bloating in IBS is associated with increased symptoms and pain severity, somatization, depression, fibromyalgia, and altered dietary fluids composition. Recognizing and addressing these factors in the diagnosis and management of patients with IBS may improve clinical outcome.

Psychological Factors May Affect the Quality of Life in Irritable Bowel Syndrome Patients More Than the Gut Itself? A Multicenter Cross‐sectional Study

Abstract:Abstract Background and Aim Both intestinal symptoms and comorbidities exist in irritable bowel syndrome (IBS) patients and influence their quality of life (QOL). More research is needed to determine how these variables impact the QOL of IBS patients. This study aimed to determine which specific factors had a higher influence on QOL and to further compare the effects of intestinal symptoms and comorbidities on QOL. Methods IBS patients were recruited from six tertiary hospitals in different regions of China. QOL, gastrointestinal symptoms, and comorbidities were assessed by different scales. Correlation analysis, multiple linear regression, and mediation model were used for statistics. Results Four hundred fifty‐three IBS patients (39.7% women, mean age 45 years) were included and no significant differences in QOL were found across demographic characteristics. Abnormal defecation (r = −0.398), fatigue (r = −0.266), and weakness (r = −0.286) were found to show higher correlation with QOL. More than 40% of IBS patients were found to suffer from varying degrees of anxiety or depression, and anxiety (r = −0.564) and depression (r = −0.411) were significantly negatively correlated with QOL (P < 0.001). Psychological factors showed the strongest impact (β′ = −0.451) and play a strong mediating role in the impact of physiological symptoms on QOL. Anxiety was found to be the strongest factor (β′ = −0.421). Conclusion Compared with other symptoms, psychological symptoms, particularly anxiety, are more common and have a more negative influence on QOL. The QOL of IBS patients is also significantly impacted by abnormal defecation, abdominal distension, and systemic extraintestinal somatic symptoms. In the treatment of IBS patients with unhealthy mental status, psychotherapy might be prioritized.

Abstract:Introduction Many patients, especially those with Irritable Bowel Syndrome (IBS), report food intolerance; however the mechanism underlying these “functional digestive symptoms” is unknown. We assessed the role of psychological factors, mucosal immune activation and visceral sensitivity on the development of lactose intolerance (LI) after ingestion of 20g lactose in IBS patients and healthy controls (HCs) with lactase deficiency: a validated, clinical experimental model of intolerance to poorly absorbed, fermentable foods (Yang et al Clin Gastro Hep 2013 in publication). Methods IBS-D patients meeting Rome III criteria (n = 277) and age/sex matched HCs (n = 64) underwent a 20g lactose hydrogen breath test (LHBT) with measurement of hydrogen production, abdominal distention, and LI symptoms. Hospital Anxiety and Depression Score assessed psychologic state. Barostat measured rectal sensitivity. Additionally, 55 IBS-D patients and 18 HCs completed colonoscopy with colon and terminal ileum (TI) biopsies for quantification of mast cells (MCs), T lymphocytes, and enterochromaffin cells (ECC). Results Hydrogen production and distention were similar in IBS patients and HCs during LHBT; however LI symptoms were more frequent in IBS (54% vs. 28%, P < 0.001), including bloating (39% vs. 14%, P < 0.001), borborygmi (39% vs. 22%, P = 0.010), pain (31% vs. 11%, P = 0.001) and diarrhoea (29% vs. 9%, P = 0.001). IBS patients were more anxious (p < 0.001) and had higher rectal sensitivity than HCs (P = 0.001). Multivariate analysis indicated that hydrogen production increased the likelihood of bloating (OR2.2 (95%CI 1.1–4.4), P = 0.028) and borborygmi (OR12.4 (3.3–45.8), P < 0.001), but not objective distention (P = 0.673). Visceral hypersensitivity also associated with bloating (OR6.6 (1.7–25.0), P = 0.005) and total symptom score (OR3.7 (1.3–10.9, P = 0.014). A planned subanalysis showed that, compared to IBS patients with no symptoms, those with LI were more anxious (p = 0.045) and more likely to have visceral hypersensitivity (p < 0.001). In those that had colonoscopy, IBS patients with LI (25/55 (45%)) had increased MCs (p < 0.006), T-cells and ECC (both p < 0.05) in proximal colonic and TI mucosa compared to patients without LI and HCs. Multivariate analysis indicated that total LI symptom score was associated with anxiety (r = 0.519, P < 0.001), MCs in terminal ileum (r = 0.650, P < 0.001) and visceral sensitivity (r = 0.629, P < 0.001). Conclusion Gas production and sensitivity to luminal distension both contribute to digestive symptoms after lactose ingestion in patients with lactase deficiency. IBS-D patients with LI are characterised by anxiety, evidence of mucosal immune activation and visceral hypersensitivity. Disclosure of Interest None Declared

Abstract:Small intestinal bacterial overgrowth (SIBO) has been implicated in the pathogenesis of irritable bowel syndrome (IBS). Psychosocial factors and low-grade colonic mucosal immune activation have been suggested to play important roles in the pathophysiology of IBS. In total, 94 patients with IBS and 13 healthy volunteers underwent a 10 g lactulose hydrogen breath test (HBT) with concurrent Tc99m scintigraphy. All participants also completed a face-to-face questionnaire survey, including the Hospital Anxiety and Depression Scale, Life Event Stress (LES), and general information. Serum tumour necrosis factor-α, interleukin- (IL-) 6, IL-8, and IL-10 levels were measured. The 89 enrolled patients with IBS and 13 healthy controls had no differences in baseline characteristics. The prevalence of SIBO in patients with IBS was higher than that in healthy controls (39% versus 8%, resp.; p=0.026). Patients with IBS had higher anxiety, depression, and LES scores, but anxiety, depression, and LES scores were similar between the SIBO-positive and SIBO-negative groups. Psychological disorders were not associated with SIBO in patients with IBS. The serum IL-10 level was significantly lower in SIBO-positive than SIBO-negative patients with IBS.

Abstract:Background: IBS and IBD are chronic gastrointestinal (GI) conditions associated with abdominal pain and abnormal bowel habits.Although IBS and IBD share common symptoms, their pathogenesis differs.Whereas IBD symptoms often arise from inflammatory or structural abnormalities, these objective markers are not consistently identified in IBS.Because symptom pathogenesis differs between disorders, we aimed to test whether symptom severity also varies between IBS and IBD.Methods: We compared GI symptoms using items developed for the NIH Patient Reported Outcome Measurement Information System (PROMIS®) that assessed frequency, severity, interference, and bother to 3 groups: (1) IBS patients from clinic and community populations, (2) IBD clinic patients, and (3) a U.S. general population (GP) sample selected by a commercial research firm (Cint) to approximate characteristics in the 2010 census.We measured 8 GI symptom domains: gastroesophageal reflux (GERD), disrupted swallowing, nausea/vomiting (N/V), belly pain, diarrhea, constipation, gas/bloating, and fecal incontinence/soling (FI).We measured overall symptom severity by averaging responses to items and transforming linearly to a 0-100 possible range (higher=worse).We compared group differences using analysis of covariance controlling for demographics and GI symptom-specific anxiety (Visceral Sensitivity Index [VSI]).To adjust for multiple comparisons for different symptoms, a p ,0.01 was used.Results: Respondents included 231 with IBS (mean age=44.8yrs; 68%F; 56.3% Caucasian), 190 with IBD (mean age=41.3yrs; 46%F; 58.4% Caucasian) and 1117 from the GP (mean age 45.7; 57%F; 72.7% Caucasian).Mean age, race/ethnicity, and education significantly differed between groups (all p ,0.01).The Table shows differences in GI symptom severity scores between groups.The GP had greater severity of GERD, disrupted swallowing and N/V compared to the patient groups.GERD severity differed among all groups with greatest severity in GP, followed by IBS, and then IBD.GP reported more disrupted swallowing than IBS and greater N/V than IBD.IBS patients reported greater severity of belly pain vs. GP and greater severity of constipation and gas/ bloating vs. GP and IBD.IBD patients had more severe diarrhea than the GP but not IBS.Conclusions: While IBS and IBD are conditions associated with greater lower GI symptom severity compared to the GP, they report less severe upper GI symptoms.Between disease states, IBS patients report numerically worse symptom severity vs. IBD for 7 of 8 GI symptom domains -only diarrhea appeared more severe in IBD.Although IBD is marked by objective inflammatory and anatomic biomarkers, this does not necessarily yield worse symptom severity than IBS -a condition without reliable biomarkers; the converse is true in this study.* P values after controlling for demographics and VSI.Values shown in bold are statistically significant.

Abstract:The effects of lactase deficiency on digestive symptoms and diet in patients with irritable bowel syndrome (IBS) have not been well defined. We assessed lactose absorption and tolerance and the intake of dairy products in healthy volunteers (controls) and patients with diarrhea-predominant IBS (D-IBS).Sixty patients diagnosed with D-IBS at the Sir Run Run Shaw Hospital, Hangzhou, China and 60 controls were given hydrogen breath tests to detect malabsorption and intolerance after administration of 10, 20, and 40 g lactose in random order 7-14 days apart; participants and researchers were blinded to the dose. We assessed associations between the results and self-reported lactose intolerance (LI).Malabsorption of 40 g lactose was observed in 93% of controls and 92% of patients with D-IBS. Fewer controls than patients with D-IBS were intolerant to 10 g lactose (3% vs 18%; odds ratio [OR], 6.51; 95% confidence interval [CI], 1.38-30.8; P = .008), 20 g lactose (22% vs 47%; OR, 3.16; 95% CI, 1.43-7.02; P = .004), and 40 g lactose (68% vs 85%; OR, 2.63; 95% CI, 1.08-6.42; P = .03). H(2) excretion was associated with symptom score (P = .001). Patients with D-IBS self-reported LI more frequently than controls (63% vs 22%; OR, 6.25; 95% CI, 2.78-14.0; P < .001) and ate fewer dairy products (P = .040). However, self-reported LI did not correlate with results from hydrogen breath tests.The risk of LI is related to the dose of lactose ingested and intestinal gas production and is increased in patients with D-IBS. Self-reported LI, but not objective results from hydrogen breath tests, was associated with avoidance of dairy products.
Symptom severity and clinical characteristics of patients with bloating

Abstract:Background: Abdominal bloating is common in functional gastrointestinal disorders (FGID). To better characterize this patient population, we evaluated clinical and psychological characteristics of bloating and analyzed their differences by bloating severity. Methods: Patients with FGIDs evaluated at a single academic outpatient referral gastroenterology clinic were surveyed. Bloating severity was classified as minimal, moderate or severe. Symptom-specific questionnaires were used to evaluate bowel habits, abdominal bloating, depression, anxiety, somatization and sleep disturbance. Associations between bloating severity, clinical characteristics and FGID subtypes were analyzed in univariate and multivariate modeling. Key results: Of 612 FGID patients included (78% female, mean age of 44 ± 16.5 years), bloating was reported as minimal in 231(37.8%), moderate in 217(35.4%), or severe in 164(26.8%). Patients with severe bloating were more likely to be female, younger, and have co-existing functional dyspepsia than those with minimal bloating (p < 0.05). Bloating severity and severity of abdominal distension were significantly correlated (p < 0.05). On multivariable regression, patients who met criteria for functional constipation and functional dyspepsia had 80% and 125% higher odds, respectively, of severe bloating compared to minimal to moderate bloating. Younger age, abdominal pain and constipation severity, and somatization scores were also independently associated with severity of bloating. Conclusions & inferences: Severe bloating is associated with younger age, and with more severe abdominal pain, constipation, and somatization. Patients who met criteria for functional constipation and functional dyspepsia are more likely to experience severe bloating.
Gender-related differences in IBS symptoms

Abstract:Objective: Women are more likely than men to report irritable bowel syndrome (IBS) symptoms as well as chronic visceral and musculoskeletal pain. The study tests the general hypothesis that female IBS patients differ from their male counterparts in symptoms related to the viscera and musculoskeletal system, and that these differences are related to the menstrual cycle. Methods: Seven hundred fourteen Rome positive IBS patients were evaluated for GI and extracolonic symptoms, psychological symptoms (SCL-90R), and quality of life (QOL) (SF-36). In addition, 54 postmenopausal women were compared with 61 premenopausal women and 54 age-matched males, all with IBS. Results: Male and female subjects reported similar GI levels of symptom severity and psychological problems. Abdominal distension associated with a sensation of bloating was more commonly reported by female patients, as were symptoms of constipation. Female patients more often reported nausea, alterations of taste and smell, and unpleasant sensations on the tongue, muscle stiffness in the morning, greater food sensitivity, and side effects from medications. Forty percent of female patients reported menstrual cycle-related worsening of symptoms, but few symptom differences were found between pre- and postmenopausal women, making it unlikely that most of the gender differences observed are directly tied to the menstrual cycle. Conclusions: Female patients report higher levels of a variety of intestinal and nonintestinal sensory symptoms despite similar levels of IBS severity, abdominal pain, psychological symptoms, and illness impact. The apparent differences in sensitivity to nonpainful visceral sensations, medications, and food may represent altered sensory processes, autonomic responses, and/or cognitive hypervigilance.

Abstract:Abdominal bloating is a subjective sensation of trapped abdominal gas, producing abdominal pressure, fullness sensation, and, in some patients, associated objective abdominal distension. In this month's edition of the journal, a new validated questionnaire to assess the prevalence and impact of gas-related symptoms is presented by Duracinsky et al., showing that gas-related abdominal symptoms are prevalent in patients with irritable bowel syndrome and have a measurable impact on patients daily life. A parallel study by Gardiner et al. assessing the severity of bloating in functional gastrointestinal disorders shows that severe bloating is associated with the severity of abdominal pain, constipation, and somatization, advancing our understanding of the clinical characteristics and relevance of gas-related symptoms in the broad spectrum of functional gastrointestinal disorders. Management of bloating includes non-pharmacological and pharmacological strategies. Dietary interventions to reduce intestinal fermentation and ingestion of food supplements like prebiotics or probiotics can reduce bloating by reducing gas production. The main targets of pharmacological treatments are to improve transit and evacuation with prokinetics, to improve intestinal gas tolerance with antispasmodics and/or neuromodulators, and to modify intestinal microbiota with antibiotics. Secretagogues act by increasing intestinal secretion and decreasing visceral sensitivity and have been reported to be an effective treatment alternative for patients with bloating associated with constipation. Biofeedback therapy addressed to correct abdomino-phrenic dysynergia may be useful for patients with objective abdominal distension, and patients with bloating associated with outlet obstructed defecation may benefit from anorectal biofeedback.
The pathogenesis of functional abdominal bloating

Abstract:Objective: To investigate the pathogenesis of functional abdominal bloating (FAB). Methods: All of the subjects recruited met the Rome III Diagnostic Criteria: 64 patients with functional abdominal distension, 33 patients with constipation-predominant irritable bowel syndrome (IBS) with moderate to severe abdominal bloating, 33 patients with functional constipation (FC) with moderate to severe abdominal bloating, and 20 healthy volunteers. Abdominal plain films were used to assess the intestinal gas volume increase. Lactulose hydrogen breath test (LHBT) was used to analyze whether the alteration of intestinal flora was present. Gastric and gut emptying rates were evaluated by non-radio-paque marks at 4 h and 48 h. The concentrations of fasting serum substance P (SP) and calcitonin gene related peptide (CGRP) were measured by ELISA method. Psychosocial risk factors were assessed with self-rating depression scale (SDS) and self-rating anxiety scale (SAS). Results: The positive rate of LHBT, gastric and gut emptying, serum CGRP and SP concentrations, and SAS and SDS scores differed significantly among the 4 groups (all P<0.05), but intestinal gas volumes had no difference (P>0.05). FAB and control groups differed obviously in the positive rate of LHBT, gastrointestinal emptying rate, serum SP concentration, and SAS and SDS scores (all P<0.05), but not in intestinal gas volumes or serum CGRP concentration (both P>0.05). Conclusion: Alteration of intestinal flora, gastrointestinal dysfunction, visceral hypersensitivity and abnormal psychological factors may play more prominent roles in the pathogenesis of FAB than the increase of intestinal gas contents.

Abstract:Results: We recruited 1819 FGID patients. There were 794 male and 1025 female patients; 703 (38.6%) patients were bloaters; 45.1% of bloaters claimed that bloating was their most bothersome symptom; 45.5% of bloaters related their symptoms with a more or less frequent stool and 67.6% felt relief with defecation. By Rome III criteria, majority of bloaters belong to FD (54.5%), IBS (45%), and FAB (20.8%). Overall, 146 (8.0%) patients fulfilled RIII‐FB. There was no significant gender difference in FB‐RIII patients (male 9.3% vs female 7.0%). Taiwan (11.4%) recorded the highest prevalence of RIII‐FB patients, followed closely by India (10.4%) and Korea (10.0%). Only 54.8% of RIII‐FB patients claimed that treatment improved their symptoms.
Bloating of functional origin is predominantly associated with IBS and IBS symptomatology in Asia

Management of abdominal bloating and distension, from subjective to objective

Abstract:Abdominal distension is a clinical occurrence that involves a measurable, objective increase in abdominal circumference, which patients report as feeling like pregnant or like having a balloon inside the abdomen. This sign is often preceded by a subjective feeling of abdominal heaviness or bloating, reported as the sensation of having a huge amount of gas trapped inside. These manifestations are highly prevalent and may reflect on their own a gut-brain axis condition, such as functional abdominal distension, or be part of other disorders such as functional dyspepsia or irritable bowel syndrome (IBS). The prevalence of abdominal distension and bloating is 3.5 %. However, when associated with other gut-brain axis disorders such as dyspepsia or IBS, prevalence grows above 50 %. The etiology and pathophysiology of abdominal bloating and distension are highly complex and represent a challenge for both the practitioner and the patient. The patient often associates these sensations with trapped gas, and attributes them to some food intolerance, hence he/she adopts a highly restrictive diet that fails to resolve distension while incurring the risk of nutritional deficiencies or secondary dysbiosis, making a directed treatment guideline necessary.

Abstract:The diagnosis of irritable bowel syndrome (IBS) should be considered when patients have had abdominal pain/discomfort, bloating, and change in bowel habits for 6 months. Patients may experience variation between periods of constipation and diarrhea. When evaluating patients with IBS, physicians should be alert for red flag symptoms, such as rectal bleeding, anemia, nighttime pain, and weight loss. Physicians also should consider other medical conditions that manifest similarly to IBS. Clinicians who are confident in diagnosing IBS based on symptoms typically do not obtain many tests unless the patient has red flag symptoms. Various etiologic mechanisms have been proposed for IBS, including abnormal bowel motility, inflammation, altered mucosal permeability, genetic predisposition, and visceral hypersensitivity. Lack of certainty about the etiology makes it difficult to develop effective management approaches; thus, management is directed toward symptom relief. Dietary changes, such as avoiding fermentable carbohydrates, may benefit some patients, especially those with bloating. Constipation-dominant IBS can be managed with antispasmodics, lubiprostone, or linaclotide, whereas diarrhea-dominant IBS can be managed with loperamide or alosetron, though the latter drug can cause ischemic colitis. For long-term therapy, tricyclic antidepressants or selective serotonin reuptake inhibitors have good efficacy. Peppermint oil and probiotics also may provide benefit.

Abstract:BACKGROUND. To this day, relatively little is known about the specifics and mechanisms by which psychological factors, such as a general tendency to anger and the style of expressing anger, can be associated with pro-inflammatory processes and visceral hypersensitivity, which contribute to the appearance of signs of irritable bowel syndrome and lead to its refractory course. AIM. The purpose of the work. to identify the specifics of differences in the presence and severity of depression, anxiety, anger expression and satisfaction with the quality of life in women (average age 39.210.6) with refractory IBS of constipated, diarrheal and mixed types (duration of the disease 33.848.1 months), based on the hypothesis that women with different types of IBS may have different styles of expressing negative emotions (for example, anger) and ways of controlling behavior. MATERIAL AND METHODS. PHQ-15 Patient Health Questionnaire-15; GAD-7 General Anxiety Disorder-7; PHQ-9 Patient Health Questionnaire-9; IBS-QOL Irritable Bowel Syndrome Quality of Life; STAXI StateTrait Anger Expression Inventory. RESULTS. Correlations between subtypes of IBS and negative aspects of the emotional sphere are observed and it is shown that subtypes of IBS-C and IBS-D demonstrate affective profiles characterized by a higher level of anxiety, depression and changes in the expression of anger with difficulties of expression both verbally and nonverbally. All types are characterized by autoaggression, suppression of anger, but these aspects are more pronounced in IBS-D and IBS-C. There is intense control of anger expression. With IBS-C and IBS-M, there is greater control over the expression of anger externally than with IBS-D. Women with IBS-C have more pronounced indicators of anger as a mental state. There are difficulties in verbal expression of anger, hetero- and autoaggression. Anger is a trait of character and is included in the structure of temperament. Women with IBS-D have pronounced indicators of anger as a reaction to a current life event, autoaggression, anger control inside. The level of affect somatization, the presence and severity of generalized anxiety disorder, depression, changes in life satisfaction are among the predictors that determine both the level of anger and the ways of its expression. CONCLUSION. Intestinal motility can be considered as an indicator of behavior control, as an indicator of the regulatory function of the patient, the intensity of affective involvement in a particular situation. Changes in the cognitive, emotional and executive components of the mental organization play only a significant role in the refractory course of IBS.

The Role of Dietary Energy and Macronutrients Intake in Prevalence of Irritable Bowel Syndromes

Abstract:Background . Irritable bowel syndrome (IBS) is a chronic gastrointestinal disorder characterized by abdominal pain and altered bowel habits in the absence of any detectable organic illnesses. Interest in the effect of dietary opponents to the IBS pathogenesis has been increased in recent years. This study aims to review previous studies to determine the relationship between IBS prevalence in community and dietary energy and macronutrients intakes according to the national nutrition surveys. Methods . A literature search was conducted in PubMed and EMBASE to September, 2018, to identify population-based studies that reported the prevalence of IBS. Daily energy intake, daily carbohydrates, and protein and fat percent contribution to energy intake (%) were obtained from study population-based national nutrition survey. The correlations of prevalence of IBS and dietary intakes were obtained by Spearman coefficient or Pearson coefficient. Results . Global prevalence of IBS was 11.7%. There was no correlation between overall prevalence of IBS of individual countries and national energy intake (P = 0.785), protein proportion (P = 0.063), carbohydrates proportion (P = 0.505), or fat proportion (P = 0.384) according to the years when the studies were conducted. No correlations were detected between dietary intake and male or female IBS prevalence. Interestingly, protein proportion was positively correlated with the prevalence of IBS in Rome III criteria (r = 0.569). Conclusion . Our findings demonstrate that dietary energy and macronutrients intake do not play a direct role in prevalence of IBS. However, IBS diagnostic criteria seem to have a bias on the correlation between prevalence of IBS and dietary intake. Further studies are needed to confirm the correlation between prevalence of IBS and specific dietary intake.
Alteration of intestinal microflora is associated with reduction in abdominal bloating and pain in patients with irritable bowel syndrome

Abstract:Objective: The influence of the gastrointestinal (GI) microflora in patients with irritable bowel syndrome (IBS) has not been clearly elucidated. This study was undertaken to see if patients with IBS have an imbalance in their normal colonic flora, as some bacterial taxa are more prone to gas production than others. We also wanted to study whether the flora could be altered by exogenous supplementation. In a previous study we have characterized the mucosa-associated lactobacilli in healthy individuals and found some strains with good colonizing ability. Upon colonization, they seemed to reduce gas formation. Methods: The study comprised 60 patients with IBS and a normal colonoscopy or barium enema. Patients fulfilling the Rome criteria, without a history of malabsorption, and with normal blood tests underwent a sigmoidoscopy with biopsy. They were randomized into two groups, one receiving 400 ml per day of a rose-hip drink containing 5 x 10(7) cfu/ml of Lactobacillus plantarum (DSM 9843) and 0.009 g/ml oat flour, and the other group receiving a plain rose-hip drink, comparable in color, texture, and taste. The administration lasted for 4 wk. The patients recorded their own GI function, starting 2 wk before the study and continuing throughout the study period. Twelve months after the end of the study all patients were asked to complete the same questionnaire regarding their symptomatology as at the start of the study. Results: All patients tolerated the products well. The patients receiving Lb. plantarum had these bacteria on rectal biopsies. There were no major changes of Enterobacteriaceae in either group, before or after the study, but the Enterococci increased in the placebo group and remained unchanged in the test group. Flatulence was rapidly and significantly reduced in the test group compared with the placebo group (number of days with abundant gas production, test group 6.5 before, 3.1 after vs 7.4 before and 5.6 after for the placebo group). Abdominal pain was reduced in both groups. At the 12-month follow-up, patients in the test group maintained a better overall GI function than control patients. There was no difference between the groups regarding bloating. Fifty-nine percent of the test group patients had a continuous intake of fermented products, whereas the corresponding figure for the control patients was 73%. Conclusions: The results of the study indicate that the administration of Lb. plantarum with known probiotic properties decreased pain and flatulence in patients with IBS. The fiber content of the test solution was minimal and it is unlikely that the fiber content could have had any effect. This type of probiotic therapy warrants further studies in IBS patients.

Abstract:AIM: It has been noticed that gastroenteritis or dysentery plays a role in pathogenesis of irritable bowel syndrome (IBS), and antibiotics can increase functional abdominal symptoms, both of which may be partly due to intestinal flora disorders. This study was to determine the change of gut flora of IBS, a cluster of abdominal symptoms. Because of the chronic course and frequent occurrence of the disease, IBS patients suffered much from it. So the quality of life (Qol) of IBS patients was also evaluated in this study.METHODS: Twenty-five Rome II criteria-positive IBS patients were recruited, and 25 age and gender-matched healthy volunteers were accepted as control. The fecal flora, including Lactobacillus, Bifidobacterium, Bacteroides, C. perfringens Enterobacteriacea and Enterococus, were analyzed quantitatively and qualitatively. We also calculated the ratio of Bifidobacterium to Enterobacteriaceae (B/E ratio) in both IBS patients and controls. In both groups, the data were further analyzed based on age difference, and comparisons were made between the younger and elder subgroups. We also evaluated the quality of life (QoL) of IBS patients and the control group using the Chinese version of SF-36 health questionnaire.RESULTS: In IBS patients, the number of fecal Bifidobacterium was significantly decreased and that of Enterobacteriaceae was significantly increased compared with that in healthy controls (both P<0.05). The mean microbial colonization resistance (CR) of the bowel in IBS patients was smaller than 1, making a significant difference compared with that in control which was more than 1 (P<0.01). There was no significant difference in gut flora between two subgroups. While in control, the elder subgroup presented more Enterobacteriacea than the younger one (P<0.05). Compared with the control group, IBS patients had significantly lower scores on all SF-36 scales, with the exception of physical functioning. However, there was no significant correlation between quality of life and enteric symptoms in IBS patients.CONCLUSION: There are intestinal flora disorders in IBS patients, which may be involved in triggering the IBS-like symptoms. IBS patients experience significant impairment in QoL, however, the impairment is not caused directly by enteric symptoms.
Prevalence and Associated Factors of Bloating: Results From the Rome Foundation Global Epidemiology Study

Abstract:Background & aims: Bloating is a common symptom in the general population and among disorders of gut-brain interaction, although its prevalence has not been well characterized. The aim of this study was to report the prevalence of bloating as a symptom in the worldwide population and to identify factors associated with this symptom in the general population. Methods: Rome Foundation Global Epidemiology Study internet survey data were analyzed. After excluding respondents with potential organic causes of bowel symptoms, the current analysis included 51,425 individuals in 26 countries. Data included diet, medical history, quality of life, and Rome IV diagnostic questions. Presence of bloating was defined as experiencing bloating at least once per week for the last 3 months. Descriptive statistics estimated prevalence by country, region, and disorder of gut-brain interaction diagnosis. Logistic regression evaluated predictors of bloating. Results: Nearly 18% of the global study population reported bloating (ranging from 11% in East Asia to 20% in Latin America). Prevalence decreased with age and women were approximately twice as likely as men to report bloating. More than one-half of respondents who reported weekly epigastric pain (71.39%), nausea (59.7%), or abdominal pain (61.69%) also reported bloating at least once per week. In logistic regression, the strongest associations were with abdominal pain (odds ratio, 2.90) and epigastric pain (odds ratio, 2.07). Conclusions: Bloating is common throughout the world. Nearly 18% of the general population experience bloating at least once per week. Reported bloating prevalence is lower in older age groups, most common in women, and strongly associated with abdominal pain.
