Abstract:Not all young people who present with a borderline syndrome are equally vulnerable. Clinical manifestations appear along a spectrum that varies from an apparently less severely disturbed psychoneurotic to those who appear with almost total ego fragmentation (Giovacchini, 1964). When the syndrome appears as a result of social system trauma in early adolescence, adequate diagnosis and appropriate intervention can make for speedy recovery. A typical scenario is the absence of a peer group that is developmentally needed, along with the family tension and social system turbulence produced by a change of location, school, or cultural environment. When executives move with their families, for example, their adolescent children are likely to react with at least depression, with at worst a borderline syndrome. The therapy of disturbed borderline young people is likely to be successful only in an environment that optimally meets developmental needs with skilled intervention in biological, psychological, and social fields. The latter should include work with the family in which the individual psychopathology of the parents, their marital relationships, and the strengths and weaknesses of the family system are all assessed.

Abstract:People of color with borderline personality disorder (PoCwBPD), in particular African-Americans and Latinxs in the inner city, present a special population at risk for unemployment and long-term impairment in psychiatric and psychosocial recovery. A growing literature points to the need for comprehensive care that addresses the symptoms of the borderline syndrome alongside social, vocational, and educational achievement. This article paints a picture of the individual, communal, and systemic complexities involved in aiding PoCwBPD achieve psychosocial recovery. After contextualizing the historical and systemic forces that impact recovery for PoCwBPD, I discuss principles of intervention to assist clinicians tailor treatment to this population. Drawing on insights from Transference-Focused Psychotherapy, an evidence-based psychodynamic therapy for BPD, I offer a theoretical cultural adaptation for people of color. This article provides clinical vignettes to illustrate the resulting model, and shows that helping the patient engage in productive activity provides the foundation for exploratory work, which in turn addresses intrapsychic obstacles to psychosocial success.

Abstract:Long-standing theories of borderline personality disorder (BPD) suggest that symptoms develop at least in part from childhood adversity. Emotion dysregulation may meaningfully mediate these effects. The current study examined three factors related to emotion dysregulation—alexithymia, affective lability, and impulsivity—as potential mediators of the relation between childhood adversity and BPD diagnosis in 101 individuals with BPD and 95 healthy controls. Path analysis compared three distinct models informed by the literature. Results supported a complex mediation model wherein (a) alexithymia partially mediated the relation of childhood adversity to affective lability and impulsivity; (b) affective lability mediated the relation of childhood adversity to BPD diagnosis; and (c) affective lability and impulsivity mediated the relation of alexithymia to BPD diagnosis. Findings suggest that affective lability and alexithymia are key to understanding the relationship between childhood adversity and BPD. Interventions specifically targeting affective lability, impulsivity, and alexithymia may be particularly useful for this population.

Abstract:Kelly's intense focus on personal construing processes is understandable given the aims he put forth when creating personal construct psychology and the socio-cultural context in which his theory arose. Plenty of space remains, however, to elucidate the interaction between public construing processes and personal constructs to the extent that these can be separated in a relational theory such as this one. Previously, this exploration was largely theoretical, and it involved drawing parallels between Kelly and theorists such as Foucault or Bauman. In this paper, the relationship between public and personal systems of constructs is theorized with clinical applications in mind, emphasizing its practical importance as a means of understanding clients and situating their change in a wider social context, while understanding the possibilities and limitations determined by constructs structuring and governing social relations. This paper will attempt to create a new framework for further expansion of Kelly's theory. A brief clinical vignette will be used to illustrate how this approach reframes borderline personality disorder as a primarily relational issue.

Abstract:Introduction More than 75% of the psychiatric disorders arise before the age of 30. Adolescence and young adulthood pose numerous developmental challenges like identity development, educational and occupational concerns, gaining autonomy and boundary-setting skills. The adolescent crises, which can involve a broad spectrum of psychiatric symptoms, demands a multidisciplinary approach to diagnosis and treatment. Objectives Our goal is to present a best practice example of an interdisciplinary day clinic through two case presentations, aiming improving innovative strategies for assessment/treatment of psychiatric disorders in young adulthood. Methods Via two comprehensive case presentations, we will introduce a psychotherapeutic day-clinic concept from a psychiatric training hospital in Germany. Results The day clinic's interdisciplinary team uses therapeutic approaches like dynamic, cognitive-behavioral, and systemic therapy to understand young adults beyond just their symptoms. Milieu-therapeutic methods, family constellations, socio-therapeutic approaches and non-verbal therapies are incorporated into our concept. Case 1: A 20-year-old male patient, previously diagnosed with schizophrenia, was referred due to symptoms of living in an unreal world with perceived magical abilities and family conflicts. In the evaluation the features of high-functioning autism spectrum disorder (ASD) were more prominent than the psychotic symptoms. Developmental history and diagnostic tools yielded the diagnosis of ASD. Magical abilities in an unreal world appears to align more closely with repetitive/restrictive patterns of behavior, hereby we excluded in the follow-up the diagnosis of schizophrenia. Psychoeducation, social-skills-training and family interventions helped him to comprehend his strengths and discover a clearer direction in his life. Case 2: Another 20-year-old male patient was referred with depressive symptoms, a sense of emptiness and self-mutilation. Following routine evaluation, we employed systemic methods (genogram constellations) to gain deeper insight into the patient's psychopathology. His mother's migration history from Thailand, coupled with unfulfilled aspirations, echoed in his recurring thought: "Where are my roots?" During follow-up, we recognized his passive stance toward therapeutic change, addressed through a systemic intervention known as 'taking the side of non-change.' This shifted his position from resistance to openness. Non-verbal approaches, family interventions, and corrective in-vivo experiences significantly contributed to his stabilization. Conclusions Specialized psychiatric centers tailored to the unique needs of young adults play a critical role in evaluating, diagnosing, and treating psychiatric crises during this developmental stage. Abstract:Borderline personality disorder and eating disorders frequently co-occur among youths. These disorders emerge in adolescence, during the critical developmental period of building an independent sense of self and the capacity to relate to one's community. Because of core differences in the development and psychopathology of borderline personality disorder and eating disorders, adjustments are required when treating these disorders when they co-occur. Few established treatment approaches can address these disorders simultaneously. Evidence-based psychotherapies for borderline personality disorder, such as dialectical behavior therapy and mentalization-based treatment, have been adapted to accommodate the shared vulnerabilities and features of the two disorders. However, these approaches are specialized, intensive, and lengthy and are therefore poorly suited to implementation in general psychiatric or primary health care, where most frontline mental health care is provided. Generalist approaches can fill this public health gap, guiding nonspecialists in structuring informed clinical management for these impairing and sometimes fatal disorders. In this overview, the authors describe the adjustment of good (or general) psychiatric management (GPM) for adolescents with borderline personality disorder to incorporate the prevailing best practices for eating disorder treatment. The adjusted treatment relies on interventions most clinicians already use (diagnostic disclosure, psychoeducation, focusing on life outside treatment, managing patients' self-destructive behaviors, and conservative psychopharmacology with active management of comorbid conditions). Limitations of the adjusted treatment, as well as guidelines for referring patients to specialized and general medical treatments and for returning them to primary generalist psychiatric care, are discussed.
Mother-child interactions in adolescents with borderline personality disorder traits and the impact of early life maltreatment

Abstract:Background: Early detection and intervention of borderline personality disorder (BPD) in adolescence has become a public health priority. Theoretical models emphasize the role of social interactions and transgenerational mechanisms in the development of the disorder suggesting a closer look at caregiver-child relationships. Methods: The current study investigated mother-adolescent interactions and their association with adolescent BPD traits by using a case-control design. Thirty-eight adolescent patients with ≥ 3 BPD traits and their mothers (BPD-G) were investigated in contrast to 35 healthy control dyads (HC-G). Maternal, adolescent and dyadic behavior was coded using the Coding Interactive Behavior Manual (CIB) during two interactions: a fun day planning and a stress paradigm. Additional effects of maternal and/or adolescent early life maltreatment (ELM) on behavior were also explored. Results: BPD-G displayed a significantly lower quality of maternal, adolescent and dyadic behavior than the HC-G during both interactions. Maternal and adolescent behavior was predicted by BPD traits alone, whilst dyadic behavior was also influenced by general adolescent psychopathology. Exploratory analyses of CIB subscales showed that whilst HC-G increased their reciprocal behavior during stress compared to the fun day planning, BPD-G dyads decreased it. Maternal ELM did not differ between groups or have any effect on behavior. Adolescent ELM was correlated with behavioral outcome variables, but did not explain behavioral outcomes above and beyond the effect of clinical status. Discussion/conclusion: Our data suggest a stronger focus on parent-child interactions in BPD-specific therapies to enhance long-term treatment outcomes in adolescent BPD patients. Further research employing study designs that allow the analyses of bidirectional transactions (e.g. longitudinal design, behavioral microcoding) is needed.

Abstract:Borderline personality disorder (BPD) is considered as a disorder of autonomy, and is related to both predisposing vulnerabilities and social relationships that fail to support basic psychological needs. Autonomy, which is defined within the self-determination theory as the capacity for self-endorsed action based on integrative, reflective awareness, is discussed as a developmental line that is dependent on specific supports from caregivers. Unresponsiveness, invalidation, or abuse by caregivers is argued to impair the capacity for autonomy and to catalyze an array of processes, both biological and psychological, which impact subsequent development and, in vulnerable individuals, can lead to BPD. Abstract:Self-disturbance is recognized as a key symptom of Borderline Personality Disorder (BPD). Although it is the source of significant distress and significant costs to society, it is still poorly specified. In addition, current research and models on the etiology of BPD do not provide sufficient evidence or predictions about who is at risk of developing BPD and self-disturbance, and why. The aim of this review is to lay the foundations of a new model inspired by recent developments at the intersection of social cognition, behavioral ecology, and developmental biology. We argue that the sense of agency is an important dimension to consider when characterizing self-disturbances in BPD. Second, we address the poorly characterized relation between self-disturbances and adverse life conditions encountered early in life. We highlight the potential relevance of Life-History Theory—a major framework in evolutionary developmental biology—to make sense of this association. We put forward the idea that the effect of early life adversity on BPD symptomatology depends on the way individuals trade their limited resources between competing biological functions during development.

Abstract:Gold Card SA is a four-session structured psychological intervention offered soon after an acute crisis presentation to people with symptoms characteristic of borderline personality disorder. This study describes individual and system-level outcomes from a large-scale health-care improvement initiative to implement Gold Card SA across South Australia. An uncontrolled pre-post study design was utilized examining service user (n = 332) patient-reported outcome measures and hospital service utilization records (6 months before and after Gold Card SA). Mixed-effects negative binomial regression analysis revealed a significant decrease in rates of service utilization across emergency department presentations (63%), mental health-related inpatient admissions (65%), and bed days (82%). Linear mixed-effect regression indicated large reductions in borderline symptoms and nonspecific psychological distress, and small to moderate improvements in psychosocial functioning. People presenting with or experiencing borderline personality disorder symptoms may benefit from a brief crisis intervention embedded within a stepped care model.

Abstract:Borderline personality disorder is a complex psychopathological phenomenon. It is usually thought to consist in a vast instability of different aspects that are central to our experience of the world, and to manifest as “a pervasive pattern of instability of interpersonal relationships, self-image, and affects, and marked impulsivity” [ American Psychiatric Association (APA), 2013 , p. 663]. Typically, of the instability triad—instability in (1) self, (2) affect and emotion, and (3) interpersonal relationships—only the first two are described, examined, and conceptualized from an experiential point of view. In this context, disorders of self have often motivated analyses of self-experience and the sense of self, affective disorders have been frequently considered in the light of emotional experience and its phenomenological structure. Patterns in the phenomenology of social experience have found comparatively little traction when it comes to the conceptualization of the interpersonal disturbances in borderline. In this paper, I argue that interpersonal instability in borderline consists in much more than fragile and shifting relationships but, most importantly, also involves certain styles in experiencing others. These styles, I suggest, may play an explanatory role for the borderline-typical patterns of interpersonal turmoil and so deserve more attention. To better describe and understand these styles, I explore the phenomenological structure of borderline affective instability and discuss the implications it might have for how a person experiences and relates to other people. Considering core aspects of borderline affective instability, such as alexithymia, emotional contagion, emotion dysregulation, and chronic emptiness, I propose borderline can be interpreted as a disturbance of interaffective exchange, which gives rise to certain ways of experiencing others that imply a social impairment.

Abstract:"Borderline Personality Disorder is a severe condition that affects self and interpersonal dimensions and emotional and behavioral regulation. Since the last decades of the 20th century, an impressive amount of research and clinical contributions on BPD came from specific fields such as psychiatry, clinical psychology, psychopharmacology, and, more recently, cognitive neuroscience. All contributions tackled the challenges of finding reliable diagnostic categories, highlighting detailed developmental trajectories, and fostering effective treatment protocols. However, as results come from different areas, it is often challenging to depict a coherent and yet multifaceted framework on this topic. In this study, we conducted a scientometric analysis of the available literature on BPD to provide a systematic and comprehensive overview of research on BPD and emphasize historical changes, intertwining between fields and new areas of investigation. Results clearly show the evolution of research on BPD starting from the initial development of the construct, passing through the studies on treatment efficacy, the results of longitudinal studies, the advances in cognitive neurosciences, and the recent dimensional conceptualization in DSM-5. Moreover, it emphasizes promising areas of investigation, such as the relations of BPD with NSSI, ADHD, and vulnerable features of narcissism."

Abstract:The notion of the border is an interesting advancement in research on the processes of meaning making within the cultural psychology. The development of this notion in semiotic key allows to handle with adequate complexity construction, transformation, stability and the breakup of the relationship between person/world/otherness. These semiotic implications have already been widely discussed and exposed by authors such Valsiner (2007, 2014), Neuman (2003, 2008), Simão (Culture & Psychology, 9, 449–459, 2003, Theory & Psychology, 15, 549–574, 2005, 2015), with respect to issues of identity/relatedness, inside/outside, stability/change in the irreversible flow of the time. In this work, after showing some of the basics of such semiotic notion of border, we discuss the processes of construction and transformation of borders through the modal articulation, defined as the contextual positioning that the person assumes with respect to the establishment of a boundary in terms of necessity, obligation, willingness, possibility, permission, ability. This modal subjective positioning acquires considerable interest from the clinical point of view since its degree of plasticity vs that of rigidity is the basis of processes of development or stiffening of relations between person/world/otherness.

Abstract:Objective: This study aimed to quantify the instability of psychotherapy process variables by using a novel data-analytic approach. The study explored instability of the working alliance over 10 treatment sessions and its relationship with self-esteem. Methods: Data were extracted from a randomized controlled trial, conducted in Switzerland, of a short-term intervention for borderline personality disorder. Sixty clients diagnosed as having borderline personality disorder were randomly assigned to receive either 10 sessions of good psychiatric management-brief version (GPM-BV) or GPM-BV combined with a relational principle called motive-oriented therapeutic relationship. Square successive differences were calculated for client and therapist ratings of alliance instability. Multilevel models were used to test within- and between-person associations of alliance instability with self-esteem. Results: Although some preliminary analyses showed an association between a more stable alliance and higher self-esteem at the start of psychotherapy, the alliance did not become more stable over time. Alliance instability was not associated with self-esteem at either the within- or between-person level. Conclusions: This study highlights the advantages, procedures, and challenges of applying square successive differences to psychotherapy research data. The results suggest that the working alliance develops in complex ways and indicate the importance of continuing to use novel methods to capture dynamic psychotherapy processes.

Abstract:Emotion dysregulation (ED) has primarily been described in patients suffering from borderline personality disorder (BPD) and is an integral part of this diagnosis, but it is also a transdiagnostic construct that can be found in several other psychiatric disorders. The strong relationships between ED and BPD may lead clinicians to underestimate ED associated to other clinical contexts. This can lead to difficulties in diagnostic and treatment orientation, especially in the context of comorbidities. In this article, after reviewing the literature on the development and functioning of emotion dysregulation, and on the evidence for emotion dysregulation in eight disorders (borderline personality disorder, pathological narcissism with/without narcissistic personality disorder, obsessive-compulsive personality disorder, antisocial personality disorder, bipolar disorder, autism spectrum disorder, complex post-traumatic stress disorder, and adult attention deficit hyperactivity disorder), we present a transdiagnostic processual model of emotion dysregulation based on core triggers and interpersonal styles to try to address this issue and to provide a simple but technical tool to help clinicians in their diagnostic assessment and treatment orientation. By focusing more on typical patterns and interpersonal dynamics than only on categories, we believe that this model may contribute to the actual need for improvement of our current psychiatric classifications, alongside other well-studied and under-used dimensional models of psychopathology (e.g., HiTOP, AMPD), and may be useful to build more specific treatment frameworks for patients suffering from ED.

Notes on countertransference in borderline conditions

Abstract:"When dealing with borderline or severely regressed patients, as contrasted to those presenting symptomatic neuroses and many character disorders," Kernberg (1975) observed, "the therapist tends to experience rather soon in the treatment, intensive emotional reactions having more to do with the patient's premature, intense and chaotic transference and with the therapist's capacity to withstand psychological stress and anxiety, than with any specific problem in the therapist's past" (pp. 16-17). The argument in the present paper contends that Kernberg's description of countertransference reactions to borderline patients is a caricature that applies only within a limited range of borderline conditions, specifically the most primitive or poorly organized level of borderline functioning or regressive borderline states. The description does not apply to the full range of borderline psychopathology, and in many cases can be misleading. It has generally been recognized that countertransference vicissitudes play an extremely important role in the therapy of borderline patients at one or another phase of the treatment. At critical points in the development of transference (TR) and countertransference (CT) and their interaction, the therapist's recognition of and capacity to deal with CT issues become crucial to the treatment progression. The present paper explores the dimensions of CT experience and provides a tentative model for the understanding of transference-countertransference (TR/CT) interactions. The central points emphasized are: (1) that borderline disorders form a spectrum of character pathology falling between the psychoses on one side and the narcissistic personality disorders on the other; (2) that these conditions reflect varying degrees of pathological disruption and varying levels of personality integration; (3) that, depending on the form of pathological organization, these conditions manifest a variety of types and degrees of intensity of TR involvement and elicit corresponding CT reactions; and, finally (4) that these forms and degrees of TR/CT call for corresponding modifications in therapeutic response.

Abstract:The purpose of this paper is to provide a developmental theoretical framework to scientific and applied work in the field The following recognizes that numerous the trajectory of The central tenets of a critical, transactional, ecological, and developmental perspective includes a synthesis of various contributions and influences grounded in theories from leading developmental scholars including the works of Sameroff (e.g., transactional), Bronfenbrenner (e.g., ecological), Bowlby (e.g., attachment), and the Frankfurt School (e.g., critical theory). The following provides an overview of the conceptual foundations that inform practice and research, with an emphasis placed on the dynamic and reciprocal transactions between the individual and their environment across time and the social complexities brought by such non-ceasing and reciprocal social relations. This overview also describes empirical support informing these conceptual foundations with special attention to longitudinal studies demonstrating these various influential factors across diverse developmental trajectories. A discussion of implications informing future research and practice endeavors as well as prevention and intervention efforts to promote Abstract Conceptual foundations inform our professional activities as practitioners and scholars. Also included is a discussion of, implications informing prevention and intervention efforts to promote academic and social- emotional learning for all children, future research directions, and advocacy stance to promote shift and change to interrupt the long-standing social and educational injustices in the field of education.
