Abstract:The rising incidence of cutaneous malignancies presents ongoing therapeutic challenges. While surgery is the primary treatment modality, its limitations, including complications such as non-healing wounds and scarring, necessitate adjunctive therapies for optimal outcomes. But conventional methods like chemotherapy and phototherapy often fail to meet these needs. In response, microneedle patches (MNPs) have emerged, offering simultaneous delivery of multiple therapies directly to tumor sites. This review delves into three critical aspects of MNPs in treating skin malignancies: 1) what are the therapeutic combinations for different tumors? 2) what potential does MNP offer in the management of tumor-related symptoms? and 3) how to select suitable materials and design rational structures for enhanced therapeutic efficacy? Focusing on skin melanoma, basal cell carcinoma, and squamous cell carcinoma, we examine MNPs’ capacity for integrated multi-therapy, as well as their role in managing hemorrhage, pain, postoperative wounds, scars, and tumor indicators. Additionally, considerations for MNP storage, transportation, and side effects are discussed, emphasizing material selection and structural design. The review also discussed the ongoing dilemmas and future perspectives of MNPs in the clinical translation and application for cutaneous malignancies.
Rapidly Dissolving Microneedle Patch for Synergistic Gene and Photothermal Therapy of Subcutaneous Tumor

Abstract:The synergistic combination of gene therapy and photothermal therapy (PTT) has been widely investigated as a promising strategy for cancer treatment. To deliver genes and photothermal agents simultaneously and accurately to a tumor site, a microneedle (MN) patch co-loaded with p53 DNA and IR820 was fabricated by a two-step casting method. Hyaluronic acid was chosen as a matrix and p53 DNA and IR820 were mainly loaded into the tips to enhance utilization and reduce waste. The MN patch could efficiently penetrate the stratum corneum, and dissolve rapidly to release p53 DNA and IR820 in the subcutaneous tumor site. Due to the efficient photothermal efficacy of IR820, the temperature of the tumor site where the MN patch was applied increased by 14.7 °C under near-infrared light irradiation. The MN patch showed excellent antitumor effects in vivo owing to the synergistic effect of gene therapy and PTT. Consequently, the p53 DNA/IR820 MN patch may be a promising synergistic strategy for subcutaneous tumor treatments.
Gold Nanocage-Based Photothermal Ablation Facilitates in Situ Vaccination for Melanoma Therapy.

Abstract:Cancer immunotherapy represents a medical breakthrough, but there are still many patients unable to benefit from it because of the low response rate. The immunosuppressive tumor microenvironment (TME) is the main barrier to immunotherapy. Alleviating intratumoral immunosuppression is critical for improving the immune therapeutic efficacy. This work developed an in situ vaccination strategy by using gold nanocage (AuNC)-based photothermal effect in combination with an adjuvant and PD-L1 suppressor. In specific, this therapeutic strategy included three components: AuNCs as an inducer for tumor antigen production via photothermal ablation, CpG oligodeoxynucleotides as an adjuvant to amplify immune responses, and JQ1 as a PD-L1 suppressor to inhibit an immune checkpoint. The results showed that the in situ vaccination efficiently activated dendritic cells and primed T cells and exhibited a high therapeutic efficacy in the melanoma-bearing mice. This therapeutic strategy can increase the infiltration of cytotoxic T lymphocytes, suppress the PD-L1 expression in the tumor, and repolarize tumor-associated macrophages from pro-tumor M2 to the anti-tumor M1 phenotype, thereby remodeling the TME via regulating the innate immune and adaptive immune responses.
A self-monitoring microneedle patch for light-controlled synergistic treatment of melanoma

Abstract:Highlights • A self-monitoring MN patch is developed to enable direct visualizing of drug release. • A dissolvable MN patch achieves light-controlled chemo-photothermal therapy of melanoma by using AIE-active polymers. • AIE-active PATC particles can enhance the ICG reliability and stability both in vitro and in vivo. Melanoma is the most aggressive and malignant form of skin cancer. Current melanoma treatment methods generally suffer from frequent drug administration as well as difficulty in direct monitoring of drug release. Here, a self-monitoring microneedle (MN)-based drug delivery system, which integrates a dissolving MN patch with aggregation-induced emission (AIE)-active PATC microparticles, is designed to achieve light-controlled pulsatile chemo-photothermal synergistic therapy of melanoma. The PATC polymeric particles, termed D/, encapsulate both of chemotherapeutic drug doxorubicin (DOX) and the photothermal agent indocyanine green (ICG). Upon light illumination, PATC gradually dissociates into smaller particles, causing the release of encapsulated DOX and subsequent fluorescence intensity change of PATC particles, thereby not only enabling direct observation of the drug release process under light stimuli, but also facilitating verification of drug release by fluorescence recovery after light trigger. Moreover, encapsulation of ICG in PATC particles displays significant improvement of its photothermal stability both in vitro and in vivo. In a tumor-bearing mouse, the application of one D/ MN patch combining with two cycles of light irradiation showed excellent controllable chemo-photothermal efficacy and exhibited ∼97% melanoma inhibition rate without inducing any evident systemic toxicity, suggesting a great potential for skin cancer treatment in clinics. Abstract:Melanoma has emerged as a significant threat to human life and health. Microneedle (MN)‐mediated transdermal drug delivery (TDD) has garnered attention in melanoma treatment for bypassing the first‐pass effect. However, the propensity of melanoma to metastasize presents substantial challenges for MN mediated local treatment. Developing systemic therapies, such as immunotherapy in combination with TDD, is crucial for achieving effective melanoma treatment. Herein, a polyvinyl alcohol (PVA) MN‐mediated multifunctional TDD system, designated MN@PDA@1‐MT/CUR/DOX@HA (MN@PMCDH), was developed for synergetic chemotherapy/photothermal/immunotherapy of melanoma. PMCDH nanomedicines penetrate deep skin layers through MNs, accumulate at tumor sites guided by hyaluronic acid (HA), and selectively release drugs in response to the acidic tumor microenvironment and near‐infrared (NIR) stimulation. Released curcumin (CUR) significantly enhances the efficacy of photothermal therapy (PTT) and chemotherapy, as well as improves the induction of immunogenic cell death (ICD) by increasing melanoma sensitivity to polydopamine (PDA)‐mediated photothermal effects and doxorubicin (DOX). Moreover, the incorporation of 1‐methyltryptophan (1‐MT) to reverse the tumor immunosuppressive microenvironment can further enhance the effects of immunotherapy. In vitro studies revealed that the MN@PMCDH system can effectively induce ICD and inhibit tumor cell growth. Additionally, remarkable deep tumor cell inhibition effects are also achieved in 3D tumor models.

Abstract:Melanoma is resistant to conventional chemotherapy and radiotherapy. Therefore, it is essential to develop a targeted, low-toxic, and minimally invasive treatment. Here, DTIC/ICG-Fe3O4@TpBD BSP/HA microneedles (MNs) were designed and fabricated, which can enhance targeting to melanoma and perform photothermal therapy (PTT) and chemotherapy simultaneously to synergistically exert anticancer effects. The system consisted of magnetic nanoparticles (DTIC/ICG-Fe3O4@TpBD), dissoluble matrix (Bletilla polysaccharide (BSP)/hyaluronic acid (HA)), and a polyvinyl alcohol backing layer. Due to the good magnetic responsiveness of Fe3O4@TpBD, dacarbazine (DTIC) and indocyanine green (ICG) can be better targeted to the tumor tissue and improve the therapeutic effect. BSP and HA have good biocompatibility and transdermal ability, so that the MNs can completely penetrate the tumor tissue, be dissolved by the interstitial fluid, and release DTIC and ICG. Under near-infrared (NIR) light irradiation, ICG converts light energy into thermal energy and induces ablation of B16-OVA melanoma cells. In vivo results showed that DTIC/ICG-Fe3O4@TpBD BSP/HA MNs combined with chemotherapy and PTT could effectively inhibit the growth of melanoma without tumor recurrence or significant weight loss in mice. Therefore, DTIC/ICG-Fe3O4@TpBD BSP/HA MNs are expected to provide new ideas and therapeutic approaches for the clinical treatment of melanoma.

Abstract:Immunotherapy has demonstrated the potential to cure melanoma, while the current response rate is still unsatisfactory in clinics. Extensive evidence indicates the correlation between the efficacy and pre-existing T-cell in tumors, whereas the baseline T-cell infiltration is lacking in low-response melanoma patients. Herein, we demonstrated the critical contribution of dendritic cells (DCs) on melanoma survival and baseline T-cell level, as well as the efficacy of immunotherapy. Capitalized on this fact, we developed a photothermal nano-vaccine to simultaneously promote tumor antigens presentation and DCs infiltration for enhanced immunotherapy. The nano-vaccine was composed of polyserotonin (PST) core and tannic acid (TA)/Mn 2+ coordination-based metal-organic-framework (MOF) shell for β -catenin silencing DNAzyme loading, which was further integrated into dissolving microneedles to allow noninvasive and transdermal administration at melanoma skin. The nano-vaccine could rapidly penetrate skin upon microneedles insertion and exert a synergistically amplified photothermal effect to induce immunogenic cell death (ICD). The MOF shell then dissociated and released Mn 2+ as a cofactor to self-activate DNAzyme for β -catenin suppression, which in turn caused a persistent CCL4 excretion to promote the infiltration of DCs into the tumor. Meanwhile, the liberated PST core could effectively capture and facilitate tumor antigens presentation to DCs. As a result, potent antitumor efficacies were achieved for both primary and distal tumors without any extra treatment, indicating the great promise of such a nano-vaccine for on-demand personalized immunotherapy of melanoma.

Abstract:Despite recent advances in melanoma treatment through the use of anti-PD-1 (aPD1) immunotherapy, the efficacy of this method remains to be improved. Here we report an innovative self-degradable microneedle (MN) patch for the sustained delivery of aPD1 in a physiologically controllable manner. The microneedle is composed of biocompatible hyaluronic acid integrated with pH-sensitive dextran nanoparticles (NPs) that encapsulate aPD1 and glucose oxidase (GOx), which converts blood glucose to gluconic acid. The generation of acidic environment promotes the self-dissociation of NPs and subsequently results in the substantial release of aPD1. We find that a single administration of the MN patch induces robust immune responses in a B16F10 mouse melanoma model compared to MN without degradation trigger or intratumoral injection of free aPD1 with the same dose. Moreover, this administration strategy can integrate with other immunomodulators (such as anti-CTLA-4) to achieve combination therapy for enhancing antitumor efficacy.

Abstract:Thanks to the growing knowledge about cancers and their interactions with the immune system, a huge number of therapeutic cancer vaccines have been developed in the past two decades. Despite encouraging results in pre-clinical models, cancer vaccines have not yet achieved significant clinical efficacy. Several factors may contribute to such poor results, including the difficulty of triggering a strong immune response and the immunosuppressive tumor microenvironment. Many strategies are currently being explored. Different types of adjuvants have been incorporated into vaccine formulations to improve their efficacy, as cancer antigens are usually poorly immunogenic. Nanoparticle systems are promising tools as they act as carriers for antigens and can be surface-modified so that they specifically target antigen-presenting cells in lymph nodes. Bioinspired nanomaterials are ideal candidates thanks to their biocompatibility. Recently, melanin-based nanoparticles were reported to efficiently localize into draining lymphoid tissues and trigger immune responses against loaded antigens. In addition, by virtue of their photochemical properties, melanin-based nanoparticles can also play an immunomodulatory role to promote anti-cancer responses in the context of photothermal therapy. In this review, we discuss the above-mentioned properties of melanin, and summarize the promising results of the melanin-based cancer vaccines recently reported in preclinical models.

Abstract:Traditional therapies for melanoma suffer from many restrictions, that calls for the development of new treatments with high effectiveness and safety. Recently, employing transdermal microneedle (MN) systems for delivering therapeutic agents has obtained increasing interest as the efficient, minimally invasive, painless, and safe natures. Designing an “all-in-one” multifunctional MN system without complicated integration that can realize enhanced tumor therapy has presented huge promising for clinical application but is still a challenging strategy. Herein, a simple responsive MN system is developed for efficient anti-melanoma by combining self-enhanced chemodynamic therapy (CDT) and photothermal therapy (PTT). This MN system, constructed by biocompatible poly(vinylpyrrolidone) (PVP) as substrate and PVP-stabilized CuO2 nanoparticles (CuO2 NPs) as therapeutic agent, shows good skin-insertion capability and dissolving property that benefit in penetrating skin efficiently to deliver the therapeutic CuO2 NPs into melanoma site. The released CuO2 NPs from MN system can not only produce copper ion (Cu2+) and hydrogen peroxide (H2O2) in response to the acidic environment for catalytic hydroxyl radical (OH) generation and thus achieve the H2O2 self-supplying CDT, but also can be utilized as a glutathione (GSH)-scavenging agent for further enhancing CDT. Moreover, owing to the loading of CuO2 NPs, such MN system exhibits excellent near-infrared (NIR) photothermal property that can induce local hyperthermia at the melanoma area for PTT. In result, the designed MN system realizes the efficient tumor inhibition with minimal side effects in vivo by the combination therapy, offering a safe and effective melanoma treatment option for patients.

Abstract:Multimodal nanoformulations with limited side-effects for tumor theranostics that catalyze a cascade of intra-cellular reactions show great potentials for tumor treatment with high specificity and efficiency. In this study, Cu-doped polydopamine nanoparticles (Cu-PDA NPs) have been prepared and embedded into microneedles (MNs) for photothermal and chemodynamic synergistic therapy against skin melanoma, by virtue of enhanced drug transdermal delivery owing to the unique core-shell structure of MNs. This multimodal tumor therapeutic strategy accumulates the high photothermal effect of Cu-PDA NPs (similar to 50.40%) to acquire the energy from NIR irradiation and convert it into heat, and good Fenton-like catalytic activity of Cu+ ions to produce toxic free hydroxyl groups (center dot OH) by reaction with excessive H2O2 in tumor cells, leading to the generation of a new minimally invasive synergistic therapy. The B16F10 mouse melanoma model demonstrates that the active delivery of Cu-PDA NPs results in greatly inhibit proliferation of tumor and induce mixed necrosis/apoptosis of tumor cells in vivo. This study offers a new avenue for the development of microneedles-based microdevices for skin melanoma by a minimally invasive delivery of multimodal nanoformulation.
Subcutaneous Rapid Dissolution Microneedle Patch Integrated with CuO2 and Disulfiram for Augmented Antimelanoma Efficacy through Multimodal Synergy of Photothermal Therapy, Chemodynamic Therapy, and Chemotherapy

Abstract:Melanoma is a malignancy of the skin that is resistant to conventional treatment, necessitating the development of effective and safe new therapies. The percutaneous microneedle (MN) system has garnered increasing interest as a viable treatment option due to its high efficacy, minimal invasiveness, painlessness, and secure benefits. In this investigation, a sensitive MN system with multiple functions was created to combat melanoma effectively. This MN system utilized polyvinylpyrrolidone (PVP) as microneedle substrates and biocompatibility panax notoginseng polysaccharide (PNPS) as microneedle tips, which encapsulated PVP-stabilized CuO2 nanoparticles as a therapeutic agent and disulfiram-containing F127 micelles to enhance the tumor treatment effect. The MN system had sufficient mechanical properties to pierce the skin, and the excellent water solubility of PNPS brought high-speed dissolution properties under the bio conditions, allowing the MNs to effectively penetrate the skin and deliver the CuO2 nanoparticles as well as the drug-loaded micelles to the melanoma site. CuO2 nanoparticles released by the MN system generated Cu2+ and H2O2 in the tumor acidic environment to achieve self-supply of hydrogen peroxide to chemodynamic therapy (CDT). In addition, Cu2+ was chelated with disulfiram to produce CuET, which killed tumor cells. And the MN system had excellent near-infrared (NIR) photothermal properties due to the loading of CuO2 nanoparticles and induced localized thermotherapy in the melanoma region to further inhibit tumor growth. Thus, the designed MN system accomplished effective tumor suppression and minimal side effects in vivo via combined therapy, offering patients a safe and effective option for melanoma treatment.

Abstract:Nanomedicine-based photodynamic therapy (PDT) for melanoma treatment has attracted great attention. However, the complex design of polymer nanoparticles and high doses of photosensitizers used in intravenous injections (for sufficient accumulation of drugs in tumor lesions) pose a huge challenge to the commercialization and further clinical application. Herein, we fabricated the carrier-free nanoassemblies of a chlorin e6 (L-Ce6 NAs)-integrated fast-dissolving microneedles patch (L-Ce6 MNs) enriching only about 3 mu g of Ce6 in the needle tips via a facile fabrication method. The L-Ce6 MNs had sufficient mechanical strength to penetrate the skin and facilitated the transportation of L-Ce6 NAs to a depth of 200-500 mu m under the skin, thereby achieving efficient and accurate drug delivery to tumor lesions. In a xenograft mouse melanoma model, the L-Ce6 MNs-based PDT with low dose of Ce6 (0.12 mg/kg) exerted efficient ablation of the primary lesions in situ through reactive oxygen species (ROS) generation. More importantly, a significant abscopal effect was also elicited by activating immunogenic cell death (ICD) and releasing danger-associated molecular patterns (DAMPs), which in turn promoted dendritic cells (DCs) maturation and the subsequent antigen presentation, thereby facilitating the T-cell-mediated immune response without synergetic immunotherapies. Collectively, our findings indicate the facile, controllable, and fast-dissolving microneedles patch with a low dose of photosensitizers presented great therapeutic potential for enhanced photoimmunotherapy.

Abstract:Traditional drug delivery methods and treatments often lack efficacy in eradicating melanoma and can cause significant toxicity to healthy tissues. Combining microneedle (MN)-based transdermal drug delivery (TDD) with in situ synergistic therapy is crucial for overcoming these challenges. Thus, a novel multifunctional nano-TDD system was developed by integrating polyvinyl alcohol (PVA) MN with hyaluronic acid (HA)-functionalized calcium peroxide/copper peroxide @ zeolitic imidazolate framework (CaO 2 /CuO 2 @ZIF-8) nanocapsule co-loaded with 1-methyltryptophan (1-MT) and disulfiram (DSF). Nanomedicines reach deeper skin layers via MNs and targeted to the melanoma site by HA. Subsequently, ZIF-8 protective layer releases 1-MT and DSF in response to the acidic tumor microenvironment. The exposed CaO 2 /CuO 2 nanoparticles decompose into Cu 2+ , Ca 2+ and hydrogen peroxide (H 2 O 2 ). Cu 2+ catalyzes the generation of hydroxyl radical (•OH) from H 2 O 2 to turn on chemodynamic therapy (CDT), and the efficiency of •OH generation can be further enhanced by depleting glutathione (GSH). Benefiting from the in-situ chelation reaction of DSF with Cu 2+ , CuET chemotherapy drugs with strong toxicity are generated. In addition, the produced Cu + can further enhance the efficacy of CDT. The massive influx of Ca 2+ exacerbates the damage to mitochondria. Ultimately, combining 1-MT to reverse the tumor's immunosuppressive microenvironment with chemotherapy and CDT-induced immunogenic cell death offers a promising and safe strategy for melanoma treatment.

Abstract:Melanoma is one of the most threatening cancers due to its metastatic capacity and its incidence is recently increasing due to ozone depletion and excessive exposure to ultraviolet. Adjuvant therapy after primary surgical resection is commonly used to prevent tumor recurrence. However, its clinical outcome is not often satisfactory, mainly due to insufficient local accumulation and the systemic toxicity of antitumor drugs. Herein, we reported a self-biodegradable tissue adhesive microneedle (MN) patch for localized and sustained delivery of doxorubicin (DOX) via microchannels for melanoma growth inhibition. DOX-loaded MN patches were fabricated by casting bioengineered mussel adhesive protein (MAP) solution containing DOX onto poly (dimethyl siloxane) molds with MN-shaped cavities under backside vacuum. The fabricated MAP-based MN patch enabled effective tissue insertion for fresh and living mice skins as well as firm surface adhesion. Notably, DOX-laden MN (DOX-MN) patch significantly suppressed B16F10 melanoma cell proliferation in vitro , while drug-free MN patch did not show any cytotoxicity for both NIH3T3 fibroblasts and B16F10 melanoma cells. In a melanoma-bearing mice model, DOX-MN patch treatment induced greater antitumor efficacy as demonstrated by significant decrease in tumor volume and weight and larger necrotic region compared with intratumoral injection of free DOX. We expect that this strategy can also integrate with other immunomodulators to provide further combinative therapy for effectively eradicating tumors.

Abstract:Dissolvable polymeric microneedles (DPMNs) have emerged as a powerful technology for the localized treatment of diseases, such as melanoma. Herein, we fabricated a DPMN patch containing a potent enzyme-nanozyme composite that transforms the upregulated glucose consumption of cancerous cells into lethal reactive oxygen species via a cascade reaction accelerated by endogenous chloride ions and external near-infrared (NIR) irradiation. This was accomplished by combining glucose oxidase (Gox) with a NIR-responsive chloroperoxidase-like copper sulfide (CuS) nanozyme. In contrast with subcutaneous injection, the microneedle system highly localizes the treatment, enhancing nanomedicine uptake by the tumor and reducing its systemic exposure to the kidneys and spleen. NIR irradiation further controls the potency and toxicity of the formulation by thermally disabling Gox. In a mouse melanoma model, this unique combination of photothermal, starvation, and chemodynamic therapies resulted in complete tumor eradication (99.2 ± 0.8% reduction in tumor volume within 10 d) without producing signs of systemic toxicity. By comparison, other treatment combinations only resulted in a 42–76.5% reduction in tumor growth. The microneedle patch design is therefore not only highly potent but also with regulated toxicity and improved safety.

Red Blood Cell Membrane-Camouflaged Melanin Nanoparticles for Enhanced Photothermal Therapy.

Abstract:Photothermal therapy (PTT) has represented a promising noninvasive approach for cancer treatment in recent years. However, there still remain challenges in developing non-toxic and biodegradable biomaterials with high photothermal efficiency in vivo. Herein, we explored natural melanin nanoparticles extracted from living cuttlefish as effective photothermal agents and developed red blood cell (RBC) membrane-camouflaged melanin (Melanin@RBC) nanoparticles as a platform for in vivo antitumor PTT. The as-obtained natural melanin nanoparticles demonstrated strong absorption at NIR region, higher photothermal conversion efficiency (∼40%) than synthesized melanin-like polydopamine nanoparticles (∼29%), as well as favorable biocompatibility and biodegradability. It was shown that RBC membrane coating on melanin nanoparticles retained their excellent photothermal property, enhanced their blood retention and effectively improved their accumulation at tumor sites. With the guidance of their inherited photoacoustic imaging capability, optimal accumulation of Melanin@RBC at tumors was achieved around 4 h post intravenous injection. Upon irradiation by an 808-nm laser, the developed Melanin@RBC nanoparticles exhibited significantly higher PTT efficacy than that of bare melanin nanoparticles in A549 tumor-bearing mice. Given that both melanin nanoparticles and RBC membrane are native biomaterials, the developed Melanin@RBC platform could have great potential in clinics for anticancer PTT.
Partitioned Microneedle Patch Based on NO Release and HSP Inhibition for mPTT/GT Combination Treatment of Melanoma

Abstract:Photothermal therapy (PTT) shows promise in cancer treatments due to its good spatiotemporal selectivity and minimal invasiveness. However, PTT has some problems such as excessive heat damage to normal tissues, tumor thermo-resistance caused by heat shock proteins (HSPs), and limited efficacy of monotherapy. Here, we construct a patch named "partitioned microneedles" (PMN-SNAP/CuS), which separates the "catalyst" bovine serum albumin-based copper sulfide nanoparticles (CuS@BSA NPs) and the "reactant" S-nitroso-N-acetylpenicillamine (SNAP) into different regions of microneedles, for enhancing mild PTT (mPTT) of melanoma. PMN-SNAP/CuS showed an excellent photothermal effect, Fenton-like catalytic activity, and nitric oxide (NO) generation ability. The combination of NO and reactive oxygen species (ROS) produced by PMN-SNAP/CuS effectively blocked the synthesis of HSPs at the source and enhanced the efficacy of mPTT. Both in vitro and in vivo results proved that PMN-SNAP/CuS significantly enhanced the inhibition of melanoma under 808 nm laser irradiation. In conclusion, our partitioned microneedle strategy based on the combination of enhanced mPTT and gas therapy (GT) provides a promising approach to enhance the therapeutic effect on melanoma.
