Abstract:Background Interstitial lung diseases (ILDs) are indispensable components of chronic respiratory diseases and global health challenges. We aimed to explore the global long-term changes in the prevalence, mortality, and disability-adjusted life years (DALYs) of ILDs; investigate the independent effect of age, period, and cohort; and project the disease burden over the next decade. Methods Data were retrieved from the Global Burden of Disease (GBD) database 2019. The joinpoint regression model was used to calculate the average annual percent change (AAPC). An age–period–cohort (APC) analysis was employed to measure the independent effect of age, period, and cohort. The Bayesian age–period–cohort (BAPC) model was used to project the global epidemiological trends until 2030. Results From 1990 to 2019, the age-standardized prevalence rate (ASPR), age-standardized mortality rate (ASMR), and age-standardized disability-adjusted life years (DALYs) rate (ASDR) of interstitial lung disease and pulmonary sarcoidosis (ILD) slightly increased from 52.66 per 100,000 [95% uncertainty interval (UI) 44.49 to 61.07] to 57.62 per 100,000 (95% UI 49.42 to 65.67), from 1.76 per 100,000 (95% UI 1.41 to 2.22) to 2.17 per 100,000 (95% UI 1.5 to 2.62), and from 41.57 per 100,000 (95% UI 33.93 to 51.92) to 46.45 per 100,000 (95% UI 35.12 to 54.98), whereas the ASPR, ASMR, and ASDR of pneumoconiosis decreased. High social-demographic index (SDI) regions possessed the highest ASPR, whereas low-middle SDI regions had the highest ASMR and ASDR, followed by low-SDI regions in ILD. Middle-SDI regions reported the highest ASPR, ASMR, and ASDR in pneumoconiosis. The age effect showed that the rate ratio (RR) was high in older adults. Period effect indicated that the RR of prevalence increased over time, whereas the RR of mortality and DALYs decreased in men but increased in women. The cohort effect exhibited that the more recent birth cohort had a higher RR than the previous cohort in prevalence. We projected that ASPR, ASMR, and ASDR would stabilize with little variation over the next decade. Conclusion The global burden of ILDs remains relatively severe, especially among older adults, in low- and middle-SDI regions. Effective measurements are expected to improve this situation.

Abstract:Objectives: We aim to investigate the contribution of Interstitial Lung Disease (ILD) to mortality in patients with Inflammatory Bowel Disease (IBD). Methods: We carried out a comprehensive retrospective, population-based epidemiological study across the United States from 2001 to 2020, using the Wide-ranging Online Data for Epidemiologic Research (WONDER) database. Mortality data were classified according to the International Classification of Diseases, Tenth Revision, with the codes J84 for ILD, K50 for Crohn's Disease (CD), and K51 for Ulcerative Colitis (UC). To discern patterns, Age-adjusted Mortality Rates (AMR) were computed, stratified by gender, geographic census region, and racial/ethnic demographics. Results: From 2001 to 2020, there were 57,967 reported deaths among IBD patients with an AMR per million significantly rising from 10.989 in 2001-2005 to 11.443 in 2016-2020 (p<0.0001). ILD was a contributor to death in 1.19% (692/57,967) of these cases, with AMR rising from 0.092 to 0.143 per million (p=0.010). The percentage of ILD-related deaths in the IBD population increased from 1.02% to 1.30% over two decades. ILD was a more common cause of death in patients with CD than with UC (54.6% vs. 45.4%), with a significant increase for both conditions from 2001 to 2020 (p<0.05). An upward trend in ILD-related mortality was observed in both sexes (p<0.05), and within the White population (p=0.010). Conclusions: The observed increase in mortality rates due to ILD among patients with IBD is concerning and highlights a critical need for systematic ILD screening protocols within the IBD patient population to facilitate early detection and management.

Variability in Global Prevalence of Interstitial Lung Disease

Abstract:There are limited epidemiologic studies describing the global burden and geographic heterogeneity of interstitial lung disease (ILD) subtypes. We found that among seventeen methodologically heterogenous studies that examined the incidence, prevalence and relative frequencies of ILDs, the incidence of ILD ranged from 1 to 31.5 per 100,000 person-years and prevalence ranged from 6.3 to 71 per 100,000 people. In North America and Europe, idiopathic pulmonary fibrosis and sarcoidosis were the most prevalent ILDs while the relative frequency of hypersensitivity pneumonitis was higher in Asia, particularly in India (10.7–47.3%) and Pakistan (12.6%). The relative frequency of connective tissue disease ILD demonstrated the greatest geographic variability, ranging from 7.5% of cases in Belgium to 33.3% of cases in Canada and 34.8% of cases in Saudi Arabia. These differences may represent true differences based on underlying characteristics of the source populations or methodological differences in disease classification and patient recruitment (registry vs. population-based cohorts). There are three areas where we feel addition work is needed to better understand the global burden of ILD. First, a standard ontology with diagnostic confidence thresholds for comparative epidemiology studies of ILD is needed. Second, more globally representative data should be published in English language journals as current literature has largely focused on Europe and North America with little data from South America, Africa and Asia. Third, the inclusion of community-based cohorts that leverage the strength of large databases can help better estimate population burden of disease. These large, community-based longitudinal cohorts would also allow for tracking of global trends and be a valuable resource for collective study. We believe the ILD research community should organize to define a shared ontology for disease classification and commit to conducting global claims and electronic health record based epidemiologic studies in a standardized fashion. Aggregating and sharing this type of data would provide a unique opportunity for international collaboration as our understanding of ILD continues to grow and evolve. Better understanding the geographic and temporal patterns of disease prevalence and identifying clusters of ILD subtypes will facilitate improved understanding of emerging risk factors and help identify targets for future intervention.

Abstract:Background: Interstitial lung disease (ILD) and pulmonary sarcoidosis are common respiratory disease with a heterogeneous distribution worldwide. In present study, we aimed to characterize the global disease burden of ILD and pulmonary sarcoidosis using data from the Global Burden of Disease Study 2019 (GBD2019).Methods: Crude and age-standardized rates (ASRs) of incidence, mortality, and DALYs for ILD and pulmonary sarcoidosis were obtained from GBD 2019. We used GBD methodology to systematically analyze the temporal and geographical trends of ILD and pulmonary sarcoidosis in 204 countries and territories from 1990 to 2019, by sex, socio-demographic index (SDI), and region.Findings: In 2019, the global incident cases due to ILD and pulmonary sarcoidosis was 24,232,080 (95% UI, 19,609,750 ~ 29,463,387), mortality case due to ILD and pulmonary sarcoidosis was 169,833 (95% UI, 118,756 ~204,802). From 1990 to 2019, the incident, death, and DALYs case number of ILD and pulmonary sarcoidosis increased by 118·6%, 166·63%, and 122·87% respectively. The global incidence of ILD and pulmonary sarcoidosis was higher in males, and it was mainly concentrated in the age group of 70~79 years in both sexes. Significant regional differences can be seen in the incidence of ILD and pulmonary sarcoidosis.Interpretation: Our study shows that the ASRs of incidence, mortality, and DALYs for ILD and pulmonary sarcoidosis are increasing. Abstract:Background There are limited data regarding the relationship between interstitial lung disease (ILD) and the natural course of COVID-19. In this study, we investigate whether patients with ILD are more susceptible to COVID-19 than those without ILD and evaluate the impact of ILD on disease severity in patients with COVID-19. Methods A nationwide cohort of patients with COVID-19 (n=8070) and a 1:15 age-, sex- and residential area-matched cohort (n=121 050) were constructed between 1 January 2020 and 30 May 2020 in Korea. We performed a nested case–control study to compare the proportions of patients with ILD between the COVID-19 cohort and the matched cohort. Using the COVID-19 cohort, we also evaluated the risk of severe COVID-19 in patients with ILD versus those without ILD. Results The proportion of patients with ILD was significantly higher in the COVID-19 cohort than in the matched cohort (0.8% versus 0.4%; p<0.001). The odds of having ILD was significantly higher in the COVID-19 cohort than in the matched cohort (adjusted OR 2.02, 95% CI 1.54–2.61). Among patients in the COVID-19 cohort, patients with ILD were more likely to have severe COVID-19 than patients without ILD (47.8% versus 12.6%), including mortality (13.4% versus 2.8%) (all p<0.001). The risk of severe COVID-19 was significantly higher in patients with ILD than in those without ILD (adjusted OR 2.23, 95% CI 1.24–4.01). Conclusion The risks of COVID-19 and severe presentation were significantly higher in patients with ILD than in those without ILD.

Abstract:Background: Optimal diagnosis and management of interstitial lung diseases (ILDs) needs access to specialized centers, frequent monitoring, and complex therapeutic options. In underprivileged areas, these necessities can often lead to barriers in delivering care. Research question: What are the ILD mortality disparities in the regions along the US-Mexico (US-MX) border? Study design and methods: We obtained ILD mortality information through death certificate queries from the Centers for Disease Control and Prevention repository. Death data were adjusted for age and stratified by US-MX border regions and nonborder regions in the United States. Log-linear regression models were used to analyze mortality trends in the period from 1999 to 2020 followed by calculation of annual percentage changes (APCs). Age-adjusted mortality rates (AAMRs) were compared across cumulative and subdemographic populations. Results: ILD-related mortality among border regions (AAMR, 5.31) was higher than nonborder regions (AAMR, 4.86). Mortality within border regions remained unchanged from 1999 to 2020 (APC, 0.3; P = .269). Nonborder regions experienced a significant rise in mortality rates (APC, 2.6; P = .017) from 1999 to 2005 and remained unchanged from 2005 to 2020. Mortality was higher within both men (AAMR, 6.57) and women (AAMR, 4.36) populations among border regions compared with their nonborder counterparts (AAMR, 6.27 and 3.87, respectively). Hispanic populations among the border regions experienced higher mortality rates (AAMR, 6.15) than Hispanic populations within nonborder regions (AAMR, 5.44). Non-Hispanic populations encountered similar mortality rates between the two regions. Mortality rates among Hispanic (APC, 0.0; P = .938) and non-Hispanic (APC, 0.2; P = .531) populations in the border regions remained unchanged from 1999 to 2020. Interpretation: These results revealed ILD-related mortality disparities among the US-MX border regions, emphasizing the importance of public health measures to increase access to equitable medical care and implement targeted interventions among these vulnerable populations.
Malignancies in Patients with Interstitial Lung Diseases: A Single Center Observational Study

Abstract:Objective: Current studies focus on the prevalence rate of lung cancer in idiopathic pulmonary fibrosis and connective tissue disease-associated interstitial lung disease (CTD-LID). Our aim was to investigate the prevalence of malignancies in patients with various subtypes of ILD. Methods: A total of 5350 patients diagnosed with ILD between January 2015 and December 2021 were retrospectively included. The prevalence of different malignancies and different ILDs was assessed using complete follow-up data. Results: A total of 248 patients (139 males; 65—IQR, 57 to 72—years) out of 5350 patients with ILD were confirmed with malignancies. A total of 69% of patients with ILD and malignances were older than 60 years old. The prevalence of malignancies in ILD patients was 4.6%, and lung cancer had the most common incidence of 1.9%, followed by malignancies in the digestive system of 0.9%. Among the different ILD subtypes, the prevalence of malignancies such as organizing pneumonia (OP), idiopathic pulmonary fibrosis (IPF), anti-neutrophil cytoplasmic antibodies-associated vasculitis-related ILD(AAV-ILD), nonspecific interstitial pneumonia (NSIP), CTD-ILD, hypersensitivity pneumonitis (HP), sarcoidosis, and other types of ILD was 6.8%, 5.0%, 4.7%, 4.3%, 2.5%, 2.2%, 1.2%, and 6.9%, respectively. The incidence of lung cancer as the most common tumor in IPF was 3.9%, with adenocarcinoma predominating (1.7%). The highest rate of malignancy occurring in RA of CTD-ILD was 2.4%. Conclusion: Older patients with ILD (≥60 years) including OP, IPF, AAV-ILD, NSIP, CTD-ILD, and HP, were associated with a higher incidence of malignancy, especially males aged from 60 to 69 years. These epidemiological results indicate that it is essential for physicians to pay more attention to the screening of and management strategies for different malignancies, according to the specific ILD subtypes.

Abstract:Background: Interstitial lung diseases (ILD) are a heterogenous group of diseases, which have pulmonary fibrosis, restrictive lung disease, and decreased diffusion capacity as a common final path. Premature death frequently results not from ILD itself but from comorbidities. Peripheral artery disease (PAD) is a common comorbid disease in different chronic lung diseases. The focus of the present study is to clarify the prevalence of PAD in ILD. Patients and methods: A total of 97 patients with ILD and 30 controls were included in the study. Patients with ILD were subdivided into two groups according to the progression of pulmonary fibrosis: progressive fibrosing and non-progressive fibrosing ILD (PF-ILD and nPF-ILD, respectively). All participants underwent standard angiological and pneumological diagnostic procedures including six-minute walking test, measurement of ankle-brachial-index, and colour-coded duplex sonography. Results: We observed no relevant differences in the baseline characteristics except age. Both, PF-ILD and nPF-ILD patients, presented with a highly increased incidence of atherosclerotic lesions compared to the control group (p<0.001). PAD was present in all patients with PF-ILD and in 73% of patients with nPF-ILD. These results were confirmed by age-adjusted regression analyses. Conclusions: The present results indicate that ILD is an independent risk factor for atherosclerosis. Patients with PF-ILD are more severely affected than nPF-ILD patients with age as a confounding variable. Atherogenesis in ILD may be mediated by increased cardiovascular risk, systemic inflammation and chronic hypoxemia.

Abstract:Interstitial lung diseases (ILDs) refer to a heterogeneous and complex group of conditions characterized by inflammation, fibrosis, or both, in the interstitium of the lungs. This results in impaired gas exchange, leading to a worsening of respiratory symptoms and a decline in lung function. While the etiology of some ILDs is unclear, most cases can be traced back to factors such as genetic predispositions, environmental exposures (including allergens, toxins, and air pollution), underlying autoimmune diseases, or the use of certain medications. There has been an increase in research and evidence aimed at identifying etiology, understanding epidemiology, improving clinical diagnosis, and developing both pharmacological and non-pharmacological treatments. This review provides a comprehensive overview of the current state of knowledge in the field of interstitial lung diseases.

Abstract:Interstitial lung disease is a common pulmonary manifestation of connective tissue diseases and results in significant morbidity and mortality. Although all connective tissue diseases are linked by underlying autoimmunity, there is significant variability in the presentation and clinical course of interstitial lung disease associated with the different types of connective tissue diseases. In this paper, we review the prevalence, patterns, predictors, and prognosis of interstitial lung disease secondary to the most common forms of connective tissue diseases. We have also highlighted the available evidence regarding treatment options for interstitial lung disease due to different connective tissue diseases.

Work-related interstitial lung disease: what is the true burden?

Abstract:Interstitial lung diseases (ILDs) include a large variety of fibrotic lung conditions caused by genetic and environmental factors. Occupational exposures might also play a significant role, but the real health burden is currently unknown. Here, we aim to evaluate the role of work-related exposures in ILDs, focussing on idiopathic pulmonary fibrosis (IPF) and hypersensitivity pneumonitis (HP). We performed a focused review of the literature on work-related HP and IPF over the past 5 years. Using a meta-analytic approach, we quantified the occupational burden of IPF and HP, and estimated that occupational exposures to metal, silica and environmental tobacco smoke increased IPF risk with a pooled odds ratio of 1.7 (95% CI 1.42–2.03). The proportion of HP cases related to workplace exposure was 17% (95% CI 7–28). Our review supports the hypothesis that occupational exposures are a significant risk factor in the aetiopathogenesis of IPF and HP. We recommend that further research be performed to identify the underlying occupational factors and the maximum permitted exposure to reduce the associated IPF and HP burden.

Abstract:KEYWORDS: Interstitial lung diseases (ILDs) include more than 100 respiratory rare diseases that affect the lung parenchyma, with a variable component of inflammation and/or fibrosis, which leads to progressive dyspnea and respiratory failure [ Citation 1–10 ]. Pulmonary fibrosis is present in several ILDs, which is irreversible by definition and may progress independently of the origin. Idiopathic pulmonary fibrosis (IPF) is the most lethal and frequent fibrotic ILD. However, many other non-IPF fibrotic ILDs may present a similar disease behavior to IPF despite the initial treatment, which is currently known as progressive pulmonary fibrosis (PPF) [ Citation 1 , Citation 2 ]. The most frequent symptoms and signs in IPF and other fibrotic ILDs include dyspnea, dry cough, and inspiratory crackles, and, in some cases, finger clubbing [ Citation 2 , Citation 3 ]. Natural history differs among patients but involves a decline in lung function that worsens quality of life and eventually leads to death. It occurs in middle-aged and older adults, and the estimated mean survival before the use of anti-fibrotic drugs in IPF was 3–5 years from diagnosis [ Citation 4 ]. The most common non-IPF fibrotic ILDs include fibrotic hypersensitivity pneumonitis (F-HP), unclassifiable ILD (uILD), connective tissue disease associated ILD (CTD-ILD), smoking related interstitial fibrosis (SRIF), and fibrotic nonspecific interstitial pneumonia (fNSIP), although less frequently other ILDs such as sarcoidosis and pneumoconiosis may also progress to the fibrotic progressive form [ Citation 11 , Citation 12 ] (, ). Although the advances in the diagnostic and therapeutic approaches have improved patient survival and prognosis, the delay in diagnosis remains frequent and one of the main unmet patient needs.

Abstract:Introduction There is a growing body of evidence suggesting a causal relationship between interstitial lung disease (ILD) and air pollution, both for the development of the disease, and driving disease progression. We aim to provide a comprehensive literature review of the association between air pollution, and ILD, including idiopathic pulmonary fibrosis (IPF). Methods We systematically searched from six online database. Two independent authors (DL and CF) selected studies and critically appraised the risk of bias using the Newcastle-Ottawa Scale (NOS). Findings are presented through a narrative synthesis and meta-analysis. Meta-analyses were performed exclusively when there was a minimum of three studies examining identical pollutant-health outcome pairs, all evaluating equivalent increments in pollutant concentration, using a random effects model. Results 24 observational studies conducted in 13 countries or regions were identified. Pollutants under investigation encompassed ozone (O 3 ), nitrogen dioxide (NO 2 ), Particulate matter with diameters of 10 micrometers or less (PM 10 ) and 2.5 micrometers or less (PM 2.5 ), sulfur dioxide (SO 2 ), carbon monoxide (CO), nitric oxide (NO) and nitrogen oxides (NOx). We conducted meta-analyses to assess the estimated Risk Ratios (RRs) for acute exacerbations (AE)-IPF in relation to exposure to every 10 μg/m 3 increment in air pollutant concentrations, including O 3 , NO 2 , PM 10 , and PM 2.5. The meta-analysis revealed a significant association between the increased risk of AE-IPF in PM 2.5 , yielding RR 1.94 (95% CI 1.30–2.90; p = 0.001). Findings across all the included studies suggest that increased exposure to air pollutants may be linked to a range of health issues in individuals with ILDs. Conclusion A scarcity of available studies on the air pollutants and ILD relationship underscores the imperative for further comprehensive research in this domain. The available data suggest that reducing levels of PM 2.5 in the atmosphere could potentially reduce AE frequency and severity in ILD patients.

Interstitial lung disease in the elderly: pathogenesis, diagnosis and management

Abstract:Advancing age is associated with increased risk for some forms of interstitial lung disease (ILD), and this risk is especially reflected by the considerably increased incidence of idiopathic pulmonary fibrosis (IPF) in the elderly. Although the causes of this increased risk are not well-defined, both ageing and IPF have been associated with shortening of telomeres due to telomerase deficiency. Thoracic imaging with high-resolution computed tomographic (HRCT) scanning plays a key role in the diagnosis of ILD in the elderly, and a characteristic appearance of the lung parenchymal changes on HRCT may provide a confident diagnosis and obviate the need for invasive testing such as surgical lung biopsy. An effective treatment for IPF remains elusive, but many patients will benefit from supportive care and treatment of various co-morbid conditions that are often found in patients with IPF.

Abstract:Background: Interstitial Lung Diseases (ILD) present a diagnostic challenge due to their nonspecific symptomatology, often leading to delayed diagnosis. This delay can significantly impact patient outcomes, necessitating an in-depth examination of diagnostic timelines in ILD cases. Materials and Methods: A retrospective study was conducted using data from our Hospital of Pulmonary Diseases. We reviewed 455 cases (2013-2020) with a primary discharge diagnosis of ILD (ICD-10 J84.X). The analysis focused on prior hospital admissions, the frequency of non-ILD diagnoses, and the patterns of misdiagnosis. The criteria included examining the number of patients previously admitted without an ILD diagnosis and identifying commonly misattributed conditions. Results: Our findings reveal that 33% (150) of ILD patients had at least one hospital presentation before receiving their diagnosis, while 26% (118) had two or more. Notably, 38% received at least one different diagnosis, and 48% had two or more disparate diagnoses in previous hospitalizations. The most frequent incorrect diagnoses were lower respiratory tract infections (80%), followed by asthma (11%), and bronchiectasis (9%). Results and Discussion: The data underscores a significant delay in ILD diagnosis, often due to unspecific symptoms and the lack of disease recognition. This delay can lead to inappropriate treatments and a potential worsening of patient outcomes, highlighting the need for increased awareness and a more systematic approach in evaluating patients presenting with non-specific respiratory symptoms.

Abstract:Lung cancer continues to be one of the leading causes of cancer-related death worldwide. There is evidence of a complex interplay between lung cancer and interstitial lung disease (ILD), affecting disease progression, management strategies, and patient outcomes. Both conditions develop as the result of common risk factors such as smoking, environmental exposures, and genetic predispositions. The presence of ILD poses diagnostic and therapeutic challenges in lung cancer management, including difficulties in interpreting radiological findings and increased susceptibility to treatment-related toxicities, such as acute exacerbation of ILD after surgery and pneumonitis after radiation therapy and immunotherapy. Moreover, due to the lack of large, phase III randomized controlled trials, the evidence-based therapeutic options for patients with ILDs and lung cancer remain limited. Antifibrotic treatment may help prevent pulmonary toxicity due to lung cancer treatment, but its effect is still unclear. Emerging diagnostic modalities and biomarkers and optimizing personalized treatment strategies are essential to improve outcomes in this patient population.

Abstract:Interstitial lung diseases (ILD) are a group of heterogeneous progressive pulmonary disorders, characterised by tissue remodelling and/or fibrotic scarring of the lung parenchyma. ILD patients experience lung function decline with progressive symptoms, poor response to treatment, reduced quality of life and high mortality. ILD can be idiopathic or associated with systemic or connective tissue diseases (CTD) but idiopathic pulmonary fibrosis (IPF) is the most common form. While IPF has a male predominance, women are affected more greatly by CTD and therefore associated ILDs. The mechanisms behind biological sex differences in these progressive lung diseases remain unclear. However, differences in environmental exposures, variable expression of X-chromosome related inflammatory genes and sex hormones play a role. Here, we will outline sex-related differences in the incidence, progression and mechanisms of action of these diseases and discuss existing and novel cellular and pre-clinical studies. Furthermore, we will highlight how sex-differences are not adequately considered in pre-clinical disease models, how gender bias exists in clinical diagnosis and how women are underrepresented in clinical trials. Future action on these observations will hopefully shed light on the role of biological sex in disease development, identify potential targets for intervention and increase female participant numbers in clinical trials.

Abstract:KEYWORDS: Interstitial lung diseases (ILDs) encompass a heterogeneous group of lung disorders, each with its own etiology, clinical presentation, and progression, characterized by inflammation and fibrosis, affecting over 4.5 million people worldwide () [ Citation 1 , Citation 2 ]. ILDs can be idiopathic (i.e. of unknown cause) or secondary to other conditions such as connective tissue diseases, environmental exposures or medication use. A hallmark feature of many ILDs is the progressive fibrosis of the interstitium surrounding the alveoli, resulting in impaired oxygen exchange and a range of debilitating symptoms. ILDs can be challenging to diagnose and manage due to their heterogeneity, with over 200 distinct types having been identified to date [ Citation 1 ]. Most ILDs have a poor prognosis, with a life expectancy of around 3–5 years after diagnosis [ Citation 3 ]. Pulmonary arterial hypertension (PAH) is a significant risk factor and patients with untreated PAH usually rapidly succumb to respiratory failure within 2–3 years after diagnosis [ Citation 3 ].
