Abstract:The patients aged ≥ 60 years account for 70％ of Chinese population with colorectal cancer.However,with the increase of age,the treatment methods such as surgery,radiotherapy,chemotherapy become inappropriate and impeding factors for the self-care of the elderly.Several clinical trials show that only a part of the elderly,who are in good health condition,can get the clinical benefits from these therapies.Therefore,this review focused on the comprehensive treatments for elderly patients with colorectal cancer,then reached the best individualized treatment,so as to improve the survival and quality of life.

Abstract:Objective: To retrospectively evaluate the safety and feasibility of endoscopic submucosal dissection (ESD) for the treatment of early gastric cancer (EGC) in elderly patients >= 75 years of age. Patients and Methods: One hundred seventy-one patients (187 lesions) treated with ESD from January 2010 to September 2013 were enrolled in our study. Subjects were classified into two groups: elderly (age >= 75 years) or non-elderly (age <75 years). Clinicopathological characteristics, resectability, curability, complications, rates of local recurrence, and residual disease were evaluated. Association of clinicopathological characteristics of the lesions with immediate bleeding was analyzed. Results: No significant differences in clinical characteristics were observed. The incidences of comorbidity were significantly different between the elderly group and the non-elderly group (P < .001). Of the elderly patients, 54.3% had two or more underlying diseases compared with 18.4% of the non-elderly patients (P < .001). Of the 98.0% of elderly patients and 97.1% of younger patients who received en bloc resection, curative resection reached 94.1% and 96.3%, respectively. Immediate bleeding occurred in 15.2% of the elderly group and 4.8% of the non-elderly group (P = .044). Operation time differed significantly (P = .039). No apparent discrepancy was observed in perforation and delayed bleeding. The differences in the invasion depth were considered between the two groups (P = .001). Logistic regression analysis revealed that the rate of immediate bleeding was associated with invasion depth (P = .003). There were no differences in the follow-up period and rates of local recurrence and residual disease. Conclusions: ESD is safe and feasible for elderly patients in the era of a graying population. Higher risk of immediate bleeding and longer operation time should be concerned.

Abstract:Colorectal cancer (CRC) is one of the leading causes of cancer-related deaths worldwide. As society ages, the number of elderly patients with CRC will increase. The percentage of patients with right-sided colon cancer and the incidence of microsatellite instability are higher in elderly than in younger patients with CRC. Moreover, the higher incidence of comorbid diseases in elderly patients indicates the need for less invasive treatment strategies. For example, care should be taken in performing additional surgery after endoscopic submucosal dissection for elderly patients with high-risk T1 CRC. Minimally invasive surgery, such as laparoscopic colectomy, would be preferable for elderly patients with CRC. Chemotherapy for elderly patients requires careful monitoring for adverse events. The aim of this review is to summarize the clinicopathological features of CRC in elderly patients, optical surgical strategies, including endoscopic and laparoscopic resection, and chemotherapeutic strategies, including postoperative adjuvant chemotherapy and systemic chemotherapy for unresectable CRC.

Abstract:Colorectal cancer (CRC) is predominantly a disease of older persons, who make up more than 70% of CRC patients. To improve the outcomes of CRC, we should pay more attention to the treatment of older patients. As we know, both surgery and chemotherapy decline rapidly with age. The findings of recent studies have disputed the previously held belief that older patients do worse after bowel surgery or chemotherapy. Evidence supports the safety and efficacy of both adjuvant and palliative chemotherapy in fit old patients. Elderly CRC patients with age-related co-morbidity can still benefit from optimal strategies.Therefore,we still have a lot of work to do to improve the overall survival and time to tumor recurrence of elderly CRC patients. Laparoscopic colonic resection is a safe and effective surgical technique in elderly patients. We look forward to the development of Natural Orifice Transluminal Endoscopic Surgery (NOTES),which will benefit the elderly CRC patients in future.

Abstract:The incidence of CRC increases sharply with age. Complicated CRC is a predominant disease in elderly patients. Emergency presentation in the elderly is associated with higher rates of advanced disease. Risk of emergency admission is closely linked to advanced age, comorbidities, and well-identified socioeconomic factors. Colorectal resection for CRC in the elderly is a procedure that is frequently performed in an emergency situation and with important pre-existing comorbidity and reduced physiologic reserve. Complicated right-sided colon cancer is the most frequent indication for nonelective colorectal resection in the octogenarian. Frequently, in the setting of obstruction, perforation, or bleeding, colectomy is a lifesaving treatment and is associated with postoperative mortality up to 48% and morbidity up to 77%. Morbidity and mortality increase with age, comorbidities, and physical frailty. Elderly CRC patients undergoing resection have a greatly increased 1-year mortality risk, as well as worst mid- and long-term survival compared to elective patients of the same age. Chronological age should not be a contraindication for radical surgery, and a curative resection should be performed whenever allowed by disease stage or patient physical status. Palliative treatment may be necessary in up to a third of over 80 patients with complicated cancer.
Changing Trends in Gastric and Colorectal Cancer among Surgical Patients over 85 Years Old: A Multicenter Retrospective Study, 2001–2021

Abstract:BACKGROUND Whether patients over 85 years old with gastrointestinal cancer should undergo surgery remains controversial. We aimed to describe the changing trends of characteristics to provide more information to decision makers, and strive to find appropriate surgical plan.AIM To describe the changing trends of characteristics to provide more information to decision makers, and strive to find appropriate surgical plan.METHODS A total of 218 gastric cancer (GC) patients and 563 colorectal cancer (CRC) patients who underwent surgery between 2001 and 2021 were enrolled in this retrospective analysis. Changes in clinicopathological features, surgical treatments, and survival status were analyzed longitudinally at 5-year intervals.RESULTS Only 14 GC patients underwent laparoscopic surgery where 219 CRC patients had this procedure. Cardia and esophagogastric junction cancer increased in GC patients, and the proportion of sigmoid colon cancer decreased in CRC patients. Pulmonary infection gradually became the most common postoperative complication, its incidence in period 4 reached 48.79%. However, the incidence of anastomotic leakage decreased from 26.79% to 9.38% (P < 0.01). Additionally, 30-d mortality significantly decreased from 32.14% to 9.01%. Increases were observed in 5-year overall survival (OS) in GC patients from period 1 to period 4 (18.18% vs 33.32%, respectively) and CRC patients (0 vs 36.32%, respectively). Disease-free survival (DFS) also increased in GC and CRC patients (7.14% vs 27.74% and 0 to 36.03%, respectively). The average survival time of GC patients following radial lymphadenectomy was higher than in patients that underwent limited lymphadenectomy (26 vs 22 mo, respectively), the same was seen in CRC patients (44 vs 33 mo, respectively). This advantage was particularly evident in patients with TNM I, but not in patients with TNM II/III period cancer.CONCLUSION The safety as well as effectiveness of surgery in ultra-elderly patients is increasing. Radical lymphadenectomy has advantages in patients with TNM I gastrointestinal cancer, but not TNM II/III.

Abstract:Six out of every 10 new colorectal cancer (CRC) diagnoses are in people over 65 years of age. Current standardized surgical approaches have proved to be tolerable on the elderly population, although post-operative complications are more frequent than in the younger CRC population. Frailty is common in elderly CRC patients with surgical indication, and it appears to be also associated with an increase of post-operative complications. Fast-track pathways have been developed to assure and adequate post-operative recovery, but comprehensive geriatric assessments (CGA) are still rare among the preoperative evaluation of elderly CRC patients. This review provides a thorough study of the effects that a CGA assessment and a geriatric intervention have in the prognosis of CRC elderly patients with surgical indication.

Abstract:Endometrial cancer incidence and related mortality are on the rise due to aging demographics. This population often presents with unfavorable features, such as myometrial invasion, non-endometrioid histology, high-grade tumors, worse prognosis, etc. The role of age as an independent prognostic factor is still debated, and screening tools addressing frailty emerge as pivotal in guiding treatment decisions; however, they are still underutilized. Treatment disparities are evident in the case of older patients with endometrial cancer, who frequently receive suboptimal care, hindering their survival. Radiotherapy and minimally invasive surgical approaches could be performed in older patients. Data on chemotherapy and immunotherapy are scarce, but their potential remains promising and data are being gathered by recent trials, contingent on optimal patient selection through geriatric assessments. Overall, we recommend personalized, screening tool-guided approaches, adherence to guideline-recommended treatments, and inclusion of older people in clinical trials to help identify the best course of treatment.

Abstract:Background The effectiveness of endoscopic submucosal dissection (ESD) has been increasingly reported. However, studies addressing the safety and application value of ESD in elderly patients with early gastric cancer (EGC) were still lacking. This meta-analysis was intended to evaluate the feasibility and safety of ESD in elderly patients with EGC. Methods A systematic search was conducted in PubMed, EBSCO, Cochrane Library, EMBASE, and Web of Science. Studies were screened out if data of elderly and non-elderly gastric cancer patients were reported separately. The qualities of included studies were assessed using Newcastle-Ottawa Quality Assessment Scale. The pooled odd ratios (ORs) with 95 % confidence intervals (CIs) were calculated. Statistical analysis was conducted using the Review Manager 5.2 (Cochrane Collaboration, Oxford, UK). Results Nine studies (eight in Japan, one in China), including a total of 30,100 lesions, met the inclusion criteria. The “en bloc” and histological complete resection rates of the elderly and non-elderly groups were similar [OR, 0.98, 95 % CI, 0.56 to 1.71; P = 0.93 and OR, 0.79, 95 % CI, 0.58 to 1.07; P = 0.13, respectively]. As for procedure-related complications, similar perforation rates [OR, 1.19, 95 % CI, 0.94 to 1.51; P = 0.15], and bleeding rates [OR, 1.13, 95 % CI, 0.83 to 1.56); P = 0.43] between the elderly and non-elderly groups were observed. Whereas, the elderly patients had a higher procedure-related pneumonia rate compared with non-elderly ones [OR, 2.18, 95 % CI, 1.55 to 3.08; P < 0.01]. Conclusions The ESD procedure appears to be a safe technique in elderly patients with EGC while appropriate approach should be taken to avoid procedure-related pneumonia.
The efficacy and safety of colonoscopy in nonagenarians: A multicenter study

Abstract:Introduction: The number of colonoscopy (CS) for the elderly is increasing. There are only a few reports focusing on CS among the very elderly aged ≥ 90-y. We aimed to analyze the efficacy of CS and of colorectal cancer (CRC) for patients aged ≥ 90-y. Methods: We retrospectively analyzed consecutive patients aged ≥ 90-y receiving CS at eight institutions from October 2016 to September 2017. Bowel preparation, complications, and endoscopic diagnosis were analyzed. The non-elderly group aged between 50-y and 64-y and elderly group aged between 65-y and 79-y were compared to very-elderly group aged ≥ 90-y. Through propensity score matching of sex and CS indications (symptomatic or asymptomatic), the number of CRC and the treatment in each group were analyzed. Results: We analyzed 125 patients receiving 154 colonoscopies (0.9%) in the very-elderly group from among 16,968 cases. Among 92 cases who received bowel-cleansing solution, good preparations were achieved in 94.5%. The rate of CS-related complications was 1.3% (2/154). The rate of CRC in the very-elderly group was 27.2% (34/125), higher than the non-elderly group (7.2%, 9/125, p < 0.01) and elderly group (8.8%, 11/125, p < 0.01). Therapeutic interventions for CRC in the very-elderly group were performed in 73.5% (24/34) patients. The mean survival of 12 patients with CRC resection was 788 days. Conclusions: CS could be performed safely for the very elderly aged ≥ 90-y with careful considerations. CRC was confirmed to be more frequent in this group with over 70% of patients receiving appropriate therapeutic intervention.

Abstract:<h3 class="Heading">Purpose</h3><p class="Para">There is no consensus on the use of cetuximab in elderly patients with metastatic colorectal cancer. To this end, a survey was carried in 17 Italian oncology centers.</p><h3 class="Heading">Methods</h3><p class="Para">The centers answered a 29-item questionnaire structured as follows: (i) demographic characteristics; (ii) medical history; (iii) assessment of RAS/BRAF mutations and DPD/UGT polymorphism before treatment; (iv) treatment schemes and side effects; (v) geriatric assessment and customization of treatment.</p><h3 class="Heading">Results</h3><p class="Para">One-third of patients are over 80 years old. The RAS/BRAF mutational status is not primarily evaluated by 17.6% of the centers, while DPD and UGT polymorphism is not evaluated by 82.4% and 76.5% of the centers. The most common therapeutic scheme is cetuximab/FOLFIRI and diarrhea is the main cause of suspension/reduction of treatment. The 70% of centers use systemic tetracyclines for skin toxicity. The 23.5% of the centers do not carry out any geriatric evaluation before the start of the therapy and those who perform it prefer the G8 (70.6%) and VES-13 (29.4%) scales.</p><h3 class="Heading">Conclusions</h3><p class="Para">Greater efforts should be made to improve the evaluation of the patient both about mutational and genetic procedures with geriatric evaluation. As for cetuximab in elderly patients, randomized studies are needed to provide guidance to physicians.</p>

Palliative Primary Tumor Resection in Elderly and Senile Patients with Metastatic Colorectal Cancer

Abstract:Late diagnosis of colorectal cancer (CRC), along with a high proportion of older patients, explain the relevance of studying the efficiency of cytoreductive surgery in geriatric patients with colorectal cancer. A comparative evaluation of the results of cytoreductive operations (removal of the primary tumor) in two groups of patients with disseminated CRC: younger and over 60 years old. It is established that the implementation of palliative resections in elderly and senile patients allows achieving encouraging long-term results, providing higher survival rates than in patients of young and middle age. More frequent development of complications after cytoreductive operations in patients of older age groups is usually associated with decompensation of the concomitant pathology, which requires its adequate correction in the preoperative period. The use of cytoreductive surgery in patients of older age groups with advanced forms of CRC significantly improves their quality of life.
Surgical Treatment for Colorectal Cancer in the Elderly Patients over 75 Years Old:An Analysis of 266 Cases

Abstract:Objective To analyze the clinical and pathological characteristics,surgical complications and prognosis of colorectal cancer (CRC) in the elderly patients,and discuss the surgical treatment for them.Methods The clinical and pathological data of 266 CRC patients over 75 years old accepted surgical treatment between January 1985 and December 2003 in Cancer Hospital of Fudan University were analyzed retrospectively.Results Among 266 patients,the age of 151 males and 115 females ranged from 75 to 91 years old.The rectum and sigmoid colon were the most popular sites in the elderly patients (69.2%),but still there were 22.2% of patients with tumors located in the right site of colon.Ninety-four patients (35.3%) had complicated diseases before operation,such as cardiovascular disease and diabetes mellitus.Two hundreds and fifteen patients (80.8%) accepted radical excision while 35 patients (13.2%) accepted palliative resection.Thirty patients (11.3%) had postoperative complications,such as intestinal obstruction,haemorrhage,infection and so on. Among them,3 patients (1.1%) died several days after operation.The overall 5-year survival rate was 51.3%.Conclusion Though the elderly CRC patients have more complicated diseases before operation and more postoperative complications,the operative mortality is still lower if much more attentions are paid during operation.Also they have the 5-year survival rate more than 50%.So the surgical treatment shouldn’t be ignored in the elderly CRC patients.

Abstract:Colorectal cancer (CRC) poses a significant global health challenge. Notably, the risk of CRC escalates with age, with the majority of cases occurring in those over the age of 65. Despite recent progress in tailoring treatments for early and advanced CRC, there is a lack of prospective data to guide the management of older patients, who are frequently underrepresented in clinical trials. This article reviews the contemporary landscape of managing older individuals with CRC, highlighting recent advancements and persisting challenges. The role of comprehensive geriatric assessment is explored. Opportunities for treatment escalation/de-escalation, with consideration of the older adult's fitness level. are reviewed in the neoadjuvant, surgical, adjuvant, and metastatic settings of colon and rectal cancers. Immunotherapy is shown to be an effective treatment option in older adults who have CRC with microsatellite instability. Promising new technologies such as circulating tumor DNA and recent phase III trials adding later-line systemic therapy options are discussed. Clinical recommendations based on the data available are summarized. We conclude that deliberate efforts to include older individuals in future colorectal cancer trials are essential to better guide the management of these patients in this rapidly evolving field.
Colorectal cancer in the elderly—surgical treatment and long-term survival

Abstract:Background and aimsThe prognosis of radical treatment for colorectal cancer in elderly patients has been subject of controversies. The aim of this study was to compare patients at the age of 75 years or older with a group of younger patients, focused on the clinicopathologic characteristics and the results of radical treated colorectal cancer.Patients and methodsA retrospective study was made to evaluate age-related surgical risk and outcome. The following criteria were analyzed in two age groups (<75 years and ≥75 years): comorbidities, tumor characteristics, type of resection, postoperative morbidity and mortality, recurrence rate, overall survival, cancer-related survival, and disease-free survival.ResultsAltogether, 517 patients were included into the study. Gender, ASA risk score, frequency of concomitant comorbidities, and tumor location differed significantly between the two age groups. Tumor characteristics were equal between the two groups. There were no differences in 30-day morbidity except in postoperative bleeding, but 30-day mortality was higher in the older age group. Mean time of follow-up was approximately 32 months. Frequencies for adjuvant, as well as for palliative (radio-) chemotherapy were lower in the older group. While cancer-related survival was lower in the higher age group, there were no differences in disease-free survival.ConclusionThe age of patients does not seem to be a prognostic factor for perioperative results; furthermore, the long-term results rather depend on the stage of disease and on adjuvant or palliative treatment, respectively, than on age.

Abstract:Background and aims: Although rectal cancer is predominantly a disease of older patients, current guidelines do not incorporate optimal treatment recommendations for the elderly and address only partially the associated specific challenges encountered in this population. This results in a wide variation and disparity in delivering a standard of care to this subset of patients. As the burden of rectal cancer in the elderly population continues to increase, it is crucial to assess whether current recommendations on treatment strategies for the general population can be adopted for the older adults, with the same beneficial oncological and functional outcomes. This multidisciplinary experts' consensus aims to refine current rectal cancer-specific guidelines for the elderly population in order to help to maximize rectal cancer therapeutic strategies while minimizing adverse impacts on functional outcomes and quality of life for these patients. Methods: The discussion among the steering group of clinical experts and methodologists from the societies' expert panel involved clinicians practicing in general surgery, colorectal surgery, surgical oncology, geriatric oncology, geriatrics, gastroenterologists, radiologists, oncologists, radiation oncologists, and endoscopists. Research topics and questions were formulated, revised, and unanimously approved by all experts in two subsequent modified Delphi rounds in December 2020-January 2021. The steering committee was divided into nine teams following the main research field of members. Each conducted their literature search and drafted statements and recommendations on their research question. Literature search has been updated up to 2020 and statements and recommendations have been developed according to the GRADE methodology. A modified Delphi methodology was implemented to reach agreement among the experts on all statements and recommendations. Conclusions: The 2021 SICG-SIFIPAC-SICE-WSES consensus for the multidisciplinary management of elderly patients with rectal cancer aims to provide updated evidence-based statements and recommendations on each of the following topics: epidemiology, pre-intervention strategies, diagnosis and staging, neoadjuvant chemoradiation, surgery, watch and wait strategy, adjuvant chemotherapy, synchronous liver metastases, and emergency presentation of rectal cancer.

Abstract:IntroductionColon cancer therapy is primarily surgical. Advanced age does not represent a contraindication to surgery. We analyse the results of surgery in ultra 75 patients undergoing surgery for colorectal cancer by examining the correlation between the comorbidity and any post-operative complications.Materials and methodsWe surgically treated 66 patients for colorectal cancer, aged over 75. The examined subjects were compromised for various reasons. We have evaluated the different influences of risk factors in elective and urgency operation.DiscussionSeveral studies have shown that age alone is not a significant prognostic factor in survival after colonic surgery. The assessment of general conditions in elderly patients, as demonstrated by the literature, is a fundamental moment in the management of colorectal cancer.ConclusionsThe surgical choice should be made case by case (custom-made), not based on age only.
