Abstract:A 65-year-old man presented with recurrent abdominal pain and vomiting for 10 years. He had undergone inguinal hernia repair 20 years before. A mass was palpated in the right lower abdomen. Laboratory values were notable for C-reactive protein of 21.8 mg/L (<10 mg/L), albumin of 32.6 g/L (35∼52 g/L), and fecal calprotectin of 1070.84 μg/g (<200 μg/g). Computed tomography showed a 3.7 × 3.4 cm liquid-gas entity adhering to the distal ileum (Figure A, arrows). Colonoscopy showed a large ulcer in the terminal ileum (Figure B). Crohn’s disease was suspected, based on the clinical and endoscopic findings. After discussing the pros (a confirmed diagnosis and relief of symptoms) and cons (potential surgical complications) of surgery, the patient, who had been experiencing poor quality of life for years, chose exploratory surgery. During laparoscopic exploration, the distal ileum was found adhering to a mesh and was removed with complete mesh excision (Figure C). The pathology revealed transmural fibrinoid necrosis and inflammatory exudation in the subserosa and serosa (Figure D). The patient was eventually diagnosed with a mesh-related intestinal fistula and had an uneventful postoperative course. Mesh-related visceral complications are rare. The sigmoid, bladder, and small bowel are the 3 most frequently involved organs. Foreign body reaction induced by mesh can result in erosion into adjacent tissue and fistula formation. The chronic process can mimic Crohn's disease, and the invisibility of the mesh material on computed tomography imaging can make it difficult to establish a causal relationship with the previous surgery. Attention to previous surgical history should be part of the work-up for suspected Crohn’s disease.
Beyond the abdominal and pelvic cavity: abdominal wall and spinal "Aunt Minnies"

Abstract:Abdominal wall and spinal soft tissue findings are frequently encountered on CT or MR imaging of the abdomen and pelvis. Many of these entities have specific imaging findings, for which a definitive diagnosis can be made without the need for further work up. These abdominal wall and spinal findings may be diagnostically challenging for sub-specialized abdominal radiologists who are unfamiliar with their appearance and appropriate management. This review article describes and illustrates pathognomonic or characteristic abdominal wall and spinal pathologies, which reside outside the abdominopelvic cavity. The cases selected all have findings that allow a confident diagnosis without further imaging or intervention. The cases presented include myonecrosis, intramuscular abscess, myositis, iliopsoas bursitis, Morel-Lavallée lesion, hydrocele of canal of Nuck, Klippel Trenaunay Weber syndrome, neurofibroma with target sign, perineural cysts, filum terminale lipoma, calvarial bone flap, transverse rectus abdominis muscle (TRAM) flap, liposuction, and hidradenitis suppurativa, among others. Although not all-encompassing, this paper will help abdominal radiologists to accurately diagnose a variety of abdominal and pelvic extra-cavitary soft tissue pathologies by identifying key radiologic findings.
Body composition analysis using abdominal scans from routine clinical care in patients with Crohn’s Disease

Abstract:OBJECTIVE: Crohn's Disease is associated with body composition changes, which have important treatment and prognostic implications. Measurement of body composition usually requires dedicated scanning or measurement, with retrospective analysis of existing datasets impossible. We sought to determine whether single slice analysis of abdominal scans, obtained during routine clinical care, in patients with Crohn's Disease accurately predicts body composition compartments.MATERIALS AND METHODS: Abdominal CT images of patients with Crohn's disease were analyzed and comparison was made with total body fat-free mass, total body fat mass, femoral neck t-score, and other parameters reported from DXA, the reference method.RESULTS: Thirty-seven subjects were identified, 15 male and 22 female, with a mean age of 43.8 years. There was significant correlation (Pearson r = 0.923, p < 0.001) between skeletal muscle area from CT and total fat-free mass measured by DXA. Similarly, total body fat mass correlated strongly (r = 0.928, p < 0.0001) with subcutaneous fat area. In this cohort of ambulatory Crohn's Disease patients, low muscle mass/sarcopenia was prevalent and predictive of lower bone mineral density.CONCLUSIONS: Fat mass, fat-free mass, and appendicular skeletal muscle index can be predicted by analysis of a single CT slice in patients with Crohn's Disease. Similar to published data from healthy subjects, the L3 vertebral body level provided the most robust correlation with most parameters. This study represents the first published use of routinely obtained abdominal imaging to demonstrate this relationship - and to predict body composition components - in patients with inflammatory bowel disease.

Abstract:History A 55-year-old woman without systemic underlying disease, such as diabetes mellitus, inflammatory bowel disease, autoimmune disease, or chronic kidney disease, presented with generalized dull abdominal pain of 1-week duration. She had ingested herbal medicine for physical conditioning for several years. Laboratory findings, including biochemistry, electrolyte levels, and complete blood count, were all within normal limits, except for elevated serum C-reactive protein level (7.719 mg/dL; normal range, <1 mg/dL). The patient underwent initial evaluation with conventional abdominal radiography. She underwent subsequent evaluation with noncontrast CT of the abdomen and colonoscopy.

Abstract:BackgroundAxial spondyloarthritis (axSpA) is a group of inflammatory diseases that may lead to ankylosis of the sacroiliac joint and spine. Fat lesion in the sacroiliac joint is an important feature in diagnosis and disease progression of axSpA. However, whether there is alteration of fatty acids (FAs) composition has not been investigated using MRI.PurposeTo investigate bone marrow FA composition of the sacroiliac joint in patients with axSpA compared to controls.Study TypeProspective.SubjectsEighty five participants (mean age, 32.3 +/- 6.1 years): 48 axSpA (25 male, 23 female) and 37 non-SpA controls (18 male, 19 female).Field Strength/Sequence3.0 T/Two multiple gradient-echo chemical shift-encoded (CSE) MRI which differed only in echo times (TEs) were scanned consecutively.AssessmentAxial multi-echo CSE MRI was performed in the sacroiliac joints in vivo. Regions of interest (ROIs) were manually placed on subchondral bone with and without fat lesion in axSpA patients, and on subchondral bone without fat lesion in controls. FA composition was computed within the ROIs using a nonlinear least square method from literature.Statistical TestsIntergroup comparisons were performed using t tests.ResultsIn axSpA, male patients had significantly higher monounsaturated FA compared to controls in areas with fat lesion in the sacrum (+12%) and in the ilium (+9%), and in areas without fat lesion in the sacrum (+10%). Significantly lower polyunsaturated FAs were found in areas with fat lesion in the sacrum (-10%) and ilium (-11%), and lower saturated FAs were found in areas without fat lesion in the sacrum (-6%). In female, patients with axSpA had significantly higher saturated FAs in areas with fat lesion in the ilium (+7%) in comparison to controls.Data ConclusionFA composition of the sacroiliac joint alters in patients with axSpA, and it can be detected using CSE MRI based analysis.

Abstract:Objective To investigate the CT and MRI manifestations of intra-abdominal aggressive fibromatosis and correlation with pathology.Methods The CT and MRI manifestations of 26 cases with intra-abdominal aggressive fibromatosis confirmed by pathological examination were analyzed retrospectively.Results 26 cases showed single solid mass,13 cases showed well-circumscribed and round-like,9 cases wrapped around the common bile duct,intestine or ureter,4 cases were lobulated which had unclear margin with surrounding tissues in pelvic.All the lesions displayed isodensity or slightly low density non-enhanced CT appearance,heterogenous high intensity FS T2WI,some larger tumors showed mixed signal.CT/MRI enhanced scan showed gradual enhancement in 26 cases.Conclusion Intra-abdominal aggressive fibromatosis have some certain imaging features,MRI can offer the histo logical features of tumors and have some correlation with pathology.

Abstract:PURPOSE: Acute epiploic appendagitis (EA) is a relatively rare, benign and local inflammatory disease involving the epiploic appendices. Unlike its mimics, EA is generally a self-limiting inflammatory disease and can be treated conservatively.CASE PRESENTATION: A 33-year-old Caucasian man presented to our emergency department with a sever and sharp left iliac fossa pain. He underwent abdominal X-ray, ultrasound (US) and computed tomography (CT) evaluations.CONCLUSION: We illustrate US and CT findings to increase the radiologists' awareness of this condition and to avoid diagnostic delay and unnecessary use of antibiotics, hospitalization and surgery.

Abstract:Median arcuate ligament syndrome (MALS), also known as celiac artery compression syndrome, is a rare gastrointestinal condition with an estimated incidence of 2 per 100 000 population. Predominantly in female patients, this syndrome is characterized by the compression of the celiac artery at its origin from the aorta by the median arcuate ligament, which at the same time is entrapping the celiac plexus, causing upper abdominal pain, notably postprandial pain, as well as nausea, vomiting, food aversion, and weight loss. We present a case of abdominal pain secondary to MALS that was appropriately diagnosed after requiring narcotic medication, which responded to surgical therapy.

Abstract:The celiac axis is commonly involved in conditions that cause the narrowing or blockage of a celiac artery atherosclerosis and other vascular disease. Celiac artery compression syndrome is defined as chronic abdominal pain occurring because of compression of the celiac artery commonly in middle-aged (40 to 60 years) females. Various etiologies include atherosclerosis of mesenteries vessels, pancreatitis, median arcuate ligament syndrome, and tumor invasion. It is an uncommon condition, and symptoms include post-prandial abdominal pain mostly in the epigastrium, weight loss, nausea, diarrhea, anorexia, and bloating. Patients are asymptomatic for a prolonged duration due to collateral blood supply to the bowel from the patent superior mesenteric and inferior mesenteric arteries. We present a case of a 67-year-old female who initially presented with signs and symptoms suggestive of small bowel obstruction, however, due to persistent abdominal pain, she underwent a CT scan suggestive of severe celiac trunk stenosis causing abdominal angina. The patient was managed conservatively and responded well with close follow-up.

Abstract:The differential diagnosis of extrahepatic abdominopelvic masses is wide. Demonstration of fat within a lesion at noninvasive imaging is an important clue for narrowing the differential diagnosis. Macroscopic fat is readily identified with both computed tomography (CT) and magnetic resonance (MR) imaging. Demonstration of microscopic fat is more difficult and may require special techniques. Identification of fat with CT is based on x-ray resorption and therefore on the attenuation (typically less than -20 HU). Several MR imaging techniques have been developed for fat suppression. Two of the most widely available are spectroscopic fat saturation and chemical shift (in-phase/opposed-phase) imaging. Entities with predominantly macroscopic fat include myelolipoma, angiomyolipoma, teratoma, liposarcoma, lipoma, epiploic appendagitis, omental infarction, and mesenteric panniculitis. Lesions with predominantly microscopic fat include adrenal adenoma and some teratomas. Other fat-containing entities involve the mesentery and bowel wall; these include fibrofatty mesenteric proliferation and submucosal fat deposition.
Ultrasound and Radiological Features of Abdominal Unicentric Castleman's Disease

Abstract:This study aimed to improve the diagnostic accuracy of abdominal unicentric Castleman's disease (UCD) by retrospectively summarizes the relatively specific imaging features of UCD. This study retrospectively collected fifteen patients with abdominal UCD confirmed by pathology. All patients were underwent ultrasound (US), computed tomography (CT), or magnetic resonance imaging (MRI) examination. The imaging findings of UCDs were analyzed by senior radiologists. Fifteen patients included 7 males and 8 females, aged 30 to 68 years old, with an average age of 51.73 +/- 13.57. In the 15 cases, 7 were located around the mesentery, 4 were located in the retroperitoneal space, and 4 in the liver. Fifteen cases contained solid masses, of which 13 had clear margins and 2 had blurred margins. The size of the mass ranged from 1.5 to 14.2 cm, with an average of 6.49 +/- 4.16 cm. US showed that 9 lesions were presented with hypo-echogenicity while 5 lesions presented with hyper-echogenicity spots. Unenhanced CT showed that the lesions were comprised of soft tissue while calcified lesions were found in 10 of the cases (66.67%, 10/15). T1-weighted imaging (T1WI) suggested the lesions as iso/hypo-signal, and mildly hyper-signal on T2-weighted imaging (T2WI). Diffusion-weighted imaging (DWI) showed different degrees of hyper-signal. Contrast-enhanced US and CT/MRI showed obvious enhancement at the arterial phase in 12 cases (85.71%, 12/14), most of which (50%, 7/14) showed continuous enhancement at the delayed phase. Feeding vessel could be seen within, or around the lesion in 5 cases (35.71%, 5/14). The study suggests that abdominal UCD commonly manifests as well-defined, homogeneous, solid, and hypervascular masses. Calcification and the presence of feeding vessel in the tumors are relatively specific features of abdominal UCD.

Abstract:Extraosseous Ewing sarcoma (EES) is a virulent tumor, notorious for its high rates of local recurrence and distant metastasis. To our knowledge, only a limited number of intra-abdominal Ewing sarcomas have been documented, manifesting in diverse locations including the gastrointestinal tract, kidneys, adrenal glands, pancreas, supraperitoneum, and retroperitoneum. Up to date only 8 cases of extraosseus Ewing sarcoma arising in jejunum are reported in the literature (Table 1). In this case, abdominal distension was the first symptom, but there was no definitive evidence to support the diagnosis of Ewing sarcoma. An abdominal CT scan initially suggested an intestinal mesenchymal tumor combined with the patient's gastrointestinal symptoms, surgical resection was performed. Interestingly, postoperative specimens enabled the identification of Ewing sarcoma through light microscopy, immunohistochemistry, and molecular genetic evaluations.

Abstract:A 77-year-old man with back pain and abdominal distention underwent F-NaF PET/CT examination to evaluate potential bone lesions. Multiple foci of increased activity were noted in the chest, which corresponded to bilateral ribs and suggested possible occult fractures. Surprisingly, increased F-NaF uptake was observed in the region of pelvis, which corresponded to calcification along the colon wall, a sequela of intestinal Japanese schistosomiasis that the patient experienced 30 years ago.

Abstract:A man in his 30s presented with a 9 month history of epigastric discomfort that occasionally radiated laterally to the upper quadrants (fig 1). Contrast enhanced computed tomography showed multiple peritoneal nodules throughout the abdomen and pelvis, and the patient was referred to the oncology clinic for biopsy, further staging, and treatment. No other signs or symptoms were present but the patient did report a fall as a teenager resulting in splenic rupture and intra-abdominal haemorrhage and a splenectomy. Abstract:A 45-year-old female patient with diffuse osteoarticular pain, particularly low back pain, was referred by a rheumatologist for an updated radiologic evaluation. The patient had experienced these symptoms for many years and was diagnosed with human leukocyte antigen B27-negative spondyloarthritis approximately 11 years prior, based on findings of bilateral erosive sacroiliitis at pelvic radiography (Fig 1A) and bone scintigraphy with technetium 99m methylene diphosphonate (Fig 1B). After 3 years of treatment with a tumor necrosis factor-α inhibitor (adalimumab), which was effective for pain, the patient was lost to follow-up. At the current presentation, approximately 8 years after being lost to follow-up, the patient presented with worsening low back pain. The presence of nonobstructing kidney stones on US images confounded the underlying cause of worsening pain. The patient also experienced fatigue and depressed mood. Routine blood tests revealed a normal blood cell count, creatinine level of 0.64 mg/dL (56.58 μmol/L) (normal range, 0.30-1.1 mg/dL [26.52-97.24 mmol/L]), C-reactive protein level of 1.1 mg/dL (normal, <1 mg/dL), and vitamin D level of 21 ng/mL (52.42 nmol/L) (normal range, 30-100 ng/mL [74.88-249.60 nmol/L]). Noncontrast MRI of the thoracic and lumbar spine (Fig 2), MRI of the sacroiliac joints (Fig 3), and CT of the abdomen and pelvis (Fig 4) were performed.

Psoas muscle abscess simulating acute appendicits: A case report

Abstract:Introduction: Psoas abscess is a rare clinical entity with vague symptomatology. We report a psoas abscess that simulated symptoms of acute appendicitis. Presentation of case: A twenty-five year old male presented pain irradiating to the right iliac fossa and lumbar region associated with thigh flexion. Laboratorial exams revealed leukocytosis with a neutrophil shift to the left. Abdominal Ultrasound showed significant intestinal distension and a small quantity of free fluid at the right iliac fossa. Laparotomy and an appendectomy were performed. During immediate post-operative, the patient evolved with worsening of the pain and the leukocytosis, therefore, we chose to maintain his antalgic posture. An abdominal Computerized Tomography scan with contrast was solicited, revealing an increase of the iliac and psoas muscles of the right side, and multiple bacterial focuses. A retroperitoneal access was performed and 300ml of purulent secretion was drained. Afterwards, we implanted a Penrose Drain. The patient had a good post-op evolution, being discharged 7days after the drainage. Discussion: The psoas muscle is a flexor of the thigh. Psoas abscess is an underdiagnosed condition, its main treatment is surgery associated with antibiotic therapy. CT scan seems to be the best choice of diagnostic image exam, although some authors prefer the nuclear magnetic resonance. Conclusion: The psoas muscle abscess is uncommon and poorly characterized in its etiology, clinical associations, and its therapeutic approach. On the other hand, acute appendicitis is the most common abdominal emergency, with a 7% death rate, and surgery is its main treatment.

Abstract:As the use of cross-sectional abdominal and pelvic imaging has increased exponentially in the past several decades, incidental musculoskeletal findings have become commonplace. These are often unrelated to the indication for the examination and are frequently referred to as the "radiologist's blind spot" on these studies. The differential diagnosis for abnormalities of the paraspinal and pelvic musculature is, in many cases, quite different from the anterior abdominal wall muscles. Furthermore, due to their relatively deep location, pathology involving the former muscle groups is more likely to be clinically occult, often presenting only incidentally when the patient undergoes cross-sectional imaging. Effective treatment of diseases of these muscles is dependent on adherence to a diverse set of diagnostic and treatment algorithms. The purpose of this review article is to familiarize the radiologist with the unique pathology of these often-overlooked muscles of the abdomen and pelvis.

Abstract:Background: There are a lot of different causes of abdominal pain; in this case, a young woman suffers from three diseases with similar symptoms. Adult intestinal mal-rotation is a rare condition of deviation from the normal 270° counter clockwise rotation of the midgut resulting in, not only mal-position of the small intestine, but also mal-fixation of the mesentery. Fitz-Hugh-Curtis syndrome is a rare complication of pelvic inflammatory disease; it involves liver capsule inflammation associated with genital tract infection, which is usually caused by Neisseria gonorrhoea and Chlamydia trachomatis. Neuroendocrine tumors are enterochromaffin cell neoplasms that arise from cells of the endocrine (hormonal) and nervous systems; the appendicular one is the most common primary malignant lesion of these tumors, it's incidence is about 0.3 - 0.9% of appendectomies done. Just for knowledge, this is the first described case of concomitant presence of all these diseases with clinical symptoms attributable to each one. Case presentation: 40-years-old woman suffers from acute abdominal pain, predominantly on the right quadrants, without abdominal distension, no guarding nor rigidity and normal intestinal peristalsis. She has a long history of abdominal intermittent pain, with cramps every 30-40 min, resolving spontaneously. She was diagnosed as intestinal mal-rotation through computed tomography scan which has evidenced a mobilized intra--peritoneal duodenum with cecum/ascending colon predominately lying on the left side and the small intestine almost entirely lying on the right side of abdomen, without evidence of effusion, edema or signs of intestinal ischemia or infarction. Exploratory laparoscopy demonstrated an inflammatory process in the hepatic-renal space, with bloody adhesions above the liver capsule; this is additional to the typical pelvic inflammatory disease signs (Fitz-Hugh-Curtis syndrome). Appendectomy was performed with histological analysis resulting in appendicular neuroendocrine tumor. Conclusions: Although the patient has an intestinal mal-rotation which could explain the abdominal painful symptoms, it is not possible to exclude other concomitant causes, such as perihepatitis on pelvic inflammatory disease or neuroendocrine tumors. Even if all these diseases are rarely seen in daily clinical practice, they should be considered in the differential diagnosis of chronic intermittent abdominal pain in a young woman.
Case Series of Abdominal Actinomycosis: An Old Diagnostic Conundrum

Abstract:Abdominal actinomycosis is a rare disease caused by the Actinomyces bacteria. We report a case series of two similar cases with a mismatch between the initial differential diagnosis, the radiological findings, the surgical findings, and the actual histological result. The first case is a 25-year-old woman with a month's history of right-sided abdominal pain. The diagnostic workup including computed tomography (CT) showed possible acute appendicitis, in addition to ascending colon looking abnormal with inflammatory changes and adjacent peritoneal nodules. A diagnostic laparoscopy revealed a normal-looking appendix and a hard mass involving the ascending colon potentially of inflammatory or malignant origin. A laparoscopic right hemicolectomy was performed. Surprisingly, the histology concluded actinomycosis. The second case is a 38-year-old woman with a six-week history of lower abdominal pain and right iliac fossa (RIF) swelling. The diagnostic workup including a CT scan showed a complex mass suggesting complicated appendicitis, pelvic inflammatory disease, or neoplastic process. Furthermore, magnetic resonance imaging (MRI) of the pelvis showed complex bilateral cystic lesions in both iliac fossas with an abscess formation within the subcutaneous fat overlying the lower pelvis. The patient underwent an ultrasound (US)-guided drainage of the subcutaneous pelvic collection and a diagnostic laparoscopy that showed an inflamed mass with dense adhesions to the anterior abdominal wall. Intraoperative biopsies were taken to confirm the diagnosis, which ultimately confirmed severe pelvic actinomycosis. Both patients were discharged home and received extended antibiotic treatment according to microbiology guidelines with good outcomes. Both cases are interesting because there was a mismatch between the initial differential diagnosis, the radiological findings, the intraoperative findings, and the final histology. Reported cases in the literature highlight the diagnostic challenges of abdominal actinomycosis, as it often presents with atypical symptoms and imaging findings sometimes mimic malignancy leading to surgical intervention.
