Abstract:Self-powered implantable medical electronic devices that harvest biomechanical energy from cardiac motion, respiratory movement and blood flow are part of a paradigm shift that is on the horizon. Here, we demonstrate a fully implanted symbiotic pacemaker based on an implantable triboelectric nanogenerator, which achieves energy harvesting and storage as well as cardiac pacing on a large-animal scale. The symbiotic pacemaker successfully corrects sinus arrhythmia and prevents deterioration. The open circuit voltage of an implantable triboelectric nanogenerator reaches up to 65.2 V. The energy harvested from each cardiac motion cycle is 0.495 μJ, which is higher than the required endocardial pacing threshold energy (0.377 μJ). Implantable triboelectric nanogenerators for implantable medical devices offer advantages of excellent output performance, high power density, and good durability, and are expected to find application in fields of treatment and diagnosis as in vivo symbiotic bioelectronics.

Abstract:The first application of an implanted triboelectric nanogenerator (iTENG) that enables harvesting energy from in vivo mechanical movement in breathing to directly drive a pacemaker is reported. The energy harvested by iTENG from animal breathing is stored in a capacitor and successfully drives a pacemaker prototype to regulate the heart rate of a rat. This research shows a feasible approach to scavenge biomechanical energy, and presents a crucial step forward for lifetime-implantable self-powered medical devices.

Abstract:Abstract Self-powered implantable devices have the potential to extend device operation time inside the body and reduce the necessity for high-risk repeated surgery. Without the technological innovation of in vivo energy harvesters driven by biomechanical energy, energy harvesters are insufficient and inconvenient to power titanium-packaged implantable medical devices. Here, we report on a commercial coin battery-sized high-performance inertia-driven triboelectric nanogenerator (I-TENG) based on body motion and gravity. We demonstrate that the enclosed five-stacked I-TENG converts mechanical energy into electricity at 4.9 μW/cm 3 (root-mean-square output). In a preclinical test, we show that the device successfully harvests energy using real-time output voltage data monitored via Bluetooth and demonstrate the ability to charge a lithium-ion battery. Furthermore, we successfully integrate a cardiac pacemaker with the I-TENG, and confirm the ventricle pacing and sensing operation mode of the self-rechargeable cardiac pacemaker system. This proof-of-concept device may lead to the development of new self-rechargeable implantable medical devices.

Abstract:Implantable medical devices are widely used for monitoring and treatment of severe diseases. In particular, an implantable cardiac pacemaker is the most effective therapeutic device for treating bradyrhythmia, however its surgical replacement is inevitable every 5-12 years due to the limited life of the built-in battery. Although several approaches of energy harvesting have been explored in this decade for powering cardiac pacemakers, the modern, commercial, and full-function pacemaker has never been powered effectively yet. Here, we report an integrated strategy for directly powering a modern and full-function cardiac pacemaker, which can pace the porcine heart in vivo by harvesting the natural energy of a heartbeat, without using any external energy storage element. The generator includes an elastic skeleton and two piezoelectric composites, which could generate a high-output current of 15 μA in vivo over state-of-the-art performance. This study makes an impressive step toward fabricating a self-powered cardiac pacemaker and resolving the power issue of implantable medical devices by piezoelectric harvesting technology.

Implanted Carbon Nanotubes Harvest Electrical Energy from Heartbeat for Medical Implants

Abstract:Reliability of power supply for current implantable electronic devices is a critical issue for longevity and for reducing the risk of device failure. Energy harvesting is an emerging technology, representing a strategy for establishing autonomous power supply by utilizing biomechanical movements in human body. Here, a novel "Twistron energy cell harvester" (TECH), consisting of coiled carbon nanotube yarn that converts mechanical energy of the beating heart into electrical energy, is presented. The performance of TECH is evaluated in an in vitro artificial heartbeat system which simulates the deformation pattern of the cardiac surface, reaching a maximum peak power of 1.42 W kg-1 and average power of 0.39 W kg-1 at 60 beats per minute. In vivo implantation of TECH onto the left ventricular surface in a porcine model continuously generates electrical energy from cardiac contraction. The generated electrical energy is used for direct pacing of the heart as documented by extensive electrophysiology mapping. Implanted modified carbon nanotubes are applicable as a source for harvesting biomechanical energy from cardiac motion for power supply or cardiac pacing.

Abstract:Objective: cardiac pacemakers require regular medical follow-ups to ensure proper functioning. However, device replacements due to battery depletion are common and account for ∼25% of all implantation procedures. Furthermore, conventional pacemakers require pacemaker leads which are prone to fractures, dislocations or isolation defects. The ensuing surgical interventions increase risks for the patients and costs that need to be avoided. Methods: in this study, we present a method to harvest energy from endocardial heart motions. We developed a novel generator, which converts the heart's mechanical into electrical energy by electromagnetic induction. A mathematical model has been introduced to identify design parameters strongly related to the energy conversion efficiency of heart motions and fit the geometrical constraints for a miniaturized transcatheter deployable device. The implemented final design was tested on the bench and in vivo. Results: the mathematical model proved an accurate method to estimate the harvested energy. For three previously recorded heart motions, the model predicted a mean output power of 14.5, 41.9, and 16.9 μW. During an animal experiment, the implanted device harvested a mean output power of 0.78 and 1.7 μW at a heart rate of 84 and 160 bpm, respectively. Conclusion: harvesting kinetic energy from endocardial motions seems feasible. Implanted at an energetically favorable location, such systems might become a welcome alternative to extend the lifetime of cardiac implantable electronic device. Significance: the presented endocardial energy harvesting concept has the potential to turn pacemakers into battery- and leadless systems and thereby eliminate two major drawbacks of contemporary systems.

Abstract:Harvesting biomechanical energy to power wearable electronic devices shows great potential in the field of the Internet of Things. An insole hybrid nanogenerator (IHN) has been designed based on a combination of multilayered triboelectric nanogenerator and arched piezoelectric nanogenerator. The IHN not only can convert the mechanical energy of the footsteps into electricity, but also distinguishes three kinds of motion states: walking, stepping, and jumping. A maximum open‐circuit voltage of 150 V and a short‐circuit current of 4.5 µA are achieved from human body motions. After walking for 8 min, the IHN can charge a 100 μF capacitor to 2.5 V. Then, a self‐powered dorsalis pedis artery monitoring system is designed, which can detect the pulse signals of the dorsalis pedis artery in real‐time. The integrated design of energy collection, storage, and utilization has important application potential in future self‐powered monitoring of patients and intelligent analysis of blood supply to the lower limbs in a professional athlete. The wearable self‐powered dorsalis pedis artery monitoring system has promising application prospects in the medical and sports fields. The energy harvesting from human body motions based on triboelectric nanogenerator and piezoelectric nanogenerator shows great potential in the power supply of wearable electronic devices.

Abstract:This paper studies the development of piezoelectric energy harvesting for self-powered leadless intracardiac pacemakers. The energy harvester fit inside the battery compartment, assuming that the energy harvester would replace the battery with a smaller rechargeable battery capacity. The power output analysis was derived from the three-dimensional finite element analysis and in vivo heart measurements. A Doppler laser at the anterior basal in the right ventricle directly measured the heart's kinetic motion. Piezoceramics in the cantilevered configuration were studied. The heart motion was periodic but not harmonic and shock-based. This study found that energy can be harvested by applying periodic bio-movements (cardiac motion). The results also showed that the energy harvester can generate 1.1 V voltage. The effect of various geometrical parameters on power generation was studied. This approach offers potential for self-powered implantable medical devices, with the harvested energy used to power devices such as pacemakers.

Abstract:Harvesting biomechanical energy from cardiac motion is an attractive power source for implantable bioelectronic devices. Here, we report a battery-free, transcatheter, self-powered intracardiac pacemaker based on the coupled effect of triboelectrification and electrostatic induction for the treatment of arrhythmia in large animal models. We show that the capsule-shaped device (1.75 g, 1.52 cc) can be integrated with a delivery catheter for implanting in the right ventricle of a swine through the intravenous route, which effectively converts cardiac motion energy to electricity and maintains endocardial pacing function during the three-week follow-up period. We measure in vivo open circuit voltage and short circuit current of the self-powered intracardiac pacemaker of about 6.0 V and 0.2 μA, respectively. This approach exhibits up-to-date progress in self-powered medical devices and it may overcome the inherent energy shortcomings of implantable pacemakers and other bioelectronic devices for therapy and sensing.

Abstract:The working life ofimplantable electronic medical devices,such as cardiac pacemaker,deep brain stimulator,nerve stimulator and so on,was limited by the capacity of their batteries.To solve this problem,in this paper we have fabricated an implanted triboelectric nanogenerator (iTENG).It can convert biomechanical energy from organ movement into electrical energy through a conjunction of triboelectrification and electrostatic induction,which produced by the contact and separation of a polydimethylsiloxane (PDMS)film and an aluminum sheet with micro/nano structures under the external force.This working mode was defined as the vertical contact-separation working mode.The size of iTENG is 1.2 cm× 1.2 cm,and it was packaged by biocompatible PDMS.In the in vitro experiment,the iTENG generated an open circuit voltage (Voc) of 12 V,a short circuit current (Isc) of 0.25 μA,and a peak power density of 8.44 mW/m2,respectively.When implanted subdermal at rat's left chest,the in vivo Voc and Isc of iTENG were also detected,which were 3.73 V and 0.14 μA,respectively.By integrated with the power management system,iTENG can be applied to power the implantable electronic medical devices,achieving a self-powered imlpantable electronic medical devices.

Abstract:Self-powered pacemakers hold great promise for future clinical translation by virtue of their extended working time and reduced high-risk repeated surgeries. Although self-powered pacemakers have been widely investigated, systematic research on their pacing results in different pacing sites is still lacking, which is crucial for expanding clinical application in the future. Here we design a self-powered pacemaker based on all-in-one piezoelectric nanogenerator (A-PENG) that can harvest biomechanical energy from the cardiac pulsation, enabling valid myocardium pacing and conduction system pacing. The A-PENG prepared by bonding is featured good stretchability, waterproofness, and biocompatibility, which can generate excellent electrical output with human movement. The A-PENG possesses higher stress transfer ability due to tailoring the distribution of the piezoelectric particles, thus enhancing output performance, which can be validated by the comparative experiment and simulation results. The maximum open-circuit (VOC) voltage and short-circuit current (ISC) are 3.1 and 3.0 times higher than routine piezocomposite films. The canine model exhibits valid pacing when the device releases electrical stimulus at right atrium, left ventricle, and His bundle. This work provides a new idea for developing self-powered pacemakers with multifunctional features and an integrated device for in vivo diagnosis and treatment.

Abstract:Harvesting biomechanical energy in vivo is an important route in obtaining sustainable electric energy for powering implantable medical devices. Here, we demonstrate an innovative implantable triboelectric nanogenerator (iTENG) for in vivo biomechanical energy harvesting. Driven by the heartbeat of adult swine, the output voltage and the corresponding current were improved by factors of 3.5 and 25, respectively, compared with the reported in vivo output performance of biomechanical energy conversion devices. In addition, the in vivo evaluation of the iTENG was demonstrated for over 72 h of implantation, during which the iTENG generated electricity continuously in the active animal. Due to its excellent in vivo performance, a self-powered wireless transmission system was fabricated for real-time wireless cardiac monitoring. Given its outstanding in vivo output and stability, iTENG can be applied not only to power implantable medical devices but also possibly to fabricate a self-powered, wireless healthcare monitoring system.

Abstract:Nanogenerator (triboelectric nanogenerator and piezoelectric nanogenerator) has experienced a rapid development since it was proposed. This technique can covert various mechanical energies into electric energy, including human motion energy, wind energy, acoustic energy and ocean energy. The converted electricity can be used for health monitoring and physiological function regulation, such as pulse detection, bioelectrical stimulation and cardiac pacing. This review summarizes the structure, working mechanism, output performance of nanogenerator and its latest progress in circulatory system, nervous system, biological tissue, sleep and rescue system. Additionally, a further analysis was also made on the application challenge of nanogenerator in clinical treatment. In the future, nanogenerator is expected to be an auxiliary power source, or even to replace battery to power medical electronic device and realize the self-powered health monitoring and physiological function regulation of human body.

Abstract:Capturing the body's mechanical energy from the heart, lungs, and diaphragm can probably meet the requirements for in vivo applications of implantable biomedical devices. In this work, we present a novel contractile cardiomyocyte (CM)-driven biogenerator based on piezoelectric nanofibers (NFs) uniaxially aligned on a PDMS thin film. Flexible nanostructures interact with the CMs, as a physical cue to guide the CMs to align in a specific way, and create mechanical interfaces of contractile CMs and piezoelectric NFs. As such, the cellular construct features specific alignment and synchronous contraction, which realizes the maximal resultant force to drive the NFs to bend periodically. Studies on contraction mapping show that neonatal rat CMs self-assemble into a functional bio-bot film with well-defined axes of force generation. Consequently, the biogenerator produces an average voltage of 200 mV and current of 45 nA at the cell concentration of 1.0 million per ml, offering a biocompatible and scalable platform for biological energy conversion.

Abstract:This paper studies the possibility of heart kinetic motion for designing a self-powered intracardiac leadless pacemaker by piezoelectric energy harvesting. A Doppler laser displacement sensor measures in vivo heart kinetic motion. Cantilevered and four-point bending piezoelectric harvesters are studied under the measured in vivo heart kinetic motion. The heart movement is above 15 mm. The cantilevered and four-point bending harvesters generate a maximum voltage of ~ 0.28 V and 0.8 V, respectively with the measured heart motion with a heart rate of 168 beats per minute. Two DC/DC converters, LTC3588 and MAX17220, combined with full-bridge rectifiers and their start-up performance are tested.Clinical Relevance-This paper analyzed the heart kinetic motion and establishes the piezoelectric energy harvesting for a new era of self-powered leadless pacemakers.

Abstract:The need to recharge and eventually replace batteries is increasingly significant for operating a variety of wearable electronic devices. Rapid advances in low power design have stimulated the requirements for portable and sustainable power sources, thus opening the possibility of using human biomechanical energy as a promising alternative power source. Respiration is a unique form of spontaneous and stable source of human biomechanical energy that is currently untapped, and has the potential to be converted to a sustainable power source for low power wearable electronic devices and integrated body sensor networks. However, effectively harvesting respiration energy characterized by low frequency and low force is currently a technological challenge, and cannot be well-achieved by classical energy harvesting methods. In this work, a triboelectric nanogenerator based on contact-separation mode is demonstrated as a small and light-weight wearable respiratory energy harvester (wREH). The wREH can also be utilized as a self-powered respiratory motion sensor capable of tracking rate and depth of respiration. A simple and cost-effective poly lactic-co-glycolic acid patterning technique is utilized to create microdome-patterned polydimethylsiloxane for increasing the effective contact area of the triboelectric surfaces, thus enhancing power output while achieving higher durability than existing nanosurface structures.
An Intracardiac Flow Based Electromagnetic Energy Harvesting Mechanism for Cardiac Pacing

Abstract:Contemporary cardiac implantable electronic devices such as pacemakers or event recorders are powered by primary batteries. Device replacement due to battery depletion may cause complications and is costly. The goal of energy harvesting devices is to power the implant with energy from intracorporeal power sources such as vibrations and blood flow. By replacing primary batteries with energy harvesters, reinterventions can be avoided and the size of the total device might be reduced. This paper introduces a device with a lever, which is deflected by the blood stream within the right ventricular outflow tract (RVOT), an attractive site for cardiac pacing. The resulting torque is converted to electrical energy by an electromagnetic mechanism. The blood flow harvester weighs 6.4 g and has a volume of 2 cm3, making the harvester small enough for catheter implantation. It was tested in an experimental setup mimicking flow conditions in the RVOT. The blood flow harvester generated a mean power of 14.39 ± 8.38 μW at 60 bpm (1 Hz) and up to 82.64 ± 17.14 μW at 200 bpm (3.33 Hz) during bench experiments at 1 m/s peak flow velocity. Therefore, it presents a viable alternative to power batteryless and leadless cardiac pacemakers.

Abstract:Long-term energy supply for electronic systems is challenging for implantable biomedical devices, like cardiac pacemakers. Energy harvesting can significantly extend the lifetime of these devices, however, no clinical translational technologies can efficiently convert the mechanical energy of the heart into electrical power without a thoracotomy and interfering with the cardiovascular functions. Almost all reported implantable cardiac energy harvesting designs sutured devices directly onto the epicardium or pericardium with potential risks to the patients. Here, we report a cardiac energy harvesting strategy, which is integrated into part of the existing pacemaker lead and otherwise with no direct contact of heart, by utilizing porous piezoelectric thin films in a bioinspired self-wrapping helical configuration for flexible integration with existing implantable medical devices. We demonstrate that this compact design can be seamlessly coupled with current leads without introducing additional implantation surgeries. In vivo studies under various conditions (anchoring, pacing, and calcium chloride infusion) are presented that demonstrate clinical translation in a porcine model. Both theoretical studies and in vitro experiments are also presented to validate the energy harvesting process. The scalability of the design is discussed, and the reported results demonstrate a 10 × 10 array of helical EH devices wrapping all through the lead (a mixed pattern of in series and parallel connections) would extend the lifetime of the pacemaker battery by 1.5 years. This innovative cardiac energy harvesting strategy represents a significant step forward for clinical translation without a thoracotomy for patients, suggesting a paradigm for biomedical energy harvesting in vivo.

Abstract:Accurately estimating the complex motion of the heart can unlock enormous potential for kinetic energy harvesting. This paper presents a foundational dataset for heart kinetic motion through in-vivo tests and investigates the most influential factors in heart kinetic motion. In-vivo tests on a living pig's heart, with signal processing, were carried out to study the heart movement by heart beating and respiration motions. A network of nine points on the heart was employed for in vivo measurements. These measurements illustrated the kinetic energy signals in displacement, velocity, and acceleration. The results indicated that the motion level varies in distinct locations over epicardium. The statistical features and autocorrelations were reported for these points, illustrating the highest displacement and acceleration. Each heartbeat generated an energy of 14.35 mJ and a power of 1.03 W. However, this available energy is not uniformly distributed. The results illustrated that not only is cardiac movement location-dependent, but the speed of cardiac displacement cycles is also location-dependent. The right atrium has the highest cardiac kinetic movement with an amplitude of 16.19 mm displacement and 16.3 m/s2 acceleration. To evaluate the energy harvesting possibility from the heart's motion, a piezoelectric energy harvester was simulated by the finite element method, implying that the energy harvesting level significantly depends on implant location over epicardium. The results of this study open the potential of designing novel energy harvesters based on accurate heart movements and provide a foundation for future investigations of energy harvesting for leadless pacemaker energy systems.
