Abstract:The use of machine learning (ML) in health care raises numerous ethical concerns, especially as models can amplify existing health inequities. Here, we outline ethical considerations for equitable ML in the advancement of health care. Specifically, we frame ethics of ML in health care through the lens of social justice. We describe ongoing efforts and outline challenges in a proposed pipeline of ethical ML in health, ranging from problem selection to post-deployment considerations. We close by summarizing recommendations to address these challenges.

Abstract:Machine learning is expected to fuel significant improvements in medical care. To ensure that fundamental principles such as beneficence, respect for human autonomy, prevention of harm, justice, privacy, and transparency are respected, medical machine learning systems must be developed responsibly. Many high-level declarations of ethical principles have been put forth for this purpose, but there is a severe lack of technical guidelines explicating the practical consequences for medical machine learning. Similarly, there is currently considerable uncertainty regarding the exact regulatory requirements placed upon medical machine learning systems. This survey provides an overview of the technical and procedural challenges involved in creating medical machine learning systems responsibly and in conformity with existing regulations, as well as possible solutions to address these challenges. First, a brief review of existing regulations affecting medical machine learning is provided, showing that properties such as safety, robustness, reliability, privacy, security, transparency, explainability, and nondiscrimination are all demanded already by existing law and regulations - albeit, in many cases, to an uncertain degree. Next, the key technical obstacles to achieving these desirable properties are discussed, as well as important techniques to overcome these obstacles in the medical context. We notice that distribution shift, spurious correlations, model underspecification, uncertainty quantification, and data scarcity represent severe challenges in the medical context. Promising solution approaches include the use of large and representative datasets and federated learning as a means to that end, the careful exploitation of domain knowledge, the use of inherently transparent models, comprehensive out-of-distribution model testing and verification, as well as algorithmic impact assessments.

Abstract:Machine learning (ML), artificial intelligence (AI), and other modern statistical methods are providing new opportunities to operationalise previously untapped and rapidly growing sources of data for patient benefit. Abstract:A promise of machine learning in health care is the avoidance of biases in diagnosis and treatment; a computer algorithm could objectively synthesize and interpret the data in the medical record. Integration of machine learning with clinical decision support tools, such as computerized alerts or diagnostic support, may offer physicians and others who provide health care targeted and timely information that can improve clinical decisions. Machine learning algorithms, however, may also be subject to biases. The biases include those related to missing data and patients not identified by algorithms, sample size and underestimation, and misclassification and measurement error. There is concern that biases and deficiencies in the data used by machine learning algorithms may contribute to socioeconomic disparities in health care. This Special Communication outlines the potential biases that may be introduced into machine learning–based clinical decision support tools that use electronic health record data and proposes potential solutions to the problems of overreliance on automation, algorithms based on biased data, and algorithms that do not provide information that is clinically meaningful. Existing health care disparities should not be amplified by thoughtless or excessive reliance on machines.

Abstract:Machine learning can have major societal impact in computational biology applications. In particular, it plays a central role in the development of precision medicine, whereby treatment is tailored to the clinical or genetic features of the patient. However, these advances require collecting and sharing among researchers large amounts of genomic data, which generates much concern about privacy. Researchers, study participants and governing bodies should be aware of the ways in which the privacy of participants might be compromised, as well as of the large body of research on technical solutions to these issues. We review how breaches in patient privacy can occur, present recent developments in computational data protection, and discuss how they can be combined with legal and ethical perspectives to provide secure frameworks for genomic data sharing.

Machine learning (ML) is the most relevant subset of AI in medicine, which will soon become an integral part of our everyday practice. Abstract:The integration of artificial intelligence (AI) and its autonomous learning processes (or machine learning) in medicine has revolutionized the global health landscape, providing faster and more accurate diagnoses, personalization of medical treatment, and efficient management of clinical information. However, this transformation is not without ethical challenges, which require a comprehensive and responsible approach. There are many fields where AI and medicine intersect, such as health education, patient-doctor interface, data management, diagnosis, intervention, and decision-making processes. For some of these fields, there are some guidelines to regulate them. AI has numerous applications in medicine, including medical imaging analysis, diagnosis, predictive analytics for patient outcomes, drug discovery and development, virtual health assistants, and remote patient monitoring. It is also used in robotic surgery, clinical decision support systems, AI-powered chatbots for triage, administrative workflow automation, and treatment recommendations. Despite numerous applications, there are several problems related to the use of AI identified in the literature in general and in medicine in particular. These problems are data privacy and security, bias and discrimination, lack of transparency (Black Box Problem), integration with existing systems, cost and accessibility disparities, risk of overconfidence in AI, technical limitations, accountability for AI errors, algorithmic interpretability, data standardization issues, unemployment, and challenges in clinical validation. Of the various problems already identified, the most worrying are data bias, the black box phenomenon, questions about data privacy, responsibility for decision-making, security issues for the human species, and technological unemployment. There are still several ethical problems associated with the use of AI autonomous learning algorithms, namely epistemic, normative, and comprehensive ethical problems (overarching). Addressing all these issues is crucial to ensure that the use of AI in healthcare is implemented ethically and responsibly, providing benefits to populations without compromising fundamental values. Ongoing dialogue between healthcare providers and the industry, the establishment of ethical guidelines and regulations, and considering not only current ethical dilemmas but also future perspectives are fundamental points for the application of AI to medical practice. The purpose of this review is to discuss the ethical issues of AI algorithms used mainly in data management, diagnosis, intervention, and decision-making processes.

Abstract:With the growing utilization of machine learning in healthcare, there is increasing potential to enhance healthcare outcomes. However, this also brings the risk of perpetuating biases in data and model design that can harm certain demographic groups based on factors such as age, gender, and race. This study proposes an artificial intelligence framework, grounded in software engineering principles, for identifying and mitigating biases in data and models while ensuring fairness in healthcare settings. A case study is presented to demonstrate how systematic biases in data can lead to amplified biases in model predictions, and machine learning methods are suggested to pre-vent such biases. Future research aims to test and validate the proposed ML framework in real-world clinical settings to evaluate its impact on promoting health equity.

Abstract:<p>Imagine the enormous amounts of data that can be generated in the medical field. Each patient has his own medical record which contains valuable information like patient allergy, chronic diseases and vaccinations. Healthcare can profit from this data when it is properly analyzed. The more data gathered, the more complicated data analytics become, therefore, machine learning can be a very useful solution not only to facilitate analysis but also to save time.</p><p>In this paper basic concepts of the medical field and machine learning will be described. We will show how data analytic can help in the healthcare process. Finally, we will present some challenges that must be carefully studied in order to obtain effective solutions in medical diagnosis.</p>

Risks and benefits of dermatological machine learning health care applications-an overview and ethical analysis

Abstract:Background: Visual data are particularly amenable for machine learning techniques. With clinical photography established for skin surveillance and documentation purposes as well as progress checks, dermatology is an ideal field for the development and application of emerging machine learning health care applications (ML-HCAs). To date, several ML-HCAs have detected malignant skin lesions on par with experts or found overlooked visual patterns that correlate with certain dermatological diseases. However, it is well established that ML-HCAs come with ethical and social implications. Objectives: Currently, there is a lack of research that establishes model design, training, usage and regulation of such technologies sufficient to ensure ethically and socially responsible development and clinical translation, specifically within the field of dermatology. With this paper, we aim to give an overview of currently discussed ethical issues relating to dermatological ML-HCAs. Methods: On the basis of a thematic, keyword-based literature search, we performed an ethical analysis against established frameworks of biomedical ethics. We combined our results with current, relevant normative machine learning ethics literature to identify the status quo of the ethics of ML-HCAs in dermatology. We describe the benefits and risks of dermatological ML-HCAs that are currently being developed for clinical purposes. Results: The potential benefits range from better patient outcomes to better knowledge accessibility to decreasing health care disparities, that is, standards of care between different population groups. The risks associated with ML-HCAs range from confidentiality issues to individual patient outcomes as well as the exacerbation of prevalent health care disparities. We discuss the practical implications for all stages of dermatological ML-HCA development. Conclusion: We found that ML-HCAs present stakeholder-specific risks for patients, health care professionals and society, which need to be considered separately. The discipline lacks sufficient biomedical ethics research that could standardize the approach to ML-HCA model design, training, use and regulation of such technologies.

Abstract:Modern electronic health records (EHRs) provide data to answer clinically meaningful questions. The growing data in EHRs makes healthcare ripe for the use of machine learning. However, learning in a clinical setting presents unique challenges that complicate the use of common machine learning methodologies. For example, diseases in EHRs are poorly labeled, conditions can encompass multiple underlying endotypes, and healthy individuals are underrepresented. This article serves as a primer to illuminate these challenges and highlights opportunities for members of the machine learning community to contribute to healthcare.

Abstract:A consequence of the fragmented and siloed healthcare landscape is that patient care (and data) is split along multitude of different facilities and computer systems and enabling interoperability between these systems is hard. The lack interoperability not only hinders continuity of care and burdens providers, but also hinders effective application of Machine Learning (ML) algorithms. Thus, most current ML algorithms, designed to understand patient care and facilitate clinical decision-support, are trained on limited datasets. This approach is analogous to the Newtonian paradigm of Reductionism in which a system is broken down into elementary components and a description of the whole is formed by understanding those components individually. A key limitation of the reductionist approach is that it ignores the component-component interactions and dynamics within the system which are often of prime significance in understanding the overall behaviour of complex adaptive systems (CAS). Healthcare is a CAS. Though the application of ML on health data have shown incremental improvements for clinical decision support, ML has a much a broader potential to restructure care delivery as a whole and maximize care value. However, this ML potential remains largely untapped: primarily due to functional limitations of Electronic Health Records (EHR) and the inability to see the healthcare system as a whole. This viewpoint (i) articulates the healthcare as a complex system which has a biological and an organizational perspective, (ii) motivates with examples, the need of a system's approach when addressing healthcare challenges via ML and, (iii) emphasizes to unleash EHR functionality - while duly respecting all ethical and legal concerns - to reap full benefits of ML.

Abstract:The use of machine learning harbours the promise of more accurate, unbiased future predictions than human beings on their own can ever be capable of. However, because existing data sets are always utilized, these calculations are extrapolations of the past and serve to reproduce prejudices embedded in the data. In turn, machine learning prediction result raises ethical and moral dilemmas. As mirrors of society, algorithms show the status quo, reinforce errors, and are subject to targeted influences – for good and the bad. This phenomenon makes machine learning viewed as pseudoscience. Besides the limitations, injustices, and oracle-like nature of these technologies, there are also questions about the nature of the opportunities and possibilities they offer. This article aims to discuss whether machine learning in biomedical research falls into pseudoscience based on Popper and Kuhn's perspective and four theories of truth using three study cases.
