Abstract:<section class="article-section__content"><h3 class="article-section__sub-title section1"> Aim</h3><p>To investigate self‐care developmental trajectories in children with cerebral palsy (CP) across all functional ability levels, according to Manual Ability Classification System (MACS) levels.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Method</h3><p>This was a prospective longitudinal population‐based study of 71 children aged from 2 years 6 months to 12 years, with CP (47 [66%] males, 24 [34%] females). Pediatric Evaluation of Disability Inventory (PEDI) measures were taken at 2 years 6 months, 3, 4, and 5 years, and the PEDI ‐ Computer Adaptive Test (PEDI‐CAT) between 8 and 12 years. At 8 to 12 years, children were classified in MACS levels I (21; 30%), II (22; 31%), III (16; 23%), IV (6; 8%), and V (6; 8%). Longitudinal analysis of the PEDI Functional Skills Scale self‐care and PEDI‐CAT daily activities domains used the published linking equation, and multilevel mixed‐effects regression modelling with interaction between age and MACS.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Results</h3><p>Between 5 and 12 years of age, children classified in MACS levels I to III continued to show progress in self‐care development (PEDI‐CAT scaled scores estimated change per year: I, 0.72; II, 0.49; III, 0.48). Children classified in MACS level IV showed an upward non‐significant trend between 5 and 8 to 12 years (estimated change 0.42; 95% confidence interval [CI] −0.04 to 0.88). Children in MACS level V showed a decline in self‐care (estimated change: −0.65; 95% CI −1.16 to −0.14).</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Interpretation</h3><p>Self‐care development attained by 8 to 12 years of age was related to the severity of manual ability impairment. Application of the linking equation from PEDI to PEDI‐CAT is somewhat uncertain at the extreme lower end of the scale. Our study supports recommendations for items to be added to the PEDI‐CAT to address floor effect.</p></section>

Self‐care and manual ability in preschool children with cerebral palsy: a longitudinal study

Abstract:AIM: To describe longitudinal development of self-care and its relationship to manual ability in children with cerebral palsy (CP) aged 18 months to 5 years across all functional abilities.METHOD: This was a prospective longitudinal population-based study of 290 children with CP (178 [61%] males, 112 [39%] females). Self-care was assessed using the Pediatric Evaluation of Disability Inventory (PEDI). At 60 months (n=242), children were classified using the Manual Ability Classification System (MACS); 113 in level I (47%), 61 in MACS level II (25%), 24 in MACS level III (10%), 14 in MACS level IV (6%), and 30 in MACS level V (12%). Measures were taken at 18 months, 24 months, 30 months, 36 months, 48 months, and 60 months of age. Longitudinal analyses were performed using mixed-effects linear regression models.RESULTS: Self-care development achieved by 60 months was negatively associated with the severity of manual ability impairment. Distinct self-care developmental trajectories were found with estimated changes in PEDI self-care scaled scores per month: 0.61 for MACS level I, 0.46 for MACS levels II, 0.31 for MACS level III, 0.16 for MACS level IV, and 0.03 for MACS level V. Children classified in MACS level V had the lowest level of self-care skills at 18 months and showed no progress in self-care development.INTERPRETATION: This study reports rate of self-care development in preschool children with CP. Self-care performance was highest in children with greatest manual ability. Clinicians may use rates of change to predict or monitor self-care performance. PEDI trajectories inform goal setting in discussions with families regarding expected levels of independence in self-care.WHAT THIS PAPER ADDS: Distinct self-care developmental trajectories in children with cerebral palsy were found according to Manual Ability Classification System (MACS) levels. Children in MACS levels IV and V with epilepsy did not show any significant change in self-care. Children in MACS levels IV and V without epilepsy demonstrated small yet significant gains in self-care performance.

Abstract:OBJECTIVE: To investigate the factors influencing the development of self-care activity, and the association between mobility and self-care activity in children with cerebral palsy (CP).METHODS: A total of 63 CP children aged ≥4 years, were studied retrospectively. Children with severe intellectual disability or behavioral problems were excluded. The relationship between the Gross Motor Function Classification System (GMFCS), the Manual Ability Classification System (MACS), and the Pediatric Evaluation of Disability Inventory (PEDI) was analyzed. Simple and multiple linear regression analyses were conducted for continuous variables, such as verbal intelligence quotient (IQ) and PEDI subscales.RESULTS: Final evaluation was done for 25 children, ranging from 4 to 11 years of age. According to GMFCS levels, the differences in PEDI-self-care scores, showed statistically borderline significance (p=0.051). Conversely, differences in PEDI-self-care scores according to CP types and MACS levels were not statistically significant. Simple linear regression analysis showed that PEDI mobility and PEDI social function significantly influence the PEDI self-care. Multiple linear regression analysis showed that PEDI mobility was the only factor significantly influencing PEDI self-care in children aged ≥7 years (R<sup>2</sup>=0.875, p=0.03).CONCLUSION: Mobility is important for the acquisition of self-care abilities in children with CP aged ≥7 years.
Factors for changes in self-care and mobility capabilities in young children with cerebral palsy involved in regular outpatient rehabilitation care

Abstract:BACKGROUND: Assessing prognosis of self-care and mobility capabilities in children with cerebral palsy (CP) is important for goal setting, treatment guidance and meaningful professional-caregiver conversations.AIMS: Identifying factors associated with changes in self-care and mobility capabilities in regular outpatient multidisciplinary paediatric CP rehabilitation care.METHODS AND PROCEDURES: Routinely monitored longitudinal data, assessed with the Paediatric Evaluation of Disability Inventory (PEDI-Functional-Skills-Scale, FSS 0-100) was retrospectively analysed. We determined contributions of age, gross-motor function, bimanual-arm function, intellectual function, education type, epilepsy, visual function, and psychiatric comorbidity to self-care and mobility capability changes (linear-mixed-models).OUTCOMES AND RESULTS: For 90 children (53 boys), in all Gross-Motor-Function-Classification-System (GMFCS) levels, 272 PEDI's were completed. Mean PEDI-FSS-scores at first measurement (median age: 3,2 years) for self-care and mobility were 46.3 and 42.4, and mean final FSS-scores respectively were 55.1 and 53.1 (median age: 6,5 years). Self-care capability change was significantly associated with age (2.81, p < 0.001), GMFCS levels III-V (-9.12 to -46.66, p < 0.01), and intellectual impairment (-6.39, p < 0.01). Mobility capability change was significantly associated with age (3.25, p < 0.001) and GMFCS levels II-V (-6.58 to -47.12, p < 0.01).CONCLUSIONS AND IMPLICATIONS: Most important prognostic factor for self-care and mobility capabilities is GMFCS level, plus intellectual impairment for self-care. Maximum capability levels are reached at different ages, which is important for individual goal setting and managing expectations.
Determinants of Developmental Gain in Daily Activities in Young Children with Cerebral Palsy

Abstract:ABSTRACT The aim of this study was to examine which child and family characteristics at the child's age of 2 years are determinants of development of self-care and mobility activities over a period of 2 years in young children with cerebral palsy (CP). Longitudinal data of 92 children, representing all levels of the Gross Motor Function Classification System (GMFCS), were analyzed. Children's self-care and mobility activities were assessed with the Functional Skills Scale of the Pediatric Evaluation of Disability Inventory. Development of self-care and mobility activities was related to several child determinants but no family determinants. GMFCS, type of CP, intellectual capacity, and epilepsy were related to the development of self-care and mobility activities, while manual ability and spasticity were related to development of mobility activities. Multivariate analysis indicated that GMFCS and intellectual capacity were the strongest determinants of development of self-care activities, and GMFCS was the strongest determinant of development of mobility activities. The change in self-care and mobility activities was less favorable in severely affected children with severe disability. Knowledge of GMFCS level and intellectual capacity is important in anticipating change over time and goal setting in young children with CP.

Self-Care Skills of Children With Pervasive Developmental Disorders

Abstract:Self-care skills are the everyday tasks undertaken so children are ready to participate in life activities. While they are typically supported by parents or carers in young children, it is expected that children develop independence while getting older and maturing. Self-care skills are assumed as precursors for most school-related tasks, are expected to be performed independently, and usually it becomes inappropriate for others to assist for tasks related to these. Assistive technology enhances to strengthen children's ability to participate in eating, dressing, bathing, and personal hygiene routines. As a self-management tool, assistive technology helps children with pervasive developmental disorders or mild intellectual disabilities promote to perform daily tasks involved in activities related to self-care skills. In this chapter, first, information about self-care problems with a focus on children with pervasive developmental disorders is given. Then, opportunities, challenges, and assistive tools are presented. Finally, future research directions in this domain are stated.
Building independence in self-care and household tasks: a qualitative study with adolescents with cerebral palsy and their caregivers

Abstract:Purpose: Adolescents with cerebral palsy (CP) may experience limitations in self-care and household tasks. The aim of the study was to understand the process of building independence in self-care and household tasks from the perspective of adolescents and their caregivers. Materials and methods: We conducted a qualitative study with a phenomenological approach with 10 adolescents (15-17 years old) and 11 caregivers from a transition service in Brazil. Semistructured, remote interviews were conducted with each participant. The interviews were recorded for transcription and content analysis. Results: Two thematic categories emerged: (1) "Thinking about independence" and (2) "Possible ways to foster independence." The first category was divided into three subcategories: "Expectations and the desire to be independent," "Obstacles to independence," "Concerns about the future." The second category was divided into four subcategories: "People and places," "Opportunities for practice," "Personal attitude" and "Adaptations that facilitate." Conclusion: Adolescents' independence in several everyday activities involves physical, socioemotional and environmental aspects. Enabling opportunities for practice, an adapted environment, support from rehabilitation services and developing collaborative relationships with caregivers are elements that may favor the independence of adolescents with CP.
Use of the Child Engagement in Daily Life and Ease of Caregiving for Children to Evaluate Change in Young Children with Cerebral Palsy

Abstract:ABSTRACT Aims: Participation in family and recreational activities, self-care, and parent ease of caregiving are important outcomes for young children with cerebral palsy (CP). The aim of this study was to examine use of the Child Engagement in Daily Life and the Ease of Caregiving for Children to measure change over time. Methods: A convenience sample of 387 parents of young children with CP (18 months to 5 years of age) completed the measures twice, a mean of 12.7 months apart. Results: For the Child Engagement in Daily Life, parents of children in Gross Motor Function Classification System level I and levels II-III reported more change for the Self-care domain (medium effect) than the Family and Recreational Activities domain (small effect) and the Ease of Caregiving for Children (small effect). The change reported by parents of children in levels IV-V on all three measures was less than the criterion for a small effect. Minimal detectable change for each measure varied from 12.1 to 14.1, out of a total possible score of 100. Conclusion: Further research is recommended to determine responsiveness to change following intervention.

Abstract:Self-care skills are integral to how children manage their daily lives without help. Without good motor skills, the children have difficulty achieving independence in self-care. This study aimed to explore the understanding on teaching of motor skills related to self-care skills, factors inhibit teaching self-care and interest to have module as guidance to preschool children from the parent's and teachers' perspectives. This was a qualitative study approach that recruited eight participants, which included both parents and teachers. Parents and teachers involved in providing care to preschool children aged four to five years were selected for this study. The data were collected and recorded through semi-structured interviews individually. All the transcribed data were analysed by using thematic analysis. Findings indicated five themes that appeared as follows: "Limited amount of knowledge", "Time constraints", "Needs guidance", "Child's factors", and "Non-standardised guidance". Parents show limited understanding of children's needs and development. Parents and teachers lack awareness role of motor skills that could support self-care independence. Development motor skills module to support self-care skills as guidance is recommended, and further study could be considered to explore more on development of motor skill module to promote early self-care independent.

Abstract:Objective Manual ability is considered one of the factors that can predict functional independence in activities of daily living. For evaluating personal tasks such as self-care, the Pediatric Evaluation of Disability Inventory (PEDI) comprises/introduces/offers a set of useful measures that assist in enhancing the capability for self-care among children and adolescents with cerebral palsy (CP). The aim of this study was to investigate the relevant factors of self-care capability and performance in children and adolescents with spastic CP. Methods This was a cross-sectional study. Seventy-six children and adolescents with spastic CP (between 5 and 18 years of age), representing levels I to IV of the Gross Motor Function Classification System-Expanded & Revised version (GMFCS), were analyzed. Multiple linear regression analysis with forward stepwise selection was conducted to examine which determinants were related to self-care capability and performance. Independent variables were age, CP type, GMFCS, Manual Ability Classification System, Box and Block Test, and grip strength in the dominant and non-dominant hands. Dependent variables were scores for the PEDI Functional Skills Scale and the PEDI Caregiver Assistance Scale. Results Results of the multiple regression analysis showed that the PEDI Functional Skills scale scores were correlated with the Box and Block Test in the dominant hand and GMFCS (Adjusted R 2 = 0.69). The PEDI Caregiver Assistance Scale scores were correlated with the Box and Block Test in the dominant hand, GMFCS, and age (adjusted R 2 = 0.71). Conclusion When considering self-care of children and adolescents with spastic CP, it is necessary to consider the evaluation of upper limb dysfunction in addition to GMFCS.

A Systematic Review Of Development On Motion-Based Games On Upper Extremities Function For Self-Care Ability Among Children With Autism Spectrum Disorder

Abstract:In reaction to the pressing issue surrounding the self-care capacities of children with autism spectrum disorder, this paper reviews the existing empirical studies proving the usefulness of motion-based games therapy for children with autism spectrum disorder. This study is a systematic review of published studies. The databases Academic Search Complete, MEDLINE Complete,CINAHL, Scopus, and Eric were searched, revealing studies that were eligible for review. A total of 20 cross-sectional studies, all of which had relatively small samples, were included in the final analysis. Five included studies were from China, and six were from the USA. Most of the 20 studies employed experimental methodologies to investigate intervention moderation in a specific direction for children with autism. Fourteen employed qualitative research to examine the intervention and had fewer than or equal to 20 participants. The number of subjects in four studies topped 100,mostly in the form of quantitative research. The other four papers used a mixed-methods approach. This review demonstrates that interventions (interactive games such as somatosensory games, particularly for hand function training) can significantly enhance autistic children's self-care abilities. However, it is vital to do in-depth research on these therapy tools, given the basic state and growth of children with ASD.
Child characteristics, caregiver characteristics, and environmental factors affecting the quality of life of caregivers of children with cerebral palsy

Abstract:Purpose: The study aimed to investigate comprehensively the determinants of the quality of life (QOL) of caregivers of children with cerebral palsy (CP) based on the International Classification of Functioning, Disability and Health for Children and Youth (ICF-CY). Methods: A total of 167 children with CP (mean age 9.06 years, SD 2.61 years) and their caregivers (mean age 40.24 years, SD 5.43 years) participated in this study. The QOL of caregivers was measured with the World Health Organization Quality of Life-BREF-Taiwan version (WHOQOL-BREF-TW). The potential determinants of QOL were collected, including child characteristics, caregiver characteristics, and environmental factors from all dimensions of the ICF-CY and analysed using multiple regression models. Results: Four multiple regression models revealed that determinants of the QOL of caregivers of children with CP was multidimensional, encompassing child characteristics (age, type of CP, fine motor impairment, other diseases, behaviour and emotions, visual impairment, hearing impairment), caregiver characteristics (general mental health, parenting stress, marital status, family coping patterns, and socio-economic status), and environmental factors (child's medication, school setting, and current rehabilitation service, caregiver's spouse's age, family life impacts, and domestic helper). Conclusions: Knowledge of the determinants of QOL could serve as a guide in a holistic approach to evaluation and intervention and help plan interventions targeted at these determinants to improve the QOL of caregivers of children with CP. Implications for Rehabilitation Caregivers of children with CP had lower QOL, except the environment QOL. The QOL determinants of caregivers of children with CP are multidimensional, including child characteristics, caregiver characteristics, and environmental factors. In addition to child characteristics of severity of fine motor impairments and emotional and behavioural problems, caregiver characteristics of general mental health, parenting stress, and coping patterns, and environmental factors of family life impacts, and school setting demonstrated important relationships with caregiver QOL.
Development of a Group Curriculum for Children With Congenital Upper Limb Difference (CULD) and Their Caregivers

Abstract:Caring for patients with congenital upper limb differences (CULD) requires an understanding of unique psychosocial challenges. The study purpose was to describe the needs of children with CULD and their caregivers to develop a group curriculum. This mixed-method study included 13 mothers and one father of children with CULD (age 3 months to 14 years; mean 7.2 ± 4.3 years) using Patient-Reported Outcomes Measurement Information System Parent Proxy Reports and caregiver interviews. Patient-Reported Outcomes Measurement Information System Peer Relationships (M = 48.1 ± 10.9) and Pain Interference (M = 44.5 ± 7.5) T-scores were average with below average scores for Physical Function: Upper Extremity (M = 31.9 ± 12.1). Caregivers expressed high interest in groups to create community and mutual support covering themes of responding to questions from strangers, social and coping skills, building self-confidence, accessing resources, advocacy skills, and advice from adults with CULD. Access to a support group with a curriculum addressing CULD-related patient and family needs may improve care.

Abstract:Abstract Low parental coparenting and self-efficacy negatively impact children’s development. Our study sought to describe and analyze the parental coparenting and self-efficacy of 84 caregivers of children with cerebral palsy (CP) in Brazil. We used a Sociodemographic Inventory (SDI), Gross Motor Function Classification System (GMFCS), Self-efficacy Scale and Coparenting Questionnaire (CQ). Data were subjected to descriptive statistics and Correspondence Analysis. ISD results showed the mothers as the main caregivers of children with CP, being mostly girls at level V in the GMFCS scale. Low levels of coparenting and self-efficacy were observed in young caregivers, caring for girls older than six years old. Self-efficacy was higher regarding basic and daily care such as feeding and performing personal hygiene, and lower regarding seizures and subjective aspects. No significant correlations were found between coparenting and self-efficacy. We concluded that interventions should be planned to increase parental coparenting and parental self-efficacy.

Abstract:<section class="article-section__content"><p>Studies investigating the performance of self‐care and visual perception in young children with autism spectrum disorder (ASD) are limited. The relationship between self‐care performance and visual perception ability in young children with ASD is not yet clearly understood. Here, self‐care performance was evaluated by the caregivers and therapists of children with ASD. The differences in self‐care performance and visual perception ability were investigated in 66 children with ASD and 66 typically developing (TD) children between the ages of 48–83 months. The relationships between self‐care and visual perception were tested in both two groups. The Assessment of Motor and Process Skills (AMPS) and the Chinese version of the Pediatric Evaluation of Disability Inventory (PEDI‐C) were used to assess the children's self‐care performance. The Test of Visual Perceptual Skills‐Third Edition (TVPS‐3) and the Developmental Test of Visual Perception‐Third Edition (DTVP‐3) were used to evaluate visual perception ability. Young children with ASD obtained significantly lower scores for self‐care performance (AMPS and PEDI‐C) and visual perception ability (TVPS‐3 and DTVP) compared with TD children. Additionally, positive correlations were found between self‐care performance and visual perception ability in young children with ASD. The results provide a valuable contribution to our understanding about self‐care and visual perception performance of young children with ASD. The findings of this research highlight the need for pediatric practitioners to include self‐care and visual‐motor integration evaluations for young children with ASD.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Lay Summary</h3><p>Young children with ASD obtained significantly lower scores for self‐care performance and visual perception ability compared with TD children. Abstract:To explore the effects that impaired ability to sit-to-stand has on upright mobility and self-care in children with cerebral palsy and how this in turn may affect their caregivers. A mixed methods research design was conducted with 25 children who had cerebral palsy with moderate to high mobility limitations (GMFCS levels III and IV) and their caregivers. Caregivers were interviewed about their child's mobility and self-care. The independence of each child's activities was rated using the mobility and self-care domains of the Functional Independence Measure for Children (WeeFIM). Two themes were identified from qualitative analyses: Difficulty in sit-to-stand was perceived by caregivers to reduce their child's ability to independently and safely perform mobility and some self-care tasks; and negatively impacted the caregivers physically and psychologically. Mobility and self-care WeeFIM scores showed that these children required moderate assistance, and that self-care tasks involving sit-to-stand (toileting and bathing) required more assistance than self-care tasks that would not be expected to involve sit-to-stand (eating and grooming). Qualitative and quantitative findings were convergent. The ability to sit-to-stand independently may be an important precursor skill for independence in upright mobility and self-care for children with moderate to severe mobility limitations.

Longitudinal development of hand use in children with unilateral spastic cerebral palsy from 18 months to 18 years

Abstract:Aim: To describe the development of the use of the affected hand in bimanual tasks in children with unilateral cerebral palsy (CP) from 18 months to 18 years. Specifically, whether early development can be confirmed in a larger cohort and how development progresses during adolescence. Method: In total, 171 participants (95 males, 76 females; mean age 3 years 1 month [SD 3 years 8 months], range 18 months-16 years at inclusion) were classified in Manual Ability Classification System (MACS) levels I (n = 41), II (n = 91), and III (n = 39). Children were assessed repeatedly (median 7, range 2-16 times) with the Assisting Hand Assessment: in total 1197 assessments. Developmental trajectories were estimated using a nonlinear mixed effects model. To further analyse the adolescent period, a linear mixed model was applied. Results: The developmental trajectories were different between participants in MACS levels (MACS I-II, II-III) in both rate (0.019, 95% confidence interval [CI] 0.006-0.031, p = 0.034; 0.025, 95% CI 0.015-0.037, p < 0.001) and limit (19.9, 95% CI 16.6-23.3, p = 0.001; 7.2, 95% CI 3.3-11.2, p < 0.003). The individual variations were large within each level. The developmental trajectories were stable over time for all MACS levels between 7 and 18 years (p > 0.05). Interpretation: Children and adolescents with unilateral CP have considerable development at an early age and a stable ability to use their affected hand in bimanual activities from 7 to 18 years in all MACS levels.
