Abstract:PurposeFractures of the lower extremity, particularly of the femur and acetabulum, may be difficult to immobilize with splinting alone. These injuries may be best stabilized with the application of various types of skeletal traction. Often, traction is applied percutaneously in an emergent setting, making the knowledge of both superficial and deep anatomy crucial to successful placement.MethodsReview was performed via PubMed search as well as referencing the Orthopaedic literature. Relevant articles to the anatomy of the knee, ankle and calcaneus as they pertain to traction placement were referenced in compiling the optimal recommendations for traction placement.ConclusionBy palpating and marking superficial landmarks and observing specific anatomic relationships, safe application of traction pins can be performed while minimizing iatrogenic injury to vital anatomic structures, and avoiding intra-articular placement which could potentially lead to joint infection.

Abstract:BACKGROUND Prior to the 1900s, femoral-shaft fractures were treated with various types of splinting. In 1907 and 1909, Steinmann and Kirschner, respectively, developed the first traction treatment modalities with the use of pins or wires under tension. A single pin is usually placed either at the distal femur or proximal tibia when applying skeletal traction for treating femoral fractures. Complications associated with skeletal traction include pin track infection and loosening,1 which may require repositioning of the pin and inefficient traction forces applied across the fracture. Pin fixation problems are particularly severe in patients with osteoporosis, which can be a relative contra-indication to external fixation and presents a challenge for any fixation technique.2 TECHNIQUE We propose a novel technique in applying skeletal traction. Two Denham pins are placed into the proximal tibia 2.5 cm posterior to and 2.5 cm distal to tibial tubercle (Fig. 1). The pins are then connected with a Hoffmann external flexator connector producing a stable construct. Traction is then applied to both pins through free weights attached to a pulley at the end of the bed (Fig. 2). This technique was used effectively in elderly osteoporotic patients who had sustained femoral supracondylar fragility fractures and were not fit for operative fixation of the fractures (Fig. 3). There was no incidence of pin loosening or pin-site infection noted.

Abstract:Introduction: High velocity trauma is the cause of maximum number of fatality in the younger age group worldwide. In the field of trauma surgery open fractures of the leg remain with the higher complication rate. Treatment options include wound debridement reduction and immobilization with cast, open reduction and plate fixation, external fixation, and IM nailing. Limb reconstruction system (LRS) is a unilateral rail system which consists of Schanz pins, rail rods, and sliding clamps. LRS was considered as definitive management for open fractures. LRS with its modular attachments, it is specifically designed to enable the surgeon to perform simple and effective surgery as it offers rigid fixation of fracture fragments, allows early weight bearing, and reduces the economic burden. Aim: The aim of the study was to determine the efficacy of mono-lateral external fixator LRS for management of the lower extremity long bone fracture (tibia/femur). Methods: A prospective study was carried out where 26 cases of lower extremity trauma (complicated) managed by LRS over a period of 5–6 months. The patients ranged from age 13 years to 67 years. Majority were male and predominant mode of injury was road side accidents. Majority of case were exposed to multiple the surgeries before LRS application. Most of the cases are of infection non-union. All the patients were treated at our hospital. These surgeries were done from June, 2005 to 2007. Any bone fragments that were protruding out we recovered with sterile dressing. The patient were taken to the operating room and treated by pulsed lavage and debridement the wound and splints. In patients, whose bone was exposed, as the wound condition improved the patients were planned for flap rotation. Assessment of quality of regenerate was done by plain radiography and clinical assessment at every follow-up. Final assessment for bone results and functional results was done using Association of the study and application of the method of Ilizarov (ASAMI) score. Results: Among 26 patients, bony results as per ASAMI score were excellent in 73.07% (n=19), good in 19.23% (n=5), fair in 3.84% (n=1), and poor in 3.84% (n=1). Functional results were excellent in 84.7% (n=22), good in 11.5% (n=3), and fair in 3.8% (n=1). All the 26 cases of lower extremity fracture were kept on LRS external fixator for a period of 5–6 months on an average depending on the rate of union, after which the fixator was removed. For 2 patients nailing was done after LRS frame removal. Average time for frame removal was 28 weeks. In most cases union was complete by 7–8 months, the least being 5 months for a tibia shaft fracture. The average hospital stay for the patients was 7 days and as compared to multi staged surgery financial burden was reduced by 40%. The average time of return to work for the patients was 3 weeks. Conclusion: Overall LRS proved to be an effective modality of treatment in cases of the lower extremity fracture with bone loss as definite modality of treatment for damage control as well as for achieving union and lengthening/transportation, deformity correction simultaneously, with the advantage of simple surgical technique, ease of application, minimal invasive, strong fixation, high patient compliance, early weight bearing, easy wound management, and the lower rate of complication.
Primary Clinical Application of Traction Reductor for Lower Limb Fracture

Abstract:OBJECTIVE:To develop a traction reductor for the reduction of lower limb fractures during the minimally invasive surgery and explore its safety and efficacy.METHODS:From February 2007 to March 2009, closed or limited open reduction plus percutaneous plate and screw internal-fixation were conducted in 34 patients with fracture of distal femur and tibia metaphysic, among which there were 3 distal femoral fractures (2 33-B, 1 33-C), 14 proximal tibial fractures (9 41-A, 3 41-B, 2 41-C) and 17 distal tibial fractures (9 43-A, 5 43-B, 3 43-C, 2 Gustilo I a), according to the Association for Osteosynthesis-Orthopaedic Trauma Association (AO-OTA) classification. Besides, closed reduction plus interlocking intramedullary nailing on tibial shaft fracture were applied in 36 patients (7 42-A, 21 42-B, 8 42-C, 2 Gustilo I a). All the 70 patients, with an average age of 37.6 years (range: 17 to 63 years) and average time before surgery of 4.7 d (range: 0.7 to 12.0 d), underwent reduction by self-designed traction reductor for lower limb fracture in the surgery. The reduction duration and C-arm fluoroscopy time were recorded. Recovery of the force line of affected limbs after surgery was determined by whether the line from anterior superior iliac spine to the interdigit between the first and second toe-web passed the patella center. And the distance from bilateral anterior superior iliac spine to medial malleolus tip as well as the difference between lower limbs were recorded to determine the recovery of length after surgery. Meanwhile, the varus-valgus and anteroposterior angulations after reduction were measured by AP and lateral X-ray.RESULTS:The reduction duration was 12.7 min (range: 7 to 31 min); X-ray fluoroscopy time, 1.3 min (range: 0.4 to 3.0 min); length difference between both lower limbs (6.5 ± 1.1) mm; and axial alignment difference (7.0 ± 1.8) mm. The X-ray result showed that varus-valgus angle was (2.75 ± 0.16)°; and anteroposterior angulation (5.13 ± 0.51)°.CONCLUSION:The traction reductor for lower limb fracture could achieve satisfying fracture reduction in the minimally invasive surgery of distal femur, tibia metaphysic and tibial shaft fracture.

In this study, we review current concepts in ankle fracture management, including posterior malleolus fixation, syndesmosis fixation, deltoid ligament repair, fibular nailing, and early weightbearing, from both a biomechanical and clinical perspective.

Abstract:Background Phalangeal fractures are amongst the most challenging injuries that hand surgeons and hand therapists treat. Traditionally, these have been managed operatively, but are often fraught with potential problems including contractures, deformities and loss of motion. Purpose To provide evidence supporting the use of non-invasive skin traction orthosis as an effective treatment option. Study design Retrospective cohort. Methods We performed a retrospective review of outpatients with phalangeal fractures treated with non-invasive skin traction orthoses in our institution from January 2021 till June 2022. Demographic information, injury specifics and radiological findings were extracted from medical records. Outcome measures included total arc of motion (TAM) and dorsal angulation angles. Results Fourteen patients (17 fractures) with a mean age of 48 years (SD21.3) were included. Ten patients had single digit injuries, while four patients had two digits in traction within the same splint. 70.6% were proximal phalangeal fractures. 76.5% of the fractures were extra-articular and 58.8% non-comminuted. Median duration of orthosis use was 18 days (IQR 8–21). Patients with forearm-based orthoses had significantly longer traction time. There was a significant improvement ( p = 0.001) from median baseline TAM (124°) to final TAM readings (245°). Younger patients with ulnar digit fractures or extra-articular fractures had a shorter rehabilitation period. There is no significant difference in clinical outcomes between the use of forearm-based or hand-based orthoses. Conclusion We recommend the use of the hand-based non-invasive skin traction orthosis as an option in managing phalangeal fractures as it is a simple, inexpensive and non-invasive procedure with promising results. Care must be taken to ensure frequent change of traction tapes to maintain good skin integrity, and to avoid loss of tension. Radiological imaging should be performed after each traction tape change to ensure good alignment is maintained.

Abstract:Recent studies report the overall incidence of distal femur fractures as 8.7/100,000/year. This incidence is expected to rise with high energy motor vehicle collisions and elderly osteoporotic fractures in native and prosthetic knees keep increasing. These fractures are more common in males in the younger age spectrum while females predominate for elderly osteoporotic fractures. Surgical treatment is recommended for these fractures to maintain articular congruity, enable early joint motion and assisted ambulation. Over the last two decades, development of minimally invasive and quadriceps sparing surgical approaches, availability of angle stable implants have helped achieve predictable healing and early return to function in these patients. Currently, laterally positioned locked plate is the implant of choice across all fracture patterns. Retrograde with capital implantation of intramedullary nails with provision for multiplanar distal locking is preferred for extra-articular and partial articular fractures. Even with these advancements, nonunion after distal femur fracture fixation can be as high as 19%. Further recent research has helped us understand the biomechanical limitations and healing problems with lateral locked plate fixation and intramedullary nails. This has lead to development of more robust constructs such as nail-plate and double plate constructs aiming for improved construct strength and to minimise failures. Early results with these combination constructs have shown promise in high risk situations such as fractures with extensive metaphyseal fragmentation, osteoporosis and periprosthetic fractures. These constructs however, run the risk of being over stiff and can inhibit healing if not kept balanced. The ideal stiffness that is needed for fracture healing is not clearly known and current research in this domain has lead to the development of smart implants which are expected to evolve and may help improve clinical results in future.

Abstract:Open fractures are an emergency where the principal aim of the treatment is to maximise the restoration of limb function while preventing the dreaded consequences of infection and non-union. The decision-making process for open injuries is influenced by a variety of criteria, such as patient age, injury features, systemic response, activity level, comorbidities, and functional requirements. A collaborative orthoplastic approach to treating these injuries is essential for minimizing complications and need to be considered as a single specialty in early and long-term management. It has been shown that early prophylactic systemic antibiotics, wound irrigation, aggressive debridement of contaminated and devitalized tissue, and appropriate fracture fixation decreases the complications in all grades of open fractures. The advantages of Gram-negative antibiotics, the use of local antibiotics, intraoperative wound cultures, the "fix and flap" approach, and Negative Pressure Wound Therapy are few of the treatment options that are still controversial. The aim of this review is to provide a comprehensive review and practice guidelines regarding the management of open fractures.

Abstract:In this review, we aim to provide a concise yet comprehensive summation of the assessment and management of humeral shaft fractures. These are uncommon but prevalent enough that they are part of any trauma surgeon's scope of practice. They have historically been treated using non-operative methods, including braces and casts, supported by published excellent results in the rate of the bone union. However, recently published studies challenge these results and suggest the outcomes might be better with surgery, but the complications of an operation such as infection and nerve injury can not be overlooked. In summary, non-surgical treatment is still the gold standard in the treatment of these fractures, but the indications for surgical management are now clearer and include early signs of delayed union and patients who are unable to have a brace fitted or are uncompliant. It is likely that these new developments will start to change practice, and therefore the treatment of humeral shaft fractures should be a topic of interest of any clinician who deals with them.

Abstract:<p>Injury-related morbidity and mortality have been one of the most common causes of loss in productivity across all geographic distributions. It remains to be a global concern despite a continual improvement in regional and national safety policies. The establishment of trauma care systems and advancements in diagnostics and management have improved the overall survival of severely injured. A better understanding of the physiopathological and immunological responses to injury led to a significant shift in trauma care from "Early Total Care" to "Damage Control Orthopedics." While most of these algorithms were tailored to the philosophy of "life before limb," the impact of improper fracture management on disability and societal loss is increasingly being recognized. Recently, "Early Appropriate Care" of extremities has gained importance; however, its implementation is influenced by regional health care policies, available resources, and expertise and varies between low and high-income countries. A review of the literature was performed using PubMed, Embase, Web of Science, and Scopus databases on articles published from 1990 to 2020 using the Mesh terms "Polytrauma," "Multiple Trauma," and "Fractures." This review aims to consolidate on guidelines and available evidence in the management of extremity injuries in a polytraumatized patient to achieve better clinical outcomes of these severely injured.</p>

Dynamic Parallel Traction's Impact on Femoral Neck Fracture Biomechanics: A 3D Finite Element Study

Abstract:Objective: This study aims to investigate the impact of dynamic parallel traction on the biomechanical characteristics of femoral neck fractures, with the goal of identifying a more effective traction method for the repair of such fractures. Methods: We constructed a three-dimensional finite element model of the femoral neck based on CT images. Using linear regression analysis, we explored the optimal adjustment strategy for traction weight over time by analyzing the relationship between different traction weights and the maximum tolerable traction time. Additionally, we validated the stress characteristics under various traction angles and analyzed the interplay among traction weight, time, and angle. Results: Among the relationships between traction weight, maximum tolerable traction time, and femoral neck stress values, the regression equation was F=175.9, R2=0.9513 (linear regression analysis), indicating that as traction time increased from 0s to 10s, the traction force decreased from 500N to 360N, while the stress value remained relatively stable. In terms of the relationship between traction angle and femoral neck stress values, the regression equation was F=123.2, R2=0.8864 (linear regression analysis), showing that the stress value rose from 5.592MPa under parallel traction to 18.944MPa at a 60-degree angle. When considering the combined effects of these factors on stress values, the regression equation was F=139.5, R2=0.9610 (linear regression analysis), demonstrating that dynamic parallel traction resulted in significantly lower stress values compared to other traction methods. Conclusion: Dynamic parallel traction can effectively reduce the force experienced by patients, promoting healing and reducing complications. This finding holds significant implications for the development of related medical devices, offering valuable insights and benchmarks for future research and design.

Abstract:Background: Rotational malalignment after operative fracture treatment of the lower extremity may be associated with increased pain and functional impairment. Despite its clinical relevance, there are no uniform management guidelines. The aim of this scoping review is to provide an overview of all available evidence to diagnose and treat rotational deformities of the lower extremity following operative fracture treatment. Methods: This scoping review was conducted following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) statement. A literature search was carried out on 22 August 2023 by two independent reviewers in the Pubmed (MEDLINE), Embase, Web of Science, and Cochrane library databases. The search strategy was developed with the assistance of a biomedical information specialist. The main search terms were tibial and femoral malrotations. Disagreements were resolved through discussion with a third reviewer. Results: After screening and quality assessment of 3929 unique identified records, 50 articles were included for qualitative synthesis. Most studies were retrospective case reports or case series. Thirty studies focused on the femur, 11 on the tibia and nine included both femur and tibia. Most of the included studies presented cases where malrotation was associated with other limb deformities. Only 18 studies focused solely on the treatment of malrotation of the lower extremities after operative fracture treatment. Regarding diagnosis, bilateral CT-scans were used in 34 studies. Regarding treatment, external fixation was used in two studies, internal fixation (either intramedullary nail or plate) in 45 studies, and in three studies the authors used both. Overall, revision surgery resulted in good clinical outcomes with low complication rates. Conclusion: This scoping review reveals that rotational malalignment following operative treatment of lower extremity fractures remains an important complication. Although it occurs frequently and is associated with severe disability for the patient, standardized guidelines regarding the terminology, diagnosis, indications for intervention and treatment are lacking. CT-scan is the most used diagnostic modality in daily clinical practice. Revision surgery, using diverse operative techniques, demonstrated positive results, significantly alleviating patient complaints with few complications. Nevertheless, an international consensus regarding the optimal management pathway is needed, and future prospective clinical studies seem therefore necessary.
Progress in Treatment for Distal Radius Fractures

Abstract:Distal radius fractures (DRFs) are one of the most common injuries in orthopaedics, accounting for up to 20% of all fractures seen in the emergency room. At present, the optimal treatment of these fractures remains controversial. Although most DRFs can be treated non-operatively, such as immobilized by plaster, splint or brace, surgical management has become more and more popular with people's living standards improved and expecting better functional outcome. Surgical treatments for DRFs include percutaneous fixation with Kirschner wires, skeletal external fixation, open reduction and internal fixation, intramedullary nails, fixation using the miniinvasive approach, arthroscopy, and total wrist arthroplasty. Currently, open reduction with volar plate fixation is the most popular and widely way for DRFs, while a mini-invasive approach is a new reliable and reproducible procedure with few complications. Despite recently surgical treatment has become more and more popular, it is also unclear whether surgical intervention will produce better long-term outcomes. The aim of this paper is to present the studies in the literatures about the treatment for DRFs and an update of existing techniques.

Abstract:Multiply injured patients with fractures are co-managed by acute care surgeons and orthopaedic surgeons. In most centers, orthopaedic surgeons definitively manage fractures, but preliminary management, including washouts, splinting, reductions, and external fixations, may be performed by selected acute care surgeons. The acute care surgeon should have a working knowledge of orthopaedic terminology to communicate with colleagues effectively. They should have an understanding of the composition of bone, periosteum, and cartilage, and their reaction when there is an injury. Fractures are usually fixed urgently, but some multiply injured patients are better served with a damage control strategy. Extremity compartment syndrome should be suspected in all critically injured patients with or without fractures and a low threshold for compartment pressure measurements or empiric fasciotomy maintained. Acute care surgeons performing rib fracture fixation and other chest wall injury reconstructions should follow the principles of open fracture reduction and stabilization.

Abstract:The thoracolumbar junction is the junction of the thoracic and lumbar spine and is subjected to high biomechanical stress. With the rapid development of the society, the morbidity of thoracolumbar spine increases every year. Thoracolumbar fracture may affect the stability of the spine and even result in paralysis via the compression of the spinal cord. As a result of advancements in modern medicine, the theoretical and practical aspects concerning thoracolumbar fracture have made great progress. However, due to the complexity of thoracolumbar fracture, controversy still exists in the treatment strategies and methods for thoracolumbar fractures. In order to provide reference for clinical practice, the research progress in the treatment of thoracolumbar fracture is reviewed in this paper.

Abstract:Calcaneal fractures, being the most common tarsal fractures, pose a therapeutic challenge. Even today there is no consensus on their ideal treatment. The authors present review of current literature focusing on this topic. Diagnostic techniques and most commonly used classification systems are discussed as well as the algorithm for conservative or surgical treatment based on the current trends. Among surgical procedures and implants, various open and minimally invasive techniques are presented, together with a variety of implants, including screws, nails and plates. Most frequent complications, their prevention and solutions are also scrutinized.
