Abstract:Tobacco use contributes to more mortality and morbidity globally than any other behavioral risk factor. Adverse effects do not spare the oral cavity, with many oral diseases more common, and treatments less successful, in the tobacco-using patient. Many of the oral health effects of cigarette smoking are well established, but other forms of tobacco, including cigars and smokeless tobacco, merit dental professionals' attention. Recently, an expanding variety of new or emerging tobacco and/or nicotine products has been brought to market, most prominently electronic cigarettes, but also including heated tobacco and other noncombustible nicotine products. The use of cannabis (marijuana) is increasing and also has risks for oral health and dental treatment. For the practicing periodontist, and all dental professionals, providing sound patient recommendations requires knowledge of the general and oral health implications associated with this wide range of tobacco and nicotine products and cannabis. This review provides an overview of selected tobacco and nicotine products with an emphasis on their implications for periodontal disease risk and clinical management. Also presented are strategies for tobacco use counselling and cessation support that dental professionals can implement in practice.

Abstract:In the last decade there has been a significant increase in the appeal and popularity of e-cigarettes. Recent national news headlines outline that one million smokers will be given a free vaping starter kit to encourage them to give up tobacco products. An independent report commissioned by the UK Government has cited promotion of vaping as a critical recommendation to ensuring England is smoke-free by 2030. Undoubtedly, the dental team will now encounter many more questions from patients keen to know more regarding the safety of electronic nicotine delivery systems and their effects on the oral cavity. However, it is often difficult to answer these questions due to a lack of evidence regarding their impact. Although there are some preliminary animal and in vitro data, additional well-designed, long-term studies are required to investigate oral health outcomes of e-cigarette use.We aim to summarise the latest evidence to better inform clinicians about the effects of vaping on oral health, particularly regarding the risks of oral cancer, so they can better inform their patients.

Abstract:Oral nicotine pouches are emerging as a new "modern oral" nicotine product. These prefilled pouches contain nicotine, flavorings, and filling agents that dissolve in the mouth. Nicotine can be derived from tobacco leaf or chemical synthesis. Traces of TSNAs and toxic chromium were detected in the pouch products. This raises the concern about general and periodontal health. This review aims to update the current oral nicotine products research relating to periodontal disease and its relevance in periodontal inflammation. Nicotine interacts with host cells and affects inflammatory responses to microbial challenges. It may directly or indirectly deteriorate periodontal tissues by activating nicotinic acetylcholine receptors, repressing PDL fibroblasts cells, increasing cellular ROS and cytokines/chemokines, growth factors, breaking microbiota balance, and dysregulating miRNAs expression. Studies show that appealing flavorings contained in nicotine pouches pose harm to periodontal innate immune responses and increase penetration of nitrosamines. In addition, flavored ONPs increase the risk of dual or poly-tobacco products among young adults, stacking up detrimental effects on the periodontium. Given the recent growth of users, further studies are needed to elucidate the impact of ONPs, even poly-tobacco use, on systemic and periodontal health. Moreover, policymakers should ensure to avoid generating a new wave of nicotine addiction among youths in the U.S.

Abstract:Tobacco products present a deadly combination of nicotine addiction and carcinogen exposure resulting in millions of cancer deaths per year worldwide. A plethora of smokeless tobacco products lead to unacceptable exposure to multiple carcinogens, including the tobacco-specific nitrosamine N′-nitrosonornicotine, a likely cause of the commonly occurring oral cavity cancers observed particularly in South-East Asian countries. Cigarettes continue to deliver a large number of carcinogens, including tobacco-specific nitrosamines, polycyclic aromatic hydrocarbons and volatile organic compounds. The multiple carcinogens in cigarette smoke are responsible for the complex mutations observed in critical cancer genes. The exposure of smokeless tobacco users and smokers to carcinogens and toxicants can now be monitored by urinary and DNA adduct biomarkers that may be able to identify those individuals at highest risk of cancer so that effective cancer prevention interventions can be initiated. Regulation of the levels of carcinogens, toxicants and nicotine in tobacco products and evidence-based tobacco control efforts are now recognized as established pathways to preventing tobacco related cancer.

Abstract:Tobacco use continues to be the leading cause of preventable disease and death in the United States. There are no safe tobacco products; however, the health risks for tobacco products exist on a continuum, with combustible products such as cigarettes being the most harmful. Decades of research have documented that cigarette smoking harms nearly every organ of the body; cigarette smoke contains nearly 7,000 chemicals, approximately 70 of which cause cancer 1 . Non-combustible tobacco products, such as e-cigarettes, generally have lower health risks to the user than combusted tobacco products. The US National Academies of Sciences, Engineering, and Medicine (NASEM) found conclusive evidence that switching completely from cigarettes to e-cigarettes reduces a person's exposure to many toxicants and carcinogens that are present in combusted tobacco cigarettes 1 .

Abstract:Smoking and tobacco chewing are the predominant causes of oral cancer. Tobacco is the second-most widely consumed psychoactive substance. There are numerous ways to quit smoking, of which one is electronic cigarettes (e-cigarettes). E-cigarette use is a brand-new, global trend. E-cigarette is a battery-operated device that heats a liquid to create a vapor that the consumer inhales. Several countries have acknowledged that the first step toward electronic nicotine delivery system (ENDS) management is a precise classification of ENDS within the limits of current legislation. Countries have currently categorized ENDS into four generations. People's perceptions about tobacco products have altered recently as a consequence of the advertising of ENDS. The likelihood of starting to smoke cigarettes was four times higher in adolescents who used ENDS, and the probability of quitting was reduced and often prolonged in those who used ENDS. In addition, ENDS normalizes smoking-like actions including inhaling in and exhaling smoke. Adverse marketing via geographic locations and social media platforms, as well as nicotine's irreversible effects on growing adolescent and young adult brains that predispose individuals to addicted behaviors, may be responsible for their rising appeal among teenagers. Despite this, ENDS use has risen among young individuals who have never smoked and undoubtedly face more health risks than those who do not use ENDS. The oral cavity is the first to encounter ENDS in individuals and where it initially affects the human system. As a known contributor to cardiovascular diseases, neurological conditions, and cancers, nicotine seems to be a serious cause for concern. This review provides a concise summary of the research on the components, mode of action, applications, and effects of e-cigarettes on oral as well as systemic systems.

Abstract:INTRODUCTION: Periodontal disease and oral cancer are common health hazards. Epidemiological investigations show that smoking, periodontal disease and oral cancer are closely related. Tobacco is one of the major risk factors for periodontitis and oral cancer.METHODS: A systematic literature review was performed. To identify relevant studies, the following online databases were searched using specific keywords: PubMed, Web of Science and CNKI.RESULTS: Tobacco not only possesses an addictive effect, but it aggravates periodontal disease by promoting the invasion of pathogenic bacteria, inhibiting autoimmune defense, aggravating the inflammatory reaction, and aggravating the loss of alveolar bone. According to current evidence, tobacco significantly aggravates the development and progression of periodontal disease and oral cancer, and periodontal disease may be related to the prevalence of oral cancer.CONCLUSIONS: Clinicians should strongly recommend that smokers undertake a strategy to stop smoking to avoid the exacerbation of nicotine-related periodontal disease and to reduce the incidence of oral cancer.

Abstract:Tobacco use has detrimental effects on health, including oral health. The emergence and increasing popularity of newer tobacco and nicotine products make tobacco use one of the major public health problems in the world. Tobacco use increases the risk of oral diseases such as oral cancer, oral mucosal lesions, periodontal disease, and dental caries, among many other oral diseases and conditions. The dental office is an excellent venue for providing cessation intervention. However, there is a lack of knowledge and training in tobacco use prevention among dental professionals. More efforts are needed for smoking cessation interventions in the dental office. Smoking cessation interventions provided by oral healthcare providers include brief educational, behavioral, and pharmacological interventions. This review provides an overview of the ill effects of tobacco use on oral health and the role of oral healthcare providers in managing and preventing tobacco dependence.
Nicotine replacement therapy and oral health: a network meta-analysis of adverse effects in randomized trials

Abstract:Background: Nicotine replacement therapies (NRTs) are the primary pharmacological option for smokers looking to quit. However, they can lead to oral health issues, including aphthous ulcers, gastric irritation resulting in tooth wear, mouth irritation, soreness, and dryness, which may decrease patient adherence. Dentists should be integrated into the tobacco cessation team to prevent, detect, and manage these oral side effects associated with NRT use. Therefore, the objective of this network meta-analysis is to examine the relationship between different forms of NRTs and their oral side effects. Methodology: Databases were screened for literature and full text articles were obtained for eligible studies. Relevant data was extracted. NMA Studio was used to perform random-effects network meta-analysis to generate mixed treatment comparison pooled estimates from odds ratios (OR) and 95% confidence intervals (CI). The forest plot was generated, and the network diagram was drawn for key comparisons. The OR and 95% CI were estimated from the data on the number of patients that reported the oral side effects in the included studies. Results: A total of 21 articles were included. The Risk of Bias assessment revealed a moderate to low risk across all domains for the included studies. The mixed treatment comparison pooled estimates indicated that nicotine gum was significantly associated with the occurrence of aphthous ulcers [2.25 (1.03, 4.90)] and gastric reflux/vomiting [4.40 (1.51, 12.80)]. Additionally, the results showed that nasal spray significantly caused mouth/throat irritation [5.37 (1.03, 27.92)] and taste disturbance [4.87 (2.83, 8.37)] in comparison to tested NRT interventions. Conclusion: Significant oral side effects are associated with NRT use, as demonstrated in this study. Dentists should implement tobacco cessation methods in their practices, including pharmacotherapy, to effectively monitor these patients. Continuous education and certification programs should be mandatory for dentists who want to practice tobacco cessation. This should also be included in the undergraduate dental curriculum to help dentists gain confidence in prescribing NRT. Standard evidence-based guidelines should be published to outline possible methods for encouraging tobacco cessation in dental settings.
Systems Toxicology Assessment of the Biological Impact of a Candidate Modified Risk Tobacco Product on Human Organotypic Oral Epithelial Cultures

Abstract:Cigarette smoke (CS) has been reported to increase predisposition to oral cancer and is also recognized as a risk factor for many conditions including periodontal diseases, gingivitis, and other benign mucosal disorders. Smoking cessation remains the most effective approach for minimizing the risk of smoking-related diseases. However, reduction of harmful constituents by heating rather than combusting tobacco, without modifying the amount of nicotine, is a promising new paradigm in harm reduction. In this study, we compared effects of exposure to aerosol derived from a candidate modified risk tobacco product, the tobacco heating system (THS) 2.2, with those of CS generated from the 3R4F reference cigarette. Human organotypic oral epithelial tissue cultures (EpiOral, MatTek Corporation) were exposed for 28 min to 3R4F CS or THS2.2 aerosol, both diluted with air to comparable nicotine concentrations (0.32 or 0.51 mg nicotine/L aerosol/CS for 3R4F and 0.31 or 0.46 mg/L for THS2.2). We also tested one higher concentration (1.09 mg/L) of THS2.2. A systems toxicology approach was employed combining cellular assays (i.e., cytotoxicity and cytochrome P450 activity assays), comprehensive molecular investigations of the buccal epithelial transcriptome (mRNA and miRNA) by means of computational network biology, measurements of secreted proinflammatory markers, and histopathological analysis. We observed that the impact of 3R4F CS was greater than THS2.2 aerosol in terms of cytotoxicity, morphological tissue alterations, and secretion of inflammatory mediators. Analysis of the transcriptomic changes in the exposed oral cultures revealed significant perturbations in various network models such as apoptosis, necroptosis, senescence, xenobiotic metabolism, oxidative stress, and nuclear factor (erythroid-derived 2)-like 2 (NFE2L2) signaling. The stress responses following THS2.2 aerosol exposure were markedly decreased, and the exposed cultures recovered more completely compared with those exposed to 3R4F CS.

Abstract:PurposeRecent findings on the effects of smoking and nicotine on oral microorganisms and on microbial interactions with human cells were reviewed. A PubMed search identified recent publications in English that addressed the effects of smoking and nicotine on oral microorganisms: growth, biofilm formation, plaque composition, enzyme regulation, and cross-species interactions. Interactions of tobacco-exposed microbes with human tissues are also discussed.Recent FindingsSmoking alters the oral microbiome. Nicotine increases the pathogenicity of oral microorganisms, either by increasing expression of virulence factors or by increasing the amount of biofilm formed. Nicotine affects human cells via nicotine acetylcholine receptors (nAChRs), and the prokaryote homolog may explain how nicotine affects microbial cells. Interactions of nicotine exposed microbial and human tissues likely contribute to cardiovascular disease.SummaryThe evidence supports the destructive oral and systemic effects of smoking and nicotine exposure, leading to development or exacerbation of dental caries, periodontitis, stomatitis, and heart diseases.

Abstract:Declines in cigarette smoking prevalence in many countries and the consolidation of the tobacco industry have prompted the introduction of other forms of nicotine delivery. These include electronic nicotine delivery systems (ENDS), heated tobacco products (HTPs) and oral nicotine products (ONPs). Evolving over time, some of these products now deliver nicotine at levels comparable to cigarettes and may serve as effective substitutes for smokers. However, certain products, especially ENDS like JUUL, have also appealed to youth and non-smokers, prompting concerns about expanding nicotine use (and potentially nicotine addiction). The tobacco industry could shift away from primarily promoting cigarettes to promoting ENDS, HTPs and/or ONPs, though at this time it continues to heavily promote cigarettes in low and middle-income countries. Differing regulatory regimes may place upward and downward pressures on both cigarettes and these newer products in terms of population use, and may ultimately drive the extent to which cigarettes are or are not displaced by ENDS, HTPs and/or ONPs in the coming decade.

Prevention of oral cancer: An overview

Abstract:Oral cancer is the sixth most common cancer in the world. It accounts for about 4% of all cancers and 2% of cancer death worldwide. Many different intrinsic and extrinsic risk factors are related to the development of oral cancer, including tobacco (cigarettes, pipes, cigars, and smokeless tobacco) use, alcohol consumption, virus infections, malnutrition, dietary habits, sun exposure to the lips and various local stimuli, etc. Approximately 75% of oral cancers are associated with tobacco use, alcohol use, or use of both substances together. Although great progresses have been achieved in the treatment of oral cancer using surgery, radiotherapy, chemotherapy,biotherapy and/or combined modalities, the 5- year survival rate of oral cancer still remains to be 64% around, and even lower in locally advanced patients. Therefore, to improve the survival rate and quality of life of oral cancer patients, the principle of prevention first must be addressed and sticked to. Early detection and treatment is still the most effective approach to reduce morbidity and mortality from oral cancer. This paper reviews the cutting edge knowledge of oral cancer prevention in many aspects, including tobacco control, decreasing alcohol consumption, healthy eating, screening for oral cancer and potentially malignant lesions, preventing recurrence or further primary cancers and minimizing morbidity.

Abstract:Tobacco use has been recognized to be a significant risk factor for the development and progression of periodontal disease. Its use is associated with increased pocket depths, loss of periodontal attachment, alveolar bone and a higher rate of tooth loss. Nicotine, a major component and most pharmacologically active agent in tobacco is likely to be a significant contributing factor for the exacerbation of periodontal diseases. Available literature suggests that nicotine affects gingival blood flow, cytokine production, neutrophil and other immune cell function; connective tissue turnover, which can be the possible mechanisms responsible for overall effects of tobacco on periodontal tissues. Inclusion of tobacco cessation as a part of periodontal therapy encourages dental professionals to become more active in tobacco cessation counseling. This will have far reaching positive effects on our patients' oral and general health.

Abstract:Tobacco cigarette smoking is a well-established risk factor for oral diseases, especially periodontal diseases. Over the past few years, electronic cigarettes (or "E-cigarettes") have gained popularity. There is some evidence that e-cigarettes may play a role in helping people quit smoking, and they are occasionally seen as a less dangerous alternative to tobacco use. However, there are worries about their effects on health, notably in terms of oral health and the emergence of periodontal disease. Limited information is available regarding the effect on oral disease. E-cigarettes come in a variety of nicotine strengths and flavoring combinations, all of which have been associated with detrimental oral health issues. A growing body of research supports the idea that vaping or using electronic cigarettes may increase the chance of developing oral health problems such as tooth decay, gum disease, periodontal disease, cracked or broken teeth, and pain in the tongue or inside of the cheek. Even if e-cigarettes were less harmful, they still could lead to the development of periodontal diseases through the decreased antioxidant capacity of saliva, the inflammatory effect, cell injury, and impaired reparability. High levels of nicotine content and menthol-favoring additives increase risk of periodontal damage in e-cigarette users. However, conflicting study results are also published for smokers who transitioned to e-cigarettes. Even though benefits or value of e-cigarettes as a substitute for tobacco cigarettes are still unknown, their impact on periodontal and general health status is still prominent.

Oral nicotine marketing claims in direct-mail advertising

Abstract:Background: Little is known regarding how oral nicotine products (eg, nicotine pouches, lozenges) are marketed to consumers, including whether potential implicit reduced harm claims are used. In the current study, we explored the marketing claims present in a sample of direct-mail oral nicotine advertisements sent to US consumers (March 2018-August 2020). Methods: Direct-mail ads (n=50) were acquired from Mintel and dual-coded for the following claims: alternative to other tobacco products, ability to use anywhere, spit-free, smoke-free and product does not contain tobacco leaf. We merged the coded data with Mintel's volume estimate (number of mail pieces sent to consumers) and calculated the proportion of oral nicotine advertisements containing claims by category. Results: Of the 38 million pieces of oral nicotine direct-mail sent to US consumers, most featured claims that the product could be used anywhere (84%, 31.8 million pieces); was an alternative to other tobacco products (69%, 26.1 million pieces); and did not contain tobacco leaf (eg, 'tobacco leaf-free', 'simple' approach of extracting nicotine from tobacco; 55%, 20.7 million pieces). A slightly smaller proportion contained claims that oral nicotine was 'spit-free' (52%, 19.8 million pieces) or 'smoke-free' (31%, 11.7 million pieces). Conclusion: Our results provide an early indication of marketing claims used to promote oral nicotine. The strategies documented, particularly the use of language to highlight oral nicotine is tobacco-free, may covey these products as lower-risk to consumers despite the lack of evidence or proper federal authorisation that oral nicotine products are a modified-risk tobacco product. Future research is needed to examine consumer perceptions of such claims.

Abstract:Nicotine is naturally present in many crops, including but not limited to tobacco, eggplant, and tomatoes. Only in the tobacco plant is nicotine present in high enough quantities (~2% of dry weight) to have pharmacological effects. People have recognized the stimulating effects of the smoke created by burning dried tobacco leaves for thousands of years, and cigarette smoking remains the most common form of nicotine uptake from tobacco. Decades of epidemiologic data show that smoking causes a number of serious diseases (including cardiovascular diseases, lung cancer, and chronic obstructive pulmonary disease [COPD]). Undoubtedly, the best way to avoid the harm from smoking is to never start. For current smokers, quitting smoking altogether is the most effective way to reduce the risk of harm and smoking-related disease. Along with other aspects such as taste and ritual, nicotine is one of the reasons people smoke. Nicotine, while addictive and not risk-free, is not the primary cause of smoking-related diseases. Indeed, experts agree that smoking-related diseases are caused primarily by chronic exposure to the harmful constituents that are produced when tobacco is burning. Nevertheless, many people still mistakenly believe that nicotine is a major cause of tobacco-related diseases. While nicotine-containing products should not be used by certain groups of people - such as minors, people with or at risk of heart disease, diabetes, epilepsy, or seizure, pregnant or breast-feeding women or women who think they may be pregnant - delivery of nicotine by less harmful means can support public health goals by encouraging smokers who would otherwise continue smoking to switch to less harmful products. It is, therefore, pivotal to address the biggest misconceptions about nicotine to empower smokers to make informed decisions. In this article, we discuss basic facts about nicotine, its effects on the human body, as well as the risks related to nicotine consumption.
The effect of nicotine-containing products on peri-implant tissues: A systematic review and network meta-analysis

Abstract:Abstract Introduction Smokers have a higher chance of developing peri-implant diseases and are therefore considered an at-risk population. Our aim was to compare peri-implant characteristics in users of electronic cigarettes (EC), waterpipes (WP), cigarettes (CS), smokeless tobacco (ST), and non-smokers (non-users of any nicotine and tobacco product; NS). Methods A systematic search of four electronic databases (PubMed, EMBASE, Web of Science, CENTRAL) was performed until April 2023, restricted to English language. Thirty-nine observational studies were included in the qualitative synthesis, of which 32 studies were included in a Bayesian network meta-analysis. Using a predesigned form, two researchers independently collected data about marginal bone loss (MBL), probing pocket depth (PPD), plaque index (PI), bleeding on probing (BOP), modified plaque index (mPI), probing pocket depth > 4mm (PPD>4), gingival index (GI), peri-implant sulcular fluid (PISF) volume, and TNF-α and IL-1β levels. QUIPS and CINeMA were used to evaluate the risk of bias and certainty of evidence. Results NS had the smallest MBL. Most nicotine-containing product users had significantly higher MBL (CS, MD:1.34 CrI: 0.85, 1.79; WP, MD:1.58 CrI: 0.84, 2.35; ST, MD:2.53, CrI: 1.20, 3.87) than NS. EC did not show significant difference compared to NS (MD:0.52 CrI: -0.33, 1.36). In secondary outcomes NS were ranked in first place. Subset analysis based on smoking habit, implant duration, and maintenance control revealed no differences in ranking probability. Conclusion Most nicotine-containing product users presented worse peri-implant parameters compared to non-smokers, while EC users did not show significant differences to NS in many outcomes. Implication Alternative nicotine-containing products are gaining popularity and are often considered less harmful by the general public compared to traditional cigarettes. This is the first network meta-analysis comparing users of four nicotine-containing products and non-smokers. This study shows that CS, WP and ST have a detrimental effect on the overall health of peri-implant tissues. EC users also presented inferior parameters compared to NS, however, the difference was not significant in many outcomes. It is essential to educate patients who are using nicotine-containing products, and to provide proper maintenance and appropriate cessation support. Well-designed multi-armed studies are needed for direct comparison of different products, including heated tobacco products. Greater transparency of confounding factors is needed regarding smoking habit and oral hygiene.

Abstract:Snuff dipping and tobacco chewing are rapidly gaining popularity in the United States. They are being heavily promoted through mass media advertising. Advertising spots on radio, television, and in the printed media use well-known sports personalities and entertainers to act as spokesmen for smokeless tobacco. Despite the esthetic drawbacks of these social acceptance. This is being accomplished, at least in part, through a skillful manipulation of the public by the tobacco industry. Use of smokeless tobacco is rapidly increasing, especially among young male athletes and students in high school and college. Advertising implies that smokeless tobacco habits are innocuous and safe. They are not. Smokeless tobacco products have shown that potential for causing cancer of the oral cavity, pharynx, larynx, and eophagus. A review of the medical and dental literature has shown 646 cases of cancer associated directly with smokeless tobacco. Smokeless tobacco can produce significant effects on the soft and hard tissues of the mouth, including bad breath, discolored teeth and restorations, excessive wear (abrasion) of the incisal and occlusal surfaces of the teeth, decreased ability to taste and small, gingival recession, advanced periodontal destruction of the soft and hard tissues, erythema of the soft tissues, leukoplakia, and cancer. Health professionals are urged to advise their patients concerning the hazards of using smokeless tobacco and to take a strong public stance in opposition to the current advertising campaigns aimed at youth. The professions should demand that smokeless tobacco products print health warning labels on the packages and in advertising copy. Finally, patients should be taught how to recognize signs of breakdown of the soft and hard tissues.
