Abstract:Background/objective: As a result of an increasing desire among physicians and parents for clinical centers that can evaluate children with known or suspected exposures to environmental toxicants, a network of federally funded "pediatric environmental health specialty units" has recently been created. This descriptive study profiles the children seen in one unit of this program. Setting: A New England, university-affiliated Pediatric Environmental Health Center (PEHC). Methods: Review and analysis of all children seen in the PEHC in calendar year 1999. Results: Over the course of the year, 281 children made 863 visits to the PEHC. Presenting complaints fell into 4 major categories: new visit for management of lead intoxication (n = 248), return visit for management of lead intoxication (n = 569), new visit for evaluation of exposure to an environmental toxicant other than lead (n = 33), and return visit for the management of exposure to a non-lead toxicant (n = 13). Among those children with new visits for a non-lead toxicant, the most common chief complaints were exposure to solvent-contaminated water (n = 7), pesticide exposure (n = 6), illness associated with proximity to a hazardous waste site (n = 6), autism from suspected mercury intoxication (n = 4), and evaluation of school-induced, building-related illness ("sick school syndrome")(n = 4). Eleven children had autism or pervasive developmental delay. Families traveled distances as great as 450 kilometers for evaluation by a pediatric environmental health clinical specialist. Every child was evaluated by a pediatrician with subspecialty training in medical toxicology. Environmental investigation of air, water, paint, dust, or land was conducted for all except 4 children (all foreign-born adoptees). Therapeutic interventions included chelation therapy, relocation to a safe environment, removal from school, and termination of chelation therapy that had been initiated by another practitioner. Third-party payors provided full reimbursement for all visits. Conclusions: The chief complaints of the children brought to pediatric environmental health specialty units are diverse, involving exposures to a wide range of toxicants from all environmental media (air, water, soil, and food). Parents desiring such an evaluation must often travel extensive distances, suggesting the need for a broader network of such centers. Third-party payors and health maintenance organizations are willing to provide full reimbursement for these services.

Abstract:SUMMARY: From climate change to hydraulic fracturing, and from drinking water safety to wildfires, environmental challenges are changing. The United States has made substantial environmental protection progress based on media-specific and single pollutant risk-based frameworks. However, today’s environmental problems are increasingly complex and new scientific approaches and tools are needed to achieve sustainable solutions to protect the environment and public health. In this article, we present examples of today’s environmental challenges and offer an integrated systems approach to address them. We provide a strategic framework and recommendations for advancing the application of science for protecting the environment and public health. We posit that addressing 21st century challenges requires transdisciplinary and systems approaches, new data sources, and stakeholder partnerships. To address these challenges, we outline a process driven by problem formulation with the following steps: a) formulate the problem holistically, b) gather and synthesize diverse information, c) develop and assess options, and d) implement sustainable solutions. This process will require new skills and education in systems science, with an emphasis on science translation. A systems-based approach can transcend media- and receptor-specific bounds, integrate diverse information, and recognize the inextricable link between ecology and human health.

Abstract:The warming of our planet matters to every child. Driven by fossil fuel-generated greenhouse gas emissions, climate conditions stable since the founding of modern pediatrics in the mid-nineteenth century have shifted, and old certainties are falling away. Children's physical and mental health are threatened by climate change through its effects on temperature, precipitation, and extreme weather; ecological disruption; and community disruption. These impacts expose and amplify existing inequities and create unprecedented intergenerational injustice. Fossil fuel extraction and combustion cause harm today and reach centuries into the future, jeopardizing the health, safety, and prosperity of today's children and future generations. Appreciating the unique vulnerability of their patients, pediatricians have become leading health advocates for climate actions necessary to protect all living and future children. Policies that reduce reliance on fossil fuels and promote cleaner air, facilitate walking and bicycling, encourage more sustainable diets, increase access to nature, and develop more connected communities lead to immediate gains in child health and equity, and build a foundation for generations of children to thrive.

Abstract:This month's article "Exposure to Air Pollution and Cardiovascular Risk in Young Children – a Pilot Project" reports novel findings that young children exposed to low levels of air pollution have early indicators of vascular damage. These results add to our understanding of the wide-ranging health effects of air pollution exposure and serve as a reminder that children are highly vulnerable to environmental exposures. All children deserve healthy environments that promote their physical and mental development, but the consequences of climate change threaten this right. Extreme weather, heat, wildfires, flooding, infectious disease, and other climate change effects have wide-ranging impact on human health with children likely to bear the highest burden of disease due to climate change. The pediatric research community should meet the urgent need for solutions to this crisis by engaging in research that identifies effective strategies to address the health effects caused by current and future climate change to protect the health of children and future generations.

Abstract:Environmental toxicants and pollutants are causes of adverse health consequences, including well-established associations between environmental exposures and cardiovascular diseases. Environmental degradation is widely prevalent and has a long latency period between exposure and health outcome, potentially placing a large number of individuals at risk of these health consequences. Emerging evidence suggests that environmental exposures in early life may be key risk factors for cardiovascular conditions across the life span. Children are a particularly sensitive population for the detrimental effects of environmental toxicants and pollutants given the long-term cumulative effects of early-life exposures on health outcomes, including congenital heart disease, acquired cardiac diseases, and accumulation of cardiovascular disease risk factors. This scientific statement highlights representative examples for each of these cardiovascular disease subtypes and their determinants, focusing specifically on the associations between climate change and congenital heart disease, airborne particulate matter and Kawasaki disease, blood lead levels and blood pressure, and endocrine-disrupting chemicals with cardiometabolic risk factors. Because children are particularly dependent on their caregivers to address their health concerns, this scientific statement highlights the need for clinicians, research scientists, and policymakers to focus more on the linkages of environmental exposures with cardiovascular conditions in children and adolescents.

Abstract:The concept of child health has evolved over many decades and has gone from defining health as the absence of disease and disability to a much more sophisticated understanding of the ways in which a confluence of many factors leads to a healthy childhood and to producing the infrastructure for a healthy lifetime. We review the evolution of these ideas and endorse the definition featured in Children's Health, the Nation's Wealth , which states tha t child health is: "... the extent to which individual children or groups of children are able or enabled to: (a) develop and realize their potential, (b) satisfy their needs, and (c) develop the capacities that allow them to interact successfully with their biological, physical, and social environments."

Abstract:Several studies have indicated that children who grow up in green environments enjoy beneficial impacts on their development. However, to date, very little attention has been paid to the types and characteristics of dedicated public open green space for children. In addition, studies on biophilic design that could help landscape architects to design open spaces for children are limited. In order to fill this gap, this perspective examines the scientific literature on the relationship between open spaces and children. The authors specifically discuss and analyze the following points: (1) pathways to and effects on children's health and well-being; (2) types and characteristics of open spaces for child-dedicated facilities (e.g., schools, hospitals, fitness camps, playgrounds, etc.), as well as for child-friendly urban open spaces. Finally, the authors provide inspiring examples and case studies of biophilic design for children's health and well-being. Conclusions from this perspective show that biophilic design could benefit both the physical and mental health of children, as well as improve children's overall resilience to pandemics and other diseases. This perspective provides, for the first time, new insights for designing biophilic and child-friendly cities, and explores areas of future research.

Abstract:Although genetic factors play a role in the etiology of atopic disease, the rapid increases in the prevalence of these diseases over the last few decades suggest that environmental, rather than genetic factors are the driving force behind the increasing prevalence. In modern societies, there is increased time spent indoors, use of antibiotics, and consumption of processed foods and decreased contact with farm animals and pets, which limit exposure to environmental allergens, infectious parasitic worms, and microbes. The lack of exposure to these factors is thought to prevent proper education and training of the immune system. Increased industrialization and urbanization has brought about increases in organic and inorganic pollutants. In addition, Caesarian birth, birth order, increased use of soaps and detergents, tobacco smoke exposure and psychosomatic factors are other factors that have been associated with increased rate of allergic diseases. Here, we review current knowledge on the environmental factors that have been shown to affect the development of allergic diseases and the recent developments in the field.

Abstract:Environmental diseases are increasing globally and the sources of contamination are multiple: indoor and outdoor air, food, cosmetics, drinking and irrigation water.Endocrine-disrupting chemicals (EDCs) are among the causes of the increasing presence of chronic diseases. There are periods of vulnerability—in particular during pregnancy, childhood, and puberty—when these substances can modify the functioning and establishment of major functions such as brain development, the various endocrine systems, the gonads, and the regulation of fatty tissue.Recent studies show the general population's widespread exposure to endocrine disruptors, either persistent (accumulating over time) or non-persistent (but with permanent exposure). However, the consideration of EDCs in clinical practice remains marginal, even though doing so could reduce the risk of diseases linked to environmental factors and positively influence the natural evolution of a chronic disease when it is diagnosed. The aim of this article is to clarify the concepts associated with the exposome and to list the main scientific data attesting to the effects of endocrine disruptors on health in order to draw up clinical, teaching, and research perspectives.

Abstract:This review highlights studies from the past 3 years that add to the understanding of the impact of environmental exposures on allergic disease. These include aeroallergens, air quality, prenatal or early life exposures, and occupational exposures. Recent studies focused on the relationship between the environment, the microbiome, and allergic disease as well as new therapeutic options are also reviewed. Lastly, there has been significant recent research that improves our knowledge of the link between health disparities and environmental exposures. These scientific advances have resulted in a better understanding that sets the foundation for current and future research dedicated to improving health outcomes by modifying environmental exposures.

Abstract:Environmental factors, here taken to include pollutants, lifestyle factors and behaviours, can play an important role in serious, chronic pathologies with large societal and economic costs, including respiratory disease. However, measurement of the environmental component in epidemiological studies has traditionally relied on much more uncertain and incomplete assessments than measurement of the genome. The 'exposome' has therefore been proposed as a new paradigm to encompass the totality of human environmental (meaning all non-genetic) exposures from conception onwards, complementing the genome. Evidently, there are large challenges in developing the exposome concept into a workable approach for epidemiological research. These include: (1) the accurate and reliable measurement of many exposures in the external environment, (2) the measurement of a wide range of biological responses in the internal environment, and (3) addressing the dynamic, life course nature of the exposome. New tools and technologies that can be applied to address these challenges include exposure biomarker technologies, geographical mapping and remote sensing technologies, smartphone applications and personal exposure sensors, and high-throughput molecular 'omics' techniques. Prospective, population-based cohort studies have recently started to implement these methods using the exposome framework. The exposome thus offers a new and exciting paradigm for improvement and integration of currently scattered and uncertain data on the environmental component in disease aetiology. This should lead to a better understanding of the role of environmental risk factors in respiratory disease and other chronic pathologies, and ultimately to better primary prevention strategies.

Abstract:Burning of fossil fuels along with deforestation and ecological disruption have led to the warming of the Earth and climate change. Children are especially vulnerable to adverse health effects of climate change associated changes in the air, soil, and water as their organs are still developing, have a faster breathing rate, higher per pound ingested and inhaled exposures, and greater relative body surface area. To protect this vulnerable population, health care professionals need to play a leading role. In 2015, the American Academy of Pediatrics (AAP) updated their original 2007 Global Climate Change and Children's Health policy statement (again updated in 2024) stating that, "failure to take prompt, substantive action would be an act of injustice to all children." Health care professionals need to educate themselves and their patients of the health risks posed by climate change and incorporate climate change counseling into their practice. They also need to go beyond the framework of the healthcare system and work collaboratively with communities, corporations, and governments to advocate for policies and solutions to mitigate and adapt to climate change. The health and wellbeing of future generations rests upon the actions we take today.

Expanding the physician's role in pediatric environmental health

Abstract:In rural Georgia, a nurse and an environmental health specialist from the local health department visit the home of a young nursing mother to evaluate her home for the presence of lead hazards. The mother's older child, a 3-year old girl, has a blood lead level of 22 micrograms per deciliter, which was discovered through routine (EPSTD) health department screening. In examining the home, the specialist finds classic environmental risks; peeling and chipping lead-based paint on windows and door frames, lead dust on window wells and floors, and a backyard that serves as a burial ground for defunct car parts and dead batteries. In the course of talking with the mother about the lead hazards he has found, he notices that she and her 8-week-old infant seem quite listless, so he asks the mother how she's coping with her new baby. Eventually, the mother discloses that during her pregnancy she craved dirt and that she had eaten bowels of it scooped from her backyard. Once she had the baby, she says, she lost her craving. The nurse immediately contacts the physician involved in this case, who arranges for the mother and infant to be admitted to a nearby medical center for chelation therapy. Testing reveals the mother's lead level at 90 micrograms per deciliter; the infant's level is staggering 85 micrograms per deciliter. Once lead levels are reduced,the physician and public health nurse arrange for a host of social services, including psychological and nutritional counseling for the mother and periodic retesting of the children. The family moves from the dilapidated rental home. However, the mother misses her appointments, and despite repeated attempts to locate her, the family is lost to follow-up.

Abstract:Concerns regarding adverse health effects of exposure to environmental pollutants among children are increasing. This subject area is particularly important in China because of environmental pollution problems associated with rapid development and a large population. The authors provide what is, to their knowledge, the first review in China of English- and Chinese-language literature regarding the current status of environmental exposures to children in China and the impact of these exposures on the health of children. Children in China are exposed to diverse environmental pollutants, including traditional pollutants such as lead and mercury and emerging pollutants such as phthalates and perfluorinated compounds. Incidence and prevalence of certain childhood diseases have increased in last decades. In China, a limited number of data on environmental exposure and children's health are available, and further high-quality studies are needed.

Abstract:The definitions and conceptualizations of health, and the management of healthcare have been challenged by the current global scenarios (e.g., new diseases, new geographical distribution of diseases, effects of climate change on health, etc.) and by the ongoing scholarship in humanities and science. In this paper we question the mainstream definition of health adopted by the WHO-'a state of complete physical, mental and social well-being and not merely the absence of disease or infirmity' (WHO in Preamble to the constitution of the World Health Organization as adopted by the international health conference, The World Health Organization, 1948)-and its role in providing tools to understand what health is in the contemporary context. More specifically, we argue that this context requires to take into account the role of the environment both in medical theory and in the healthcare practice. To do so, we analyse WHO documents dated 1984 and 1986 which define health as 'coping with the environment'. We develop the idea of 'coping with the environment', by focusing on two cardinal concepts: adaptation in public health and adaptivity in philosophy of biology. We argue that the notions of adaptation and adaptivity can be of major benefit for the characterization of health, and have practical implications. We explore some of these implications by discussing two recent case studies of adaptivity in public health, which can be valuable to further develop adaptive strategies in the current pandemic scenario: community-centred care and microbiologically healthier buildings.
