Abstract:The discussion on the scientific character of psychoanalysis is continuous and multifaceted. Nowadays the extent and variety of the implied aspects in it is even increased. In this context this article presents for its analysis and assessment a reconsideration of Freud’s line of argument regarding the scientific character of psychoanalysis. Against a quite spread view that conceives Freud’s scientific perspective as a reflex of general traits of his time such as positivism, sustained by recent research on Freud this article proposes the reconsideration of Freud’s perspective in a more exhaustive and nuanced way and holds that it is based on a composed by integrated parts, unified and consistent line of argument maintained through all his work. The reconsideration of the design of Freud’s argument allows to hold that his own formulation is still fruitful and suitable for the present, particularly as it possibilitates, justifies and requires the connection of psychoanalysis to the development of knowledge.
The differential effect of psychotherapy and psychoanalysis with anaclitic and introjective patients: the Menninger Psychotherapy Research Project revisited

Abstract:Analyses of the data from the Menninger Psychotherapy Research Project (MPRP) have consistently indicated little difference in the therapeutic outcome between patients seen in psychoanalysis and those seen in psychotherapy. Reanalysis of the data from the MPRP, utilizing a distinction between two broad configurations of psychopathology (Blatt, 1974, 1990a; Blatt and Shichman, 1983), however, indicates that patients whose pathology focuses primarily on disruptions of interpersonal relatedness and who use primarily avoidant defenses (anaclitic patients), and patients whose pathology focuses primarily on issues of self-definition, autonomy, and selfworth and who use primarily counteractive defenses (introjective patients) differ in their responsiveness to psychotherapy and psycho-analysis. Based on recently developed procedures for systematically evaluating the quality of object representation on the Rorschach, reanalysis of the Menninger data reveals that anaclitic patients have significantly greater positive change in psychotherapy, while introjective patients have significantly greater positive change in psychoanalysis. These statistically significant patient-by-treatment interactions are discussed in terms of their clinical implications as well as the importance of differentiating among types of patients in studies of therapeutic outcome and of therapeutic process.
The Epistemic Predicament of a Pseudoscience: Social Constructivism Confronts Freudian Psychoanalysis

Abstract:Social constructivist approaches to science have often been dismissed as inaccurate accounts of scientific knowledge. In this article, we take the claims of robust social constructivism (SC) seriously and attempt to find a theory which does instantiate the epistemic predicament as described by SC. We argue that Freudian psychoanalysis, in virtue of some of its well‐known epistemic complications and conceptual confusions, provides a perfect illustration of what SC claims is actually going on in science. In other words, the features SC mistakenly ascribes to science in general correctly characterize the epistemic status of Freudian psychoanalysis. This sheds some light on the internal disputes in the field of psychoanalysis, on the sociology of psychoanalytic movement, and on the “war” that has been waged over Freud's legacy with his critics. In addition, our analysis offers an indirect and independent argument against SC as an account of bona fide science, by illustrating what science would look like if it were to function as SC claims it does.

Abstract:A recent meta-analysis revealed that mental health and baseline psychological impairment affect the quality of life and outcomes in different chronic conditions. Implementing mental health care in physical care services is still insufficient. Thus, interdisciplinary communication across treatment providers is essential. The standardized language provided by the diagnostic statistical manual favors a clear conceptualization. However, this approach might not focus on the individual, as thinking in categories might impede recognizing the continuum from healthy to diseased. Psychoanalytic theory is concerned with an individual's unconscious conflictual wishes and motivations, manifested through enactments like psychic symptoms or (maladaptive) behavior with long-term consequences if not considered. Such modifiable internal and external factors often are inadequately treated. However, together with the physical chronic condition constraints, these factors determine degrees of freedom for a self-determined existence. The effect of therapeutic interventions, and especially therapy adherence, relies on a solid therapeutic relationship. Outcome and process research still investigates the mechanism of change in psychotherapeutic treatments with psychanalysis's focus on attachment problems. This article examines existing knowledge about the mechanism of change in psychoanalysis under the consideration of current trends emerging from psychotherapy research. A clinical example is discussed. Additionally, further directions for research are given. The theoretical frame in psychoanalytic therapies is the affect-cognitive interface. Subliminal affect-perception is enabled via awareness of subjective meanings in oneself and the other; shaping this awareness is the main intervention point. The interactional ingredients, the patient's inherent bioenvironmental history meeting the clinician, are relevant variables. Several intrinsic, subliminal parameters relevant for changing behavior are observed. Therapeutic interventions aim at supporting the internalization of the superego's functions and at making this ability available in moments of self-reflection. By supporting mentalization abilities, a better understanding of oneself and higher self-regulation (including emotional regulation) can lead to better judgments (application of formal logic and abstract thinking). Thus, this facilitates enduring behavior change with presumably positive effects on mental and physical health.

Abstract:The growing number of individuals seeking treatment for mental disorders calls for intelligent and responsible decisions in health care politics. However, the current relative decrease in reimbursement of effective psychotherapy approaches occurring in the context of an increase in prescription of psychotropic medication lacks a scientific base. Using psychodynamic psychotherapy as an example, we review the literature on meta-analyses and recent outcome studies of effective treatment approaches. Psychodynamic psychotherapy is an effective treatment for a wide variety of mental disorders. Adding to the known effectiveness of other shorter treatments, the results indicate lasting change in many cases, especially for complex and difficult to treat patients, ultimately reducing health-care utilization. Research-informed health care decisions that take into account the solid evidence for the effectiveness of psychotherapy, including psychodynamic psychotherapy, have the potential to promote choice, increase mental health, and reduce society's burden of disease in the long run.

Abstract:All contemporary psychotherapies agree that (failing) emotion regulation is central to psychological disorders and that psychotherapy is about improving emotion regulation. In his research on the "emotion-laden" complex Jung put an emphasis on the role of failing emotion regulation in contributing to psychological disorders as well as to change in the process of psychotherapy, but he left this field of research and took a very different direction in favour of his archetype concept. Psychodynamic approaches generally argue that changes in emotion regulation are accomplished through corrective emotional experiences in the therapeutic relationship. Insights from affective neurosciences and attachment research have had a major influence on how the therapeutic relationship is constructed in contemporary psychodynamic approaches. There is a lack of similar developments in analytical psychology, which leads to substantial differences between the models of Jungian psychotherapy in contrast to other contemporary psychodynamic approaches. The implications of these differences for the practice of psychotherapy and especially the role of the therapeutic relationship are pointed out.

Abstract:Theory &Psychology, Ahead of Print. The long-enduring problem of assessing psychoanalysis as a science is still under debate and perhaps this is because the focus has been more on questions of demarcation than an evaluation of the qualities proper to this psychological approach. Here we outline this discussion and attempt to clarify Freud's conceptions of natural science and naturalism. We show that Freud commits himself to a nonreductionist view of the mind that is consistent with his phenomenon-oriented attitude. Drawing on a difference between ontological and methodological naturalism, we argue that Freud understood psychic phenomena as a process, and this is key to comprehending the qualities and scope of his metapsychological theory.

Abstract:This paper critically examines the validity of Freudian psychoanalysis within the framework of Moberger's characterisation of pseudoscience as bullshit with scientific pretensions. The central question addressed is whether Freudian psychoanalysis qualifies as "bullshit," following Moberger's guideline of looking for systematic fallacies. The analysis centres on two fundamental critiques against psychoanalysis: one posited by Popper, contending that psychoanalytic interpretation is excessively flexible, and another by Glymour, asserting that Freud's interpretative method baselessly posits associations as causes. This paper argues that both criticisms rest on misunderstandings and asserts that Freudian psychoanalysis does not commit the alleged fallacies. It also offers positive evidence that Freud was not a bullshitter. The conclusion drawn is that psychoanalysis should not be regarded as bullshit, and hence does not qualify as pseudoscience on Moberger's criteria. Consequently, the paper suggests that Freudian psychoanalysis deserves a fairer hearing then many have given it.

Abstract:Although commentators periodically declare that Freud is dead, his repeated burials lie on shaky grounds. Critics typically attack an archaic version of psychodynamic theory that most clinicians similarly consider obsolete. Central to contemporary psychodynamic theory is a series of propositions about (a) unconscious cognitive, affective, and motivational processes; (b) ambivalence and the tendency for affective and motivational dynamics to operate in parallel and produce compromise solutions; (c) the origins of many personality and social dispositions in childhood; (d) mental representations of the self, others, and relationships; and (e) developmental dynamics. An enormous body of research in cognitive, social, developmental, and personality psychology now supports many of these propositions. Freud's scientific legacy has implications for a wide range of domains in psychology, such as integration of affective and motivational constraints into connectionist models in cognitive science.

[The current mind-brain theories in analytical philosophy of mind and their epistemic significance for psychiatry]

Abstract:This article begins with an orientational survey of the historical evolution of analytical philosophy of mind (APM) which was formulated in the last 40 years as "philosophy of mind" in the Anglo-Saxon scientific-cultural world and which, in the meantime, dominates to a great extent contemporary German philosophy. Then there follows a discussion of the currently most popular mind-brain theories in philosophy. In comparison to the more marginal dualist variants (interactionism, epiphenomenalism, parallelism), it is mainly the monistic positions of non-reductive, reductive and eliminative materialism and the materialist functionalism underlying it, which determines analytical philosophy of mind and its influence on psychopathology and psychiatry. Under the additional influence of modern brain research methods, particularly neuroimaging, it is progressively developing into a subdiscipline of neuroscience, a complex and increasingly more firmly established scientific discipline which comprises the totality of all sciences dealing with neuronal functions, including the close epistemic associations of APM and neuroimaging. This is the effective epistemic central idea determining the theory of the neuronal network which, in the form of a connectionist psychopathology, is intended to make possible a fundamentally new access to the comprehension of psychiatric forms of illness. In this respect it is evident, however, that the perception of the naturality of the mind as the fundamental thesis of APM and thus of connectionism cannot be followed through, since, up to now, neither from the phenomenality of the mind (especially the quality of senses, "Qualia") nor from intentionality of the mind (i. e. the ability to act intentionally, free from the constraint of the causality of nature and thus in self-responsible fashion) has proved it possible to reconstruct a generally accepted naturalist theory. Furthermore, it has not been possible to reformulate it in an exclusively physical, i. e. non-phenomenological concept and terminology which is, above all, free from the intentionality idiom. The consequence of this, however, is that a connectionist psychopathology can only represent a subpersonal, i. e., subhuman area and that in order to establish a personal psychopathology, naturalistic unreduced theories of experience-qualities and intentional acts of completeness are absolutely essential. The neuroscientific-connectionist paradigm of psychopathology must therefore - at least for the present - be supplement by the paradigm of a non-natural (e. g. phenomenological-hermeneutic) psychopathology. This result can only encourage the relinquishing of epistemically one-sided materialist and other monistic mind-brain theories of APM in favour of an epistemically open pragmatic interactionist dualism as the scientific position which best represents the current state of knowledge.

Abstract:Work at the intersection of philosophy and psychiatry has an extensive and influential history, and has received increased attention recently, with the emergence of professional associations and a growing literature. In this paper, we review key advances in work on philosophy and psychiatry, and their related clinical implications. First, in understanding and categorizing mental disorder, both naturalist and normativist considerations are now viewed as important – psychiatric constructs necessitate a consideration of both facts and values. At a conceptual level, this integrative view encourages moving away from strict scientism to soft naturalism, while in clinical practice this facilitates both evidence‐based and values‐based mental health care. Second, in considering the nature of psychiatric science, there is now increasing emphasis on a pluralist approach, including ontological, explanatory and value pluralism. Conceptually, a pluralist approach acknowledges the multi‐level causal interactions that give rise to psychopathology, while clinically it emphasizes the importance of a broad range of "difference‐makers", as well as a consideration of "lived experience" in both research and practice. Third, in considering a range of questions about the brain‐mind, and how both somatic and psychic factors contribute to the development and maintenance of mental disorders, conceptual and empirical work on embodied cognition provides an increasingly valuable approach. Viewing the brain‐mind as embodied, embedded and enactive offers a conceptual approach to the mind‐body problem that facilitates the clinical integration of advances in both cognitive‐affective neuroscience and phenomenological psychopathology.

Abstract:Even if psychoanalysis no longer prevails with the extraordinary enthusiasm it inspired after World War II, it still retains an important place for many in the mental health field. This paper's objective is to describe the current situation of psychoanalysis and psychoanalytic therapies in the French health system, showing how, in our country, it has developed to this unique position. So far, influenced by the evolution of psychoanalysis in France, historical, cultural, and societal factors added with a strong lobby from psychodynamic scholars and strong support offered by clinical psychology, psychoanalysis and psychoanalytic psychotherapies have maintained a significant practice framework in France. Despite being challenged by the dominant global position of DSM psychiatry and cognitive psychology, psychoanalysis' influence remains strong as it does not affect only psychology and psychiatry but also extends to the humanities and social sciences. New practice types and future directions for psychoanalysis and psychoanalytic therapies are then discussed.

Abstract:We read with great interest the article by Moncrieff et al. presenting a systematic umbrella review of the evidence of the serotonin theory of depression [1]. The authors conducted a meaningful and relevant review, and we appreciate the continued attention in the scientific literature to the challenge of molecular psychiatry of serotonin theory in predominantly depressive disorders. The biological perspective plays a crucial role in psychiatry; mental disorders are increasingly understood in terms of biological mechanisms linked to changes in the structure, chemistry, and function of the brain, and that mental illness does indeed have a biological basis [2]. However, George L. Engel presented the biopsychosocial model (BPS) as a medical model to explain mental illnesses in 1977. Since then, this model has become widely accepted around the world [3], At the same time, as described in some of the reviewed articles, research systematically explained the interaction of three major dimensions (biological, psychological, and social) in the development of psychiatric disorders [4].

Abstract:Behavioural psychotherapy has long historical roots. Recently it has led to effective treatment for selected neuroses, including phobic, obsessive-compulsive and sexual disorders. Potent therapy has become a tool of experimental psychopathology which advances theory and practice. A pervasive principle is exposure of the patient to those stimuli which evoke his discomfort until this subsides. Level of arousal during exposure does not affect outcome. Theoretical issues are reviewed which decide when exposure will be sensitizing or habituating. Both psychoanalytical and conditioning models of neurosis are out of date, and models derived more directly from clinical experiment are becoming possible. The aetiology of phobias and rituals can be seen as failed extinction rather than enhanced acquisition. Relevant phylogenetic and biological factors are discussed. At the other extreme, well-documented faith-healing indicates huge gaps in our knowledge of psychotherapy.

Abstract:In this paper I discuss the diminishing role played by psychoanalytic/psychodynamic psychotherapy in public sector mental health treatment services and explore the impact of this on the psychoanalytic profession and its trainees. I draw on the discipline of pragmatics to argue that we need to move from a theory‐based approach to clinical practice to a research‐based model in which we draw on an integration of evidence from a range of disciplines to build an understanding of the developmental and neuroscientific basis of psychological and emotional problems, with a particular focus on complex PTSD and its overlap with borderline personality disorder. I discuss how process research and outcome studies provide additional tools to add to our understanding of the clinical interaction in the consulting room. I discuss the implications of these findings for clinical practice and the need for clinicians to begin to integrate research into their training and professional development.

Abstract:Psychoanalysis and Politics argue that psychoanalysis is at its root a social or group theory, to which a theory of individual psychology is integral. This formulation follows from Freud's Group Psychology, which defines individual psychology as a derivative of group psychology, "still incomplete." The article historicizes the analytic conception of the individual in terms of the authors' conception of personalize, spelled out in Secrets of the Soul. Three versions of psychoanalytic social theory are discussed: Freudo-Marxism, the New Left and feminism and the "relational turn."

Abstract:This brief commentary critically examines the study "Psychotherapies for the Treatment of Borderline Personality Disorder: A Systematic Review" by Crotty et al. (2023) It highlights several methodological and reporting concerns that impact the study's credibility and conclusions. Key issues include the retrospective registration of the study protocol, discrepancies in authorship and protocol content, lack of clarity in inclusion and exclusion criteria, and limitations in geographical scope without clear justification. Furthermore, the letter discusses inconsistencies in the risk of bias and quality of evidence assessments, particularly in the application of the Grading of Recommendations Assessment, Development, and Evaluation system. These methodological shortcomings question the study's findings, contrasting with other comprehensive reviews in the field. The critique emphasizes the importance of methodological rigor and transparency in systematic reviews, especially those influencing clinical practice and policy decisions in mental health care. What kind of «thing» is mental illness? Listening to Kraepelin, Jaspers and Kronfeld

Abstract:Since its beginnings psychiatry controversely debated about the appropriate concept of illness, about nosology and the diagnostic terms and procedures based upon it. This paper discusses the approaches introduced by Emil Kraepelin (1856–1926), Karl Jaspers (1883–1969) and Arthur Kronfeld (1886–1941). Whereas Kraepelin postulated the existence of «natural entities» with regard to mental illness, Jaspers acknowledged the heuristic value of the term «disease entity», but declared it a «regulative idea» in a Kantian sense, i.e. an appropriate and useful, albeit never fully achievable aim. Kronfeld, in a way situated «between» Kraepelin and Jaspers, defended the notion of disease entities. However, he spoke clearly against primarily anchoring these entities in neighbouring scientific fields like neurobiology or social sciences. Psychiatry should stay (or become) «autological», in the first place using the psychological perspective. In full respect for neighbouring approaches as scientific fields in their own right, he rejected their premature transfer to mental phenomena as «heterological», i.e. as unduly narrowing down the scope of psychiatry. These issues, up to debate 100 years ago, still are of utmost relevance for psychiatry in the 21st century, a field struggling with its rather fragile scientific identity.

Abstract:Past research on help-seeking behaviour and compliance among the mentally ill has mainly been focused on the examination of individual and structural determinants, while the impact of the socio-cultural context has been largely neglected. To examine this impact, we conducted a representative survey among German citizens who were eligible to vote. By means of vignettes describing different mental disorders we recorded the lay public's preferences for various treatment methods, together with their subjective reasons for voicing these particular preferences. While we found that psychotherapy was generally held in high esteem by the lay public, psychopharmacotherapy was rejected by the vast majority of respondents. This pattern was observed for all of the mental disorders included in our survey. While the public's image of psychotherapy is largely determined by popular views on psychoanalysis, public opinion about psychotropic drugs is strongly influenced by characteristics associated with tranquillizers. Among the different psychotherapeutic approaches, psychoanalysis is the preferred method among respondents in the western part of Germany, while the lay public in the eastern part tends to endorse group therapy. Finally, our research showed a considerable difference between the conceptions held by the lay public and those of psychiatric experts with regard to the adequate treatment of mental disorders. This, in turn, should not be without consequence for help-seeking behaviour and patient compliance.

Abstract:<p>Reflective practice has gained traction in clinical psychology largely to address the fact that practitioners must frequently "use their heads" when scientific data are not readily available. Despite their widespread adoption, reflective practice techniques are largely lacking in supportive outcome evidence. We contend that the reflective practice literature has remained largely disconnected from basic psychological science, especially work on the limitations of (a) introspection as a means of becoming aware of one's biases, (b) self‐assessment, and (c) acquiring expertise from experience. To realize its potential, the reflective practice literature will need to forge closer connections with work on social cognition and debiasing, and to determine whether its techniques enhance patient outcomes and the validity of clinicians' judgments and predictions.</p>
